3. [Hygienic estimation of functional reserves and adaptive capabilities of students].

- Abstract
In the article there are presented data on characteristics ofpeculiarities of the functional state of medical 1-6 years students of higher educational institutions. The results were obtained with the aid of variation pulsometry. Students were shown to have typical elevated tone of the sympathetic nervous system, especially for students of the 1, 3, 5 and 6 courses, that is confirmed by the amplitude mode (AMo), characterizing the sympathetic activity of autonomous nervous system (ANS), which is an average of the students 1 year accounted for 38.6 ± 1.89%, for students of the 3 course - 38.5 ± 1.72%, for students of the 5 year (40.9 ± 3.25 %) and the students of 6 course (46.7 ± 2.59%). There was determined the trend to the centralization of the heart rate control, as evidenced by a reduced proportion of high-frequency waves (HF) by 29.2% to 35.2%, exceeding by 3.6 to 14.4 times in waves of the very low frequency (VLF) relative to the average standard values; the high proportion of students from 41% to 52%, with a mismatch of the sympathetic and parasympathetic compartments of the autonomic nervous system in the regulation of biological processes of adaptation. For medical students of higher education institutions there are typical functional reserves reduced from 20.5 % to 97.6% and a decrease in the proportion of students with a satisfactory adaptation by 40.4% from the 1 to the 6th year.

- Abstract
In the paper there are presented results of investigations of indices of the physical development of male adolescents aged of 11-17 years - residents of the 1-2 generation from new coming European migrants in the Magadan region during 2008-2014. There were observed 418 adolescents - representatives of the 1 generation, and 463 cases from the 2 generation. Representatives of the 2 generation in the initial period (11-12 years) were established to show higher tempos of growth. Maximal gains in the body length, body mass, and chest circumference in adolescents of the 1 generation occur at the age of 13-14 years, but in the 2 generation it is seen one year later - at the age of 14-15 years. In adolescents of each generation there was noted the high variability of the level of major indices of the physical development. However, among representatives of the 1 generation there is less proportion of cases with normal or average values of body length, body mass, and chest circumference along with larger proportion ofpersons with diverse deviations, as well deficit as excess in comparison with the 2 generation. At the final stage of the puberty age (17 years) more adolescents with microsomatic (25%) and disharmonic (38.2%) body constitution occur among the subjects from the 1 generation as compared to those of the 2 generation (19.7% and 28.2%, respectively).

- Abstract
The present publication is devoted to the search for the objective criterion for the differentiation of criminal responsibilities for committing non-violent sexual offence against the juveniles under the age of 16 years. It has been shown that the current version of articles 134 and 135 of the criminal code of the Russian Federation does not adequately reflect the true character and the social danger of such actions. The formal methods of the legal, logical, and concrete sociological analysis were employed to substantiate the necessity of differentiation of criminal liability for the non-violent sexual abuse depending on the degree of potential harm to the health of the under-age individuals.

In recent years, in sanitary science the interest in issues of young people has dramatically increased, that has been actualized by the necessity of a new objective assessment of the role of the youth in modern sanitary-ecological, sociocultural and economic processes. In this regard, a special interest is aroused by young students, the health of whom is determined largely by the condition of the habitat. So far complex assessment ofriskfactors in urbanized areas, prioritization of impacts, the establishment ofpriority impacts, the detection of informative territorial environmentally dependent indices ofpopulation health are presented to be important tasks in the solution of the regional challenges. The aim of this work was the assessment of the state of health of students of the Bratsk State University, residing in conditions of ecological trouble. In the territory of the city of Bratsk the environmental situation is characterized by long term multi-component air pollution with highly toxic vehicular and industrial emissions. In the work throughout the 4 years following-up there was executed an assessment of the physical development of students, there was determined structure and revealed the an increased level of morbidity rate of students of Bratsk state University in terms of educational space, which is characteristic for territories of ecological trouble in the city of Bratsk. Parameters of physical development of students in dynamics of training were established to be not liable to variation. It confirms the stability of the morphological signs as compared to functional ones. In the analysis of dynamics of indices of the harmonicity ofphysical development there was revealed a gain in the proportion of harmoniously developed students by the end of training. Physiometric indices of students to the fourth course of training were characterized by the increase offunctional capabilities of the respiratory system.The indices of the cardiovascular system performance had a tendency to the growth of the hypertensive response in boys and the increase in the number of people with normotensive response to physical stress by the end of training. To the IV course of training there was observed the increase in the number of young boys with stringency of mechanisms of adaptation to stress, whereas in young girls satisfactory adaptation was seen.

