7. Hipertensão neonatal: quando é apenas transitória

A hipertensão neonatal é um problema clínico raro. A principal causa é renovascular. Os autores apresentam uma causa rara de hiperaldosteronismo transitório secundário a infeção renal. Um recém-nascido (RN) de termo de 22 dias, sem antecedentes de relevo, é enviado ao serviço de urgência por má evolução ponderal. Após análises de sangue e urina é diagnosticado com uma infeção do trato urinário (ITU) e iniciada terapêutica com ampicilina e cefotaxima. Durante a hospitalização, foram observados níveis persistentes de pressão arterial (PA) >P97, tendo sido realizado ecografia renal e eco-Doppler que não mostraram alterações, assim como o estudo hormonal e a função renal analítica normais, excluindo hiperplasia adrenal congénita. Em D9 de internamento, apresentou normalização dos valores de PA. As análises laboratoriais revelaram níveis de aldosterona de 335,4ng/dl (N 2-70 ng/dl) e renina de 1684 mUI/L (N

O Neuroblastoma é um tumor das células primitivas do sistema nervoso simpático com idade média de diagnóstico de 17 meses, representando 10% das neoplasias da infância e sendo responsável por 15% das mortes por cancro na infância. Os sintomas clínicos são variáveis conforme a localização, e tem também uma gravidade e prognóstico muito variáveis. Descrevemos um caso de uma criança de dois anos que recorre ao serviço de urgência (SU) com queixas iniciais de gemido intermitente e desconforto ocasional, acabando por lhe ser diagnosticado um neuroblastoma cervical.

A torção do cordão espermático é uma emergência cirúrgica bem reconhecida na prática clínica. Contudo, a sua apresentação clínica num doente com criptorquidia é atípica, podendo simular um abdómen agudo. Isto pode atrasar o diagnóstico e, por conseguinte, o devido tratamento. Perante casos de torção do cordão espermático, a investigação e orientação precoces são fundamentais, de forma a evitar complicações e preservar a função testicular. Desta forma, é essencial o reconhecimento desta entidade clínica pelos clínicos para a sua intervenção atempada. Os autores descrevem um caso de torção de cordão espermático num testículo criptorquídico no canal inguinal numa criança de nove anos, que se apresentou com dor abdominal, inicialmente peri-umbilical, com posterior migração para a fossa ilíaca direita.

10. Mycoplasma Pneumoniae uma infecção emergente? Relato de casos e revisão da literatura

A bactéria Mycoplasma pneumoniae (M. pneumoniae) é uma causa comum de pneumonia adquirida na comunidade em idade escolar, podendo manifestar-se sob variadas formas. Os autores reportam dois casos de infeção por M. pneumoniae. Rapariga 10 anos, clinicamente com febre, cefaleia e tosse seca com quatro dias de evolução. Audíveis crepitações e sibilos inspiratórios no campo pulmonar direito. Imagiologicamente verificou-se hipotransparência na base direita associada a derrame pleural ipsilateral. O painel PCR (Polymerase Chain Reaction) respiratório foi positivo para M. Pneumoniae. Adolescente 13 anos, asmática, clinicamente com tosse seca e dispneia com uma semana de evolução. Apresentava sinais de dificuldade respiratória, hipoxemia e, sibilos e crepitações bilaterais. Imagiologicamente verificou-se infiltrado bibasal com derrame pleural de pequeno volume. O painel PCR respiratório identificou o M. Pneumoniae. Os dois relatos de caso corroboram diversidade de sintomas e gravidade clínica que a infeção porM. pneumoniae pode manifestar: desde sintomas respiratórios superiores a inferiores ou mesmo sintomas extra-respiratórios como, manifestações mucocutâneas, anemia hemolítica ou alterações neurológicas. Para o diagnóstico estão disponíveis as serologias e a pesquisa por PCR da bactéria nas secreções respiratórias; no entanto, estes exames apresentam baixa sensibilidade e especificidade, respetivamente. Assim, o diagnóstico deve assentar essencialmente na clínica. A terapêutica de primeira linha são os macrólidos. Globalmente, tem-se verificado um aumento das infeções por M. Pneumoniae com crescentes resistências aos macrólidos e formas de apresentação mais graves. Torna-se, portanto, fundamental uma elevada suspeita diagnóstica por forma a realizar o diagnóstico precocemente e instituir atempadamente a terapêutica adequada.

