Objective. To provide an overview of the status of the childhood vaccination schedule in the Americas, outline program structures, and identify updated implementation strategies to improve vaccination coverage following the COVID-19 pandemic. Methods. A group of experts in pediatrics, epidemiology, vaccines, and global and public health discussed the current status of the childhood vaccination schedule in the Americas, describing the program structure and identifying new implementation strategies that have the potential to improve vaccination coverage in the post-pandemic context, after the challenges COVID-19 presented for more than two years. Results. The Americas currently face a high risk of resurgence of diseases that were previously controlled or eliminated. Therefore, it is important to find new strategies to educate citizens on the risks associated with lower vaccination rates, especially in children. Conclusions. New strategies along with strong mobilization of the population and advocacy by citizens are necessary to prevent antivaccination groups from gaining a stronger presence in the region and jeopardizing the credibility of the Expanded Program on Immunization.

The Pan American Health Organization (PAHO) and its Member States have been leading the efforts to eradicate wild poliovirus in the Region of Americas since smallpox's successful elimination in 1971. The region became the first to be certified free of wild poliovirus in 1994. However, in July 2022, an unvaccinated patient with no recent travel history was diagnosed with poliomyelitis in the United States of America. In response to the emergence of a circulating vaccine-derived poliovirus in the United States, PAHO established the Polio Incident Management Support Team. This team has been coordinating response efforts, focusing on: coordination, planning, and monitoring; risk communication and community engagement; surveillance and case investigation; vaccination; and rapid response. In this paper, we identified and documented best practices observed following establishment of the Incident Management Support Team (September 2022–2023) through a comprehensive review and analysis of various data sources and country-specific data from the polio surveillance dashboard. The aim was to share these best practices, highlighting technical support and implementation of polio measures by Member States. Despite several challenges, the Americas region remains polio-free. Polio risk is declining, with a July 2023 assessment showing fewer countries at medium, high, and very high risk. This progress reflects improved immunization coverage, surveillance, containment, health determinants, and outbreak preparedness and response. The PAHO Polio Incident Management Support Team has played a key role in supporting these efforts.

ABSTRACT Objective To describe vaccination coverage and hesitation for the basic children’s schedule in Belo Horizonte and Sete Lagoas, Minas Gerais state, Brazil. Methods Population-based epidemiological surveys performed from 2020 to 2022, which estimated vaccine coverage by type of immunobiological product and full schedule (valid and ministered doses), according to socioeconomic strata; and reasons for vaccination hesitancy. Results Overall coverage with valid doses and vaccination hesitancy for at least one vaccine were, respectively, 50.2% (95%CI 44.1;56.2) and 1.6% (95%CI 0.9;2.7), in Belo Horizonte (n = 1,866), and 64.9% (95%CI 56.9;72.1) and 1.0% (95%CI 0.3;2.8), in Sete Lagoas (n = 451), with differences between socioeconomic strata. Fear of severe reactions was the main reason for vaccination hesitancy. Conclusion Coverage was identified as being below recommended levels for most vaccines. Disinformation should be combated in order to avoid vaccination hesitancy. There is a pressing need to recover coverages, considering public health service access and socioeconomic disparities.

ABSTRACT Objective To describe timely vaccination completion and obstacles in the first 24 months of life in Brazil, examining associations with maternal race/skin color. Methods Study participants were 37,801 children born in 2017 and 2018 included in the National Immunization Coverage Survey. We calculated prevalence and 95% confidence intervals for timely vaccine completeness and obstacles at 5, 12 and 24 months of life, according to maternal race/skin color. Associations were analyzed using logistic regression. Results 7.2% (95%CI 6.3;8.2) of mothers faced difficulties in taking their children to be vaccinated, and 23.4% (95%CI 21.7;25.1) were not vaccinated when taken. These proportions were 75% (95%CI 1.25;2.45) and 97% (95%CI 1.57;2.48) higher, respectively, among Black mothers. At least one vaccination was delayed among 49.9% (95%CI 47.8;51.9) and 61.1% (95%CI 59.2;63.0) of children by 5 and 12 months, respectively. These rates were higher among Black/mixed race mothers. Conclusion There are racial inequalities in both the obstacles faced and in vaccination rates in Brazil.