- Abstract
There were performed biochemical and cytological studies of blood in healthy workers of chrysotile asbestos plant in order to determine the functional state of the body, depending on the length of service and working conditions. Mentioned differences between the functional state of the organism were determined in the longitudinal study, under construction on schedule time series with the test group at specified intervals (every year for 7 years surveyed, the same group of healthy workers). The differences between the indices allow one to discriminate different levels of the functional state of the organism for the workers employed at processing complex with experience of 4 and 5 years, and for the employees in the mining and transport workshop with the experience of 5 and 6 years. As a result the work experience of 5 years is considered to be an occupational hazard for workers at processing complex, and 6 years of experience - as an occupational hazard for workers of mining and transport workshop.

The purpose of the study was to examine the problem of falls in the elderly. A prospective cohort study of 537 patients over 65 years using validated questionnaires and functional tests. 56,58 % of patients had at least one fall event in the 12 months prior to the observation. 82,76 % of all falls are over the age of 85 years. Significantly more patients younger than 85 years fell in the autumn and winter (to 17,64 and 25,93 %, respectively) and over the age of 85 years in 50 % of cases (95 % CI 28,2-71,8) fell summer. Polypharmacy and comorbid conditions increases the risk of falls. 32,89 % of people over 65 years in the fall received fractures. However, a fracture of the femoral neck in 60 % of cases diagnosed over the age of 85 years. Hypodynamy observed in 69,77 % of cases.

- Abstract
The article considers the findings of hygienic assessments of educational conditions in urban and rural schools in Mongolia and socio-economic conditions of living of schoolchildren and analysis of anthropometric measurements of schoolchildren aged of 7-16 years in relation to the assessment of harmonicity of their physical development. Hygienic conditions in Mongolian schools are characterized by improper organization of the educational - accomplishment process, high occupancy rate in classes causing insufficient space for a pupil and disconformity of school furniture parameters of school desks and chairs to growth and age requirements of pupils. Comparative assessment of socio-economic factors revealed (выделено автором) that the majority (84.1%) of rural pupils reside in uncomfortable houses and gerdwellings, 58.5 % - in large families with many children, 46.3 % - in families with poor living conditions. There were obtained data about differences in physical development of urban and rural schoolchildren pronounced with age by the gain in the number of children with disharmonious development and children with lower physical indices of the development, delayed by 1-2 years appearance of the crossing of growth and growth shift typical for rural children in comparison with their urban counterparts. According to results of multivariate regression analysis most significant factors for the formation of physical development of school children in descending order are academic workload, the place of residence, the number of children in the family, uncomfortable living conditions, household income and parent’s education. In the presence of these factors, the risk of the formation of disharmonious physical growth of school children increases by 1.8-2.8 times. The probability of the risk offormation of disharmonious physical development of schoolchildren living in rural areas was found to be by 2.5 times more than for their urban counterparts. The results can be usedfor scientific substantiation and development of targeted health measures.

- Abstract
The study of somatotypes of the constitution is an important point in planning of the improvements of measures among the population in various regions of Russia. The purpose of the work was to reveal features of age dynamics of somatotypes of the constitution in students of youthful age of the Baikal Region by means of somatotyping according to scheme by Nikityuk B. A. and Kozlova A.I (1990) with taking into account their functional group of health. There were examined 1286 Slavic young males, natives of the Irkutsk region, aged of 17-20 years, from them, according to data of the medical examination 996 were referred to the 1st (main) and 290--to the 2nd (preparatory) functional group of health for physical exercises. There were established significant differences in somatotypes of the constitution in young men of the 1st and 2nd functional groups of health. In both functional groups there is noted a significant amount of young males with transitional somatotypes that testifies to incompleteness of growth processes of their organism. The obtained results of a somatotyping are used in the educational process for a training individualization on physical culture of students of IRGTU, and also in construction of independent physical--improving programs.

