2. Extremismo de direita estatal e governança médica: a experiência brasileira durante a pandemia de COVID-19

Medical corporations have secured their authority for professional self-regulation from the State and have responded throughout history by way of their scientifically grounded statements which confer rationality to State interventions in the field of health. During the pandemic in Brazil, there was a solid right-wing association between state governance and medical governance that supported a policy to address the pandemic based on scientific denialism, off-label use of ineffective drugs, and the disregard of certain preventive measures, which together were responsible for most of the more than 700,000 deaths that occurred in the country. This article examines the phenomenon of the interaction of these two forms of governance in a context of authoritarian neoliberalism and thus seeks to reflect on the necessary limits to medical autonomy at all its levels.

The COVID-19 pandemic affected an already critical state of the Italian National Healthcare System and of the Welfare State in general. The aim of this work is to describe how the challenge imposed by pandemic has been faced, and the possible directions that social and health policies could take in the country, also in light of the adoption of the National Recovery and Resilience Plan. Through the analysis of the deep-rooted contradictions in public policies and of the areas where the intervention is more urgent, the work underlines the structural factors which are leading to a progressive shift in the vision of health, from a perspective centred on the role of the State and the protection of the public interest, to one where the role of the private sector and the idea of individual interest are ever stronger, with a consequent change in the balance between economics, legal and political systems.

As part of the Chilean Student movement, graduate students in the Masters Program in Clinical Psychology at the University of Chile organized a protest forum on July 13th, 2011. The protest took place on the steps of the University’s central building and was entitled “History, Public Policy, and Mental Health.” During the protest various academics made presentations and led discussions with the audience (which included pedestrians walking along Alameda Ave in the center of the city). This article is a transcription of one of those presentations. It questions the public health policies which were implemented in Chile during the last several decades and looks at their consequences.

5. The response to COVID-19 in Argentina, Brazil, and Mexico: challenges to national coordination of health policies

Abstract: The article analyzes the fight against COVID-19 in three Latin American countries: Argentina, Brazil, and Mexico. A multiple case study was carried out in a comparative perspective, based on a bibliographic review, documentary analysis, and secondary data, considering characteristics of the countries and the health system, evolution of COVID-19, national governance, containment and mitigation measures, health systems response, constraints, positive aspects and limits of responses. The three countries had distinct health systems but were marked by insufficient funding and inequalities when hit by the pandemic and recorded high-COVID-19 mortality. Structural, institutional, and political factors influenced national responses. In Argentina, national leadership and intergovernmental political agreements favored the initial adoption of centralized control measures, which were not sustained. In Brazil, there were limits in national coordination and leadership related to the President’s denialism and federative, political, and expert conflicts, despite a universal health system with intergovernmental commissions and participatory councils, which were little used during the pandemic. In Mexico, structural difficulties were associated with the Federal Government’s initial reluctance to adopt restrictive measures, limits on testing, and relative slowness in immunization. In conclusion, facing health emergencies requires strengthening public health systems associated with federative, intersectoral, and civil society coordination mechanisms and effective global solidarity mechanisms.