Abstract Objective: analyzing the effectiveness of an educational intervention on the knowledge of nursing professionals regarding the immunization of people with the human immunodeficiency virus. Method: a quasi-experimental study evaluated professionals’ knowledge through a knowledge test applied before and after the development of an online training course. The data was analyzed using frequency, median, mean, standard deviation, and association tests. Results: the sample consisted of 77 nursing professionals whose mean age was 43.2 years (SD+/-8.2). More than half of the individuals worked in basic health units (58.4%), 22.1% worked in specialized services that provide clinical monitoring for people with the human immunodeficiency virus, and 42 (54.5%) were nursing assistants or technicians. ABSTRACT Objective: To analyze the vaccination coverage and abandonment rates among children under two years old in Brazil, from 2015 to 2021. Methods: A time-series ecological study. The dependent variables of the research were "vaccination coverage" and "abandonment rate", both assessed by Brazilian region. The data were extracted in July 2022 from the Information System of the National Immunization Program. The Prais-Winsten technique was used for the trend analysis, with the aid of the STATA 16.0 software. Results: The mean vaccination coverage in Brazil was 76.96%, with a decreasing trend during the period (Annual Percent Change=-5.12; confidence interval — CI95% -7.81; -2.34); in 2015, the rate was 88.85% and it dropped to 62.35% in 2021. In turn, the overall abandonment rate was 24.00% in 2015 and 9.01% in 2021, with a mean of 10.48% and a stationary trend (Annual Percentage Change=-9.54; CI95% -22.92; 6.12). In 2021, all the vaccines presented coverage values below 74.00% in the country. Conclusions: The vaccination coverage rate trend among children under two years old was stationary or decreasing for all the immunobiologicals in all Brazilian regions, with the exception of yellow fever in the South and Southeast regions. There was an increase in the abandonment rate trend for the Meningococcal C vaccine in the country and, specifically in relation to the regions, for BCG in the North, Northeast, and Midwest and for Meningococcal C in the North and Northeast.

ABSTRACT Objective: To evaluate the impact of the state action-research project on vaccination coverage in children under two years of age in the state of Minas Gerais, according to the size of the municipalities, comparing the years 2021 and 2022. Methods: This is a study nested within the state action-research project, a before-after community clinical trial carried out in 212 municipalities in the state of Minas Gerais. This study used secondary data on Vaccination Coverage (VC), Homogeneity of Vaccines (HVC) and Abandonment rate of multi-dose vaccines. After classifying municipalities by size and vaccination coverage rates were equitably classified, an analysis of secondary data on 12 immunobiologicals indicated for the age group in question and their abandonment rate of multi-dose vaccines was carried out. Results: There was an increase in the proportion of municipalities classified as small that reached the vaccination coverage target set by the National Immunization Program (PNI) after the action-research project was carried out. There was an increase in the proportion of small municipalities classified as having a low abandonment rate for the rotavirus vaccine, in the adequate homogeneity of vaccination coverage and in the classification of risk as very low risk and low and medium risk, all with a statistically significant difference. Conclusion: There was an influence of municipal size on the effectiveness of the actions applied to increase vaccination coverage, explaining that proposing individualized actions for each municipality is essential to improve vaccination coverage.

Abstract Objective: To analyze the temporal trend of human papillomavirus (HPV) vaccination coverage among the female population aged 10 to 14 years, living in the state of Goiás, Brazil, between 2014 and 2022. Methods: This was an ecological time series study using data from the Brazilian National Health System Information Technology Department (Departamento de Informática do Sistema Único de Saúde - DATASUS); the annual vaccination coverage rate was calculated based on the number of second doses administered; the trend of the rates was analyzed using the Prais-Winsten model. Results: A total of 407,217 second doses of the quadrivalent HPV vaccine were administered to the female population aged 10-14 years, with annual vaccination coverage rates ranging from 12.3% (2019) to 30.0% (2015), and an annual percentage change (APC) of 0.7% (95%CI 0.9; 0.2; p-value = 0.030). Conclusion: In Góias state, the quadrivalent HPV vaccine coverage rate was below the national target (80%), showing a stationary trend in the time series.

Objective: to identify factors associated with adherence to the COVID-19 vaccine during pregnancy. Method: cross-sectional and analytical study with 348 postpartum women in shared accommodation at the Municipal Maternities of Recife-PE. Data was collected through interviews during the months of June to September 2022. Pearson’s Chi-Square or Fisher’s Exact tests and the Poisson regression model were applied for statistical analysis. Results: 17.2% of pregnant women adhered to the complete vaccination schedule, and adherence was associated with access to the internet/TV/radio (p-value = 0.011), routine prenatal vaccination (p-value = 0.019), safety of the efficacy of the COVID-19 vaccine and partner support (p-value = 0.020). Postpartum women without access to the internet/TV/radio, and who feel confident about the effectiveness of the vaccine, had higher prevalence rates for adhering to COVID-19 vaccination, with PRs of 2.56 and 3.25, respectively. Conclusion: there was evidence of low adherence to the vaccination schedule against COVID-19 during the gestational period, considering the number of recommended doses and the interval between them. Therefore, professionals in their clinical practice must make pregnant women aware of the importance of immunization and compliance with the vaccination schedule.

ABSTRACT The immunization campaign against COVID-19 started in Brazil in January 2021 after strong pressure from society on the federal government, which had created a series of ideological obstacles against vaccines, especially those produced with Chinese inputs. This article analyzes the impact of far-right ideology on the spatial distribution of vaccine coverage against COVID-19 in Brazilian municipalities. By means of hierarchical models, it was identified that, maintaining constant socio-demographic characteristics and the structures of the Unified Health System, the degree of bolsonarismo in the municipalities had a negative impact on the coverage rates of the first, second and, especially, of the third dose.

