Comparative analysis of dissemination by H. pylori of the bile portions in patients of a control group, suffering an acute calculous cholecystitis (ACCH), was performed. Dissemination of H. pylori in a control group was significantly less, than in a bile portions of patients, suffering ACCH. While analyzing the rate and degree of dissemination by H. pylori of the gastic and gallbladder mucosa biopsies of patients, suffering chronic non-calculous cholecystitis, associated with duodenogastric reflux and gastroduodenitis, bacteria were revealed trustworthy more often and in more number, than in a gallbladder mucosa in patients, suffering ACCH.

The results of a survey of school children with chronic gastroduodenitis when applying at an early period the medical rehabilitation with method low-frequency light-magnetotherapy. During treatment of hospital was evaluated vegetative-trophic status with methods of cardiointervalography and thermovision functional tests. In normalizes clinical parameters was correction in dynamics of the vegetative status in children, it confirms the effectiveness of the therapy. It is proved, that the use of low-frequency light-magnetotherapy has a positive effect on the vegetative--trophic provision an organism and normalizes the vegetative dysfunction.

Objective: To establish the diagnostic value of the determination of leukocyte composition of inflammatory infiltrate in chronic gastroduodenite in childhood., Patients and Methods: We examined 103 patients aged 8-17 with chronic gastroduodenitis associated with Helicobacter pylori. To detect Hp and Epstein-Barr viral infections we used esophagogastroduodenoscopy, quick urease test, bacterioscopy of gastrobiopsies. We performed the analysis of the cellular composition of the inflammatory infiltrate., Results: It was found that the number of lymphocytes and neutrophils located in the lamina and intraepithelial increases, which is associated with the degree of inflammation. Increased to the maximum was the number of intraepithelial lymphocytes, both in the body, and in the antrum. Intensity of leukocyte infiltration is directly correlated with the increased prevalence of inflammation. Persistence of Epstein-Barr virus (35.9% of patients) is followed by more severe intraepithelial lymphocytic infiltration in the stomach. In 4-6 months after treatment 18 patients with severe gastritis were repeatedly studied for the inflammatory infiltrate. A significant decrease in the number of intraepithelial neutrophils was found., Conclusion: Cellular composition of the infiltrate is an objective characteristic of chronic inflammation in the gastric mucosa. Persistence of Epstein-Barr virus is accompanied by an increase in the amount of intraepithelial lymphocytes and neutrophils.

Objective: Our aim was to reveale the features of histological mucosa with different composition of the microflora of stomach and duodenum in children with chronic gastroduodenitis., Methods: The study included 122 children with chronic gastroduodenitis from 5 to 17 years old. All patients underwent endoscopy with histology of biopsy specimens of gastric and duodenal mucosa in 94 patients and in 48 patients among them with morphometry of biological material. Identification of herpes simplex virus 1, 2 types, cytomegalovirus, human papilloma virus 16, 18 types, Helicobacterpylori was carried out using polymerase chain reaction in biopsies of gastroduodenal mucosa and gastric juice., Results: Children with chronic gastroduodenitis depending on the identified microorganisms were divided into groups: 1st (n = 51)--without detectable microorganisms, 2nd (n = 28)--with associations of H. pylori and viruses, 3rd (n = 15)--only with viruses, 4th (n = 28)--with H. pylori. Thus, in patients with H. pylori inflammation was noted mainly in the antral mucosa and dystrophic changes--in the duodenal mucosa. In the presence of viruses the inflammation was expressed more in the fundal stomach and duodenum. When viral-bacterial associations were observed most pronounced inflammatory-dystrophic process was localized throughout the gastroduodenal mucosa., Conclusion: In children with chronic gastroduodenitis in the presence of bacterial, viral and mixed microflora in the stomach and duodenum identified characteristic histological features of the mucosa, manifested in a different localization and severity of inflammatory and dystrophic changes.

