This article is devoted to how the first Soviet constitutions were perceived by first-wave Russian émigrés. Despite the relatively large number of lawyers among them, their analysis of the constitutions was not based on the theory of law. Instead, émigrés measured them against their compliance with the historical development of Russia as well as their compatibility with other laws and by-laws. Russians abroad were primarily concerned about the constitutions' implementation. Any study of this topic will therefore discuss not only these Russians' perception of the documents, but also of the Soviet state and its policies in the 1920s. Especially interesting are the arguments about the role of the Soviets and the Soviet system. This paper examines and compares views held by representatives of different political circles, identifying similarities and differences. Most of the émigrés' analyses of the Soviet system in the 1920s resulted in the following assessments: formal legislation was not being observed in practice; management was being gradually centralized; a transition was taking place away from the dictatorship of the proletariat and towards the dictatorship of the party, the oligarchy, and the autocracy of Stalin; the functions of the Soviet organs were being duplicated and replaced by the Communist Party; and bureaucracy was burgeoning. The paper discusses the peculiarities of the English loanword loser, which, according to the author, has become one of the key concepts in the usage of contemporary society. The paper focuses on the usage of this word in different languages, and to be more precise, in the practice of different cultures while placing considerable emphasis on the Russian discourse. Also, general cultural reasons for the word being so popular across different cultures (the feeling of the world crisis, the prevalence of virtual models of behaviour, life in "the society of performance") and an attempt to distinguish different modifications of the notion in the Russian language are presented in the paper. It is claimed that in the Russian language the word loser and other concepts connected with it are a construct of the fight among different life ideologies, whereas due to the "appeal to moral evaluations" (Wierzbicka) this word becomes an element of moral discourse. Having become the description of all possible life problems, the word loser becomes an instrument for people to regularly reflect on their "life project". [ABSTRACT FROM AUTHOR]

In modern literary criticism, the concept and so-called genre 'migration literature' is commonly associated with the experience of exile, often for political reasons; by contrast, writers who have left their country for reasons other than political are labelled as 'migrant-writers', 'writers abroad', or 'diaspora writers'. The use of such a different terminology to categorise authors and their writings highlight the fact that there are some distinctive characteristics distinguishing them. While I do share this perspective to a certain degree, I also would like to draw attention to a major literary trend of the last two decades: the appearance of writers who expatriate voluntarily without being persecuting politically but yet are in a situation which I define as 'self-exile' or 'voluntary exile'. Despite their different languages and countries of origin and residence, an analysis of their texts demonstrates that these authors are united by two common features: 1) a reflection on the tragic past of their compatriots, who have experienced forced mass emigration, and an attempt to find echoes of this experience in everyday life; and 2) an awareness of their own position (that is, the situation of self-exile) as a productive process and creative basis for their writing work. The hypothesis I suggest in this paper is that the texts written by 'writers who are in self-exile' are characterised by certain writing strategies and themes typical of migrant writers yet they also have some unique features, which are related to the voluntary experience of leaving their home country. Home is to be understood broadly, not in terms of a certain geopolitical location, but as belonging to one single culture, community, and language. The aim of this article is to examine these very features. This article focuses on the novels by two famous contemporary authors: Aleksej Makušinskij (Russia/Germany) and Winfried Sebald (Germany/United Kingdom). In their works, the representation of the condition of self-exile has led the authors to develop a multilingual discourse and recreate the new transitory literary world. [ABSTRACT FROM AUTHOR]

This volume contains papers submitted to the 12th Annual International Conference of the Bulgarian Comparative Education Society (BCES), held in Sofia and Nessebar, Bulgaria, in June 2014, and papers submitted to the 2nd International Partner Conference, organized by the International Research Centre 'Scientific Cooperation,' Rostov-on-Don, Russia. The volume also includes papers submitted to the International Symposium on Comparative Sciences, organized by the Bulgarian Comparative Education Society in Sofia, in October 2013. 11. RUSSIAN LITANIES: TYPES AND CHARACTERISTICS OF THE CENRE.

