Na medida em que a população mundial está envelhecendo, a demência está se constituindo em importante problema de saúde pública, particularmente nos países em desenvolvimento. Investigações epidemiológicas nestes países são escassas e apresentam dificuldades metodológicas adicionais, principalmente no que se refere à adequação sociocultural dos instrumentos utilizados para a definição de casos. Tendo em vista estas preocupações, foi fundado o "Grupo de Pesquisa em Demência 10/66", que é constituído por uma rede internacional de pesquisadores, predominantemente de países em desenvolvimento. O nome do grupo tem como referência o paradoxo de que menos de 10% dos estudos populacionais sobre demência são dirigidos aos 2/3 ou mais de casos de pessoas com demência que vivem em países em desenvolvimento. O objetivo do artigo é atualizar informações da literatura sobre as diferenças de prevalência e incidência de demência encontradas em países desenvolvidos e em desenvolvimento.

The effects of organizational justice on workers' health have been investigated in several areas of work. However, the systematization of available information on the effects related to cardiovascular diseases (CVD) and diabetes is scarce. This article aims to systematically review the association between organizational justice and CVD and metabolic disease in adult workers. The search strategy included the terms organizational justice, coronary heart disease, cerebrovascular disease, systemic arterial hypertension, diabetes mellitus and CVD. This study investigated the following databases: MEDLINE, EMBASE, and LILACS. The quality of the studies was assessed using the instrument developed by the National Institute of Health. Results: This study identified 1,959 titles. After evaluation, eight studies were selected. Individuals with a high perception of organizational justice showed a lower risk of CVD and metabolic disease, whereas low organizational justice presented repercussions for the cardiovascular and metabolic health of workers. The development of strategies to promote organizational justice must be prioritized and thus mitigate its impacts on workers and institutions.

OBJECTIVE To present the development and validation of the WebAd-Q Questionnaire, a self-report instrument to monitor adherence to antiretroviral therapy in HIV/AIDS centers in Brazil. METHODS The WebAd-Q is an electronic questionnaire that has three questions about the use of antiretrovirals in the last week. It was constructed from interviews and focus groups with 38 patients. Its validity was tested in a study with a sample of 90 adult patients on antiretroviral therapy for at least three months. We used electronic monitoring bottles, pill counting, and self-report interview to compare adherence. The WebAd-Q was answered on the sixtieth day, twice, with at least one hour of interval. The viral load of the patients was obtained from the service records. We have analyzed the agreement between the answers to the WebAd-Q, the associations, and the correlations with viral load and performance compared to other measures of adherence. RESULTS Among the invited patients, 74 (82.2%) answered the WebAd-Q. No difficulties were reported to answer the questionnaire. The average answer time was 5 min 47 sec. The set of three questions of the WebAd-Q obtained agreement of 89.8%, with Kappa of 0.77 (95%CI 0.61-0.94). The non-adherence answers of the WebAd-Q were associated with detectable viral load. We obtained moderate viral load correlations with the non-adherence scale according to the WebAd-Q. For the three questions of the WebAd-Q, patients with non-adherence answers were also reported as less adherent according to the other measures of adherence. CONCLUSIONS The WebAd-Q answered all the issues considered relevant in the validation of questionnaires, was well understood by patients, was associated with viral load, and obtained good agreement and good performance compared to the other measures. The feasibility analysis of its implementation still depends on a national study on its applicability.