The use of qualitative research methodology is essential for health education, which has the function of offering a field experience with the approach of communities in different contexts. Contributing to insertion and field contact, the conduct of research on the community, the knowledge of the realities and difficulties faced by residents both in the context of quality of life, the social determination of health. Goals To present the use of elements of qualitative research methodology in a health field experience in the urbana occupation Vista Alegre do Juá in Santarém-Pará, Brazilian Amazon Region. Methods This is an experience report that used for its construction elements of the qualitative research methodology, such as the snowball technique for approaching the occupation leaders, the oral report of key informants and observation for knowledge of the site. In loco visits were made with local leaders and also remote contact via whatsapp. Research was carried out with Santarém municipal bodies, such as the Municipal Secretary of Urbanism and Public Services (SEMURB), Municipal Secretary of Infrastructure (SEMINFRA), Social Assistance Reference Centre (CRAS), Municipal Secretary of Health (SEMSA), Technical Nucleus of Health Surveillance (NTVS) and the Epidemiological Monitoring Unit for the gathering of information on the occupation. Results It was observed that the Vista Alegre do Juá occupation suffered few changes, in the sense of producing significant improvements in the lives of its population. The occupation is still in the process of regularisation to become a de facto neighbourhood. Thus, the location still has many needs, such as the absence of public services, even though its representatives seek the necessary improvements, aiming at more government support for the community. Conclusions Despite the efforts of its representatives in the search for improvements so that the community can receive more government support, the lack of public services, such as school, basic sanitation or even transportation, and especially a Basic Health Unit (BHU) is something that makes the conditions of this community even more difficult to maintain life. In 2004, the World Health Organization (WHO) proposed the Age-Friendly Principles, in an attempt to optimize care for the needs of the older population. This concept was part of the Portuguese political agenda in 2017, and it is important to understand whether the national and regional health and ageing programmatic documents follow this trend, defining strategies that contribute to the provision of specialized, integrated and efficient care for the older population. A content analysis was developed, using NVIVO, seeking to understand the priority concerns, the relevance of ageing and the inclusion of the WHO Principles in the programmatic documents. It was concluded that governance measures are the most prominent, compared to long-term care (social). The programs address older people, although in a smaller number than initially expected. The WHO Principles are explicitly referred to only in the National Strategy for Active and Healthy Ageing. Such results highlight the need for health policies to take on increased concern for the older population, integrating specific responses to their needs, by the WHO Principles. No less important is the need for better coordination and articulation of national and regional documents, where national proposals are reflected in the different territorial contexts. This exploratory and qualitative study aimed to analyze the knowledge, perceptions and practices of Oral Health Teams and Community Health Agents about public health policies that regulate the use of medicinal plants and herbal medicines in the context of integrative and complementary practices, including their incorporation, recognition, challenges and possibilities in the local routine of the Brazilian Unified Health System (SUS). Eight Basic Health Units were selected in the municipality of Itabuna, Bahia, Brazil, and interviews were conducted with 8 dentists, 8 oral health assistants/technicians and 8 community health agents, totaling 24 participants. For data analysis, the Collective Subject Discourse technique was used. The knowledge, perceptions and practices of the actors involved in the study about public policies for the introduction of medicinal plants in the SUS were permeated by the hegemonic culture, traditional and fragmented health education, the administrative (un)interest of the municipal authorities, and individual subjectivities that are produced and reproduced in the local oral health services and the primary care. Implemented in 2003, the National Oral Health Policy currently faces a scenario of lack of funding and political prioritization. In this context, the identification of possible local and regional setbacks is fundamental. Thus, the dental care in the Northern macro-region of Paraná between 2013 and 2019 was evaluated through an exploratory study performed based on secondary data from the Unified Health System Outpatient Information System and the public reports of the e-Gestor platform. When analyzing the averages of basic individual dental procedures and collective oral health actions, the coverage of the first programmatic dental consultation, the total number of oral health teams by modality, the coverage of oral health teams and the traditional model, and the ratio between the population and the total number of oral health teams, we observed falls, especially from 2017, in all of these indicators. The results place the macro-region analyzed in a worrisome context and puts other localities on alert, as they show dismantling that inflicts risk to the historical achievements of confronting oral health inequities and overcoming a curative and mutilating dental care model. The evaluation of correlation between health indicators and vaccination against Covid-19 is essential for implementing strategies to fight the disease. Objective: To analyze the spatialtemporal distribution of health indicators and their correlation with vaccination against Covid-19 in the state of Paraná. Methods: Ecological, cross-sectional study of secondary data analysis through bulletins of the Health Department of Paraná from 2021 to 2022, using as variables Incidence Rate (IR), Mortality Coefficient (MC), Lethality Coefficient (LC), Dose 1 (D1) and Vaccination Coverage (VC) of Covid-19. The correlation among the variables was analyzed with the Spearman test and exploratory analysis of univariate and bivariate spatial data using the software GeoDa. Results: There was a positive correlation for the analysis of VC and D1 with IR and MC, and a negative correlation for VC and D1 with LC. Univariate spatial analysis showed positive autocorrelation. In the bivariate analysis, there was a negative spatial correlation between VC and LC, with low-high clusters being highlighted, and a positive spatial correlation between VC and IR with low-low clusters. Conclusion: Knowledge on the influence of space on health indicators can contribute to the organization of policies for regional coping with the pandemic. The dissemination of the results allows the promulgation of the benefits of vaccination in reducing the severity of the disease in the State. [ABSTRACT FROM AUTHOR]

