Correct diagnosis of vertical root fractures is in most cases difficult because of lack of specific symptoms and signs, and/or radiological conclusive aspects; therefore, the differential diagnosis with other pathological entities can be challenging. The purpose of this study is to evaluate, clinically and radiologically, a number of 35 teeth with vertical root fracture. For early identification of vertical root fracture, should be carefully combined the reason of patient presentation, clinical examination and rigorous interpretation of radiographs (periapical, bitewing and CBCT if necesary). Diagnosis of vertical root fracture is based on some subjective parameters; is important to recognize at an early stage vertical root fracture and to inform the patient, to recive adequate information about proper treatment. Vertical root fracture can sometimes be difficult to distinguish from incorrect endodontic treatments or some periapical pathology; vertical root fracture is the main cause of extraction of endodontic treated teeth.
