RESUMO Introdução: Os efeitos provocados pela COVID-19 em longo prazo são desconhecidos. O presente estudo tem como objetivo avaliar os fatores associados com a qualidade de vida relacionada à saúde e os desfechos em longo prazo em sobreviventes à hospitalização por COVID-19 no Brasil. Métodos: Este será um estudo multicêntrico de coorte prospectivo, aninhado em cinco ensaios clínicos randomizados desenhados para avaliar os efeitos dos tratamentos específicos para COVID-19 em mais de 50 centros no Brasil. Pacientes adultos sobreviventes à hospitalização por infecção por SARS-CoV-2 comprovada ou suspeita serão seguidos por um período de 1 ano, por meio de entrevistas telefônicas estruturadas. O desfecho primário é o escore de utilidade para qualidade de vida relacionada à saúde após 1 ano, avaliado segundo o questionário EuroQol-5D3L. Os desfechos secundários incluirão mortalidade por todas as causas, eventos cardiovasculares graves, reospitalizações, retorno ao trabalho ou estudo, condição funcional física avaliada pelo instrumento Lawton-Brody Instrumental Activities of Daily Living, dispneia avaliada segundo a escala de dispneia modificada do Medical Research Council, necessidade de suporte ventilatório em longo prazo, sintomas de ansiedade e depressão avaliados segundo a Hospital Anxiety and Depression Scale, sintomas de transtorno de estresse pós-traumático avaliados pela ferramenta Impact of Event Scale-Revised e autoavaliação da condição de saúde, conforme a Escala Visual Analógica do EuroQol-5D3L. Serão utilizadas equações de estimativas generalizada para testar a associação entre cinco conjuntos de variáveis (1 - características demográficas, 2 - condição de saúde pré-morbidade, 3 - características da doença aguda, 4 - terapias específicas para COVID-19 recebidas e 5 - variáveis pós-alta atualizadas) e desfechos. Ética e disseminação: O protocolo do estudo foi aprovado pelos Comitês de Ética em Pesquisa de todas as instituições participantes. Os resultados serão disseminados por meio de conferências e periódicos revisados por pares. Abstract Introduction: The long-term effects caused by COVID-19 are unknown. The present study aims to assess factors associated with health-related quality of life and long-term outcomes among survivors of hospitalization for COVID-19 in Brazil. Methods: This is a multicenter prospective cohort study nested in five randomized clinical trials designed to assess the effects of specific COVID-19 treatments in over 50 centers in Brazil. Adult survivors of hospitalization due to proven or suspected SARS-CoV-2 infection will be followed-up for a period of 1 year by means of structured telephone interviews. The primary outcome is the 1-year utility score of health-related quality of life assessed by the EuroQol-5D3L. Secondary outcomes include all-cause mortality, major cardiovascular events, rehospitalizations, return to work or study, physical functional status assessed by the Lawton-Brody Instrumental Activities of Daily Living, dyspnea assessed by the modified Medical Research Council dyspnea scale, need for long-term ventilatory support, symptoms of anxiety and depression assessed by the Hospital Anxiety and Depression Scale, symptoms of posttraumatic stress disorder assessed by the Impact of Event Scale-Revised, and self-rated health assessed by the EuroQol-5D3L Visual Analog Scale. Generalized estimated equations will be performed to test the association between five sets of variables (1- demographic characteristics, 2- premorbid state of health, 3- characteristics of acute illness, 4- specific COVID-19 treatments received, and 5- time-updated postdischarge variables) and outcomes. Ethics and dissemination: The study protocol was approved by the Research Ethics Committee of all participant institutions. The results will be disseminated through conferences and peer-reviewed journals.