The aim of this article is to reflect on the Policy for the Integral Health for Lesbian, Gay, Bisexual, Transvestite and Transsexual people (LGBT) in the context of health policy in Brazil and in light of Antonio Gramsci’s theory of the Enlarged State, embracing the domains of the state, public policies and health. This study is conducted through bibliographic research and retains a historical and critical character. Furthermore, it describes the Gramscian concept of the Enlarged State, approaches health reform, and analyzes the institution of the National Policy for LGBT Integral Health, especially from the perspective of vulnerability in the context of health public policies. It concludes, in the light of Gramscian theory, that civil society is important to the construction of public policies for LGBT individuals.

ABSTRACT Objectives: to analyze the level of nurses’ engagement in political advocacy by performing cluster analysis. Methods: observational study, with a quantitative approach. A total of 184 nurses working in primary, secondary, and tertiary care in a city in the south of Brazil completed the Policy Advocacy Engagement Scale. Data analysis consisted of descriptive statistics, cluster analysis, analysis of variance, and chi2. The Institutional Review Board approved the study. Results: four differentiated clusters were found according to professional experience, level of healthcare complexity, and unit. The cluster analysis revealed that patient advocacy for community-based obtained the highest mean, indicating that political advocacy is effective in organizational environments and that professional qualification favors greater engagement in political advocacy. Conclusions: the results reveal that nurses play an active role in political advocacy, seeking to promote positive changes in health, especially those working in tertiary care, the nurse group that obtained the highest means.

20. Different remote realities: health and the use of territory in Brazilian rural municipalities

ABSTRACT OBJECTIVE To characterize remote rural Brazilian municipalities according to their logic of insertion into socio-spatial dynamics, discussing the implications of these characteristics for health policies. METHODS Starting from the category of analysis – the use of the territory – a typology was elaborated, with the delimitation of six clusters. The clusters were compared using socioeconomic data and the distance in minutes to the metropolis, regional capital, and sub-regional center. Mean, standard error and standard deviation of the quantitative variables were calculated, and tests on mean differences were performed. RESULTS The six clusters identified bring together 97.2% of remote rural municipalities and were called: “Matopiba,” “Norte de Minas,” “Vetor Centro-Oeste,” “Semiárido,” “Norte Águas,” and “Norte Estradas.” Differences are observed between the clusters in the analyzed variables, indicating the existence of different realities. Remote rural municipalities of “Norte Águas” and “Norte Estradas” clusters are the most populous, the most extensive and are thousands of kilometers away from urban centers, while those in “Norte de Minas” and “Semiárido” clusters have smaller areas with a distance of about 200 km away from urban centers. The remote rural municipalities of the “Vetor Centro-Oeste” cluster, in turn, are distinguished by a dynamic economy, inserted into the world economic circuit due to the agribusiness. The Family Health Strategy is the predominant model in the organization of primary health care. CONCLUSION Remote rural municipalities are distinguished by their socio-spatial characteristics and insertion into the economic logic, demanding customized health policies. The strategy of building health regions, offering specialized regional services, tends to be more effective in remote rural municipalities closer to urban centers, as long as it is articulated with the health transportation policy. The use of information technology and expansion of the scope of telehealth activities is mandatory to face distances in such scenarios. Comprehensive primary health care with a strong cultural component is key to guaranteeing the right to health for citizens residing in such regions.