Objective: To evaluate the prevalence of health-related behaviors according to presence and type of depression in Brazilian adults., Methods: Based on a sample of 49,025 adults (18 to 59 years) from the National Survey on Health 2013 (PNS 2013), we estimated the prevalence of health-related behaviors (smoking; passive smoking; frequent or risky alcohol consumption; leisure time physical activity; time watching TV; and eating pattern indicators), according to the presence of depression (minor and major), evaluated by the Patient Health Questionnaire - 9 (PHQ-9), and the report of depressive mood (in up to seven days or more than seven days) over a two-week period. Prevalence ratios were estimated by Poisson regression., Results: Evaluated by the PHQ-9 scale, 9.7% of the Brazilian adults had depression and 3.9% presented major depression. About 21.0% reported depressive mood and, in 34.9% of them, that feeling has been present for more than seven days. In individuals with major depression (PHQ-9), higher prevalence was found in almost all unhealthy behaviors analyzed, in particular, smoking (PR = 1.65), passive smoking (PR = 1.55), risk alcohol consumption (PR = 1.72), TV for ≥ 5 hours/day (PR = 2.13), consumption of fat meat (PR = 1.43) and soft drink (PR = 1.42). The prevalence ratios tended to be lower in those with minor depression. Similar results were observed in adults with depressive mood., Conclusions: This study detected relevant association between depression and health behaviors, in particular for smoking and physical activity. The associations found with the PHQ were similar to those observed with the application of a single question about depressive mood. Our results indicate the importance of assessing the presence of depression and the frequency and severity of symptoms when implementing actions for the promotion of healthy behaviors.

OBJECTIVE To describe the prevalence of common mental disorders in Brazilian adolescent students, according to geographical macro-regions, school type, sex, and age. METHODS We evaluated 74,589 adolescents who participated in the Cardiovascular Risk Study in Adolescents (ERICA), a cross-sectional, national, school-based study conducted in 2013-2014 in cities with more than 100,000 inhabitants. A self-administered questionnaire and an electronic data collector were employed. The presence of common mental disorders was assessed using the General Health Questionnaire (GHQ-12). We estimated prevalence and 95% confidence intervals of common mental disorders by sex, age, and school type, in Brazil and in the macro-regions, considering the sample design. RESULTS The prevalence of common mental disorders was of 30.0% (95%CI 29.2-30.8), being higher among girls (38.4%; 95%CI 37.1-39.7) when compared to boys (21.6%; 95%CI 20.5-22.8), and among adolescents who were from 15 to 17 years old (33.6%; 95%CI 32.2-35.0) compared to those aged between 12 and 14 years (26.7%; 95%CI 25.8-27.6). The prevalence of common mental disorders increased with age for both sexes, always higher in girls (ranging from 28.1% at 12 years to 44.1% at 17 years) than in boys (ranging from 18.5% at 12 years to 27.7% at 17 years). We did not observe any significant difference by macro-region or school type. Stratified analyses showed higher prevalence of common mental disorders among girls aged from 15 to 17 years of private schools in the North region (53.1; 95%CI 46.8-59.4). CONCLUSIONS The high prevalence of common mental disorders among adolescents and the fact that the symptoms are often vague mean these disorders are not so easily identified by school administrators or even by health services. The results of this study can help the proposition of more specific prevention and control measures, focused on highest risk subgroups.

Objective: To describe the prevalence of self-reported previous medical diagnosis of depression in the adult (18 years or older), Brazilian population according to sociodemographic factors., Methods: Data from the 2013 National Health Survey, a population-based survey, were used. The self-reported previous medical diagnosis of depression, received at some point during the patient's life, was investigated. Prevalence and their respective confidence intervals of 95% (CI 95%) were calculated, stratified by gender, age group, education level, race/skin color, for Brazil, place of residence, major regions, federative units and capitals., Results: The prevalence of self-reported diagnosis of depression in adults in Brazili was of 7.6% (95%CI 7.2 - 8.1), being higher in women (10.9%; 95%CI 10.3 - 11.6) and among people between 60 and 64 years old (11.1%; 95%CI 9.1 - 13.1). Furthermore, the highest prevalence was among individuals with no formal education or those with incomplete primary education 8.6% (95%CI 7.9 - 9.3), for those with complete tertiary education 8.7% (95%CI 7.5 - 9,9); and for those who declared themselves as white (9.0%; 95%CI 8.3 - 9.6). For place of residence, the self-report was higher in individuals living in urban areas (8.0%; 95%CI 7.5 - 8.4) and in the South (12.6%; 95%CI 11.2 - 13.9)., Conclusion: The analysis shows the importance of understanding the access to the diagnosis of depression in Brazil. It is necessary to improve access to quality health services throughout the country to include the underprivileged population. Reducing disparities in access to health services is crucial to ensuring that social rights are universal and equal.