A classificação da perturbação neurológica funcional (PNF) tem evoluído ao longo da História, desde fenómenos sobrenaturais até à mais recente síndrome conversiva. Não estão disponíveis dados nacionais, mas de acordo com a literatura estima-se uma incidência em Neuropediatria de cerca de 23%. O diagnóstico, anteriormente considerado de exclusão, hoje é afirmativo, exigindo por isso uma avaliação neurológica cuidadosa e sobretudo uma comunicação eficaz no sentido de desmistificar estigmas e promover a aceitação da doença. Uma abordagem biopsicossocial é essencial, permitindo identificar fatores desencadeantes e que perpetuam os sintomas. As estratégias terapêuticas incluem: avaliação multidisciplinar, comunicação empática do diagnóstico, plano de tratamento personalizado e envolvimento familiar. Por fim, a comunicação eficaz e a intervenção precoce são cruciais na gestão da PNF em adolescentes. Embora a literatura apresente estratégias promissoras, a implementação prática pode variar sendo necessárias normas de orientação e dados estatísticos nacionais para otimizar os cuidados.

12. Massagem Infantil em Lactentes: efeitos e potenciais vantagens à luz da evidência atual

A massagem infantil é uma terapia complementar que tem suscitado um interesse crescente na comunidade científica pelos seus potenciais benefícios no alívio sintomático de condições pediátricas frequentes. O seu mecanismo fisiopatológico ainda não está completamente estabelecido, porém é uma prática que favorece a comunicação e o vínculo com os cuidadores, promovendo uma sensação de conforto e bem-estar do lactente. Segundo a literatura mais recente, a massagem infantil parece ser promissora no alívio da dor, na diminuição da icterícia, no tratamento adjuvante da obstipação funcional, na melhoria da qualidade do sono e no crescimento e desenvolvimento infantis. Assim, como terapêutica não farmacológica, parece ter benefícios, sendo praticamente desprovida de efeitos adversos. Contudo, atualmente, ainda existem poucos estudos randomizados acerca desta temática, pelo que a qualidade da evidência é baixa, constituindo uma limitação para a sua recomendação universal na prática clínica.

13. Referenciação Hospitalar em Idade Pediátrica - o olhar dos cuidados de saúde primários

Introdução: A referenciação hospitalar, cumprindo critérios bem estabelecidos e seguindo parâmetros de qualidade, permite uma melhor gestão de recursos humanos e económicos. O nosso objetivo foi caraterizar a referenciação à consulta hospitalar em idade pediátrica de uma Unidade de Saúde Familiar pertencente ao Agrupamento de Centros de Saúde do Grande Porto III, durante o ano de 2019. Material e métodos: Estudo retrospetivo e descritivo que incluiu indivíduos em idade pediátrica, que frequentaram as consultas de Saúde Infantil e Juvenil desta Unidade de Saúde Familiar, durante o ano de 2019 e que foram referenciados a consulta hospitalar via AlertP1®. Resultados: A taxa de referenciação global foi de 11,1%, 40,3% no grupo etário com idade superior a nove anos. As consultas hospitalares mais solicitadas foram as das áreas cirúrgicas/médico--cirúrgicas. 88,9% foram motivadas por patologias do foro somático (vs 11,1% do foro do desenvolvimento). Verificou-se uma associação estatisticamente significativa entre entidade de quem partiu a iniciativa para a necessidade de referenciação e motivo da referenciação, sendo as patologias do desenvolvimento/comportamento maioritariamente sinalizadas pela Escola/Infantário. Quanto à qualidade das referenciações, a maioria foi classificada como “Aceitável”, com uma correlação positiva entre anos de prática clínica e má qualidade das referenciações. Verificamos que mais de 50% das consultas não respeitaram os 120 dias admitidos para uma referenciação de prioridade normal fixados pela portaria que regula a “Consulta a Tempo e Horas”. Conclusão: A nossa taxa de referenciação foi superior à observada por outros autores, com 5,2% dos pedidos a serem recusados.

A discinesia ciliar primária é uma síndrome autossómica recessiva, de clínica muito variável. Se associada a situs inversus, sinusite crónica e bronquiectasias, denomina-se Síndrome de Kartagener. A sua clínica pode ser muito variável e inespecífica, incluindo dificuldade respiratória neonatal. Descrevemos um caso de Síndrome de Kartagener num bebé de um ano, que se manifestou aos seis dias com dificuldade respiratória, de agravamento progressivo. Este caso representa o paradigma da importância da atuação do Médico de Família, na valorização dos sinais e sintomas, que permitiu o diagnóstico precoce de uma síndrome pouco prevalente.