The article considers the microbe specter of mucous coat of esophagus and stomach in children with gastro esophageal reflux disease and chronic gastroduodenitis. In patients with diseases of upper parts of digestive tract, the opportunistic pathogenic bacteria are isolated more often and their strains have aggression factors and cytotoxicity. In children with reflux disease the microbe landscape of esophagus is larger both in numerical and specific respect as compared to healthy adolescents and children with isolated gastroduodenitis.

At present, chronic gastroduodenitis (CGD) occupies one ofthe leading places in the structure of diseases of the gastrointestinal tract in children. In the etiology of CGD, along with the leading pathogenic Helicobacterpylori (HP), the role of the fungal flora increased. The aim of the work was to evaluate the functional activity of neutrophils in children with the CGD, associated with HP and Candida albicans. Among 110 children in the age from 7 to 17 years with chronic gastroduodenitis, associated with Helicobacter pylory(HP), as well as the association of HP with Candida albicans and the markers of secondary immune insufficiency, a study of the phagocytic activity and immune phenotype of neutrophils by flow cytofluorimetry was conducted. Differentiated peculiarities of the phagocytic activity of neutrophils in association with bacterial pathogens (HP) and fungal flora were identified. The transformation of the immune phenotype was combined with a significant depression of the phagocytic and microbicidal functions, more pronounced with the association of HP and Candida albicans. Circulating mannano protein antigen of Candida albicans influenced on the surface of phenotype of neutrophils, increasing the expression of protopathic and HLADR-receptors, and decreasing the expression of adhesion receptors and cytolysis. Thus, in case of chronic gastroduodenitis in children, there was a considerable transformation of the phenotype of neutrophil with differentiated characteristics at the association with bacterial (HP) pathogens and fungal flora. The obtained data should be taken into account when carrying out medical activities, and the doctors should include in the composition of complex therapy of CGD, associated with Candida albicans, drugs, aimed at immunocorrection of the identified violations

Aim: To determine a population level and significance of Helicobacter pylori in normal microbiocenosis and dysbiosis of mucosal microflora in the esophagogastroduodenal zone., Material and Methods: Qualitative and quantitative composition of mucosal microflora was defined in biopsy specimens from different parts of the esophagus, stomach and duodenum and clinical and histological examinations were made in 50 healthy volunteers, 130 duodenal ulcer patients, 24 patients with gastric ulcer, 36 with chronic gastritis and 24 with chronic esophagitis., Results: H. pylori is a component of normal mucosal microbiota of the oesophagus, stomach and duodenum. These bacteria protect normal gastrointestinal microflora. Exacerbations of peptic ulcer, chronic gastritis and oesophagitis are accompanied with overgrowth of dysbiotic mucosal microflora and reduction of H. pylori content in most cases. Healing and scar formation occur both in the presence and absence of H. pylori., Conclusion: The presence of H. pylori in mucosal microbiocenosis in the oesophagogastroduodenal zone has no independent significance in exacerbation of ulcer disease, gastritis and oesophagitis and does not require eradication.

Number of children with chronic gastroduodenitis and duodenal ulcer, is increasing every year. A major role in the development of chronic inflammation of the mucous membrane of the gastroduodenal zone play cytoprotective properties of mucus produced blennogenic structures of these bodies. Therefore, the appointment of therapies to help improve the cytoprotective properties of gastric and duodenal ulcers, remains valid. Presents the results of the study lektingistohimicheskogo slizeobrazovaniya with chronic gastroduodenitis in adolescents.

This study included 112 patients presenting with duodenal ulcer disease and 65 ones with chronic duodenitis treated with the use of sinusoidal modulated current (SMC) electrophoresis in combination with peloidotherapy (peat muds of the Uva health resort, Republic of Udmurtia). This treatment was shown to produce positive effect on the proliferative activity of duodenal epithelium.

