Introdução: A fraqueza muscular faz parte da síndrome consumptiva, que é reconhecida há longo prazo como uma característica da TB. As citocinas pró-inflamatórias são as principais agentes causadoras das alterações metabólicas da TB. O objetivo deste estudo é avaliar a força muscular respiratória e periférica de pacientes hospitalizados com TB pulmonar e relacionar esses achados com os níveis séricos de leptina, IL-6 e TNF-α. Métodos: Estudo de caso controle, com coleta de dados prospectivas. Pacientes hospitalizados com diagnóstico de TB pulmonar foram incluídos e controles pareados por sexo e idade (1:1). Foi aferida a força máxima voluntária de preensão manual, através da dinamometria manual (DM), e foram medidas a pressão inspiratória máxima (PIM) e a pressão expiratória máxima (PEM) através de um manovacuômetro. As citocinas pró-inflamatórias são as candidatas iniciais como agentes causadoras das alterações metabólicas da TB. O objetivo deste estudo é avaliar a força muscular respiratória e periférica de pacientes hospitalizados com TB pulmonar e relacionar esses achados com os níveis séricos de leptina, IL-6 e TNF-α. Métodos: Estudo transversal, com coleta de dados prospectiva. Pacientes hospitalizados com diagnóstico de TB pulmonar. Foram aferidas a força máxima voluntária de preensão manual, a pressão inspiratória máxima (PImáx) e a pressão expiratória máxima (PEmáx). Sangue venoso foi coletado da região cubital para a dosagem de leptina, IL-6 e TNF-α. Resultados: Foram incluídos 36 pacientes. A média de idade foi de 37,9±16,8 anos e a maioria (69,4%) era do sexo masculino e da raça branca (55,6%). Quatorze pacientes (38,4%) eram HIV positivos. O IMC foi ≤ 18,5 kg/m2 em 15 (41,7%) pacientes. A PEmáx, PImáx e a força de preensão manual bilateral encontravam-se reduzidas na maioria dos pacientes. As mulheres e os pacientes com infiltrados pulmonares em lobos superiores na radiografia de tórax apresentaram níveis séricos maiores de leptina. Os pacientes que referiram febre e aqueles com cultura positiva para Mycobacterium tuberculosis tiveram níveis séricos mais elevados de IL-6. Também foram encontradas uma correlação positiva entre IMC e níveis séricos de leptina, e uma correlação inversa entre a idade e os níveis séricos de TNF-α. Conclusão: Identificamos uma redução da força muscular respiratória e periférica nos pacientes com TB ativa, e demonstramos uma associação entre os níveis séricos de leptina, TNF-α e IL-6 e algumas características clínicas e radiológicas. Não houve associação entre a força muscular respiratória e periférica com os níveis séricos de leptina, IL-6 e TNF-α. Estudos futuros, com um tamanho amostral maior e com um grupo controle, são necessários para elucidar esses achados. Background: Muscle weakness is part of the wasting syndrome, which is recognized for long-term as a characteristic of TB. Proinflammatory cytokines are candidates as the initial causative agents of the metabolic changes in TB. The objective of this study is to assess the peripheral and respiratory muscle strength in patients hospitalized with pulmonary TB and relate these findings with leptin, IL-6 and TNF-α serum levels. Methods: Cross-sectional study with prospective data collection. Hospitalized patients diagnosed with pulmonary TB. Maximal voluntary handgrip strength, maximal inspiratory pressure (MIP) and maximum expiratory pressure (MEP) were measured. Venous blood was collected from the cubital region for leptin, IL-6 and TNF-α dosage. Results: We included 36 patients. The mean age was 37.9 ± 16.8 years and the majority (69.4%) were male and white (55.6%). Fourteen patients (38.4%) were HIV positive. BMI was ≤ 18.5 kg / m2 in 15 (41.7%) patients. The MEP, MIP and bilateral handgrip strength were reduced in most patients. Women and patients with pulmonary infiltrates in the upper lobes on chest radiography showed higher serum leptin levels. Patients who reported fever and those with positive culture for Mycobacterium tuberculosis had higher serum levels of IL-6. We have also found a positive correlation between BMI and serum leptin levels, and an inverse correlation between age and serum levels of TNF-α. Conclusion: We identified a reduction of peripheral and respiratory muscle strength in patients with active TB, and demonstrated an association between serum levels of leptin, TNF-α and IL-6 and some clinical and radiological features. There was no association between respiratory and peripheral muscle strength with leptin, IL-6 and TNF-α serum levels. Further studies with a larger sample size and a control group are needed to elucidate these findings.