Asthma is an etiologically heterogeneous disease resulting from a complex interaction between genetic. The genetic aspects involved in asthma, which were analyzed from the perspective of the traditional model of multifactorial inheritance, were susceptibility, host factors, and environmental exposures. In the present paper, studies on their family aggregation, concordance in twins, and heritability were analized; as well as the current knowledge about candidate genes, genome wide association studies, and epigenomics contributions and other omic studies that have increased our knowledge about their pathophysiology and environmental interactions.

Introduction: Accurate identification of paediatric patients with severe asthma is essential for an adequate management of the disease. However, criteria for defining severe asthma and recommendations for control vary among different guidelines., Material and Methods: An online survey was conducted to explore expert opinions about the definition and management of severe paediatric asthma. To reach a consensus agreement, a modified Delphi technique was used, and practice guidelines were prepared after the analysis of the results., Results: Eleven paediatric chest disease physicians and allergy specialists with wide expertise in severe asthma responded to the survey. Consensus was reached in 50 out of 65 questions (76.92%). It was considered that a patient has severe asthma if during the previous year they have required 2 or more cycles of oral steroids, required daily treatment with medium doses of inhaled corticosteroids (with other controller medication) or high doses (with or without other controller medication), did not respond to optimised conventional treatment, or if the disease threatened the life of the patient or seriously impairs their quality of life. The definition of severe asthma may also include patients who justifiably use health resources on a regular basis, or have psychosocial or environmental factors impeding control. For monitoring, the use of questionnaires designed specifically for paediatric population, such as CAN or ACT, is recommended. As regards treatment, the use of omalizumab should be considered prior to the use of oral corticosteroids., Conclusions: This paper provides consensus recommendations that may be useful in the management of severe paediatric asthma., (Copyright © 2015 Asociación Española de Pediatría. The incidence of asthma and obesity is increasing, therefore they have been classified as public health problems; epidemiology suggests a link between these diseases. It has been detected a relationship between the body mass index and lung function, moreover some works show a direct correlation between the aforementioned index and severity of asthma. By a search for articles in indexed journals from medical databases with the key words asthma and obesity: pathogenesis, inflammation, adipokines, hypoxia, nutrition, pregnancy, this paper deeps in the knowledge about basic elements that offer an asthma and obesity link. It was found that the association between body mass index and asthma is more frequent in women. Asthma and obesity might be influenced by genetic elements and fetal programming; at the same time obesity could influence asthma by several mechanisms such as inflammation, hormones and mechanical respiratory dysfunction. The existing coincidence between several inducers and factors which exacerbate these diseases as well as in some molecular routes shows a potential relation between both pathological entities.

In recent years there has been a significant increase in the popularity of electronic cigarettes among adolescents and young adults worldwide. E-cigarettes have reached the market without the extensive preclinical toxicology testing or long-term safety studies required for conventional therapeutics and medical devices. Their effectiveness as a smoking cessation intervention, their impact at the population level and whether they are less harmful than combustible tobacco products are highly controversial. Studies show measurable adverse biological effects on organ and cell health in humans, animals and in vitro. E-cigarette use also increases the risk of respiratory diseases such as chronic obstructive pulmonary disease (COPD) and asthma. There is a serious risk of complications associated with e-cigarette use, such as EVALI (E-cigarette or Vaping Product Use-Associated Lung Injury) - serious lung damage associated with, especially for adolescents. This paper reviews the effects of e-cigarettes on respiratory health.

Unilateral absence of a pulmonary artery is an uncommon congenital heart disease. It can be related to respiratory symptoms such as asthma, an unusual finding in some of these patients. This paper reports the case of a 4-year-old male with recurrent respiratory infections and asthma symptoms, in who further studies found agenesia of right pulmonary artery with pulmonary hypoplasia of the same side.

Together with industrial development in Mexico has come a major increase in the generation of air pollutants. Some of these pollutants include compounds such as ozone, that like tobacco smoke, are related to the severity of certain respiratory diseases such as asthma. Although asthma affects people of all ages, it has been shown to primarily affect children and adolescents. Certain genetic polymorphisms have been found to be related to the symptoms or severity of the disease. The use of biomarker candidates has been proposed to study such genetic variations in several populations to detect and control this illness in its early stages. In addition, new research topics have been suggested that suggest a role for the metabolism of xenobiotics in the development of asthma. Furthermore, analysis of these polymorphisms in a Mexican population of children is being used as the first step in the development of strategies for detecting vulnerable groups. This paper reviews some genetic polymorphisms associated with allergic responses and their relationship with toxic exposure.

