The water level of the liman, volume and area of surface reduced to record low values as a result of intensification of evaporation process in 2014. By the autumn of 2014 the liman was on the verge of extinction. Entry of seawater in the liman through the pipeline began in December of 2014 to prevent environmental degradation of the liman. Purpose of this publication consists in analyzing the changes of hydrological and morphometric characteristics of Kuyalnik liman in 2014-2016 associated inflow of sea water. Methods of study. Hydrological and morphometric characteristics of the liman were calculated on the basis of: field studies, bathygraphic curves, data of satellite images «Landsat». Results. There was dynamics of change of water surface area, volume, level and salinity of liman’s water during the driest periods, at the time and after the inflow of seawater into the liman. The results of measurements of discharge of seawater and coastal watercourses. The effect of evaporative processes taking into account temperature conditions (Odessa, Lyubashevka) on liman’s hydraulicity after the cessation of seawater supply was analyzed. Conclusion. Inflow of seawater into the liman after the first entry of seawater allowed to temporarily stabilize water-salt regime of the liman, however, hydrological and morphometric characteristics of the liman in autumn of 2015 almost returned to the values of early autumn of 2014. During subsequent inflows of seawater into the liman it is desirable to maintain the regime observed during the inflow of water in 2014-2016.

Relevance. Astilbe chinensis has clinical efficacy against many diseases, making this plant a potential natural source for treating diabetes and obesity with a mild, non-aggressive effect. Overall, the study of the biological properties of A. chinensis leaves is currently in its infancy.Aim. In this study, the anti-amylase and anti-lipase potential of aqueous extract of Astilbe chinensis leaves was investigated as an important source of natural complex substances with therapeutic potential. Methods. The total content of phenolic compounds was determined by the Folin-Ciocalteu method and flavonoids by the colorimetric method according to the complexation reaction with aluminum chloride. The effects of aqueous/tea extracts of A. chinensis leaves on pancreatic amylase and lipase were studied via in vitro models in comparison with acarbose and orlistat, respectively. For these tests, the artificial substrates 1,2-O-dilauryl-rac-glycero-3-glutaric acid ester (6-methylresorufin) and 2-chloro-4-nitrophenol oligosaccharide were used.Results. The study of the effect of aqueous extract of A. chinensis leaves on pancreatic amylase and lipase showed a comparable inhibitory effect of 18% on both enzymes.

In multiple sclerosis (MS), the development and introduction of disease-modifying treatments (DMTs) into clinical practice can improve treatment outcomes. Interferon beta drugs are commonly used as DMTs. Among these drugs, sampeginterferon beta-1a (Tenexia®) was recently approved – an innovative original development of BIOCAD that is the next pegylated interferon beta-1a in its class. Clinical observations of patients with relapsing-remitting MS who received Tenexia® are presented. In the first clinical observation, the patient initially received glatiramer acetate as DMT, which led to exacerbations and an increase in neurological disturbances. The patient was switched to treatment with Tenexia®, during which no exacerbations and no increase in neurological disturbances were observed. In the second observation, the patient, who had been suffering from MS for 11 years, did not receive any DMTs. A year ago, the drug Tenexia® was prescribed as a DMT for the first time, with no exacerbations, no increase in neurological disturbances or negative changes on MRI during treatment. In the third observation, a patient suffering from MS for 5 years was initially prescribed teriflunomide as DMT, with exacerbations and an increase in neurological disturbances noted during the treatment. The patient was switched to Tenexia®, which led to a stable remission. The presented observations reflect the positive experience with the use of the drug sampeginterferon beta-1a in real-life clinical practice.

The productivity of agricultural crops is determined by the choice of varieties, planting material, the optimal time of sowing and planting depending on the climatic conditions of the region, the use of organic and mineral fertilizers, and care methods. Planting material plays an important role in the growth and development of onions and the formation of yield. Onions are very demanding in terms of nutrition, this is due to the length of the growing season, growth rates and poorly developed root system. To obtain high and high-quality onion yields, it is necessary to use fertilizers in an accessible form. Water-soluble complex mineral fertilizers quickly and evenly penetrate deep into the soil. Fertilizers improve the growth and development of plants, regulating nutrition processes, increase the yield and quality of the resulting products. As a result of many years of research, it was found that optimizing the mineral nutrition of onions through the use of fertilizers in active form provides a significant increase in the yield of onions. A comparative assessment of the productivity of onions when grown from sets of different fractions using water-soluble complex mineral fertilizers in the conditions of the Udmurt Republic has been little studied.Materials and methods. The purpose of the research: to improve the technology of cultivating onions depending on the fraction of the set and fertilizing with water-soluble complex mineral fertilizers. In 2020–2021 On onions of the Stuttgarter Riesen variety, a two-factor experiment was carried out to study the effect of fertilizing with Aquarin and Rastvorin fertilizers when using different fractions of sets for planting. The studies were carried out using generally accepted methods.Results and its discussion. In the Udmurt Republic, the optimal for planting Stuttgarten Riesen onions is the medium and large fraction of the set, and the positive effect of water-soluble complex mineral fertilizers Aquarin and Rastvorin on the yield and its structure has been revealed. However, it is more economically profitable to use the fine fraction of sets when growing onions.