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES The aim of this study was to evaluate the influence of TENS, applied at the PC6 acupuncture point, on the modulation of symptoms of nausea, vomiting, anxiety and ingestion of antiemetics associated with antineoplastic chemotherapy in women with CM diagnosis. A randomized clinical trial was conducted in which 31 women were divided into two groups: 16 in the placebo group (PG) and 15 in the treatment group (TG). As eligibility criteria were included women with indication for neoadjuvant or adjuvant QT, of high and moderate emetogenic potential, aged between 35 and 65 years and who signed the Free and Informed Consent Term. Women who had central nervous system metastasis, a diagnosis of Carpal Tunnel Syndrome referred by the patient, those who were unable to receive electrostimulation in the upper limb due to skin integrity conditions or who had decreased sensitivity under the electrodes were excluded), in addition to those who were not performing the first QT treatment, and those who presented cognitive deficits. The research was published in the Brazilian Registry of Clinical Trials (ReBEC) under number RBR-4ccv6f. In order to delineate the profile of the volunteers, a sociodemographic, clinical history and oncological treatment questionnaire was used. To evaluate the occurrence of N / V, the Multinational Association of Supportive Care in Cancer (MASCC) Antiemesis Tool (MAT) was used; for the monitoring of symptoms, the Edmonton Symptom Assessment System (ESAS) questionnaire was used, and the Beck Anxiety Inventory (IAB) was used to assess the level of anxiety. The researchers were separated into the roles of therapist and evaluator. The therapist applied the TENS protocol, while the evaluator, blinded to procedures and group randomization, applied the questionnaires. The TENS-FESHTM model was applied before infusion of the chemotherapy drugs for 30 minutes in a reserved room at the HUSM QT outpatient clinic. The volunteers were submitted to a single application of TENS in each cycle of QT with high and moderate emetogenic power. Statistical analyzes were performed using the Test T for continuous variables and Fisher's Exact test for the categorical variables, and finally Friedman's ANOVA to compare the groups throughout the chemotherapy cycles. For all tests the alpha significance level of 5% was adopted. There was no difference between the groups regarding the characterization variables of the sample. The GP showed an improvement over TG in the symptoms of nausea in 24h (0.017) and 48h (0.043) after the administration of the second cycle of chemotherapy. The physiotherapeutic approach with the use of TENS in the studied sample had no effect on the control of N / V symptoms and anxiety caused by the treatment of antineoplastic chemotherapy, as well as on antiemetic intake. O estudo propôs-se a avaliar a influência da TENS, aplicada no ponto de acupuntura PC6, na modulação dos sintomas de náuseas, vômitos, ansiedade e ingesta de antieméticos associados à quimioterapia antineoplásica em mulheres com diagnóstico de câncer de mama (CM). Para isso foi realizado um ensaio clínico randomizado em que foram avaliadas 31 mulheres, divididas em 2 grupos: 16 no grupo placebo (GP) e 15 no grupo tratamento (GT). Como critérios de elegibilidade foram incluídas mulheres com indicação de QT neoadjuvante ou adjuvante, de alto e moderado potencial emetogênico, com faixa etária entre 35 e 65 anos e que assinaram o Termo de Consentimento Livre e Esclarecido. Foram excluídas as mulheres que apresentavam metástase para sistema nervoso central, diagnóstico de Síndrome