The article aims to distinguish linguistic and pragmalinguistic characteristics of the Russian speech genre of litany which was first described by the American sociologist and anthropologist Rice. The paper focuses on illocutionary properties of litany and aims at identifying the basic pragmatic types of the speech genre in Russian fiction and memoirs. Taking into account illocutionary criteria, four main types of the genre were distinguished: a litany-complaint (prototypical type of litany which is based on the idea that speakers give an account of their misfortunes and expect sympathy and understanding of the addressee); a litany-self-justification (it is realized in the situations when speakers misbehave, and wishing to reduce their guilt, they refer to objective external factors which supposedly influenced their behaviour); a litany-accusation (it is realized when the addressee is guilty of the speakers' misfortunes at least partly, and it gives a possibility for the speaker to combine complaints with different types of accusations ranging from reproaches to imprecations); and litany-bragging (it combines both complaints about life and realization of positive strategy of self-presentation). Apart from that, the article embraces such features of litanies as relative contextual autonomy (i.e. tenuous connection with the previous context in the discourses of monologue and dialogue), the prevalence of modalities of obligation and necessity (it is demonstrated that deontic and alethic modal meanings prevail in the genre of litany, whereas the alethic modality might be obscured by the speaker and manifest as deontic), specific exploitation of death simulacres as well as basic types of the primary axiological opposition models 'good-bad' (it includes such versions as "I/we feel bad - you feel good", "I/we feel bad - they/others feel good", "it was good before - now it is bad"). The article concludes that the litany is a communicative phenomenon reflecting cultural aspects and it stresses the necessity of further research into the specifics of the speech genre of litany (or its analogues) in other communicative cultures. The aim of this article is to trace the origins of Russophobic stereotypes and myths in Japan and to analyze their features and traits. In the article the authors scrutinize writings by Japanese politician and public figure Uchida Ryohei (1874--1937) which are dedicated to the Russian Empire and the "Manchurian question." Uchida's writings were found in the collections of the Russian State Military Historical Archive and the National Diet Library of Japan. The hypothesis of the article is that Uchida Ryohei was one of the main ideologists and creators of Russophobic stereotypes and myths in the Japanese Empire. It was around Uchida Ryohei that proponents of anti-Russian views and people who were impatient to start the war against the Russian Empire united. The article goes on to demonstrate that Uchida Ryohei's writings and political activities played a key role in the initiation of the Russo-Japanese war (1904--1905) and in shaping a negative image of Russia in Japanese public opinion at the beginning of the 20th century. What's more, Uchida's cultural heritage is still in demand in modern Japan. [ABSTRACT FROM AUTHOR]

