This research presents the analysis of a comment as a reactive speech genre. The authors focus on its specific variation – Internet comment on various online materials (news, posts, blogs), widely presented in virtual communication. The purpose of the article is to substantiate the position that the Internet comment is a hybrid speech genre which is characterized, on the one hand, by the features of a traditional comment as a secondary genre and, on the other hand, by special features resulting from the virtual communication environment. The research solves several tasks: 1) to identify typological features of a comment as a reactive genre; 2) to establish the peculiarities of the Internet comment genre. The empirical material of the research consists of the contexts selected by a continuous sampling method from various Internet resources (news portals, blogs, entertainment news sites). The research is based on comparative method and the method of discourse analysis. As a result of a complex, intergrated analysis of various types of comments, a number of features inherent with the genre of the Internet comment have been revealed: polysubjectivity, the realization of centripetal and centrifugal tendencies which determine the development of dialogue between users, reflecting the subjective perception of the source text by the authors of Internet comments and their subjective cognitive and speech reactions, the realization of a special communicative purpose, language design and content, expressed through the predominance of modus over dictum, the peculiarities of the text design related to its technical aspect. In the context of paradigmatic and syntagmatic relations, the genre of the Internet comment is characterized by radial and wave principles of organization. Thus, the authors conclude that the Internet comment is a hybrid genre of virtual communication combining the characteristics of the traditional comment and other Internet genres.

With age, the incidence of cardiovascular diseases is steadily increasing worldwide. Living in the harsh climatic and geographical conditions of the Arctic is associated with premature aging, earlier and more frequent development of age-associated diseases. Apparently, these processes are caused by functional changes in large vessels, the state of which has not yet been studied. Aim and methods: to identify correlations of aging and biological age indicators with the parameters of central hemodynamics and arterial stiffness measured using applanation tonometry by the «Sphygmocor» device in middle-aged men living in the European North of Russia. For the first time, positive associations of parameters of peripheral and central blood pressure, augmentation index, and negative correlation of the time of return of the reflected wave and pulse pressure amplification with indicators of aging were noted, which indicates an age-related decrease in arterial elasticity. There was also a negative association of the subendocardial viability ratio with the northern experience and biological age. Theoretically considered, the results demonstrate the potential involvement of blood vessels in the pathogenesis of premature aging. The practical significance of the work is related to the need to develop preventive measures aimed at controlling blood pressure, reducing arterial stiffness and cardiovascular risk in middle-aged northerners.

The content of vascular endothelial growth factor-A (VEGF-A) in blood plasma and its relationship with lipid and extracellular matrix metabolism in working-aged men (19-69 years), living and working in the European part of the Arctic zone of the Russian Federation (Russian Arctic), were studied. No age dependence of the plasma VEGF-A content was found. The correlation analysis, performed in different age groups, revealed significant associations of VEGF-A level with lipid parameters (CS, LDL-C, Apo B, atherogenicity coefficient, Apo B /Apo A1 ratio) and extracellular matrix metabolism (blood TIMP-4, MMP-2, MMP-3, MMP-9, hyaluronan, total and peptide-bound hydroxyproline, glycosaminoglycans). The established correlations indicate the formation of relationships between angiogenesis, atherogenesis and fibrosis at a specific period of life of northerners in the Russian Arctic.

Research objective - to carry out studying of changes of metalproteinases of blood at patients with epitelial cancer of skin in the postoperative period. Changes of TIMP-1 and MMP-2, MMP-7 and MMP-9 of blood it has been studied at 98 patients with various cancer of skin whose age was 55±3 years. From them bazalnokletochny cancer of skin - at 39 people, planocellular cancer is skin - at 39 patients. Researches were conducted before expeditious treatment, in the first postoperative days, for the fifth, seventh, tenth postoperative days and in 18 months after the undergone operation. For comparison at 20 patients operated with skin fibroma. Studying of indicators was carried out for the same days. At all patients the diagnosis is confirmed morphologically before performing expeditious treatment. Expeditious treatment was carried out under local anesthesia, at cancer broad excision of education was carried out, at fibroma - education excision. The conducted researches of indicators of inhibitor of metalproteinases-1, matriksny metalproteinases of blood at patients with various skin tumors show that in pathogenesis of development of a tumor of epitelialny tumors the great value is given to increase in inhibitor of metalproteinases-1, matriksny metalproteinases of blood. So, it is established that prior to expeditious treatment in all groups, both malignant, and good-quality increase in all studied indicators has been registered. It should be noted that increase in these indicators depended on malignancy of a tumor. So, in group with good-quality formations of skin this increase was minimum, and in group with planocellular cancer of skin - maximum. It is possible to assume that each tumor produces in TIMP-1, MMP-2, MMP-7 and MMP-9 blood, and this quantity depends on the nature of malignancy of a tumor. Besides, in the course of the research it has been established that performance of surgical treatment at benign tumors is led to correction of this indicator that it is shown by normalization of these indicators. At the same time at bazalnokletochny cancer of skin only to partial decrease in an indicator, and at planocellular cancer of skin these indicators didn't change. From this it is possible to make the conclusion that surgical treatment doesn't lead to correction of these indicators at malignant new growths of skin.

