RESUMO Objetivo: Verificar a percepção da fala de indivíduos com malformação de orelha e perda auditiva unilateral utilizando dois tipos de amplificação: amplificação sonora individual (AASI) convencional e softband (faixa com vibrador ósseo). Método: Foram selecionados 15 indivíduos, de ambos os sexos, com malformação congênita de orelha externa e/ou orelha média, diagnóstico de perda auditiva unilateral do tipo condutiva ou mista de grau moderado a severo, idade entre 15 e 25 anos, e encaminhamento para amplificação realizado pelo médico otorrinolaringologista. Após a adaptação com AASI e softband, foi realizada avaliação da percepção da fala sem uso da amplificação, com AASI acoplado ao arco e vibrador ósseo (convencional) e com uso do softband (faixa com vibrador). Os indivíduos foram avaliados por meio do Hearing in Noise Test (HINT) nas condições de silêncio e de ruído. Resultados: Foram avaliados sete indivíduos com malformação de orelha unilateral, sendo 57,1% na orelha direita e 42,9% na orelha esquerda. Quanto ao tipo e grau da perda, 71, 4% da amostra possuía perda auditiva condutiva moderada. No teste de percepção de fala na condição de silêncio, ruído frontal e ruído lateral, em três situações: sem amplificação, com o uso do AASI convencional e com o uso do softband, os resultados com uso de dispositivos de amplificação apresentaram-se melhores em todas as condições. Conclusão: Os indivíduos apresentaram melhora sutil, porém não significativa, na percepção de fala tanto em situações de silêncio, ruído frontal e lateral independentemente do tipo de amplificação. ABSTRACT Purpose: To verify the speech perception in subjects with ear malformation and unilateral hearing loss, fitted with two types of amplification as follows: conventional hearing aids and softband (band with vibrator bone). Method: The study included fifteen subjects of both sexes who presented congenital malformation of the middle or outer ear, diagnosed with unilateral conductive or mixed hearing loss, moderate to severe hearing loss, age range between 15 to 25 years and, prescription from a specialist doctor for hearing device fitting. We performed the speech perception assessment without amplification after the hearing aid and softband fitting, with the hearing aid linked to the bone vibrator (conventional) and the softband (band with the bone vibrator). The subjects were evaluated using the Hearing in Noise Test (HINT), in silence and in noise. Results: Seven subjects with unilateral ear malformation were evaluated, 57.1 % had impairment in the right ear and 42.9 % in the left ear. Regarding the type and the level of hearing loss, 71 % of all subjects included in the sample presented moderate conductive hearing loss. The assessment of speech perception was performed during silence, frontal noise, lateral noise and, during three specifics situations: no amplification, with conventional hearing aid and with the softband. The results with the amplification devices were positive in all evaluated conditions. Conclusion: Evaluated subjects presented improvement in speech perception, in silence, frontal noise and lateral noise situations, regardless of the type of amplification; however, the difference was not statistically significant.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES This research aimed to investigate the speech recognition in silence and in noise of subjects presenting unilateral hearing loss, without and with a hearing aid and to analyze the benefit, self-perception of the functional performance, satisfaction and the use of hearing aid in these subjects. Were evaluated 11 adults, presenting unilateral hearing loss, mixed or sensorineural type, from mild to severe degrees. The speech recognition was assessed using the Sentence Lists test in Brazilian Portuguese; the functional performance of the hearing was evaluated through the Speech Spatial and Qualities of Hearing Scale questionnaire; the satisfaction through the Satisfaction with Amplification in Daily Life, both in Brazilian Portuguese; and to evaluate the use of the hearing aid was analyzed the report of the patient. The hearing aid adaptation provided a speech recognition benefit in all evaluated positions, in silence and in noise. The subjects did not mention great limitations in the activities of communication during the use of the hearing aid. They were satisfied with the use of sound amplification. Most of the subjects do not make effective use of hearing aid. The discontinuation of the use can be justified by the difficulty of perceiving the restriction of participation caused by hearing loss, as well as the benefit of the prosthesis, besides the concern with the costs with the batteries and aesthetic aspects. Despite presenting benefit in speech recognition in silence and in noise and satisfaction with hearing aid, most subjects with unilateral hearing loss do not make effective use of the hearing aid. Esta pesquisa teve como objetivo investigar o reconhecimento de fala no silêncio e no ruído de sujeitos com perda auditiva unilateral, sem e com prótese auditiva, e analisar o benefício, autopercepção do desempenho funcional, satisfação e o uso de prótese auditiva nestes sujeitos. Foram avaliados 11 adultos, com perda auditiva unilateral, do tipo mista ou neurossensorial, de graus leve a severo. O reconhecimento de fala foi avaliado por meio do teste Listas de Sentenças em Português Brasileiro; o desempenho funcional da audição foi avaliado por meio do questionário Speech Spatial and Qualities of Hearing Scale; a satisfação por meio do Satisfaction with Amplification in Daily Life, ambos em português brasileiro; e para avaliação do uso da prótese auditiva foi analisado o relato do paciente. A adaptação de prótese auditiva proporcionou benefício no reconhecimento de fala em todas as posições avaliadas, no silêncio e no ruído. Os sujeitos não referiram grandes limitações nas atividades de comunicação com a utilização da prótese auditiva. Demonstraram-se satisfeitos com o uso da amplificação sonora. A maioria dos sujeitos não faz uso efetivo da prótese auditiva. A descontinuidade do uso pode ser justificada pela dificuldade de perceber a restrição de participação causada pela perda auditiva, assim como o benefício da prótese, além da preocupação com os custos com as pilhas e aspectos estéticos. Apesar de apresentarem benefício no reconhecimento de fala, no silêncio e no ruído e satisfação com a amplificação sonora, a maioria dos sujeitos com perda auditiva unilateral não faz uso efetivo da prótese auditiva.