ABSTRACT OBJECTIVE Analyze the impact of the state research-action project on immunization indicators (vaccination coverage – VC, homogeneity of vaccination coverage – HVC, dropout rate – DR, and risk rating) before and after the intervention in municipalities and priority Regional Health Administrations/Regional Health Superintendencies (RHA/RHS). METHODS The state research-action project was a before-after community clinical trial conducted in 212 municipalities belonging to eight RHA/RHS in the state of Minas Gerais, Brazil. The study sample comprised RHA/RHS with a decreasing trend for routine vaccination coverage in children under one year from 2015 to 2020. This study used secondary VC and DR data from 10 immunobiologicals recommended for children younger than two years from January to December 2021 (pre-intervention period, prior to the state research-action project) and from January to December 2022 (post-intervention period). The categorical variables were presented in proportions, and initially, a comparison was made between those of DR, HVC, and the risk rating for the transmission of vaccine-preventable diseases, according to the two periods (2021 and 2022), using the McNemar test. RESULTS All immunization indicators increased after conducting the research-action project. In 2021, 80.66% of the state’s municipalities had a risk rating for the transmission of vaccine-preventable diseases as “high and very high.” In 2022, the value reduced to 68.40%. CONCLUSIONS Risk rating for the transmission of vaccine-preventable diseases is an important mechanism to assist managers in defining priorities. The state research-action project used a method that enabled the construction and execution of unique action plans for each municipality, directing the improvement of immunization indicators in the state.

Abstract: Brazil has seen a decrease in vaccination coverage since 2016. This study analyzes the immunization status of children born during the COVID-19 pandemic in Fortaleza, Northeastern Brazil. This is a longitudinal analysis that included vaccination data of 313 children aged 12 and 18 months. Vaccination cards were checked for dose application considering the schedule of immunization recommended by the Brazilian Ministry of Health. Factors associated with no retention of vaccination cards and incomplete immunization by 18 months were identified by Tobit regression analysis. About 73% of mothers presented their child’s vaccination card. Non-availability of vaccination cards was associated with maternal age < 25 years and mothers with paid jobs. Only 33% and 45% of the children aged 12 and 18 months had all vaccines up to date, respectively. For 3-dose vaccines, the delay rate was around 10% for the first dose application, but 40% for the third dose. Despite delays, most children with available vaccine cards had coverage above 90% by 18 months of age. Adjusted factors associated with incomplete vaccination included living in a household with more than one child (p = 0.010) and monthly income of less than one minimum wage (p = 0.006). Therefore, delays in child vaccine application were high during the COVID-19 pandemic but a considerable uptake by 18 months of age was found. Poorer families with more than one child were particularly at risk of not fully immunizing their children and should be the target of public policies.

ABSTRACT Objective: The study aimed to analyze the factors that affect the community’s attitude towards COVID-19 vaccination in Tangerang District. Methods: A cross-sectional study was used. Convenience sampling was used to select 400 respondents. Inclusion criteria are living in Tangerang District, aged between 18 and 55, and earning a living. An online questionnaire was used and passed validity and reliability tests. This study received ethical approval. Results: Most respondents had a high level of education (48.50%), low income (72.50%), high knowledge (78%), and a positive attitude (76.50%) regarding vaccination against COVID-19. The Chi-square test revealed a correlation between knowledge and attitudes towards COVID-19 vaccination (p=0.001), as well as education levels (p=0.001), but there was no correlation between income and attitudes (p=0.094). Conclusions: Health professionals should engage in extensive socialization and face-to-face visits with people with limited access to information to promote a positive attitude and expand the scope of COVID-19 vaccination.

ABSTRACT Objectives: to synthesize scientific evidence on vaccine hesitancy in children under five years of age and its associated factors. Methods: a scoping review, conducted according to the methodological structure proposed by the JBI. Searches were carried out in the Latin American and Caribbean Center on Health Sciences Information, Scientific Electronic Library Online and PubMed databases, including gray literature. Studies in English, Spanish and Portuguese were included, without temporal delimitation. Editorials, studies that did not address vaccine hesitancy in children under five years of age and were not aligned with the objective and research question were excluded. The sample consisted of 18 articles. Results: misinformation, concern about adverse effects, distrust about efficacy, affliction regarding administration simultaneously, and insecurity in relation to the laboratories were the reported reasons. Conclusions: strategies are needed to combat the lack of information about immunobiological agents, as misinformation was the main factor in parents’ vaccine hesitation.

