The most important task in providing care to patients with myocardial infarction is maintaining myocardial contractility. The article discusses issues related to the influence of genetic characteristics of patients on the repair of infarcted myocardium, the remodeling process, and restoration of left ventricular systolic and diastolic function. One approach to improving the predictive ability of genetic testing is to combine information about many nucleotide sequence variants into a single risk score, often called a polygenic risk score. The article examines recent publications on the creation and use of polygenic risk scores. The use of genetic methods during examination, further consideration of the individual characteristics of each patient when choosing therapy and prescribing a course of rehabilitation will allow for an individual approach to each patient, which in turn should have a positive impact on the disease prognosis.

Our knowledge regarding chemical structure and properties of heparin and its derivatives, including biological properties in blood plasma, on the cell surface and while interacting with receptors, has been progressively growing. New insights are followed by the expansion of therapeutic opportunities and indications for the use of heparins. There are prerequisites for the creation of new generation drugs with modified properties that reduce a bleeding risk while applied for a non-anticoagulant goal. The non-anticoagulant heparin properties allow to consider it as a candidate for pathogenetic treatment of patients with COVID-19. This review focuses on the anticoagulant and non-anticoagulant heparin properties as well as the underlying molecular mechanisms.

Aim.To study the association of single-nucleotide polymorphismrs3025058(5а/6а) with the development of stroke in patients of the East Siberian population with cardiovascular pathology and risk factors for its development. Materials and methods.The study involved 260 patients with stroke (age [57.0; 51.062.0]) and 272 patients of the control group (age [55.0; 51.062.0]). Among the patients who underwent stroke, 157 men and 103 women. The control group included 170 men and 102 women. The examination of the main group included: collection of complaints, anamnesis, clinical examination, computed tomography of the brain, electrocardiography, echocardioscopy, ultrasound duplex scanning of the extracranial brachiocephalic arteries, 24-hour monitoring of blood pressure and heart rate, analysis of the blood coagulation system. The patients of the main group had the following cardiovascular pathology and risk factors: arterial hypertension, paroxysmal supraventricular tachycardias, dyslipidemia, atherosclerosis of extracranial brachiocephalic arteries, disorders of the hemostasis system. The control group was examined within the framework of the international project HAPIEE. Molecular genetic research was carried out by real-time PCR. Statistical processing of the material was carried out using the Statistica for Windows 7.0, Excel and SPSS 22 application software. Results.The study established statistically significant associations between the 5a/5a genotype and the 5a allele and stroke in the general group of patients, as well as in the subgroup of men, subgroups of patients with extracranial brachiocephalic arteries atherosclerosis and dyslipidemia. In the subgroup of patients with cardiac arrhythmias, statistically significant results were obtained only for allele 5a, and in the subgroup of women with stroke, subgroups of patients with arterial hypertension and hypercoagulation, no significant associations ofrs3025058(5a/6a) polymorphism with stroke were found. Conclusion.Genotype 5a/5a and allele 5a of the single-nucleotide polymorphismrs3025058(5а/6а) increase the risk of stroke in individuals from the East Siberian population, including those in the presence of such risk factors as extracranial brachiocephalic arteries atherosclerosis and dyslipidemia.

Atrial fibrillation is one of the most common heart rhythm disorders. The most prominent risk factor for atrial fibrillation is advanced age. Population ageing contributes to an increase in both the prevalence of this pathology and socio-economic burden of the disease for society in general and the patient in particular. Adequate therapy and prevention of atrial fibrillation requires the search for novel prognostic risk markers for disease development, progression, and patients response to therapy. One of these markers is the length of telomeres structures at the ends of chromosomes that protect them from degradation during cell division. The article provides an overview of world studies, both confirming and disproving the role of leukocyte telomere length in atrial fibrillation development.

In patients of the experimental group, the following cardiovascular pathology and risk factors were present: arterial hypertension, paroxysmal supraventricular tachycardias, dyslipidemia, atherosclerosis of the brachiocephalic arteries, and disorders of the hemostatic system. The control group was surveyed in the framework of the international “HAPIEE” project. Molecular genetic research was performed by real-time PCR. Statistical processing of the material was carried out using the following software: Statistica for Windows 7.0, Excel and SPSS 22. Results. When studying the association of SNP rs10507391 (A>T) with the acute CVA development in all the analyzed groups and subgroups of patients, a link was established between the rare TT genotype and the T allele and an increased risk of acute CVA.Conclusion. TT genotype and T allele of the SNP rs10507391 (A>T) increase the risk of acute CVA in patients regardless of previous cardiovascular pathology and its risk factors, including patients with arterial hypertension, supraventricular tachyarrhythmias, atherosclerosis of brachiocephalic arteries, impaired lipid metabolism and hemostasis system.

Objective. To demonstrate the possibility of having a spondyloarthritic mask of osteomalacia in patients of both sexes.Materials and methods. Two clinical cases of osteomalacia occurring under the mask of spondyloarthritis in patients treated at the Clinical Rheumatology Hospital of Saint-Petersburg, Russia, as well as similar cases described in the literature, were analyzed.Results. In the cases described, patients were diagnosed with a disease from the group of spondyloarthritis based on a number of individual symptoms, such as pain in the lower back and stiffness, as well as instrumental examination data. At the same time, with in-depth evaluation, the pain in the lower back did not did not met to the inflammatory pain ASAS criteria, and there were also no signs of general laboratory activity of the disease. The conventional therapy for spondyloarthritis was ineffective. With the re-evaluation of clinical, laboratory and instrumental data, the diagnosis was changed to osteomalacia. Appointment of adequate therapy with vitamin D after a review of the diagnosis resulted in both a regression of clinical symptoms and an improvement in laboratory parameters.Conclusions. If the patient has pain in the lower back, especially without clear signs of inflammatory, no signs of general laboratory activity, further examination is necessary to clarify possible osteomalacia. Spondyloarthritis should be only diagnosed in cases with the secondary nature of symptoms excluded.