Objective: To identify the relationship between emergency visits for acute respiratory illnesses, acute bronchial asthma crisis (ABAC) and acute respiratory infections (ARI), in children less than 14 years of age, and the daily changes of air pollutant levels, in two central hospitals of Havana City, between October 1st 1996 and March 16 1998., Material and Methods: An ecological time series study was conducted. We assessed the relationship between the presence of acute respiratory illnesses, ABAC, and IRA, and exposure to levels of particles less than 10 microg/m3 (PM10), smoke, and sulfur dioxide (SO2); negative binomial regression models were used to assess latency periods of one to five days as well as the cumulative effect of seven days before the emergency visit., Results: The levels of atmospheric pollutants were low, in general. The 24-hour mean values for PM10 levels, smoke, and SO2, were 59.2 microg/m3 (SD=29.2), 27.7 microg/m3 (SD=21.2), and 21.1 microg/m3 (SD=20.1), respectively. An increase of 20 microg/m3 in the daily average of black smoke was associated with a 2.2% increase (95% CI 0.9-3.6) in the number of emergency visits for ABAC. A 20 microg/m3 increase in the daily average of black smoke and SO was associated with an increase in ARI of 2.4% (95% CI 1.2-3.6), and 5% (95% CI 1.3-5.3), respectively, with a 5-day lag. We also observed a cumulative effect for all the contaminants studied., Conclusions: These results suggest that air pollution levels in Havana City affect children's respiratory health; therefore implementation of control measures is necessary, especially those aimed at decreasing motor vehicle exhaust fumes. According to the Centers for Disease Control and Prevention, there is an annual rate of 7 million deaths because of tobacco use, and this rate is expected to step up to 8 million deaths in 2030 by virtue of smoking-related health complications. Furthermore, it is pathetic to know that almost more than 23% of high school students use tobacco, which if continued at the current rate; about 5.6 million teenagers would be vulnerable to death as mentioned by the World Health Organization. On the other hand, smokers do not only harm themselves but also increase the incidence of pneumonia, asthma, and bronchitis for passive smokers, which by the way contribute to the development of lung cancer and cardiovascular diseases. Moreover, infant death syndrome and slow-growing lung cancer are prevalent in children with continuous exposure to second-hand smoke. These adverse effects emerge from the complex mixture of chemicals present in submicron-sized particles suspended in cigarette smoke like carbon monoxide, benzene, nitrogen oxides, and other gases. Thus, it seems that tobacco smoking has been spreading worldwide as an epidemic with devastating lethal disorders represented in diverse pulmonary, neurological, and cardiovascular-related diseases. Wherefore, this paper discusses the harmful effects of tobacco smoke on human health in accordance with the modern advanced genetic technologies used in exploratory investigations for the relation between smoking and the emergence of human diseases.