Political linguistics attracts attention of many researchers, including exponents of cognitive approach, to discourse analysis in linguistics. Within the framework of this approach, metaphorical representation of agents of political communication, namely of countries, political institutions and politicians, is absorbedly studied. At this, political and media discourses created by politicians, journalists, reporters, speechwriters, etc. are put to analysis. We believe that one more agent of political communication is still beyond the scope of cognitive research. That is people, SOCIETY. Many aspects of political communication depend on society -- character and quality of addressant-adressee interaction, effect that power bodies and mass media have on public, public reaction and judgement of this or that social phenomenon, possible direction of this reaction, etc. DISCOURSE OF PUBLIC OPINION is a unique discourse unit, absorbing traits of institutional, media and popular discourse - it occurs as a reaction of projection of institutional and media discourse, and exists and flourishes in popular discourse. Therefore, communication within discourse of public opinion is quite specific and has its own characteristics that are in focus of this research. DISCOURSE OF PUBLIC OPINION represents attitudinal interpretation of a socio-political event and/or its participants, opinion exchange, participation in solution of significant problems of the state and society, influence on conception and stereotypes of communicators, formation or specification of their world outlook. PUBLIC OPINION is often represented in comments to some articles in mass media, in chats, blogs, talk shows, surveys and other interactive kinds of communication. This communication usually abounds in conceptually marked linguistic units. In this paper, we consider main theoretical issues of PUBLIC OPINION DISCOURSE and present results of our cognitive analysis of the public opinion discourse in the situation of the Russian-Georgian military conflict and the information war of 2008 found in comments to corresponding articles on websites of the central British media. Namely, this article considers conceptual metaphors presenting the image of the Georgian President Mikhail Saakashvili, so rich in attitudinal interpretation of international society that was so preoccupied by his personality and actions. Our research began with trying to understand why it was that the thought and methods of Freudian psychoanalysis spread so widely and quickly in Russia. We hypothesized that it was partly due to the contiguity between the language of Freudian psychoanalysis and that of literary studies. To test this hypothesis, we analyzed several studies of literary criticism with a psychoanalytical slant, which well demonstrated this contiguity, thus validating our hypothesis. Furthermore, during our research, we discovered that the Russian school of literary studies, starting from the beginning of the 20th century, had discovered the analogies existing between the language of the unconscious and the language of poetry – an insight that anticipated the later developments in literary studies in the West. The article concentrates on the characteristic of the historiography of EU - Russia relations in international security issues during 1992-2010. The author observes the transformation of the international security concept and highlights that after the Cold War the broadening of the traditional view on this matter has become the widelyspread tendency in modern science. Ukrainian and foreign security studies have been reviewed according to this approach. The author points out that there is a great amount of works, which deal with this problem. The results of researches on different aspects of EU - Russia relations in the security sphere, such as the dialogue about the border and illegally immigration questions, the struggle against international terrorism, drug and human trafficking beings, the ecological cooperation and interaction in outer space are summarized. The article stresses that the majority of works devoted to the Russia - EU relations in international security touches upon the law side or one party's activity, although its interaction is of our scientific interest. The main tendencies and peculiarities of the Ukrainian, Russian, European and American historiography are analyzed. The author finds out the uninvestigated issues, which open wide perspectives for researchers in their future studies of EU - Russia relations in the sphere of international security. - Abstract
Leonid Vasilyevich Sobolev is an outstanding pathologist, whose name definitely should occupy an honorable place in the galaxy of great scientists of Russia. However, his name was undeservedly forgotten, and the role of his work was underestimated. This Russian scientist made the most important discovery: he proved in an experiment that it is the islets of Langerhans in the pancreas that secrete a humoral regulator, "factor X", the deficiency of which leads to diabetes mellitus. This mysterious islet factor will be isolated from the pancreas of dogs in 1921 by the future Nobel laureate Frederick G. Banting and will become part of the medicine as insulin. However, there is every reason to believe that the discovery of F. G. Banting, who repeated a series of experiments by Leonid Vasilyevich Sobolev, is, in fact, secondary, which is the research subject in this paper.

- Abstract
Background: Primary hyperparathyroidism (PHPT) is an endocrine disorder characterized by excessive secretion of parathyroid hormone (PTH) with upper-normal or elevated blood calcium levels due to primary thyroid gland pathology. PHPT is a rare pathology in children, with a prevalence of 2-5:100,000 children according to the literature. Due to the non-specificity of clinical manifestations at onset (nausea, vomiting, abdominal pain, emotional lability), the disease may remain undiagnosed for a long time., Aim: To study the features of the course and molecular genetic basis of primary hyperparathyroidism in children., Materials and Methods: Retrospective observational study of 49 patients diagnosed with primary hyperparathyroidism. All patients underwent a comprehensive laboratory-instrumental and molecular genetic study at the Institute of Pediatric Endocrinology, Endocrinology Research Center of Russia in the period 2014-2022., Results: The first clinical symptoms of PHPT were noted at the age of 13.8 years [10.6; 1 5.2], among which fatigue, headaches, dyspepsia, lower limb pain, and fractures were the most common. The age of diagnosis was 15.81 years [13.1; 16.8], all children were found to have high levels of PTH, total and ionized calcium, with hypophosphatemia in 93.9% of patients (n=46) and hypercalciuria in 43% (n=21). Five out of 49 patients (10.2%) were found to have ectopy of the thyroid: 3 showed an intrathyroidal location, 2 in the mediastinal region. Molecular genetic study revealed mutations in 32.7% of patients (n=16, CI (21; 47)), mutations in MEN1 being the most frequent (n=11). Pathogenic variants in CDC73 were detected in 3 patients, RET - in 2. Among the operated 39 patients, adenoma of the thyroid was detected in 84.6% of cases (n=33), hyperplasia in 7.7% (n=3), atypical adenoma in 5.1% (n=2), carcinoma in 5.1% of cases (n=2)., Conclusion: The paper presents the peculiarities of the course and the results of molecular genetic study of pediatric PHPT. This sample is the largest among those published in the Russian Federation.

