4. Fixed bilateral endo-exo prostheses in feline femur - case report

ABSTRACT The use of endo-exo prosthesis is an alternative to improve the quality of life of amputees, allowing correct distribution of body weight, reducing skin problems and pain related to the implant, and reducing lameness. The aim of this paper is to report the use of a fixed bilateral endo-exo femur prostheses in a cat with amputation prior to treatment. The endoprostheses were made with titanium and the exoprostheses were made with 3D printing technology. A longitudinal craniolateral surgical approach was used and the skin incision was made over the knee-joint, then fascia lata incision, followed by biceps femoris and vastus lateralis muscles retraction, incision through the joint capsule to expose the tibiofemoral joint, disarticulation, and tibia and fibula removal. Afterwards, a femoral leveling osteotomy and insertion of the implant in the medullary canal were performed, being attached in the cis and trans cortical bone. The patient was evaluated in the immediate post-operative and periodically thereafter, showing satisfactory results, without macroscopic signs of rejection, wound dehiscence or infection, fracture, or injury of limbs, and presented adapted walking. Furthermore, it was observed the return to common behaviors for the species.

Cerebral palsy (CP) is a common cause of physical disability in the first years of life, with repercussions on all functions of the individual. The objective of the study is to analyze the acquisition process of sphincter control in children with CP. It is an observational, cross-sectional and descriptive study, carried out with 31 caregivers of children with CP. An interview with 33 questions was conducted. Caregivers with an average age of 37.2 years, predominantly females with low educational and socioeconomic levels. Of the children, with an average age of 7.5 years, 45.16% acquired sphincter control, 87.1% of caregivers did not receive guidance for toilet training. In children with CP, there may be a delay in the acquisition of motor development, which was found in the study, sitting and walking are important developmental acquisitions to obtain sphincter control. Caregivers did not receive guidance regarding toileting. Of the studied children with CP, most have not yet acquired sphincter control despite being old enough to do so. [ABSTRACT FROM AUTHOR]

Introduction: Oxygen consumption (VO2) is indicative of cardiorespiratory fitness (CRF) and lower levels are related to a higher risk of total mortality among individuals with cancer whose therapy can have adverse consequences on the cardiovascular system. Objective: To examine the associations of patient-reported sociodemographic, clinical, anthropometric outcomes and functional variables with CRF in 69 women (55±10 years) and to identify whether walking is a predictor of peak VO2 variation in this population with breast cancer (BC). Method: Female BC survivors receiving hormone therapy after two types of surgery (breast-conserving and mastectomy) underwent a CRF test on a cycle ergometer to measure peak VO2. A questionnaire containing sociodemographic, clinical data, patient reported outcomes (PROs) (depressive symptoms, sleep quality, fatigue, body image) and self-reported walking and tests to measure body fat percentage, waist circumference, flexibility and shoulder range of motion (RoM) were performed. Results: Unemployment and retirement were associated with low CRF, as was the use of aromatase inhibitors instead of tamoxifen. Depressive symptoms, worse body image, greater waist circumference, less flexibility and shoulder RoM were also associated with low CRF. Walking duration, controlled for age and body mass index (BMI), is a 13% predictor of peak VO2 variance in this sample. Conclusion: These factors must be considered in understanding the CRF profile of BC survivors. As walking was a predictor of peak VO2 variance, it should be recommended as a type of physical activity for patients with BC using hormone therapy.