By wskazać takie rozwiązania przeprowadzono badania wśród pielęgniarek różnych placówek opieki zdrowotnej a kryterium doboru ankietowanych było wykonywanie pracy w oddziałach takich jak: internistyczny, pediatryczny, geriatryczny, rehabilitacyjny, chorób płuc oraz poradniach przyszpitalnych, gdzie są leczeni pacjenci z astmą oskrzelową. Pielęgniarki propagując działania edukacyjne wpływają na zmianę postaw chorego z biernej na czynną i poprzez to wywierają korzystny wpływ na przebieg choroby. Edukacja chorych w dużej mierze zależy od wiedzy pielęgniarek. Jak pokazały przeprowadzone badania wiele pielęgniarek pracujących z chorymi na astmę nie posiada wystarczającej wiedzy, by prowadzić skuteczną edukację, która zmotywuje do zmiany stylu życia i konkretnych działań na rzecz zapobiegania zaostrzeniom i pełnej kontroli choroby. Po analizie badań zaproponowano konkretne zmiany a jedną z nich jest wyznaczenie konkretnych osób odpowiedzialnych za edukację w danej placówce. Logistics solutions for educational function in nursing Abstract Aim of study is to suggest logistics solutions for nursing personnel in implementation of educational function in hospital wards. In order to indicate such solutions the research among nurses from different healthcare facilities was conducted. A criterion for the selection of respondents was performance of work in different wards such as: internal medicine, pediatric, geriatric, rehabilitation, lung diseases and clinics where the patients with bronchial asthma are treated. Nurses who popularize educational activities influence the change of sick person’s attitude from passive to active and as the result they have a positive impact on course of the disease. Education of sick ones depends on nurses’ knowledge to a great extent. As the research showed, many nurses working with patients with asthma do not have enough knowledge to effectively educate them. Yet, this education should motivate patients to change their life styles, to take specific actions preventing from exacerbations and to fully control the disease. After analysis of research, specific changes were suggested and one of them was to designate specific persons responsible for education in a particular healthcare facility. The role of controlled dosage physical exercise in the treatment of owerweight children with asthma V.I. Velychko, Y.I. Venger, Y.I. Bazhora, Y.O. Pichugina Odessa State Medical University Abstract. The study includes 115 children aged 6 to 11 years with exogenous asthma of varying severity, with different levels of control. Children with asthma were divided into two groups according to the level of physical development: I group (60 children) with harmonic physical development and II group (55 children) overweight. It was found that overweight children who have overweight greater than (4,2 ± 0,3) there are patients with severe bronchial asthma and higher than (2,5 ± 0,2) there are children with the uncontrolled progress of the disease in comparison with a group of patients with a harmonious physical development. There were recommendations on physical exercise for overweight children of 6-11 years with moderate asthma depending on the degree of control of asthma during the major periods of rehabilitation. It is shown that the efficacy of integrated application of dosed physical exercise and lipid-lowering diet with exception of carbohydrate is 1.7 times better in comparison with using only a standard diet in addition to the basic treatment of asthma. Данила Галицького Danylo Halytskiy Lviv National Medical University Abstract Analysis of the research shows that in all groups the increased intensity of lysis proteins was found. These changes probably occurred as a result of a significant imbalance in the system of tissue proteinase inhibitors. In particular, we have found, that there is an increase of azoalbumin by 49,43% (p ≤0,05) against of control group, in the later period on the 26th day of experimental bronchial asthma (BA). On the 33rd day, the level of lysis experienced even greater growth by 85,9% (p ≤0,05) in comparison with the animal intact group, which indicates about the intense stimulation of proteolytic activity in the lungs. Determination of azokasein in the lungs in this experimental model showed a progressive increase of its contents in the lungs on the 26th and 33rd days by 64,6% (p ≤0,05) and 96,3% (p ≤0,05) against of control sizes, that confirms the activation processes of proteolysis. The research of azokolagen has the important value for characteristic of the activity of protein lysis. We have found its gradual increase in the lung, depending on the duration of the antigenic factor. So, on the 26th and the 33rd day of experimental (BA) the increase of content of azokolagen by 70,8% (p ≤0,05) and 79,2% (p ≤0,05) was observed by comparing with the first group of guinea pigs. The content of protein inhibitors in animals with experimental BA has also undergone significant changes in the later period. There was found the authentic reduction of α2-macroglobulin, accordingly, by 30,7% (p ≤0,05) and by 48,5% (p ≤0,05) comparatively to