It is obvious that secondary raw materials resulting from the industrial processing of partial fish species, in particular, the roe film-caviar complex, can be widely used for the production of various food products, including functional ones. There is an obvious need to conduct research on the chemical composition of the roe film-caviar complex of silver carp, carp, pike perch and catfish, which will allow us to adequately evaluate rational approaches to the production of phospholipid and protein products from them that are in demand on the market. The purpose of the research was to study and analyze the chemical composition and energy value of the roe film-caviar complex of silver carp, carp, pike perch and catfish in order to extract valuable substances from it. Data on the total lipid content in the roe film-caviar complex of pike perch and catfish are practically not provided, however, the indicators for catfish eggs and those obtained experimentally almost coincide, but for pike perch there are differences. The identified concentrations of total protein in the composition of the studied caviar make it possible to recommend its use in the technology of protein isolates and textures. The total content of minerals (ash) in the roe film-caviar complex is higher than in fish meat, and averages 1.5...2.0%. The results obtained for determining the mass fraction of ash in the roe film-caviar complex of silver carp, carp, pike perch and catfish can be considered quite satisfactory. Analysis of data on the chemical composition and energy value of the roe film-caviar complex of silver carp, carp, pike perch and catfish, which according to morphological characteristics correspond to the IV stage of maturity, shows that in terms of the chemical composition these products belong to high-protein raw materials, while the caviar of all of the above fish, except, perhaps, pike perch, it also belongs to fatty raw materials, while the calculated energy value does not allow this raw material to be classified as dietary.

Introduction. The creation of new medicines and their introduction into production depends, in particular, on the organization of scientific research in pharmaceutical universities. The ability to fully utilize all the material, financial and human resources available to a university determines the possibility of effectively generating pharmaceutical innovations. This, in turn, is possible if there are methods of scientific analysis of the process of scientific activity of a university.Aim. Creation of methods for scientific analysis of the process of scientific activity of a university, allowing, on the basis of regularly collected information about the scientific activity of departments, to identify areas for improving the organization of scientific research.Materials and methods. The study used logical, analytical-synthetic research methods, modeling, and the method of expert assessments. The scientific reports of the Perm State Pharmaceutical Academy (PSFA) departments for 2019 were studied.Results and discussion. A model of the functioning of the scientific activities of a university has been developed using the example of PSFA, reflecting the degree of interaction of individual departments with each other. Original methods for assessing the effectiveness of individual departments were developed based on the ratio of material, financial, and human resources invested in the work of the departments, and the scientific and practical output obtained as a result of scientific activities.Conclusion. The created model for organizing the scientific activities of the university made it possible to identify departments as “generators” and “successors of scientific research”, as well as the phenomenon of imbalance or lack of interaction between departments. Original methods for assessing human resources and scientific output of the department have been created. To evaluate the work of the department, a rule was formulated for the balance between the resources invested in the work of the department and the scientific result obtained, as well as the balance between the scientific output itself, expressed in scientific publications, and the practical output, expressed in received patents, grants and implementation acts.

The article examines the characteristics of the formation of the rural settlement network in the territory of modern Omsk region during the last quarter of the 19th century through the early 20th century. The relevance of this study is driven by societal and governmental interest in rural history, as well as by the expansion of the source base, which is related to the popularization of museum collections and personal archives. Throughout the research, the author analyzes a wide range of sources and identifies features of the territory’s settlement, lifestyle, and everyday life of peasants, as well as the nature of relationships between settlers and natives. The author concludes that natural and geographical conditions played a significant role in shaping the network of rural settlements, organizing economic life, and the daily routines of peasants. Particular attention is paid to the study of the economic life of peasants, as it was one of the defining factors in the formation of the rural settlement network of the territory. The investigation reveals that agriculture was the main occupation for peasants in the Omsk Irtysh region, despite the limited amount of arable land available. Livestock breeding was confined to its economic significance. Additional income for peasants depended on their settlement location and was determined by the natural resources of the territory. Natural and geographical conditions also dictated the external appearance of settlements.

This paper analyzes the literature data on the practical use of plants of the genus Filipendula as a nutritional component and a promising source of biologically active substances. For immunomodulatory action, the flowers and leaves of the meadowsweet are mainly used, which have demonstrated a significant effect in numerous in vitro and in vivo models. Among the diseases in which the immunomodulatory properties of plants of the genus Filipendula are found the most application are inflammatory processes of the upper respiratory tract, a number of inflammatory disorders of the skin, as well as a diaphoretic and antispasmodic, in bronchial asthma, etc. An analysis of the literature data allows us to conclude that these plants have a beneficial effect on human health, both individually and as a component of food, due to their unique phytochemical profiles.

Relevance. Pumpkin is a food, medicinal and fodder crop of the Cucurbitaceae family. It is valued for its high content of vitamins, minerals, organic acids. At present, thanks to the huge variety of species and modern selection, it is possible to select pumpkin varieties for cultivation in almost any region, including areas with short and not too hot summers. Udmurtia belongs to the zone of risky farming, the end of spring frosts is observed on average in the first ten days of June, and autumn frosts on the soil are possible at the end of August. Due to the short frost-free period, it is necessary to choose earlier maturing varieties, grow the crop through seedlings and use temporary shelters. The pumpkin fruit is usually large in size, is a thick layer of pulp, covered with a dense peel. When cutting such a fruit, the pumpkin is not stored in the future, it requires urgent processing. In this connection, in recent years, portioned varieties of pumpkin, the size of the fruit of which does not exceed 1.5–3.0 kg, have become increasingly in demand.Materials and Methods. The purpose of the research: a comparative assessment of varietal characteristics of growth and productivity of pumpkin species. In 2020–2021 in the conditions of the Udmurt Republic, experiments were carried out to study the characteristics of growth and productivity of pumpkin varieties. We studied varieties of pumpkin: Pearl, Crumb, Honey Tale, Marble, Rossiyanka, Smile, Candied fruit.Results and its discussion. Within two years, pumpkin varieties of large-fruited and nutmeg were studied. On average, for two years, the pumpkin Medovaya skazka distinguished itself with the highest yield. In 2021, the yield for the Medovaya Skazka variety was 64.7 t/ha. The yield increase was obtained due to an increase in fruit weight, which in 2020 amounted to 2.9 kg, in 2021 - 6.5 kg. The studied varieties of pumpkin differed in quality indicators of fruits. On average, over two years, the varieties Rossiyanka and Mramornaya showed an increase in the content of water-soluble sugars in fruits. The fruits of the pumpkin Candied fruit and Kroshka were distinguished by a high content of dry matter.