This paper had as its starting point the existence of a pedagogical project that is still carried out daily with children riding their bicycles in Praça da República, located in the city of São Paulo. The method employs two internet social networks, Instagram and WhatsApp, with which it is possible to generate images and conduct interviews and brief dialogues. It aims to answer some questions: What city within a city is manifested in the experiences of children riding their bikes in the Republic Square? What can we deduce from this practice what occurs or is possible to occur within a public space when we consider the presence of children, as, for example, in the Motoca na Praça project, when children actually officially occupy the square? Is there a production of space, even if ephemeral, that results from the presence of children? Perhaps a simpler question is, where is childhood located in this square? Historically is it possible to see it produced, present or absent, in the transformations of any given space by the presence of children? We rely on the philosopher Henri Lefebvre's notion of the production of social space to hypothesize what would follow from the production of urban spaces that were transformed by the presence of children of all ages, from infancy on. A presente pesquisa conceitua o comportamento de mão esquerda denominado Walking. Frank Koonce (1997), autor do termo, o descreve como o constante contato dos dedos com o espelho do braço do violão, propiciando, assim, fluência mecânica e sonora. Metodologicamente, procedemos a uma pesquisa bibliográfica acerca dos expedientes técnicos de sustentação do som que compõem o Walking, tendo como principal aporte teórico a Teoria da Digitação, de Alisson Alípio. Complementarmente, adotamos como referencial a abordagem da prática do violão denominada Aim- Directed Movement (ADM), proposta por Anderson (1980), por considerarmos que o Walking não pode ser compreendido unicamente como uma sucessão de ações dos dedos sobre as notas, mas — principalmente — como um conjunto de movimentos que permitem a sustentação do som entre tais ações. Os resultados desta pesquisa mostram, portanto, que o Walking é uma sucessão de intenções e que, como tal, é uma perspectiva motora que orienta o processo de digitação.

15. Factors associated with use of assistive walking devices among older adults in Brazil

Abstract Background Walking devices and other forms of assistive technology (AT) can benefit older adults by supporting mobility and social interactions, but usage outside of high-income countries is generally low. Objective To examine the factors associated with AT use and whether AT use is associated with higher levels of social participation among older adults in Brazil. Method The 2013 Brazilian National Health Survey interviewed 23,815 individuals 60 years or older. Descriptive and logistic regression analyses were used to examine AT use, including canes and walkers, to assist with walking and social participation. Results Among older adults with mobility difficulty, 34.0% (95% CI 31.2- 36.9) reported using AT. Prevalence of the use of AT for walking increases with age: 21.4% of those 60-69 years reported using AT while 58.5% of those 90 years or older did. AT was negatively associated with participation in social activities. Conclusion Our analyses focused on older adults with mobility limitations who need appropriate transportation and environment adaptations to engage socially. Contrary to studies in more developed countries, among Brazilians, AT use is negatively associated with social interactions. The resulting confinement seems to lead to social isolation.

Abstract Objective To determine the effect of treadmill walking on maternal heart rate (MHR) and cardiotocographic parameters (basal fetal heart rate [FHR], active fetal movements [AFM], number of accelerations and decelerations, and short-term variation [STV] and long-term variation [LTV] of fetal heart rate) in pregnant women at 36 weeks. Abstract Introduction Exercising regularly has benefits for people with bipolar disorder. Nevertheless, as a group, these patients tend to be less physically active than the general population and little is known from the viewpoint of the patients about the barriers and facilitators to such a practice. Objective To know the barriers and facilitators perceived by people with bipolar disorder for the practice of exercise. Methods This study had a descriptive, qualitative, exploratory nature. The investigation method used for data collection was a semi-structured in-depth interview, using grounded theory as theoretical framework. Results The data analysis generated two main areas of interest: adherence to regular physical exercise (barriers and facilitators) and the participants’ exercise history and perception of disease management, as described below. The main findings were: most of our sample did not exercise regularly, nor knew how exercise can positively influence their disorder; with regard to adherence to physical exercise, the presence of symptoms and stigma were the most important barriers to the practice of physical exercise. Social support, especially from family and friends, could be a facilitator to the practice of exercise. Conclusions Even considering the limitations for generalization of qualitative and exploratory studies, understanding perceived barriers and facilitators for the practice of exercise among people who suffer with bipolar disorder may contribute to the promotion of activities in which people with mental illness can participate.

Type 2 diabetes is a common, costly, and chronic disease that people with this disease have a lower level of health than other people. ABSTRACT The performances of healthy individuals in an incremental shuttle walking test performed in a hallway (ISWT-H) and on a treadmill (ISWT-T) were compared to assess their physiological responses during aerobic training sessions with the speeds estimated from both tests. This was a cross-sectional study with 55 healthy subjects, who were randomized to perform the ISWT tests with 24 hours between them. Training sessions were held using a treadmill at 75% of the speeds obtained from the ISWT-H and ISWT-T. Measurements included walking distance, oxygen uptake (VO2), carbon dioxide (VCO2) production, heart rate (HR), and ventilation (VE). There was a significant difference between walking distances (ISWT-T: 823.9±165.2 m and ISWT-H:685.4±141.4 m), but similar physiological responses for VO2 (28.6±6.6 vs. 29.0±7.3 ml-1.kg-1.min-1), VCO2 (1.9±0.7 vs. 1.9±0.5 1), HR (158.3±17.8 vs. 158.6±17.7 bpm), and VE (41.5±10.4 vs. 43.7±12.9 1). The estimated speeds were different for the training sessions (5.5±0.5 km/h and 4.9±0.3 km/h), as well as the VO2, VCO2, VE, and HR. It was concluded that in healthy young adults, ISWTs carried out in a hallway and on a treadmill are not interchangeable. Since the ISWT-H was determined to have lower speed, the training intensity based on this test may underestimate a patient’s responses to aerobic training.