It is shown that biliary disorders in a combination with chronic duodenitis are accompanied by infringements of bowel microflora. It is revealed that more expressed changes of structure of microflora were observed at chronic duodenitis 2nd and 3rd degree. The results of investigation of 87 children suffering from chronic gastroduodenitis and atopic dermatitis are presented. All the child underwent fibrogastroduodenoscopy with duodenal mucosal biopsy and diagnostics of Helicobacter pylori infection, lambliasis. Hystomorphological and immunohystochemical studies of mucosal biopsy specimens for Epstein - Barr virus were carried out. The role of atopy and infection factors in genesis of chronic gastroduodenitis was evaluated.

Endoscopical and histological features of oesophagogastroduodenal zone, parameters of pH-metry and electrogastroenterography, qualitative and quantitative characteristics of microbiocenosis were studied in 80 female persons with postcholecystectomy syndrome more then a year after cholecystectomy. In the presence of duodenogastral reflux the most natural is the combination of distal oesophagitis, antral atrophic gastritis and duodenitis, accompanied with low level of gastric acidity, gastric hypokinesis and duodenal dyskinesis, dysbacteriosis of mucosal microflora with its quantitative increase and appearance of bacteria with expressed pathogenicity non-typical for this biotope. These data should be taken into consideration for determination of pre- and postoperative treatment tactics for patients with gallstones.

There is a rise in the rates of acute erosive gastropathies in patients with cardiovascular and other somatic diseases. The role of acute erosive gastropathy-induced hemorrhages in the tanatogenesis of these diseases is underestimated; the problems of their prevention, diagnosis, and treatment remained unsolved. Many factors, mainly acute or chronic ischemia of the gastroduodenal mucosa, as well as its age-related involution, Helicobacter pylori infection, reflux gastritis, multiple organ dysfunction, drug-induced damage, etc., which are implicated in the pathogenesis of acute erosive gastropathies in patients with cardiovascular and other somatic diseases, are summarized.

A total of 4684 autopsies made at Moscow multidisciplinary hospitals in 2002-2008 were studied to reveal the incidence and morphological features of acute gastroduodenal erosions and ulcers in coronary heart disease (CHD), cerebrovascular disease (CVD), and chronic obstructive pulmonary disease (COPD). Cases with combined, background, and concomitant diseases that could be independent causes of gastroduodenal lesions were excluded. Patients older than 60 years with infarctions of the myocardium or brain, a concurrence of CHD, CVD, and COPD, and various diseases with chronic heart failure syndrome, particularly in the presence of arterial hypertension and diabetes mellitus should be referred to as a risk group for acute gastroduodenal erosions and ulcers and their induced hemorrhages.

The results of investigation of 49 children suffering from chronic gastroduodenitis and atopic dermatitis are presented. All the child underwent serological blood analyses for Epsteine-Barr virus antibodies and fibrogastroduodenoscopy with antral and duodenal mucosal biopsy. Hystomorphological and immunohystochemical studies of mucosal biopsy specimens for Epsteine-Barr virus were carried out. The role of Epsteine-Barr virus in genesis of chronic gastroduodenitis is detected.

Compatibility of such signs, as not differentiated connective tissue dysplasia and chronic gastroduodenitis is considered. 170 patients of school age with the confirmed diagnosis chronic gastroduodenitis are surveyed with the purpose of revealing signs of connective tissue dysplasia. It is shown, that symptoms of connective tissue dysplasia meet more often at children with chronic gastroduodenitis, than at children without inflammatory gastroduodenal disease. Following features of connective tissue dysplasia are most typical for children with gastroduodenal diseases: external (wrong bearing, flat-foot, a thin skin and others) and internal (a vegetative dysfunction, a biliary dyskinesia and others). Those features have the high informative importance for prognosis of inflammatory gastroduodenal disease in children.

The current study was designed to test the role of bacterial overgrowth in the small intestine in the pathogenesis of chronic pancreatitis. It was established that the bacterial overgrowth in the small intestine is noted in the majority of patients with chronic pancreatitis. We found an association between clinical manifestations of chronic pancreatitis, active duodenitis, duodenal lymphangiectasia and papillitis and bacterial overgrowth in the small intestine.

