- Abstract
Objective: To generate reference values for spirometry in Brazilian children 3-12 years of age and to compare those values with the values employed in the equations currently in use in Brazil., Methods: This study involved healthy children, 3-12 years of age, recruited from 14 centers (primary data) and spirometry results from children with the same characteristics in six databases (secondary data). Reference equations by quantile regressions were generated after log transformation of the spirometric and anthropometric data. Skin color was classified as self-reported by the participants. To determine the suitability of the results obtained, they were compared with those predicted by the equations currently in use in Brazil., Results: We included 1,990 individuals from a total of 21 primary and secondary data sources. Of those, 1,059 (53%) were female. Equations for FEV1, FVC, the FEV1/FVC ratio, FEF between 25% and 75% of the FVC (FEF25-75%) and the FEF25-75%/FVC ratio were generated for white-, black-, and brown-skinned children. The logarithms for height and age, together with skin color, were the best predictors of FEV1 and FVC. The reference values obtained were significantly higher than those employed in the equations currently in use in Brazil, for predicted values, as well as for the lower limit of normality, particularly in children with self-reported black or brown skin., Conclusions: New spirometric equations were generated for Brazilian children 3-12 years of age, in the three skin-color categories defined. The equations currently in use in Brazil seem to underestimate the lung function of Brazilian children 3-12 years of age and should be replaced by the equations proposed in this study.

- Abstract
This study proposed the application of structural equation modeling (SEM) to investigate variables associated with preterm birth based on a theoretical model analyzed previously by hierarchical logistic regression. The data came from a population-based case-control observational study of hospital births to mothers residing in Londrina, Paraná State, Brazil (June 2006 to March 2007). For the SEM, the study considered the association between socioeconomic characteristics and psychosocial aspects pertaining to reproductive history, work and physical activity, complications during the pregnancy, and fetal characteristics. It also considered the relationship between these associations and the outcome preterm birth mediated by adequacy of prenatal care. The weighted least square mean and variance adjusted estimator (WLSMV) was used for categorical data and robust maximum likelihood (MLR) for odds ratios. Three latent variables were created: socioeconomic vulnerability, family vulnerability, and unwanted pregnancy. The effect of socioeconomic and family vulnerability and unwanted pregnancy on prematurity occurred indirectly through inadequacy of prenatal care. The proposed methodology allowed using constructs, verifying the role of mediation by inadequacy of prenatal care, and identifying the variables' direct and indirect effects on the outcome preterm birth.

- Abstract
Objective: To analyze the factors associated with recurrent falls in community-dwelling older adults from Cuiabá., Methods: This is an epidemiological, prospective, concurrent cohort study with a two-year follow-up. In-home surveys were conducted in 2012 and 2013. The data were treated by the software Epi Info and SPSS. Bivariate analysis investigated associations between risk factors and recurrent falls by calculating the relative risk (RR) of the cumulative incidences with a confidence interval of 95% (95%CI). Multiple analysis with Poisson regression included all variables with p < 0.20 in the crude analyses. The significance level of 5% (p < 0.05) was adopted as significant association for remaining in the final model., Results: Most older adults (77.6%) had recurrent falls. The variables significantly associated with recurrent falls were older adult's income of up to two minimum salaries (RR = 1.62; 95%CI 1.04-1.77), absence of arthritis or arthrosis (RR = 1.32; 95%CI 1.10-1.48), having regular to very bad self-perceived health (RR = 1.44; 95%CI 1.12-2.04), and having visual impairment (RR = 1.23; 95%CI 1.01 -1.69)., Conclusions: Falls in older adults are associated with low education levels, regular to very bad self-perceived health, visual impairment, and recurrent falls.