Introduction: Vaccination makes it possible to eradicate, eliminate or control immunopreventable diseases globally. The new coronavirus pandemic is known to have challenged health systems worldwide in the delivery of essential services, including immunization programs, because routine vaccination and mass vaccination campaigns could contribute to the spread of COVID-19. The World Health Organization (WHO) estimates that at least 80 million children will be susceptible to immunopreventable diseases because of failing vaccination coverage during the pandemic. Objectives: The present study aimed to evaluate the possible drop in childhood vaccination coverage in the state of Minas Gerais during the pandemic of COVID-19. Methods: This is a descriptive, cross-sectional type study, based on the collection of secondary data available in DATASUS. Results: The pandemic caused by COVID-19 and its consequences influenced negatively the collective immunization plan for children in the state of Minas Gerais, evidencing a drop in vaccination coverage. Strategies aimed at immunization actions and updating children's vaccination schedules are essential in order to avoid the recrudescence of already eradicated or controlled diseases.

ABSTRACT Objective: The aim of this study was to analyze the temporal trends in vaccination coverage (VC) during the first year of life of children in Brazil. Methods: Data on VC for the first year of life from 2011 to 2020 for Bacille Calmette-Guerin (BCG), hepatitis B, polio, pentavalent, and triple viral vaccines at the national, regional, and state levels were obtained from the Information System of the National Immunization Program. Trends were analyzed using Prais-Winsten generalized linear regression models and average annual percent change (APC) estimates. Results: Decreasing trends were observed for the BCG (APC −3.58%; p0.05). During the study period, no increasing trends were observed in any territory or vaccine. Conclusions: This study shows a recent significant reduction and decreasing trends in VC during the first year of life of children in Brazil, indicating the need for interventions to curb this ongoing phenomenon and to recover acceptable VC rates in the country.

Abstract Objective: to analyze the temporal trend of coverage and dropout rate for triple viral vaccine in Brazil, according to the country’s Federative Units and Macro-Regions, between 2014 and 2021. Methods: this was an ecological time series study, using data from the National Immunization Program Information System and the Live Birth Information System; joinpoint regression models were used. Results: in Brazil as a whole annual vaccination coverage was below 95% and ranged from 92.3% (2015) to 54.4% (2021); the second dose of the vaccine showed a decreasing temporal trend in the period (average change over the period = -5.8; 95%CI -10.5;-0.8); the temporal trends were stationary and decreasing in the country’s Federative Units; the dropout rate ranged from 22.2% (2014) to 37.4% (2021). Conclusion: there was a downward trend in vaccination coverage and an increase in the dropout rate in Brazil as a whole and in the country’s Federative Units.

Objective. To document the evolution of socioeconomic and geographical inequalities in childhood vaccination in Mexico from 2012 to 2021. Methods. Repeated cross-sectional analysis using three rounds of National Health and Nutrition Surveys (2012, 2018, and 2021). Dichotomous variables were created to identify the proportion of children who received no dose of each vaccine included in the national immunization schedule (BCG; diphtheria, pertussis, and tetanus-containing; rotavirus; pneumococcal conjugate; and measles, mumps, and rubella [MMR]), and the proportion completely unvaccinated. The distribution of unvaccinated children was analyzed by state, and by socioeconomic status using the concentration index. Results. The prevalence of completely unvaccinated children in Mexico was low, with 0.3% children in 2012 and 0.8% children in 2021 receiving no vaccines (p = 0.070). Notwithstanding, for each vaccine, an important proportion of children missed receiving any dose. Notably, the prevalence of MMR unvaccinated children was 10.2% (95% CI 9.2–11.1) in 2012, 22.3% (95% CI 20.9–23.8) in 2018, and 29.1% (95% CI 26.3–31.8) in 2021 (p < 0.001 for the difference between 2012 and 2021). The concentration index indicated pro-rich inequalities in non-vaccination for 2 of 5 vaccines in 2012, 3 of 5 vaccines in 2018, and 4 of 5 vaccines in 2021. There were marked subnational variations. The percentage of MMR unvaccinated children ranged from 3.3% to 17.9% in 2012, 5.5% to 36.5% in 2018, and 13.1% to 72.5% in 2021 across the 32 states of Mexico. Conclusions. Equitable access to basic childhood vaccines in Mexico has deteriorated over the past decade. Vigilant equity monitoring coupled with tailored strategies to reach those left out is urgently required.

Introduction: Human Papillomavirus (HPV) infection is the most common sexually transmitted infection in women. About 80% of sexually active women will have contact with this virus at some age in their lives. Most infections will be transient, but when the infection becomes persistent, associated with high oncogenic risk HPV, there may be progression to cancer, especially cervical cancer. The best way to prevent HPV infection is through the use of vaccines. Objective: To assess which are the most prevalent types of HPV in the city of Florianópolis, Brazil and if the majority of the diagnosed types are contained in the HPV vaccines currently available on the market and in the public health sector. Methods: More than 14,727 HPV tests were evaluated for the diagnosis of genital HPV infection in women from Florianópolis. The prevalence of infection was evaluated according to age of the women. HPV detection was performed using molecular biology tests, such as hybrid capture (for diagnosis of the HPV group, high or low oncogenic risk) and PCR (viral genotyping) techniques. Results: The diagnosis of HPV infection was made for women between one and 102 years of age. The highest positivity of the exams was observed in women aged 20–25 years (51% of the exams). The most prevalent age group was 31–35 years old (23.5%), and the lowest was for women aged 70 and above (0.6%). High oncogenic risk HPV was detected in 94.1% of positive samples and was the most frequent in all age groups. Mixed infection (high- and low-risk HPV) was more prevalent in the 66–70 age group (25.6%). The most frequent genotypes were non-16/18 high oncogenic risk HPV (77% of positive cases). HPV 16 was found in 17.1% of positive cases, and HPV 18 in 6.5%. Conclusion: The most prevalent types of HPV in Florianópolis in the last 6 years are non-16/18 high oncogenic risk HPV types, viral types not covered by the current HPV vaccine available in the public health sector in Brazil.