The aim of the present study was to determine the frequency and the character of duodenal lesions in patients with chronic viral hepatitis B (CVH-B), to elucidate the dependence of these parameters on the activity and duration of hepatitis, virulence, and portal blood flow. A total of 206 patients with CVH-B were examined. It was found that CVH-B was associated with the development of chronic duodenitis in 23.3% of the patients. Endoscopic studies revealed different degree of affection of duodenal mucosa. Superficial focal and moderately severe duodenitis occurred more frequently (in 33.3 and 37.5% of the patients) than manifest duodenitis (12.5%). Erosive duodenitis concomitant with Helicobacter pylori infection was diagnosed in 16.7% of the patients. The main factors contributing to the development of chronic duodenitis included viral infection, its intensity, duration of hepatitis, and disturbed portal circulation. Severity of duodenitis was closely related to the severity of CVH-B.

The article analyzes the influence of course treatment with vegetomodulating preparations on morphofunctional changes in the gastric and duodenal mucosa. Thirty-eight patients with chronic gastroduodenitis dwelling in ecologically unfavorable territories were examined and treated. The study demonstrated positive changes in the histomorphological picture of the gastroduodenal mucosa in children who received conventional standard therapy in combination with the beta-adrenoblocker anaprilin and the M-cholinolytic atropine compared to controls who received only the basic therapy.

Three methods of ozone therapy (intravenous injection of ozonized physiologic saline, oral intake of ozonized low-mineral water and combination of the two treatments) were compared by efficacy in 215 patients with gastroduodenal pathology associated with Helicobacter pilori. All the techniques of ozone therapy proved effective in relation to regress of clinical symptoms, regeneration processes, eradication of Helicobacter pylori, but the combined regimen was the best.

A hundred and nine patients aged 9-16 years who had chronic gastroduodenitis (CGD) were examined. All the children had an exacerbation of gastroduodenitis. In patients with erosive and superficial gastroduodenitis, the gastric juice levels of primary, secondary, and intermediate lipid peroxidation (LPO) products were higher than those in controls and in patients with other endoscopic forms. The increased acid-secreting function causes a rise of gastric juice LPO products. Helicobacter pylori persistence leads to the increased accumulation of LPO products in the gastric juice. The pathogenetic significance of impaired LPO in the development of CGD is discussed.

The aim of the research was to evaluate peculiarities of ulcerous and erosive lesions of gastroduodenal zone (UELGDZ) in patients with arterial hypertension (AH). The authors studied clinical and endoscopic parameters in 442 patients with UELGDZ and AH, and 160 patients without AH (control group). The feature of the AH group was more frequent HP-invasion, aggravation of ulcerous process, development of complicated and atypical forms of the disease. In the patients with AH the clinical manifestations of chronic erosive gastritis were more diverse and severe; unfavorable clinical course prevailed. Risk of cardiovascular complications varied from low to very high, correlating with the severety of AH and UELGDZ.

The purpose of the study was to define duodenal epithelial function in duodenal diseases and to assess the potentialities of correction of detected dysfunctions. The electrical properties (EPs) of the duodenal epithelium were explored in patients with primary chronic duodenitis, duodenal peptic ulcer (DPU) on an exacerbation and in the late postoperative period after selective proximal vagotomy. Mucosal biopsy specimens were obtained at fibrogastroduodenoscopy with biopsy forceps and placed in a sign-alternating field with the parameters, adequate bioelectric epitheliocytic characteristics: 28 V at a voltage sign change rate of 1 Hz, and a current strength of 200 microA. Bioelectrical cell reactions (nuclear fluctuation amplitude) were recorded by light microscopy using a MOB-1A ocular micrometer at minutes 10, 30, and 60 of the onset of electrical field action. A significant reduction was found in the amplitude of duodenal epithelial nuclear fluctuations in the examinees, which was most pronounced in DPU on an exacerbation. Duodenal epithelial EPs were shown to undergo regular changes during therapy and may serve as an additional test in evaluating the efficiency of therapy.