do Túnel do Carpo referido pela paciente, aquelas impossibilitadas de receber a eletroestimulação no membro superior devido às condições de integridade da pele ou que apresentavam sensibilidade diminuída sob os eletrodos (sensibilidade referida), além daquelas que não estavam realizando o primeiro tratamento de QT e ainda, as que apresentaram déficit cognitivo. A pesquisa foi publicada no Registro Brasileiro de Ensaios Clínicos (ReBEC) sob número RBR-4ccv6f. Para delinear o perfil das voluntárias foi utilizado um questionário sociodemográfico, de histórico clínico e de tratamento oncológico. Para avaliar ocorrência de N/V foi utilizado o formulário Multinational Association of Supportive Care in Cancer (MASCC) Antiemesis Tool (MAT); para o monitoramento de sintomas utilizou-se o questionário Edmonton Symptom Assessment System (ESAS), e para avaliar o nível de ansiedade utilizou-se o Inventário de Ansiedade de Beck (IAB). Os pesquisadores foram separados nas funções de terapeuta e avaliador. O terapeuta aplicou o protocolo de TENS, enquanto o avaliador, cegado quanto aos procedimentos e à randomização dos grupos, aplicou os questionários. Foi aplicada a TENS, modelo TENS-FESHTM, antes da infusão das drogas quimioterápicas por 30 minutos, em uma sala reservada no ambulatório de QT do HUSM. As voluntárias foram submetidas a uma única aplicação da TENS em cada ciclo de QT com alto e moderado poder emetogênico. Para análises estatísticas foi realizado o Teste T para variáveis contínuas e o teste Exato de Fisher para as variáveis categóricas, e por fim, ANOVA de Friedman para comparar os grupos ao longo dos ciclos de quimioterapia. Para todos os testes foi adotado o nível de significância de 5%. Não houve diferença entre os grupos quanto às variáveis de caracterização da amostra. O GP apresentou melhora em relação ao GT nos sintomas de náuseas nas 24h (0,017) e 48h (0,043) após a administração do segundo ciclo de quimioterapia. A abordagem fisioterapêutica com o uso da TENS na amostra estudada não teve efeito no controle dos sintomas de N/V e ansiedade, provocados pelo tratamento de quimioterapia antineoplásica, bem como na ingesta de antieméticos.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES Introduction: For the 2018-2019 biennium, approximately 59,700 new cases of breast Cancer (CA) are expected each year. Among the therapies employed for the treatment of CA is chemotherapy (QT), a technique that has been shown to be effective in the management of this disease by attacking cancer cells. However, it can lead to numerous adverse reactions, including nausea and vomiting (N/V), fatigue and reduced Quality of Life (QL). Thus, the investigation of the use of Transcutaneous Electrical Stimulation (TENS) in chemotherapy-induced symptoms in women with breast CA, while complementing the exercise of the physical therapist could be a tool in improving the health problems of this population, since the fear by treatment and its side effects even contributes to its abandonment, reducing the chances of therapeutic success. Objective: To evaluate the repercussions of TENS applied at the acupuncture point PC6 on N/V modulation and its implications on fatigue and QL in women with breast CA during chemotherapy treatment. Method: We evaluated women from 35 to 65 years old, diagnosed with breast CA, who were in attendance at the QT outpatient clinic of the Santa Maria University Hospital, using a sociodemographic questionnaire, a Multinational N/V incidence questionnaire. Association of Supportive Care in Cancer Antiemesis Tool (MAT), quality of life of the European Organization for Research and Treatment of Cancer Quality of Life Supplementary Questionnaire (QLQ - C30) and fatigue the Functional Assessment of Cancer Therapy Fatigue (FACT-F). After the initial evaluation, the volunteers were randomized through opaque and sealed envelopes to compose the Placebo Group (GP), which received the placebo intervention (the device remained off) or to the TENS Group (GT), in which the patients received the placebo. TENS treatment during the QT period. The study was blinded being a therapist for the