A prevenção da obesidade infantil é uma questão crítica para a saúde pública, pois a grande maioria do excesso de peso é adquirido ainda em idade pré-escolar. De acordo com a Organização Mundial de Saúde, em 2016, 41 milhões de crianças apresentavam excesso de peso ou obesidade com menos de 5 anos. A obesidade é difícil de combater, persistindo na idade adulta, e aumenta o risco de doenças crónicas. Embora os comportamentos de estilo de vida modificáveis, como alimentação saudável e atividade física, sejam determinantes no sucesso do combate à obesidade, outros fatores da sociedade atual decorrentes do avanço tecnológico têm um peso marcado nesta doença. Para prevenir a obesidade na primeira infância, é importante entender os fatores de risco e as suas interações. As intervenções multidisciplinares demonstram ser eficazes, quando há disponibilidade e motivação, principalmente do ambiente familiar e escolar, onde as crianças estão inseridas. No entanto, desenvolver e avaliar intervenções complexas para a prevenção da obesidade é um desafio para todos os envolvidos, profissionais de saúde, crianças, famílias e outros cuidadores.Esta revisão tem como objetivo discutir as linhas mais recentes para a prevenção e abordagem da obesidade infantil, analisando as evidências das intervenções e discutindo as barreiras da sua aplicação.

19. Educational strategies for preventing accidents in childhood: a systematic review

Abstract: Accidents in childhood have a high morbidity and mortality rate and are often preventable, which reinforces the importance of educational measures to prevent unintentional injuries. This study aimed to identify and describe useful educational strategies for preventing childhood accidents in communities. This systematic review was guided by PRISMA (2020) and registered on the PROSPERO platform (ID: CRD42024500956). A search strategy was developed by combining the descriptors “Accident Prevention”, “Child”, and “Health Education” with the Boolean operator AND, applied to the PubMed/MEDLINE, Web of Science, LILACS, and SciELO databases. A total of 5,037 studies were located, including observational articles published from 2018 to 2023, with children aged 0-12 years and/or their parents/caregivers. The quality of the studies was assessed based on the Qualitative Studies Checklist and the Research Triangle Institute Item Bank instruments. The bibliographic sample consisted of 30 articles, mostly classified as high quality, with a population of 4,510 adults and 54,190 children from various countries. Educational strategies for accident prevention were described, aimed at parents and guardians, children, and both. This review, addressing innovative educational strategies for preventing childhood accidents, highlights playful approaches for children and visual methods for caregivers. Implementation faces challenges related to evaluation and socioeconomic factors, making rigorous criteria and prolonged follow-ups important for continuous effectiveness.

20. Clinical aspects of co-infection by HIV and syphilis in pregnancy and exposure in newborn intended at Pediatric Hospital of Santa Catarina

Introduction: Sexually transmitted infections frequently affect pregnant women and, consequently, newborns. HIV and syphilis are vertically transmitted to children and co-infection requires special attention due to its clinical implications. Objective: To describe clinical aspects of HIV/syphilis coinfection during pregnancy and the exposure of newborns to infections treated at a pediatric reference hospital in Santa Catarina between 2015 and 2020. Methods: Observational, descriptive study, secondary to a line of research “Epidemiological description of children exposed to HIV” from January 2015 to December 2020 in a tertiary pediatric hospital in Santa Catarina. Results: 678 medical records were analyzed with ICD Z.206 (contact with and exposure to HIV), in which 71 (10.5%) newborns were exposed to HIV and Syphilis co-infection. Of these, 37 (52.1%) were male, 14 (19.7%) confirmed a diagnosis of HIV and 30 (42.2%) of congenital syphilis. Of the co-infected pregnant women, 38 (53.5%) were diagnosed with HIV prior to pregnancy, 53 (74.6%) used ART and 40 (52.1%) used harmful substances. Furthermore, 34 (46.4%) pregnant women had a minimum number of 6 prenatal consultations, 35 (49.3%) had vaginal births and 29 (40.8%) had undetectable HIV RNA quantification at the time of delivery. HIV prophylaxis for newborns occurred in 66 (92.9%) of cases. Conclusion: It is concluded that the studied population was mostly made up of pregnant women who were diagnosed with HIV infection prior to pregnancy. Of these, the majority were using ART, but the minority carried out the minimum number of consultations recommended by the Ministry of Health. Regarding newborns, the majority received HIV prophylaxis.