ABSTRACT Objective: to analyze vaccination coverage spatial distribution in children under one year old and the socioeconomic factors associated with meeting the recommended goals in Minas Gerais. Methods: an ecological study, carried out in 853 municipalities in the state. Pentavalent, poliomyelitis, meningococcal conjugate, yellow fever, rotavirus, and 10-valent pneumococcal conjugate vaccination coverage were analyzed. Scan statistics and multiple logistic regression were performed to identify spatial clusters and factors associated with meeting coverage goals. Results: spatial analysis revealed clusters with risk of low coverage for all vaccines. Number of families with per capita income of up to 1/2 wage, Minas Gerais Social Responsibility Index and percentage of the poor or extremely poor population were associated with meeting the established goals. Conclusions: the results are useful for designing interventions regarding the structuring of vaccination services and the implementation of actions to increase vaccination coverage in clusters with less propensity to vaccinate.

Vaccines are considered one of the most important public health advancements of the modern age and the most cost-effective preventive method of curbing the spread of several diseases with high morbidity and lethality. However, vaccination coverage rates around the world have been threatened by the vaccine hesitancy phenomenon, considered by the World Health Organization (WHO) as one of the ten greatest menaces to public health in the world. Vaccine opposition is a mentality driven by various interests, as old as the vaccine itself, predominantly using the media and social networks, which have an enormous influence on people. They often spread false information about vaccines, frequently with no adequate scientific backing, negatively impacting vaccination rates. To understand the possible correlation between the information spread on social media and childhood vaccination rates, we performed a narrative review on the subject, with a qualitative approach, in original or review articles, indexed in national and international databases and/or journals, in Portuguese and English, published between 2011 and 2021, available in full and free of charge. One hundred and seventy-two articles were extracted from the literature, 49 of which were submitted to analysis. The use of content analysis in a narrative literature review made it possible to interpret the main publications that in the last decade point to the impact of social media as one of the main factors linked to vaccine hesitancy in children. The articles show that communication is presented as a determinant for adherence or vaccine hesitancy, and social media are a useful tool for disseminating information that can influence and mobilize the population, promoting its acceptance or having a negative impact on childhood vaccine coverage rates. Social Media has become fertile ground for the creation and spread of fake news, hence the importance of the health professional's engagement in strengthening the population's confidence in relation to immunization, minimizing vaccine hesitancy.

The study investigates the epidemiological aspects of hepatitis B in children and adolescents, discussing the results found in the light of bioethical principles. This is a cross-sectional, retrospective study, based on a time series of cases, whose notification data were extracted from the Notifiable Diseases Information System (SINAN) and vaccination data from the National Immunization Program Information System - SIPNI, from 2011 to 2020. 889 cases of hepatitis B were reported in the study population. Most participants were aged between 15 and 19 years old 738 (83%), female 549 (62%), brown skin color 637 (72%) living in the urban area 490 (55%) and with incomplete elementary education 279 (31%). The probable source of infection was through sexual intercourse 450 (51%). The most common clinical form was Chronic Hepatitis 496 (56%). Most of those infected with Hepatitis B were unvaccinated individuals 298 (34%). In the light of bioethics, the results show that the practice of vaccination can contemplate bioethical principles, since the degree of autonomy depends on a type of planned intervention and the greater the relationship between benefit and burden, taking into account the principles of beneficence and not maleficence, the less decisive is this autonomy. The present study is an analysis of the literature on the bioethical aspect of autonomy in relation to immunization against Hepatitis B and HPV in adolescence. A bibliographic review was carried out based on the prism method, which included articles in Portuguese and English which were searched on the Scientific Electronic Library Online (SCIELO), Latin American and Caribbean Literature on Health Sciences (LILACS) and the National Library of Medicine (PUBMED), using the following descriptors Hepatitis B Vaccines, bioethics, Humans. The results demonstrated the efficiency of health professionals in contacting the individual and convincing him/her through an open dialogue and creation of bonds. It was also evidenced the study evidenced the patient's right to progressive autonomy, in which he gains the right to some choices such as immunizations of sexually transmitted diseases, such as Hepatitis B and HPV, however it is necessary for the adolescent to be educated about immunization. Therefore, the adolescent's right to progressive autonomy is recognized, regarding the choice of immunizations against Hepatitis B and HPV, however, it must be considered that these individuals are instructed on the individual and/or collective benefits of vaccination, contemplated by health education activities by qualified professionals, aiming to expand the global coverage of immunization with diseases that characterize public health problems. To verify the status and the immunization coverage of measles in Brazil It is a descriptive retrospective epidemiological study about the immunization coverage of measles in Brazil based on data provided by the Department of Informatics of the Unified Health System. From the data analysis, there was a greater number of cases in the North region. The highest number of registered cases was in 2019. The most affected individuals were males. The age group = 1 year old represented 41,12% of cases, and people of mixed color, 47% of cases. Six deaths occurred during the study period. Finally, in relation to vaccination coverage, with immunization with two doses of the MMR vaccine, there was greater coverage in 2019. Among the geographic regions, the highest coverage index was in the South region. There is a need to intensify awareness campaigns about the importance of vaccination. [ABSTRACT FROM AUTHOR]