BACKGROUND. Ultrasound study significantly expanded the possibilities of bedside diagnosis in patients with respiratory failure. Using ultrasound, it is possible to determine the volume of lung damage in the form of collapsed alveoli and infiltration areas with preserved airness of the lung tissue. AIM OF STuDY To study the possibility of assessing the recruitment maneuver of the alveoli based on changes in the ultrasound signs of lung tissue damage.MATERIAL AND METHODS. A prospective study was performed in the Clinic of Anesthesiology and Resuscitation of S.M. Kirov Military Medical Academy. The study included 36 patients who were treated in the period from 2010 to 2017 with a duration of respiratory support of at least 48 hours and oxygenation index less than 300 mmHg. For 36 patients, 48 alveoli recruitment maneuvers were performed according to a step-by-step method under the control of dynamic compliance and average tidal volume. Ultrasound determined the type and extent of destruction of lung tissue by signs of infiltration and consolidation.RESULTS. In the studied patients, after carrying out a maneuver of recruitment of the alveoli, arterial blood oxygenation indices increased statistically significantly, PaCO2 level decreased, pulmonary tissue compliance improved, respiratory volume grew. All this confirmed the mobilization of the alveoli and improved lung ventilation. Ultrasonographic evaluation of lung tissue showed a significant decrease in the severity of the ultrasound sign of infiltration after recruitment maneuver from 46.5 (38; 57.5) to 37.5 (30.5; 49.5). However, recruitment had practically no effect on the volume of the consolidated area of lung tissue: the general consolidation index before (4 (3; 5)) and after (4 (3; 5)) the maneuver had no statistically significant differences.CONCLUSIONS. The pneumonia-affected consolidated lung tissue has a low recruitment potential and the volume of consolidation does not change with the growth of PEEP. After the recruitment maneuver, the number of B-lines decreases, indicating a decrease in infiltration and an increase in lung airness.Authors declare lack of the conflicts of interests.

Material and methods. The study involved 260 patients with stroke (157 men and 103 women) and 272 patients of the control group (170 men and 102 women). The following cardiovascular pathology and risk factors were present in the patients of the main group: arterial hypertension, paroxysmal supraventricular tachycardias, dyslipidemia, atherosclerosis of the brachiocephalic arteries, disorders of the hemostatic system. All patients underwent clinical, instrumental and molecular genetic examination. Statistical processing of the material was carried out using the set of application programs Statistica for Windows 7.0, Excel and SPSS 22. Results. As a result of the study, no statistically significant associations of genotypes and alleles of the rs1800801 polymorphism (C>T) with stroke were found in any of the analyzed groups and subgroups. Conclusion. SNP rs1800801 (C>T) does not have a significant effect on the development of stroke in people of the East Siberian population, regardless of the preceding cardiovascular pathology and risk factors.

Background. Atrial fibrillation (AF) is the most common type of heart rhythm disturbance, leading to the development of lifethreatening conditions, such as cardio embolism, heart failure, and even sudden cardiac death. In recent years, the genetic aspects of AF have been actively discussed. The largest number of genetic predictors of AF was identified after a full genome-wide association studies (GWAS). Given that so far no studies of the association of rs10824026 polymorphism of chromosome 10q22 with the development of AF have been conducted in the Russian population, we conducted this clinical study. Aim. Checking the associations of the development of AF with the single-nucleotide polymorphism rs10824026 of the SYNPO2L gene in the East-Siberian population. Materials and methods. The study design was formed in accordance with the National Standard of the Russian Federation Good Clinical Practice, GOST P 52379-2005. The study uses design - “case-control”. The main group of patients - patients with known cardiac arrhythmias by the type of AF (n=106, average age 57.0±9 years, men 49.4%, women 50.6%), the group was formed using the criteria of the World Health Organization and the European Society of cardiologists. The control group (n=105, average age 57.0±9 years, men - 50.0%, women - 50.0%) was selected by age and gender from the DNA bank of international studies MONICA (Multinational MONItoring of trends and determinants in cardiovascular disease) under a joint agreement with the Research Institute of Therapy and preventive medicine - Novosibirsk. DNA was isolated by phenol-chloroform extraction. Among other things, among the research methods, routine laboratory methods were used; instrumental data; and invasive tactics such as CAG. Results. As a result of clinical genetic testing, it was found that the frequency of G/G polymorphism of the SYNPO2L gene in patients with AF shows a statistically significant difference.

The study involved 260 patients with acute CVA (age [57,0; 51,0-62,0]) and 272 patients of control group (age [55,0; 51,0-62,0]). Among patients with acute CVA there were 157 men and 103 women. The control group included 170 men and 102 women. Examination of the experimental group included: acquisition of complaints, anamnesis, clinical examination, computed cerebral tomography, electrocardiography, echocardioscopy, ultrasound duplex scanning of extracranial brachiocephalic arteries, 24-hour monitoring of blood pressure and heart rate, analysis of the blood coagulation system. In patients of the experimental group, the following cardiovascular pathology and risk factors were present: arterial hypertension, paroxysmal supraventricular tachycardia, dyslipidemia, atherosclerosis of the brachiocephalic arteries, hemostatic system disorders. The control group was examined as part of the international HAPIEE project. Molecular genetic studies were performed by real-time PCR.Results. In all analyzed groups and subgroups of patients, a statistically significant relationship was found between the CC genotype and the C SNP allele rs662799 (A>G) and an increased risk of stroke.Conclusion. The CC genotype and the C SNP allele rs2230806 (C>T) increases the risk of acute cerebrovascular accident in patients regardless of previous cardiovascular pathology and risk factors, including in patients with arterial hypertension, supraventricular tachyarrhythmias, atherosclerosis of brachiocephalic arteries, and disorders of lipid metabolism and hemostatic system.

There were no statistically significant differences in the frequencies of genotypes and single nucleotide polymorphism rs556621 alleles (G>T) in the subgroup of patients with stroke and those in the control group. The frequency of the rare TT genotype among patients with stroke was 13.3%±4.16, among healthy individuals – 8.8±3.37% (p=0.1). Gender differences when comparing the frequencies of genotypes and alleles were also not detected (p>0.05). The frequencies of the TT genotype were approximately the same in the subgroup of patients with arterial hypertension (13.1%±4.22) and in the control group (7.4±5.25%; p>0.05). No significant differences were observed in the frequencies of the rare genotype of the studied polymorphism in the subgroup of patients with supraventricular tachycardias (20.0±14.37%), hypercoagulability (15.9±7.64%) and the control group (8.8±3.37%), p>0.05. A statistically significant relationship was found between the rare genotype TT of single nucleotide polymorphism rs556621 (G>T) and the development of stroke in patients with dyslipidemia and atherosclerotic lesions of the coronary arteries (p=0.041; odds ratio 1.86, 95% confidence interval 1.02-3.41).Conclusion. The genotype of TTs of single nucleotide polymorphism rs556621 (G> T) increases the risk of developing stroke in patients with dyslipidemia and atherosclerosis of the brachiocephalic arteries compared with carriers of the GG and GT genotypes. The obtained data are recommended to be considered when prescribing lipid-lowering and antithrombotic therapy.