- Abstract
There was revealed the structure of deteriorations in the nutritional status of schoolchildren in the city: the most of students has normal nutritional status, but there was noted the high prevalence of excessive body weight and obesity among children and teenagers. Risk factors for development of deteriorations of the nutrition state were detected as follows: irrational food regimen, qualitative compartment offood, factors of educational environment, lifestyle. The main role in system of control of the nutritional status in children is referred to the correction of socio-hygienic factors which prove to be the priority ones in the shaping of the nutritional status in students. As the main condition determining the nutrition state of the up-to-date schoolchildren and the quality of their life in the whole the social cultural level of children and adolescents must be regarded as a result of the hygienic education and training in fundamentals of healthy lifestyle. Priority protective factors of the gain in the part of schoolchildren with normal nutritional status (optimalfood regimen, optimal dietary habits, sufficient level of physical activity) laidfrom the child age in conditions of the family, sufficient level of the physical activity and the implementation of the other element of hygienically expedient day regimen served as the base for the elaboration of the system of the control of nutritional status. Algorithm of the control of the nutritional status in the students of educational institutions includes the creation of healthcare educational environment, optimization of nutrition and physical activity, the shaping of the culture of healthy lifestyle, health-improving measures for children with disorders of nutritional status and their psychological pedagogical supports at the stage of the correction of the nutritional status, improvement of the medical service for the early detection of deviations of nutritional status with the estimation of the efficiency of the system ofpreventive and health-improving measures.

- Abstract
To determine age-relatedpeculiarities of trace element system in oldpeople, residing in different natural and geographical regions, the content of 25 trace elements in hair samples of women from Arkhangelsk (average age is 87,0±0,98 years) and Magadan (average age is 80,1±1,25 years) was determined by spectrometric methods. In both groups there was established element misbalance characterized by lower concentration of such essential elements as Ca, Mg, Co, Cu and higher concentration of Na. The excess of K and Zn is more expressed in hair of female residents of Arkhangelsk in comparison with the city of Magadan there was revealed significantly more higher content of aluminum, boron, silicon, in the city of Magadan there are significantly higher concentrations of selenium and tin. According to the frequency of occurrence the excess of elements in the body of residents of the city of Arkhangelsk is comparable to that of residents of the city of Magadan, but has its own features. In Arkhangelsk there is more pronounced the excess ofpotassium, there was found high content of zinc, whereas in the city of Magadan there was revealed an excess of manganese and arsenic. Square of the figure, reflecting the deficiency of elements in female residents of the city of Magadan is more than in the city of Arkhangelsk, which is manifested by larger deficiency of chromium, zinc, iron. Identified regional differences in the content of macro- and microelements are related to biogeochemical and ecological features of the regions, which can determine the specifics of the aging diseases and time of their occurrence.

The purpose of the research is to study the gender aspects of the employment of adolescents, working in the time free from their study, and its impact on the livestyle and health, to assess the risks to health of students. There was studied the employment of senior pupils and students of vocational schools (1.2 thousand persons at the age from 14 till 18 years) in 5 regions of the Russian Federation. The results indicate to a prevalence of illegal employment of the minors. Among young men 62% work part time, among young women - 37%. Violations of hygienic requirements for working conditions and labor law violations are more common in young men employment. The first experience of work can render negative impact on the lifestyle and the state of health. In the working adolescents there is observed the higher prevalence of the behavioral risk factors (smoking, alcohol, drugs, sleep deprivation and others). Adolescents with work experience have lower health status. The analysis of odds ratio (OR), relative risk (RR), etiological fraction (EF) showed that to the higher health risk there are exposed teenagers working constantly throughout the year. In this group among girls there is a high prevalence of chronic diseases (RR = 2; OR = 3.9), of complaints of indigestion (RR = 2; OR = 2.5). In the group of young men, working continuously, the percentage of frequently ill with acute respiratory infections is higher (RR = 3.3; OR = 3.7), there is a higher number of complaints of heartburn (RR = 2.1; OR = 2.6), back pain (RR = 1.8; OR = 2.2). Working young men have higher health risk by comparison with working young women.

The transition to a new stage of the development - the information society is an objective reality and has an influence on all areas of the activity of the society, including the establishment of a child as an object of the hygienic research. In conditions of the general informatization of the society, the appearance of so-called “clip thinking,” explains the maladjustment of educational technologies to mechanisms of children ’ and teenagers ’perception and is confirmed by the growth of the school pathology and the gain in the morbidity rate. In the investigation on the example of the educational institutions of Irkutsk it was executed the evaluation of the impact of the intensification of informatization of education and personal development. For the investigation there were formed 2 groups ofpreschools with different levels of informatization in the same preschool institution of the central district of the city of Irkutsk but in different periods of time. In total there were observed 211 children aged of 5.5 to 6.5 years. For the study the influence of the intensification (and informatization of training there were formed 2 groups of small schoolchildren with different levels of intensification (and informatization) of education. The total number of cases accountedfor 465 children aged of 7-9 years. There were suggested methodical approaches to the estimation of the health status of the children, with taking into account the inevitable influence offactors of informatization and the intensification of education. The performed investigations have allowed to reveal the following tendencies in the shaping of the psychophysical state of health and development of children: an increase of level of informatization of education and personal and accomplishment; intensification of learning working; reduction of the attention level; imagination and visual divergence; capability to the linear differentiation and construction of inferences; fear to fail to meet the expectations of surrounding people and low resistance to stress; the increase speed of data processing along with fall in quality; the gain in hyperactivity.