control. As for the research of α1-protease inhibitor in the lungs in the dynamics of the development of the experimental asthma we observe its decrease by 27,7% (p ≤0,05) on the 26th day in comparison with the first group of the animals. Subsequently on the 33rd day of the experiment the decrease of the investigated marker reached 36,1% (p ≤0,05) against intact animals. Evaluating the results of conducted biochemical researches of indicators of proteinase inhibitory system, we can make a conclusion about significant advantage of process of protein catabolism, which increases with the development of pathological process and is a powerful damaging factor and at the same time reducing protection. So, there was found gradual intensive growth of azoalbumin, azokaseein and azokolagen in the lungs in the later period of the formation of asthma, that is especially expressed on the 33rd day of the experiment. The dynamics of the proteinase inhibitor activity is characterized by the decrease of all investigated markers, both on the 26th and on the 33rd day of experimental asthma, that indicates about their oppression. The obtained results give reason to think about the imbalance in the proteinase-inhibitory system with advantage of proteyinase potential on the background of the depression of inhibitors, especially in the later period. Key words: asthma, proteinase-inhibitory system. Резюме Аналіз результатів дослідження свідчить про те, що в усіх групах було встановлено підвищення інтенсивності лізису протеїнів. Зазначені зміни, ймовірно, виникли внаслідок істотного дисбалансу у системі тканинних інгібіторів протеїназ. Зокрема, нами було встановлено, що в пізній період на 26-у добу експериментальної моделі бронхіальної астми (БА) відбувається підвищення азоальбуміну на 49,43% (р ≤0,05) проти групи контролю. На 33-ю добу рівень його лізису зазнавав ще більшого зростання на 85,9% (р ≤0,05) в порівнянні з тваринами інтактної групи, що свідчить про інтенсивну стимуляцію протеолітичної активності в легенях. Визначення азоказеїну в легенях при даній експериментальній моделі показало поступове підвищення його вмісту в легенях на 26-у і 33-ю доби відповідно на 64,6% (р ≤0,05) і 96,3% (р ≤0,05) проти контрольних величин, що підтверджує активізацію процесів протеолізу. Важливе значення для характеристики активності лізису білків має дослідження також азоколагену. Нами виявлено його поступове зростання в легенях в залежності від тривалості дії антигенного фактора. Так, на 26-у і 33-ю доби експериментальної БА спостерігалося зростання вмісту азоколагену відповідно на 70,8% (р ≤0,05) і 79,2% (р ≤0,05) при порівнянні з першою групою морських свинок. Вміст білкових інгібіторів у тварин з експериментальною БА також зазнавав виражених змін у пізньому періоді. Встановлено достовірне зниження рівня α2-макроглобуліну відповідно на 30,7% (р ≤0,05) та на 48,5% (р ≤0,05) відносно контролю. Щодо дослідження α1-інгібітора протеаз в легенях у динаміці розвитку експериментальної бронхіальної астми, то спостерігаємо його зниження на 27,7% (р ≤0,05) на 26-у добу в порівнянні з І групою тварин. В подальшому на 33-ю добу експерименту спад досліджуваного нами маркера досягнув 36,1% (р ≤0,05) проти інтактних тварин. Оцінюючи результати проведених біохімічних досліджень показників протеїназо-інгібіторної системи, можна зробити висновок про суттєву перевагу процесів катаболізму протеїнів, яка зростає по мірі розвитку патологічного процесу і є потужним пошкоджувальним фактором, і, водночас, зниження системи захисту. Так, встановлено поступове інтенсивне зростання азоальбуміну, азоказеїну та азоколагену в легенях у пізній період формування бронхіальної астми, особливо виражене на 33-ю добу експерименту. Динаміка активності інгібіторів протеїназ характеризується зниженням всіх досліджуваних маркерів, як на 26-у, так і на 33-ю доби експериментальної БА, що свідчить про їх пригнічення. Одержані результати дають підставу думати про порушення балансу в системі у протеїназо-інгібіторній системі з перевагою протеїназного потенціалу на тлі депресії інгібіторів, особливо у пізній період. Резюме Нарушения показателей протеиназно-ингибиторной системы в легких морских свинок в поздний период формирования экспериментальной бронхиальной астмы. Анализ результатов исследования свидетельствует о том, что во всех группах было установлено повышение интенсивности лизиса протеинов. Отмеченные изменения, вероятно, возникли в результате существенного дисбаланса в системе тканевых ингибиторов протеиназ. В частности, нами было установлено, что в поздний период на 26-ые сутки экспериментальной модели бронхиальной астмы (БА) происходит повышение азоальбумина на 49,43% (р ≤0,05) против группы контроля. На 33-ю сутки уровень его лизиса испытывал еще больший рост на 85,9% (р ≤0,05) по сравнению с животными интактной группы, которая свидетельствует об интенсивной стимуляции протеолитической активности в легких. Нами выявлен его постепенный рост в легких в зависимости от длительности действия антигенного фактора. На 26-ые и 33-ие сутки экспериментальной БА наблюдался рост содержимого азоколлагена соответственно на 70,8% (р ≤0,05) и 79,2% (р ≤0,05) при сравнении с первой группой морских свинок. Содержимое белковых ингибиторов у животных с экспериментальной БА также испытывал выраженные изменения в позднем периоде. Установлено достоверное снижение уровня α2-макроглобулина соответственно на 30,7% (р ≤0,05) и на 48,5% (р ≤0,05) относительно контроля. Оценивая результаты проведенных биохимических исследований показателей протеиназо-ингибиторной системы, можно сделать вывод о существенном преимуществе процессов катаболизма протеинов, которая растет по мере развития патологического процесса и является мощным разрушительным фактором, и, в то же время, снижение системы защиты. Так, установлен постепенный интенсивный рост азоальбумина, азоказеина и азоколлагена в легких в поздний период формирования бронхиальной астмы, особенно выраженное на 33-ие сутки эксперимента. Динамика активности ингибиторов протеиназ характеризуется снижением всех исследуемых маркеров, как на 26-ые, так и на 33-ие сутки экспериментальной БА, что свидетельствует об их притеснении. Полученные результаты дают основание думать о нарушении баланса в системе в протеиназо-ингибиторной системе с преимуществом протеиназного потенциала на фоне депрессии ингибиторов, особенно в поздний период. Данила Галицького Danylo Halytskiy Lviv National Medical University Article The gradual increase of contents of the azoalbumin, azokasein and azokolagenhas been established in the research already in the early period of forming of experimental bronchial asthma (BA). So, we are observing the increase of indicators of azoalbumin in the early period of the development of BA (on the 5th,19th day ), accordingly on 31,8%(p≤0,05) and 42,4% (p≤0,05) in comparison with control group. The similar tendency of changes was discovered during the research of the level of azokasein, which also were increasing in the conditions of the development of BA, as on the 5th day by 45,1% (p≤0,05), so on the 19th day of the experimental model of the disease by 51,25% (p≤0,05) in comparison with the group of intact animals. Similar data were obtained in the early period of bronchial asthma according to the contents of azokolagen. Its increase by 50% (р ≤0,05) and 58,3% (р ≤0,05) was discovered in comparison with the control in condition of forming of this pathology. The investigation of protein inhibitors, in particular, α1-protease inhibitor (α1-IP) in the early period of the experiment has showed its slight increase by 14,7%(р≤0,05) in comparison with the first group of animals on the 5th day of the experiment, that may prove about the inclusion of compensatory mechanisms of protection of proteinase-inhibitory system. However, on the 19th day of the experimental BA, the indicators α1-IP is located on the level of control quantities (р≤0,05). In relation to the activity of the α2-macroglobulin in the lungs on the researched days of the experimental BA, it is increasing by 29,7% (р≤0,05) on the 5th day and it doesn’t suffer significant changes on the 19th day of the experiment in comparison with the group of the intact guinea pigs (р≤0,05). Therefore, our researches of biochemical indicators of proteinase-inhibitory system in the lungs discovered the increase of proteolytic activity on the background of insufficient marked compensatory mechanisms of protein inhibitors with advantage of damage mechanism over protection mechanisms in the early period of the development of experimental model BA. Реферат У роботі встановлено поступове зростання вмісту азоальбуміну, азоказеїну та азоколагену вже в ранній період формування експериментальної бронхіальної астми (БА). Так, спостерігаємо підвищення показників азоальбуміну в легенях у ранній період розвитку БА (5-а, 19-а доби) відповідно на 31,8% (р ≤0,05) і 42,4% (р ≤0,05) в порівнянні з контрольною групою. Аналогічний напрям змін виявлено під час дослідження рівня азоказеїну, який також зростав за умов розвитку БА , як на 5-у добу на 45,1% (р ≤0,05), так і на 19-у добу цієї експериментальної моделі хвороби на 51,25% (р ≤0,05) в порівнянні з групою інтактних тварин. Подібні дані одержані в експерименті в ранньому періоді бронхіальної астми щодо вмісту азоколагену. Встановлено його зростання відповідно на 50% (р ≤0,05) і 58,3% (р ≤0,05) в порівнянні з контролем за умов формування цієї патології. Дослідження білкових інгібіторів, зокрема, α1-інгібітора протеаз (α1-ІП) в ранні терміни експерименту показало його незначне зростання на 14,7% (р≤0,05) в порівнянні з І групою тварин на 5-у добу експерименту, що може свідчити про включення компенсаторних механізмів захисту протеїназо-інгібіторної системи. Проте, вже на 19-у добу експериментальної БА показники α1-ІП знаходяться на рівні контрольних величин (р≤0,05). Щодо активності α2-макроглобуліну в легенях на досліджувані доби експериментальної БА, вона підвищується на 29,7% (р≤0,05) на 5-у добу і не зазнає достовірних змін на 19-у добу експерименту в порівнянні з групою інтактних морських свинок (р≤0,05). Таким чином, проведенні нами дослідження біохімічних показників протеїназо-інгібіторної системи в легенях виявили зростання протеолітичної активності на тлі недостатньо виражених компенсаторних