Business structures, scientific organizations and the state are the key subjects of national innovation systems, interacting, they form the so–called «triple helix». The subject of research in the article is the problem of the formation of the «triple helix» mechanism for the purposes of regional innovative development. In Europe, the mechanism of innovative development according to the «triple helix» model is the main one in regional development, and its components are equal partners, all three subjects interact with each other through the development of horizontal ties. In the Russian version, the mechanism has certain specifics. State funds remain the largest source of funding for Russian science. Basic scientific research is carried out by academic, not university structures. Universities do little to commercialize research results. It can be argued that in the Russian regions, the «triple helix» model as such is practically absent, however, there is some progress in its formation, there are successful examples. The authors systematize the main problems associated with the introduction of the «triple helix» mechanism and indicate directions for their solution.

In agriculture, the use of microbiological fertilizers is becoming more and more widespread. The presence of microorganisms in them allows improving soil fertility, increasing the productivity of crops, while ensuring the cultivation of environmentally friendly products. There is a wide variety of microbiological fertilizers, so determining the type of preparation for garden strawberries when grown in the conditions of the Udmurt Republic is relevant. The purpose of the research: a comparative assessment of the effect of various types of microbiological fertilizers on the productivity of garden strawberries.Materials and Methods. Studies on the effect of microbiological fertilizers on the yield and its structure of garden strawberries were carried out on the territory of Vostochny LLC, Zavyalovsky District, Udmurt Republic. In 2016, 2018 studies were carried out on the feeding of garden strawberries of the Darenka vari- ety of the first and third years of fruiting with microbiological fertilizers on soddy-medium-podzolic medium-loamy soil. The scheme of the experiment included the following options: Water (control), Baikal EM 1, Emix, Humat EM. The total area of the plot is 2,9 m2. The accounting area of the plot is 1,8 m2. Placement of variants by a systematic method, in six-fold repetition.Results. Harvesting was carried out in five terms. Studies have proven the positive effect of the studied microbiological fertilizers on plant productivity. The highest yield of berries was 833,6 g/m2, which was obtained using Baikal EM 1. The largest berry was obtained in 2016, also when treated with Baikal EM 1, and amounted to 145 g. lo to increase the number of berries from the bush. So, in 2016 their number was 45,2; in 2018 – 42,0 pieces.

A 43-year-old female zoologist has an occupational dermatomycosis of the face, neck and upper part of chest caused by T. mentagrophytes. It was mistakenly diagnosed and treated as "Rosacea, papulopustular subtype, ocular rosacea" for 1.5 years. Symmetrical, atypical clinical manifestations with recurrent blepharoconjunctivitis developed as a result of self-treatment using a topical steroid. The detection of cicatrizing folliculitis in the eyebrow area and an arcual peripheral edge in fresh foci on the chest helped to suspect dermatomycosis. The definitive diagnosis was made after the detection of abundant coarse septate mycelium during microscopic investigation of skin scales from foci on the face and chest with 30% KOH and an abundant growth of Trichophyton mentagrophytes during a cultural research study of skin scales, eyebrow hair and eyelashes from lesions. The use of terbinafine for 4 weeks led to the recovery of the patient.

Currently there are a large number of psoriasis classifications, slightly different from each other, used worldwide. Some of these classifications contain the disease forms allocated descriptively, without the pathogenesis characteristics in their basis. Among the dermatologists, there is lack of common understanding of various psoriasis forms peculiarities. And that affects both the diagnostic process and the approaches to therapy. The authors propose a revised unified clinical classification of psoriasis, including psoriatic arthritis. This method reflects the up-to-date view on the classification of psoriasis in Russia and globally.

The development of protective coatings based on pectin substances that serve as natural structure-forming agents developed from a secondary resource base is focused on removing a complex problem in the production of finished products by deep processing of basic raw materials and developing an original film material. At the same time, the technical result of obtaining pectin-containing film structures is their ability to protect food products from microbiological spoilage, natural losses during storage and the preservation of quality and safety indicators. An important preparatory procedure that determines the efficiency and intensity of extraction processes can be attributed to the operation of dispersing the feedstock, since it directly affects the size of the contact surface area of the phases involved in mass transfer. However, it is clear that the greater the degree of grinding, the higher the efficiency of the process, but an excessive increase can lead to additional unjustified energy costs and, as a result, to an increase in the cost of the finished products sold. In this regard, the purpose of the research is to analyze existing technologies for producing pectin-containing extracts, in which watermelon rinds can be used as secondary resources and to intensify the extraction process by conducting a rational preparatory procedure for grinding the raw materials. The object of the research are watermelon rinds as non-recyclable waste from processing watermelon raw materials. According to the obtained graphs, and taking into account the high rates of gelatinization of the obtained pectin extracts, it is possible to recommend a complex treatment of watermelon rinds, including, in addition to the operations of preliminary preparation of raw materials, ultrasonic exposure and acid hydrolysis, and it is desirable to use food acids, for example, acetic or citric, instead of aggressive sulfuric and hydrochloric acids. As a result of the study of the dispersed composition, it has been concluded that the average equivalent particle size of the dispersed raw materials does not exceed the recommended limits, that is, the result obtained can be considered acceptable.

