Objectives. This paper describes and analyzes embedded implementation research and the empirical processes of planning for utilization, strategies to promote utilization and the sustainability of utilization of results from research led by decision-makers in Latin America and the Caribbean. Methods. This qualitative, descriptive and interpretive study is based on the findings from semistructured interviews with members of teams working under the Embedding Research for the Sustainable Development Goals initiative (2018–2019) as well as their responses to a self-assessment follow-up questionnaire 1 year after the project was completed. Results. Altogether 13 teams from 11 countries participated in the Initiative. Nine teams had a core team composed of a decision-maker as the principal investigator assisted by a researcher as co-principal investigator. Four teams included more than one co-principal investigator; and in five teams, the originally assigned principal investigator was replaced. There was an interesting relationship between the expected utilization of research results, the utilization strategies, the sustainability of research uptake and the teams’ collaboration modalities. When decision-makers and co-principal investigators were active participants, the intention to use the results and strategies for utilization were clearly oriented to improve implementation. In teams with basically a formal collaboration between the two principals, plans for utilization were unclear or focused on producing academic knowledge. The participation of implementers below the rank of principal investigator decision-maker may be relevant. Conclusions. Embedded implementation research is an innovative tool that may foster the utilization of research and strengthen health programs and services. Considering the internal dynamics of such research teams will enhance planning and strategies for research utilization as well as the sustainability of practical and actionable findings.

Objectives. To (i) ascertain stakeholders’ perceptions of the contextual factors and resources necessary to successfully implement the AMORE platform, a tool that provides accessibility assessments for health care services, considering factors such travel time and traffic conditions, and (ii) identify potential barriers to and facilitators for enhancing spatial accessibility to health care services within the Colombian urban context. Methods. In this qualitative study, semi-structured interviews were conducted with a purposive sample of seven key stakeholders. The sample was drawn from individuals involved in development of policies in Colombia, service providers, and users, among others, who had expertise in the field. Interviews were conducted until saturation was reached. Results. The participants had positive views on the appearance of the AMORE platform, highlighting its user-friendly visualization. Suggestions were made about the variables used in the dashboard, the implementation of the platform, potential usage areas, and barriers and facilitators to implementation and use. Barriers included economic, political, and personnel challenges, while facilitators included creating a minimum viable product at a low cost and building interinstitutional and international cooperation. Conclusions. Innovations such as the AMORE platform have the potential to support decision-making processes across various sectors, including public policies and internal processes within private organizations, academia, and the community. However, implementing such a tool has financial, contextual and environmental challenges. The study identified key factors that were considered prerequisites for successfully implementing the AMORE platform in Colombian cities.

Abstract Objective: to map the scientific literature on the validity of instruments for evaluating the quality of services provided in primary health care for chronic diseases related to systemic arterial hypertension, diabetes mellitus, leprosy and tuberculosis. Method: scoping review, following the Joanna Briggs Institute method and described in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. 13 databases and gray literature were included. The selection of studies was carried out after removing duplicates and individual and paired evaluation. The data was extracted based on an elaborate script and presented in tables and charts. Results: the analysis of 28 selected studies showed that the majority were from Brazil, followed by China and Malaysia. Almost half of the validated instruments were generic, and the specific ones covered the evaluation of diabetes mellitus and leprosy. The types of validation carried out were content and construct. Conclusion: there is a need to construct specific instruments due to the scarcity of studies on the process of validating instruments for evaluating the quality of services provided by primary health care for chronic diseases.