RESUMO Objetivo: Avaliar a prevalência de incapacidades físicas, cognitivas e psiquiátricas, fatores associados e sua relação com qualidade de vida em pacientes sobreviventes de internação em unidades de terapia intensiva brasileiras. Métodos: Um estudo de coorte prospectivo multicêntrico está sendo conduzido em dez unidades de terapia intensiva adulto clínico-cirúrgicas representativas das cinco regiões geopolíticas do Brasil. Pacientes com idade ≥ 18 anos que receberam alta das unidades de terapia intensiva participantes e permaneceram internados na unidade de terapia intensiva por 72 horas ou mais, nos casos de internação clínica ou cirúrgica de urgência, e por 120 horas ou mais, nos casos de internação cirúrgica eletiva, serão incluídos de forma consecutiva. Estes pacientes serão seguidos por 1 ano, por meio de entrevistas telefônicas estruturadas 3, 6 e 12 meses pós-alta da unidade de terapia intensiva. Dependência funcional, disfunção cognitiva, sintomas de ansiedade e depressão, sintomas de estresse pós-traumático, qualidade de vida relacionada à saúde, re-hospitalizações e mortalidade em longo prazo serão avaliados como desfechos. Discussão: O presente estudo tem o potencial de contribuir para o conhecimento a respeito da prevalência e dos fatores associados à síndrome pós-cuidados intensivos na população de pacientes adultos sobreviventes de internação em unidades de terapia intensiva brasileiras. Ademais, a associação entre síndrome pós-cuidados intensivos e qualidade de vida relacionada à saúde poderá ser estabelecida. ABSTRACT Objective: To establish the prevalence of physical, cognitive and psychiatric disabilities, associated factors and their relationship with the qualities of life of intensive care survivors in Brazil. Methods: A prospective multicenter cohort study is currently being conducted at 10 adult medical-surgical intensive care units representative of the 5 Brazilian geopolitical regions. Patients aged ≥ 18 years who are discharged from the participating intensive care units and stay 72 hours or more in the intensive care unit for medical or emergency surgery admissions or 120 hours or more for elective surgery admissions are consecutively included. Patients are followed up for a period of one year by means of structured telephone interviews conducted at 3, 6 and 12 months after discharge from the intensive care unit. The outcomes are functional dependence, cognitive dysfunction, anxiety and depression symptoms, posttraumatic stress symptoms, health-related quality of life, rehospitalization and long-term mortality. Discussion: The present study has the potential to contribute to current knowledge of the prevalence and factors associated with postintensive care syndrome among adult intensive care survivors in Brazil. In addition, an association might be established between postintensive care syndrome and health-related quality of life.

Cochlear implants have transformed the field of otology after becoming the standard care for hearing rehabilitation in patients with severe to profound sensorioneural hearing loss that has progressed to the point that benefit from hearing aids is limited. Only 50 years ago there were no effective treatment for deafness or severe to profound hearing losses. Since Alessandro Volta in the early 1800’s provided the first account of electrical stimulation of the auditory system, a fascinating series of experiments involving electrically evoked hearing has been described, culminating with the first cochlear implant performed by Dr House in 1961. At that time, patients had some basic frequency discrimination and could identify words in small closed sets.1 The changing point for CI came when Dr House enlisted the partnership of Mr Jack Urban, an electrical engineer, resulting on landmark changes in the history of CIs.1 The current technique for cochlear implantation (CI) surgery requires a mastoidectomy to gain access to the cochlea for electrode array insertion, drilling in close proximity to bone-embedded nerves, blood vessels and other structures, which can result in complications for the patient.2 Within all fields of surgery there has been a push towards minimizing the invasiveness of procedures to limit co-morbidity as well as costs.3 On the past 20 years, bioengineering and surgical principles were united to reduce surgical trauma by using image-guided surgical systems (IGS) that drill a single tunnel towards the cochlea. Using pre and intraoperatively acquired CT (computed tomography) image, these systems allow surgeons to determine the boundaries of the surgical field and positions of vital anatomical structures.3 Credits for introducing IGS to Otolaryngology goes to Schlöndorff.4,5 Early attempts to minimize CI surgery included the endomeatal approach, where a trough was drilled in the posterior external auditory canal (EAC) to accommodate the connecting wire from the internal receiver to the electrode array. This approach was complicated by infection, wire extrusion into the EAC, and cholesteatoma6,7. Later efforts have focused on smaller incision sites8,9, micromastoids, known as “Veria Operation”10 and single drill troughs to the middle ear via the attic11. Research on RCI (robotic cochlear implantation) has so far focused on individual elements of the procedure, such as image-based surgical planning, guided keyhole trajectory using surgical templates, industrial robotic manipulators, and skull- mounted passive kinematic structures. Design for electrode insertion systems and reproducible options for cochlear access using robotic force feedback control, have also been addressed. For the past 20 years, groups around the world have contributed for the creation and refinement of minimally invasive surgical approaches, starting with the creation of anatomical datasets for surgical training and temporal bone study. The steps to achieve the current status of the robotic surgical procedure will be described in this chapter.