The purpose of the comments is the analysis of Professor Dmitry D. Pletnev's article Hypertension (experience in analyzing its genesis) from the point of view of modern ideas about arterial hypertension. Currently, a small number of publications of Professor D.D. Pletnev, a Russian scientist and clinician, unjustly convicted in 1937, have been preserved. The small work includes not only a deep and critical analysis of the pathogenetic mechanisms of this disease, but also an assessment of diagnostic methods, as well as a detailed description of the clinical picture. The paper, published in 1935, remains relevant and of interest to clinicians and medical historians.

- Abstract
Objective: This paper presents an analysis of polymorphic gene loci, the products of which are directly involved in the molecular mechanisms of regulation of neurophysiological processes., Material and Methods: The sample of subjects consisted of 128 unrelated males and females living in the European part of Russia. The study assessed the frequency of occurrence of 11 single-nucleotide substitutions located in genes encoding serotonin receptors, ciliary neurotrophic factor, uncoupling protein 2, methylenetetrahydrofolate reductase, methionine synthase, methionine synthase reductase, dipeptidyl carboxypeptidase 1, gamma-coactivator of the receptor activated by the proliferator peroxisome and neurotrophic factor of the brain. Genotyping of the samples was carried out by PCR with fluorescence detection and analysis of the polymorphism of the lengths of restriction fragments., Results: The distribution of polymorphism genotypes corresponded to the Hardy-Weinberg equilibrium with the exception of rs1801133 MTHFR (χ
2 =5.3088, p =0.0212) in which a decrease in heterozygosity was observed. These studies of minor allele distribution have no statistically significant deviations from the European population, but there are deviations from the Asian, African, and Latin American populations., Conclusion: Statistically significant correlations of allele frequencies in the study group with populations from other regions and the studies conducted in them are the basis for the inclusion of selected single-nucleotide polymorphisms in the list of a limited set of molecular genetic markers, which makes it possible to supplement the system of mental health monitoring and improve the professional training of people in extreme professions.- Published

- Abstract
This paper explores the potential of recent research on metacognition to offer new avenues to assess and address the phenomenon of fragmentation in schizophrenia, which was described by E.Bleuler as «splitting». The concepts of metacognition characterize and quantify alterations or decrements in the processes by which fragments or pieces of information are integrated into a coherent sense of self and others. A method for assessing metacognition is presented along with research examining the presence and importance of metacognitive deficits in schizophrenia. Greater levels of metacognitive deficits have been detected in different phases of schizophrenia and linked to poorer psychosocial outcomes. These data were obtained both in foreign and preliminary Russian studies. The authors suggest that treatments, which successfully target metacognitive capacity, may uniquely promote wellness and recovery in schizophrenia.

- Abstract
This paper discusses modern approaches to the diagnosis and management of patients with piriformis syndrome. Epidemiological data on the prevalence of this syndrome in neurological practice are presented. The anatomical features of this region, leading to the formation of piriformis syndrome, are described in detail. The authors provide diagnostic criteria based on neurological examination and manual muscle testing and discuss the differential diagnosis of piriformis syndrome. New possibilities of treating this syndrome using a fixed combination of diclofenac and orphenadrine registered in the Russian Federation as a drug for intravenous infusion neodolpasse are discussed in detail. The authors cite materials from their own work, a clinical case of managing a patient with piriformis syndrome using neodolpasse is analyzed.