Abortion is one of the main causes of maternal mortality, because health inequalities and the nonexistence of public health policies that protect women and allowing them to perform abortion on adequate sanitary conditions. The induced and unsafe abortion is a product of several social determinants, like gender, sociodemographic, ethnic-racial and socioeconomic factors, with direct results on women's health, like comorbidities and mortality. This theoretical reflection aims to understand the abortion-related Social Determinants of Health and the existing social protection policies in Brazil. The impact of the unsafe abortion as a public health problem and the maintenance of punitive laws leads to underreporting and lack of the real set of the abortion problem and that makes the creation of effective public policies unfeasible. The researches about abortion from Social Determinants of Health perspective are fundamental to identify what influences this phenomenon on order to create, implement and evaluate equanimous and effective health policies. Objective: to understand how the implementation of the e-SUS Primary Care system has been processed and its impact on the daily life of the health teams. Method: a qualitative research study, conducted in a municipality in the inland of the state of São Paulo with professionals who work in Primary Health Care and use the e-SUS Primary Care system as a work tool. Semi-structured interviews and thematic data analysis were used with Kotter’s three-phase approach. Results: a total of 17 professionals, nurses, physicians, dentists and community agents were interviewed. The implementation of e-SUS Primary Care and its impact on the daily life of health teams were understood in terms of mandatory implementation; weaknesses for implementation, such as absence of material resources and implicit imposition for the use of the system; fragile training for deployment and learning from experience. Conclusion: a harmful incentive process was observed, conducted from the perspective of institutional pressure, use of the system to justify the work performed and, on the other hand, there was the creation of collaborative learning mechanisms between the teams.

Background: Although the National Smoking Control Program - PNCT is one of the most effective in Latin America, resulting in a continuous decline in the prevalence of smoking until 2013, its prevalence has been shown to be important in some regions of Brazil. Objective: To analyze the situation of the PNCT network in the Municipality of Rio de Janeiro in 2015, regarding the uptake of smokers from the perspective of Social Network Analysis. Method: This is an exploratory-descriptive and qualitative study. Information was obtained through telephone calls made to all Health Units participating in the PNCT, totaling 183 calls. To complement the analysis of the approach through the analysis of social networks, we used the methodology known as SWOT matrix - strengths, opportunities, weaknesses, and threats. Results: From a total of 183 units, 68 responded. Although more than half of the consultations took place through referrals from health professionals, a significant number took place through informal means. Conclusion: It is concluded that several factors related to weaknesses, strengths, opportunities, and threats exist. However, if there is little lack of resources and strategies to improve uptake, there is much to be done so that telephone calls are opportunities for change. [ABSTRACT FROM AUTHOR]

Participatory practices, within broader processes of rescaling and governance, have been identified as the solution to the “democratic deficit” in healthcare. Conversely, scholars have underscored how these practices could well be used for a contrary purpose, particularly in a top-down process. The first part of this paper outlines a theory-based evaluation framework oriented towards the analysis of institutional practices fostering citizens’ involvement in healthcare decision-making processes and how this involvement can act as a driver for the democratisation of the healthcare system. Following this interpretation, the second part of the paper analyses the new local governance structure adopted by Tuscany (Italy) in the healthcare sector, in particular the later stages of its adoption and diffusion and more than ten years after its institutionalisation. This leads us to identify certain crucial issues to be addressed when institutions promote re-visiting decision-making processes.

