Spinal column pathology plays an important role in the etiology and pathogenesis of acute and chronic spinal blood supply disturbances. The aim of the study was to determine a role of degenerative-dystrophic changes, arising as a consequence of Scheuermann's disease in a mechanism of this pathology development. In patients aged 17-65 years, subjected to study of the causes of acute (8 cases) and chronic (7 cases) ischemic spinal disturbances development a significance of spinal column structures changes in juvenile kyphosis for evolution of spinal cord vascular pathology was established.

A study was made of the spinal cord of 28 neonates and of children aged up to 6 years with the anamnesis load, who died of different diseases. According to the morphological findings, massive hemorrhages were discovered in the thoracic part of the spinal cord in those who died at an age under 6 months. In the later periods one could find different stages of the organization of hemorrhages in the form of commissural process primarily around roots and vessels and dyscirculatory vascular disorders in the spinal cord. The lesions discovered were compared with the clinical manifestations in 40 patients of varying age, who suffered natal injury to the thoracic part of the spinal cord. They showed up by atrophic and spastic paraparesis of the lower limbs, syringomyelitic syndrome at the level of the thoracic part, scoliosis of the thoracic part of the spine. The latter one was induced by asymmetric atrophy of the muscles of the interscapular area.

The authors analyse the peculiarities of the clinical course of compression neuropathies of the limb peripheral nerves in the late stages and the most adequate methods of diagnosis. Differentiated microsurgical treatment was substantiated in 181 patients. The indications for various methods of microsurgical decompression were determined on the grounds of clinico-neurological and instrumental examination. The accumulated experience showed that external neurolysis, epineurotomy, partial and "deepened" internal neurolysis, as well as combined plastic operations with the use of microsurgical techniques were the most adequate methods of microsurgical treatment of patients with compression neuropathies of peripheral nerves of the limbs in the late stages.

Proceeding from the literature data on a positive effect of a combination of dimexide and corticosteroids on scar tissue, the authors treated 42 patients with chronic cerebral leptomeningitides by transcerebral dimexide + hydrocortisone "superelectrophoresis". 10 mg of hydrocortisone and 10 mg of 50% solution of dimexide were required for one session. The course consisted of ten sessions. Comparison with a control group composed of patients receiving the conventional treatment showed a statistically significant increase in the number of patients discharged with improvement and considerable improvement. The treatment was associated with no side effects.

Hormonal spondylopathy as a disease entity in considered on the basis of the findings provided by an examination of 207 patients with this disease. It has been established that the early signs of spondylopathy appear prior to menopause and are caused by various endogenous and exogenous factors. The pathogenesis, clinical picture and X-ray diagnosis are discussed. The results obtained suggest that the appropriate therapy should be etiopathogenetic at the early stage of the disease and replacement one at the late stages. Divided doses of sex hormones are recommended.

In order to elaborate an epidurographic technique, the water-soluble contrast substance (verografin), oil solution of iodine, iodolipol and air were used in experimental studies on animals and in cadavers. The studies demonstrated that oil and gaslike contrast substances penetrate into the vein and create a threat of embolia formation. For epidurography the most acceptable are water-soluble contrast substances. The method of an ascending verografin epidurography was elaborated and used in clinical practice. Some indications and contraindications for epidurography are outlined. A group of 396 patients with osteochondrosis of the lumbar part of the spine was examined. The method allowed a topic diagnosis of the disc hernia not only along the spine, but in the cross-section of the motor segments (median, paramedian and lateral hernias). The studies make it possible to exclude a disc prolapse in a nonhernial form of osteochondrosis. In 32 patients with spinal tumors epidurography permitted to determine the initial growth of the tumor, its size and relation to the dura mater.

Under observation there were 16 patients with chronic leptopachymeningitis of tuberculous etiology. The disease got complicated with ischemic disturbance of the spinal circulation which developed acutely in 5, and chronically in 14 patients. The clinical manifestations corresponded to the level of the ischemic lesion in the spinal cord and the disease course. Isotopic myelography revealed a complete blocking of the subarachnoidal space. In the cerebrospinal fluid a relative protein-cell dissociation was observed. Pathomorphological examinations of the spinal cord revealed in 5 cases an ischemic decay of the cord and formation of cavities at the level of lymphoid infiltrations to the meninges or of vascular affections.

One hundred and fifty children delivered with cesarean section were investigated on four occasions over 5 years. In infancy the neurologic signs were detected which were caused by perinatal CNS damage. The signs occurrence increased over the first months and then leveled out. At this point the psychoneurologic disorders emerge and enhance under adverse environmental impacts. The dispensary follow-up is necessary for such patients.