- Abstract
Introduction: Prevalence of low back pain (LBP) is expected to increase worldwide with aging of the population but its prevalence in older people is not clear, mainly in developing countries., Objective: To estimate the prevalence of LBP in older Brazilians., Methods: Electronic searches on SciELO, LILACS, MEDLINE, EMBASE and CINAHL, as well as hand-searching identified studies investigating prevalence of LBP in older Brazilians aged 60 years or over. Two independent reviewers selected studies fulfilling the inclusion criteria, assessed risk of bias for each included study and extracted relevant data. Meta-analysis was conducted when enough homogeneity allowed and the GRADE system was used to summarize the overall quality of the evidence., Results: Sixteen studies were included with a total of 28,448 participants. Data from point- and period-prevalence of LBP were obtained. Meta-analysis was conducted for 13 studies reporting point-prevalence. Pooled point-prevalence of LBP was 25.0% (95% CI 18.0-32.0). Other three studies investigated period-prevalence: one-week prevalence=15.0% (95% CI 13.0-18.0); six-month prevalence=43.0% (95% CI 42.0-44.0); and 12-month prevalence=13.0% (95% CI 11.0-16.0). Sensitivity analyses were performed for point-prevalence and exclusion of studies with poorer methodological quality tended to increase the estimated prevalence of LBP., Conclusion: Moderate-quality evidence showed that at any point in time one in four older Brazilians suffers from LBP. This was the first systematic review investigating nationwide data on the prevalence of LBP in older people and contributes important clinical and epidemiological evidence for policymakers., (Copyright © 2016 Elsevier Editora Ltda. - Abstract
The purpose of this study was to examine the prevalence of bad self-rated health in the elderly and associated demographic and socioeconomic factors. This was an epidemiological, cross-sectional study. Data were collected through interviews using the BOAS (Brazil Old Age Schedule) questionnaire. The study included 573 elderly residents in the urban area of Cuiabá-MT, Brazil. The adopted measure of association was the prevalence ratio. For multivariate analysis, multiple Poisson regression was used. The prevalence of poor/very poor self-rated health was 30.9% and the associated factors were income, number of morbidities and urinary incontinence. The association of demographic and socioeconomic factors with poor/very poor self-rated health among the elderly in this study shows the need for assistance and policies that minimize the impact of these factors on the lives and health of the elderly.

- Abstract
Objective: To evaluate the spirometry values predicted by the 2012 Global Lung Function Initiative (GLI) equations, which are recommended for international use, in comparison with those obtained for a sample of White adults used for the establishment of reference equations for spirometry in Brazil., Methods: The sample comprised 270 and 373 healthy males and females, respectively. The mean differences between the values found in this sample and the predicted values calculated from the GLI equations for FVC, FEV1, and VEF1/FVC, as well as their lower limits, were compared by paired t-test. The predicted values by each pair of equations were compared in various combinations of age and height., Results: For the males in our study sample, the values obtained for all of the variables studied were significantly higher than those predicted by the GLI equations (p < 0.01 for all). These differences become more evident in subjects who were shorter in stature and older. For the females in our study sample, only the lower limit of the FEV1/FVC ratio was significantly higher than that predicted by the GLI equation., Conclusions: The predicted values suggested by the GLI equations for White adults were significantly lower than those used as reference values for males in Brazil. For both genders, the lower limit of the FEV1/FVC ratio is significantly lower than that predicted by the GLI equations.

- Abstract
This article sought to conduct a critical analysis of the state of the art of the scientific production in counseling practices in nutrition for infants under two years of age, discussing the opportunities and obstacles in terms of strategic challenges for implementing healthy nutrition practices. It is a bibliographic study, with the core interest being counseling on infant nutrition. The majority (58.1 %) of the articles analyzed was published between 2008 and 2011. The educative actions of counseling on infant feeding were directed at health professionals and caregivers to children under two years of age. As opportunities in the practice of nutritional counseling the following stand out: an improvement in the performance of health professionals, the adoption of healthy eating practices by caregivers, and an improvement in the nutritional status of the children. As obstacles, the following stand out: a lack of training of caregivers and professionals regarding complementary healthy feeding and difficulties regarding the practice of counseling by a health care professional. This evidence reaffirms the importance of incorporating counseling guidance on infant feeding, as permanent practices in policies for child health care.