механізмів білкових інгібіторів з перевагою механізмів пошкодження над механізмами захисту вже у ранній період розвитку експериментальної моделі БА. Реферат Особенности сдвигов состояния протеїназо-ингибиторной системы в легких морских свинок в раннем периоде экспериментальной бронхиальной астмы. В работе установлен постепенный рост содержимого азоальбумина, азоказеина и азоколлагена уже в ранний период формирования экспериментальной бронхиальной астмы (БА). Наблюдаем повышение показателей азоальбумина в легких в ранний период развития БА (5-я, 19-я сутки) соответственно на 31,8% (р≤0,05) и 42,4% (р≤0,05) по сравнению с контрольной группой. Аналогичное направление изменений выявлено во время исследования уровня азоказеина, который также увеличевался при условиях развития БА, как на 5-е сутки на 45,1% (р ≤0,05), так и на 19-е сутки этой экспериментальной модели болезни на 51,2% (р≤0,05) по сравнению с группой интактных животных. Подобные данные получены в эксперименте в раннем периоде БА относительно содержимого азоколлагена. Установлен его рост соответственно на 50% (р≤0,05) и 58,3% (р≤0,05) по сравнению с контролем за условиями формирования этой патологии. Матеріали та методи. Комплексно обстежено 224 дитини, хворі на БА, віком 6 – 17 років, із визначенням гомоцистеїну та судинного ендотеліального фактора росту. В якості контрольної групи було обстежено 40 здорових дітей. Результати. Встановлено, що вірогідність виникнення небажаних ефектів найвища при застосуванні монотерапії ІГК (OR 8,889; 95 % CI 4,803 – 16,449) та комбінованій терапії із кромонами (OR 1,111; 95 % CI1,051 – 2,734). Визначено, що чинниками ризику зростання вмісту гомоцистеїну більше 17,36 (11,3 – 24,5) мкмоль/л та VEGF вище 431,66 (238,04 – 658,09) пг/мл в сироватці крові дітей, хворих на БА були застосування ДАІ без доставочного пристрою та збільшення дози ІГК. Abstract Purpose - estimation of basic anti-inflammatory therapy with inhaled glucocorticoids in the development of endothelial dysfunction in children with asthma. Materials and methods. Comprehensively examined 224 children with asthma, aged 6 - 17 years, with determination of homocysteine and vascular endothelial growth factor. 40 healthy children were examined as a control group. Results. Established that the likelihood of adverse effects when using the highest GCI monotherapy (OR 8,889; 95% CI 4,803 - 16,449) and combination therapy with mast cell membranes stabilizeres (OR 1,111; 95% CI1,051 - 2,734). Determined that the risk factors increase homocysteine than 17.36 (11.3 - 24.5) mmol / l and VEGF above 431.66 (238.04 - 658.09) pg / ml in serum of children with asthma were using of doses aerosol inhalators without conveying device and without increasing of IGC dose. Rozwiązaniem tego problemu jest objęcie pacjentów programem szkoleniowym dotyczącym terapii astmy i możliwości kontroli choroby. Abstract Lack of control of asthma is most often caused by failure patients' medical recommendations (noncompliance). The difficulties in the control of asthma, increase the likelihood of status asthmaticus. Patients with uncontrolled asthma often use the visits, they are more often hospitalized and more likely to get on Hospital Emergency Ward. The solution to this problem is to extend patient training program on asthma therapy and the ability to control the disease. El objetivo del trabajo consistió en identificar la posible asociación entre las concentraciones de inmunoglobulinas con la exposición a factores ambientales de riesgo e indicadores de morbilidad respiratoria. Se estudiaron 224 niños de la zona expuesta y 159 de la zona no expuesta a la contaminación atmosférica originada por el transporte. Mediante la entrevista a los padres se realizó una encuesta sobre factores ambientales de riesgo, antecedentes familiares de trastornos respiratorios y sintomatología respiratoria en los últimos 12 meses. Se cuantificaron las inmunoglobulinas IgA, IgG, IgM e IgE. Se encontró una fuerte asociación de las concentraciones elevadas de IgE con los antecedentes patológicos familiares de asma, síntomas respiratorios sugestivos de asma bronquial y otras enfermedades respiratorias; las concentraciones bajas de IgG se asociaron con la exposición a la contaminación del aire originada por el transporte.The objective of this paper was to identify the possible association between the concentrations of immunoglobulins, the exposure to environmental risk factors and the indicators of respiratory morbidity. 224 children from the zone exposed to the air pollution caused by transportation and 159 from the non-exposed zone were studied. Parents were interviewed and a survey was done on the environmental risk factors, family history of respiratory disorders and respiratory symptomatology during the last 12 months. IgA, IgG, IgM and IgE were quantified. It was found a strong association of the elevated IgE concentrations with the family pathological history of asthma, respiratory symptoms suggesting bronchial asthma and other respiratory diseases. The low IgG concentrations were associated with the exposure to air pollution produced by transportation.