Reduction of technological losses of natural gas during production, transportation and storage is of major energy and environmental importance. Losses of natural gas are the target for active criticism by «green energy» representatives. It is necessary to develop and popularize the new technological solutions aimed at reducing the indirect impact on the environment associated with the production and use of liquefied natural gas, to demonstrate the readiness of specialists of this energy sector to take into account modern social preferences. The article presents the numerical analysis of the energy efficiency for one of the stages of large-scale LNG production, namely, the LNG delivery from a storage tank to a LNG carrier vessel via a cryogenic pipeline. The mathematical model of the thermal enclosure of the pipeline has been considered, and the production cryoproduct losses due to thermal inertia of pipeline materials have been estimated. The performance of the schemes recommended for utilization of LNG vapors has been evaluated

Relevance. One of the most important elements of the technology of cultivation of strawberries for increasing productivity and improving the quality of berries is the use of fertilizers that can regulate the growth, development, yield and quality of products.Methods. During 2016-2018, the impact of pre-planting application of complex mineral fertilizers on the yield and quality of strawberries in the Udmurt Republic was studied. The analysis of meteorological conditions during the growing season of the crop according to the research results is carried out.Results. The results of the use of complex fertilizers Azofoska, Perm-yagodnoye and Fusco-yagodnoye on garden strawberries when grown in the conditions of the Udmurt Republic are presented. It was found that the use of these fertilizers in comparison with the control (Azofoska) contributes to an increase in yield and has a positive effect on the quality of berries. The aftereffect of the studied fertilizers on the second and third year of fruiting of wild strawberries is noted. According to the results of research, the highest yield of berries of the garden strawberry was obtained in 2016 in the first year of fruiting when applying Chamfer-berry (1.33 kg/m2 ). The pre-planting application of the FaskoYagodnoye fertilizer provided a reliable increase in yield at all times of berry harvesting, and on average for three years by 0.13 kg/m2 with an NSR of 0.04 kg/m2 . The content of dry matter, water-soluble sugars and vitamin C in the berries corresponded to the characteristics of the Darenka variety and to a greater extent depended on the meteorological conditions of the growing season of strawberries.

Aim. Comparative analysis of inhospital outcomes after open thrombectomy versus conservative in patients with acute lower limb artery thrombosis and coronavirus disease 2019 (COVID-19).Material and methods. In this retrospective, comparative study for the period from April 1, 2020 to December 1, 2021, 167 patients with acute lower limb artery thrombosis and COVID-19 were included. Depending on the treatment strategy, two following groups were formed: group 1 — open thrombectomy (n=136) + drug treatment (anticoagulant (unfractionated heparin) and antiplatelet (acetylsalicylic acid 125 mg 1 time per day) therapy; group 2 — only drug therapy (n=31). This group consisted of patients who refused surgical revascularization. In all cases, a psychiatrist examined for personality disorders that did not allow a critical assessment of their condition and the consequences of refusing surgical treatment. At admission to the hospital, all patients received prophylactic-dose unfractionated heparin (5000 IU 3 times/day). In the development of acute arterial thrombosis, 80 IU/kg (maximum 5000 IU) of unfractionated heparin was administered intravenously, followed by transfer to intravenous infusion at an initial rate of 18 IU/kg per hour with the partial thromboplastin time monitoring. Analgesic and antiplatelet therapy (acetylsalicylic acid 125 mg 1 time/day) was also prescribed.Results. Myocardial infarctions, ischemic strokes were not recorded. There were no significant intergroup differences in mortality rates (group 1: n=52, 38,2%; group 2: n=7, 22,6%; p=0,09; odds ratio (OR)=2,12; 95% confidence interval (CI): 0,85-5,27), limb amputation (group 1: n=63, 46,3%; group 2: n=9, 29,0%; p=0,07; OR=2,11; 95% CI: 0,9-4,91). However, there was a trend towards a decrease in the frequency of these events in the conservative therapy group. After open thrombectomy, retrombosis developed in 50,7% (n=69) of cases, whilethrombosis after retrombectomy followed by amputation — in 46,3% (n=63). There were no hemorrhagic complications in both groups.Conclusion. Open thrombectomy with concomitant medical therapy and single conservative therapy without surgical revascularization in the present study showed comparable rates of death and lower limb amputations in patients with COVID-19.

Therefore, their early diagnosis with the assessment of a clinical phenotype and unfavorable prognostic factors and the timely initiation of therapy are important. The paper provides the expert consensus on the definition of the early stage of Ps, PsA, and IBDs, their treatment goals and main unfavorable prognostic factors. It also gives the rationale for the early use of biological agents in patients with immune-mediated inflammatory diseases.

The paper provides the experts agreed opinion on the definition of the early stage of Ps, PsA, and IBDs, the goals of therapy and main unfavorable prognostic factors for the course of these diseases and gives the rationale for the early use of biological agents in patients with immune-mediated inflammatory diseases.

Relevance. A large reserve to increase the production of tomatoes for different periods of cultivation and purposes of use are highly productive new varieties and heterotic hybrids. In this regard, one of the important conditions now is the maximum compliance of newly created varieties and hybrids with modern technologies that allow obtaining competitive products. In addition to the main indicators (high yield, early maturity, favorable maturation), varietal features such as transportability of fruits,high taste appeal (especially salad-type hybrids), appearance, resistance to major diseases and manufacturability are also important. In recent years, tomato and other vegetable crops have grown mainly hybrids of the first generation F1 and line (the result of targeted selection), less often populations. For more than a decade, only hybrid varieties have been grown in industrial greenhouse vegetable production.Methods. The article presents the results of 7-year research on the yield, its structure and quality of fruits of indeterminate tomato hybrids. Research on the yield and quality of fruits of indeterminate tomato hybrids was carried out in 2011-2017, in JSC "Zavyalovsky Greenhouse plant" of the Udmurt Republic.Results. Hybrid F1 tomato Ismail, F1 Macarena, F1 Mace, F1 Geronimo and F1 Starbuck were distinguished by high productivity. This indicator was respectively: 34.5 kg / m2; 33.1 kg / m2; 32.3 kg / m2; 32.0 kg / m2 and 31.2 kg / m2. Tomatoes F1 Yakimanka, F1 Foronti, F1 Starbuck, F1 Taganka and F1 Mace stood out for large-fruited. More than 30 mg / 100 g was contained in F1 Izmail F1 Ochakov, F1 Mariachi, F1 Kokhava vitamin C fruits. The nitrate content in tomato fruits was not high for years of research, it was within the MPC.