- Abstract
This study was performed with the objective to analyze the barriers to diagnosing tuberculosis in the aged and access to health services in the city of João Pessoa, Paraíba, Brazil. This qualitative study included the participation of seven aged women with tuberculosis. Interviews were used for data collection. The empirical material was organized using Atlas.ti 6.0, and analyzed according to the techniques of discourse analysis. The identified barriers related to the access to health services to confirm the diagnosis were: the operating hours of family health units; transferred responsibilities; home visits without controlling communicants; delay of the health service in suspecting the disease and the patient's repeated visits to the health center before being informed about the diagnosis. Despite the identification of common barriers that tuberculosis patients of all ages must deal with, because of the vulnerability of the elderly, health services should implement control actions so as to prevent the disease becoming a common condition in this population.

- Abstract
Bearing in mind that the Brazilian Unified Health System (SUS) is a social process in construction, and as health professionals are important individuals in this process, the role of permanent education as an important instrument to ensure humanized care is highlighted. The scope of this paper is to discuss the experience of the training course for health professionals of a public health outpatient unit, based on the prospect of humanized treatment, seeking the implementation of a sanitary model committed to the formal values contained in the SUS ideals. The teaching-learning methodology used is based on problem-solving, derived from the processing of a problem situation taken from the experience of teachers. The professionals identified that the standard established in the way of thinking and acting in health is unsatisfactory to meet the challenges faced in the sector. The strategies used contributed to systematize the content through reflection on the theoretical benchmarks submitted, by eliciting reflective and critical thought, which are fundamental aspects for broadening and increasing the process of empowerment of professionals. The course stimulated group action, placing the discussion on the humanization of health actions on the agenda.

- Abstract
The study aimed to identify, through an integrative literature review, scientific knowledge, produced between 2005 and 2009 about the mistreatment of the elderly. It was selected sixteen studies in the data banks LILACS, SciELO and BDENF. Of the sixteen studies, 56.25% comprised original articles. The journal that published the most was the Public Health Journal (25%), being the majority of the studies from the Southeast (56.25%). The survey revealed that the physical violence was prevalent among the elderly; the home was the environment where aggression occurs more frequently; and family members were the main aggressors. Among the elderly attacked, it was observed that women are the victims most affected. These findings raise the development of other researches that might clarify the multiple dimensions of violence against the elderly.

- Abstract
Objective: To evaluate the prevalence and nutritional and social determinants of overweight in a population of schoolchildren in Southern Brazil., Methods: Cross-sectional descriptive study of 5,037 children of both genders, between 6 and 10.9 years of age, from public and private schools of Maringá, Paraná, Brazil. Evaluation of factors associated with excess weight (overweight and obesity) included gender, age, school type, socioeconomic level, education of the head of the family, eating habits, and means of commuting to school. After univariate analysis (Fisher's exact test), we adjusted a logistic regression model and used Wald's test for decision-making (p < 0.05)., Results: The mean age was 8.7 ± 1.3 years, with 52.8% females; 79.1% of the students attended public school and 54.6% had families of socioeconomic class A or B. Regarding nutritional status, 24% of children were overweight (7% obesity, 17% overweight). Being male, attending a private school, and having a head of the family with over four years of education were significantly associated with excess weight. In relation to food, inadequate intake of carbohydrates was associated with a 48% greater chance of overweight/obesity (p < 0.001; OR: 1.48; 95% CI: 1.25-1.76)., Conclusion: The prevalence of overweight found in this study is approximate to that reported in national studies. Its association with gender and inadequate food intake indicates that these factors should be considered in initiatives aimed at preventive measures in childhood.

The article is a literature review on the emergence of human infections caused by Corynebacterium ulcerans in many countries including Brazil. Articles in Medline/PubMed and SciELO databases published between 1926 and 2011 were reviewed, as well as articles and reports of the Brazilian Ministry of Health. It is presented a fast, cost-effective and easy to perform screening test for the presumptive diagnosis of C. ulcerans and C. diphtheriae infections in most Brazilian public and private laboratories. C. ulcerans spread in many countries and recent isolation of this pathogen in Rio de Janeiro, southeastern Brazil, is a warning to clinicians, veterinarians, and microbiologists on the occurrence of zoonotic diphtheria and C. ulcerans dissemination in urban and rural areas of Brazil and/or Latin America.