After studying the Association of the TT genotype and the T allele with the development of lone atrial fibrillation (AF) and constructing a logistic regression model, it was revealed that the probability share of lone AF in the presence of the TT genotype is 6,2%; for the absence of lone AF – 95,1%. The model predicts the development of lone AF with a sensitivity of 17,7%. The overall percentage of correct predictions is 65,4%. Background. To study the prediction of the probability of occurrence of lone AF depending on the predictor genotype of polymorphism rs2200733 in chromosome 4q25 using the method of multiple logistic regression and to determine the measure of statistically significant influence of the predictor genotype on the development of pathology. Materials and methods. 247 patients with AF (113 with lone and 134 with secondary) were examined. The control group was represented by 182 healthy people. All of them were performed a specific range of functional and laboratory methods of research, including molecular genetics. Logistic regression analysis was used to predict the development of lone AF. Results. According to the odds ratio, the presence of the TT genotype increases the risk of AF by 1,8 times (in the presence of the T allele, the risk of lone AF increases by 2,0 times), a logistic regression model is constructed. Model determination coefficient R2=0,062. The specificity of the model in terms of predicting the absence of an lone form of AF is 95,1%. The model predicts the development of an lone form of AF with a sensitivity of 17,7%. The total percentage of correct predictions is 65,4%. Conclusion. According to the logistic regression model, the probability of lone AF in the presence of the patient’s TT genotype is 6,2%; the absence of lone AF – 95,1%. The total percentage of correct predictions is 65,4%.

Atrial fibrillation (AF) is one of the most common tachyarrhythmias, contributing to both environmental and genetic factors, a clear understanding of which can be extremely important for determining management tactics and predicting the disease course. The article provides a brief overview of studies on genetic predictors of AF, in particular, SNP markers found on chromosome 10. Establishing a relationship between the identified SNPs on chromosome 10 and functional genes, changes in the structure or regulation of which can affect the development of AF, opens the veil of understanding how these SNPs affect the pathogenesis of AF.

Prothrombin time (PTT) is a laboratory indicator that allows to evaluate the external pathway of blood coagulation (activity of factors I, II, V, VII and X). An INR indicator was introduced to standardize the results of the PTV test. INR = (patient’s PTT/PTT norm) ISI. The control of the INR is gaining popularity with the help of portable coagulometers. On the russian market are CoaguChek XS, qLabs Electrometer. The main problem is their high cost and high cost of operation. Russian scientists have long wondered about import substitution. However, in the literature there is no information on testing development. When developing an experimental model for measuring INR and PTV, we were guided by the results of the literary search in the scientometric databases Elibrary, Scopus, PubMed and WoS, taking into account the estimated cost of the developed model. Background. To conduct the initial stage of testing an experimental model with a comparison of the results obtained with the data of a certified laboratory. Materials and methods. We examined 70 patients (26 men and 44 women) taking «Warfarin». The material for measuring the experimental model was capillary blood. We used the qLabs® PT-INR Test Strip as a test system. Results. We obtained the Difference of the PTT of the tested model and the results of the certified laboratory ± 1–2 seconds. PTT prothrombin calibrator was 13 sec. Prothrombin ratio prothrombin calibrator was 1.0. Patient №1. PTT on the tested device 34 seconds. ISI troboplastin on test strip was 1.0. INR = (34 sec / 13 sec × 1.0)1.0 = 2.62. The result of a certified laboratory: PTT 36 sec, INA 2.86. Patient №2. PTT on the tested device 31 sec. INR = (31 sec / 13 sec × 1.0)1.0= 2.38. The results of the certified laboratory: PTT 32 sec, INR 2.48. Conclusions. According to the presented results obtained during testing of the developed portable model in patients on warfarin therapy, comparable results of PTT and INA were obtained with a certified laboratory.

Background. The search for genetic predictors of stroke development is actively studied in all developed countries due to the need to address the primary prevention of socially significant diseases. Gene CYP17A1 is one of the many genes that can participate in the formation of predisposition to the development of stroke.Aim. To examine association of the single nucleotide polymorphisms rs1004467 CYP17A1 gene with the development of stroke.Material and methods. The study included 184 patients with stroke (active group; 113 men and 71 women; mean age 55.06±9.40 years), 131 of them with ischemic stroke and 41 with hemorrhagic stroke. The comparison group included 251 people with hypertension without stroke (160 men and 91 women; mean age 59.21±6.62 years) and control group – 157 persons without cardiovascular diseases (106 men and 51 women; mean age 54.94±6.64 years). Study participants underwent clinical (collection of complaints, anamnesis, physical examination), laboratory (blood chemistry, lipid profile) and instrumental (computer tomography of the brain, ultrasound duplex scanning of extraand intracranial blood vessels, electrocardiography, echocardioscopy) examination to verify the diagnosis, molecular genetic study of gene polymorphism СУР17А.Results. The study established the association of homozygous genotype AA rs1004467 CYP17A1 gene with the risk of stroke, which is confirmed bya statistically significant increase in the number of carriers of homozygous genotype AA in the common allele A rs1004467 CYP17A1 gene in patients of the main group (73.4±3.3%) compared to the control group (61.8±3.9%; p=0.03). Statistically significant increase in the number of carriers of genotype AA rs1004467 of CYP17A1 gene in patients with hemorrhagic stroke (70.7±7.1%) compared with the control (61.8±3.9%; p=0.01) was revealed. There are no statistically significant associations between the rs1004467 genotypes of CYP17A1 gene and the risk factors of stroke (hypercholesterinemia, burdened hereditary, hypertension, increased intima-media thickness of brachiocephalic arteries).Conclusions. The ability to consider the genetic determinants of brain stroke allows to identify proactively the risk factors of this complex cascade process. Studies of gene polymorphism as a factor of genetic predisposition to various human diseases allow to determine the individual possibilities of optimal therapy for each patient, considering the identified risk factors.

