RESUMO Diversos fármacos são utilizados no tratamento da epilepsia e, assim como outros medicamentos, podem induzir a ocorrência de efeitos adversos, alguns tão graves que geram a necessidade de descontinuidade e substituição da terapia. A carbamazepina pode levar a alterações nos sistemas cardiovascular, respiratório e neurológico, sendo descritos na literatura casos de indução de miastenia gravis como distúrbio neuromuscular. Este estudo relata o caso de um cão que desenvolveu polirradiculoneuropatia desmielinizante, tendo como provável desencadeante a terapia com carbamazepina. O paciente apresentou tetraplegia, ausência de reflexos espinhais nos quatro membros, fraqueza cervical, diminuição do reflexo palpebral bilateral e esforço respiratório. A eletroneuromiografia demonstrou sinais de desmielinização. Este, portanto, é o primeiro relato de associação entre carbamazepina e polirradiculoneuropatia desmielinizante em cão. ABSTRACT Different drugs are used in the treatment of epilepsy and, like other drugs, may induce the occurrence of adverse effects, some of them so severe that the drug must be discontinued and replaced. Carbamazepine may lead to changes in the cardiovascular, respiratory, and neurological systems, and cases of induction of myasthenia gravis as a neuromuscular disorder have been described in the literature. This paper reports the case of a dog that developed demyelinating polyradiculoneuropathy, probably triggered by carbamazepine. The patient presented tetraplegia, absence of spinal reflexes in the four limbs, cervical weakness, decreased bilateral eyelid reflex and respiratory effort. Electroneuromyography showed signs of demyelination. This, therefore, is the first report of association between carbamazepine and demyelinating polyradiculoneuropathy in dogs.

Orientador: João Batista de Miranda Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciencias Medicas Resumo: O objetivo deste estudo foi analisar os efeitos dos exercícios em cadeia cinética aberta (CCA) e cadeia cinética fechada (CCF) no tratamento da síndrome da dor femoropatelar (SDFP). Para tanto, 24 voluntários portadores de SDFP foram aleatoriamente divididos em dois grupos: grupo I (n=12): realizou exercícios em CCA; Grupo II (n=12): realizou exercícios em CCF. Os grupos foram submetidos a oito semanas consecutivas de tratamento, que consistiu de três sessões semanais realizadas em dias alternados. Para análise dos padrões de ativação dos músculos vasto medial oblíquo (VMO) e vasto lateral (VL) os sinais eletromiográficos (EMG) foram adquiridos com eletrodos bipolares de superfície, quantificados pela raiz quadrada da média (rot mean square ¿ RMS) e normalizados pela contração isométrica voluntária máxima do quadríceps. Por meio de escalas avaliou-se a intensidade da dor e funcionalidade dos voluntários. A análise dos valores da razão VMO/VL nos grupos I e II demonstrou que não houve diferenças significativas entre os tempos pré e pós-tratamento nas fases concêntrica (p>0,05) e excêntrica (p>0,05) dos exercícios em CCA e CCF. Apesar disso, o músculo VMO apresentou menor taxa de ativação em relação ao VL na fase excêntrica do exercício em CCF. No. of bitstreams: 0 Previous issue date: 2003-12-01 Objective: to describe the peripheral neuromuscular system abnormalities in adult RA patients using electroneuromyography (ENM) in order to determinate their prevalence. Potential correlation between electroneuromyographic findings and demographic, clinical, laboratorial and radiological data was ako examined. Methods: sixty four adult RA patients were evaluated for demographic, clinical, laboratorial and radiological parameters, and ENM was performed in all of them. Results and conclusions: ENM abnormalities were found in 14 (21.9%) RA patients. Carpal tunnel syndrome (CTS), the most prevalent abnormality, was detected in 9 patients (14.1%) and peripheral polyneuropathy (PPN) was present in 8 patients (12.5%). PPN associated with CTS was observed in 3 patients (4.7%) and 1 patient (1.6%) presented PPN associated with ulnar nerve compression at the elbow. In half of the PPN cases, only sensitive neural Gbers were involved. In the other half, motor involvement was also detected. The primary pattern lesion was axonal loss in all PPN cases. Compressive neuropathy was present in 50% of PPN patients. There was statistically significant correlation between the presence