Chagas disease is a neglected chronic condition that presents high morbidity and mortality burden, with considerable psychological, social, and economic impact. The disease represents a significant public health issue in Brazil, with different regional patterns. This document presents the evidence that resulted in the Brazilian Consensus on Chagas Disease. The objective was to review and standardize strategies for diagnosis, treatment, prevention, and control of Chagas disease in the country, based on the available scientific evidence. The consensus is based on collaboration and contribution of renowned Brazilian experts with vast knowledge and experience on various aspects of the disease. It is the result of close collaboration between the Brazilian Society of Tropical Medicine and the Ministry of Health. This document shall strengthen the development of integrated control measures against Chagas disease in the country, focusing on epidemiology, management, comprehensive care (including families and communities), communication, information, education, and research. more...

This study aims to investigate the contribution of cultural resources to Community-Based Integrative Care (CBIC), to consolidate it as a model of community-based mental health and a political strategy for local health, and to identify the cultural strategies most used in CBIC sessions. This is a qualitative study, conducted in the city of João Pessoa, state of Paraíba, Brazil, with ten therapists. We used semi-structured interviews and afield diary, from September, 2008, to March, 2009, then proceeded to the interpretive analysis of the data. It was evident that the inclusion of cultural resources contributes to the consolidation of CBIC, for it reclaims and strengthen values, and it underscores the personal and social identity of individuals, encouraging effective participation. The main cultural resources used were music, dynamics and prayers. The conclusion was that cultural resources are an important resource for the work of the therapist, for it strengthens bonds and helps people to give a new meaning to their suffering. more...

Objective: To estimate the prevalence of lateral and anteroposterior postural changes in female adolescents and to investigate whether these changes are associated with certain socioeconomic, demographic, anthropometric, or behavioral variables., Methods: This epidemiologic survey included a representative sample of 495 high school students from regular day school programs in the city of São Leopoldo, Rio Grande do Sul, Brazil, which is in the South region of Brazil. The students, who were 14 to 18 years old, were assessed in October and November of 2004. Postural changes were defined as skews in the spinal curvature, identified through noninvasive postural assessment., Results: The prevalence of lateral changes was 66% (95% confidence interval (CI): 61.5% to 70.0%) vs. 70% for anteroposterior changes (95% CI: 65.2% to 73.5%). Lateral changes were more prevalent in students with a normal body mass index (prevalence ratio (PR) = 1.32; 95% CI: 1.09 to 1.59) and in those who watched television for more than 10 hours weekly (PR = 1.16; 95% CI: 1.02 to 1.32). The prevalence of anteroposterior changes was higher in students whose parents/guardians had no schooling or only had elementary schooling (female guardians: PR = 1.30, 95% CI of 1.09 to 1.55; male guardians: PR = 1.20, 95% CI of 1.02 to 1.40) and in students who were overweight or obese (PR = 1.33; 95% CI of 1.19 to 1.48)., Conclusions: The high prevalence of postural changes observed is reason for concern since these changes can translate into spinal problems in the medium to long term. Objective: To evaluate asymptomatic patients with chronic Chagas disease to determine prevalence of ventricular arrhythmias, left ventricular dysfunction, and B-type natriuretic peptide (BNP) plasma levels., Methods: One hundred and six patients from the Chagas disease outpatient clinic underwent clinical evaluation, electrocardiogram (ECG), cardiothoracic index (CTI), ambulatory electrocardiogram (Holter monitoring), echocardiogram, and BNP measurement and then were distributed into three groups: GI, with normal ECG (n = 50); GIIA, with ECG changes characteristic of Chagas disease (n = 31); and GIIB, with other ECG changes (n = 25)., Results: The most common electrocardiographic changes were the following. GIIA: complete right bundle branch block (35%), left anterior hemiblock (35%), and electrically inactive areas (32%); GIIB: inferolateral repolarization change (28%), and left ventricular overload (24%). Mean CTI index values were similar (p = 0.383). Ventricular arrhythmia prevalence was higher in the GIIA (77%) and GIIB (75%) groups than in the GI group (46%) (p = 0.002). Ventricular dysfunction was more prevalent in the GIIA (52%) and GIIB (32%) groups than in the GI group (14%) (p = 0.001). Systolic dysfunction was more prevalent in the GIIA group (29%) than in the GIIB (20%) and GI groups (2%) (p < 0.001). Diastolic dysfunction was more prevalent in the GIIA (42%) and GIIB (28%) groups than in the GI group (12%) (p = 0.005). Mean B-type natriuretic peptide levels were 30 +/- 88 pg/mL in the GI group, 66 +/- 194 in the GIIA group and 24 +/- 82 for the GIIB group (p = 0.121), respectively., Conclusion: Arrhythmias and left ventricular dysfunction are more prevalent in asymptomatic patients with chronic Chagas disease and abnormal ECG than in patients with normal ECG.