Background. The study of the incidence of risk factors (RF) of stroke for primary and secondary prevention of this disease remains relevant. There are modifiable and non-modifiable RF.Aim. To study the frequency of the main RF in patients with ischemic and hemorrhagic stroke in the urban population of Krasnoyarsk (Eastern Siberia).Material and methods. Examination of 124 patients with an ischemic stroke and 28 with hemorrhagic was performed. Patients received treatment in neurologic center of Krasnoyarsk in 2014 year. The main criterion of inclusion was the confirmed diagnosis of an ischemic or hemorrhagic stroke. All patient had the clinic-tool and neurovisualizing examination and collecting the individual and family anamnesis with examination of modifiable and non-modifiable RF. The research of the neurologic status at patients both with ischemic, and with a hemorrhagic stroke was conducted by the standard technique. Based on results of computer or magnetic resonance imaging of a brain the diagnosis of an ischemic or hemorrhagic stroke was established.Results. A distinctive feature of patients with hemorrhagic stroke was the occurrence of a vascular accident in younger age (6 years earlier) in comparison with ischemic stroke. Among patients with ischemic stroke elevated body mass index (BMI) was in 87 (70.2%) patients, burdened heredity was in 8.1% of cases, and 43.5% of patients were smokers. Among patients with hemorrhagic stroke 16 (57.1%) patients had elevated BMI. People with burdened heredity were not identified among them. Smoking was found in 22 (78.6%) patients in this group.Conclusion. Improvement of treatment and prevention of an ischemic stroke are in many respects connected with clarification of RF and pathogenic mechanisms of its development. The rational pharmacotherapy directed to pointed impact on individual RF can reduce the risk of development of stroke not only at individual, but also at the population level.

Background. To study the dynamics in the quality of life of patients with acute coronary syndrome (ACS) with emergency revascularization by percutaneous transluminal coronary angioplasty and stenting the coronary arteries (CA). Materials and methods. 150 patients with ACS, percutaneous transluminal coronary angioplasty and stenting the coronary arteries (CA) were included. 65 patients (group 1) performed all procedures of second phase of cardiac rehabilitation program. 85 patients (group 2), who refused of participation in rehabilitation program, were tested according to the test-questionnaire Beck and SF-36. Giant cell myocarditis (GCM) is a rare autoimmune disease with the leading role of T-cell disorder in its pathogenesis. The associations are known of this disease with various autoimmunities, tumors, drug hypersensitivity. In the literary data there are just single cases of HCM development in HIV infected patients. The article presents clinical case and provides some discussion on the pathogenesis of HCM, making to think of HIV infection as one of possible causes of its development.

Atrial fibrillation (AF) is one of the most prevalent tachiarrhythmias, with at date non fully understood etiology. Recently, the attention is paid to genetic determinants of AF.Aim. To assess the role of rs2200733 polymorhpism on the chromosome 4q25 in development of AF in Russian population.Material and methods. Totally, 76 patients studied with AF, and control group, number 73 persons without cardiovascular pathology. All participants underwent laboratory and genetic investigations.Results. A statistically significant prevalence of genotype ТТ is found (21,21% vs 4,11%, р=0,015) and allele Т (34,85% vs 19,86%, р=0,03) in the group of patients with the lone AF comparing to the controls. Therefore, TT genotype and T allele of rs2200733 chromosome 4q25 could be predictors of unknown origin AF development.

Aim. To study the association of the hereditary sick sinus syndrome (SSS) with gene α2B-adrenergic receptor (ADRA2B) polymorphism.Material and methods. 29 families with hereditary primary SSS from the database of the Chair of Therapy № 1 of Krasnoyarsk State Medical University named after prof. V.F. Voyno-Yasenetsky were included in the study. Group 1 included probands (20 women and 9 men, 58±0.15 y.o.), group 2 – proband relatives of I, II and III degree (65 males and 68 females, 39±0.13 y.o.), group 3 (control) — 89 healthy volunteers. Clinical examination (physical examination, ECG, bicycle ergometry, ECG monitoring, atropine test, electrophysiological study, echocardiography) was performed in all probands and their relatives. The diagnosis of SSS was confirmed by transesophageal left atrium stimulation in 75 individuals. Genotypic examination of gene ADRA2B I/D polymorphism was performed in 213 individuals: 75 SSS-patients, 49 their healthy relatives, 89 healthy volunteers.Results. 3 types of ADRA2B genotypes (II — homozygous wild, ID — heterozygous, DD — homozygous mutant) were founded by allele-specific polymerase chain reaction. Significant prevalence of the homozygous genotype of more rare alleles DD in SSS-patients (28±5.2%) compared to the control group (8.99±3.0%) was found.Conclusion. Study of the genetic marker can be used to identify predisposition to hereditary SSS in the population and individual-family level. SSS due to mutations in genes that regulate cell function of sinus node and the sinoatrial conduct occurs, apparently, extremely rarely.

Aim. To evaluate short- and long-term prognosis of myocardial infarction (MI) depending on occurrence of pathologic Q-wave on ECG and time of its appearance.Material and methods. 616 patients with initial ST-elevated MI were included into the study — 254 women and 362 men, aged 62.1±11.2. All patients were split up three groups depending on occurrence of pathologic Q-wave on ECG and time of its appearance. First group consisted of 311 patients with early Q-wave (during first 6 hours from onset of MI symptoms). Second group consisted of 120 patients with late Q-wave (from 6 to 24 hours from onset of MI symptoms). Third group consisted of 185 patients with non-Q-wave MI.Results. The hospital lethality in the patients with early Q-wave was significantly higher than this in patients of the second and the third groups. However, we didn’t reveal significant differences between patients with late Q-wave and patients without Q-wave on ECG. The incidences of cardiac death (sudden death and fatal MI), non-fatal MI and hospitalization because of unstable angina was assessed during next two years. During the follow-up period patients of three groups had similar incidences of the non-fatal MI and hospitalization because of unstable angina. However, incidence of cardiac death was significantly higher in early Q-wave group in comparison with non-Q-wave group.Conclusion. Occurrence of pathologic Q-wave on ECG and time of its appearance should be taken into account to assessed short- and long-term prognosis of MI.

Aim. To prove the advantage of the long-term controlled antihypertensive therapy compared with the real usual standard therapy in patients with mild to moderate arterial hypertension.Material and methods. It was multicenter, randomized, prospective parallel-group study in patients with mild to moderate hypertension. One part of patients (treatment group) received the strongly regulated stepped antihypertensive therapy based on ACE-inhibitor spirapril, the second one (control group) continued their usual standard therapy prescribed by doctors of polyclinics or other patient care institutions. The study lasted one year.Results. 1742 patients were enrolled in the study, 854 patients were included in the treatment group and 888 – in the control group. 220 patients dropped out of the study by different reasons, and 1552 patients finished the study. There were 651 (37,6%) men and 1081 (62,4%) women. It was substantial decrease in blood pressure in both group, but distinctions between systolic and diastolic blood pressure in the treatment and control group during the study were significant.The target levels of blood pressure (systolic < 140 mm Hg and diastolic < 90 mm Hg) were registered substantially more frequent in the patients of treatment group than in control group (69,4% и 39,3% after three months and 83,6% и 66,9% after 12 months of the treatment correspondingly).Conclusion. Controlled stepped antihypertensive therapy resulted into more significant decrease in systolic and diastolic blood pressure and more frequent achievement of target levels of blood pressure in comparison with control group. It supposes better prognosis of patients.