Introduction: Burns are tissue injuries caused by contact with heat sources. They represent a global public health problem. They have major impacts on children. The severity and intensity of burns are related to the etiological agent and their consequences are a major obstacle for the patient. Method: The study analyzed data on hospital admissions for burns in children from 2012 to 2022, using information from SIH-SUS. Hospitalization rates per 100,000 inhabitants were calculated for each year, considering dependent variables. The researchers used standardized coefficients and simple linear regression to analyze the data. Results: Data from 91,091 hospitalizations for burns in children, 0-14 years old, were included. There was stability in the general hospitalization rate (mean rate 17.963; ß = 0.119; p = 0.163). In females there was an increasing trend (mean rate 14.346; ß = 0.169; p = 0.029); in males, there was stability (mean rate 21.426; ß = 0.069; p = 0.504). Females behaved with stability in all age groups; the male with an increase in the range 0-4 years (mean rate 42.264; ß = 0.613; p = 0.003), stability in the range 5-9 years (mean rate 14.189; ß = -0.21; p = 0.867) and reduction in the range 10-14 years (mean rate 9.871; ß = -0.328; p = 0.007). The South Region demonstrated an increasing trend (mean rate 26.952; ß = 1.091; p = 0.001). Conclusion: There was stability in the general hospitalization rate. The female sex tended towards increase and the male towards stability. There was stability in Brazilian regions, except in the South.

ABSTRACT Objective: The aim of this study was to describe the epidemiological characteristics of pediatric patients with sickle cell disease (SCD) and evaluate the use of medicinal plants in these patients concomitantly with their drug treatment. Methods: This is a review of the medical records of pediatric patients at a public institution with tertiary care. The selection criterion was to be a child or adolescent with SCD undergoing pediatric follow-up at this outpatient clinic. In the medical records of the patients selected, records of the use of plants for medicinal purposes were sought. Results: In total, 154 records (100% of total active patients followed in this clinic) were reviewed: 99 children and 55 adolescents. The predominant genotype was SS (58.4%), followed by SC (29.2%). The use of at least one medication for SCD was reported in 95.5% of the medical records. The use of medicinal plants was reported by 70.1% of patients, with 276 citations in the medical records referring to 64 different types of plants. Six plants were used for the treatment of SCD, the main one being Lemonvine/Ora-pro-nóbis (Pereskia aculeata). The use of medicinal plants was reported for flu-like symptoms and/or COVID-19 (both for prevention and treatment) by 60.5% of the citations, with 35 different plants reported for this use, exclusively or not. This use was followed by pain symptoms (14.2% of citations). Conclusions: The majority of patients in this study use both conventional and traditional/complementary/alternative medicine, highlighting the need for more studies in the area, with a special focus on patient safety.

ABSTRACT Objective: To recognize and address Patau’s syndrome, despite its rarity and associated low life expectancy, through the presentation of a case study of a 2-year-old patient receiving Home Care services. Case description: We present a female patient who defied the odds with a prolonged survival, possible due to Home Care. She was delivered via cesarean section at 31 weeks + 4 days due to restricted uterine growth. The mother, aged 36, had received proper prenatal care and was in good health. The diagnosis of Patau’s syndrome was confirmed through karyotyping after birth. Despite the severe clinical nature of the case, the patient, now with two years old, receives specialized home-based care, supported by a tracheostomy and gastrostomy. A dedicated 24-hour nursing technician ensures continuous monitoring, and the patient benefits from regular medical check-ups, physiotherapy five times a week, weekly speech therapy sessions, monthly consultations with a nutritionist, and ongoing psychological support for her family members. Comments: This multidisciplinary approach has resulted in a slight motor response, highlighting the positive impact of comprehensive care on her overall well-being. The existence of a robust support network for families facing similar challenges is crucial, and a multidisciplinary care can effectively prevent complications associated with this impactful syndrome.

Methods: Cross-sectional study, developed in a Brazilian public and tertiary hospital, between December 2021 and March 2022. Data collection was hybrid, and three instruments were used: Sociodemographic Questionnaire, Durel Religiosity Scale, and the Alcohol Use Disorders Identification Test. For statistical analysis, the following tests were used: χ2, Fisher’s Exact, Mann-Whitney and Spearman’s Correlation Coefficient, in addition to analyses of linear correlation strength and bivariate logistic regression. Introduction and Objectives: Attention-Deficit/Hyperactivity Disorder (ADHD) affects 2.2% of children and adolescents. The risk of substance use disorders (SUD) is twice as high among people with ADHD, requiring targeted approaches. This study aimed to evaluate the effect of methylphenidate on the development of substance use disorders among children and adolescents with ADHD, including alcohol, tobacco, and other drugs. Methods: A systematic search was conducted on January 26th, 2023. Characteristics of the study and participants were extracted and the risk of bias evaluated. Results: From the 2,854 articles identified and 14 registry entries, seven reports were included in the review, accounting for 860 participants, with 526 from intervention (61.2%) and 334 from control groups (38.8%). Treatment with methylphenidate was consistently protective against drug use. Discussion: Methylphenidate treatment may be effective in reducing SUD risk, especially in young drug users. Preventive measures are needed, including health promotion and interventions tackling health determinants, particularly in vulnerable groups. Further high-quality studies are required to strengthen our findings.

