Objective. To present a clinical observation of a patient with a moderate course of coronavirus infection and COVID-19-associated liver damage. Materials and methods. A clinical case of patient P., 56 years old, with a diagnosis of coronavirus infection COVID-19 and changes in liver function of a mixed nature, regarded as COVID-19-associated liver damage, treated with ursodeoxycholic acid (UDCA), is described. A dynamic assessment of physical data, general and biochemical blood tests, ultrasound of the abdominal cavity was performed after two months. Results. Against the background of ongoing therapy with the use of ursodeoxycholic acid, there was an improvement in the patient's general well-being, positive laboratory dynamics of alanine transferase (ALT), aspartate transferase (AST), and total bilirubin. Ultrasound of the abdominal cavity revealed normalization sizes of the right and left lobes of the liver, compared with the initial data. Conclusion. The use of ursodesoxycholic acid in a patient with COVID-19-associated liver damage was accompanied by positive dynamics in the form of normalization of hepatic enzymes and improvement of general condition. More research is needed to investigate the mechanism of action of UDCA in patients with COVID-19 and liver pathology.

Objective of the study. A 5-year prospective study was aimed to evaluate the efficacy of Ursosan treatment for dissolving gallstones.Material and methods. Patients who had single gallbladder stones were randomized to group A, in which 47 people completed the study (16 men and 31 women, median age is 67.6 years), and group B – 41 people 14 men and 27 women, median age is 68, 1 years). All patients of group A continuously received continuous treatment with ursodeoxycholic acid drug Ursosan at a dose of 10 mg /kg per day for 5 years. The patients of group D did not receive Ursosan; these individuals received periodic courses of spasmodic drugs. The patients passed clinical examination, laboratory tests, transabdominal ultrasound of the liver and biliary tract before the study, 2 times a year for 5 years and after the study.Results. After 5 years of treatment, stones were detected in 10.6% of patients in group A. Along with the great progress in the fight against malignant neoplasms through the improvement of surgical care and the development of chemotherapy, the search for strategies and opportunities to prevent cancer is becoming increasingly important and promising. Early detection and rational treatment of chronic precancerous diseases must be a priority. In this regard, the possible role of ursodeoxycholic acid (UDCA, ursosan, etc.) has been closely studied in recent years. An overview of the role of ursodeoxycholic acid (UDCA) in the prevention of cancer is presented. The pathogenetic basis of this action is the proven normalizing effect of UDCA on bile acid metabolism, microbiome condition and apoptosis. The paper summarizes the latest data on the ability of UDCA to withstand malignization in the liver, throughout the gastrointestinal tract and beyond. These numerous experimental and clinical studies allow us to assert that UDCA drugs, except for the well-known and versatile hepatoprotective effect, can be considered as effective and promising means of cancer prevention throughout the digestive tract and associated organs. Further research into the broad spectrum of UDCA’s ability to prevent gastrointestinal malignization and beyond is reasonable and promising.

The article presents international materials on studying the issue of biliary sludge (BS). The authors also share their own data resulting from conducted studies, where the first group included 1107 patients with BS; the second – 208 patients with different forms of BS; the third group – 40 patients with regular course of BS, in the absence of pharmacological influence; the fourth group – 60 patients (20 patients for each type of BS) with UDCA Exhol 500 mg No. 50 and antispasmodic agent Sparex 200 mg No 30. Intake of Exhol at an initial dose of 15 mg/kg/day was effective in 97% of cases and BS elimination was achieved in 12 weeks. The BS elimination rate depended on its type (1st – 4 weeks, 2–3rd types – 8–12 weeks).

The article gives information on the scope of ursodeoxycholic acid (UDCA). The most evident positive effects of therapy for cholelithiasis (SCI) and in primary biliary cirrhosis (PBC). The probability of using UDCA as a means of preventing CLS in cases of biliary sludge (SB) and in the risk group in elderly and elderly people is discussed. The data of our own study of the results of UDCA in 34 patients with non-alcoholic fatty liver disease, combined with the dyskinesia of the gallbladder (BL), BS and gastroesophageal reflux disease (GERD) are presented. The article analyses information about the functional disorders of the gall bladder and the sphincter of Oddi, the principles of treatment of patients with peculiarities in the gall bladder, the main drugs for the functional disorders of the biliary system are provided, possibilities of the domestic drug ursodeoxycholic acid (UDCA) Urdoxa are provided.Conclusion: According to modern recommendations, the use of the Urdoxa drug is the basis for initial therapy for patients with biliary sludge and for functional disorders of the gall bladder and sphincter of Oddi.

The article tells about the indications for use of ursodeoxycholic acid (UDCA). The most significant positive effects were demonstrated in primary biliary cirrhosis (PBC) and cholelithiasis (CL). The possibility of using UDCA for CL prophylaxis during biliary sludge (BS) and in risk groups -people of elderly and senile age - is discussed. The article presents the results of the authors’ study of UDCA administration in 32 patients with gallbladder deformity (GD) and BS. Grinterol (UDCA) demonstrated high clinical efficacy, safety and no side effects.

The article describes the experience of treatment of erosive reflux esophagitis in 30 patients after gastric resection, having mixed reflux determined by 24-hour esophageal pH-impedancemetry. Pharmacotherapy for background and maintenance treatment included a combination of PPI (Omez) with UDCA (Livodexa). The efficacy of different dosages of the drugs was evaluated in 2 groups of patients during background treatment: group 1 received Livodexa 10 mg/kg in combination with Omez 20 mg per day, group 2 received Livodexa 15 mg/kg in combination with Omez 40 mg/day. During maintenance treatment, subgroup 1 received Livodexa 2.5 mg/kg and Omez 20 mg, subgroup 2 - 5.0 mg/kg Livodxa and Omez 20 mg per day for 2 months. Changes in the clinical and endoscopic patterns were evaluated. The study showed that, according to the clinical and endoscopic data, Livodexa 15 mg/kg and Omez 40 mg/day for 112 days for erosive RE was effective in 76.6% (23) of cases. In the remaining 7 (23.4%) patients erosions healed by the 140th day of treatment. With regard to maintenance treatment, the combination of Livodexa 5.0 mg/kg with Omez 20 mg/day was effective.

In article we provided description of rare form of autoimmune pancreatitis. This rare form characterized by specific morphology picture, increase level of Ig, common comorbidity with other autoimmune diseases, positive result of corticosteroid therapy. This clinical case shows a situation with unapproved surgery treatment of a young woman that leads to disability and severe complications. Article includes short historical part about abundance, specific of this disease, classification and modern ways of treatment. We pay additional attention to differential diagnosis between autoimmune pancreatitis and cancer before perform any treatment especially surgery.