Here we present the study of enzymes involved in the regulation of extracellular matrix metabolism. It was the first study of this kind in the European part of the Arctic zone of the Russian Federation (AZRF). The contents of matrix metalloproteinases (MMP-1, -2, -3, -9) and tissue inhibitors of matrix metalloproteinases (TIMP-1, -2, -4) were measured in blood plasma of 91 men in AZRF (67o N) and 14 men in Western Siberia. The contents of MMP-1, MMP-9, TIMP-1 and TIMP-4 in plasma of northerners were higher compared to those of the residents of Western Siberia (55o N). Age-related dynamics of MMP and TIMP contents had a mixed trend in northerners. The maximum contents of MMP-1 and MMP-9 and of MMP-3 were observed in 30-39 and 40-49 years old groups, respectively. The contents of these enzymes tended to decease with age. The maximum contents of TIMP-1 and TIMP-2 and of TIMP-4 were in 50-59 and 60-69 years old groups, respectively, while the minimum contents of these enzymes were in the group of young men up to 29 years old. Hyaluronidase activity was minimal in 30-39 years old group and increased with age up to maximum values at 50-59 years old. Age-related imbalance of MMP/TIMP system (MSE content reduction with age vs TIMP content increase in older age groups) can be considered one of the reasons of the identified age-related increase of interstitial fibrosis and premature aging of the northerners.

The analysis of 6-sulfatoxymelatonin (6-SMT) levels in working-age men (aged 19-69) who live in the European part of the Arctic zone of the Russian Federation is presented. Their urine 6-SMT levels were similar to those of comparison group (residents of Western Siberia). The level of 6-SMT decreased with age and was the lowest in 40-49-year group. Age group 60-69 tended to demonstrate increasing 6-SMT levels. Thus, they were insignificantly different (p>0,05) compared to those of people under 29. We showed that alcohol influenced 6-SMT level. The level was lower in alcohol-consuming north men compared to non-consumers. Free testosterone level in plasma of northerners decreased with age. In 60-69-year group, the level was 50% of that in under-29-year group. Compared to controls, we observed the activation of lipid peroxidation that was not compensated by increased activity of glutathione reductase and plasma SH groups. Age-related decrease in the melatonin metabolite, as an endogenous antioxidant, was accompanied by a gradual accumulation of lipid peroxidation products and lower antioxidative protection mediated by SH groups.

The results of the correlation analysis between the metabolite melatonin - 6-sulfatoxymelatonin (6-SMT) in the urine and the indicators of aging, interstitial fibrosis, the system of antioxidant protection in men in the Arctic are presented. Negative correlation of medium strength between 6-SMT with calendar age, biological age and molecular marker of aging p16INK4a in leukocytes was noted. A relationship of 6-SMT to fibrosis indices was found. It manifested itself in a negative correlation with sulfated glycosaminoglycans in the urine, with collagen markers: free hydroxyproline, peptide-linked hydroxyproline, and total hydroxyproline. Age-related decrease in the level of 6-SMT may be associated with an age-related increase in interstitial fibrosis in northerners. A positive correlation of medium strength between 6-SMT and hormones: free testosterone level (r=0,53, p=0,0002), total testosterone (r=0,43, p=0,003) and cortisol (r=0,33, p=0,007) was shown. Negative correlations of 6-SMT with the content of ceruloplasmin (r=-0,63, p=0,001) and SCORE scale (r=-0,52, p=0,001) indicate the relationship between melatonin and the antioxidant defense system and the risk of cardiovascular pathology.The results of the correlation analysis between the metabolite melatonin - 6-sulfatoxymelatonin (6-SMT) in the urine and the indicators of aging, interstitial fibrosis, the system of antioxidant protection in men in the Arctic are presented. The main extracellular matrix components in Arctic residents were studied. Northerners had increased levels of total glycosaminoglycans, hyaluronan, and collagen IV in plasma and both general and peptide-bound hydroxyproline in urine, which indicates an accelerated metabolism of the main extracellular matrix components compared with comparison group (residents of Siberia). Age-related remodeling of extracellular matrix in northerners manifested in changing ratio and quantity of its main components. Levels of total glycosaminoglycans, hyaluronan, fibronectin, hydroxyproline and its fractions increased with age while the level of collagen IV changed insignificantly. Average positive correlation between extracellular matrix components and biological aging indicators is suggestive of relationship between these two processes: aging - which is accelerated in the Arctic and pathological remodeling of extracellular matrix as it is associated with accelerated aging. Changes in local regulation system including those related to matrix metalloproteinases and their tissue inhibitors may be one of the reasons for pathological remodeling of extracellular matrix.