- Abstract
Objective: To describe the clinical results and the degree of satisfaction of patients submitted to thoracic sympathectomy at the level of the fourth and fifth ribs (R4-R5) for the treatment of axillary hyperhidrosis., Methods: We included 118 patients diagnosed with axillary hyperhidrosis and having undergone axillary sympathectomy at the R4-R5 level between March of 2003 and December of 2007 at the Paraná Federal University Hospital de Clínicas, located in the city of Curitiba, Brazil. All procedures were carried out by the same surgeon. Data regarding the resolution of axillary hyperhidrosis and the degree of patient satisfaction with the surgical outcome, as well as compensatory hyperhidrosis in the early and late postoperative periods (after 7 days and after 12 months, respectively), were collected., Results: Of the 118 patients evaluated, 99 (83.9%) and 81 (68.6%) showed complete resolution of the symptoms in the early and late postoperative periods, respectively. Compensatory hyperhidrosis occurred in 49 patients (41.5%) in the early postoperative period and in 77 (65.2%) in the late postoperative period. Of those 77, 55 (71.4%) categorized the compensatory hyperhidrosis as mild. In the early postoperative period, 110 patients (93.2%) were satisfied with the surgical results, and 104 (88.1%) remained so in the late postoperative period., Conclusions: Sympathectomy at the R4-R5 level is efficient in the resolution of primary axillary hyperhidrosis. The degree of patient satisfaction with the long-term surgical results is high. Mild compensatory hyperhidrosis is the main side effect associated with this technique.

- Abstract
This study about the work of micro politics was carried out by the Buccal Health Team (ESB) in the Family Health Program (PSF) of Alagoinhas, Bahia State, and has as central theoretical purpose the specific and singular forms in the practice of daily work, using the technologies (hard, light-hard and light). The methodological trajectory is based on the historical-social current in view of a dialectic approach of qualitative nature. The techniques of data collection used were: semi structured interview, observation of the work process and documental analysis. The analysis of the data was oriented by the hermeneutics-dialectics, allowing to compare the different levels of analysis, articulating the theoretical with the empirical evidence. The results reveal that the Family Health Teams are multidisciplinary, but have still not developed an interdisciplinary work, hence occurring juxtaposition of skills. Each unit plans their work process according to the singularities of the social subjects, implementing different characteristics in how to welcome, inform, attend and refer. An effort in changing the work process can be perceived in the perspective of amplified clinic with the health community agent standing out as a social/collective subject.

- Abstract
Critical analysis of the Latin American bibliographical production over the last 10 years regarding the adherence/non-adherence to treatment of people with chronic health problems: leprosy, tuberculosis, hypertension, diabetes and AIDS. Thirty six articles were analyzed identifying the variables: year of publication, publication area and kind of study. Most of the articles (27) were produced by professionals of the medical area, in epidemiological studies, and of the nursing area (7) in qualitative and quanti-qualitative studies. The scientific production on the subject increased until 2002, when it began to drop. The authors repeatedly defended the idea that the role of the patient is to follow the recommendations of the health professional and that the patient is free to follow or not the treatment, the professional however being exempt from the responsibility for the consequences of this decision. The greater part of factors pointed out by the authors as contributing to non-adherence relates to the patient, showing that the major responsibility for the adherence / non-adherence to treatment is conferred upon him. The measures the authors indicate for solving the problem point to responsibility of professionals, health services, governments and teaching institutions.