Negative trends in incidence rates of children and adolescents of the city of Penza are due to the influence of both environmental factors as well as the educational process. Hygienic trouble in the city of Penza determines air pollution emissions of road transport, as evidenced by the high levels of morbidity rate in children and adolescents. The priorities for the correction factors are conditions and the organization of nutrition, physical education. There are identified the most important lifestyle factors for senior pupils that need the solution.

Analysis of the prevalence of the main classes of ecologically dependent diseases according to the appealability (diseases of the respiratory system, diseases of digestive organs, diseases of blood and blood-forming organs, diseases of the circulatory system, diseases of the skin and subcutaneous tissue, diseases of the genitourinary system, neoplasms, congenital anomalies, infectious diseases) was performed with the use of official statistical report forms No. 12 (ICD-10) over the period of2000-2014. Informational Database of parameters of the environment was represented by the factor modules: 6 - sanitary, and 5-environmental ones). By means of the method of the regression analysis there was established the relationship between environmental factors and the prevalence of ecological-dependent diseases, there were calculated values of the factor loads influencing on the indices of the morbidity rate in children and adolescents. The analysis of morbidity according to classes showed that during the study period, in the total there was a gain in the indicator ecologically dependent morbidity rate in children by 33.3%, in adolescents - by 35.3%. In the children and adolescent population there are prevalent diseases of the respiratory system - 67.3% and 52.45%, respectively; further in children and adolescent groups diseases of the digestive system - 9.45% and 17.08% take place; then in the children cohort are infectious diseases, and in adolescents - diseases of the genitourinary system. The level of ecologically dependent morbidity rate is determined by the complex impact of environmental and hygienic factors, with a predominance of sanitary-hygienic component and related with it parameters of the environment of habitation, such as chemical pollution of urban areas (soil, air, water sources), nevertheless biotropic factors (temperature, humidity, fog, number of days with biologically active solar radiation) make a significant contribution. The revealed various degree of the response of children and adolescents through the prevalence rate level to the exposure of ecological and hygienic factors of environment should be taken into account in the development and carrying out medical-preventive measures.

- Abstract
Among the risk factors that have priority in shaping the health of the population, a leading place is held by a rational nutrition. Presented results indicate to the absence of a stable group eating regularly, with the rational regularity and having all the necessary meals and the recommended food set. In the population of the adult people of the city of Irkutsk there is revealed the role of the nutritional factor in the formation of morbidity rate level of the adult population. It was found that the risk of diseases of the circulatory system accounted of 2.5 (95% CI: 2.1-3.6), for infectious and parasitic diseases - 2.4 (95% CI: 1.7-3.5), for the endocrine system - 2.2 (95% CI: 1.4-3.4), andfor the urinary system - 2.3 (95% CI: 1,7-3,0).

There are presented results of the cytome analysis of epithelium of buccal mucosa in 167 children aged of 5-7 years, permanently residing in the city of Magnitogorsk and visiting municipal kindergartens. Frequencies of the main indices of genotoxic and toxic effects were: cells with micronuclei - 0.29±0.04 %, with protrusions ofnuclei - 1.47±0.17 %, with double nuclei -1.96±0.13 %, with two nuclei - 3,28±0,16 %, with perinuclear vacuole - 9.66±0.86 %, with pycnosis of nucleus - 4.60±0.31 per 1000 cells, with karyorrhexis - 0.86±0.15per 1000 cells, with lysis of nucleus - 54.34±3.90 per 1000 cells, the condensed chromatin cells - 10.59±1.35 per 1000 cells. Most of these indices didn’t differ from the same detected earlier in buccal epithelial cells from children of the same age in Moscow, except cells with perinuclear vacuole, which frequency in Moscow cohort was twice higher. Frequencies of cells with these anomalies were associated with the content of 19 out of 326 revealed components of contamination of the total-winter snow samples, taken on the territories of the kindergartens, which were visited by examined children. There was shown lack of differences from basic values of main indices previously establishedfor the children of the same age in Moscow, with the exception of cells with perinuclear vacuole, the rate of which in Moscow was twice higher. There were revealed confounding factors associated with test indices: gender of the child, the annual number of acute respiratory diseases, health group, income per one person in family, alcohol intake by parents.