The electrokinetic properties of erythrocytes of the duodenal and buccal epitheliums were studied in patients with chronic duodenitis and with duodenal ulcer at exacerbation in the remote period after selective proximal vagotomy; the diagnostic value of the studied parameters was demonstrated, the presence of correlation bonds between them was shown.

Primary chronic gastroduodenitis (PCGD) accounts for 60-85% of the diseases of the gastroduodenal zone. In our study 90 patients with PCGD were divided into three groups getting one of the following therapies: electric sleep, hofitol, electric sleep plus hofitol. The effects of the treatments were assessed with updated techniques including computed pH-metry. Hofitol showed a good effect on dyspepsia, enhanced the alkalizing ability of the duodenal bulb. Electric sleep relieved pain and asthenoneurotic syndromes, decreased high acidity of the gastric juice in the body of the stomach. Electric sleep in combination with hofitol normalized macroscopic picture of the upper gastrointestinal tract and corrected imbalanced immunity.

Examining the gastroduodenal mucosa (GDM) in 61 patients with chronic obstructive pulmonary diseases (COPD) showed changes in the areas studied in all cases. GDM atrophy increased with the severity of COPD. Helicobacter pylori are essential in developing gastric pathology in patients with mild bronchopulmonary disease. The severity of atrophic gastritis increases with the longer duration of COPD and with more severity of hypoxia and bronchial obstruction.

Endoscopic and morphological examinations of the upper gastrointestinal tract (GIT) were made in 80 patients with nonspecific ulcerative colitis (NUC). Tests for Helicobacter pylori infection and gastric juice acidity were also conducted. It was found that in many patients NUC was associated with development of nonspecific esophagitis, gastritis and duodenitis. The endoscopic examination revealed erosions of the abdominal esophagus in 13.8% cases, erosive-ulcerative alterations in the stomach and duodenum in 27.5% of NUC patients. The most marked changes occurred in patients with severe NUC. Alterations in the upper GIT were not related with either Hp or the acidity. Morphological examinations of the biopsies from the esophagus, stomach and duodenum demonstrated changes similar to those in biopsy material from the colon. The same type of morphological changes in mucosa from the upper GIT and the colon in NUC indicates that their pathogenesis is also the same.

The paper considers the principles of classification of chronic duodenitis. The current classification of chronic duodenitis may be divided into three parts: endoscopic, histological, and etiological. The characterizing terms of the endoscopic part are as follows: erythema, hemorrhages, atrophy, erosions, follicles. The endoscopic localization of duodenitis is as follows: proximal and distal portions (panduodenitis, including bulbitis in the proximal portion and postbulbitis in the distal portion, palpilitis). The nonspecific terms of the histological part are as follows: activity, inflammation, and atrophy. Its specific terms are: ventricular metaplasia, ventricular heterotopia, Helicobacter pylori, granulomas, lymphatic follicles, eosinophilic infiltration, lymphocytic infiltration. The following morphological categories of duodenitis are: nonatrophic, nodular, granulomatous, eosinophilic, and lymphocytic. The morphological localization of duodenitis is as follows: proximal and distal portions (panduodenitis, including bulbitis in the proximal portion and postbulbitis in the distal portion). The following degrees of its severity are: none changes, mild, moderate, and sever. The types (etiological factors) of duodenitis are as follows: acid-associated, drug-dependent, toxic (eliminating), and isolated nonspecific (idiopathic). Its special forms are: H. pylori-induced, granulomatous (Crohn's disease, sarcoidosis), nodal (Crohn's disease, sarcoidosis, acid-associated diseases), eosinophilic (unspecified allergens), lymphocytic (gluten-induced).

Three groups of children aged 4 to 14 years who complained of having dyspepsia were examined. Group 1 included 31 children without tuberculous infection; Group 2 comprised 30 newly tuberculosis-infected children; Group 3 consisted of 35 children who had an over one-year history of the infection. Esophagoduodenoscopy and biopsy indicated that the incidence of Helicobacter infection and gastritis was higher in children with tuberculous infection than in those without it and it was directly related to the duration of infection. The specific features of chronic gastritis in the infected children were the totality of gastric mucous lesion with more profound antral changes, the predominance of progressive gastritis, as appeared as neutrophilic infiltration, gastric mucous contamination with Helicobacter and Candida, and metaplasia.