Social context can play a important role in the etiology and prevalence of mental disorders. The aim of the present study was to investigate risk factors for common mental disorders (CMD), considering different contextual levels: individual, household, and census tract. The study used a population-based sample of 2,366 respondents from the São Paulo Ageing & Health Study. Presence of CMD was identified by the SRQ-20. Sex, age, education, and occupation were individual characteristics associated with prevalence of CMD. Multilevel logistic regression models showed that part of the variance in prevalence of CMD was associated with the household level, showing associations between crowding, family income, and CMD, even after controlling for individual characteristics. These results suggest that characteristics of the environment where people live can influence their mental health status.

Introduction: The prevalence of violence in intimate relationships, suffered or perpetrated, was estimated among undergraduate students, be it physical, psychological and sexual, describing the overlap between them. This is an original study and there is scarce literature on this matter in Brazil. Men and women were investigated and some issues on gender violence were discussed., Methods: The study is part of the multicenter survey "International Study of Violence in Dating - IDVS" conducted in 2002 - 2003, using its standardized instrument. A total of 362 students, from two universities in the state of São Paulo, one public and one private, answered the self-reported questionnaire, being 37% male and 63% female, mean age of 20 years old. Suffered and perpetrated intimate partner violence during life was examined., Results: Among all interviewees, 75.9% suffered and 76.4% perpetrated some kind of violence throughout life. Psychological violence was the most prevalent type, followed by sexual violence, both suffered and perpetrated. The great superposition between suffered and perpetrated violence (83.9%) reflects the reciprocity of aggression, with no observed difference between men and women. The results of this study are consistent with previous literature on dating violence, which shows high prevalence of suffered and perpetrated violence, besides reciprocity for both men and women., Conclusions: Intervention actions at this stage of intimate relationships may potentially impact on subsequent marital partnership situations.

Background: Atrial fibrillation (AF) is a controversial risk factor for dementia., Objective: The objective of this study was to assess the association between AF and dementia in the "Sao Paulo Ageing & Health" (SPAH) study participants., Methods: SPAH is a cross-sectional, population-based study of elderly people living in a deprived neighborhood in Sao Paulo, Brazil. Dementia diagnosis was performed according to the 10/66 study group protocol based on Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria. Diagnosis of AF was made using a 12-lead electrocardiogram (ECG) recording, which was assessed by two cardiologists. Data on demographics and cardiovascular risk factors were also obtained., Results: Dementia was diagnosed in 66 (4.3%) and AF in 36 (2.4%) of 1,524 participants with a valid ECG. The crude odds ratio (OR) for dementia in participants with AF was 2.8 (95% confidence interval [CI]: 1.0-8.1; p=0.06) compared with individuals without AF. When analyzing data according to sex, a positive relationship was found in women (OR 4.2; 95% CI: 1.24-15.1; p=0.03). After age-adjustment, however, this association was no longer significant (OR 2.2; 95% CI: 0.6-8.9; p=0.26)., Conclusion: There was no independent association between AF and dementia in this sample. The prevalence of AF may be low in this population owing to premature cardiovascular death.

Objective: To investigate the influence of brain-derived neurotrophic factor (BDNF) gene variations on cognitive performance and clinical symptomatology in first-episode psychosis (FEP)., Methods: We performed BDNF val66met variant genotyping, cognitive testing (verbal fluency and digit spans) and assessments of symptom severity (as assessed with the PANSS) in a population-based sample of FEP patients (77 with schizophreniform psychosis and 53 with affective psychoses) and 191 neighboring healthy controls., Results: There was no difference in the proportion of Met allele carriers between FEP patients and controls, and no significant influence of BDNF genotype on cognitive test scores in either of the psychosis groups. A decreased severity of negative symptoms was found in FEP subjects that carried a Met allele, and this finding reached significance for the subgroup with affective psychoses (p < 0.01, ANOVA)., Conclusions: These results suggest that, in FEP, the BDNF gene Val66Met polymorphism does not exert a pervasive influence on cognitive functioning but may modulate the severity of negative symptoms.