Diferentes formulações têm dado destaque à Atenção Primária à Saúde como porta de entrada preferencial do sistema de saúde, base das redes de atenção e função de coordenação do cuidado. A regulação assistencial (ou do acesso), por sua vez, vem sendo considerada, no Brasil, uma estratégia e tecnologia central de gestão das redes. Este artigo objetivou caracterizar a interface entre atenção primária e regulação assistencial nas formulações e diretrizes nacionais sobre regulação assistencial do SUS, considerando o período de 2002 a 2017. Adotou-se a metodologia da análise documental, utilizando fontes de domínio público, notadamente portarias e manuais técnicos do Ministério da Saúde, organizando os conteúdos extraídos nas dimensões "conceitos de regulação", "organização e gestão da regulação" e "estratégias e ferramentas de regulação", dentro das quais se buscou caracterizar o lugar da atenção primária. Observou-se maior destaque para a interface entre regulação assistencial ambulatorial e atenção primária a partir de 2011 com a noção de microrregulação, de modo mais marcante nos documentos e iniciativas relacionados à gestão da atenção primária do que nos específicos da regulação, enfocando a elaboração de dispositivos de regulação a partir da atenção primária, destacando-se o Telessaúde. Different formulations have emphasized Primary Health Care as the preferred gateway to the health system, the basis of care networks and the care coordination function. Access regulation, in turn, has been considered, in Brazil, as a strategy and central technology for network management. This article aimed to characterize how primary care is placed in the national formulations and guidelines on health care regulation of the SUS from 2002 to 2017. The methodology was the documentary analysis, using public domain sources, notably ordinances and technical manuals of the Ministry of Health, organizing the extracted contents in the dimensions "concepts of regulation", "organization and management of regulation" and "strategies and tools of regulation", which sought to characterize the place of primary care. Greater emphasis was given to the interface between health care regulation and primary care from 2011, more prominently in the documents and initiatives related to primary care management than in the specifics of regulation, focusing on the ambulatory dimension of access regulation and in the elaboration of regulation devices based on primary care, highlighting Telehealth. In this study we present an overview of the dissemination of the new Coronavirus (SARS-Cov-2) in the municipalities of the Alto Solimões micro-region in the state of Amazonas, Brazil. The data used comes from the Monitoring System of cases of COVID-19 infection in the municipalities of the State of Amazonas, and from the Monitoring System of cases of COVID-19 among indigenous peoples in Brazil, which are organized in the database and WebSig coordinated by researchers from the federal universities of Rio Grande do Sul and Amazonas, and the university of the State of Amazonas. We systematize and analyze information from municipal and state public agencies responsible for preventing and combating the pandemic, giving special attention to data on indigenous health, present in the bulletins of the Special Secretary of Indigenous Health, but, which are questioned by reports made by indigenous organizations. By comparing these data with the geographical, political and socio-economic characteristics of the Alto Solimões microregion, the numbers of new coronavirus infections and of deaths resulting from complications of the disease, we forecast a scenario of vulnerability and risk for the population, especially for the indigenous peoples. The ineffectiveness of public health policies in the context of the pandemic reveals a worsening of situations in which the services and actions of health professionals are deficient, which favours the spread of the virus in a cross-border context. Resumo A Rede de Cuidado à Pessoa com Deficiência (RCPD) foi implementada no Sistema Único de Saúde (SUS) para expandir o acesso aos serviços às pessoas com deficiência (PCD). Uma vez que seu funcionamento depende de mecanismos robustos de cooperação e pactuação entre os entes federados, este artigo analisa a dinâmica da Comissão Intergestores Bipartite (CIB) de São Paulo na condução da implementação da RCPD. Realizou-se uma análise documental das atas da CIB-SP de 2011 a 2019. O referencial teórico se pautou na análise de políticas públicas, utilizando abordagem multidimensional a partir das dimensões política, estrutura e organização. Na dimensão política, as pautas centrais são referentes à responsabilidade dos entes federados e demandas originárias do Ministério da Saúde e Ministério Público. A dimensão estrutura ocorre prioritariamente por pautas protocolares de informes de credenciamento/descredenciamento. Já na dimensão organização, foram classificadas como centrais: avaliação dos serviços; necessidades populacionais; oferta, fluxos, regulação assistencial e dispensação de órteses, próteses e meios auxiliares. Conclui-se que, quando não induzida por atores externos, a discussão é protocolar, reduzida aos informes de credenciamento de ações e serviços e adquire centralidade com demandas de órgãos externos e com o surgimento de novos problemas de saúde pública. Abstract The Care Network for People with Disabilities (RCPD) was implemented in the Brazilian National Health System (SUS) to expand the access to services to people with disabilities (PWD). Since its functioning depends on robust cooperation and pact mechanisms between federated states, this article analyzes the dynamic of the Bipartite Inter-manager Commission (CIB) of São Paulo in implementing RCPD. Document analysis of the records of the CIB-SP from 2011 to 2019 was carried out. The theoretical referential was based on the analysis of public policies, using a multidimensional approach from the political, structural, and organizational dimensions. In the political dimension, the main rulings relate to the responsibilities of federated states and demands from the Ministry of Health and Public Ministry. The structural dimension comprises mainly protocol rulings regarding accreditation/de-accreditation. In the organizational dimension, at last, were classified as central: service evaluation; populational needs; offer, flow, regulation of assistance and dispensing of orthotics, prostheses, and auxiliary means. In conclusion, when it is not conducted by external actors, the discussion follows protocol, being reduced to accreditation informs of actions and services and gains centrality with demands from external organs and the emergence of new public health problems.