The article is devoted to the scientometric analysis of chess as a scientific direction in Russia and abroad. As part of the study, an assumption made about the lack of due attention to the presented scientific topics in our country and the outstripping growth in the countries of Europe and North America. An assessment is made of a pool of publications on the subject in the Scopus and RSCI databases, highlighting the structure and research fronts, a critical assessment of the relevance and particularly affected organizations dealing with a given problem. The expertise of scientific publications has been implemented, which are concentrated in publications on the subject and their scientometric indicators. The conclusion is formulated about the increased interest in the thematic block from various aspects of special knowledge, while the most promising advantage is the study of chess by mathematicians and specialists in the field of artificial intelligence, which is extremely important for the journal. The study conducted in the sectoral appropriation of scientometrics (computer sciences) will be interesting to consider, not only with digital competencies, but also for specialists in the field of scientific data analysis.

Tumor necrosis factor alpha (TNF-α) is one of the key inflammatory cytokines in the pathogenesis of psoriasis and psoriatic arthritis, and its inhibition with genetically engineered biological drugs ensures control of the main symptoms of these diseases. Certolizumab pegol is a PEGylated (linked to polyethylene glycol) Fab’ fragment of a monoclonal antibody that inhibits human TNF-α. It is approved for treatment of moderate to severe plaque psoriasis and psoriatic arthritis. This review summarizes the results of randomized clinical trials of efficacy and safety of certolizumab pegol in plaque psoriasis (CIMPASI 1, CIMPASI 2, and CIMPACT) and psoriatic arthritis (Rapid-PsA). The represented data demonstrate high efficacy of certolizumab pegol is bio-naïve patients and in patients previously exposed to other TNF inhibitors or IL17 inhibitors. We describe three patients with a rare form rosacea-like chronic lupus erythematosus which at the beginning is diagnosed as rosacea due to similarity clinical picture. Netakimab, the original monoclonal antibody against IL-17A, is an innovative drug for the treatment of moderate-to-severe plaque psoriasis in patients who have indications for systemic therapy or phototherapy. Netakimab was approved in Russian Federation, registration certificate number ЛП-005439 from 04.04.2019. This article outlines the first 12-week results of a phase III clinical trial in patients with psoriasis.Materials and methods. The BCD-085-7 study (PLANETA) is a comparative, randomized, double-blind, placebo-controlled phase 3 clinical study of the efficacy and safety of netakimab in patients with moderate-to-severe plaque psoriasis. This review presents the results of the first 12 weeks. The study is ongoing at the moment, the total duration of treatment for each patient is 3 years (154 weeks). Patients were randomized in a ratio of 2:2:1 into one of three arms: group 1 received netakimab subcutaneously at a dose of 120 mg once a week for the first three weeks (induction) and then once every 2 weeks up to week 10, group 2 received netakimab subcutaneously at a dose of 120 mg once a week for the first three weeks (induction) and then once every 4 weeks up to week 10, group 3 received a placebo subcutaneously on day 1 at weeks 0, 1, 2, 4, 6, 8 10. In order to maintain a double-blind design, placebo was administered to patients in group 2 at weeks 4 and 8.Results. Both netakimab groups showed a significant superiority over placebo (p < 0.001) and the absence of statistically significant differences between the two regimens of therapy (p > 0.05) across all endpoints. PASI 75 at week 12 was reached by 77.65 % of patients using netakimab once every 2 weeks and 83.33 % of patients using netakimab once every 4 weeks (ITT population). The rate of clear and almost clear skin (sPGA 0–1) was reached by 81.18 and 79.76 % of patients using netakimab once every 2 weeks and once every 4 weeks, respectively. The safety assessment showed no statistically significant differences between the groups, the incidence rate of adverse events in netakimab arms was not higher than in the placebo arm. There were no cases of early withdrawal due to adverse events and cases of grade 4 toxicity according to CTCAE 4.03. During the 12 weeks of the study, one serious adverse event was registered in group 2 (pneumonia grade 3), which was recovered without any consequences. The immunogenicity assessment showed binding antibodies formation at week 12 in one patient who received BCD-085 every 2 weeks. Neutralizing antibodies were not detected. Conclusion. Netakimab showed high efficacy in the treatment of psoriasis, more than 80 % of patients achieved PASI 75 and sPGA 0–1 (clear and almost clear skin) by the week 12 of treatment. The drug showed a favorable safety profile and low immunogenicity. Based on the study results the regimen once a week during the first 3 weeks (induction), then once every 4 weeks was chosen for medical use in patients with psoriasis.

Objectives. Cryogenic surgical instruments have been successfully used to conduct tumor ablation. For a long time, cryoablation procedures in Russia were carried out using cryodestructors cooled with liquid nitrogen, which are able to remove heat from the ablation object with high heat flux density and quickly form a cryoablation zone, while being relatively cheap and easy to operate. However, these instruments turned out to be unsuitable for minimally invasive surgeries; therefore, they are squeezed out of practical medicine and cryosurgical instruments are cooled by throttling argon gas. This led to the purpose of the study - the choice of equipment for the organization of local supercooling of pathological tissue.Method. To solve the problem of optimizing the cryogenic pipeline, a method was chosen for finding the Pareto-optimal solution. To solve this problem, it is sufficient to increase the pressure in the fluid flow directed to the NCS using a liquid micropump. In the role of quality criteria in this task, we selected: hydraulic loss power and heat loss power.Result. The following results were obtained: the minimum pressure in the vessel, which ensures the movement of the fluid in a single-phase state, according to the magi-line of 1 m length is 0.75 MPa; With this pressure, through a line with a diameter of 1 mm, the flow rate is maintained up to 6 kg / h; heat dissipation ability of the instrument reaches 608 watts. The thermal load on the cooling system of the heatdissipating device of the device for CA is unsteady and is formed due to the heat reserve accumulated in the patient's tissues.Conclusion. The use of liquid nitrogen in the undersized cryosurgical equipment makes it possible to overcome the noted drawbacks of liquid cryodestructors.