Neste artigo descrevemos o segundo ciclo de construção e avaliação do Acropolis, uma plataforma de computação social que permite aos cidadãos construir e compartilhar suas narrativas pessoais sobre histórias noticiosas complexas ou de longa duração. Resultados experimentais produzidos por meio de avaliações quantitativas e qualitativas demonstraram, através de evidências positivas, que a nossa abordagem proposta foi capaz de apoiar o envolvimento dos cidadãos na curadoria colaborativa de conteúdo de notícias. Usando a metodologia de pesquisa ADR concluímos o segundo ciclo deste estudo com um conjunto de recomendações de design para a construção de plataformas semelhantes. In this article we describe the second design and evaluation cycle of Acropolis, a social computing platform that allows citizens to build and share their personal narratives about complex or long-term news stories. Experimental results produced through quantitative and qualitative evaluations have shown, by positive evidence, that our proposed approach was able to support citizen engagement in collaborative curation of news content. By using the ADR research methodology, we concluded the second cycle of this study with a set of design recommendations for building similar platforms.

Este trabalho é uma investigação acerca da construção de uma cultura técnica agropecuária naquela que foi a Colônia Ijuí, uma colônia estatal, mista, criada em 1890 no noroeste do Estado do Rio Grande do Sul. O processo de construção de uma cultura técnica agropecuária em Ijuí, estudado neste trabalho, tem início em 1890 com a criação da colônia. Concorreram para a construção da cultura técnica agropecuária em Ijuí, imigrantes europeus e seus descendentes, nacionais/caboclos e o estado, num processo híbrido em que conhecimentos nacionais e imigrantes se somaram na sua construção, a qual se apresentará como dominante até meados da década de 1950, quando uma nova onda modernizadora, chamada de Revolução Verde, passa a ser inserida na região. O período estudado caracteriza-se pela tentativa de modernização agropecuária por parte do Estado com a inserção de uma mão-de-obra dita mais qualificada do que a nacional - a do imigrante europeu. As principais fontes utilizadas são relatórios oficiais da Intendência e da Prefeitura Municipal de Ijuí, o Jornal Correio Serrano, entrevistas e imagens fotográficas de fotógrafos imigrantes.

A situação da dependência de substâncias psicoativas tem se caracterizado como um grave problema no âmbito da saúde pública brasileira e mundial, requisitando das ciências e dos serviços de atenção estudos e ações sustentadas em conhecimento especializado, que dêem conta da complexidade da situação. No cenário atual da saúde existem diversos modelos de análise à dependência, sustentados em diferentes perspectivas epistemológicas, teórico e metodológicas, formando uma miscelânea de propostas. O presente artigo, baseado em pesquisa qualitativa realizada em um município de médio porte de Santa Catarina, objetiva descrever as implicações dos atuais modelos de análise e atenção da dependência de álcool e outras drogas nos serviços de saúde básica. Verificou-se que as concepções sobre a drogadição ainda estão sustentadas em modelos de análise dicotômicos e deterministas, em contradição com os princípios do SUS. Constatou-se também a falta da capacitação das equipes de Estratégia da Saúde da Família para lidarem com a problemática da dependência de álcool e outras drogas