Experience of the treatment of gunshot wounds and pancreas traumas during domestic armed conflict in the North Caucasus (1994-1996, 1999-2002). The paper presents our own experience of surgical treatment of gunshot wounds, and pancreatic damage. Pathophysiological mechanisms of development of fire pancreatitis are presented. Tasks for surgery and their solutions are defined. The reasons for reoperation are analysed. It was found that the surgical approach depends on the nature and localization of pancreatic damage. The features of the surgical intervention are shown.

Assessment of the quality of life for male inhabitants of the Russian European North demonstrated decreased indices of physical health component (the scale of role functioning determined by physical condition) and mental health component (the scales of social functioning and role functioning determined by emotional state and mental health) compared to the inhabitants of Siberia as a comparison group. Men aged up to 29 had the highest values at all scales. The values decreased with age. The most prominent decrease was observed for the scales of general well-being and pain intensity. There was a moderate decrease for the scales of physical functioning, role functioning determined by emotional state, and role functioning determined by physical condition. Quality of life was closely related to self-reported health of northerners, pathological index, molecular marker of aging, and biological age. Negative correlation between these values implies the deterioration of physical and mental health with biological age. The quality of life for northerners also correlated with hormonal status. There was a relationship between testosterone and three scales of physical health components; between cortisol and one scale of physical component; and between both dehydroepiandrosterone sulfate and insulin and the scales of physical and mental components of health. The effect of various hormones on different age groups of northerners was demonstrated.

The fibronectin, being one of main components of extracellular matrix of myocardium, is involved into post-infarction repair process. However, the role of fibronectin in this process in patients with myocardium infarction in various periods of their life continues to be a controversial issue. To decide this point the study was organized concerning analysis of content of fibronectin in blood serum in 75 males of three different age groups (prior to 49 years, 50-59 years, 60 -69 years). It is demonstrated that in two groups (prior to 49 years and 50-59 years) the maximal content of fibronectin was marked at second phase regarding to data at first and third phase of reparation process. Whereas in patients aged 60-69 years the maximal content of fibronectin was detected at first phase of reparation process regarding to such in patients aged 50-59 years. The lower content of fibronectin was detected in third phase of reparation process regarding to analogous indicator in other two age groups of patients. These dynamics of content of fibronectin in patients of elder age apparently is an evidence of lower reparation potential of extracellular matrix.

The paper gives the results of a study of the levels of glycosaminoglycans (GAG) and hydroxyproline (HOP) in 87 apparently healthy individuals of both sexes, aged 19 to 60 years. There is a decrease in the levels of GAG, total HOP, and peptide-bound HOP with advanced age. These indices are higher in subjects less than 45 years old than in those over the age. The found age differences are comparable with higher serum hyaluronidase and collagenolytic activities in subjects younger than 45 years than in those over the age 45 years. Collagen metabolism occurs more intensively in young subjects (under 45 years of age) than in old ones (over 45). There are also gender-related differences: the men had higher levels of GAG and peptide-bound HOP and serum hyaluronidase activity than the women, which suggests that the metabolic rates of collagen and proteoglycans are higher. This difference is complimented by the qualitative characteristics of the process: there is a preponderance of the synthesis of collagen over its degradation with the accelerated degradation of proteoglycans in the young men. In Rh-positive subjects, the content of GAG was 22-fold higher than that in Rh-negative ones.

Acute colonic perfusion impairments (thrombosis, infarction, and ischemia) have evident clinical manifestations and are reasonably well described in the literature. At the same time, the problems of chronic peripheral circulatory disorder in the large bowel and the impact of the disorder on its motility are inadequately considered. By summarizing the data available in the literature and their findings, the authors consider the etiology and pathogenesis of organic and functional colostasis in the context of changes in the colonic angioarchitectonics.