of ENM abnormalities and the duration of disease (p < 0.001). OBJETIVO: Analisar os resultados obtidos pela ressonância magnética (RM) e eletroneuromiografia (ENMG) em pacientes com diagnóstico de esquistossomose medular (EM). MÉTODO: Foram analisadas 18 RM da coluna vertebral tóraco-lombar e 24 ENMG dos membros superiores e inferiores de pacientes com diagnóstico definido ou altamente provável de EM nas suas formas clínicas mielorradiculítica e mielítica. RESULTADOS: Entre as 18 RM realizadas, 16 (88,8%), mostraram alterações caracterizadas por imagens hipointensas em aquisições pesadas em T1, hiperintensas em T2, com captação heterogênea do contraste gadolínico e/ou realce das meninges e cauda equina. Entre as 24 ENMG, 23 (95,8%) mostraram quadro de multirradiculopatia bilateral das raízes nervosas L2, L3, L4, L5, S1, S2, assimétrico em 10 casos (41,6%). CONCLUSÃO: A RM e a ENM demonstraram ser exames úteis para o diagnóstico da EM ao revelarem anormalidades que se repetiram sob um determinado padrão em 88,8% e 95,8% dos casos, respectivamente. Apesar de não terem especificidade para a EM, estes procedimentos devem ser realizados rotineiramente com o objetivo de fortalecer o diagnóstico presuntivo desta doença. OBJECTIVE: To analyze the usefulness of magnetic resonance imaging MRI and electroneuromyography (ENMG) in the diagnosis of schistosomiasis of the spinal cord (SSC). METHOD: 18 MRI of the thoracolumbar spine and 24 ENMG of the upper and lower limbs were carried out on patients with a definite diagnosis of SSC in the clinical forms of myeloradiculitis or thoracic transverse myelitis. RESULTS: Of the 18 MRI carried out, 16 (88.8%) showed abnormalities on T1 and T2 weighted images and heterogenous pattern of enhancement with contrast material injection. Of the 24 ENMG, 23 (95.8%) showed a feature of bilateral multiradiculopathy of roots L2, L3, L4, L5, S1, S2, asymmetric in 10 cases (41.6%), characterized by signs of denervation in the lumbar paraspinal musculature and long duration polyphasic potentials in increased proportion, as well decrease of the recruitment of motor units. CONCLUSION: In spite of the non-specificity of both exams, the MRI and the ENMG showed to be useful for the diagnosis of SSC, by revealing abnormalities that repeated in 88.8% and 95.8% of the cases, respectively.

Tivemos como objetivo investigar por eletroneuromiografia (ENMG) e potenciais evocados somatossensitivos a possibilidade de acometimento do nervo periférico na mielopatia pelo HTLV-I (HAM), correlacionando os achados com os parâmetos clínicos e com a síntese intratecal de anticorpos anti HTLV-I. Os pacientes tinham sorologia negativa para HIV e apresentaram VDRL negativo. Outras causas para mielopatia ou neuropatia periférica foram excluídas. De 32 pacientes que realizaram ENMG, em 34,3% ela foi considerada sugestiva de neuropatia periférica. Esta foi sobretudo assimétrica (82%), sensitivo-motora (90%),com padrão axonal (54,5%) ou misto (45,4%). Em 63,6% dos casos de neuropatia periférica, havia sintomas correlatos. O potencial evocado auditivo foi anormal em apenas um caso. O potencial evocado visual foi anormal em 28,5% dos casos. Não havia sinais ou sintomas auditivos ou visuais. Em 85,5% dos casos obteve-se potencial evocado sensitivo alterado. Desses, 50% tinham manifestação clínica compatível. Em 28% dos pacientes com potencial evocado sensitivo anormal houve concomitância de ENMG sugestiva de neuropatia periférica. The objective of the present study was to correlate electroneuromyography (ENMG) and evoked potentials findings with clinical aspects and intrathecal synthesis of HTLV-I antibodies production on HTLV-I myelopathy (HAM). Patients were seropositive for HTLV-I by different assays and seronegative for HIV and VDRL. They had no other causes of myelopathy and peripheral neuropathy. Peripheral neuropathy was stablished in 34.3% of the cases by ENMG. Peripheral neuropathy was mostly asymmetric (82%), sensory motor (90%), axonal (54,5%) or of a mixed type (45,4%). In 63.6% of these cases related symptoms were observed. ABR was abnormal in one patient and the PRVEP in 28.5% ,who were symptom-free. The SEP was abnormal in 85.7% of the cases, half of them presenting clinical complaints. In only 14% of the individuals with clinical manifestations, SEP was normal. In 28% of patients with abnormal SEP the ENMG disclosed a peripheral neuropathy.
