Leprosy is a chronic infectious disease caused by Mycobacterium leprae, an intracellular bacillus of airborne transmission. The disease affects the skin and peripheral nerves and can cause neurological sequelae. The bacillus multiplies slowly in the host and the disease probably occurs due to malfunctioning in host immune response. This review addresses the role of some specific micronutrients in the immune response, such as Vitamins A, D, E, C, Zinc and Selenium, detailing their mechanisms of actions in infectious diseases, and in leprosy. The immune response to pathogens releases harmful substances, which lead to tissue damage. This review discusses how a decreased level of antioxidants may contribute to an increased oxidative stress and complications of infectious diseases and leprosy. As the nutrients have a regulatory effect in the innate and adaptative immune responses, a perfect balance in their concentrations is important to improve the immune response against the pathogens. _______________________________________________________________________________ RESUMÉN: La lepra es una enfermedad infecciosa crónica causada por el Mycobacterium leprae, un bacilo intracelular de transmisión aérea. La enfermedad afecta la piel y los nervios periféricos y causa secuelas neurológicas. El bacilo se multiplica lentamente en el hospedador y posiblemente la enfermedad ocurre por el mal funcionamiento de la respuesta inmunitaria del hospedador. Esta revisión aborda el papel de algunos micronutrientes específicos en la respuesta inmunitaria, tales como las vitaminas A, D, E, C, el cinc y el selenio, detallando sus mecanismos de acción en las enfermedades infecciosas y en la lepra. La respuesta inmunitaria a los patógenos libera sustancias nocivas que producen lesión tisular. Esta revisión también aborda cómo una menor cantidad de antioxidantes puede contribuir a un aumento del estrés oxidativo y a complicaciones de las enfermedades infecciosas y la lepra. Puesto que los micronutrientes poseen un efecto regulador de la respuesta inmunitaria innata y adaptativa, es importante un equilibrio perfecto de sus concentraciones para mejorar la respuesta inmunitaria frente a los patógenos.

As leishmanioses constituem um complexo de doenças causadas por protozoários do gênero Leishmania, as quais incluem diversas espécies responsáveis por desencadear formas clínicas distintas. A espécie Leishmania infantum, disseminada no Brasil e na América do Sul, está relacionada com comprometimento das vísceras, principalmente baço, fígado e medula óssea, apresentando alto índice de morbidade e mortalidade. É classicamente demonstrado que uma resposta mediada por linfócitos Th1 e Th17 conferem resistência na LV. Nesse sentido, as vias de sinalização ativadas, bem como os mediadores gerados durante a infecção, podem auxiliar no desenvolvimento da resposta imune protetora. Dentre as citocinas produzidas, destaca-se a IL-1 (IL-1? e IL- 1?) que possui papel importante na geração do processo inflamatório conferindo resistência às infecções fúngicas, bacterianas, e também na leishmaniose cutânea. No presente estudo, nosso objetivo foi determinar o papel da via de sinalização mediada por IL-1R durante a infecção experimental por Leishmania infantum. Nossos resultados demonstram que tanto IL-1? quanto IL-1? são produzidas durante a infecção in vivo por L. infantum, sugerindo sua participação na LV. De maneira interessante, a via de sinalização mediada por IL-1R parece estar envolvida na susceptibilidade ao protozoário, visto que animais geneticamente deficientes para IL-1R (IL-1R KO) são mais resistentes à infecção, apresentando menor número de parasitos no baço e fígado nas fases crônicas de infecção. Ainda, o fenômeno de susceptibilidade foi associado a uma regulação negativa da resposta inflamatória, principalmente acerca de células Th1 e também de Th17. A inflamação neutrofílica também foi alterada em animais IL-1R KO infectados. Foi observado aumento do número de neutrófilos bem como da produção de CXCL1 (KC), mediador quimioatraente para neutrófilos no baço de animais IL-1R KO, XVIII mas sem alterar o perfil de ativação dessas células. Ao determinar a citocina envolvida na regulação do processo inflamatório e consequentemente na susceptibilidade ao parasito, observamos que preferencialmente a IL-1? possui papel na progressão da doença, visto que a deleção gênica de enzimas essenciais para a liberação de IL-1? como Caspase 1/11 não alterou o número de parasitos no baço e fígado. Porém, na ausência de