A violência familiar, considerada um importante problema de saúde pública, acarreta impactos na trajetória de desenvolvimento e na saúde daqueles que a vivenciam. O presente trabalho tem o objetivo de caracterizar o fenômeno da violência familiar e avaliar a aplicabilidade da teoria sistêmica na compreensão das dinâmicas familiares violentas. São apresentadas, brevemente, as origens históricas e epistemológicas do pensamento sistêmico. Discutem-se características dos serviços atualmente disponíveis para atendimento de famílias cujas relações são permeadas pela violência, analisando-se possibilidades de atuação profissional pautadas na perspectiva sistêmica. Aponta-se a importância da intervenção interdisciplinar, realizada de maneira integrada entre distintas instituições, considerando a integralidade enquanto princípio do SUS. Ressalta-se a necessidade de dirigir a atenção para além do usuário e da queixa, buscando incluir dados do contexto familiar e social. Enfatiza-se a relevância de produções científicas sobre o tema, visando a oferecer subsídios para elaboração de programas de assistência à clientela e de capacitação profissional

- Abstract
Background: Refractory cardiogenic shock (CS) is associated with high mortality rates, and the use of venoarterial extracorporeal membrane oxygenation (VA-ECMO) as a therapeutic option has generated discussions. Therefore, its cost-effectiveness, especially in low- and middle-income countries like Brazil, remains uncertain.Objectives: To conduct a cost-utility analysis from the Brazilian Unified Health System perspective to assess the cost-effectiveness of VA-ECMO combined with standard care compared to standard care alone in adult refractory CS patients., Methods: We followed a cohort of refractory CS patients treated with VA-ECMO in tertiary care centers located in Southern Brazilian. We collected data on hospital outcomes and costs. We conducted a systematic review to supplement our data and utilized a Markov model to estimate incremental cost-effectiveness ratios (ICERs) per quality-adjusted life year (QALY) and per life-year gained., Results: In the base-case analysis, VA-ECMO yielded an ICER of Int$ 37,491 per QALY. Sensitivity analyses identified hospitalization cost, relative risk of survival, and VA-ECMO group survival as key drivers of results. Probabilistic sensitivity analysis favored VA-ECMO, with a 78% probability of cost-effectiveness at the recommended willingness-to-pay threshold., Conclusions: Our study suggests that, within the Brazilian Health System framework, VA-ECMO may be a cost-effective therapy for refractory CS. However, limited efficacy data and recent trials questioning its benefit in specific patient subsets highlight the need for further research. Rigorous clinical trials, encompassing diverse patient profiles, are essential to confirm cost-effectiveness and ensure equitable access to advanced medical interventions within healthcare systems, particularly in socio-economically diverse countries like Brazil.

Objective: To investigate whether family participation in intensive care unit interdisciplinary bedside rounds affects family satisfaction., Methods: A cross-sectional study was conducted at a 56-bed, adult, mixed intensive care unit of a tertiary hospital in Southern Brazil. From May to June 2019, family members of patients who stayed in the intensive care unit for at least 48 hours were invited to participate in the study at the time of patient discharge. The main exposure variable was participation in intensive care unit bedside rounds during the intensive care unit stay. Family satisfaction was assessed by using the Brazilian version of the Family Satisfaction in the Intensive Care Unit questionnaire., Results: Of the 234 screened individuals, 118 were included. Eleven participants withdrew consent. A total of 107 individuals were assessed; 58 (54%) reported being present during bedside rounds, and 49 (46%) reported never being present. General satisfaction and satisfaction with the decision-making process were higher among families who were present during rounds than among families who were not (p = 0.01 and p = 0.007, respectively)., Conclusion: The presence during interdisciplinary rounds was associated with improved general satisfaction and satisfaction with the decision-making aspect. This outcome indicates that efforts must be directed to conduct studies with more robust methodologies to confirm this association.

19. The 6-Minute Walk Test predicts long-term physical improvement among intensive care unit survivors: a prospective cohort study.