Palliative care is governed by the principles of reaffirming the importance of life. For children, adolescents and young adults, this comprehensive care provided by a multidisciplinary team has proven to be a challenge, especially with regard to home care. Therefore, the objective is to understand the supply of palliative care services in pediatric oncology and young adults, at home, as well as the coverage of care by health region in Santa Catarina. This is an ecological study, with secondary data from the Ambulatory Information System (SIA/SUS) and Hospital Information System (SIH/SUS), for the state of Santa Catarina, from 2018 to 2021. The records were investigated of care offered to the age groups from 0 to 29 years of age. To characterize palliative care, the name already presented in the literature was followed. The Greater Florianópolis Macroregion was the only one to present records of home hospitalization. Overall, there was a drop in attendance during the first year of the COVID-19 pandemic. It was identified that the distribution was not homogeneous in all macro-regions, but the disparity in the supply of records of procedures at home was notable, which leads us to reflect on whether the low number only reflects the little care provided, or the lack of records in systems linked to productivity in the SUS. Abstract Background In hospital settings, dermatology can offer substantial clinical support for the diagnosis and management of skin conditions, reducing morbidity and mortality. Thus, the study aimed to analyze the profile of referrals and consultations performed by the Dermatology Service of the Santa Casa de Misericordia de Porto Alegre, from August 2018 to January 2020. Methods This study is descriptive, quantitative, and retrospective, conducted through data collection and review of medical records and referrals. The variables included were clinical data of referrals, in-patients profiles, dermatological diagnoses, complementary exams, therapeutic conduct, and recommended follow-ups. Results A total of 1020 referrals were analyzed, which resulted in 641 consultations (328 men, 313 women). The most prevalent skin disease groups were ‘Dermatitis and Eczema’ (33.1%) and ‘Other infectious skin diseases (21.8%), while the most frequent ICD-10 were ‘Drug eruptions - L27’ (9.9%) and ‘Other and unspecified dermatitis - L30’ (6.6%). Corticoids were the most recommended treatments (27.7%), followed by antifungals (13.1%). ‘Consultation Discharge’ (44%) and ‘Outpatient’ Dermatology follow-up (27%) were the most frequent causes for ending consultation. Study limitations Among the study limitations, the authors highlight its retrospective nature, with data analysis based on referrals and medical records, which may present inaccurate or incomplete information. In addition to this, the study may demonstrate a certain degree of subjectivity due to the review and interpretation process conducted by the researchers. However, the definition of objective criteria based on previous studies attenuates such possible bias. Furthermore, considering that the Dermatology teams are composed of a preceptor dermatologist and residents, the established diagnoses were not submitted to third-party verification, except in the cases of skin biopsies and cultures. Thus, the professional’s experience and skills may have influenced the dermatological diagnoses. Conclusions These findings underlie the importance of Dermatology in hospital assistance, contributing to the management of a wide range of skin conditions.

This study evaluates the perception of caregivers of the quality of neuropediatric physiotherapy services offered at specialized rehabilitation centers (SRC) in Brazil. This is a cross-sectional, quantitative, evaluative study whose target population was caregivers of children in rehabilitation. The AQSF-Neuroped instrument was used to evaluate the quality of three SRC based on the perception of 320 caregivers. The one-proportion hypothesis test was used based on each AQSF-Neuroped item and dimension. The Weight of Evidence and Latent Class Analysis models were used to evaluate the influence of variables on the perception of quality. A mean overall quality score of 133±14 points indicated a positive perception of quality; however, there were aspects related to physical structure (Interpersonal Dimension) and access (Organizational Dimension II) that needed to be improved (p-values<0.05) to better meet the expectations of caregivers. By group formation, the LCA enabled identification of the caregivers' characteristics that determined either a positive or negative perspective of the quality. ABSTRACT Objectives: to improve an instrument that measures postpartum women’s satisfaction with obstetric care. Methods: action research, developed from a preliminary version of an instrument prepared by nurse-midwives working in public services in the Federal District. The analysis of the results of application of instrument carried out in a pilot test, analysis of evidence of instrument validity, literature review, focus group with the instrument’s developers and interview with the target audience were carried out. Results: factorial analysis showed three existing factors in the construct. Seven nurses participated, discussing the instrument reformulation, and 20 mothers reported their perceptions about the care received during childbirth, generating five thematic units. Final Considerations: instrument improvement occurred through item and response scale reconstruction and reorganization, in addition to application of a pre-test with the target population, resulting in an instrument composed of 13 items.

ABSTRACT OBJECTIVE To evaluate congenital syphilis prevention actions in primary health care services in the state of São Paulo. METHODS Cross-sectional evaluative research that used indicators extracted from the Survey of Evaluation and Monitoring of Primary Care Services ( Avaliação e Monitoramento de Serviços da Atenção Básica – QualiAB) in the state of São Paulo in 2017. An evaluative matrix composed of 31 indicators of prevention of congenital syphilis, categorized into four domains of analysis: diagnosis and treatment of acquired syphilis (10); basic infrastructure and resources (7); prevention of congenital syphilis during prenatal care (7); and educational actions and prevention of sexually transmitted infections (7). The frequency of services with positive responses for each indicator and the percentage of service performance were calculated based on the proportion of indicators reported per service and the overall average observed. Subsequently, services were classified into four quality groups, and associations between groups and each indicator, type of organizational arrangement and location were estimated. RESULTS 2,565 services participated, located in 503 municipalities, with an overall average performance of 74.9%. The domain “diagnosis and treatment of acquired syphilis” had the highest performance (89.8%), followed by “infrastructure and basic resources” (79.5%), “prevention of congenital syphilis in prenatal care” (73.3%) and “educational actions and prevention of sexually transmitted infections” (56.8%). There was a significant difference between quality groups and all indicators and types of organizational arrangements. CONCLUSIONS The evaluated services have limitations in the development of actions to prevent congenital syphilis, mainly related to health education and actions included in prenatal care, such as screening and adequate treatment of pregnant women and their partners. Changes are needed in the work process, with the expansion of educational and surveillance actions, as well as the qualification of the teams to effectively comply with the protocols.