ABSTRACT OBJECTIVE To evaluate the effect of implementation intentions as an intervention strategy to promote walking in adults with type 2 diabetes mellitus (T2DM). METHODS We conducted a controlled and randomized trial, with 12 months of follow-up, involving 65 people with T2DM recruited from primary health care units and allocated them in the control group (CG, n = 32) and intervention group (IG, n = 33). The IG received the implementation intention strategy to promote walking and the CG remained in follow-up for conventional treatment in primary health care. The researchers were blinded by anthropometric measurements and the filling of the instruments. RESULTS After twelve months of follow up, the IG presented a statistically significant increase in the leisure time physical activity when compared with the CG (p = 0.0413) and showed a significant decrease in waist circumference (p = 0.0061). No significant difference was observed regarding body mass index and glycated hemoglobin among groups. CONCLUSIONS Implementation intention was effective in promoting walking and improving clinical indicators in adults with T2DM.

Abstract We analyze the main reasons for the transport mode choice (private vehicle, public transportation, bicycle, walking, and others) among university students in Campinas. Analyses are based on primary data collected from 1,179 students through online and personal interviews in the second semester of 2014. Multiple Correspondence Analysis and Cluster Analysis are jointly used to identify common choice groups. Public transport prevails among low-income users, and the main motive for their choice is the lack of alternatives. Students who can afford higher costs tend to prefer a private vehicle, which is seen as being safer and more comfortable. In turn, the adoption of an active mode of transport remains mainly restricted to a small group of users with positive attitudes towards health and the environment. The study finally discusses the implications of structural constraints, such as infra-structure and public safety, on public policies aimed at a sustainable urban transport system.

This case report describes the exercise program on a hospitalized 54-year-old male patient with cardiogenic shock waiting for a heart transplant assisted by an intra-aortic balloon pump, a temporary mechanical circulatory support device. The temporary mechanical circulatory support device, an intra-aortic balloon pump, was placed in the left subclavian artery, enabling the exercise protocol. Measurements and values from Swan-Ganz catheter, blood sample, brain natriuretic peptide (NT-proBNP), and high-sensitivity C-reactive protein (hs-CRP), as well as the six-minute walk test (6MWT) and venous oxygen saturation (SvO2) were obtained before and after an exercise protocol. The exercise training protocol involved the use of an unloaded bed cycle ergometer once a day, for a maximum of 30 minutes, to the tolerance limit. No adverse events or events related to the dislocation of the intra-aortic balloon pump were observed during the exercise protocol. The exercise program resulted in higher SvO2 levels, with an increased 6MWT with lower Borg dyspnea scores (312 meters vs. 488 meters and five points vs. three points, respectively). After completing the ten-day exercise protocol, the patient underwent a non-complicated heart transplant surgery and a full recovery in the ICU. This study showed that exercise is a feasible option for patients with cardiogenic shock who are using an intra-aortic balloon pump and that it is well-tolerated with no reported adverse events.