The state of health in children and adolescents during the time of education in secondary schools is getting worse: the prevalence rate of functional disorders and chronic diseases is increasing. There was executed the assessment of the overwroughtness of the training-education process in secondary school institutions. The prior characteristics of the educational activity are: intellectual, sensory, emotional loads; their monotony and mode of educational activity, school work intensity. The system of indices of the overwroughtness of the education labour was formed with account ofpsycho-physiological characteristics of children's organism. There was performed the research of the overwroughtness of educational activity in 820 schoolchildren. The overwroughtness of the educational process was evaluated on the basis of chronometry research, questionnaire survey of teachers and parents, and with taking into account the organization of educational process. In questionnaire survey there were involved 52 teachers and 389 parents. The assessment of overwroughtness factors of the educational activity was carried out in 5-10th classes for academic disciplines: Russian Language, Literature, Mathematics, History, Geography, Foreign Languages. It was found that the characteristics of "mental load" in schoolchildren are established to be the one of the main limiting factors that allow to refer training activities to "overwroughted of first degree" (3.1 class). Sensory loads on the studied academic subjects are rated as the second class of overwroughtness and are "permissible." The monotony of loads was revealed to correspond to 1st and 2nd class of overwroughtness and is therefore an optimal or acceptable. Training activities in the traditional educational institution corresponds to the 2nd class of overwroughtness - "permissible." The arduousness of the educational process in an innovative school is rated as the third class of the first degree ofthe overwroughtness (3.1). According to results of the study of the arduousness of the educational labor the study of mathematics in grades 5-9 on indices of the mental load was noted to correspond to the 1st degree of arduous activity (3.1 class). The acceptable level of the arduousness of the training and educational labour in nearly all classes was established in the Russian language, Literature, History and Geography, Foreign language. High school students are established to be exposed to effects of emotional load more than mid-level students. There are developed proposals on the optimization of the learning process, allowing to reduce the level of intellectual, sensory, emotional load.

In this work the results of treatment of 116 patients with polytrauma older age groups was analysed, the comparison group consisted of 55 patients with multiple injuries aged 20 to 40 years. The objective of the study was the search of criteria of the objectification assess the severity of condition of patients of elderly and senile age with polytrauma on the basis of laboratory parameters and evaluation of the influence of concomitant somatic pathology on the level of lethality. We analysed the significance of each of the selected indicators and their entirety, thereby simplifying the formation of evidence-informed way to predict fatal outcome in patients of elderly and senile age with polytrauma.

- Abstract
The article presents results of study of the impact of iodine deficiency and technogenic fluoride on the state of the thyroid gland in children. On the example of two districts of the city of Bratsk there were executed dynamic investigations (2002 and 2012), including the estimation of the pollution of ambient air and soil by fluorine compounds, levels of iodine intake by the body, the clinical examination of children aged from 5 to 7 years d and interviewing of their parents. In the course of the medical examination there were executed: physical examination by the pediatrician, endocrinologist, ultrasound examination of the thyroid gland, the determination both of serum hormone content by radioimmunoassay and urinary excretion offluorine and iodine. Concentrations of hydrogen fluoride and a solidfluorides in ambient air led to the accumulation offluoride ion in the soil. The iodine entering with drinking water and food, was established to provide only 37.5-50% of the daily requirement of iodine. Increased fluoride ion content in urine and milk teeth in children is associated with the concentrations of the fluorine-containing pollutants in the ambient air and soil. The fluoride pollution against the background of the natural iodine deficiency was established to increase the frequency of functional and morphological disorders of the thyroid gland in children.

They include: rural life satisfaction is largely dependent on the availability of family than on the health and financial status; in a special house for single elderly life satisfaction is determined only by the state of health due to the large number of chronic diseases, in stationary social services very important factor is loneliness, isolation that creates some feeling of being tired of life, depression, provocation the abuse of alcohol.