Aim: Morphological study of gastric and duodenal mucosa in patients with vibration disease., Materials and Methods: 64 patients with vibration disease stage I and II were examined endoscopically, histologically, bacterioscopically and morphometrically irrespective of the presence or absence of gastrointestinal complaints., Results: Erosive gastroduodenitis, non-erosive gastroduodenitis and gastritis, polyps in the stomach and gastric ulcer were diagnosed in 19(29.7%), 40(62.5%), 2(3.2%) and 1(1.6%) patients, respectively. Small intestinal metaplasia occurred in 22% of patients. Helicobacter pylori occurred most frequently in the antral part of the stomach (90.24%). Advanced dissemination often combined with metaplasia of the small bowel. Morphometrically, there were slight differences from relevant values in patients with chronic gastroduodenitis who were not exposed to occupational hazards. Vibration disease patients exhibited impairment of duodenal vessels (large endothelial nuclei, thick arteriolar walls) 2 times more frequently than controls., Conclusion: In vibration disease, gastric and duodenal mucosa, as a rule, shows signs of chronic gastroduodenitis.

62 Chernobyl wreckers with erosive gastroduodenitis (50 cases) and ulcer (12 cases) were admitted to hospital where they received standard drugs and local He-Ne laser treatment of erosions and ulcer located in the antral stomach and duodenal bulb. Histological, bacterioscopic, morphometric and immunohistochemical examinations have identified chronic inflammation and disturbance of local immunity. After treatment epithelialization of erosions was not accompanied by normalization of morphometric and immunohistochemical parameters. This suggests a high risk of recurrences.

The paper summarizes the results of clinical observations and special examinations conducted in 45 children with chronic gastroduodenitis by using combined exposure to sinusoidal modulated currents and ultrasound. The dynamics of clinical symptoms, functional status of the stomach and its adjacent gastroduodenal organs, endoscopic findings, morphological status of the gastroduodenal mucosa, gastric mucosal dissemination of Helicobacter pylori, systemic and regional humoral immunological parameters, abdominal echography indicate high therapeutical efficiency of the combined therapy as compared to that performed in controls (without physiotherapy). Mechanisms of formation of therapeutical action are clarified, differentiated indications are defined.

Investigations designed to study gastric mucosa (GM) in patients with chronic primary gastroduodenitis with the aid of electron microscopy showed the abnormal changes in the cellular, subcellular membranes and capillaries to be of the same type as they are in peptic ulcer (PU) though poorly manifest. Changes in GM under chronic atrophic gastroduodenitis (ChGD) suggested involutive changes while in chronic secondary gastroduodenitis (ChSGD) it was generally not different from the mucosa ultrastructure in healthy subjects except for changes in surface epithelium. Based on the histoenzymologic studies it has been ascertained that in ChPGD there occurs a rise in lysosomal enzymes similar to that encountered in PU, reflecting the intensity of catabolic processes, decrease in the activity of enzymes of the capillary wall and mitochondria respiratory chain against the background of compensatory enhancement of the activity of the glycolytic enzyme lactate dehydrogenase. In ChAGD the activity of all the enzymes was low, while in ChSGD it did not differ significantly from that in healthy subjects.

Biopsies of stomach and duodenum different regions mucosa obtained from patients with gastroduodenitis (liquidators of consequences of accident in Chernobyl nuclear station--LCA CNS-- and control patients) were studied cultured in vitro. The following are the differences found. Mononuclear cells, differentiating into macrophages were exposed from control biopsies. Mononuclear cells exposed from LCA CNS biopsies formed rosette-like accumulations after mitotic dividing on a distance from the explant. Part of cells within the rosettes acquired processes and formed syncytial structure.