- Abstract
Objective: To evaluate the ability of the 6-Minute Walk Test to predict long-term physical functional status improvement among intensive care unit survivors., Methods: Thirty-two intensive care unit survivors were prospectively evaluated from February 2017 to August 2018 in a post-intensive care unit outpatient clinic in Brazil. Individuals with intensive care unit stays > 72 hours (emergency admissions) or > 120 hours (elective admissions) attending the post-intensive care unit clinic four months after intensive care unit discharge were consecutively enrolled. The association between the 6-Minute Walk Test distance at baseline and physical functional status was assessed over 8 months using the Barthel Index., Results: The mean 6-Minute Walk Test distance was significantly lower in intensive care unit survivors than in the general population (405m versus 557m; p < 0.001). Age (β = -4.0; p < 0.001) and muscle weakness (β = -99.7; p = 0.02) were associated with the 6-Minute Walk Test distance. A 6-Minute Walk Test distance was associated with improvement in physical functional status over the 8-month follow-up (odds ratio for each 10m of 1.07; 95%CI 1.01 - 1.16; p = 0.03). The area under the Receiver Operating Characteristic curve for the 6-Minute Walk Test prediction of physical functional status improvement was 0.72 (95%CI 0.53 - 0.88)., Conclusion: The 6-Minute Walk Test performed 4 months after intensive care unit discharge predicted long-term physical functional status among intensive care unit survivors with moderate accuracy.

- Abstract
Introduction: The long-term effects caused by COVID-19 are unknown. The present study aims to assess factors associated with health-related quality of life and long-term outcomes among survivors of hospitalization for COVID-19 in Brazil., Methods: This is a multicenter prospective cohort study nested in five randomized clinical trials designed to assess the effects of specific COVID-19 treatments in over 50 centers in Brazil. Adult survivors of hospitalization due to proven or suspected SARS-CoV-2 infection will be followed-up for a period of 1 year by means of structured telephone interviews. The primary outcome is the 1-year utility score of health-related quality of life assessed by the EuroQol-5D3L. Secondary outcomes include all-cause mortality, major cardiovascular events, rehospitalizations, return to work or study, physical functional status assessed by the Lawton-Brody Instrumental Activities of Daily Living, dyspnea assessed by the modified Medical Research Council dyspnea scale, need for long-term ventilatory support, symptoms of anxiety and depression assessed by the Hospital Anxiety and Depression Scale, symptoms of posttraumatic stress disorder assessed by the Impact of Event Scale-Revised, and self-rated health assessed by the EuroQol-5D3L Visual Analog Scale. Generalized estimated equations will be performed to test the association between five sets of variables (1- demographic characteristics, 2- premorbid state of health, 3- characteristics of acute illness, 4- specific COVID-19 treatments received, and 5- time-updated postdischarge variables) and outcomes., Ethics and Dissemination: The study protocol was approved by the Research Ethics Committee of all participant institutions. The results will be disseminated through conferences and peer-reviewed journals.

- Abstract
Objective: To establish the prevalence of physical, cognitive and psychiatric disabilities, associated factors and their relationship with the qualities of life of intensive care survivors in Brazil., Methods: A prospective multicenter cohort study is currently being conducted at 10 adult medical-surgical intensive care units representative of the 5 Brazilian geopolitical regions. Patients aged ≥ 18 years who are discharged from the participating intensive care units and stay 72 hours or more in the intensive care unit for medical or emergency surgery admissions or 120 hours or more for elective surgery admissions are consecutively included. Patients are followed up for a period of one year by means of structured telephone interviews conducted at 3, 6 and 12 months after discharge from the intensive care unit. The outcomes are functional dependence, cognitive dysfunction, anxiety and depression symptoms, posttraumatic stress symptoms, health-related quality of life, rehospitalization and long-term mortality., Discussion: The present study has the potential to contribute to current knowledge of the prevalence and factors associated with postintensive care syndrome among adult intensive care survivors in Brazil. In addition, an association might be established between postintensive care syndrome and health-related quality of life.