Objective: To review the literature regarding the diagnosis of first episode psychosis in the context of emergency psychiatry., Method: Review of empirical and review articles selected by electronic search in the database PubMed., Results: Specific features of emergency care--single, brief and cross section assessment, and with little information--may jeopardize the diagnostic process. These limitations can be circumvented by application of operational diagnostic criteria, the use of scales and structured interviews and a short period of observation, between 24-72 hours. Diagnoses of bipolar disorder, schizophrenia, psychotic depression and delusional disorder developed in the context of emergency have good stability, but not the diagnoses of brief psychotic disorder, schizophreniform disorder and schizoaffective disorder. First episode psychosis can occur in the course of the use of psychoactive substances, with relatively frequent maintenance of psychotic symptoms even after cessation of the use of the substance. The rational use of subsidiary tests may help the differential diagnosis of psychotic episodes due to general medical conditions., Conclusion: Diagnoses of first psychotic episode can be adequately performed during psychiatric emergencies, if routines are implemented based on scientific evidence.

Background: The determination of the prevalence of electrocardiographic alterations in the older age strata of the Brazilian population represents important information with clinical and epidemiological purpose., Objective: To verify the prevalence rates of atrial fibrillation, enlarged Q/QS waves (Minnesota code 1.1-1.2) and left bundle branch block., Methods: In a population-based study, 1,524 participants (921 women and 603 men) aged > 65 years and living in Sao Paulo, Brazil, were submitted to electrocardiographic assessment at rest as well as anthropometric and blood pressure measurements, in addition to fasting blood collection for the measurement of glycemia, total cholesterol and fractions., Results: The age-adjusted prevalence for enlarged Q/QS waves was 12.1% (men, 17.2%; women, 9.6%), 2.4% for atrial fibrillation (men 3.9%; women, 2.0%); and 3.1% for left bundle branch block (men, 3.1%; women, 3.8%). For atrial fibrillation (both sexes), enlarged Q/QS waves (men) and left bundle branch block (women) there was an increase in frequency according to the age stratum. After adjusted for age, sex, diabetes mellitus and dyslipidemia, the odds ratio among the frequencies of enlarged Q/QS waves; arterial hypertension was 2.4 (95% CI: 1.4 -3.9) being 5.1 (95%CI: 1.8 -14.4) for women and 1.7 (95%CI: 0.95-3.1] for men., Conclusion: The comparison of these data with those from other studies showed a high prevalence of enlarged Q/QS waves in this population, with a direct association with the prevalence of arterial hypertension.

Objective: To estimate the socioeconomic and sociodemographic factors associated with the daily intake of five servings of fruit and vegetables by elderly individuals living in low income areas, identifying the main fruits and vegetables which compose the diet of this population., Methods: This is a cross-sectional population-based study with 2,066 low income elderly individuals (> or =60 years) living in the city of São Paulo, Southeastern Brazil, in 2003-2005. To assess the fruit and vegetable intake a Food Frequency Questionnaire was administered. The answers were transformed into daily intake and compared with the recommendations of the World Health Organization (five or more servings per day). The relationship between recommended fruit and vegetable intake and socioeconomic variables was analyzed using logistic regression models., Results: Of the participants, 60.5% were women and 39.5% were men. Approximately one third of the elders (n=723; 35.0%) did not consume any kind of fruit or vegetable on a daily basis and 19.8% reported a daily intake of five or more servings of fruits and vegetables. This intake was positively associated with income and years of schooling., Conclusions: The fruit and vegetable intake of low income elderly individuals in the city of São Paulo was insufficient according to the recommendations of the World Health Organization and is associated with unfavorable socioeconomic conditions.