- Abstract
Aim: Systematization of data on antibiotic resistance of Helicobacter pylori in Russia over the past 10 years., Methods: A search for studies were carried out in electronic databases MEDLINE/PubMed, EMBASE, Cochrane, Russian Science Citation Index. We analyzed the papers published over the past 10 years (2011 to 2020). In the final analysis, publications were selected from periodical peer-reviewed foreign and Russian publications, in which validated tests for assessing the microorganism resistance were used with detailed descriptive statistics, which allows the obtained data to be included in the meta-analysis., Results: The final analysis included 11 studies (808 isolates). H. pylori resistance to clarithromycin was 10.39% (95% confidence interval CI 7.10314.219), metronidazole 33.95% (95% CI 15.32955.639), amoxicillin 1.35% (95% CI 0.281 3.202), levofloxacin 20.0% (95% CI 12.63728.574), tetracycline 0.98% (95% CI 0.3532.163). Double clarithromycin-metronidazole resistance was reported in 2.7% (95% CI 1.1364.345)., Conclusion: This meta-analysis is the first analytical work that allows objectifying the current structure of H. pylori antibiotic resistance in the Russian Federation. The data obtained in the analysis of publications over the past 10 years indicate a low resistance of the microorganism to clarithromycin (less than the regulated threshold of 15%), which allows us to consider the triple regimen of eradication therapy as a first-line therapy in our country.

30. [Advantages and limitations of initial combination therapy in pulmonary arterial hypertension patients in Russia].

- Abstract
Pulmonary arterial hypertension (PAH) is severe and often times rapidly progressive disease with fatal outcome. The concept of initial combination of PAH-specific therapies in high risk patients at baseline was first described in the European guidelines on pulmonary hypertension (PH) in 2009, and in low or intermediate risk patients at baseline in 2015. Interestingly, that in Cologne Experts Consensus, and then in the 6th World Symposium on PH medical community started considering initial combination therapy as one of the most important pillars in PAH treatment algorithms in 2018. As of August 2020, as many as 8 formulations of 7 reference PAH-specific drugs are licensed for medical use in the Russian Federation. On top of that, 6 abbreviated drugs (generics) have also become available few years ago. Unfortunately, intravenous and subcutaneous prostacyclin analogs (PCA) and tadalafil are not approved for PH patients treatment in the Russian Federation. In this narrative review paper we attempted to describe studies on initial dual combination therapy with PAH-specific drugs registered in Russia, i.e. ambrisentan and riociguat, macitentan and riociguat, macitentan and sildenafil in low or intermediate risk patients at baseline, as well as iloprost inhaled and sildenafil, iloprost inhaled and bosentan in high risk patients. Some beneficial pharmacological effects due to the synergy between ambrisentan plus riociguat, and inhaled iloprost plus sildenafil appear to be interesting and require further clinical confirmation. Other initial combinations of PAH-specific agents require large-scale clinical trials as well.

- Abstract
This manuscript summarizes consensus reached by the International Anorectal Physiology Working Group (IAPWG) for the performance, terminology used, and interpretation of anorectal function testing including anorectal manometry (focused on high-resolution manometry), the rectal sensory test, and the balloon expulsion test. Based on these measurements, a classification system for disorders of anorectal function is proposed. Aim to provide information about methods of diagnosis and new classification of functional anorectal disorders to a wide range of specialists general practitioners, therapists, gastroenterologists, coloproctologists all who face the manifestations of these diseases in everyday practice and determine the diagnostic and therapeutic algorithm. Current paper provides agreed statements of IAPWG Consensus and comments (in italics) of Russian experts on real-world practice, mainly on methodology of examination. These comments in no way intended to detract from the provisions agreed by the international group of experts. We hope that these comments will help to improve the quality of examination based on the systematization of local experience with the use of the methods discussed and the results obtained. Key recommendations: the International Anorectal Physiology Working Group protocol for the performance of anorectal function testing recommends a standardized sequence of maneuvers to test rectoanal reflexes, anal tone and contractility, rectoanal coordination, and rectal sensation. Major findings not seen in healthy controls defined by the classification are as follows: rectoanal areflexia, anal hypotension and hypocontractility, rectal hyposensitivity, and hypersensitivity. Minor and inconclusive findings that can be present in health and require additional information prior to diagnosis include anal hypertension and dyssynergia.