RESUMO Objetivo Analisar o desempenho fonológico e do vocabulário de crianças com perda auditiva unilateral. Métodos Participaram do estudo 12 crianças com perda auditiva unilateral, seis delas com perda condutiva por malformação congênita de orelha e seis com perda sensorioneural congênita ou adquirida no primeiro ano de vida, de qualquer grau, configuração, em qualquer ouvido e sem outro comprometimento associado, na faixa etária entre 3 anos e 7 meses e 7 anos e 8 meses. Foram aplicadas as provas de fonologia e vocabulário do “Teste de Linguagem Infantil – ABFW”. Resultados Trinta e três por cento das crianças apresentaram desempenho diferente do esperado para a faixa etária, no que concerne ao desenvolvimento fonológico (2 crianças) e lexical (2 crianças). Na prova de fonologia, 1 criança com perda sensorioneural apresentou processos produtivos não esperados para a faixa etária (simplificações das líquidas e ensurdecimento de fricativas) e baixos índices de acertos no inventário fonético. Uma criança com perda auditiva condutiva por malformação congênita de orelha apresentou plosivação de fricativas, simplificação de líquidas e resultado limítrofe, na análise do inventário fonético (75%). Na prova de vocabulário, 2 crianças com perda sensorioneural apresentaram médias percentuais de Designação Verbal Usual abaixo das esperadas para as idades, nos diversos campos conceituais. Não houve diferença entre os grupos sensorioneural e condutivo nas provas de fonologia e de vocabulário. Conclusão Crianças com perda auditiva unilateral apresentam risco para o desenvolvimento de linguagem. Destaca-se a importância do acompanhamento de linguagem e audição neste grupo, uma vez que o monitoramento pode propiciar intervenções oportunas e eficazes, prevenindo possíveis alterações da linguagem e dificuldades escolares. ABSTRACT Purpose To analyze the phonological and vocabulary performance of children with unilateral hearing loss. Methods Participants were twelve subjects with unilateral hearing loss of any degree and configuration, with deficit in any ear and without other associated impairments – six with conductive hearing loss due to congenital malformation of outer and/or middle ear, and six with congenital sensorineural hearing loss or acquired in the first year of life. Subjects’ ages ranged from 3 years and 7 months to 7 years and 8 months. The phonology and vocabulary tasks of the “ABFW – Child Language Test” were applied. Results Thirty-three percent of the children presented performances different than expected for their age groups regarding phonological (2 children) and lexical developments (2 children). On the phonology test, one child with sensorineural hearing loss presented phonological processes that were not expected for the age group (liquid simplification and fricative devoicing) and low indices of correct production in the phonetic inventory. One child with conductive hearing loss due to malformation presented stopping and liquid simplification, and borderline results in the analysis of the phonetic inventory (75%). On the vocabulary test, two children with sensorineural unilateral hearing loss presented mean of Usual Verbal Designations below the expected for their ages in different semantic fields. There were no differences between children with sensorineural and conductive hearing loss in the phonology and vocabulary tests. Conclusion Children with unilateral hearing loss are considered at risk for language development. We emphasize the importance of language and auditory monitoring of these children, providing early and efficient interventions, thus preventing possible language disorders and learning difficulties.

A deficiência auditiva (DA) unilateral é caracterizada pela diminuição da audição em apenas uma orelha podendo acarretar dificuldade acadêmica, alterações de linguagem e dificuldade sócio-emocionais. OBJETIVO: Avaliar a autopercepção do handicap auditivo de sujeitos adultos, portadores de DA sensório-neural unilateral, não usuários de Aparelho de Amplificação Sonora Individual. MATERIAL E MÉTODO: Estudo Prospectivo com 52 sujeitos adultos, com média de 34.5 anos e de ambos os gêneros (26 do gênero feminino, 26 do masculino) com deficiência auditiva: tipo neurossensorial unilateral, de graus variados; responderam a um questionário para a avaliação do handicap sendo utilizado o "Hearing Handicap Inventory for Adults" (HHIA). RESULTADOS: Foram pontuadas as subescalas dos aspectos emocionais e sociais/situacionais sendo encontrados 73,1% de presença do handicap entre os graus leve, moderado e significativo e em maior porcentagem (88,5%) no gênero feminino. CONCLUSÕES: A aplicação do questionário mostrou-se um procedimento eficiente, pois a DA unilateral pode, não raramente, comprometer aspectos sociais e emocionais do sujeito adulto e o mesmo grau de DA podem reagir de forma diferente, indicando que a grande variabilidade na autopercepção do handicap auditivo está associada a aspectos não audiológicos. Hearing impairment (HI) is characterized by unilateral hearing loss in one ear and can result in learning difficulties, language impairment and socio-emotional difficulties. To assess the perception of hearing handicap in adult subjects, patients with unilateral sensorineural HI, non-users of individual hearing aids. MATERIALS AND METHODS: Prospective study with 52 adult subjects with a mean of 34.5 years of age, from both genders (26 females, 26 males) with hearing loss: sensorineural unilateral, in varying degrees, responded to a questionnaire for assessing hearing handicap, and for that we used the Hearing Handicap Inventory for Adults (HHIA). RESULTS: We scored the subscales of the emotional and social/situational aspects, and we found 73.1% of the handicap present being mild, moderate and significant, but at a higher percentage (88.5%) in females. CONCLUSIONS: The use of the questionnaire proved to be an effective procedure, because the unilateral HI may, not infrequently, compromise social and emotional aspects of the adult subject and the same degree of HI who can react differently, indicating that the wide variability in the perception of the hearing handicap is associated with non-audiological aspects.