IL-1? foi observado um perfil de resistência associado ao aumento dos níveis de IFN-? e IL-17 nos órgãos alvos da doença. Confirmando o papel regulador, o tratamento com anticorpo monoclonal anti-IL1? em animais selvagens (do inglês wild type - WT) infectados reverteu o fenótipo de susceptibilidade e aumentou a resposta imune adaptativa na LV. Ao determinar o papel de IL-1?, macrófagos IL-1? KO diferenciados a partir de precursores de médula óssea, quando infectados com L. infantum, apresentaram concentrações elevadas de óxido nítrico no sobrenadante da cultura, quando comparados aos macrófagos IL-1R KO ou WT, confirmando um papel regulador da resposta imune efetora à infecção por L. infantum. Os efeitos reguladores de IL-1? parecem ser mediados por IL-10, visto que animais IL-1R KO possuem redução significativa dessa citocina mensurada no fígado durante a 6ª semana de infecção. Em conjunto, no presente trabalho demonstramos que a sinalização via IL-1R, com prevalência da IL-1?, regula negativamente a resposta imune efetora do perfil Th1 e Th17 . Consequentemente, há o comprometimento da migração de neutrófilos para o sítio de infecção e ativação de macrófagos permitindo o escape do parasito e estabelecimento da infecção. The leishmaniases are a set of diseases caused by a protozoan of the genus Leishmania, which will lead to different clinical outcomes depending on the specific specie infecting the patient. The specie Leishmania infantum, already spread throughout Brazil and South America, is associated to visceral damage, mainly to the spleen, kidney and bone marrow, presenting high rates of morbidity and mortality. It is classically demonstrated that a response mediated by Th1 and Th17 lymphocytes promotes resistance to VL. Thus, the activated signaling pathways, as well as mediators generated during the infection, can assist in the development of a protective immune response. Among the expressed cytokines, the IL-1 (IL-1? e IL-1?) stands out for its important role in the inflammatory process and resistance to fungi and bacterial infections, and also in cutaneous leishmaniasis. In the present study, our aim was to determine the signaling role via IL-1R during experimental infection by L. infantum. Our results show IL-1? and IL-1? are produced in vivo infection by L. infantum, suggesting your participation in VL. Interestingly, the signaling pathway mediated by IL-1R appears to be involved in susceptibility to protozoal, since the animals genetically deficient for IL-1R (IL-1R KO) are more resistant to infection, presenting fewer parasites in the spleen and liver in chronic stages of infection. Additionally, the susceptibility phenomenon was associated with downregulation of the inflammatory response, mainly on Th1 cells and Th17. The neutrophilic inflammation has also been changed in IL-1R KO infected animals. Was observed an increase in neutrophils and production of CXCL1 (KC) in the spleen of IL- 1R KO animals, but without changing the activation profile of these cells. In determining the regulatory cytokine involved in the inflammatory process and therefore the susceptibility to the parasite, observed that IL-1? preferably has role in disease XX progression, since genic deletion of essential enzymes for IL-1? secretion (i.e.: Caspase 1/11) did not alter the number of parasites in the spleen and liver. However, in the absence of IL-1? was observed resistance profile associated with increased IFN-? and IL-17 in the target organ disease. Confirming the regulatory role, treatment with antiIL1? monoclonal antibody in WT mice infected reversed the susceptibility phenotype and increased adaptive immune response in LV. To determine the role of IL-1?, macrophages from IL-1? KO differentiated from bone marrow when infected with L. infantum showed high concentrations of nitric oxide in the culture supernatant when compared to macrophages from IL-1R KO and WT, confirming a regulatory role of the effector immune response to L. infantum infection. IL-1? regulatory effects appear to be mediated by IL-10, since IL-1R knockout animals have significant reduction of this cytokine measured in the liver during the 6th week of infection. Taken together, the present study demonstrated that IL-1R signaling pathway, with prevalence of IL-1?, negatively regulates effector immune response of Th1 and Th17 profile. Consequently, the impairment of neutrophil migration to the site of infection and activation of macrophages allowing the escape of the parasite and the establishment of infection.