Introduction and objectives: Deliberate self-poisoning (DSP) is a frequent cause of admission at the emergency department during adolescence. Since the beginning of the COVID-19 pandemic, a higher number of cases was reported. This retrospective study aimed to determine DSP prevalence, between 2018 and 2022, as well as the medication used and the method for obtaining it, within the catchment area of a Tertiary Pediatric Hospital Emergency Department (ED) in Portugal. Methods: Retrospective study, including patients aged 10 to 17 that lived in the catchment area of the ED and were admitted for DSP less than 7 days prior. By searching with the keywords “DSP” and “deliberate self-poisoning”, 2975 records were found, of which 1403 cases were included. Statistical analysis was performed in SPSS with a significance level of 0.01. Results: Out of 1403 cases, 87.9% were female, mostly in mid-adolescence and with a previous follow-up appointment with a child and adolescent psychiatrist. The patient’s own psychotropic medication were the most used drugs. Episodes with no suicidal intention were more frequent. After the ED, most cases were discharged with a referral for psychiatric follow-up. There was a higher number of DSP in 2021 (p < 0.01), with a prevalence of 123.8/100,000. Discussion: DSP is growing among adolescents. Cases increased significantly in 2021. We highlight the need for prevention campaigns and vigilance systems to monitor and modify this trend.

Introduction and objectives: To evaluate compliance with per capita food supply at lunch in daycare centers. Methods: Visits were made on five consecutive days at lunchtime to six daycare centers enrolled in the “Creche com Sabor e Saúde” project (C2S), meaning “Daycare Centers with Taste and Health”. To quantify food portions for different age groups (6-8 months, 9-11 months, 12-23 months, and 24-35 months), each meal component (carbohydrate sources, protein sources, and vegetables) and dessert (fruit) underwent random selection and was weighed three times. The averages obtained were compared to C2S project guidelines for per capita food supply, in line with national and international recommendations. Results: One hundred and forty-five dishes and 244 desserts were analyzed. No dishes were served to infants under nine months. Per capita amounts for protein-supplying foods were fulfilled (9-11 months: 14.2 g; 12-23 months: 20.3 g; 24-35 months: 23.9 g). Carbohydrate-rich foods were provided in quantities exceeding the recommendations, particularly for the 12-23 month age group (62.2 g vs. 40 g). For infants under 12 months, no vegetables were offered, and above this age, the average amount provided fell below the minimum guideline of 20 g (12-23 months: 9.4 g; 24-35 months: 10.8 g). The quantity of fruit offered exceeded the recommendations for the 6-8 month (84.6 g > 30-50 g) and 9-11 month (133.3 g > 40-70 g) age groups, aligning with guidelines for the 12-35 months. Discussion: Daycare centers inconsistently followed per capita food supply guidelines for lunch, with deficits in vegetable intake and excesses in carbohydrate-rich foods and fruit that surpassed the recommended quantities.

The Portuguese Neonatal Society updates the growth charts recommended for term and preterm infants. The suitability of the growth chart depends on the gestational age, the purpose of the measurement, and the life cycle stage. To classify intrauterine growth at birth, the Fenton 2013 growth charts, which are based on anthropometric records at birth, are the most appropriate for both term and preterm infants. For monitoring postnatal growth in full-term infants, the WHO 2006 Growth Prescriptive Standards are strongly recommended. To specifically monitor weight loss in the initial postnatal days, the NEWT® (http://newbornweight.org) nomogram is recommended. To assess body weight changes in preterm infants while in hospital, an accurate open-access online calculator (www.growthcalculator.org), based on weight trajectories that take into account the initial physiological weight loss, is recommended. The Fenton 2013 growth charts can be employed concurrently to monitor growth in length and head circumference. To assess growth in preterm infants following their discharge from hospital, the Intergrowth-21 prescriptive standards are appropriate for infants born at more than 27 weeks of gestation, up to 64 weeks postmenstrual age. Beyond this age, the prescriptive WHO 2006 growth standards should be employed.

Introduction: Bezoars are masses resulting from the ingestion and accumulation of undigested material in the gastrointestinal tract. They can be divided into trichobezoars (hair), phytobezoars (plant fibers), pharmacobezoars (drugs), and lactobezoars (cow’s milk). The most common symptoms are abdominal pain, nausea, vomiting, anorexia, and weight loss. Case report: We report the clinical case of a 13-year-old boy with a trichobezoar that had developed over the course of several years, which resulted in gastric perforation. A surgical laparotomy was performed to remove the trichobezoar and close the gastric wall. Discussion: Trichobezoars can present in the form of an acute abdomen. In these situations, surgical treatment is necessary.