Se planteó que el asma bronquial intrínseca y alérgica ha sido definida como un trastorno inflamatorio crónico persistente, basados en un creciente cuerpo de evidencias. Se ha visto que las características clínicas del asma bronquial están relacionadas con parámetros celulares, así como con sustancias producidas por los procesos de inflamación bronquial, por lo tanto se están investigando medios de evaluación y monitoreo de la inflamación asmática según estas variables que pudieran permitir, al mismo tiempo, el diagnóstico y la evolución del asma. Desde que los eosinófilos, linfocitos T, mastocitos, macrófagos, neutrófilos, células epiteliales y estructurales, varios mediadores proinflamatorios y proteínas, particularmente las de los gránulos de los eosinófilos, han sido implicados en la patogénesis del asma bronquial, se ha anticipado que varios de ellos pudieran ser diagnósticos del asma bronquial o servir como marcadores para monitorear la inflamación bronquial subyacente. Se hizo una revisión del estado de opinión actual en relación con estos mediadores y se expusieron en el presente trabajo.Intrinsic and allergic bronchial asthma has been defined as a persistent chronic inflammatory disorder based on increasing evidences. It has been proved that the clinical characteristics of bronchial asthma are connected with cellular parameters a well as with substances produced by bronchial inflammation processes. Therefors, took for evaluation and monitoring of asthmatic inflammation are being studied according to these variables that may allow to know the diagnosis and evolution of asthma at the same time. As eosinophils, T-limphocytes, mastocytes, macrophages, neutrophils, epithelial and structural cells, some proinflammatory mediators and proteins, particularly those from the eosinophil granules, have been involved in the pathogenesis of bronchial asthma, it has been anticipated that some of them may be used in the diagnosis of bronchial asthma or may serve as markers for monitoring subjacent bronchial inflammation. It was made a review of the present state of opinion regarding these mediators, which were included in this paper.