Morphological electron-microscopic evaluation of gastroduodenal mucosa in subjects who worked in the zone of radionuclide contamination after the Chernobyl accident in 1986-1987 and therefore exposed to low-dose ionizing radiation (18, 62 sGy on the average) discovered deficiency of tissue immunocompetent cells, dystrophic changes in the epithelium and stroma. In epithelial cells there were structural shifts in microvillae, organoids and nuclei. Epithelium, stroma and endothelium of blood vessels contained dark cells with thick hyaloplasm.

Morphologic investigation of 72 gastroduodenal biopsy samples from 29 children with chronic persistent and chronic active hepatitis, hepatic cirrhosis, revealed signs of chronic gastroduodenitis. The degree of the gastroduodenal zone lesion was directly related to severity of the primary condition. Comparative data on histopathological changes in the stomach and duodenum in concurrent pathologies suggest that chronic duodenitis may be the primary condition.

Mucous membrane of the antral region of the stomach and that of duodenum were studied in 36 teenagers, 21 of them had erosions revealed endoscopically and histologically. Morphometric indices of the inflammation were more pronounced when erosions were present; the latter did not influence the local production of IgA, IgM and IgG. The content of IgG was particularly high. Contamination with Helicobacter pylori was found in all cases and was more pronounced in erosive gastroduodenitis. High frequency of lymphoid follicles formation was found, particularly in the antrum.

A new system for the detection of Helicobacter pylori DNA, based on the method of directed amplification, has been developed. Primers for specific detection of H. pylori were selected from a nucleotide sequence of 16 S-p RNA. The sequences of the primers had a few nucleotide substitutions as compared with the sequences of closely related microorganisms. An essential condition for the attainment of reaction specificity was the rise of annealing step temperature to 66 degrees C. Sensitivity of the system was in the range of 3 to 30 fg of DNA, or 20 to 100 bacterial cells. Using the proposed system for the detection of H. pylori DNA clinical specimens (stomach biopsy sample, gastric juice and wash-offs of oral cavity), obtained from 49 patients with antral gastritis, were analyzed. The method of H. pylori detection in clinical specimens using polymerase chain reaction (PCR) turned out to be more sensitive compared with microbiological tests. By application of PCR H. pylori DNA was detected in subgingival pockets.

The evidence obtained on 280 ulcer patients, 107 of whom have preulcer erosive gastritis, duodenitis or gastroduodenitis, suggest the conclusion on ulcerogenesis dependence on the type and site of erosions, response of blood germs, characteristics of protein, carbohydrate and electrolyte metabolism. Erosive processes in the gastroduodenal mucosa seem a facultative preulcer state.

A total of 53 patients with dermatomyositis aged 4-14 with the disease duration of three months to eight years were under observation. Twenty seven patients complained of stomach-ache, II felt pain during palpation, 30 lost appetite. Electrogastrography revealed disorders in bioelectric activity of the stomach muscles in 99.9% of the patients. Endoscopic studies revealed lesions in the stomach and duodenum in all the patients, in 39 of them gastroduodenitis in the stage of exacerbation was diagnosed, in 7 erosion, in 3 peptic and duodenal ulcer. In 81.3% of the children during clinico-laboratory remission of dermatomyositis gastroduodenitis persisted at the stage of exacerbation. The vessels lying in the mucosa of the antral area of the stomach and duodenum underwent peculiar changes: arterial walls looked thickened, nuclei of the endothelial cells were enlarged, rounded up. One could observe infiltrates around the vessels consisting mainly of lymphocytes. Morphometric indices of the mucosa were also different in the antral area of the stomach and duodenum. Apparently, changes revealed in the gastric and duodenal mucosa of children with dermatomyositis occur due to vascular processes developing as a result of the main disease. This should be taken into consideration in the treatment of such children.