The evaluation of correlation between health indicators and vaccination against Covid-19 is essential for implementing strategies to fight the disease. Objective: To analyze the spatialtemporal distribution of health indicators and their correlation with vaccination against Covid-19 in the state of Paraná. Methods: Ecological, cross-sectional study of secondary data analysis through bulletins of the Health Department of Paraná from 2021 to 2022, using as variables Incidence Rate (IR), Mortality Coefficient (MC), Lethality Coefficient (LC), Dose 1 (D1) and Vaccination Coverage (VC) of Covid-19. The correlation among the variables was analyzed with the Spearman test and exploratory analysis of univariate and bivariate spatial data using the software GeoDa. Results: There was a positive correlation for the analysis of VC and D1 with IR and MC, and a negative correlation for VC and D1 with LC. Univariate spatial analysis showed positive autocorrelation. In the bivariate analysis, there was a negative spatial correlation between VC and LC, with low-high clusters being highlighted, and a positive spatial correlation between VC and IR with low-low clusters. Conclusion: Knowledge on the influence of space on health indicators can contribute to the organization of policies for regional coping with the pandemic. PURPOSE: To evaluate the HPV vaccine coverage in adolescents and young adults relatives of women with cervical cancer. METHOD: A Prospective study held in the oncology department at a tertiary referral Hospital, using an adapted form. RESULTS: Epidemiological data were collected from 102 women with CC and 59 family members between 9-21 years. The degree of kinship between these young woman and cancer patients treated at the Hospital in the study was daughter (56%), sister (3%), niece (22%) and granddaughter (19%). Most of these girls (81%) were instructed to carry out HPV prevention, with the school (39%) being the main responsible for promoting vaccination. Regarding the reasons for not performing the immunization, the lack of information about the vaccine was the most mentioned (63%). When asking the girls relatives if they would vaccinate their daughters, 17% said they would not and none of these relatives knew the function of the vaccine. CONCLUSION: despite advances in HPV vaccination programs, there is still a great deal of misinformation about the topic in the population, especially among family members of the target audience. The school was identified as the protagonist in access to information and can be used as a means of access to the vaccine for young women and information for their families.

ABSTRACT Objective: To analyze the spatial behavior of hepatitis A, measles, mumps, and rubella (MMR), and varicella vaccination coverage in children and its relationship with socioeconomic determinants in the state of Minas Gerais. Methods: This ecological study investigated records of doses administered to children, extracted from the Immunization Information System of 853 municipalities in Minas Gerais, in 2020. We analyzed the vaccination coverage and socioeconomic factors. Spatial scan statistics were used to identify spatial clusters and measure the relative risk based on the vaccination coverage indicator and the Bivariate Moran Index, and thus detect socioeconomic factors correlated with the spatial distribution of vaccination. We used the cartographic base of the state and its municipalities and the ArcGIS and SPSS software programs. Results: Hepatitis A (89.0%), MMR (75.7%), and varicella (89.0%) showed low vaccination coverage. All vaccines analyzed had significant clusters. The clusters most likely to vaccinate their population were mainly located in the Central, Midwest, South Central, and Northwest regions, while the least likely were in the North, Northeast, and Triângulo do Sul regions. The municipal human development index, urbanization rate, and gross domestic product were spatially dependent on vaccination coverage. Conclusions: The spatial behavior of hepatitis A, MMR, and varicella vaccination coverage is heterogeneous and associated with socioeconomic factors. We emphasize that vaccination records require attention and should be continuously monitored to improve the quality of information used in services and research.