ABSTRACT OBJECTIVE To build an integrated database of individual and service data from the cohort of people who started antiretroviral therapy (ART), from 2015 to 2018, in Brazil. METHODS Open cohort study that includes people aged 15 years or older who started ART from 2015 to 2018, with follow-up in services of the Brazilian Unified Health System (SUS), and who responded to the 2016/2017 Qualiaids national survey. The source of individual data was the related HIV database, derived from the probabilistic linkage between data from the SUS systems of diagnostic information, medication, tests, and deaths. The data source for the services was the services’ response database to the Qualiaids survey. After analysis of consistency and exclusions, the database of individuals was deterministically related to the database of services. RESULTS The cohort comprised 132,540 people monitored in 941 SUS services. Of these services, 59% are located in the Southeast region and 49% followed 51 to 500 cohort participants. The average performance of organization and management of patient care ranged from 29% to 75%. Most of the cohort participants are male, black and mixed, aged between 20 and 39 years old, and have between 4 and 11 years of schooling. Median baseline T-CD4 was 419 cells/mm3, 6% had an episode of tuberculosis, and 2% died of HIV disease. CONCLUSION For the first time in Brazil, this cohort provides the opportunity for a joint analysis of individual factors and services in the production of positive and negative clinical outcomes of HIV treatment.

Abstract Objectives: to evaluate coverage and identify factors associated with adequate prenatal care for adolescent mothers in Rio Grande, Southern Brazil, in a period of 13 years. Methods: over the years 2007, 2010, 2013, 2016 and 2019, a single standardized questionnaire was applied to all mothers at hospital level in this municipality. The outcome consisted of the proportion of puerperae women who started prenatal care in the first trimester and performed 6+ medical visits, completed 2+ HIV, syphilis, and urine tests. Chi-square test was used for proportions, Poisson regression in the multivariate analysis and the prevalence ratio (PR) as measure of effect. Results: among the 12,645 identified mothers, 2,184 (17.3%) were adolescents. Of this total, 35.2% (CI95%= 33.2%-37.2%) had received adequate prenatal care, ranging from 13.8% (10.8%-16.7%) in 2007 to 52,5% (46.8%-58.1%) in 2019. The adjusted analysis showed a higher PR for adequate prenatal care among those of older age and better schooling level, white/brown skin color, with a partner, never had an abortion, were primiparous and non-smokers. Conclusion: there was substantial improvement in coverage of adequate prenatal care over the period studied, but there is still a long way to go. The increase in this coverage depends on reaching adolescents who are younger, less educated, black skin color, living without a partner, multiparous and smokers.

Solving complex problems can be challenging as they often involve multiple layers of related issues and factors. Observational research is a helpful tool for understanding healthcare's complex and contextually dependent problems; however, observations can be time-consuming and resource-intensive, particularly when including the analysis process. As a result, researchers may utilize other qualitative methods, such as interviews or focus groups. However choosing a strategy different than observations could miss subtleties of care that happen in practice. It is easy to underestimate the value of data gathered through firsthand observations of patient-provider and team interactions. One solution to making observations a more convenient method in healthcare research is collaborating in the analysis process. Research collaboration involves establishing an interprofessional research team with diverse backgrounds and professional perspectives. In this way, the group comprises individuals from various roles and different professional backgrounds to ensure exhaustive findings and improve the reliability and accuracy of the results. The diversity in the team represents the intricate dynamics in the complex system of care. Although there are guidelines for collaborative analysis in a traditional ethnographic study, there must be more focus on healthcare research. This paper explains the concepts and features of collaborative analysis in interprofessional research. This approach offers a systematic way to construct a code book, which can produce comprehensive and valuable insights into the complex dynamic of care.

ABSTRACT The COVID-19 pandemic reinforced the need for global efforts to grant universal health coverage and access, which imposes management challenges for Primary Health Care (PHC). This study aimed to develop and apply an instrument to assess the PHC Units’ responsiveness to COVID-19, based on co-production efforts between university researchers and PHC technical teams. The instrument composed of two modules, included identification, operating hours, workforce, work process, structure, equipment, furniture, supplies, Personal Protection Equipment (PPE), Symptomatic Respiratory Patient (SRP) examinations and follow-up, information, surveillance, integration, communication, and management. All the 165 PHC Units in Brasília were invited to complete the instrument. Main results: there was physical structure adaptation (adequate configuration of waiting rooms, internal and external spaces allowing safe distance); provision of PPE and COVID-19 tests; active search for SRP/COVID-19 suspects by phone, mobile or home visits; monitoring flows of patient transfer and telehealth implementation. In conclusion, the PHC Units reorganized their services to meet the demands of the pandemic context. Providing information about structure and responsiveness of PHC Units may subside health systems for planning and decision-making at different levels of management, which is crucial to determine strategies to empower and reinforce PHC responsivity in situations of pandemics and other calamities.