application of the TENS protocol and another researcher for the collection procedures. TENS, Neurodyn Portable TENS model of Ibramed brand, was applied before infusion of chemotherapeutic drugs for 30 minutes at acupuncture point PC6 with frequency of 10Hz and 150us. Women underwent a single application of TENS before each QT cycle and questionnaires administered before each intervention. The MAT questionnaire was also applied by telephone contact at 24h and 48h post QT. Results: We evaluated 31 women, 16 in the GP and 15 in the GT. The groups were homogeneous regarding sociodemographic and medical treatment variables. Regarding the pre, 24h and 48h post-QT emetic symptoms, a difference was observed between the groups regarding the presence of nausea in the last 24h at 24h and 48h post- QT only in Cycle 2 for the GP. Regarding the variables Fatigue and QL, no difference was observed between the groups during the entire QT. Conclusion: TENS does not appear to be an eligible therapy for N/V reduction, fatigue and QL control during the period of high emetogenic QT. Introdução: Para o biênio 2018-2019 são esperados cerca de 59.700 casos novos de Câncer (CA) de mama para cada ano. Entre as terapias empregadas para o tratamento do CA encontra-se a quimioterapia (QT), uma técnica que vem se mostrando eficaz no manejo dessa doença atacando células cancerígenas, no entanto pode levar a inúmeras reações adversas, entre elas as náuseas e vômitos (N/V), fadiga e redução da Qualidade de Vida (QV). Dessa maneira, a investigação da utilização da Estimulação Elétrica Transcutânea (TENS) nos sintomas induzidos pela quimioterapia em mulheres com CA de mama, enquanto complementar ao exercício do fisioterapeuta poderia ser uma ferramenta na melhoria dos problemas de saúde dessa população, uma vez que o temor pelo tratamento e seus efeitos colaterais contribui até mesmo para o seu abandono, diminuindo as chances de sucesso terapêutico. Objetivo: Avaliar as repercussões da TENS aplicada no ponto de acupuntura PC6 na modulação de N/V e suas implicações sobre a fadiga e QV em mulheres com CA de mama durante o tratamento quimioterápico. Método: Foram avaliadas mulheres de 35 a 65 anos de idade, com diagnóstico de CA de mama, que estavam em atendimento no ambulatório de QT do Hospital Universitário de Santa Maria, por meio de questionário sociodemográfico, questionário de incidência de N/V da Multinational Association of Supportive Care in Cancer Antiemesis Tool (MAT), qualidade de vida da European Organization for Research and Treatment of Cancer Quality of Life Supplementary Questionnaire (QLQ – C30) e fadiga o Functional Assessment of Cancer Therapy Fatigue (FACT-F). Após a avaliação inicial, as voluntárias foram randomizadas através de envelopes opacos e lacrados para compor o Grupo Placebo (GP), que recebeu a intervenção placebo (o aparelho permaneceu desligado) ou para o Grupo TENS (GT), no qual as pacientes receberam o tratamento com TENS durante o período de QT. O estudo foi do tipo cegado sendo um terapeuta para aplicação do protocolo de TENS e outro pesquisador para os procedimentos de coleta. Foi aplicada a TENS, modelo Neurodyn Portátil TENS da marca Ibramed, antes da infusão das drogas quimioterápicas por 30 minutos no ponto de acupuntura PC6 com frequência de 10Hz e 150us. As mulheres foram submetidas a uma única aplicação da TENS antes de cada ciclo de QT e os questionários aplicados antes de cada intervenção. O questionário MAT também foi aplicado através de contato telefônico nas 24h e 48h pós QT. Resultados: Foram avaliadas 31 mulheres sendo 16 no GP e 15 no GT. Os grupos eram homogêneos em relação às variáveis sociodemográficas e de tratamento médico. Quanto aos sintomas eméticos pré, 24h e 48h pós Ciclo de QT foi observado diferença entre os grupos em relação à presença de náusea nas últimas 24h nas 24h e 48h pós QT apenas no Ciclo 2 para o GP. Quanto às variáveis Fadiga e QV não foi observado diferença entre os grupos durante toda a QT. Conclusão: A TENS parece não ser uma terapia elencável para redução de N/V, fadiga e controle da QV durante o período de QT de alto poder emetogênico.