The objective was to raise the apprehensions, feelings and expectations faced by parents regarding the transmission of the diagnosis of atypical development from a physical therapy viewpoint. This is a descriptive study of qualitative analysis, made with parents of children with atypical development, referred to physical therapy. A questionnaire was formulated with open questions and topics such as the presentation of the diagnosis and expectations related to the child's future. The data obtained were preanalyzed, explored, and the results were processed for conclusion and interpretation. Eleven parents were evaluated. The diagnoses presented were Down syndrome, non-progressive chronic encephalopathy, brain malformation and severe cerebral anoxia in children with a mean age of 33.8 ± 28.7 months. The transmission of the diagnosis was made by pediatricians, after birth, directly, only one case there was emotional support. Many parents had difficulties accepting the diagnosis, their expectations for the future were: walking, studying, professional success, independence, being happy and even none, expecting to be surprised. Abstract Objective : to investigate the association between utilitarian walking and walking as exercise, and socio-demographic, clinical and functional covariates related to these walking types. Methods : a cross-sectional exploratory study was conducted with 148 older adults (aged 60 and over). Walking frequency and duration was assessed using the IPEQ-W (Incidental and Planned Exercise Questionnaire - Version W). Socio-demographic, clinical conditions, level of disability and mobility were also assessed. Types of walking were compared among the variables using the Mann-Whitney test and non-parametric Spearman rho correlations were used to investigate the association between the types of walking and the variables. Results : the participants performed a mean of 1.1 (±2.1) h/week of walking as exercise and 2.2 (±2.3) h/week of utilitarian walking. ABSTRACT OBJECTIVE: To verify the prevalence and factors associated with active transportation to school (ATS) among Brazilian adolescents attending public schools. METHODS: Crossectional study with a representative sample of 1,984 adolescents (55.9% girls). Sociodemographic variables included were: gender, age, parental schooling, and socioeconomic status. Psychosocial factors included were: social support from parents and friends for physical activity. Walking, cycling, or skateboarding to school were considered models of active transportation. Binary logistic regression models verified sociodemographic and psychosocial factors association with ATS, adopting p < 0.05. RESULTS: The prevalence of active transportation to school was 37.7% (16.2% boys and 21.5% girls). For boys, ATS was associated with: social support from parents in practicing physical activity together as a family (OR = 1.57; 95%CI 1.09–2.25), giving them rides (OR = 1.56; 95%CI 1.04–2.32), and remarking their good performance on it (OR = 1.73; CI95 1.08–2.76); as well as the social support from friends in practicing physical activity together (OR = 2.23; 95%CI 1.35–3.69). For girls, the likelihood of using ATS increased with age (OR = 1.43; 95%CI 1.06–1.92) and having friends who practice physical activity together with them (OR = 1.48, 95%CI 1.04–2.10). CONCLUSION: Age and social support for physical activity were associated with ATS. Parents who practice together, give rides, and remark on physical activities increase the likelihood of adolescent boys using ATS. Social support from friends to physical activity increased the likelihood of both genders using ATS.

ABSTRACT Objective: To verify the reproducibility of the six-minute walk test (6MWT) performance and its physiological variables in healthy students. Methods: This is as prospective cross-sectional study. The sample consisted of healthy students aged 6-12 years old from public and private schools in the region of Florianópolis City, Santa Catarina State, (Southern Brazil). The medical state was considered according to the health records and scores on the International Study of Asthma and Allergies in Childhood (ISAAC) and the spirometric values of forced expiratory volume in the first second and forced vital capacity above 80% of what was predicted. Two 6MWTs were conducted with a 30-minute interval between them, following the recommendations from the American Thoracic Society. Physiologic variables were recorded using the portable telemetric gas analyzer K4b2 (Cosmed®, Italy). For analysis, the dyspnea index, the perception of effort and performance variables identified in both 6MWT were considered. Data distribution was verified with the Shapiro-Wilk test and statistical analysis included paired t-test or Wilcoxon test, and intraclass correlation coefficient (ICC). The significance level adopted was 5%. Results: A total of 22 students with a mean age of 10.2±1.5 years participated in the study. The covered distance and the variation of oxygen consumption reproducibility between the two 6MWTs presented ICC=0.76 and ICC=0.86, respectively. There was also similar behavior of the physiological variables when comparing the two tests (p=0.001), especially the minute volume (MV), the oxygen consumption (VO2), and the carbon dioxide production (VCO2). Conclusions: The 6MWT showed reproducible values, both in performance and physiological parameters, in the healthy students analyzed.