ABSTRACT Objectives: to assess the operationalization of Primary Health Care in the COVID-19 pandemic, according to Primary Care Assessment Tool: PCATool-Brasil attributes, from physicians’ and nurses’ perspective. Methods: a cross-sectional study, carried out with 99 physicians and nurses from Basic Health Units in a state in northeastern Brazil, with the aid of the adapted instrument PCATool-Brasil. Results: Essential Score was classified as high performance (6.6) and General Score as low performance (6.5). First Contact Access, Care Integration, and Community Guidance scores were 6.6). Conclusions: the attributes of Primary Health Care, in general, showed values above or close to the cut-off point in the assessment. These data can support strategies for local and national managers to strengthen Primary Health Care in the COVID-19 pandemic and future public health emergencies.

ABSTRACT Objective: To report and discuss the experience of the Circle of Culture in a school space, with attention to the social identity of adolescents. Method: Action research, conducted under the assumptions of the Circle of Culture, from August to December 2019. Participants were 16 adolescents, enrolled in Elementary School, in a state public school, in the rural district of a city of São Paulo. Data collection took place through photographic records, participant observation and field diary. Results: The relations of friendship were the central agenda of the Circles of Culture when dialogues about their structuring and influence on the identity construction were developed. Conclusion: Circles of Culture mediated by health professionals in the school setting have the power to problematize the reality of each adolescent’s life and, simultaneously, dialogue about the common, an aspect that empowers identity projects.

Introduction: There are several barriers discouraging clinicians from undertaking research, including insufficient funding, lack of time, organizational issues and lack of support. The strengthening of research capacity is perceived from three levels: characteristics of the researcher, the environment, and organizational issues. To date, Portugal is lacking studies on this subject. The aim of this study was to identify the best practices to promote research in Portuguese Primary Health Care. Methods: We conducted a qualitative study using semi-structured interviews with family doctors with broadly recognized research work and other stakeholders. We selected a sample by convenience and snowball sampling. From a total of 14 doctors invited by email, 12 responded positively, and we subsequently included two other stakeholders. We conducted the interviews in digital or face-to-face formats. Two team members handled the coding of interviews independently. We kept all recordings and transcripts confidential, only accessible to researchers. Results: We identified 16 strategies: 1) increasing institutional support; 2) creating support structures; 3) redefining the residency program; 4) investing in research training; 5) redefining curriculum evaluation; 6) establishing dedicated time for research; 7) increasing funding; 8) improving access to research data; 9) being a research driver; 10) establishing a research culture; 11) working in collaboration; 12) creating formally organized research groups; 13) creating autonomous research centers; 14) improving the definition of the research subjects and study designs; 15) reviewing procedures for ethics’ committees; and 16) reviewing the current selection of articles for publication. Conclusion: Overall, a greater proportion of interviewees identified the following as the most relevant strategies for research promotion: institutional support, including technical and scientific support from public institutions, private entities and academic centers; the reorganization of working hours with protected time for research; increased funding directed towards research and breaking isolation in research, promoting teamwork with clinicians within the same area or from different professional backgrounds.

Objective: To evaluate the perception and self-confidence of Dental interns regarding endodontic treatment performed on patients. Material and Methods: A total of 111 dental interns from 5 different educational institutes of a state in South India participated in a questionnaire-based survey. The questionnaire consisted of 12 questions related to root canal procedures performed on patients. Analysis was done using SPSS version 17.0. Results: Sixty-five interns (59%) performed less than 10 root canal treatments on patients during the course of the internship. Twenty-eight interns (25% of the intern population) took up more than 10 root canal treatment cases, whereas only 18 interns (16%) did not perform the treatment on patients and answered the questionnaire based on their experience with extracted teeth model work. Only five interns out of 111 were ‘Very confident’ and 22 were ‘Confident’ in performing the root canal treatment. Conclusion: The present study indicates that the self-perceived confidence level among dental interns can affect their clinical performance. It also recommends additional hands-on workshops, problem-based learning and clinical guidance for the dental interns to become more confident with the root canal procedure.

