This study is a continuation of (Report I) identification of the occupational risk of arterial hypertension (AH) in 13 occupational groups (3842 workers, men). In previous work there was eliminated the influence of traditional factors of the cardiovascular risk, in this study there was implemented the identification of the components of a healthy worker effect (HWE) and the elimination of their influence on the occupational risks of hypertension. Identification and removal of components HWE--the effect of a healthy recruitment (EHR) and the effect of the healthy worker persisting to work (EHWPW--was carried out by the analytic rearranging of the standardized for age and obesity prevalence rate of arterial hypertension with the use of own methodological approaches. For the determination of the presence and severity of EHR there was performed an analysis of the initial prevalence rate of arterial hypertension in the youngest age groups (under 31 years). To overcome HER standardized for age and obesity indices of the arterial hypertension prevalence rate were adjusted by the ratio of the frequency of arterial hypertension in the most young occupational and reference comparable groups. Identification of HWPW was executed by comparing the frequency of AH among workers retiring within 3 years from the occupational groups when compared to the whole sample. Then on the additional risk value there was adjusted the overall prevalence rate of AH in the occupation profession to overcome EHWPW. As a result of the consistent correction and elimination of the influence of HWE components on the prevalence rate of AH, there were obtained risks values, primarily reflecting the impact of occupational factors which can be considered as true occupational risks. Factors of the cardiovascular risk and HWE significantly modified true occupational risks for AH in a number of occupational groups up to inversion. At the same time, the pronouncement of EHR has a paramount importance in the modifying effect.

Reliability of mine foremen influences greatly the group microclimate, productivity and safety of work in mines. Engineering and technical personnel with high level of professional reliability shows the best parameters of psychophysiologic status. Criteria are worked out to score the most informative psychophysiologic functions in mine foremen during the preliminary and regular check-ups of such personnel.

Lipid and antioxidant state of clinically normal subjects working in deep mines was studied under natural conditions during one month. Disorders in lipid metabolism and intensified peroxidation were recorded in these subjects. A prophylactic ration has been recommended considering the food status, alimentary risk factors and extreme conditions of miners' labour.

Pulsimetry proved to be the most acceptable method for estimating energy requirements of miners working under extreme conditions with constantly changing ergometric parameters. It has been established that the mean daily energy requirements of miners working in deep coal mines under conditions of high temperature comprise 3500 kcal. Energy requirements for the main types of the physical activity of workers in deep mines have been calculated, basing on basal metabolism, metabolic constants for varying activities have been determined.

Based on the original analysis of the data pertaining to labour routes and the peculiarities of job-placement in mines, the article summaries the hygienic principles for job-placement of coal-miners and provides classification criteria for labour routes in mines. An expert analysis is provided of 146 professions and occupations in coal-mining for workers with different health states and labour ability conditions.

200 newly-engaged young miners were medically examined in an outpatient department. The routine examination techniques were supplemented with studies of the changes after 2 minute-long inhalations of 0.6% acetylcholine chloride and mine dust (78 mg/m3), hemodynamics and electrocardiography indices, as well as the indices of the immunologic and hormonal state. Established was diversified manifestations of the respiratory tract's response to the aerogenic irritations, combined with deviations in the immunologic state indices. The studies provide basis for further improvements in the preventive examination techniques for workers in coal-mining industries.

Vegetative equilibrium disorders in hypertensive-diseased miners resulted from pathologic developments in the limbic -reticular structures. Those were characterized by a hyperfunction of the ergotropic nerve formations. The vegetative control reorientations were accompanied by narrowing the amplitude of the homeostatic regulation vegetative reactions and engaging the mechanisms of adaptation. The vegetative state stabilization in the diseased workers was attained through a partial avoiding ot the most labour-intensive forms of activity and choosing more preferable labour operations. But the latter did not entirely exclude the labour conditions which would result into inadequate vegetative reactions.