ABSTRACT Objective: To understand how the process of construction of the Group of Pregnant Women and/or Pregnant Couples of the Federal University of Santa Catarina from 1996 to 2016 was established. Method: Qualitative research of a social historical nature, involving nine health professionals who participated in the group of pregnant women and/or pregnant couples in this period. The date of collection took place between February and May 2017. Data analysis was guided by Orem's Self-Care Theory and Bardin's content analysis technique. Results: The process of construction of the group of pregnant women had strong influence from the public health policies of the University Hospital Maternity and its assistance of the time. Likewise, it was created based on the need for guidance to pregnant women and their companions to deal with the pregnancy-puerperal process with the perspective of a multidisciplinary and integral work, stimulating self-knowledge and self-care. Conclusion: The group emerged as an extension project and strengthened by public health policies, its scientific evidences and by the assistance philosophy of a maternity school. The group building process involved a set of actions, planning and execution of activities, building an educational space, free of charge, with an exchange of mutual experience and open to the community.

This investigation aims to present and to discuss how the Sanatório São Julião circulated in the press media of Campo Grande, between 1941 and 1970. More specifically, it analyzes practices of assistance to the Sanatório and its internees by society through news published by some newspapers, e.g., Correio do Estado, O Matogrossense and Jornal do Comércio. The analyzes of primary sources suggest that Campo Grande society, driven by certain social images of Leprosy in the print media, supported the inmates at the Sanatório São Julião with donations of diverse genres, from foods to high values, made by "generous" people. Objective: To identify the safety actions to elderly patients attended at health basic units and at hospital services in the nurses' conception. Materials and Methods: Study with a descriptive and qualitative approach, developed in two cities of Rio Grande do Sul. The study subjects were 28 nurses that worked at health basic units and hospital services. Results: The data from the study allowed the construction of two categories. Safety of the elderly in preventing falls and safety of the elderly in the administration and use of medication. Conclusion: In order to minimize risks and generate safety for elderly, it is necessary to implement multiple strategies, such as the restructuring of services, reorganization of care and, especially, the reconfiguration of the health work process. - Abstract
Evaluability of the Brazilian Regulatory Policy within the SUS: a preliminary proposal Evaluability studies allow determining the purposes of the policy and the evaluation focus, assisting a thorough understanding and prior appreciation of the possibilities of policy evaluation. Thus, this article analyzes the evaluability of the Brazilian Regulatory Policy, from January to May of 2018, with an exploratory and qualitative approach, in order to define how this policy can be evaluated and the elaboration of the logical and theoretical models of evaluation, according to the seven- element system by Thurston and Ramaliu. The results contributed to a better understanding of the structure and operationalization of the policy to reach their results and impacts on health. However, the Policy has many components and activities, so it is difficult to elaborate indicators and a global policy assessment matrix. It is suggested a cut of the policy for an evaluation process. The Brazilian Regulatory Policy was published 10 years ago but any proposal to evaluate it has been submitted. It is known that regulation in the health sector brings numerous challenges, and it requires a proposal of evaluation that goes beyond the dimensions of efficiency and equity, including the training and activities of health professionals, accessibility to health services and health technology assessment. - Abstract
Background: Cleft lip and palate is the most prevalent craniofacial anomaly. Although treatment and rehabilitation initiatives by SUS have existed since 1993, there are few studies on the reality of Brazilian centers. Objective: This study evaluated the implantation of attention to the person with cleft lip and palate in a Brazilian rehabilitation center. Method: Case study evaluation research to assess the degree of implantation in a rehabilitation center. In the first stage, we formulated a logic model of the attention to the person with cleft lip and palate based on international and national literature. The matrix was elaborated with the criteria, indicators and expected standards of care, which was validated by specialists through the Delphi consensus technique. Visits with on-site observation and semi-structured interviews were also carried out with key-informants of the center. Results: We observed advanced level of implementation in Management (89.2%) and its subdimension, Organization (94.7%), as well as in Rehabilitation (84.9%). The A subdimension shows weaknesses (48.8%), mainly due to the lack of meetings, training and support to permanent education, monitoring of results and evaluation of users' perceptions, health education spaces, accomplishment of active search of the missing cases and support. Conclusion: The evaluated center presented advanced implementation, fulfilling more than 80% of the evaluation. We recommend greater interaction between management and professionals to intervene on the identified points of improvement. The developed instrument contributes to the evaluation and implementation of other centers of reference in Brazil. [ABSTRACT FROM AUTHOR]

The paper analyzes the obstacles currently placed for the implementation of a universal and equitable health system and characterizes social movements that stand before these barriers. Departing from the constitutional definition of the right to health in 1988 identifies two issues not fully equated by the very design of the Unified Health System/SUS: decentralization and universalization without securing additional resources. Some issues have distorted the SUS in its implementation: underfunding, the reform of the state, pressures towards the social movements and the convergence of agendas with the World Bank. Concludes that the current threats to the right of health depart come from the consolidation of big business in health, privatism and ideological inflection of some dominated classes to the capital project. Three lines are identified in the current struggle for the right to health: a flexible health reform, mass democracy and the need for social revolution.