- Abstract
Objective: To determine the diagnostic accuracy of positron emission tomography/computed tomography (PET/CT) using fluorine-18-deoxyglucose ([18F]-FDG) for the evaluation of a solitary pulmonary nodule (SPN)., Methods: Prospective analysis of 53 consecutive patients submitted to PET/CT between March 2005 and May 2007 for the evaluation of an SPN. Of those, 32 met the criteria for inclusion in the present study. The lesions were evaluated for location, size, radiotracer uptake and maximum standardized uptake value (SUV). The FDG-PET/TC results were correlated with other predictors of malignance (age, gender, smoking status, nodule size and nodule location). The definitive diagnosis was established through histopathology or through clinical/radiological follow-up for at least one year., Results: Fourteen malignant SPNs were found. Through analysis of the ROC curve, we established an SUV of 2.5 as the most appropriate cut-off point, since it correctly identified 13 of the 14 malignant SPNs. The results below that point revealed one false positive for neoplasia out of a total of 14. The semiquantitative method presented a sensitivity of 92.9%, specificity of 72.2%, positive predictive value of 72.2%, negative predictive value of 92.9% and accuracy of 81.2%. The multivariate analysis showed a statistically significant association with SPN malignancy only for nodule location in the upper lobes (p = 0.048) and SUV (p = 0.07)., Conclusions: The results obtained suggest that the SUV of [18F]-FDG is a useful predictor of neoplasia in SPN, with a high negative predictive value, which allows malignancy to be safely ruled out, showing its relevance in the diagnostic approach to pulmonary nodules.

- Abstract
Objective: To compare the most recent (2006) predicted values of forced vital capacity and forced expiratory volume in one second for spirometry in Brazilians with those obtained using other reference equations and to validate the findings through comparisons with a new sample of normal Brazilians., Methods: Forced spirometry was performed, in accordance with the Brazilian Thoracic Society guidelines, in 643 nonsmoking adult Caucasians. The predicted values obtained by Brazilian researchers in 1992 and those obtained by four groups of foreign researchers were compared with the new Brazilian predicted values obtained in 2006. In the second phase, the mean values obtained in 65 adult females and 79 adult males were compared with the predicted values obtained using the various reference equations., Results: A t-test for paired samples revealed significant differences between the predicted values obtained using the six equations and those obtained using the 2006 Brazilian equation. In the second phase, the mean predicted values obtained by Crapo et al., as well as those obtained by Hankinson et al. for Mexican-Americans, were similar to those found in the new sample. However, when the predicted values obtained in the new sample were compared with those obtained by those authors, discrepancies were found, with high and low predicted values. The mean values obtained using the 2006 Brazilian equation presented the smallest differences in comparison with the mean values obtained in the new sample., Conclusions: These results underscore the importance of using prediction equations for spirometry that are appropriate for our population.

- Abstract
The study discusses the conflicting situations that arise while receiving oral health teams in Alagoinhas, Bahia, Brazil. The main orientation for the Family Health Program is based on analyzing health care work flowcharts. The current qualitative research used semi-structured interviews and practical observation as the data collection techniques. There were 17 study subjects: group I (dentists and dental assistants - 6); group II (other health workers - 6); and group III (users - 5). Users' first contact with the family health team is in the reception, often in a tense and conflicting atmosphere, but with the potential for alternatives for change, as a privileged space for the use of low-key technologies. The therapeutic process varies: e.g. clinical consultation, emergency care, scheduled follow-up, and referral to other health services in the system. However, oral health teams conduct the reception process in different ways, depending on the practitioners' commitment and unique characteristics.

- Abstract
Diffuse pulmonary ossification is a rare condition of unknown pathogenesis in which mature bone is found in the pulmonary parenchyma. It is almost invariably discovered as an incidental finding at autopsy. Most commonly, it affects middle-aged men and is asymptomatic. We present the case of a 75-year-old man in which the chest X-ray showed diffuse interstitial infiltrate. Diagnosis was based on histopathological examination by open-lung biopsy, which revealed interstitial fibrosis with pulmonary ossification.