Objectives. The equipment for general cryotherapy (WBC) has been used for more than 40 years without a clear understanding of the mechanism of therapeutic action, the technological conditions of patient safety, the nominal cost of energy in cryostation systems. Uncertainty in the fundamental issues of the production and operation of cryotherapy equipment has caused a gradual degradation of the WBC method in countries that have long determined the development of this section of cryomedical technology.Method. In Russia, thanks to the advanced development of the scientific basis of the production and operation of facilities for the WBC, this method is developing dynamically. Exports of cryo-therapeutic equipment are growing. In the foreign literature, the number of publications denying the effects of the WBC is growing. These articles describe research performed on foreign-made equipment, which is significantly inferior to Russian samples. Such publications discredit the WBC method, hamper the export of Russian equipment, and hinder the spread of effective treatment methods in Russia.Result. The results of the analysis of differences in design and technological parameters of domestic and foreign equipment are presented. The technique of numerical analysis and optimization of WBC technology is described. It is shown that the basis of the effectiveness of Russian equipment is rational design solutions and high power supply of cryostatting equipment.Conclusion. The individual technology of the procedures combines high therapeutic efficacy with relatively low energy costs, and also provides the traditional medicine principle of individuality of treatment. Research results should be popularized for the gradual distribution of single-unit installations in all medical facilities.

This article is about the case of actinic reticuloid — the rare dermatosis which clinical presentation is similar to atopic dermatitis, T-cell lymphoma. Good treatment effect was obtained by long cycles (2 cycles for 3 months) of hydroxychloroquine and sun protective therapy included sunscreens SPF 50, nicotinic acid, sun-safe clothes which blocked ultraviolet radiation without any glucocorticosteroid drugs and cytostatic treatment.

Professor Sergei Iakovlevich Kulnev, student and follower of the founder of the Russian syphilidology Veniamin Mikhailovich Tarnovsky, was the head of the Department of Skin and Venereal Diseases at St. Petersburg Medical Institute for Women in the difficult years of its formation – from 1903 to 1924. In fact, the merit of establishing the Department with clinic in the newly formed educational institution for women belongs to him. In 1913, with the active participation of S.Ia. Kulnev, the separate building for the Department of Skin and Venereal Diseases was built, and to this day, it is still located at this premise. Also, for almost a quarter of a century (from 1901 to 1924) S.Ia. Kulnev was the chief physician of the Kalinkinsky Hospital, the oldest specialized dermatological and venereal hospital in Russia and in Europe, where, in fact, Russian syphilidology was born.In Soviet times, the merits of S.Ia. Kulnev who was an actual state councillor and representative of the famous noble Russian family, as an organizer of the Department with Clinic of Skin and Venereal Diseases, was undeservedly ignored.His activity appeared to be in the shadow of achievements of his brightest students, graduates of Medical Institute for Women A. A. Sakhnovskaia and O.N. Podvysotskaia, who alternately headed the Department after the death of Sergei Iakovlevich.The objective of the article, dedicated to the 160th anniversary of the birth of Professor Kulnev, was the desire to pay tribute to the memory of a remarkable physician, teacher and man, whose whole life was devoted to the development of Dermatovenereology and the organization of women’s medical education in Russia. The property of S. Ia. Kulnev is not only a thing of the past. His works laid the foundation of today’s educational, scientific and medical activities of the Dermatovenereology Department of Pavlov First St. Petersburg State Medical University.

A review of the literature on the etiology, pathogenesis and clinical manifestations of papillomatosis confluent and reticulated Gougerot–Carteaud (PCR) is presented. The diagnostic criteria and differential diagnosis of PCR are discussed. A 36-year-old man with obesity grade 2 and typical manifestations of PCR is described. The treatment with azithromycin during 6 days appeared to be sufficient to decrease the rash which had existed for 7 years.

An extremely rare case of sinus histiocytosis of the non-Langerhans cell histiocytoses type is described in 55-year-old patient with isolated localization of cutanious lesion on the right cheek without involvement of nodular structures. Main skin lesion was an erythematous-cyanotic node with multiple milia-like lesions on the top of it. Histologically revealed feature was a large number of plasma cells, besides phenomenon of emperipolesis was described, which was first falsely perceived by pathologists as phagocytosis. In immu-nohistochemistry assay the changes were characterized by proliferation of large polygonal histiocytes with accumulation of pentalamellar markers in its cytoplasm (protein S-100, CD68).