Objective: to describe nursing professionals' knowledge of preparing and administering insulin to hospitalized children and adolescents. Methods: a cross-sectional study with 64 nursing professionals from a highly complex public children's hospital, recruited by convenience sampling. A questionnaire was used to collect data, and the level of professional knowledge and the quality of care were analyzed using the positivity index. The ethical and legal principles of research were complied with. Results: there was a predominance of women (94.4%) with an average age of 37.16+8.09 years and an average time working in pediatrics of 13.90+8.91 years. Of the 19 items on insulin, those considered poor assistance (eight) prevailed, followed by borderline assistance (five), which may have an impact on the preparation and administration of insulin with glycemic and metabolic repercussions for pediatric inpatients. Conclusion: weaknesses were found concerning nursing professionals' knowledge of various aspects of insulin use in children and adolescents in the hospital environment. Contributions to practice: the study addresses gaps in the nursing team's knowledge of insulin injection, providing opportunities for improvement.

ABSTRACT Objective: The aim of this study was to describe the phases of a decannulation protocol and the results from its application in hospitalized children. Methods: This is a retrospective, observational study. Data were collected from medical records of decannulated patients followed up in a pediatric hospital in Belo Horizonte, Minas Gerais between 2011 and 2021. Results: Among the children followed up in the service (n=526), 23% (n=120) were successfully decannulated. Children aged between 2 months and 16 years, with a mean age of 4 years, 69% of whom were male, were evaluated. About 75% of the patients have tracheostomy due to upper airway obstruction and 60% of these due to acquired subglottic stenosis. At the beginning of the decannulation protocol, 5.5% of the patients had moderate oropharyngeal dysphagia, while 80.4% had normal swallowing. Correction in the upper airway pre-decannulation was performed in 39.5% of the patients, dilation in 63.8%, and endoscopic correction was performed in 55.3%. After performing the decannulation, none of the patients had complications. Conclusions: The described decannulation protocol is safe, since no complications such as death and need for recannulation happened.

36. Brazilian children’s quality of life during the COVID-19 pandemic: analysis of contextual factors and dimensions

ABSTRACT Objective: The aim of this study was to investigate the contextual factors associated with the quality of life (QOL) of Brazilian children aged 0–12 years during the strict period of social isolation. Methods: This observational cross-sectional study was conducted between July and September 2020 using an online questionnaire on QOL-related family factors and the Pediatric Quality of Life Inventory (PedsQL™). Results were analyzed by multinomial logistic regression analysis. Results: The sample had 849 children, mostly from the South Region of Brazil (75%), white (83%), with typical development (79%), sedentary (68%), using screen (85%) for >3 h/day (44%). Their mothers were their main caregivers (90%). The following variables were significantly associated with high scores of QOL: typical health status (OR 2.38; 95%CI 1.60–3.55; screen time ≤2 h/day (OR 1.62; 95%CI 1.17–2.24); social distancing considered as “easy” (OR 1.67; 95%CI 1.20–2.32), and stimulation of the child by the family (OR 1.93; 95%CI 1.08–3.45). Conclusions: This study indicates that the family context can influence children’s QOL, especially during the COVID-19 pandemic and home environment reorganization.

ABSTRACT Objective: To compare the presence of neutralizing antibodies against SARS-CoV-2 found in the breast milk and blood of vaccinated lactating women with those not vaccinated. Data source: The study was registered in the International Prospective Register of Systematic Reviews (PROSPERO) under CRD42021287554 and followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Cohort, case-control, and cross-sectional studies that evaluated antibodies against SARS-CoV-2 in the milk and blood of vaccinated mothers and had as control group unvaccinated mothers were eligible. Health Sciences Descriptors (DeCs), Medical Subject Headings (MeSH) and Emtree descriptors were used for the Virtual Health Library (VHL), Medical Literature Analysis and Retrieval System Online (Medline/Pubmed), and Embase databases, respectively. In the Web of Science and Scopus, the strategy was adapted. No restrictions on the publication period and language were set. Data synthesis: The search identified 233 records, of which 128 duplicates and 101 papers that did not meet the inclusion criteria were excluded. Hence, four cohort studies were eligible. Nursing mothers vaccinated with the Pfizer-BioNTech and Moderna vaccines showed antibodies against SARS-CoV-2 in their blood and breast milk. Conclusions: Vaccinated lactating women had higher levels of immunoglobulin G (IgG) and A (IgA) in serum and breast milk than unvaccinated women.