In the article there are presented results of studies on the hygienic assessment of working conditions and health status of 1200 workers of mining and processing enterprises (MPE) developing deposits of iron ores of the Kursk Magnetic Anomaly: Lebedinskiy MPE, Mikhaylovskiy MPE, Stoylenskiy MPE. There were revealed the differences in the character and intensity of adverse occupational factors, there was made the quantitation of the occupational risk level to the health workers of MPE both factories and quarries. The structure of occupational diseases in MPE factories is formed by dust lung diseases and the occupational pathology of the organ of hearing. In the structure of professional pathology in quarries workers vibration disease (61.5%) prevails. The obtained results indicate to the need of the development of the system of measures for the reduction in general and occupational morbidity rate of workers, the creation of safe working conditions, improving the early diagnosis of occupational and common diseases at the stage of preliminary and periodic medical examinations.

Examination covered 118 medical staffers with length of service 25,7±6,3 years. Materials included work conditons, case histories, physical and laboratory examinations, cardiointervalography. To identify emotional burnout syndrome, the authors used simultaneous testing with <
> (MBI) and questionnaire < >. According to emotional burnout syndrome components, somatic disease manifestations, cognitive, emotional, motivational and attitudinal disorders were assessed. Findings are that vegetative nervous system in medical personnel with long length of service is overstrained, functional resources are exhausted, energetic level is under normal level - that is supported by clinical and laboratory data. 36,4% of the medical staffers were diagnosed as having vegetative dysfunction syndrome. Complaints of psycho-physical, social and psychologic and/or behavoral symptoms were seen in 7% of the examinees; hypodynamia - in 53%; excuses for sedatives or psychoactive drugs intake - in 53%. Findings are that according to periodic medical examinations risk groups should be formed and specific medical and prophylactic measures should be provided. - Published

Assessment of the quality of life for male inhabitants of the Russian European North demonstrated decreased indices of physical health component (the scale of role functioning determined by physical condition) and mental health component (the scales of social functioning and role functioning determined by emotional state and mental health) compared to the inhabitants of Siberia as a comparison group. Men aged up to 29 had the highest values at all scales. The values decreased with age. The most prominent decrease was observed for the scales of general well-being and pain intensity. There was a moderate decrease for the scales of physical functioning, role functioning determined by emotional state, and role functioning determined by physical condition. Quality of life was closely related to self-reported health of northerners, pathological index, molecular marker of aging, and biological age. Negative correlation between these values implies the deterioration of physical and mental health with biological age. The quality of life for northerners also correlated with hormonal status. There was a relationship between testosterone and three scales of physical health components; between cortisol and one scale of physical component; and between both dehydroepiandrosterone sulfate and insulin and the scales of physical and mental components of health. The effect of various hormones on different age groups of northerners was demonstrated.

With the aim of the substantiation of hygienic measures for the improvement of the prevention of nutrition-related diseases among adolescents there was performed an analysis of indices of the health-related quality of life, lifestyle features, the character of nutrition and health of adolescents aged of 13-17 years in the territory of the Omsk region (n = 427). The study of the health-related quality of life was performed with the use of the validated Russian-language version ofthe general questionnaire PedsQL-4.0. According to the results of the study there were identified the priority factors influencing the formation of the quality of life of adolescents. The nutrition of adolescents has a significant influence on the formation of quality of life. Among the many parameters that characterize the nutrition, the greatest contribution to the quality of life is made by the factor of "the amount of consumed meat products". The decline in quality of life was noted in adolescents with chronic diseases or disorders of the nutritional status as overweight. There were marked forward-looking possibilities of prevention of nutrition-related diseases on the base of indices of the quality of life, including measures for correction of adolescent nutrition, in-time detection of disturbances of the nutritional status, early diagnosis and treatment of chronic diseases.

Senior and senile age is characterized by psychological, social and somatic heterogeneity of patients. To ensure high level of quality of life and to optimize the functioning of staff of geriatric hospitals we propose to introduce the distribution of pensioners within departments based on their psychosomatic status. On the analogy of the «medical triage», authors propose to introduce socio-psychological grading of older persons living in geriatric centers, focusing on their ability for self-care, the degree of preservation of cognitive functions, individual psychological characteristics. Testing of this grading was made on the basis of the Scientific and Methodological Gerontological Center. As a result of introduction of the principle of socio-psychological grading, positive dynamics in the psychological state of patients, better performance of the staff, and improved subjective assessment of the quality of life of older persons were registered.