Objective: To empirically compare the Cox, log-binomial, Poisson and logistic regressions to obtain estimates of prevalence ratios (PR) in cross-sectional studies., Methods: Data from a population-based cross-sectional epidemiological study (n = 2072) on elderly people in Sao Paulo (Southeastern Brazil), conducted between May 2003 and April 2005, were used. Diagnoses of dementia, possible cases of common mental disorders and self-rated poor health were chosen as outcomes with low, intermediate and high prevalence, respectively. Confounding variables with two or more categories or continuous values were used. Reference values for point and interval estimates of prevalence ratio (PR) were obtained by means of the Mantel-Haenszel stratification method. Adjusted PR estimates were calculated using Cox and Poisson regressions with robust variance, and using log-binomial regression. Crude and adjusted odds ratios (ORs) were obtained using logistic regression., Results: The point and interval estimates obtained using Cox and Poisson regressions were very similar to those obtained using Mantel-Haenszel stratification, independent of the outcome prevalence and the covariates in the model. The log-binomial model presented convergence difficulties when the outcome had high prevalence and there was a continuous covariate in the model. Logistic regression produced point and interval estimates that were higher than those obtained using the other methods, particularly when for outcomes with high initial prevalence. If interpreted as PR estimates, the ORs would overestimate the associations for outcomes with low, intermediate and high prevalence by 13%, almost by 100% and fourfold, respectively., Conclusions: In analyses of data from cross-sectional studies, the Cox and Poisson models with robust variance are better alternatives than logistic regression is. The log-binomial regression model produces unbiased PR estimates, but may present convergence difficulties when the outcome is very prevalent and the confounding variable is continuous.

Objective: To estimate the prevalence of the burnout syndrome and of common mental disorders among community-based health agents, also identifying associated factors., Methods: A cross-sectional survey was carried out in 2006 with 141 community-based health agents who had been working for at least six months in primary care units in the city of São Paulo (Southeastern Brazil). The participants answered three questionnaires: one about sociodemographic, health- and work-related characteristics, the Self Reporting Questionnaire, and the Maslach Burnout Inventory, which allows the assessment of three dimensions of the burnout syndrome: emotional exhaustion, depersonalization and reduced personal accomplishment. Multivariate logistic regression was used to examine associations between variables., Results: Overall, 24.1% of the interviewees presented burnout syndrome. Moderate or high levels of emotional exhaustion, depersonalization and reduced personal accomplishment were observed in 70.9%, 34.0% and 47.5% of the participants, respectively. The prevalence of common mental disorders was 43.3%. Positive correlations between the three dimensions of burnout, were observed. Presence of common mental disorder was independently associated with higher levels of emotional exhaustion and reduced personal accomplishment., Conclusions: The high frequency of intense levels of burnout and the high prevalence of common mental disorders among community-based health agents point to the need of intervention strategies in these individuals' daily lives and of further studies to better understand the actual picture and the determinants of burnout.

Objective: To evaluate the influence of socioeconomic conditions on the association between common mental disorders and the use of health services and psychoactive drugs., Methods: This was a population-based cross-sectional study conducted in the city of Botucatu, Southeastern Brazil. The sample was probabilistic, stratified and cluster-based. Interviews with 1,023 subjects aged 15 years or over were held in their homes between 2001 and 2002. Common mental disorders were evaluated using the Self-Reporting Questionnaire (SRQ-20). The use of services was investigated in relation to the fortnight preceding the interview and the use of psychotropic drugs, over the preceding three days. Logistic regression was used for multivariable analysis, and the design effect was taken into consideration., Results: Out of the whole sample, 13.4% (95% CI: 10.7;16.0) had sought health services over the fortnight preceding the interview. Seeking health services was associated with female gender (OR=2.0) and the presence of common mental disorders (OR=2.2). 13.3% of the sample (95% CI: 9.2;17.5) said they had used at least one psychotropic drug, especially antidepressives (5.0%) and benzodiazepines (3.1%). In the multivariable analysis, female gender and the presence of common mental disorders remained associated with the use of benzodiazepines. Per capita income presented a direct and independent association with the use of psychoactive drugs: the greater the income, the greater the use of these drugs was., Conclusions: Lower income was associated with the presence of common mental disorders, but not with the use of psychotropic drugs. The association of common mental disorders and the use of psychotropic drugs in relation to higher income strengthens the hypothesis that inequality of access to medical services exists among this population.