Objectives. To estimate the prevalence of missed opportunities for vaccination (MOV) in Latin America and the effect of interventions targeting health systems, health workers, patients, and communities on MOV. Methods. Searches were conducted in MEDLINE, EMBASE, CINAHL, and LILACS electronic databases and relevant organizations were contacted, including the Pan American Health Organization (PAHO), to identify studies meeting eligibility criteria. A pair of reviewers identified 27 randomized and non-randomized studies quantifying the effectiveness of any intervention for reducing MOV and 5 studies assessing the rate of MOV in Latin America. Results are reported narratively when criteria to pool results were not met, and the certainty of this evidence was assessed using the GRADE approach. Results. Evidence suggests the rate of MOV in Latin America ranged from 5% to 37% with a pooled estimate of 17% (95% CI [9, 32]) (low certainty) and that monetary incentives to healthcare teams, training for healthcare teams on how to communicate with patients, and educational interventions for caregivers probably reduce MOV (moderate to very low certainty). Conclusions. There is insufficient evidence supporting the implementation of any intervention as policy based only on the potential reduction of MOV without considering several factors, including costs, feasibility, acceptability, and equity.

Objective: To analyze the current state of vaccination coverage, describe and analyze the childhood vaccination schedule offered by SUS in Almenara-MG between 2015 and 2021. Methods: a bibliographic review and a descriptive-quantitative documentary study, which not only addresses the importance and validation of infant and prenatal vaccination, but also analyzes data obtained from the Municipal Health Department of the city of Almenara-MG. Results: A decline in vaccination coverage was observed in the municipality of Almenara; mainly in the last four years, most of the offered Immunobiological did not reach adequate coverage. ABSTRACT OBJECTIVE Describe the temporal evolution of morbimortality due to Covid-19 and vaccination coverage during the health emergency in Brazil. METHODS Number of cases and deaths due to Covid-19 were extracted from the public panel of the Brazilian Ministry of Health, according to epidemiological week (EW) and geographic region. Data on vaccines and variants were obtained, respectively, from the Information System of the National Immunization Program and the Genomic Surveillance System of SARS-CoV-2. RESULTS Three peaks of deaths characterized the evolution of the Covid-19 pandemic: in EW 30 of 2020, in the EW 14 of 2021 and in the EW six of 2022; three case waves, starting in the North and Northeast regions, with higher rates in the third wave, mainly in the South region. Vaccination started in the epidemiological week three of 2021, rapidly reaching most of the population, particularly in the Southeast and South regions, coinciding with a reduction exclusively in the mortality rate in the third wave. Only from the beginning of the second wave, when Gama was the dominant variant, 146,718 genomes were sequenced. From the last EW of 2021, with vaccination coverage already approaching 70%, the Omicron variant caused an avalanche of cases, but with fewer deaths. CONCLUSIONS We noticed the presence of three waves of Covid-19, as well as the effect of immunization on the reduction of mortality in the second and third waves, attributed to the Delta and Omicron variants, respectively. However, the reduction of morbidity, which peaked in the third wave during the domination of the Omicron variant, remained the same. The national and centralized command of the pandemic confrontation did not occur; thus, public administrators took the lead in their territories. The overwhelming effect of the pandemic could have been minimized, if there had been a coordinated participation of three spheres of the Brazilian Unified Health System administration, in the joint governance of the pandemic fight.