- Abstract
Susceptibility of horn fly populations to insecticides was evaluated at the Tabuleiros Costeiros region in the states of Alagoas, Bahia, and Sergipe from April 2001 to December 2002. A total of 37 bioassays were conducted in 23 ranches from 12 municipalities. Fly susceptibility was determined by the impregnated filter paper method using the active ingredients diazinon, permethrin, and cypermethrin. A high susceptibility to diazinon was found, with LC50 varying from 0.23 to 1.41 microg/cm2 and resistance ratios (RR) = 1.3. On the other hand, resistance to pyrethroids was detected in all studied populations. Cypermethrin LC50 ranged from 19.94 to 1,928 microg/cm2 and RR from 21.2 to 4,484.4. Although resistance levels to permethrin were much lower (RR<6), all LC50 were significantly higher than the LC50 obtained from the susceptible population. All ranches relied on pyrethroids for horn fly control, most cypermethrin (88.46%) and deltamethrin (80.77%) products. Spraying, by using manual backpack sprayers, was routinely used in all ranches. History of insecticide use as well as inadequate control practices help to explain the spread resistance of horn fly populations to pyrethroids detected in the region.

- Abstract
Unlabelled: Cisplatin is an antineoplastic drug for cancer treatment in children and adults. The side effects of cisplatin ototoxicity are significant: irreversible bilateral hearing damage to high frequencies (4 kHz - 8 kHz). Reports recognize some drugs that are associated with cisplatin to obtain an otoprotector effect. The ototoxicity mechanisms of cisplatin are related to injury of hair cell oxidation mechanism, especially of outer hair cells., Aim: Using otoacoustic emissions distortion products (DPOEA) and scanning electron microscopy we intended to verify the action of amifostine, a radioprotective drug that has well known antioxidant characteristics and otoprotector effects to cisplatin injury., Study Design: Experimental., Material and Methods: We used an experimental guinea pig model. The study was performed as follows: group 1: 6 animals, 12 ears, cisplatin 8.0 mg/Kg/day (IP), 3 days. Group 2: 6 animals, 12 ears, amifostine 100 mg/Kg/day (IP) and after 90 minutes, cisplatin 8.0 mg/Kg/day (IP), 3 days and group 3: 3 animals, 6 ears, amifostine 100 mg/Kg/day (IP), 3 days., Results: DPOEA were present before and after treatment in groups 2 and 3. The normal cilium architecture of outer hair cells was supported in all cochlear turns in groups 2 and 3. We concluded that amifostine has a potential otoprotector effect against cisplatin ototoxicity and could be used in clinical trials.

- Abstract
Aimed to evaluate the lovastatin action on Schistosoma mansoni oviposition, infected mice with 100 plus minus 10 cercariae of the LE strain were used. Thirty days after infection the animals were treated with 100, 200 and 400mg/kg of lovastatin, per os, during five consecutive days and then sacrificed 7, 15, 30 or 60 days after treatment. We analyzed: distribution of worms in mesenteries and liver; mortality of worms in the liver; alteration of the oogram; eggs counting in the jejunum and liver; presence of intrauterine eggs and morphology of the worms from the treated and control groups (infected and not treated animals). Significant statistical differences were found between treated and control groups when the presence of intrauterine eggs was considered and also, alteration on the oogram, eggs at different stages of development in jejunum, liver and in the body length of males and females. The morphological study of the worms showed that the degenerative modifications occurred, mainly in the reproductive system, with reduction and alteration of the viteline follicles and the ovary of the females. Also modifications in the males testicles were observed. The results suggest that the drug under study reduces, considerably, the female S. mansoni, oviposition increases the worms' size, leads to alteration in the reproductive system of males and females. It also may induce death of a significant part of the worm populations at the dose of 400mg/kg.

- Abstract
The authors studied a sample of students entering the first grade in the Osasco public school system in order to determine both the prevalence of anemia and nutritional status. Osasco is part of the Greater S o Paulo Metropolitan Area. Diagnosis of anemia was made through the hemoglobin concentration of blood from digital puncture. World Health Organization (WHO) levels were used to define anemia. Nutritional Status assessment. was made through weight/age and height/age indices, using Z score distribution and the National Center for Health Statistics (NCHS) reference levels. Prevalence of anemia was 51%. Prevalence levels varied according to the schools' geographic location: 56.9% in peripheral neighborhoods and 41.7% in central areas. Children with illiterate parents had a higher prevalence of this condition. Risk of anemia was higher for children who were over eight years of age when entering the first grade. Acute malnutrition was not found. Prevalence was higher than expected and points to the urgent need to establish an anemia control program for schoolchildren in this population.