The article contains data on the results of the on-the-spot studies of coal mine controllers' labour conditions and relating hygienic factors. It was established that the work at the dispatcher's control point in coal mines was considerably affected by unfavourable factors (low degree of illumination, constructional shortcomings of the control point and seat) with concomitant neuropsychic stress conditions. Revealed were specific functional shifts in CVS and CNS, neuromuscular disorders and the analyzers' malfunctioning. A set of measures was proposed for labour conditions improvement, ergometric perfection of the working place and reduction of neuroemotional tension.

The contributors proposed a differentiated diagnostic technique which included 10 most informative physiologic, anthropometric and biochemical+ indices to provide the most reliable data (95% reliability) on the adaptation and compensatory mechanisms under local vibration and in combination with noise and dust, and with due account of the individual sensitivity to local vibration. Testing of the technique demonstrated its high accuracy in determining the levels of "exhaustion", and of "the normal state" of the adaptation-compensatory mechanisms, and its applicability for vibration disease prognosis in individual++ workers, thus establishing corresponding in preventive medical examinations, occupational selection, preventive measures' planning, and in expert solutions requiring precise diagnostic techniques.

The article deals with the methological issues of preventive measures in the process of technological restructuring of mines in the Russian Federation, with due account of the hygienic stratification of different mines as to the degrees of the labour conditions' potential hazardness, regularities in and prognostics of occupational pathologies decreases and morbidity with temporary disability rates.

The article proposes results of research into exhausts' major components absorption in accumulated and floating dusts in ore mines in which self-propelled diesel vehicles are predominantly used. It was established that ore-mine aerosol particles had a large specific surface and contained a high percentage of carbon-black (1,1-35.1%). Different results were received for both organic and non-organic substances' sorption indices. In the samples, in addition to 3,4-benzapyrene, 11 more polycyclic aromatic hydrocarbons were identified, with concentrations 10-15 times higher than those of 3,4-benzapyrene.

Among the factors producing effects on the formation of indexes of morbidity of the people working in Donbass, the main role is played by the ecological situation in the region, working conditions and lifestyle of the working people. On the basis of morbidity indexes analysis proposals and measures aimed at the preservation and improvement of health of the working people are formulated.

The pattern of forming of the adaptational process to occupational-industrial factors of working in a coal mine depending on the conditions of carrying out practical work was determined. It was found out that the adaptation of young boys to the working activity in deep horizons was accompanied by a more marked tension of the functional status and regulation mechanisms compared to working in less deep horizons.

Coal miners' intensive labour with concomitant unfavourable working conditions have changed the occupational diseases' structure. Effective antidust measures were used which contributed to a lower dust-related morbidity, but the number of cases of locomotor system and vibration related diseases increased. In recent years, a tendency has become apparent towards a general decrease of occupational traumatism, particularly, accidents with severe and lethal traumatism.

A hygienic assessment of the working conditions at the open-cast mines of KATEK and YuYaUK showed that the conditions were better at KATEK than at the Neryungrin mine, due to milder climate, non-transport system of stripping, wheel-type trenching, and railway transport in the mining areas. No cases of occupational disease were registered. The working conditions and the character of work may influence the general morbidity, in particular the development of cardiovascular disease. The efficacy of health promotion actions is analysed and recommendations on further improvement of the working conditions are presented.

Clinical and functional examination of the miners who suffered from coronary heart disease showed a higher ectopic activity of the myocardium during their work under underground conditions and during the subsequent 4 hours after its termination. It also indicated a significantly higher fatal cases of sudden coronary death in the miners in the aforementioned periods. To prevent the development of complications occurred in miners, the status of the cardiovascular system was proposed to be dynamically controlled.

The results of the technological studies and chamber experiment provided the basis for the conception according to which noise information parameters contributed to its biologic activity identically with energetic characteristics. The calculation results obtained by the regression model showed that noise MAC with account of its information value should be at least 5-10 dBA less than the existing ones. Information parameters should also be regulated.

During the next 20 years normalization of occupational factors at Donbas mines should be ensured by technical reconstruction of mines on the basis of the development of new technological circuits, automatization and mechanization means, the means of collective protection, physiologic labour rationalization.