A case of belatedly diagnosed late cardiovascular syphilis that had continued progressing despite two courses of ceftriaxone therapy is described. The patient had a combination of all possible complications of syphilitic mesaortitis including aortic regurgitation, coronary insufficiency, as well as fusiform aneurysm of the ascending aorta and the proximal aortic arch. Current issues of diagnosis and therapy of late cardiovascular syphilis are discussed.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease that mainly affects the sites of the apocrine glands; HS is characterised by relapsing painful knots and abscesses, leading to the formation of fistulas and scarring. Modern ideas about the pathogenesis of HS allow us to consider it as a kind of inverse acne, since the mechanism of development of HS and inverse acne does not differ. Detailed studies of patients with HS in recent years have provided the basis for several authors to suggest that HS is a systemic, chronic, immune-mediated progressive disease. The key role of the proinflammatory cytokine tumor necrosis factor Į (TNF-Į) in the development of the inflammatory process during HS was determined. Elevated TNF-Į levels are found in the blood and HS lesions, having a positive correlation of the TNF-Į level with the severity of the disease. At the present time, the treatment with a TNF-Į blocker, adalimumab (the only one genetically engineered biological preparation approved for treatment of HS/inverse acne in the world and in Russia), is recommended as a first-line therapy in the absence of an adequate response to standard systemic therapy in the case of active HS of moderate or utmost severity. The presented clinical case confirms the fact that for HS patients it is very important to determine the correct diagnosis as quickly as possible and to prescribe the appropriate treatment according to the developed recommendations. Erroneous diagnoses, inadequate therapy (surgical treatment, short courses of antibiotics, etc.) contribute to the progression of the disease, to the appearance of common symptoms due to the attachment of secondary infection and increase in general intoxication, to limited working capacity, to the emergence of persistent and gross scars, as well as to the deformations of affected skin. Our clinical case of adalimumab (Humira) treatment of a patient with an extremely severe HS form demonstrated that the application of this medication can be effective and well tolerated.

The current paper features the peculiarities of colonization of the Kolosovsky district of the Omsk region in the 16th – early 20th centuries. The author integrally approaches the study of this process, analyzing the main ways of settling and economic development of the area. Considerable attention is paid to the factors that conditioned the process of development of the territory. The formation of the rural settlement network of the district, in many ways, was determined by the vectors of state policy, in particular, the policy of resettlement of peasants from the country's low-land regions. Favorable geographical and climatic conditions, the presence of the river artery made the territory of the Kolosovsky district attractive for settlers, who both established their own settlements and settled in old-timer villages. The history of the region is considered in the mainstream of the history of the state, taking territorial features into account. The article outlines the stages of development of the territory, characterizes each of them, by emphasizing the economic activity development. The author gives specific dates for the formation of villages, analyzing the available foundation versions.

А clinical case of a rare form of pustular psoriasis, known as “acrodermatitis continua suppurativa Hallopeau” is described. The involvement of the distal phalanges of the fingers and toes in combination with localized psoriasis vulgaris is observed. Specific features of this case are the presence of two different forms of the disease and comorbidities, that limits treatment options. The article provides current data on the clinical features and management of acrodermamtitis continua suppurativa Hallopeau. An acceptable personalized treatment with systemic retinoids and topical substances containing betamethasone dipropionate and calcipotriol has been proposed.

The tertiary cutaneous syphilis has become extremely rare as a result of the effective treatment of early syphilis with penicillin and occasional antibiotic therapy for concomitant infections. We report a case of tertiary gummatous syphilis associated with asymptomatic neurosyphilis that was initially misdiagnosed as basal cell carcinoma of the skin. A 67-year-old woman presented with the 18-month history of multiple, firm, bluish-red asymptomatic nodes on the parietal scalp, chin and lateral part of the trunk that had developed without preceding trauma. One node on the trunk had small central ulceration covered with dry yellowish crust. On the left retroauricular area the brownish-purple atrophic scar was observed. Blood tests revealed reactive VDRL, ELISA and TPHA assays. Skin biopsy has showed endarteritis with endothelial swelling and caseating granulomas along with plasmocytic infiltrate in the dermis and subcutaneous tissue. Increased lymphocyte count and the positive VDRL test with cerebrospinal fluid have confirmed the diagnosis of asymptomatic syphilitic meningitis in this patient. The treatment with aqueous penicillin intravenously has lead to resolution of the skin infiltrates and progressive more than 4-fold reduction of follow-up serum VDRL titers. This case is reported as a reminder of the possibility of tertiary syphilis with misleading clinical patterns in patients infected during the epidemics of 1995-2000. We would like to emphasize the necessity to perform timely serological and pathomorphological study to prevent misdiagnosis and mistreatment.

The aim of the study was to determine the level of cytokines in the serum of patients with psoriatic arthritis (PsA) and psoriasis vulgaris (Ps) and to identify key immune markers associated with clinical manifestations of psoriasis. Materials and methods. The study involved 52 patients with psoriatic arthritis (PsA) and 20 patients with psoriasis vulgaris (PS) by multiplex analysis of proteins using Bio-Plex device and commercial test kits 23-Plex and 27-Plex, 8-Plex. Assessment of the severity and prevalence of psoriatic skin lesions in patients was conducted by index PASI (Psoriasis Area and Severity Index). Indicators of acute-phase activity (ESR, CRP), CEC were defined in all patients. the comparison group consisted of patients with a diagnosis of patchy scleroderma (10 patients) and patients with atopic dermatitis (10 patients). 13 healthy individuals were examined as a control group. Main results. Significant elevation of cytokines: IL-2, IL-6, GM-CSF, IFN-y and TNF-a was found in the serum of patients with PS compared with the group of apparently healthy individuals, and GM-CSF, IFN-y compared with patients with atopic dermatitis. It was determined that the detected levels of Il-В in serum has a direct correlation with the prevalence and severity of PS. Inverse correlation between ESR and the level of IL-8, GM-CSF and the CEC, IFN-y and the CEC and the direct correlation between the level of TNF-a and CRP in serum were shown. Conclusion. The obtained data indicate shifts in the system of pro-inflammatory and anti-inflammatory cytokines in psoriasis, which can be considered as a manifestation of endogenous homeostatic mechanisms designed to limit the intensity of the inflammatory process. Immune markers associated with clinical manifestations of psoriasis, in particular, the prevalence and severity of PS (index PASI), indicators of acute-phase activity (ESR, CRP), were identified.