A leishmaniose visceral (LV) é uma doença crônica e potencialmente fatal causada, no Brasil, pelo protozoário Leishmania infantum. A resistência à LV é relacionada ao desenvolvimento de uma resposta imune celular eficiente. Para isso, células dendríticas (DCs) reconhecem produtos antigênicos do parasito, através dos Toll-like Receptors (TLRs), e, após ativação, produzem fatores, que irão orquestrar o recrutamento celular e desenvolvimento da resposta imune protetora. Dentre os vários TLRs, tem sido demonstrado que o TLR9 está relacionado com a resistência à infecção em modelo de leishmaniose cutânea. No presente estudo, nosso objetivo foi determinar o papel do TLR9 durante a infecção por L. infantum. Nossos resultados demonstram que a expressão de TLR9 é modulada positivamente durante a infecção in vivo e in vitro pela L. infantum. O TLR9 é essencial para o controle da infecção contra L. infantum, uma vez que camundongos TLR9-/- são mais susceptíveis à infecção, apresentando maior número de parasitos no baço e fígado na 4ª e 6ª semana pós-infecção. A fenotipagem de leucócitos presente no baço demonstrou que camundongos TLR9-/- possuem menor número de neutrófilos. De maneira semelhante, a análise imuno-histoquímica mostrou uma reduzida marcação de células 7/4+ (específica para neutrófilos) no fígado de animais TLR9-/-. A falha na migração de neutrófilos observada em animais TLR9-/- não está associada a uma incapacidade de ativação dos polimorfonucleares, mas sim a deficiências na produção de fatores quimiotáticos, como CXCL1 (KC) e CXCL2 (MIP-2) nos órgãos-alvo. Investigando o mecanismo de ação, as células dendríticas oriundas de animais TLR9-/- falham em seu processo de ativação frente à infecção in vitro e in vivo com L. infantum, apresentando um fenótipo semi-maduro, com redução de expressão de MHC-II e moléculas coestimuladoras. Interessante que essas células apresentaram deficiência na produção de mediadores quimiotáticos de neutrófilos como KC e MIP-2, e consequentemente, induzindo o menor recrutamento de neutrófilos in vitro. Em conjunto, nossos resultados demonstram que o TLR9 é essencial para o controle da infecção por L. infantum, através de um mecanismo dependente do recrutamento de neutrófilos para o sítio inflamatório mediado pela ativação apropriada de fatores quimiotáticos de neutrófilos pelas DCs. Visceral leishmaniasis (VL) is a chronic and potentially fatal disease caused by protozoan Leishmania infantum, in Brazil. The resistance against VL is related to the development of cellular immune response. During infection, the dendritic cells (DCs) recognize antigenic products through Toll-like Receptors (TLRs) and then orchestrate the cellular recruitment and immune response development. Among several TLRs, it has been showed that TLR9 is related to resistance to cutaneous leishmaniasis. In the present study, our aim was to determinate the role of TLR9 during L. infantum infection. Our results demonstrated that TLR9 is up regulated during in vitro and in vivo L. infantum infection. TLR9 is critical for protective immunity against L. infantum, since TLR9-/- mice infected were more susceptible to infection, displaying high amounts of parasites in spleen and liver, at 4th and 6th weeks post-infection. Phenotyping the leukocytes into the spleen, TLR9-/- mice presented reduced neutrophils when compared to WT. Likewise, imunohistochemistry analyses showed the reduced of 7/4+ cells (specific to neutrophils) staining into the TLR-9-/- liver. The failure of neutrophil migration is not associated to their stage of activation impaired, but due the reduced levels of CXCL1 (KC) and CXCL2 (MIP-2) (neutrophil chemoatracttant) produced into into the spleen cells culture from infected TLR9-/-. Furthermore, DCs from TLR9-/- presented a semi-mature stage during in vitro and in vivo L. infantum infection, showing reduced expression of MHC-II and co-stimulatory molecule. Interestingly, the ability of DC to produce the neutrophil chemotact mediators (KC and MIP-2) was completely reduced by that derived from TLR9-/- mice, affecting neutrophil migration into Boyden chamber. Altogether, we demonstrated that TLR9 presents a critical role in the protective response against L.infantu through the mechanism dependent of crosstalk between neutrophil recruitment and DC activation.