During the last decade the quality of children's health declined, the level of children's aggression and aggressiveness increased. The consistent trend to increasing the quantity of children with the signs of motor disinhibition in different forms and manifestations was noted. In a study on the example of educational institutions of Irkutsk there was made an assessment of the impact of the intensification and informatization of education and up-bringing with the use of the index of the level of informatization. In preschool children over the information period mental performance was found to be characterized by an increase in the speed and decline in the quality of information processing, there are dominated increased aggressive background and unmotivated fears "out home", there are reduced values of endurance ratio of the cardiovascular system, higher levels of morbidity rate were noted. There was proposed a hygienic assessment of informatization of education and up-bringing with the use of the index of the level of informatization.

Using a complex of morphofunctional characteristics, parameters of the biological age have been analyzed, and groups with delayed, moderate and accelerated aging rates have been outlined. Regional peculiarities in distribution of incidence of different variants of aging rates have been revealed. In Moscow men and women, the delayed variant of development of age involution changes is the most widespread. In Tiraspol women, compared to the Moscow group, incidence of people with accelerated aging rates is 2 times higher, which can be stipulated by influence of a complex of biosocial factors.

The authors analysed health state and morbidity of women engaged into professional navy service in North Fleet, age features of health state in these servicewomen, and results of medical stationary examination and checkup results, regular medical examination results and the servicewomen morbidity over 12 years (about 15.5 thousand cases), and 697 reports on the servicewomen diseases according to military medical (navy) commission. The article covers causes of changes in navy fitness category for the servicewomen, analysis of comorbidity, suggestions of new approaches to comorbidity analysis.

The aim of the research was to investigate the general somatic status and to investigate the influence of dominating part of vegetative nervous system, following the different types of senile cataract formation in human. Thus, the domination а sympathetic nervous system and correlated systemic dystrophic changes of the tissues in patients with cortical cataract have been found for the first time. Moreover, patients with nuclear cataract had the domination of parasympathetic nervous system and another character of dystrophic changes. Therefore, the type of a forming age-related cataract can be offered as an available clinical marker of character of neurodystrophic process occurring in an organism.

This article describes an original technique, designed tooptimize the level of intensity of physical activity used in the course of routine physical edication in classes for pupils of the specialized medical groups. The basis of this technique is the principles evaluate the response of students of special medical group to progressively increasing exercise test with which sets the optimal level of activity for each of the students with the features of the state of his health. The practical application of this method allows you to install exactly the level of exercise used in the course of the lessons of physical education in special medical groups, which improves the health of students with variations in health status, and does not increase the likelihood of any complications of their diseases.

There was performed an assessment of anthropometric indices of physical development and functional parameters of the cardiovascular system, psychological and social status in 770 students of the higher educational institutions in the city ofSamara. There was revealed the presence of I-III degree obesity in 13.2% of young males and underweight in 19.1% of young females. Stress and disruption of the processes of the adaptation process were shown to be observed in 7.6% and 6.1% of students, respectively. There was found a tendency to hypertension in 12.6% of young males. Revealed changes are related to lifestyle of the modern student. The main factors for the improvement of the life quality is the duration of sleep and ultimate nutrition. Initially, the lower level of physical and functional capabilities is compensated by virtue of the correct organization of social and psychological factors and prevents possible deviations from the part of health.

The authors analyzed the health status of military personnel (males) of the Northern Fleet who did call-up military service (n = 420) and served on contract (n = 1370) in special diving positions in the Kola Arctic over the past few years. For the comparison of the rate of the decline of health level in different conditions of service the authors presented the averaged results of studies previously performed in the same period of time in the service personnel of coastal military units in the European North, troops from the surface ship crews and crew nuclear submarine. In the article there are considered the features of the primary disease and injury. Research and monitoring of causative-consecutive relationships between the features of military professional activity and the level and structure of primary morbidity rate allow to form the main directions of vocational rehabilitation for fleet specialists.

Comparative analysis of the morbidity rate of students was implemented on the basis of a special educational institution for students who are prone to crime and delinquency and for pupils of the lyceum-boarding school. The analysis showed that the state of health of the surveyed children was characterized by high indices of morbidity rates, the high occurrence of chronic diseases among students and the progressive decrease in the indices of the health. The highest morbidity rates were noted in classes of diseases of the nervous system, digestive system, circulatory system, diseases of the eye and adnexa, and musculoskeletal system. The higher level of incidence of different pathologies in students of the specialized secondary school gives evidence of the possibility of unfavorable prognosis for the future health of adolescents and can refer them to the risk group.