The data are given about the history of the invention and development of antimicrobial therapy in the XX-th and the beginning of the XXI-th centuries. There are presented data on the pharmacological effects of antibacterial agents from different groups that are used for the treatment of patients with pyodermatosis, dose schedules, pharmacokinetics, adverse effects, contraindications, and medicine interactions. The main causes leading to the formation of resistant microorganism strains are pointed. Advances of broad-spectrum antibacterial antibiotics for treatment the pyodermic patients are demonstrated on the example of amoxicillin+clavulinic acid, josamycin and doxycycline monohydrate. The arguments justifying the need to refuse empiric administration of antibacterial therapy are presented. To select a rational causal initial therapy of patients with pyoderma it is necessary to use the results of microbiological study of etiology and pathogenic microflora sensitivity to antimicrobial preparations.

A review article is addressed the issue of the diagnosis and treatment of neurosyphilis that is developing against the background of HIV-infection. HIV-infected patients are at higher risk of neurologic, ocular and auricular manifestation of syphilis as well as treatment failures and relapses. Diagnosis of neurosyphilis in HIV-positive patients is complicated because both infections cause similar changes in the cerebrospinal fluid (CSF). The effectiveness of neurosyphilis treatment in patients with HIV co-infection is difficult to estimate, since the normalization of their CSF goes slower comparing to HIV-negatives. The increase in incidence of syphilis and HIV co-infection is anticipated in the coming years. This necessitates a comprehensive study of the problem and requires the development of new approaches to neurosyphilis diagnosis and treatment in co-infected patients.

The introduction of penicillin has been a breakthrough in the treatment of syphilis. For 70 years, penicillin remains the preferred drug for the treatment of all forms of the disease; its effectiveness has been proven by wide experience gained from clinical observations. However, a very limited number of published meta-analyzes, systematic reviews and data from randomized controlled trials on the comparative assessment of the effectiveness of various antibiotics and treatment regimens are currently available. Some aspects of syphilis treatment are insufficiently developed, particularly, the treatment schedules for neurosyphilis, syphilis in pregnancy, syphilis in HIV-infected individuals and persons allergic to penicillin. There are disagreements in the assessment of the clinical significance of serofast state after a course of antibiotic therapy. There is no uniform approach to the management of patients with treatment failures. This article compares the Russian standards for syphilis treatment set out in the «Federal guidelines for the management of patients with syphilis» with the current European recommendations of the International Union against Sexually Transmitted Infections (IUSTI) and the recommendations of the US Centers for Disease Control and Prevention (CDC). The peculiarities of these guidelines, their strengths and shortcomings, as well as controversial issues of syphilis therapy are discussed. The differences between the domestic and foreign recommendations are more significant in the section concerning the treatment of syphilis than in the section of diagnosis.

A review article highlights the practical issues of design, implementation and effectiveness estimation of STI prevention programs aimed to correct the behavior leading to infection. the importance of epidemiological modeling method for the organization of preventive interventions is discussed. the prospects of the multidisciplinary behavioral approach to STI prevention are demonstrated.

Results of sampling epidemiological investigation of patients with dermatologic diseases, which sought treatment in the specialized Russian outpatient facilities over two-month period, and assessments of approaches to treatment of patients with atopic dermatitis, psoriasis and eczema are presented. Data of 2991 individual registration cards, which were filled out by the dermatovenerologists providing primary specialized care to patients in outpatient facilities of Perm, Samara and St. Petersburg, was analyzed. It was observed that women were prevalent among patients with dermatoses, and most of women and men were of working age (82.2 and 89.2% corresponding). The nosological structure of appealability consists of 25.6% patients with dermatose, psoriasis (14.5%), eczema (13.4%), seborrhea and acne (8.6%), atopic dermatitis (5.8%), and pyodermatitis (3.2%). Using standardized indexes for assessment it was discovered that among applied outpatients 80% persons, presented with atopic dermatitis, and 50% persons, presented with psoriasis, had moderate and severe appearances of dermatoses. In therapy of patients with atopic dermatitis, psoriasis and eczema, specialists used widely standard system treatment - topic glucocorticosteroid agents for external care.

Atopic dermatitis (AD) is a common skin condition treated by dermatologists all over the world. Guidelines for management and treatment of AD have appeared in Europe, USA and Russia recently. the paper contains comparison of European, American and Russian guidelines. Definition of AD, epidemiology, pathophysiology of the disease, clinical features, diagnostic criteria, and risk factors are consistently matched. the analysis demonstrated that guidelines do not contain serious contradictions and principal differences thus stressing their supplementation of each other.

Despite the abundance of existing laboratory methods the diagnosis of syphilis still faces many challenges. Though direct detection of T. pallidum plays an important role in early manifest forms of the disease, serological tests remain the mainstay of diagnosis. Traditional syphilis screening algorithm based on nontreponemal tests with subsequent confirmation using treponemal tests is a standard worldwide. Recently, the ability to automate the treponemal tests promotes the increasingly widespread implementation of reverse algorithm when these tests are used for syphilis screening. None of the current serological algorithms are able to reliably differentiate between active and previously treated syphilis, which causes uncertainty in the management of patients. There is no «gold standard» for the diagnosis of neurosyphilis, ocular, auricular and visceral syphilis. The interpretation of serological tests in children born to seropositive mothers is also complicated. Diagnosis of congenital syphilis in newborns and, consequently, the prescription of antibiotic therapy often depends on assessment of the adequacy of maternal treatment during pregnancy, which leads to subjective decisions. This article provides a comparative analysis of the «Federal guidelines for the management of patients with syphilis» and their foreign analogues, discusses significant peculiarities of these guidelines and reviews current concerns and controversies in syphilis diagnosis.

The history of the Department of Dermatovenereology of the First St. Petersburg State Medical University from its foundation in 1901 till 1956 is traced in the article. These years of formation and development of the Department were associated with the activities of the outstanding dermatologists such as T.P. Pavlov, S.Ya. Kulnev, A.A. Sakhnovskaya, O.N. Podvysotskaya.