The present research conceptualizes the left-hand behavior called Walking. Frank Koonce (1997), author of the term, describes it as the constant contact of the fingers with the fingerboard of the guitar, thus providing mechanical and sound fluency. Methodologically, we proceeded to a bibliographical research about the technical expedients of sustaining the sound that compose Walking, having as main theoretical support the Theory of Fingering, by Alisson Alípio. In addition, we adopted as a reference the approach to guitar practice called Aim-Directed Movement (ADM), proposed by Anderson (1980), considering that Walking cannot be understood only as a succession of finger actions on the notes, but -- mainly -- as a set of movements that allow the sound to sustain between such actions. The results of this research show, therefore, that Walking is a succession of intentions and that, as such, it is a motor perspective that guides the fingering process. [ABSTRACT FROM AUTHOR]

The Chronic Obstructive Pulmonary Disease (COPD) is characterized by chronic airflow obstruction or limitation, and pulmonary rehabilitation (PR) corresponds to a therapy that requires returning patients with greater functional capacity, with therapeutic physiotherapy being an essential component of PR. The six-minute walk test (6MWT) is used as a practical way to assess the physical capacity of COPD patients after the training program. This study aimed to highlight possible changes in the physical capacity of the individual with COPD after a physical therapy intervention using the 6MWT '. A sample was selected for 37 COPD patients with a mean age of 64.29 years (+10.29), being applied by the 6MWT and by the incremental test below the maximum on the cycle ergometer. Participated in an 8-week physical therapy intervention program, twice a week lasting 50 minutes and subsequently reassessed. As the distance media covered in the 6MWT 'before and after training were respectively 152.86 m and 250.0 m, with no statistical difference (p = 0.138). However, there was an average increase of 97.14 meters in the distance covered after the intervention. A significant reduction in HR (p = 0.03) was noted without incremental testing on the cycle ergometer after the intervention. The practice of group-oriented walking is increasingly intensified in communities, especially with elderly, in which the benefits of daily activities and other parameters that influence the quality of life of these patients. This study aimed to identify, evaluate and compare the functional capacity of two elderly groups practitioners and non-practitioners physical activity in Farroupilha-RS. We evaluated 30 elderly women allocated in two groups, according to the practice or not of regular physical exercise. Six minute walk test (6MWT) was performed according to ATS protocol (2002), which also includes measurement of blood pressure (BP), heart rate (HR), respiratory rate (RR) and peripheral saturation (SpO2). Statistical analyses was performed using SPSS Statistic Data 21 (StatisticalPackage for Social Sciences), through Student's test, Mann-Whitney and qui-square test, considering p-value=0.05. There were differences between GCAN and GCON regarding initial systolic blood pressure (p-value=0.05), final systolic blood pressure (p-valeu=0.01) and final SpO2 (p-value=0.01). In conclusion guided walking proved to be effective in reducing blood pressure levels and peripheral saturation and the study was equivalent to other studies related to the distance covered in the 6MWT. A functional capacity and a full maintenance of the physical and mental abilities developed throughout the life. The study aimed to analyze self-perception about its functionality for Daily Life Activities (AVDS), relating the performance tests of the upper and lower limbs. This is a crosssectional study with longevous residents from Galópolis neighborhood of Caxias do Sul, RS. It was used the AFASII questionnaire, Manual Grip Test, Chair Sit and Lift Test and Timed Up Go Test. Thirty - one individuals with mean age of 83.23 years were investigated, being 74.19% female. The self-perception of general health was described by the longevous as good (14) and regular (12). An ease of execution such as AVDS (24), difficulty in climbing 10 steps or a ladder of stairs (10) and difficulty in lowering itself to an object on the ground (7). Longevous that reported ease of use of the AFASII questionnaire to carry bags and heavy objects, sit and stand without the use of hands, walk 400 meters and leave the house presented better performance in the Manual Grip test, Sit and Lift and Timed Up and Go tests. Most of the longevous referred a good perception of health, presented good performance in the tests and functional capacity higher than expected. [ABSTRACT FROM AUTHOR]

ABSTRACT Several exercise modalities improve the symptoms of Parkinson’s Disease (PD). Among the variety of physical exercises, Nordic walking has been used. The aim of this study was to summarize scientific literature on effects of Nordic walking on patients with PD by a systematic review of randomized clinical trials. The following electronic databases were selected: MEDLINE by Pubmed, Cochrane, PEDro, SCOPUS and Web of Science and articles identified by manual search, without restriction of date and language. The reviewers evaluated the articles and selected studies according to the eligibility criteria. The following data were extracted from the selected studies: publication identification, participants’ characteristics (sex, age, disease stage, duration of disease), experimental intervention characteristics, control group characteristics, duration, follow-up time, outcome measures and main results. Nordic walking programs with moderate and high intensities, with a minimum of 12 sessions of 60 minutes in a period from 6 to 24 weeks promoted positive effects on the severity, gait, balance, quality of life, functional capacity and motor function in patients with PD.