Priority health indices (the morbidity rate, the resistance status, the physical development, the orthopedic status, the level of bone mineralization, functional reserves of the body, health groups) in schoolchildren aged 7-18 years with a different state of the musculoskeletal system (MSS), learning in secondary schools were studied with the aim of the development of prevention programs. The high incidence of MSS diseases was determined to be combined with a high level of overall morbidity rate, on separate classes--with diseases of the nervous, endocrine, digestive systems, increasing with age ofthe student. Comprehensive assessment of health is much worse in schoolchildren with functional MSS diseases, as well as in girls. This demonstrates the need for careful monitoring of these children by experts in different fields, differentiated prescription, prevention and wellness programs throughout all the period of schooling.

Interactions between quality of life for group of elderly people and social-medical care contribution to it have been discussed. There have been showed satisfaction of quality of life depending on getting care in different care system's organizations. There have been defined high level needs of elderly and aged people in medical and social care (especially of 80-89 years group in urban and rural areas). There have been exposed that elderly people living in sparsely populated areas in theirs everyday activity traditionally leaned on support on spouse's and children's help because of resources of social system protection and medical-social service less available in arctic territories which decreased the quality of life.

- Abstract
The paper describes the results of a sociological study of delivery of healthcare in the Republic of Altai to search for ways to improve its quality and to provide access to the local population. Analysis was made with regard to an interdisciplinary, comprehensive approach to considering the range of problems associated with not only the health of local residents and identifying risk factors leading to diseases, but also by determining the possibilities that could promote the minimization of causes that have a considerable impact on the occurrence of diseases and also hamper healthcare delivery in the human settlements of Gornyi Altai, which are difficult of access. The investigation has used sociological and statistical methods. It has been ascertained that the available healthcare forces and means should be today employed at the regional level, by applying the principles of necessary sufficiency in conjunction with the local population's social motivation to be involved in the activity associated with the rendering of medical services. In addition, it is necessary to systemically use mobile medical units as a significant factor for the optimization of medical care to the population living in Russia's regions which are difficult of access.

The article presents submitted data of population dynamics of adolescent (10-17 years old) in Russian Federation over the period of 1995-2012 In the presence of reduction by 8.2 million of adolescent population the top-priority task of adolescent health and life maintenance was declared. The article deals with physical growth and development statistics of schoolchildren of 15-19 years old: as opposed to peers of 80-es increase of length, body weight and circumference of chest as well as reduction of muscle strength is observed. On the ground of the data analysis of authoritative statistical reporting the increase of morbidity rate by factor of 1.4 was detected over the last 10 years. It has been established that the true morbidity level is above the authoritative by the factor of 1.5. The number of adolescent at the age of 10-15 referred to 1st and 2nd health groups decreased almost on 20%, at the same time pathological processes become chronic. The reproductive and mental health of adolescent at the age of 15-17 years was analyzed over the last 10 years. The rate of mental health disorders in adolescent living in country is more than in the peers living in cities by the factor of 1.2-1.4. The rate of period disorders, salpingitis and oothecitis increased in girls under 10 years and ones at the age of 15-17. The rate and structure of child disability was analyzed. It was fixed that leading causes of adolescent disability are mental disorders, diseases of nervous system, congenital anomalies. The most commonly encountered seed of physical dysfunction is capacity to study. It is demonstrated that there is misreporting on both child disability in totally and adolescent disability in Russian Federation. After the analysis of particular provisions of legislation concerning medical and social issues of child disability the week points were detected. The morbidity rate of adolescent was studied: primary cause of death in adolescent is extrinsic factor (more than 70%), the leading one is suicide. Road traffic accidents and accidental poisoning comes then. The medical and social consequences of high morbidity, disability and mortality rates of adolescent were considered as labour, reproductive and military potential of society and state. Short-term and long-term plans focused on decrease of morbidity, disability and mortality rates of adolescent were formulated on the basis of submitted data. search for solution is supposed to be nationwide and local as well (public health service, education, social protection).

- Abstract
The aim of the study was to evaluate reproductive health of descendants of people who experienced effects of adverse environmental factors, such as radiation and chemical contamination (the descendants themselves were unaffected by these factors). Reproductive health of women was assessed by mathematical modeling. Factors of greatest importance for the health status of the descendants were distinguished among the 76 ones studied. It was shown that reproductive health of the subjects descending from the people exposed to radiation deteriorated more significantly than of those whose ancestors were affected by chemical factors.