Leishmaniose visceral (LV) é uma doença crônica e potencialmente fatal causada pelas espécies Leishmania infantum/chagasi e Leishmania donovani. O desenvolvimento da resposta Th1 é classicamente associado à proteção contra esses parasitos, mas também há uma correlação positiva entre a produção de citocinas relacionadas com o padrão Th17 e a proteção contra LV por L. donovani em seres humanos. No entanto, a participação de Th17 durante a infecção por L. infantum/chagasi permanece desconhecida. O objetivo desse estudo foi avaliar a participação de Th17 e citocinas relacionadas, além do mecanismo pelo qual tais células operam durante a resposta imune do hospedeiro contra L. infantum/chagasi. Os resultados mostraram que o parasito é capaz de induzir grandes quantidades de TGF-, IL-1, IL-6 e IL-23 por células dendríticas derivadas da medula óssea (BMDC), citocinas envolvidas na indução e/ou manutenção do perfil Th17. Assim, co-cultivando células do baço de camundongos C57BL/6 naves com BMDCs infectadas com L. infantum/chagasi observou-se uma significativa produção de IL-17 por células T. Esses achados foram confirmados por experimentos in vivo onde se constatou a produção de IL-17 no fígado e no baço de camundongos WT infectados, sendo o pico de produção dessa citocina observado na 4ª e 6ª semanas após a infecção. O padrão de resposta do tipo Th17 é crítica para a imunidade protetora contra L. infantum/chagasi, uma vez que camudongos IL-17R-/-, IL-23p19-/- e IL-6-/- mostraram aumento da carga parasitária nos órgãos alvo da infecção, sendo que a susceptibilidade observada em camundongos IL-17R-/- foi associada com o aumento da produção de IL-10 por linfócitos, sugerindo que a IL-17 regula negativamente a produção de IL-10 levando ao controle da infecção causada pelo parasito. Ainda, a ausência da sinalização via IL-17R gerou uma diminuição da inflamação hepática, decorrente de uma menor capacidade proliferativa de linfócitos frente ao estímulo com conA. De maneira interessante, na ausência de IL-10 houve potencialização na produção de IL-17 por camundongos infectados, e esses foram mais resistentes à infecção, apresentando números reduzidos parasitos no baço e no fígado. Além de promover proteção através da modulação de IL-10, a IL-17 foi capaz de potencializar a produção de NO in vitro e in vivo. Juntos, nossos resultados demonstram que a L. infantum/chagasi é capaz de desencadear o padrão Th17 de resposta imune, o qual promove proteção do hospedeiro durante a infecção. The development of Th1 response is classically associated with protection against these parasites, but recent data also show that there is a positive correlation between the Th17-related cytokines production and the protection against LV by L. donovani in humans. However, the role of this CD4+ T cells subset during L. infantum/chagasi infection remains unknown. In this study, our aim was to evaluate the Th17 and related cytokines participation, besides the mechanism by which these cells playing during the L. infantum/chagasi infection.The results showed that the parasite induces high amounts of TGF-, IL-1, IL-6, and IL-23 by bone marrow-derived dendritic cells (BMDC), cytokines involved with Th17 induction and/or maintenance. Accordingly, naïve-C57BL/6 spleenocytes co-cultured with L. infantum/chagasi-infected BMDC produced significant amounts of IL-17 by TCD4+. Interestingly, IL-17 was produced in high amounts in the liver and spleen of WT infected-mice, being peaked at 4th and 6th weeks after infection. The Th17 is critical for protective immunity against L. infantum/chagasi, since that IL-17R-/-, IL-23p19-/- and IL-6-/- infected-mice showed increasing of parasite load associated with enhancement of IL-10 production in IL-17R-/- compared to WT infected-mice. Strictly, in the absence of IL-17 signaling, a smaller inflammatory infiltrate was observed in the liver. Interestingly, IL-17 production was potentiated in IL-10-/- infected-mice, and they were more resistant to infection, showing reduced parasites numbers in the spleen and liver. In addition to promoting protection through the downmodulation of IL-10, IL-17 enhanced the NO production. Together, our results demonstrate that L. infantum/chagasi trigger Th17 response that promotes the host protection during infection.