Physiotherapy is a specialty that works together with the multidisciplinary team in the care of cancer patients, using various resources and techniques for this purpose. Objective Claify how physiotherapy has been working within the oncology setting, which therapeutic resources are used, in which phases of the treatment they act, and which complications of cancer are more common in the service of the city of Salvador/ Bahia. Materials and Method: A descriptive observational study. The population was all physical therapists who worked in two oncology reference centers in Salvador, Bahia, Brazil, and all the physiotherapists who worked in these units. A specific questionnaire was used for data collection. Results: Were considered eligible for the study 45 physiotherapists, 89% has some kind of specialization, but only 4% are specialized in oncology. The mostfrequent types of cancer includes breast, head and neck, and lung cancer. Being lymphedema and limitation of range of motion the impairments more cited by the physiotherapist. Conclusion: The provision of all these resources by the centers allows a suitable physiotherapeutic treatment for this population that has specific characteristics. This is an important result, since the two institutions offer the service through the SUS, thus offering quality care to the users, especially to the low-income people, who are usually excluded from the best quality services. Background: Hospitalizations due to primary care sensitive conditions (HPCSC) represent an important health indicator, which can guide the access and quality of health care in the context of primary care. Objective: To assess the prevalence and factors associated with HPCSC in children aged 0 to 10 years old, in northern Minas Gerais, comparing the indicator with a similar study carried out 10 years ago. Methods: Cross-sectional study with a random sample of selected pediatric hospitalizations from June/2016 and august/2017 was carried out. The variables associated with HPCSC were identified after multiple analysis, using Poisson Regression, with robust variance. Results: 376 hospitalizations were analyzed. The proportion of HPCSC was 29.0%, significantly lower than the previous study, with a proportion of 41.4%. The variables male gender (PR = 0.91; 95% CI: 0.86 - 0.97), low birth weight (PR = 0.91; 95% CI: 0.85 - 0.97) and maternal education below eight years (PR = 0.93; 95% CI: 0.88 - 0.98) were identified as protective factors. Conclusion: There was still a high prevalence for HPCSC, but with an important reduction in 10 years. Only sociodemographic variables were shown to be associated with HPCSC. [ABSTRACT FROM AUTHOR]

Background: Only a few studies have addressed the quality of oral health care. Objective: To assess the association of the structure and process conditions with oral health performance indicators in the Family Health Strategy of Recife. Method: A cross-sectional quantitative and evaluative study based on secondary data referring to 2014 from the Ministry of Health. The Chi-Square test and the Odds Ratio, with simple and multiple regression, were calculated considering a significance level of 5% and confidence interval of 95%. Results: Among the 112 oral health teams (OHT) assessed, there was an association of the OHT parity with the minimum Family Health Strategy teams and the survey of schoolchildren with oral health needs with the highest coverage value of the first dental programmatic appointment (OR = 4.30 = CI = 1.33-13.93, OR = 27.47, IC = 2.24-336.34, respectively). In turn, the presence of parity, modality II OHT, and scheduled guarantee return for the treatment continuity were associated with the highest ratio between completed treatment and first dental appointment (OR = 5.35, CI = 2.01-14.20; OR = 3.70, CI = 1,33-10,27; OR = 5,03, CI = 1,42-17,78, respectively). To present qualitative results from a group intervention aimed at improving education and interprofessional collaboration in multidisciplinary residents and medical residents, with emphasis on the context of hospital care. This is a qualitative study, in which nine residents participated. The intervention developed with the participants was divided into four meetings lasting two hours each and promoting activities aimed at fostering interprofessional competences. The meetings were audio recorded and later transcribed, and the transcripts were subjected to thematic content analysis. Two basic categories were configured, whose corresponding reports indicate that, according to the participants, the intervention was beneficial in the sense that, dialogically, it enabled them both to recognize obstacles to teamwork in the health field and to outline expedients capable of overcoming them. ABSTRACT Artificial intelligence develops rapidly and health is one of the areas where new technologies in this field are most promising. The use of artificial intelligence can modify the way health care and self-care are provided, besides influencing the organization of health systems. Therefore, the regulation of artificial intelligence in healthcare is an emerging and essential topic. Specific laws and regulations are being developed around the world. In Brazil, the starting point of this regulation is the Lei Geral de Proteção de Dados Pessoais (LGPD – General Personal Data Protection Law), which recognizes the right to explanation and review of automated decisions. Discussing the scope of this right is needed, considering the necessary instrumentalization of transparency in the use of artificial intelligence for health and the currently existing limits, such as the black-box system inherent to algorithms and the trade-off between explainability and accuracy of automated systems.