For vaccine-preventable diseases to be controlled, eliminated or eradicated, adequate vaccination coverage is necessary. The practice aims to: present the experience of the municipality of Icapuí, Ceará, in real-time vaccination monitoring of children; improve information about vaccination coverage for children under five years of age; favoring the protection of vaccine-preventable diseases in the target public and monitoring in real time children who are eligible for vaccination and who have delayed vaccines, as well as carrying out an active search for children who have not received the vaccine. The present study is descriptive, of the experience report type, where the experience in a project promoted by the health service in the city of Icapuí, Ceará is described. It started in July 2021 with the participation of the Municipal Health Department of the municipality of Icapuí along with the Primary Care technical team with professionals from the Primary Health Care Units (UAPS). The Human Papilloma Virus (HPV) is the most widespread and common sexually transmitted virus, in both sexes, responsible for several types of cancer. Vaccination is one of the primary prevention strategies. A community intervention was implemented to promote adherence to the HPV vaccine and an understanding of its importance. This was a retrospective cohort study regarding the vaccination coverage rate (TxCV) in the last seven years (1/1/2013 to 31/12/2020). The sample counted 136 adolescents, 71 boys and 65 girls, aged to 10-13 years. In its operationalization, the methodology used was Health Planning and for theoretical nursing framework, the Health Promotion Model. The community nursing intervention improved group immunity rates (primary prevention) and contributed to the youth/families being more competent to use health information in an empowered way (health promotion). Introduction: Human papillomavirus (HPV) is the fourth leading factor in cancer mortality in women in Brazil and has been a target of the Brazilian Unified Health System (SUS for short, in Portuguese) vaccination campaigns since 2014. This study aimed to expose possible impacts of the campaigns and understand the reasons affecting adherence to the program by a comparative analysis of the North and Southeast regions of Brazil. Methods: The study was conducted between January 2020 and September 2021 and analyzed data from the DATASUS-MS database, comparing the North and Southeast regions of Brazil. These regions were chosen based on data on the incidence of cervical cancer, as well as on socioeconomic data that highlight inequalities within the country. Results: The HPV vaccination coverage target was 80% (4.16 million) with a complete vaccination schedule. In 2014, adherence between the first and second doses dropped by 57% (188.164/331.230) in the North region and 73.5% (1.126.376/1.530.557) in the Southeast region. From 2014 to 2018, the number of preventive gynecological examinations in girls up to 14 years of age increased by 193% (933/318) in the North region and 12.4% (3793/3374) in the Southeast region. Conclusion: Neither of the regions reached the vaccination coverage target. However, we can infer that the campaign encouraged individuals to seek gynecological care. It is necessary to extend the reach of public health care and to contextualize and tailor the campaign to the specific social groups, thus ensuring the protection of more girls and women. ABSTRACT OBJECTIVE To analyze the epidemiological profile of cases and the pattern of spatial diffusion of the largest measles epidemic in Brazil that occurred in the post-elimination period in the state of São Paulo. METHOD A cross-sectional study based on confirmed measles cases in 2019. Bivariate analysis was performed for socioeconomic, clinical, and epidemiological variables, according to prior vaccination and hospitalization, combined with an analysis of spatial diffusion of cases using the Inverse Distance Weighting (IDW) method. RESULTS Of the 15,598 confirmed cases, 2,039 were hospitalized and 17 progressed to death. The epidemic peak occurred in epidemiological week 33, after confirmation of the first case, in the epidemiological week 6. Most cases were male (52.1%), aged between 18 and 29 years (38.7%), identified as whites (70%). Young adults (39.7%) and children under five years (32.8%) were the most affected age groups. A higher proportion of previous vaccination was observed in whites as compared to Blacks, browns, yellows and indigenous people (p < 0.001), as well as in the most educated group compared to the other categories (p < 0.001). The risk of hospitalization was higher in children than in the older age group (RI = 2.19; 95%CI: 1.66–2.88), as well as in the unvaccinated than in the vaccinated (RI = 1.59; 95%CI: 1.45–1.75). The pattern of diffusion by contiguity combined with diffusion by relocation followed the urban hierarchy of the main cities’ regions of influence. CONCLUSION In addition to routine vaccination in children, the findings indicate the need for immunization campaigns for young adults. In addition, studies that seek to investigate the occurrence of clusters of vulnerable populations, prone to lower vaccination coverage, are essential to broaden the understanding of the dynamics of transmission and, thus, reorienting control strategies that ensure disease elimination.

ABSTRACT OBJECTIVE To analyze the number of yellow fever vaccine doses administered before and during the covid-19 pandemic in Brazil. METHODS This is an ecological, time series study based on data from the National Immunization Program. Differences between the median number of yellow fever vaccine doses administered in Brazil and in its regions before (from April/2019 to March/2020) and after (from April/2020 to March/2021) the implementation of social distancing measures in the country were assessed via the Mann-Whitney test. Prais-Winsten regression models were used for time series analyses. RESULTS We found a reduction in the median number of yellow fever vaccine doses administered in Brazil and in its regions: North (-34.71%), Midwest (-21.72%), South (-63.50%), and Southeast (-34.42%) (p < 0.05). Series showed stationary behavior in Brazil and in its five regions during the covid-19 pandemic (p > 0.05). Brazilian states also showed stationary trends, except for two states which recorded an increasing trend in the number of administered yellow fever vaccine doses, namely: Alagoas State (before: β = 64, p = 0.081; after: β = 897, p = 0.039), which became a yellow fever vaccine recommendation zone, and Roraima State (before: β = 68, p = 0.724; after: β = 150, p = 0.000), which intensified yellow fever vaccinations due to a yellow fever case confirmation in a Venezuelan State in 2020. CONCLUSION The reduced number of yellow fever vaccine doses administered during the covid-19 pandemic in Brazil may favor the reemergence of urban yellow fever cases in the country.

The present article discusses the importance of equity in health care access with a focus on the acquisition and distribution of vaccines -- strategic and essential inputs in the context of COVID-19. It also addresses aspects that increase the risk of transmission, reinfection, and emergence of new variants, such as fake news, pandemic denial, and the anti-vax movement, which perpetuate the epidemic cycle. The importance of planning and designing whole-of-society health policies to manage the pandemic, within a national and global context of joint control is underscored, considering the need to ensure universal and timely access to vaccines and inputs and to the logistics necessary for vaccination. Equity is examined as the global guiding principle in reducing COVID-19 vaccination vulnerability, with an emphasis on the Brazilian setting, where, despite a strong vaccination expertise, a scenario of decreasing investment in science and technology, precarious logistical infrastructure, and corruption endangers access to vaccines. Finally, possible paths that can be explored and transformed into initiatives to be performed by various sectors in society are presented -- for example, the prioritization of regions or vulnerable social groups for vaccine distribution -- to ensure that equity in health care access in terms of COVID-19 vaccine coverage is achieved.