Objective: To study factors related to preference for cesarean delivery, among pregnant women without medical complications., Methods: A cross-sectional study was carried out among 156 pregnant women, in a private clinic in the city of Osasco, State of São Paulo, from October 2000 to December 2001. The pregnant women were at 28 weeks of pregnancy or more, with no formal contraindication for vaginal delivery at the time of the interview. Sociodemographic data and past and present obstetric history were assessed by applying a questionnaire. The pregnant women were specifically asked what their current preference for delivery was. Pearson's Chi-square test and logistic regression for multivariate analysis were performed with a 5% significance level., Results: Sixty-seven pregnant women (42.9%) said they had little motivation to undergo vaginal delivery. In the multivariate analysis, the following variables were statistically significant: previous vaginal birth (p=0.001; ORadj=0.04; 95% CI=0.01-0.12); husband's monthly income greater than 750 reais (p=0.006, ORadj=3.44; 95% CI=1.38-8.33). The women with a previous vaginal delivery presented 25-fold lower chance of choosing cesarean delivery. The opinion that the previous delivery experience was unsatisfactory was marginally associated with the main outcome (p=0.06; ORadj=0.42; 95% CI=0.16-1.05)., Conclusions: Motivation for cesarean delivery is associated with influences such as the type and degree of satisfaction with previous delivery and income.

Objective: There is an increasingly number of demented elderly in Brazil but empirical data on the resulting impact on their informal caregivers is still poorly documented. The purpose of this study was to evaluate the impact on informal caregivers of elderly with dementia., Methods: A cross-sectional study was conducted at a psychogeriatric unit in the city of Sao Paulo, Brazil. Structured and semi-structured interviews were administered to 49 elderly patients with dementia and their caregivers. The impact on the caregiver was measured using the Zarit Burden Interview., Results: Most patients were females, had an average score of 12.2 in the Mini Mental State Examination and high incidence of disruptive behavior. Caregivers were predominantly females, generally wives or daughters who lived with their patients. The mean impact score was 32.4 (SD 16.7). Variables statistically associated with the level of impact were: degree of kinship between patient and caregiver (p=0.011); caregiver's psychiatric symptoms (p<0.001), and duration of caregiving role (p=0.001)., Conclusions: A high impact on caregivers of demented elderly who required psychogeriatric intervention was detected. Healthcare providers should be able to provide the needed support to caregivers in order to minimize problems to both patients and their caregivers.

As the world population is ageing, dementia becomes an important public health problem, particularly in developing countries. Epidemiological research in these settings is scarce and present additional methodological difficulties, mainly regarding the socio-cultural adequacy of instruments used to identify cases of dementia. As a result of these concerns the 10/66 Dementia Research Group was founded to fill this gap. This is an international network of investigators, mostly from developing countries, and the group's name was based on the paradox that less than 10% of the population-based studies on dementia are directed to 2/3 or more cases of people with dementia living in developing countries. The aim of the paper is to update data in the literature regarding the differences in dementia prevalence and incidence seen in developed and developing countries.

This study provides the cancer mortality pattern for a cohort of 10,017 electric power plant workers from 1978 to 1994. Standard mortality ratio (SMR) analysis showed lower mortality from all causes (0.47: 0.43-0.51) and from all groups of causes including neoplasms (0.73: 0.60-0.90) among these workers, in comparison with the same sex and age bracket in Rio de Janeiro. In relation to specific cancer sites, standard cancer mortality ratios (SCMRs) greater than one were observed for neoplasms of the small bowel (10.35: 3.34-32.09), gallbladder (2.64: 0.99-7.03), pancreas (1.83: 0.91-3.66), kidney (2.91: 1.39-6.10), and bladder (2.60: 0.58-4.12), as well as for melanoma (1.82: 0.46-7.28). Important limitations were the workers' relatively young age and the short period of time since job admission (less than 15 years). The small number of deaths for each anatomic site led to instability in the study results, with many non-significant SCMRs and wide confidence intervals. Due to these limitations, study results are difficult to interpret in light of currently available evidence.