Aim. To evaluate the incidence of cardiac rupture (CR) in patients with acute myocardial infarction (MI) as well as predisposing factors during 15-year follow-up.Material and methods. 1453 patients with acute MI who were admitted to Krasnoyarsk cardiological center during 1989, 1993 and 2003 years were studied. All patients were divided into three groups: group I - 447 patients, who were on treatment in 1989, group II - 475 patients of 1993, and group III – 531 patients of 2003. There were no differences in groups on patient sex, location and depth of MI, previous MI, arrhythmias, heart failure and diabetes mellitus.Results. CR was found in 18 of 447 patients (4%) of group I (1989), in 16 of 475 patients (3,4%) of group II (1993) and in 10 of 531 patients (1,9%) of group 3 (2003), (p1,3 Conclusion. Effective BP and HR control is the most effective way to prevent CR in patients with MI.

Aim. To assess genetic issues in the ischemic stroke development in AF and to invent an analytical programmed complex for genetic risk estimation of stroke development in a patient with AF.Material and methods. Totally 43 patients studied with AF and stroke in anamnesis, 78 — with AF and no stroke. Controls consisted of 188 persons without cardiovascular pathology. The participants underwent ECG, EchoCG, Holter ECG-monitoring, exercise test, TELAS, thyroid gland hormones analysis. For confirmation of the ischemic nature of stroke in participating probands we performed computed tomography of the brain. All participants also underwent molecular genetic testing.Results. According to the odds ratio, allele A polymorphism -455G>A of gene FGB increases the risk of ischemic stroke development in atrial fibrillation 1,7 times comparing to those patients without the allele; genotypes with the rare T allele in homoand heterozygous state of polymorphism 807С>Т of gene GPI? increases 2,5 times the risk of stroke in AF; presence of allele C polymorphism -5Т>С of gene GPI?? increases 1,9 times the risk of stroke comparing to its absence; presence of the rare genotypes allele C in homoand heterozygous state -5Т>С of gene GPI?? increases 2,3 times stroke risk; allele A of the polymorphism 10976G>A gene FVII decreases the risk of stroke 2,6 times. According to the results, there was a “Clinical-genetic riskometer of the ischemic stroke in AF” developed. Using this informational-analytic complex it is possible to estimate genetic risk of the stroke.Conclusion. Therefore, the study has shown that homozygous genotype AA of the rare polymorphism 455G>A gene FGB, heterozygous genotype СТ and homozygous genotype ТТ of the rare polymorphism allele 807С>Т gene GPI?, heterozygous genotype TC and homozygous genotype CC by the rare allele polymorphism -5Т>С gene GPI?? are defined as genetic predictors of ischemic stroke development in atrial fibrillation. Allele А of polymorphism 10976G>A gene FVII is protective against ischemic stroke in patients with AF. Knowing the parameters of genetic assessment in AF, it is possible to calculate genetic risk of ischemic stroke development in AF via invented by us a “Clinical-Genetic riskometer of ischemic stroke in AF”.

Aim. To study the relation of mononucleotide polymorphism G?C (rs619203) of ROS1 gene with the risk of ischemic and hemorrhagic stroke development.Material and methods. Totally 152 patients studied (92 males, 60 females) with acute brain circulation disorder (stroke), of those 124 with ischemic stroke, and 28 with hemorrhagic, and 475 healthy people (320 males, 155 females) of controls. All patients underwent standard neurological, clinical and instrumental investigation in SCC FMBA (Krasnoyarskcity) and molecular-genetic investigation of DNA in SRI of Therapy and Prevention Medicine of SD RAMS (Novosibirskcity). Statistics included standard algorithm of statistical procedures.Results. The results of the study showed statistical predominance of prevalent genotype GG of ROS1 gene in ischemic stroke patients, comparing to control group.Conclusion. Homozygous genotype of GG gene of ROS1 is a risk factor for ischemic stroke.

Studying of interrelation of polymorphic allelic rs619203 option of a gene of ROS1 with cardiovascular diseases at persons with the ischemic stroke is carried out. For the purpose of studying of a role of single nucleotide polymorphism of rs619203 of a gene of ROS1 in development of strokes of a genotiping 124 patients with ischemic stroke and cardiovascular pathology and 475 people of group of control. Persons with ischemic stroke are divided into subgroups according to a sex, the affected artery, the diseases which are available cardiovascular, risk factors, such as the raised body weight index, the burdened heredity on a stroke, smoking. For verification of the diagnosis by all patient standard neurologic examination, a computer tomography of a brain, and also the retrospective analysis of results of the previous inspections (are conducted at their existence), for definition of a genotype on ROS1 gene ONP - molecular and genetic research DNA. The received results showed prevalence of a homozygous genotype of GG on extended allel among patients from ischemic stroke having the arterial hypertension in comparison with control group. Thus, the genotype of GG has conditionally protective effect concerning development of ischemic stroke in persons with arterial hypertension.

The article deals with the issue of genetic determination of atrial fibrillation. In particular, it is shown that a rare polymorphism rs2200733 T allele on chromosome 4q25 statistically more frequently (p=0,029) in patients with atrial fibrillation compared with the control group data. The question of the functional significance of this polymorphism in the development of electrical instability of the atrial myocardium is also addressed.