Gastroesophageal reflux disease (GERD) is a common chronic disease of the upper gastrointestinal tract with long course of the disease and followed by different symptoms significantly reducing quality of life (QoL) in pts. Assessment of QoL in pts with GERD may be of value for comprehensive evaluation of treatment effect as well as for monitoring of pts during treatment course in a real clinical practice., Aim: Development of the Russian version of GERD-HRQL questionnaire to assess symptomatic outcomes of GERD in research and real clinical practice., Materials and Methods: GERD-HRQL questionnaire (V. Velanovich, USA) consists of 11 items: 10 items for assessment of the most frequent symptoms/problems related with QoL in pts with GERD and 1 item for assessment of patient-reported global satisfaction with health condition., Results: In accordance with international guidelines, the new language version of the tool may be used in research and clinical practice after cross cultural adaptation, linguistic validation and psychometric testing. This paper presents the results of cross-cultural adaptation and linguistic validation of the Russian version of GERD-HRQL. The following steps of linguistic and cultural adaptation of GERD-HRQL for Russia were conducted after the permission from the author of GERD-HRQL was obtained: forward translation with creation of two forward translations of GERD-HRQL in Russian, reconciliation and expert evaluation of translations of GERD-HRQL in Russian and creation of the preliminary version of GERD-HRQL in Russian, back translation, harmonization, creation of the first test-version of GERD-HRQL in Russian, cognitive debriefing and decentering, creation of the second test-version of GERD-HRQL in Russian, final expert evaluation and development of the final test-version of GERD-HRQL in Russian. Satisfactory face validity of the Russian test-version of GERD-HRQL was shown: face validity indices were 0.98, 0.95, 0.92, and 0.97., Conclusion: As the result of translation, cross cultural adaptation and linguistic validation, the Russian version of GERD-HRQL for the use in pts with GERD in Russia was developed. The Russian version of GERD-HRQL may be used in research studies and clinical practice after testing its psychometric properties.

Depression is one of the leading causes of decreased quality of life and social functioning of patients. In the context of preventive medicine, the prevention of depression becomes a priority. To achieve the goals of prevention, it is necessary to identify specific population risk groups - individuals with a high genetic risk of depression. The paper describes the project aimed at developing a genetic test system based on polygenic risk scores (PRS) for depression, considering the multi-ethnicity and multicultural diversity of the Russian population. As a result of the study, data on the genetic architecture of depression (GWAS) and PRS for depression will be obtained for the first time. The emergence of a genetic test system developed in the study of the Russian population and in the conditions of a constant decrease in the cost of genetic research will allow an effective transition to preventive medicine in the area of mental health.

Objective: To develop an electronic registry of patients with MS in the Russian Federation and to analyze the collected epidemiological and clinical data., Materials and Methods: Patients aged 18 and older with definite multiple sclerosis were included in the study. We designed a specialized electronic individual registration form (registry) to gather information about study participants. This paper presents data collected in the project between September 25, 2017 and March 25, 2020., Results: We collected information on 2728 patients. This paper presents the results of data analysis from the registry of MS patients, including demographic, clinical and epidemiological characteristics, the type and timing of treatment received. Based on the data obtained, certain issues with primary diagnosis of multiple sclerosis and delayed initiation of disease-modifying therapy can be highlighted, which contribute to the transition of the disease to a secondary progressive course and disability increase.

- Abstract
The paper describes the results of 15 consecutive Russian clinical trials in the treatment of different types of acute leukemias, conducted by Russian cooperative group within the last 27-years and included more than 25 hundred patients. It was shown that the 5-years overall survival in AML younger than 60 years patients improved from 20 to 33%, in ALL - from 38 to 65%, in APL - from 0 to 95%. The cooperative work resulted in balanced clinical recommendations and protocols, reproducible in regional hospitals. Though till now there is a big difference in the long term outcome in acute leukemias patients treated in coordinating or regional centers mostly due to much higher early death rate: 7.5-9.5% vs 18-25%. This parameter should be used as criteria of the total efficacy of hematological service in each region of Russia. It became possible within the last years to integrate the minimal residual disease monitoring into clinical trials thus providing a clue parameter for choosing the further therapy - allogeneic stem cell transplantation. Non - chemotherapeutic approach with arsenic trioxide and all - trans retinoid acid had dramatically changed the toxicity and duration of treatment with very high efficacy. The Russian cooperative group was the first that started to include pregnant women with acute leukemias into clinical trials. This prospective and prolonged experience has shown that pregnancy in acute myeloid leukemia is an extremely poor prognostic factor, but it does not influence the outcome in acute lymphoblastic leukemia.