Morphological data on the biopsies obtained from the antral gastric mucosa of 150 chronic gastritis and gastroduodenitis patients provided grounds for recognition of 3 degrees of gastritis activity. Helicobacter pylori (CP) occurred in the antral gastric portion in 100% of the cases. CP presence and mucosa contamination, plasmocytic infiltration, dystrophic changes in the surface and glandular epithelium were found to correlate. Administration of ampiox in a daily dose of 1.0 g for 10 days resulted in a rapid attenuation of painful and dyspepsia syndromes which ceased on treatment day 7. Endoscopically and morphologically, there were more rapid epithelialization of the erosions and inhibited activity of gastritis compared to the control group. In biopsies obtained on treatment week 3-4 CP was not registered in 67% of cases or was present in much less amounts. The control subjects manifested CP universally, the morphological positive trend was unremarkable.

Clinico-morphological study of the gastroduodenal system and also morphometric investigations of the degree of gastroduodenitis activity and the condition of the local immune system were made in 30 children aged from 4 to 15 years with juvenile rheumatoid arthritis (JRA). Microbiological, histological and electron microscopic methods for revealing Campylobacter pylori (CP) were used. Interrelationship between various clinical manifestations of JRA with the nature of gastroduodenal system affection and with the incidence of microbial colonization of SP was determined. All the patients with JRA had lesions in the gastric and duodenal mucosa, and some morphological features were revealed in them. SP was diagnosed in 86.7 per cent. There was a relation between the microbial clonization of SP and the degree of the rheumatoid process activity. It is suggested that SP does not play an etiological role in the development of gastroduodenitis in JRA but may serve as a pathogenetic factor in the development of an erosive-ulcerous lesion.

Histological, morphometric, and immunohistochemical studies of biopsy specimens from the mucosa of the body and antral portion of the stomach, the bulb and descending portion of the duodenum were performed in 27 children with systemic lupus erythematosis (SLE) and in 12 with chronic gastroduodenitis. The inflammatory infiltrate from SLE patients was found to contain higher levels of young and mature fibroblasts than that from patients with chronic gastroduodenitis, mature fibroblasts being prevalent. The inflammatory changes in the gastric and duodenal mucosa were ascertained to be associated with the progression of SLE. In exacerbation of SLE, the walls of vessels (arterioles) exhibited immune complexes classified mainly as IgG and, to a lesser degree, as IgM. In remission, the luminescence of the vessels decreased. The serum level of immunoglobins did not correlate with their regional production in the gastric and duodenal mucosa.

The authors describe lesions of the upper alimentary tract in 91 children aged 4 to 15 years suffering from pyelonephritis. Intragastric pH-metry, endoscopy and histological examination of biopsy specimens from the gastric and duodenal mucosa were carried out, as a result of which gastroduodenal pathology was diagnosed in 3/4 of the patients. In 69.4% of the children, biopsy specimens of the mucous membrane of the antrum of the stomach showed Campylobacter pylori. Such combined pathology requires, in addition to the treatment of pyelonephritis correction of the diet, administration of agents that may contribute to the decrease of upper alimentary tract inflammation. If Campylobacter pylori are detected, the treatment eliminating those bacteria is to be administered.

The clinical, x-ray, endoscopic and morphological studies performed in 156 children afflicted with gastroduodenitis have shown that concomitant lesions of the colon are of paramount importance in the genesis of this illness. The immunomorphological assay is the most informative one in the assessment of the pattern of structural alterations in the colonic mucosa. Examination of 71 colonic biopsy specimens has disclosed that 38 patients had minimal alterations in the mucosa characteristic of colodyskinesias and 22 had the signs of chronic inflammation, which plays an important part in goal-oriented therapy and prediction of the disease under consideration.

Gastric biopsies from 90 children (from 5- to 15-year-old) with chronic gastroduodenitis were examined for Campylobacter pylori (CP) by bacteriological, histological (Giemsa stain, argentation by Warthin-Starry, acridine orange stain) and electron microscopical methods. CP is detected in 81.2% children. The degree of colonization was low or moderate. The increase of occurrence was noted with age and duration of disease. Persistence of CP went down with the increase of atrophic change in the gastric mucosa and duodenum, but there was no clear-cut correlation with the morphological variant of gastritis and the stage of disease. Colonization with CP is regarded as an important part in the pathogenesis of chronic gastroduodenitis in children.