Aim. To reveal genetic predictors of the ischemic stroke in patients with AF. Material and methods. Totally 121 patient studied with AF and 155 their relatives of I-III grade of relation. First group consisted of 43 probands with AF and stroke anamesis and 54 their relatives, the second group – 78 probands with AF without stroke and 101 their relative. Control group consists of 188 subjects. The patients underwent ECG, EchoCG, Holter ECG-monitoring, VEM, TELAS, molecular-genetic investigation.Results. Genotype AA of polymorphism -455 G>A of gene FGB statistically significantly predominated in subjects with AF and stroke comparing to those with AF without stroke (20,94% vs 6,4%; p=0,036). Risk relation shows that the chance of stroke development with AA homozygous genotype at rare allele is 3,9 times higher than in its absence. Overall the genotypes CT and TT by the rare allele 807C>T gene ITGA2 statistically significantly predominated in the patients with AF and stroke comparing to controls (76,7% vs 56,9%, p=0,026). The chance of stroke development while carrying CT and TT by the rare allele is 2,5 times higher than in their absence. Genotypes TC and CC by the rare polymorphism 5 T>C gene GPIBA statistically more significantly were found in patients with AF and credibly increasesthe chance for stroke 2,3 times. Allele A of polymorphism 10796 G>A gene F7 statistically more significant rarer found in patients with AF and stroke comparing to controls (6,98% vs 16,22%; p=0,043). Chance of stroke in absence of allele A is 2,6 times higher, than in its presence.Conclusion. The study revealed that homozygous genotype AA by the rare polymorphism -455G>A gene FBG, heterozygous CT and homozygous TT by the rare allele 807C>T gene ITGA2, heterozygous TC anf homozygous CC by the rare – 5 T>C gene GPIAB are set as genetic predictors for ischemic stroke development in AF. Allele A of the polymorphism 10976 G>A gene F7 show protective effect in the ischemic stroke development in AF. Genetic risk estimation might improve the primary ischemic stroke prevention in AF.

Aim. The purpose of this study was to access clinical application of the 9p21.3 locusgenotype in patients with myocardial infarction (MI).Material and methods. Totally 500 (411 male, 89 female) patients with MI youngerthan 65 years old [Ме 54,0; Q25 48,0; Q 59,0] and 31 autopsy data (24 male,7 female) in case of fatal MI were collected. All participants were included in the 75study after written informed consent form. Control group consisted of 535 participants (423 male, 112 female) without coronary artery disease [Ме 42,6;Q25 39,0; Q 44,3]. Genome DNA was extracted from venous blood or myocardialtissue using the phenol-chloroform extraction method. Two SNPs rs10757278 andrs1333049 (locus 9p21.3) were tested. Prospective follow-up period lasted 2 years.Results. We revealed a direct strong association of the locus 9р21.3 (rs1333049)with MI. Logistic regression odds ratio (OR) of MI for homozygous genotype CCrs1333049 was 1,71 [95% CI: 1,16-2,52], р=0,006. In patient who didn’t underwent PCI and carried of risk allele C rs1333049 had significant higher risk of recurrent 75acute coronary syndrome (ACS): during 1 year after MI – OR=4,91 [95% CI: 1,4516,66];during2yearafterMI–OR=3,77[95%CI:1,50-9,52].Conclusion.We suppose these data might be effectively utilized for improvement of onset of MI and long-term outcome prognosis.

Aim. Forecasting of emergence of cardiac conduction defects (CCD) by means of a method of multiple logistic regression for implementation of early diagnostics, primary prevention and the personified approach to treatment.Material and methods. To all probands with CCD and to their relatives clinicinstrumental research on the following program was conducted: clinical examination, electrocardiography, echocardiography, Holter monitoring, stresstest, genealogical and molecular and genetic researches. For the differential diagnosis between primary and secondary CCD to probands and their relatives according to indications coronarography and an isotope scanning was carried out. According to design of research, it was selected 81 probands with CCD (26 families with RBBB (right bundle branch block) and 55 families with LBBB (left bundle branch block). Patients were subdivided into 2 subgroups: 1 subgroup – 61 patients from idiopathic RBBB (26 пробандов and 35 sick relatives with RBBB); The 2nd subgroup – 121 patients with idiopathic LBBB (55 пробандов and 66 sick relatives with LBBB);Results. We presented an innovative technique of an assessment of risk of development of CCD, such as RBBB and LBBB. This mathematical model of forecasting of CCD in families of Krasnoyarsk. For forecasting of level of determination of RBBB the logistic regression model expressed by the following equation was received. Measure of definiteness of RBBB – 28,0%. For forecasting of a measure of determination of LBBB the logistic regression model expressed by the following equation was received. Determination level for LBBB – 29,0%. Conclusion. The logistic regression model of forecasting of CCD is an additional mathematical method of the forecast of level of CCD in families. It is on the practical level important that relatives of patients with CCD are truly threatened concerning development of these pathologies of carrying-out system of heart since this pathology has genetic determinacy and is accurately traced in families with monogenic type of inheritance. It is necessary to emphasize that as emergence of a disease is caused by a complex of hereditary and environmental factors, existence of genetic predisposition at an individual isn’t fatal inevitability of his incidence. Therefore forecasting of these diseases in families by means of computer technologies will allow to carry out an early stage of primary prevention for incidence prevention.

Aim. To study relationship of idiopathic atrioventricular and intravenricular disorders of cardiac conductivity with mononucleotide polymorhism (MNP) A/G gene TBX5. Material and methods. Totally 260 persons with primary cardiac conductivity disorders included (with AV-blocks of 1, 2, 3 grades, complete His right bundle branch block, complete His left bundle branch block, His rleft anterior bundle branch block) and 257 persons without any cardiovscular diseases. Patients were grouped according to their nosological type, age, gender. All patients underwent standard cardiological examination and molecular-genetic sampling of DNA. Results. The obtained results showed statistically significant predominance of the rare genotype GG (MNP marker rs3825214) gene TBX5 in the group of patients with conduction disorder at left His bundle branch and in subgroup of women with the pathology mentioned.Conclusion. The data obtained makes clear that the presence of GG genotype (rs3825214) gene TBX5 increases chances for idiopathic rhythm disorders development mostly in women. These results can be used during the primary prevention of the pathology mentioned.

Clinical case of subacute myocarditis is presented, complexity of its diagnosis and management are considered.