This article aims to describe the evolution of the transition in mortality from breast and cervix cancers in Brazil in relation to socioeconomic development. There were data obtained of mortality from breast cancer and cervical cancer in Brazil for a period of 20 years, corresponding to a censitary range. Deaths were corrected and rates standardized. In addition, social indicators were measured for all three census years included in the time series. It was observed that the Brazilian states have a lot of heterogeneity with respect to social indicators and mortality for both cancers, and there seems to be a pattern between socioeconomic status and mortality from breast and cervix cancer. It is therefore necessary to carry out the assessment and planning of actions, considering development scenarios, to ensure universal and equal access of women to public health policies.

RESUMO O artigo apresenta uma análise comparada de dez países selecionados sobre as relações entre governança participativa, perfis socioeconômicos e sistemas de saúde com resultados sanitários e de Indicadores de Governança Global. As fontes principais foram bases de dados produzidas e/ou compiladas pelo Banco Mundial. O modelo analítico se apoia em enfoque institucionalista para tratar de proteção social e governança participativa – esta, como utilizada, recobre as noções de participação social, porosidade governamental e regulação responsiva. Os resultados mostram uma sólida convergência entre perfis socioeconômicos mais distributivos, sistemas sanitários com maior financiamento público e universalismo e melhores indicadores de governança. Esta análise reforça os argumentos sobre trajetórias institucionais socialmente virtuosas e sujeitas a reforços positivos capazes de produzir melhores resultados sociais e políticos ao longo do tempo. ABSTRACT This paper presents a comparative analysis of ten selected countries regarding the established relationships of participative governance, socioeconomic profiles, and health care systems with health outcomes and Global Governance Indicators. Significant sources were databases produced or compiled by the World Bank. The analytical model adopts an institutionalist approach to address social protection and participative governance – the latter, as used, recovers notions of societal participation, government porosity, and responsive regulation. Outcomes show a solid convergence of more distributive socioeconomic profiles, more universalist health systems with higher government financing, and better governance indicators. This analysis supports the arguments that socially virtuous institutional paths subjected to positive feedback favor better social and political outcomes over time.

Access for disabled persons in 63 services of 12 municipalities in Vale do Paraíba and São Paulo’s North Coast was evaluated in an exploratory study, with a quali-quantitative design carried out 2008 - 2010 with observation guidelines of service reception, 63 structured interviews with the people in charge and with 596 users, 57 being disabled persons, resulting in a characterization of the service provided, personnel, assistential activities, working shifts, scheduling, transportation, disabled users, reasons for requiring the services, waiting time and reception. Access facilitators were the proximity to the homes, healthcare strategies in the families, rehabilitation professionals and reduced waiting time. Complicating factors were the mandatory presence of the users or their representatives at scheduling, the absence of or difficulties with public transportation in 41% of the municipalities, the absence of adapted transportation in 75% of them, the existence of environmental barriers and the predominance of biomedical assistance and few rehabilitation actions in the basic care services. It was concluded that it is essential for policies to take such specificities into consideration in order to guarantee the right to health care of this population group. O acesso de pessoas com deficiências em 63 serviços de 12 municípios do Vale do Paraíba e Litoral Norte de São Paulo foi avaliado em estudo exploratório, com desenho quali-quantitativo realizado entre 2008 e 2010. Foi utilizado roteiro de observação da recepção dos serviços, 63 entrevistas estruturadas com seus responsáveis e 596 com usuários, sendo 57 pessoas com deficiência, resultando na caracterização de serviços, equipes, atividades assistenciais, turnos de funcionamento, agendamento, transporte, usuários com deficiência, motivos de procura, tempo de espera e acolhimento. Constatou-se como facilitadores do acesso a proximidade do domicílio, a presença da estratégia de saúde da família, de profissionais de reabilitação e o tempo de espera reduzido. Os fatores dificultadores foram a exigência do agendamento com a presença do usuário ou representante, a ausência ou dificuldades no transporte público em 41% dos municípios, a ausência de transporte adaptado em 75% deles, a existência de barreiras ambientais e a prevalência de assistência de caráter biomédico e poucas ações reabilitacionais nos serviços de atenção básica. Conclui-se que é fundamental que as políticas considerem essas especificidades para garantir o direito à saúde desse grupo populacional.