ABSTRACT Objective: To describe how smartphone applications can contribute to the management of epilepsy in children and adolescents. Data source: This is an integrative review conducted on the Medline, PubMed, and SciELO databases, based on the descriptors “epilepsy” and “smartphone.” Original studies published between 2017–2023 in Portuguese or English that addressed the research question were included. Theses and dissertations, duplicate studies, literature reviews, and studies that did not answer the research question were excluded. Data synthesis: A total of 178 studies were located, of which six were selected for this review. The sample included 731 participants (631 children and adolescents with epilepsy and 100 caregivers). The applications allow for the collection of seizure frequency; timing and type of crisis; reminders for medication administration; and information about sleep quality. They can store these data for healthcare professionals, caregivers, and users to monitor the progress of the condition. Conclusions: The use of applications in managing seizures in children and adolescents with epilepsy shows promising results by promoting continuous and personalized monitoring. Further studies are needed to optimize beneficial outcomes and overcome challenges.

The FRE was classified as “does not practice,” “practices ≤300 min/week,” and “practices >300 min/week” by the Physical Activity Questionnaire for Adolescents. Poisson and multinomial logistic regressions, adjusted for sex, sexual maturation, and nutritional status analyzed the association of FRE and BID. Results: The BID prevalence was 28.3%; 52.4% of the adolescents wanted to reduce their silhouettes; and 48.7% did not practice FRE. Adolescents who practiced FRE >300 min/week had a 28% higher prevalence for some level of BID (PR 1.28; 95%CI 1.08–1.52) and a 46% lower chance of wanting to reduce silhouettes (OR 0.54; 95%CI 0.35–0.82), compared to nonpractitioners. There was no interaction between FRE and nutritional status in association with BID. Conclusions: The adolescents who practice FRE >300 min/week are likely to have some level of BID and are less likely to report the desire to increase their silhouettes, regardless of their nutritional status.

ABSTRACT Objective: To verify the use and identify advantages of molecular methods for congenital infections diagnosis in cerebrospinal fluid of neonates. Data source: The review was registered in the International Prospective Register of Systematic Reviews (PROSPERO), under CRD42021274210. The literature search was performed in databases: PubMed, Virtual Health Library/ Latin American and Caribbean Center on Health Sciences Information (VHL/BIREME), Scopus, Web of Science, Excerpta Medica database (EMBASE), Cochrane, ProQuest, and EBSCOhost. The search was carried out from August to October 2021 and updated in December 2022, respecting the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The selection sequence was: 1) Duplicate title removal; 2) Examination of titles and abstracts; 3) Full-text retrieval of potentially relevant reports; and 4) Evaluation of the full text according to eligibility criteria by two independent authors. Inclusion criteria considered randomized and non-randomized control trials, longitudinal, cross-sectional, and peer-reviewed studies in humans, published in English, Spanish, Italian, and Portuguese, with newborns up to 28 days old who had congenital neuroinfections by toxoplasmosis, rubella, cytomegalovirus, herpes simplex (TORCH), and others such as Treponema pallidum, Zika, parvovirus B-19, varicella zoster, Epstein-Barr, and SARS-CoV2, diagnosed by polymerase chain reaction (PCR). Two evaluators extracted the following information: author, year of publication, nationality, subjects, study type, methods, results, and conclusion. Data synthesis: The most studied pathogen was herpes simplex. Several articles reported only nonspecific initial symptoms, motivating the collection of cerebrospinal fluid and performing PCR for etiological investigation. Conclusions: Molecular methods are effective to detect pathogen genomes in cerebrospinal fluid, which can impact clinical evolution and neurological prognosis.

ABSTRACT Objective: To describe cases of acute kidney injury (AKI) in children diagnosed with COVID-19, associated risk factors, clinical aspects and outcome of cases. Methods: Retrospective study, carried out in a pediatric hospital between March 2020 and September 2021, with patients with COVID-19 who were diagnosed with AKI, studying information present in medical records such as comorbidities, age, gender and use of nephrotoxic medications. Results: We studied 40 cases, and male individuals were significantly more affected (62.5%; p=0.025). AKI was a severe complication of COVID-19 infection, with 100% of the sample requiring admission to the Intensive Care Unit and 22.5% dying. The most prevalent comorbidities analyzed in this study were epilepsy, cerebral palsy and heart disease. Most patients were classified according to Kidney Disease: Improving Global Outcomes (KDIGO) criteria as KDIGO 1 (42.5%), and required orotracheal intubation (67.5%). The frequency of use of nephrotoxic medications and need for dialysis was low, with percentages of 35 and 17.5%, respectively. Among the children who died, 70.4% had some comorbidity and 88.8% received invasive ventilation. Conclusions: AKI in children with COVID-19 infection is associated with severe conditions. Despite the severity, most patients were discharged alive from the hospital.