Background: Association of Income Level and Ischemic Heart Disease: Potential Role of Walkability Association of ischemic heart disease (adjusted for traditional risk factors and socioeconomics variables) and income level (A), and walkability z-score (B), and association of walkability z-score and income level (C)., Background: Socioeconomic status has been linked to ischemic heart disease (IHD). High-income neighborhoods may expose individuals to a walking-promoting built environment for daily activities (walkability). Data from the association between income and IHD is lacking in middle-income countries. It is also uncertain whether walkability mediates this association., Objectives: To investigate whether income is associated with IHD in a middle-income country and whether neighborhood walkability mediates the income-IHD association., Methods: This cross-sectional study evaluated 44,589 patients referred for myocardial perfusion imaging (SPECT-MPI). Income and walkability were derived from participants' residential census tract. Walkability quantitative score combined 4 variables: street connectivity, residential density, commercial density, and mixed land use. IHD was defined by abnormal myocardial perfusion during a SPECT-MPI study. We used adjusted mixed effects models to evaluate the association between income level and IHD, and we performed a mediation analysis to measure the percentage of the income-IHD association mediated by walkability. We considered p values below 0.01 as statistically significant., Results: From 26,415 participants, those living in the lowest-income tertile census tract were more physically inactive (79.1% versus 75.8% versus 72.7%) when compared to higher-income tertile census tracts (p < 0.001). Income was associated with IHD (odds ratio: 0.91 [95% confidence interval: 0.87 to 0.96] for each 1,000.00 international dollars increase in income) for both men and women equally (p for interaction = 0.47). Census tracts with a higher income were associated with better walkability (p < 0.001); however, walkability did not mediate the income-IHD association (percent mediated = -0.3%)., Conclusions: Income was independently associated with higher prevalence of IHD in a middle-income country irrespective of gender. Although walkability was associated with census tract income, it did not mediate the income-IHD association.

The motor performance tests are important indicators to identify the motor capacity of the elderly person, as they allow us to assess the performance in activities related to daily life, which sometimes undermine quality of life. From this perspective, this study aims to analyze the association between motor performance and quality of life of elderly people living in community. This is an exploratory-descriptive and analytical research, with a cross-sectional and quantitative approach, performed in a Family Health Unit, with a sample of 66 elderly people of both genders. We collected information on sociodemographic topics and health conditions. The data on motor fitness were obtained by means of the flexibility/mobility, sit-to-stand, 2.44m-walk and balance tests; and, in order to assess quality of life, we used the WHOQOL-bref questionnaire. We performed descriptive statistical analysis and applied the Pearson's Chi-square test (χ²) with p≤0,005. We found a significant statistical difference among the variables on balance and physical domain (p=0.001), psychological domain (p=0.001), social relationships domain (p=0.001) and environment domain (p=0.001). We noted that there was an association between motor performance (balance activity) and quality of life (psychological domain, social relationships and environment) in the surveyed elderly individuals. Several limitations are detected in individuals with degeneration of knee joint. These changes are evaluated in parallel, excluding the fact that they should bring results to the same situation: the evaluation of the functionality, described in the International Classification of Functioning Disability and Health (ICF) by the World Health Organization (WHO). Objective: to relate the evaluated items in several instruments and tools of quantification of the functionality with the ICF in their domains and severity qualifiers. Methods: The items of the evaluation tools used were in association with the codes contained in the ICF, as well as the severity levels of the instruments with the ICF qualifiers, according to Farias Neto et al (2016) protocol. The instruments used were: Western Ontario and McMaster Universities Osteoarthritis Index, sit back and raise Test, Six-Minute Walk Test, Octobalance, Dorsiflexion Test and Radiographic Evaluation. Results: the highest category in these instruments was related to mobility. No instruments were found that related to social aspects and environmental aspects of the ICF. Conclusion: according to the results found, the instruments used did not include all aspects of functionality described by the ICF, mainly items related to mobility. [ABSTRACT FROM AUTHOR]