This article describes the main models for embedding research and the successful experiences and challenges faced in joint work by researchers and decisionmakers who participated in the Embedding Research for the Sustainable Development Goals (ER-SDG) initiative, and the experience of the Technical Support Center. In June 2018, funding was granted to 13 pre-selected research projects from 11 middle- and low-income countries in Latin America and the Caribbean (Argentina, Bolivia, Brazil, Colombia, Dominican Republic, Ecuador, Guatemala, Guyana, Haiti, Paraguay, and Peru). The projects focused on the system-, policy-, or programlevel changes required to improve health and build on the joint work of researchers and decisionmakers, with a view to bringing together evidence production and decision-making in health systems and services. The Technical Support Center supported and guided the production of quality results useful for decision-making. This experience confirmed the value of initiatives such as ER-SDG in consolidating bridges between research on the implementation of health policies, programs, and systems, and the officials responsible for operating health-related programs, services, and interventions. It highlighted the importance of both respecting and taking advantage of each context–and the specific arrangements and patterns in the relationships between researchers and decisionmakers–through incentives for embedded research.

Abstract Introduction: Multidisciplinary clinics are the best approach towards Chronic Kidney Disease (CKD) patients in pre-dialysis phases. The few studies regarding kidney transplant recipients (KTR) compare multidisciplinary and non-multidisciplinary clinics. Methods: In this study, we compared the quality of multidisciplinary CKD care between 101 KTR and 101 propensity score-matched non-transplant pre-dialysis patients (PDP). Prevalence of patients without specific treatment at any time and percent time without specific treatment for CKD complications were the main outcomes and patient and kidney function survival, glomerular filtration rate (GFR) decline, prevalence of CKD-related complications, and percent time within therapeutic goals were the exploratory ones. Results: Time within most goals was similar between the groups, except for diastolic blood pressure (83.4 vs. 77.3%, RR 0.92, CI 0.88-0.97, p = 0.002) and hypertriglyceridemia (67.7 vs. 58.2%, OR 0.85, CI 0.78-0.93, p < 0.001), better in non-transplant PDP, and for proteinuria (92.7 vs. 83.5%, RR 1.1, CI 1.05-1.16, p < 0.001), better in KTR. Patient survival and GFR decline were similar between the groups, although non-transplant PDP tended to progress earlier to dialysis (9.9% vs. 6.9%, HR 0.39, p = 0.07, CI 0.14-1.08). Discussion: The similar findings between non-transplant PDP and KTR suggests that good and comparable quality of multidisciplinary is a valid strategy for promoting optimal clinical management of CKD-related complications in KTR.

This study sought to analyze the coordination and services for the management of care for people with tuberculosis (TB) in Primary Health Care (PHC) in the city of Porto Velho-RO, from the perspective of health professionals. This is a descriptive, survey-type study, carried out cross-sectionally from a quantitative approach, with nurses, physicians, nursing technicians/assistants and community health workers, through interviews using the Primary Care Assessment Tool (PCATool) questionnaire), after meeting the ethical precepts, analyzing only the list of services and coordination dimensions, based on descriptive analysis. A total of 266 health professionals were interviewed, and based on the answers, an overall average regular score was obtained for the list of services (3.38), despite the provision of social incentives, directly observed treatment and groups of individuals with TB has been unsatisfactory. The coordination dimension was satisfactory (4.54), but it was identified that counter-referral was regular from the perspective of professionals. The understanding of the operational problems faced during the management of TB in the PHC of the city allows reorienting practices and offering actions and subsidies for the construction of a new paradigm of care for these individuals. Physiotherapy is a specialty that works together with the multidisciplinary team in the care of cancer patients, using various resources and techniques for this purpose. Objective Claify how physiotherapy has been working within the oncology setting, which therapeutic resources are used, in which phases of the treatment they act, and which complications of cancer are more common in the service of the city of Salvador/ Bahia. Materials and Method: A descriptive observational study. The population was all physical therapists who worked in two oncology reference centers in Salvador, Bahia, Brazil, and all the physiotherapists who worked in these units. A specific questionnaire was used for data collection. Results: Were considered eligible for the study 45 physiotherapists, 89% has some kind of specialization, but only 4% are specialized in oncology. The most frequent types of cancer includes breast, head and neck, and lung cancer. Being lymphedema and limitation of range of motion the impairments more cited by the physiotherapist. Conclusion: The provision of all these resources by the centers allows a suitable physiotherapeutic treatment for this population that has specific characteristics. This is an important result, since the two institutions offer the service through the SUS, thus offering quality care to the users, especially to the low-income people, who are usually excluded from the best quality services. Objective: to investigate the clinical and surgical profile of patients who performed keratoplasty in a university hospital. Methods: It is a cross-sectional, observational study. The surgical description from the medical records of transplanted patients in the years 2019 and 2020 was assessed to analyze social and clinical data. Data were processed using the Microsoft Office Excel 2017 program. Numerical variables were analyzed using descriptive statistics in the Bioestat program version 5.3. Descriptive information was analyzed qualitatively. Results: 167 procedures were performed in this period, with an average age of 45 years old, male individuals, recipients of tissue donated in Pará or sent from the State of Ceará. Transplants were mostly optic, elective, with an average corneal donor button of 8mm and recipient of 7.5mm. As for the indications, most were due to corneal dystrophies, followed by leukoma and perforation of the eyeball, with a predominance of the penetrating surgical technique, interrupted suture, and local anesthesia. Conclusion: The knowledge of these data is important to consider that the clinical and surgical profile of patients in the hospital studied is close to the national reality, especially in clinical indication. Introduction: Evidence shows most patients are not recognised by their attending healthcare professionals as having palliative needs. This feasibility study aimed to aid healthcare professionals identify hospital patients with palliative needs. Material and Methods: Mixed-methods, cross-sectional, observational study. The patient inclusion criteria comprised: age over 18 years old, being mentally capable to give consent judged as such by participating healthcare professionals, and if unable, having a legal substitute to consent, having a diagnosis of an incurable, potentially life-threatening illness. Field notes were taken for reflexive purposes. Outcome measures included: Integrated Palliative Care Outcome scale, surprise question, phase of illness, referral request status, The Eastern Cooperative Oncology Group Performance Status and social needs assessment. An interim data collection period meeting assessed implementation outcomes in each context. A web-based survey was sent to all participating healthcare professionals at the end of data collection period to explore overall experiences of participation and implementation outcomes. Results: Forty-two departments in four hospitals were contacted. The study was presented in nine departments. The field notes were vital to understand the recruitment process and difficulties experienced: time constraints, fear of additional work, department dynamics and organisation, relationships between departments and need of training in palliative care and research. One department agreed to participate. There were six participating healthcare professionals and only 45 patients included. Three participating healthcare professionals responded to the web-based survey. Conclusion: There is an urgent need to provide generalist palliative care training to clinicians.