Objective: to study whether genotyping for single nucleotide polymorphisms (SNPs) rs10757278 and rs1333049 on the 9p21.3 locus may be applied to myocardial infarction (MI) risk stratification using the SCORE scale in young people (less than 45 years).Subjects and methods. A group of patients with MI (n = 103) and a control group (n = 111) showed no statistically significant differences ingender, age, hypertension, diabetes mellitus, hypercholesterolemia, overweight and obesity, abdominal obesity, and smoking history. The phenolchloroform method was employed to extract genomic DNA from venous blood. Genetic tests were carried out using real-time polymerasechain reaction systems (TagMan and AB 7900HT).Results. There was a statistically significant association of rs1333049 and rs 10757278 with the development of MI. The odds ratio for the latter was 2.53 (95 % confidence interval (CI) 1.31-4.89) in carriers of the risk allele C of rs1333049 and 2.11 (95 % CI 1.11–4.01) in those with the risk allele G of rs10757278. The statistical significance also remained with consideration for the family history of MI. Multiple logistic regression analysis established that the presence of the C allele of rs1333049 in the genotype was of great predictive value as compared with the high/very high risk of fatal and nonfatal events according to the SCORE scale.Conclusion. SNPs 1333049 and rs10757278 on the 9p21.3 locus are the predictors of MI in young people, which are independent of both traditional risk factors and family history. Having regard to the pattern of an association, it will suffice to genotype one of them, namely rs 1333049, in our population

Abstract. Prevalence of TNFα gene promoter polymorphism at -857 position was investigated in normal persons and among patients with rheumatoid arthritis. We detected a relation of this polymorphism to the levels of TNFα production by mononuclear cells of healthy donors. Changes in relative frequencies of the TNFα genotypes have been shown in rheumatoid arthritis. The -857С>T polymorphism of TNFα gene proved to be informative as a molecular marker reflecting efficiency of anti-TNFα therapy in rheumatoid arthritis.

Aim. To study the association of hereditary sick sinus syndrome (SSS) with connexin 40 gene (Cx40) polymorphism. Material and methods. 29 families with hereditary SSS were involved into prospective study. Probands were 20 women and 9 men (aged 58±0.15). Relatives, I-III degree of kinship, were 65 men and 68 women (aged 39±0.13). Clinical and instrumental examination was performed in all probands and their relatives. Diagnosis of SSS was verified by transesophageal atrial electrostimulation. Molecular genetic investigation of SSS patients and their relatives was carried out in laboratory of medical genetics of Research Institute of Therapy , Siberian Branch of Russian Academy of Medical Sciences. Results. 71 SSS patients, 44 their healthy relatives, I-III degree of kinship, 197 subjects of control group were genotyped for the polymorphism of 44G>A of gene Cx40. According to allele-specific polymerase chain reaction 3 types of genotypes ADRA2B (II — homozygous wild, ID — heterozygous, DD — homozygous mutant) were found in SSS patients, their relatives and healthy subjects of control group. Conclusion. Significant predominance of the heterozygous genotype 44G>A was found in SSS (45,07±5,9%) patients in comparison with subjects of the control group (29,44±3,2%).

The purpose of this study was to investigate association between 9p21.3 locus single nucleotide polymorphisms (SNP) and severity of coronary atheromatous burden. A total of 255 Caucasians (211 male, 44 female) with myocardial infarction younger 65 years old (mean 52,56±7,98 years) were enrolled. Genome DNA was extracted from venous blood using the phenol-chloroform extraction method. Two SNPs rs10757278 and rs1333049 (locus 9p21.3) were tested using real-time polymerase chain reaction (PCR) according to protocol (probes TaqMan, Applied Biosystems, 7900HT). The coronary angiograms were reviewed by independent angiographers who were blinded to the results of the genotype (Philips Allura Xper FD10). The total number of lesions, Gensini scores and SYNTAX score were derived. For the first time in males of Siberian population, genotype CC rs1333049 and genotype GG rs10757278 demonstrated a direct strong association with severity of coronary atheromatous burden

Variability of the main morphometric features of imago flies Calliphora vicina R.-D. (Diptera, Calliphoridae) of two samples was studied. First sample consists of individuals caught in the wild (park ecosystems of Dnipropetrovsk), the second one – specimens cultured in the laboratory under the constant temperature and humidity. Possible using of C. vicina R.-D. as a bioindicator of anthropogenic factors is analysed. Environmental factors may act as the stimulators of adaptive changes in physiological functions, as the constraints that cause impossibility of the existence of certain species in particular conditions, and as modifiers that determine the morpho- anatomical and physiological changes in organisms. The most significant differences between studied samples were found for the width (“laboratory” individuals are characterized by larger head size) and for the length of limbs segments. The fluctuating range of the head width in specimens collected in the wild is much wider, due to the heterogeneity of the micro-climatic conditions of the larvae development and trophic resources. Maximal negative asymmetry of the head width is observed in individuals C. vicina R.-D. of the “natural” sample as compared with “laboratory” individuals. Among imagoes caught in the wild the individuals with a relatively wide head are dominated, as evidenced by the large positive value of kurtosis. At the same time, the distribution of values in “laboratory” individuals is almost normal. In adults bred in the laboratory the shortening of segments of the leg pair I is observed in comparison with the individuals of “natural” sample. Similar results were recorded for other insect groups cultivated in a laboratory. For most other linear measurements of the C. vicina R.-D. body the differences between samples are not registered. Ephemeral existence of the substrate of blow flies leads to higher prevailing evolutionary adaptation of species to varying micro-climatic conditions inside cadaveric material. A minor variation of morphometric characteristics indicates the resistance to changes in abiotic environmental factors. Thus, use of Calliphora vicina R.-D. (in the temperature range of +17…+28 °C) as an indicator species of changes in the environmental temperature factor is not appropriate.

Aim. To investigate the association between single-nucleotide polymorphisms (SNPs) rs10757278 and rs1333049 of the 9p21.3 locus and family history of coronary heart disease (CHD) in myocardial infarction (MI) patients. Material and methods. The MI group (n=243) and the control group (n=280) were comparable by such parameters as age, gender, arterial hypertension, diabetes mellitus, hypercholesterolemia, overweight and obesity, abdominal obesity, and smoking history. However, the groups were significantly different in terms of family history of CHD (p=0,004). Genome DNA was extracted from venous blood using the phenol-chloroform extraction method. The SNPs rs10757278 (9p21.3) and rs1333049 (9Sр21.3) were tested using real-time polymerase chain reaction (PCR) and the AB 7900HT device, according to the producer’s protocol (TaqMan probes, Applied Biosystems, USA). Results. A statistically significant association between MI and the CC rs1333049 genotype or the GG rs10757278 genotype was observed in the whole study sample and separately in men and women. The MI odds ratio (OR) for carriers of the CC rs1333049 genotype was 2,02 (95% confidence interval, CI, 1,32–3,08) in the whole sample, 1,91 (1,11–2,95) in men, and 2,91 (1,22–6,91) in women. For carriers of the GG rs10757278 genotype, respective OR values were 1,98 (1,30– 3,02), 1,77 (1,08–2,87), and 2,94 (1,26–6,87). Among participants without CHD in family history, OR for the CC rs1333049 genotype (1,92, 95% CI 1,16–3,16) and the GG rs10757278 genotype (1,82, 95% CI 1,10–3,00) were comparable for those among MI patients with CHD in family history (respective OR 2,19 (1,28–3,75) and 2,23 (1,31–3,81)). Conclusion. For the first time in Russia, the association between MI-related polymorphisms of the 9p21.3 chromosome and family history of CHD was analysed. Two SNPs (rs1333049 and rs10757278) of the 9p21.3 locus predicted MI risk independently from either conventional risk factors or CHD in family history.