Introduction: Various particularities and guarantees arising from the issue of LGBT rights are observed. Several public health policies have been implemented by the federal government to address the specific issues of this population. However, the disparity in treatment suffered by these individuals in the public network, added to the high rates of violence and discrimination suffered by the population, ends up remaining constant and thus creates a challenge for public institutions. The objective of the study is to carry out a critical analysis of the effectiveness of public policies for the LGBTQIA+ population in recent decades. Methodology: This is an integrative literature review carried out through the Scielo and BVS databases containing publications between 2009 and 2022. Results: For the composition of the sample, 7 scientific articles were selected following the inclusion and exclusion criteria, in addition to theoretical books and materials from the Ministry of Health obtained throughout the research. Conclusion: Despite the progress of services in meeting the demands of the LGBTQIA+ population, in practice, there are still gaps in the implementation of policies to meet them. Therefore, public health policies should be more precise and effective to reach all citizens and, consequently, promote an improvement in the health condition and vulnerability of the LGBT community. Introducción: Se observan diversas particularidades y garantías derivadas del tema de los derechos LGBT. El gobierno federal ha implementado varias políticas de salud pública para abordar los problemas específicos de esta población. Sin embargo, la disparidad de trato que sufren estas personas en la red pública, sumada a los altos índices de violencia y discriminación que sufre la población, termina por mantenerse constante y por ende genera un desafío para las instituciones públicas. El objetivo del estudio es realizar un análisis crítico de la efectividad de las políticas públicas para la población LGBTQIA+ en las últimas décadas. Metodología: Se trata de una revisión integrativa de la literatura realizada a través de las bases de datos Scielo y BVS que contienen publicaciones entre 2009 y 2022. Resultados: Para la composición de la muestra se seleccionaron 7 artículos científicos siguiendo los criterios de inclusión y exclusión, además de libros teóricos y materiales del Ministerio de Salud obtenidos a lo largo de la investigación. Conclusión: A pesar del avance de los servicios en la atención de las demandas de la población LGBTQIA+, en la práctica aún existen vacíos en la implementación de políticas para atenderlas. Por lo tanto, las políticas públicas deben ser más precisas y efectivas para llegar a todos los ciudadanos y, en consecuencia, promover una mejora en la condición de salud y vulnerabilidad de la comunidad LGBT. Introdução: São observadas variadas particularidades e garantias que decorrem da questão dos direitos LGBT. Diversas políticas de saúde pública vêm sendo implantadas pelo governo federal, para atender as questões específicas dessa população. Porém, a disparidade de tratamento sofrida por esses indivíduos na rede pública, somado a índices altos de violência e discriminação sofridas pela população acaba se mantendo constante e cria-se assim um desafio para as instituições públicas. O objetivo do estudo consiste em realizar uma análise crítica da efetividade das políticas públicas para a população LGBTQIA+ nas últimas décadas. Metodologia: Trata-se de uma revisão integrativa da literatura realizada através das bases de dados Scielo e BVS contendo publicações entre 2009 a 2022. Resultados: Para composição da amostra foram selecionados 7 artigos científicos seguindo os critérios de inclusão e exclusão, além de livros teóricos e materiais do Ministério da Saúde obtidos ao longo da pesquisa. Conclusão: Apesar do progresso dos serviços no atendimento às demandas da população LGBTQIA+, na prática, ainda existem lacunas na implementação de políticas para atendê-las. Sendo assim, as políticas públicas em saúde devem ser mais precisas e efetivas para alcançar todos os cidadãos e, consequentemente, promover melhoria da condição de saúde e vulnerabilidade da comunidade LGBT.