Objective: To analyze the knowledge of professionals working in a Nursing Home about the Nursing Process before and after the awareness workshop., Methods: This is strategic action research, developed with nursing professionals and managers of a Nursing Home in Rio Grande do Sul, Brazil. Data were collected between January and June 2023, through semi-structured interviews before and after an awareness workshop. Discursive textual analysis of the data was carried out., Results: The central category "Understanding about the Nursing Process in Nursing Homes" emerged, which was unitized into two units of meaning and three categories of analysis., Conclusion: Data revealed non-use and lack of knowledge of the Nursing Process before awareness raising. Afterwards, a deeper understanding of the topic and its importance was identified. Awareness-raising workshops contribute to transformation of knowledge.

Objective: To evaluate output of endodontic in oral public healthcare by analyzing differences in instrumentation techniques, quantitative and qualitatively. Material and Methods: Endodontic services were differentiated by instrumentation techniques: hand techniques with stainless steel instruments (SS) or rotary using NiTi rotary instruments. Secondary data on the productivity of 2011 were obtained from the information system. Student’s t and Mann-Whitney tests were used. Qualitative data were based on semi-structured interviews, direct observation and field diary. Results: Service I employed SS hand techniques, service II used both techniques, while NiTi rotary system was used in service III. Objective: To evaluate the satisfaction of adolescents undergoing dental treatment between 2016 and 2018 at the Dental School of the Federal University of Minas Gerais (FAO UFMG). Material and Methods: A descriptive cross-sectional study using the Oral Health Services Satisfaction Assessment Questionnaire (QASSaB). The structured questionnaire has eleven questions divided among the dimensions: dentist / patient relationship, relationship with other professionals, effectiveness, accessibility/availability, technical-scientific quality, cleanliness and physical environment, acceptability, effectiveness/resolution, each with five response options. Descriptive data analysis was performed using absolute and relative frequencies. Results: A total of 145 adolescents participated in this study, 60 males and 85 females; 74.85% of adolescents rated dental treatment received as excellent, 76.7% reported having no symptoms after treatment, 46.2% considered it easy/very easy to get treatment, 65.6% reported that waiting time was short/very short, 90% considered dental equipment modern/very modern, 98% regarded the cleanliness of the waiting room and bathrooms as being excellent, 83.4% reported that they always received an explanation about the treatment, and 51.57% were totally satisfied with the appearance of the treated teeth and their chewing capacity. Conclusion: It was possible to observe that the adolescents, users of FAO UFMG, were satisfied with the service in the studied dimensions: human relations, effectiveness, accessibility/availability, technical-scientific quality, physical environment/cleanliness, acceptability and resolvability/effectiveness.