In this paper we studied the frequency of allelic variants of genes TNFRI at positions -609 and -1207 and TNFRII at positions -1709 and -3609 in patients with rheumatoid arthritis and in population control group. It was shown that the resulting frequency distribution of alleles and genotypes of TNFRI gene promoter in a group of population control is characteristic for Asian populations, for TNFRII gene polymorphisms and the frequency distribution differed from the European and Asian populations. Comparative analysis of allele and genotype frequencies of TNFRI gene promoter at positions -609 and -1207 and TNFRII and -3609 and positions -1709 in patients with rheumatoid arthritis and in population control group showed no statistically significant differences. However in the analysis of combinations has been shown that in patients with rheumatoid arthritis compared with the control population was significantly less common combination of genotypes TNFRI-609GT + TNFRII-3609CC.

The role of hereditary factors in the development of coronary heart disease and myocardial infarction is known, but the underlying pathogenetic mechanisms remain understudied. Multiple studies of candidate genes (those coding protein synthesis, participating in atherosclerosis pathogenesis, or regulating hemostasis and inflammation) have not been particularly successful. The main problem of these studies has been a very low level of reproducibility of the results obtained. By contrast, whole-genome association studies have provided new perspectives in this research area. This review discusses the modern relevant evidence and new directions for future research.

Objective. To estimate the extent and pattern of myocardial damage in different types of rheumatoid arthritis (RA) during disease-modifying antirheumatic drug (DMARD) or biological therapy. Subjects and methods. Seventy-one patients with RA were examined; some of them received biological therapy with infliximab, while the others took DMARDs. A group of patients with incipient RA was also identified. B-type brain natriuretic peptide levels were estimated and electrocardiography, echocardiography (EchoCG), and cardiac magnetic resonance imaging (MRT) using the contrast medium Dotarem were conducted in all the patients. The follow-up totaled 6 months. A control examination was made at the moment of randomization and 6 months posttreatment. Results. Tn the bulk of patients, the level of B-type brain natriuretic peptide did not differ from the reference values, however, its lower level was observed in the incipient RA group, which was associated with the absence of cardiovascular diseases and with a younger age group. There were no negative EchoCG changes in myocardial viability values. Cardiac MRT demonstrated that the majority of patients had the similar changes that failed to affect myocardial kinetics and ejection fraction. These changes were not found in incipient RA patients without cardiovascular diseases. No improvement in myocardial viability was recorded in the patients receiving the biological therapy. Conclusion. Thus, cardiac MRT showed the similar changes that failed to affect myocardial kinetics and ejection fraction in patients with RA during both methotrexate and infliximab therapy.

The intake of Simvastatin in a dosage of 40 mg daily within two months in patients with ischemia heart disease has caused increase in the general percentage of ω6 fat acids (FA) at a constant level of ω3 FA, that was not accompanied by change of a ratio ω3/ω6 FA. Despite of progressing increase in total concentration of ω7 FA (at women) and increase of a level ω9 FA (at men) during simvastatin treatment at women and at men led to decrease in the total percentage of nonsaturated fat acids concerning corresponding parameters at healthy donors. The Simvastatin treatment was accompanied by increase in blood plasma in patients with ischemia heart disease (and first of all at women) percentage of the separate saturated fat acids, and also their total level. The index of nonsaturation of fat acids of blood plasma is decreased.

Aim. To compare alteplase (AL) and streptokinase (SK) effectiveness in patients with various severity of acute myocardial infarction (AMI). Material and methods. In retrospective analysis, 650 AMI patients were divided into three groups. Group I consisted of 183 subjects administered AL, Group II Р 232 individuals receiving SK, and Group III - 235 participants not administered any thrombolytic therapy (TLT). Each group was divided into three subgroups, according to Peel coronary prognostic index (CPI). Subgroup A included patients with Peel CPI of 1-8 points, Subgroup B Р 9-16 points, and subgroup C Р more than 16 points. Clinical outcomes of patients receiving AL, SK, or no TLT, were compared for all subgroups. Results. Preliminary results of calculation of the effect of amplification of sublimation due to concentration of the solar radiation by a conical hole in the surface dust layer on a cometary nucleus are given. The temperature of ice on the bottom of the crater is calculated for different geometrical parameters of the crater. The dependence of the effect of intensification of sublimation on the geometrical parameters is considered for a specific case when a single active region is exactly on the pole of the nucleus and this pole is directed to the Sun.

The efficiency and safety of therapy with abatacept in combination with methotrexate were studied in patients with rheumatoid arthritis. The performed therapy was shown to reduce the activity of an immune inflammatory process and the magnitude of articular syndrome and to improve the patients' functional status.

Aim. Analysis of associations between idiopathic disturbances of cardiac conduction (DCC) and polymorphism of mitochondrial genome. Material and methods. A family examination was performed in 431 probands with various DCC and 1347 relatives of the first, second and third degree of kinship (the study group). All the examinees were divided into four subgroups. These included 158 probands with atrioventricular block (A VB) of various degree and their 518 relatives (subgroup 1); 50 probands with a complete right bundle-branch block (BBB) and their 161 relatives (subgroup 2); 108 probands with a complete left BBB and left anterior branch of the His bundle and their 152 relatives (subgroup 3); 115 probands with sick sinus syndrome (SSS) and their 327 relatives (subgroup 4). The control group consisted of 104 probands without clinical ECG manifestations of cardiac diseases and their 321 relatives. All the examinees have undergone ECG, atropin test, echocardioscopy, electrophysiological examination of the heart and mitochondrial DNA (mDNA). Results. Comparison of the incidence of mDNA D-loop restriction sites in the group of patients with idiopathic DCC and controls has found higher frequency of the Hae III 16517 site in the group of the patients (p = 0.0480). By location of the blocks (atrioventricular and intraventricular), the site occurred more frequently in patients with AVB (86.36%). The variant "+" by the site of Hae III 16517 mDNA was found to associate with disturbances of cardiac conduction, more closely in AVB. Conclusion. Variability of mDNA may be an etiological factor of idiopathic DCC pathogenesis.