ABSTRACT Objective: The aim of this study was to evaluate the functioning and associated factors in children and adolescents with osteogenesis imperfecta (OI). Methods: This is a cross-sectional study conducted on 30 children and adolescents with OI. Medical records, use of bisphosphonates, socioeconomic status, handgrip strength, balance, joint hypermobility, ambulatory level, and the Pediatric Evaluation of Disability Inventory—Computer Adaptative Test (PEDI-CAT) scores were assessed. Data is presented as mean and standard deviation and Student’s t-test or Mann–Whitney U test. Categorical data is presented as frequency and analyzed using Fisher’s exact test. Within-group analyses were conducted using ANCOVA or Wilcoxon signed-rank test. Correlations used Kendall’s Tau-b test. Results: The participants involved in this study were 6–18 years old. The sample was separated into two groups according to disease severity. The moderate/severe OI group (n=10) presented a lower height and muscular strength than the mild group (n=20). Muscle weakness was observed in all participants with OI when compared with the normal population. No differences were observed between the groups in the PEDI-CAT scores except for the mobility domain. There were correlations between the PEDI-CAT mobility domain and the number of fractures, OI type, weight, and balance; there was also a correlation between the PEDI-CAT daily activities, mobility, responsibility, and social/cognitive domains. Conclusions: The findings suggest that children with moderate/severe forms of OI can achieve the same function levels as children with mild OI. Fractures can have a major influence on the functional level, and treatment should focus on the prevention and rehabilitation of these events when they occur.

ABSTRACT Objective: The objective of this study was to establish the genotype-phenotype correlation between karyotype results and the neurological and psychiatric alterations presented in patients with Turner syndrome (TS). Methods: A retrospective study was conducted on the medical records of 10/140 patients with TS and neurophysiological abnormalities seen at a university hospital in southern Brazil. In addition, a literature review spanning the period from January 1, 2012 to January 1, 2023 was carried out using the PubMed and Virtual Health Library databases. Results: Our study showed a potential correlation between neurological and psychiatric alterations in patients with TS. These findings are in accordance with those described in literature such as a high prevalence of learning or intellectual disabilities. However, our sample found more seizure episodes than those reported in other studies. Conclusions: The correlation established could be due to X chromosome dose–effect, as the review suggests that sex chromosome number and hormonal development can be associated with verbal, social, and cognitive skills or impairments.

ABSTRACT Objective: In order to study the scenario of respiratory infections in pediatrics after the emergence of SARS-CoV-2 in Brazil, this study aimed to compare characteristics of children admitted for SARS or upper airway infection caused by either RSV or SARS-CoV-2. Methods: This was a cross-sectional study involving children up to 48 months of age admitted to a tertiary pediatric hospital with a diagnosis of SARS or upper airway infection between April of 2020 and April of 2021. Respiratory secretion samples were collected 2-5 days after hospitalization, and antigen/PCR tests for viral etiologies were performed. In this analysis, patients with laboratorial diagnosis of SARS-CoV-2 and/or RSV were selected, and their clinical and epidemiological characteristics were compared using logistic regression. Results: Our sample initially comprised 369 participants. SARS-CoV-2 and RSV infections were confirmed in 55 (15%) and 59 children (16%), respectively. Mean age was 12 months (0-48 months), and 47 were female. The following characteristics were significantly more frequent in patients with RSV when compared with those with COVID-19: younger age (OR = 0.94; 95% CI: 0.90-0.98); lower frequency of fever (OR = 0.18; 95% CI: 0.05-0.66); and more frequent upper airway symptoms: cough (OR = 7.36; 95% CI: 1.04-52.25); and tachypnea (OR = 6.06; 95% CI: 1.31-28.0). Conclusions: Children with RSV-related SARS were younger, had lower frequency of fever at admission, but had a higher frequency of signs of upper airway infection and lower systemic inflammation when compared with children hospitalized for COVID-19 during the first year of the pandemic.

- Abstract
Introduction: Dehydration is the contraction of the extracellular volume due to reduced fluid intake or water loss from the body, usually associated with electrolyte disorders. This hydroelectrolyte loss can occur through the skin, urine, respiratory tract and digestive tract. This article addresses dehydration due to gastrointestinal loss. Objective: To review the current clinical criteria in the literature for the indication of different rehydration modalities in the pediatric age group. Methods: The literature review was conducted by searching the PubMed, Lilacs, SciELO, and UpToDate databases, involving works published preferably in the last five years, in addition to other relevant literature on the topic. Conclusion: There is a clear trend of increasing studies that encourage oral rehydration and the use of a nasogastric tube for this purpose, in those situations where intravenous hydration would be used. However, about dehydration due to gastrointestinal loss, further studies are needed to create more conclusive therapeutic protocols that encourage the use of oral rehydration via nasogastric tube in children with severe dehydration, whose clinical condition allows, thus reducing the length of hospitalization and possible complications.
