Medical studies have shown that the organs of one donor can save / help 50 people. Organs that can be transplanted are the heart, kidneys, liver, lungs, pancreas, intestine, skin, bone and cornea. The donation of human organs, cells or tissues can be made from a living person or a deceased person, both cases require the express, free and prior consent of the donor or his heirs and essentially donation must be altruistic, free. Globally, organ donation by a deceased donor still stirring ethical and medical controversy, among others, because there is no European legislation for the diagnosis of brain death, there is a discrepancy manifest between the visions of various countries on the protocol declaration of brain death or clinical and biological death in the end. We performed a study of MTWA in post-myocardial infarction patients to clarify the predictive accuracy and usefulness of MTWA compared to other invasive and non-invasive techniques. We enrolled 120 patients (74 men, 46 women, mean age 62.3 ± 15.2 years in men, and 64.2 ± 13.8 years in women) with a history of myocardial infarction but no prior sustained ventricular arrhythmias. Patients were assessed by ECG, echocardiography, Holter, signal averaged ECG, MTWA, and electrophysiology study. Mean follow-up was 14 months. The MTWA test had a good negative predictive value for arrhythmic events in post-MI patients and can be used for risk stratification. Objectives. To evaluate the left ventricular diastolic function in patients with cirrhosis and to establish its relation to the degree of severity of liver disease as well as to biological markers of cardiac dysfunction. Methods. We included 72 patients diagnosed with liver cirrhosis in different degrees of disease progression. Cardiac parameters were evaluated by two-dimensional echocardiography and color Doppler. Results. In terms of diastolic echocardiographic function, 26 patients had normal diastolic function, 36 had grade 1 diastolic dysfunction, 8 had grade 2 dysfunction, and 2 patients had grade 3. Diastolic dysfunction was unrelated to age, gender and etiology of cirrhosis. Of all the echocardiographic parameters, left ventricular hypertrophy, right cavity dimensions, E wave deceleration time, left atrial volume were significantly higher in patients with diastolic dysfunction, correlated with the severity of cirrhosis. Significantly elevated NT-proBNP and TnI were seen among the patients with diastolic dysfunction. Conclusions. The present study shows that although diastolic dysfunction is common in cirrhosis, it is usually incipient and most often correlates with the severity of the liver disease (Child-Pugh class).