Preparation of autopsy material for a histological study for correct diagnosis is not less important than that of a biopsy specimen. Based on their accumulated experience, the authors propose a number of modernizations to prepare autopsy material for the study of lung, bone marrow, and intestinal abnormalities, which require no higher work intensity, but at the same time contribute to a unified approach to investigating tissues and organs in certain pathological processes in pediatric care.

Dynamics of biochemical markers of reparative fibrosis in patients with myocardial infarction (MI) with complicated course did not differ from that in the group of patients without complications. Development of complications was associated with elevation of blood serum content of glucosaminoglycans in phase 3 of reparative fibrosis compared with group of patients without complications. This elevation was significant in the group of patients with rhythm disturbances. Statistically significant increase of total urinal oxyproline was found in patients with cardiac aneurism in phase 1 and in patients with cardiogenic shock in phase 3 of reparative fibrosis. These subject allow to speak about participation of reparative fibrosis in development of complications. Complications of myocardial infarction developed in patients aged 58 years and older.

The content of the major components of the extracellular matrix was studied in 60 patients with spinal osteochondrosis over time during complex rehabilitative therapy including radon-containing mineral baths (before, 7-9 and 14-15 days after treatment). The complex rehabilitative treatment using radon baths in patients with spinal osteochondrosis was shown to result in a reduction in the serum level of glycosaminoglycans (GAG) and total oxyproline by the end of a course of therapy, which suggests the lower rate of destructive processes in the extracellular matrix. The determination of GAG concentrations and oxyproline forms in patients with asteochondrosis may be used to evaluate the efficiency of the treatment performed.

The content of ceruloplasmin was studied in W/SSM rats with hereditary galactosemia. Carbohydrate component constitutes about 40% of the molecule of this antioxidant. The content of ceruloplasmin in 2- and 11-month-old W/SSM rats was elevated compared to the corresponding parameters in Wistar rats.

In animals with SiO(2)-induced inflammation, exposure in radon-containing water promoted normalization of fibronectin content on days 3 and 10 and reduced the content of glycosaminoglycans on days 3 and 20 compared to the corresponding values in intact animals and rats with SiO(2)-induced inflammation not exposed to radon.

The aim of the study was to research the efficiency of sertraline (zoloft) in depressions, anxious states and obsessive-compulsive disorders. Diagnosis of the mental disorders was carried out according to ICD-10. 72 children (59 boys, 13 girls) aged 6-18 years were treated. There were 32 inpatients and 40 outpatients. Therapy with sertraline was performed during 8 weeks with a gradual increase (titration) and individual selection of the dose from 12.5 to 100 mg/day. During the therapy clinical observation was combined with the patients' examination using Hamilton Depression Scale and Hamilton Anxiety Scale (HAM-D and HAM-A), and a Clinical Global Impression Scale (CGI). It was established that sertraline was very effective and safe drug in children (it has no influence on cognitive functions, has neither myorelaxing or sedative effects). Activity of this drug is characterized by quick manifestation of thymoanaleptic and anxiolytic effects. It mild depressive states 50 mg/day is a significant dose; in more severe depressions and obsessive-compulsive disorders the dose in juveniles was to 100 mg, the duration of the therapy was more than 2 months.

Scanning electron microscopy revealed changes in erythrocyte morphology induced by the synthetic antioxidant phenozan-1. Phenozan-1 at 10(-7)-10(-5) M acts as an echinocytogen. This effect of the antioxidant is, probably, due to its distribution in the external monolayer of the erythrocyte membrane and associated structural changes.

Data on anatomo-clinical analysis of 36 cases of death due to general overheating of a body are presented. On the basis of these data can be stated that in most cases the effect of high ambient temperature combined with great physical exercise and prolonged exposure to the caused death within short (up to 3 days) periods of time as a result of cessation of CNS functioning. Without physical exercise and direct insolation death usually occurred later (on the average of 5-9 days) from acute renal and renal-hepatic failure. Morphologic changes were unspecific.

The results have been presented of the zymosan (Z) effect mechanism in the reaction of the lipid peroxidation and the parts' separation of the protection antioxidant. The experiments were made on 32 Wistar rats. At the experimental series 25 mg/kg and 100 mg/kg of Z were injected. Control rats were injected with physiological saline alone.