The authors discuss the classification and etiopathogenesis of spinal muscular atrophy, prospects for its pathogenetic therapy and prevention in Russia.

- Abstract
Prostate cancer (PC) is one of the most important problems in modern oncology. According to statistical data, PC ranks second in the cancer morbidity structure in the Russian Federation and developed countries and its prevalence has been progressively increasing over the past decade. A need for early diagnosis and maximally accurate morphological verification of the diagnosis in difficult clinical cases (inconvenient tumor location for standard transrectal biopsy; gland scarring changes concurrent with prostatitis and hemorrhage; threshold values of prostate-specific antigen with unclear changes in its doubling per unit time; suspicion of biochemical recurrence or clinical tumor progression after special treatment) leads to revised diagnostic algorithms and clinically introduced new high-tech invasive diagnostic methods. This paper gives the first analysis of literature data on Russian practice using one of the new methods to verify prostate cancer (transrectal prostate cancer under magnetic resonance imaging (MRI) guidance). The have sought the 1995-2015 data in the MEDLINE and Pubmed.

- Abstract
Spasticity treatment is one of the key aspects of the contemporary cerebral palsy (CP) rehabilitation that influences on the effectiveness of other methods. The paper presents the first Russian document that unites the recommendations for the BTA treatment of CP and could be used as the guideline for the multilevel injections. The Russian consensus on the multilevel botulinum toxin A (BTA) treatment of spastic CP is based on the international data and the results of national studies. The authors describe typical CP spasticity patterns in the upper and lower extremities, give recommended intervals for the BTA (Abobotulinum toxin A) dosages for the whole injection procedure and for the separate muscles. The method of dosage calculation for functional segments is also described. Attention is paid to the frequency, optimal intervals between the repeated injections and the whole duration of BTA treatment. The authors discuss effectiveness and safety of BTA, factors that potentially influence the results of the injections, including ultrasound and electromyography control, and indications for the continuation and termination of treatment.

The paper gives historical information on the foundation and establishment of the Russian X-Radiology Research Center, as well as the advances that have been fundamental in the further clinical, scientific, and educational activities of the center. It details information of the recent advances of the center in diagnosis, including radiation, clinical and molecular gene, and combined treatments for cancer patients, including surgery, polychemotherapy, and radiotherapy. The achievements of each subdivisions of the Center are given. The paper shows the latest advances of the Center and its workers in researches, which have been embodied in their dissertations, monographs, and guides. Of great importance is the educational process the Center's workers are successfully doing in, by training specialists of different profile.

- Abstract
The complex of such phenomena as anxiety and fear in children of the early age and their relationship with risk factors for psychic pathology were investigated. Eighty cases of anxiety-phobic disorders in children of the first five years of life were studied. The types of behavioral and somatic reactions that allowed to reveal not only the clinically expressed phenomena of anxiety and fear but the higher readiness to them were described. The first anxiety-phobic reactions appeared at the age when emotional functions were not completely formed and might be considered as the presentations of emotional dysontogenesis. The authors assume that characteristics of fear expression in the early age allow to suspect a mental disease which might be timely diagnosed in case of its manifestation. The conclusions made in the paper may be useful for clinical practice of pediatricians, children neurologists, psychologists and psychiatrists.

The paper is devoted to investigation of possibilities of infection prophylactics in performing surgical intervention. The necessity of organization of the system of epidemiological observations of the outcomes of surgical interventions in treatment-and-prophylactic institution was demonstrated. The results of epidemiological observations and microbiological monitoring performed at the hospitals of St. Petersburg showed using epidemiological diagnostics to be necessary for adequate administration of antibacterial medicines to patients.

- Abstract
An analysis of the chapter "Medical help for the substance abuses, clinic, treatment of the abusers, biological studies of the abuses' in the Proceedings of the last two Russian congresses of psychiatrists (XIII in 2000 and XIV in 2005) reveals the vast multiplicity of topics. The majority of topics featured in the XIII Congress does not provide any follow-up information in the materials of the XIV Congress thus not allowing to figure out the tendencies or consequences in the field of addiction studies. There is no uniformity in the vocabulary used in the chapter analyzed. The dominant topics such as alcoholism and opiate abuse make up a small part of the total amount of papers.