ABSTRACT Objective To evaluate the prevalence and factors associated with non-vaccination against influenza in the risk group. Methods A cross-sectional, population-based study, carried out in the city of Rio Grande (RS). The outcome was defined as belonging to risk groups and not having been vaccinated in the last 12 months. Demographic, socioeconomic, behavioral variables, and access for health services were analyzed. Results In this study, 680 individuals participated. The prevalence was 46.0% (95%CI: 41.8-50.3), ranging from 27.9% (elderly) to 81.8% (pregnant women). Young adults, single, intermediate socioeconomic bracket, smoker, with depressive symptoms, who did not perform physical activity and did not consult a physician in the last year, had a higher prevalence of non-vaccination. Conclusion Half of the sample was not vaccinated in the period. Due to the similarity of influenza-like illness and the coronavirus 2019 disease (COVID-19), increasing vaccination would minimize mortality and use of hospital beds due to influenza, optimizing the response of hospital capacity.

ABSTRACT Objective: To describe the population assisted in a genetics outpatient clinic, in a medium-sized town, with respect to diagnosis, type of inheritance, and local impact of genetic care. Methods: Medical records and genetic consultation forms from 2006 to 2018 were reviewed. The variables analyzed were age, sex, origin, current residence, reason for consultation, professional who requested evaluation, final diagnosis, additional exams and their results. Results: A total of 609 patients were seen, 65.9% aged 0 to 12 years. Genetic syndromes were suspected in 15.1%, and 11% presented developmental delay. Neurogenetic disorders stood out among adults. Mendelian inheritance was more prevalent (17.8%). Requests for genetic consultation have doubled in the last 5 years, with 44.4% due to suspected genetic syndrome. Conclusion: Genetic consultations have shown to be an important tool for inpatient care, reducing the waiting time to initiate treatment, attenuating potential associated costs, and guiding the families of patients. Outpatient care provided diagnosis and genetic counseling for users from the city and surrounding region, decreased costs and offered a training environment in medical genetics.

Abstract: Background: Skin cancer is a highly prevalent condition with a multifactorial etiology resulting from genetic alterations, environmental and lifestyle factors. In Brazil, among all malignant tumors, skin cancers have the highest incidences. Objective: To retrospectively evaluate the incidence, prevalence and profile of basal cell carcinoma, squamous cell carcinoma and cutaneous melanoma in Campos dos Goytacazes and region. Methods: In total, 2,207 histopathological reports of a local reference hospital were analyzed between January 2013 and December 2015, of which 306 corresponded to the neoplasms studied. Results: Of the 306 reports evaluated, 232 basal cell carcinomas (75.9%), 55 squamous cell carcinomas (18%) and 19 cutaneous melanomas (6.5%) were identified. The face was the most involved anatomical site (58.8%) and women (51%) were the most affected gender. The temporal analysis revealed a decrease in the overall incidence of 3.4% from 2013 to 2014 and 5.4% from 2014 to 2015. There was a 10.1% increase in basal cell carcinomas and 38% in melanomas in this period; however, there was a decrease in the number of squamous cell carcinomas of 14.8% during the studied years. Study limitations: Some samples of cutaneous fragments had no identification of the anatomical site of origin. Conclusion: Research that generates statistical data on cutaneous tumors produces epidemiological tools useful in the identification of risk groups and allows the adoption of more targeted and efficient future prevention measures.

ABSTRACT OBJECTIVE: To evaluate the annual variation of oral health and primary care coverage, the tooth extraction ratio, and the average of supervised toothbrushing in Brazilian municipalities according to social development and population size. METHODS: Public secondary data were analyzed. The outcomes were health service indicators (oral health coverage, primary health care coverage, tooth extraction ratio, and average of supervised tooth brushing) estimated for all Brazilian municipalities annually from 2008 to 2015. Mixed-effect multilevel regression models with random intercept and slopes were fitted with a cross-interaction term to estimate the annual percent variation according to the Municipal Human Development Index (MHDI) and population size. RESULTS: Municipalities with low MHDI presented an annual increase in oral health and primary care coverage of 2.65% and 2.23%, respectively, which was significantly higher than municipalities with medium and high MHDI. Oral health and primary care coverage were 69.26% and 35.00% lower among municipalities with a large population. Municipalities with medium and high MHDI showed an annual decrease in tooth extractions of 5.15% and 5.02%, respectively. An annual decrease was observed in the average of supervised toothbrushing of 9.81% and 4.57% in municipalities with low and medium MHDI, respectively. The tooth extraction ratio was higher among larger municipalities; the relation is inverse for supervised toothbrushing. CONCLUSIONS: The access to primary care and oral health services increased in Brazil, while a decrease occurred in mutilating treatment and provision of preventive actions, with disparities among municipalities with different MHDI levels over time.