La hipótesis higiénica intenta dar explicación coherente al incremento del asma en países desarrollados. Diversos estudios centran su atención en el papel que desempeñan las inmunizaciones sobre el sistema inmune y la influencia que ejercen sobre la atopia. El objetivo de este trabajo fue determinar las diferencias entre las concentraciones séricas de anticuerpos contra el antígeno de superficie del virus de la hepatitis B (anti-AgsHB), IgE total (IgE) y área de la prueba cutánea con el antígeno (Ag) vacunal, de la vacuna cubana contra la hepatitis B, Heberbiovac-HB, en niños asmáticos y no asmáticos. Se evaluó la influencia de la gravedad del cuadro clínico del asma sobre la respuesta inmune frente al Ag: se realizó estudio prospectivo y transversal, constituido por 103 niños asmáticos atendidos en el Hospital Infantil Docente "William Soler" y 37 no asmáticos de un área de salud del Centro de Atención Primaria "Julián Grimau" de la Ciudad de La Habana, Cuba. Todos los niños fueron vacunados con la vacuna Heberbiovac-HB, según esquema cubano de vacunación. Se realizó el test de ANCOVA para determinar las diferencias entre ambos grupos. En el grupo de niños asmáticos, se realizó un test de Duncan pos-hot, para establecer las diferencias, según la severidad del asma. Las diferencias significativas se centraron en las pruebas cutáneas, siendo menor el área de la prueba cutánea en asmáticos con respecto a los no asmáticos; la prueba cutánea control (PCC) y la prueba cutánea vs. antígeno vacunal de la hepatitis B (PCHB), PCC (166,5 vs. 233,6 mm²; p= 0,0077) y la PCHB (1 020,4 vs. 1 318,15 mm²; p= 0,0173). La severidad del cuadro clínico en asmáticos se relacionó de manera directa con las concentraciones de IgE e inversa con el área de la PCHB. Los niños asmáticos respondieron con menor área en la prueba cutánea retardada al compuesto vacunal, que los no asmáticos. La respuesta se deterioró a medida que se incrementó la gravedad del asma y las concentraciones de IgE.The hygienic hypothesis tries to give a coherent explanation to the increase of asthma in the developed countries. Several studies focus their attention on the role played by immunizations in the immune system and on the influence they exert on atopy. The objective of this paper was to determine the differences among the serum concentrations of antibodies against the hepatitis B surface antigen (anti-AgsHB), the total IgE (IgE) and the area of the skin test with the vaccinal antigen (Ag) of the Cuban vaccine against hepatitis B, Heberbiovac-HB, in asthmatic and non-asthmatic children. The influence of the severity of the clinical picture of asthma on the immune response against Ag was evaluated. A prospective and cross-sectional study that included 103 asthmatic children attended at "William Soler" Children Teaching Hospital and 37 non-asthmatics from a health area of "Julián Grimau" Primary Care Center in Havana City, Cuba, was conducted. All the children were vaccinated with the Heberbiovac-HB vaccine according to the Cuban vaccination scheme. ANCOVA test was made to determine the differences between both groups. In the group of asthmatic children, Duncan's post-host test was carried out to establish the differences according to asthma severity. The significant differences were centered on the skin tests. The area of the skin test in asthmatics was lower than that of the non-asthmatics; the control skin test (CST) and the skin test against the hepatitis B vaccine antigen (STHB), CST (166.5 vs. 233.6 mm2; p=0.0077) and STHB (1 020.4 vs. 1 318.15 mm2; p=0.0173). The severity of the clinical picture in asthmatics was directly related to the IgE concentrations and inverse to the area of STHB. The asthmatic children responded with less area to the vaccinal compound in the delayed skin test. The answer deteriorated as the severity of asthma and the IgE concentrations increased.

El objetivo del presente trabajo fue determinar el uso de los servicios ambulatorios por pacientes asmáticos en edad pediátrica y los gastos de bolsillo relacionados con su atención. Se empleó una cohorte de 232 asmáticos (5 a 14 años), seleccionados aleatoriamente en 2 áreas de salud del municipio Cerro, en Ciudad de La Habana, quienes fueron visitados en sus hogares mensualmente durante 6 meses. Se aplicó un cuestionario que exploró variables relacionadas con la enfermedad y los gastos adicionales por alimentos, transporte y medicamentos. Para el análisis se utilizaron proporciones, medias y percentiles (25-50-75). La media de contactos con los servicios de salud por paciente fue 3,6 (DS 4,5). La utilización de los servicios de emergencia representó el 49,1 % del total de consultas. La severidad de la enfermedad constituyó un factor predictor del uso de los servicios ambulatorios en dieferentes niveles de atención. El 27 % de los pacientes no contactó con los servicios de salud, cuyo uso no se asoció ni a la escolaridad ni a la ocupación de los padres (pThe objective of the present paper was to determine the use of outpatient services by asthmatic pediatric patients and the costs of their care. A cohort of 232 asthmatic children (5 to 14 years of age) was used, who were randomly selected from two health areas of Cerro municipality in the City of Havana and monthly visited at home for 6 months in a row. The administered questionnaire explored variables linked to disease and extra costs from food, transportation and medication. For the analysis, ratios, means and percentiles (25-50-75) were used. The mean of contacts with health care services per patient was 3.6 (SD 4,5). The use of emergency services accounted for 49.1 % of the total number of appointments. The disease severity was a predictive factor for the use of outpatient services at different levels of care. Twenty seven percent of patients did not contact any health service, the use of which was associated neither to schooling nor thetype of work of the parents (p
