Abstract

RESUMO Em 2015, o Brasil enfrentou uma epidemia de infecção pelo vírus Zika que se propagou por países do mundo. Posteriormente, recomendações acerca dos critérios de notificação de casos de síndrome congênita do Zika (SCZ) foram divulgadas através de protocolos. As mudanças frequentes nessas recomendações podem ter afetado o gerenciamento clínico e o acesso a suporte pós-diagnóstico por crianças afetadas mas não identificadas. No presente estudo, 39 casos de SCZ notificados no estado do Espírito Santo no período de 2015 a 2016 foram reclassificados quanto ao seu diagnóstico de acordo com o protocolo atualmente vigente, diferente daquele que vigorava em 2015. Pela reclassificação, apenas oito dos 36 casos seriam confirmados, respeitando o critério de dois ou mais sinais ou sintomas da SCZ com ou sem microcefalia e confirmação sorológica. Ainda, pela diminuição no perímetro cefálico que define microcefalia, 17 casos passaram a não se enquadrar nessa condição. Apesar de o protocolo vigente não utilizar somente o perímetro cefálico como critério para notificação e confirmação da SCZ, cabe ressaltar que este achado ainda é o maior sinalizador para as equipes de saúde, indicando um risco da não detecção precoce da SCZ. Seria prudente uma revisão dos casos “descartados” no momento de transição entre protocolos, a fim de avaliar se foram corretamente classificados.

Abstract

Health care workers (HCW) are at increased risk of latent tuberculosis infection (LTBI) from occupational exposure to Mycobacterium tuberculosis. The objective was to determine the prevalence of and risk factors for LTBI among primary HCW in five Brazilian cities. We conducted a cross-sectional study, from 2011 to 2013, among primary HCW, using a structured questionnaire and an evaluated for LTBI using the Quantiferon-TB Gold in-tube test. The magnitude of the associations was assessed using hierarchical logistic regression models. Among 708 HCW, the LTBI prevalence was 27% (n = 196; 95%CI: 24%-31%). We found that the following factors were positively associated with LTBI in primary HCW: age > 50 years (OR = 2.94; 95%CI: 1.44-5.99), absence of a BCG scar (OR = 2.10; 95%CI: 1.28-3.43), self-reported ex-smoker status (OR = 1.80; 95%CI: 1.04-3.11), being a nurse (OR = 2.97; 95%CI: 1.13-7.83), being a nurse technician (OR = 3.10; 95%CI: 1.26-7.60), being a community health agent (OR = 2.60; 95%CI: 1.06-6.40), and irregular use of N95 masks (OR = 2.51; 95%CI: 1.11-5.98). In contrast, HCWs who do not work in health care facilities with a TB control program were less likely to have LTBI (OR = 0.66; 95%CI: 0.45-0.97). This study demonstrated a substantial occupational risk of LTBI among primary HCW in Brazil. The Brazilian TB control program, as well as local programs, need to target these high-risk HCW with education, as well as with better personal protective equipment to prevent acquisition of new TB infection.

The No Child Left Behind Act of 2001 (NCLB) builds on a tradition of gradually increasing federal involvement in the nation's public school systems. NCLB both resembles and differs from earlier federal education laws. Over the past five decades, conservatives in Congress softened their objections to the principle of federal aid to schools and liberals downplayed fears about the unintended consequences of increased federal involvement. The belief in limited federal involvement in education has been replaced by the presumption by many legislators that past federal investments justify imposing high stakes accountability requirements on schools.

6. Evaluation of Functional Cognition in People Experiencing Homelessness: A Scoping Review

Abstract

The identified prevalence of cognitive impairment in people experiencing homelessness has placed a focus on the use of measures of cognition to predict functioning and ability to move successfully into housing. Functional cognition is the cognitive ability to perform daily life tasks, incorporating cognitive functioning, performance skills, and patterns. Given the increased research on cognitive status and functional skills within people experiencing homelessness, a scoping review was conducted to synthesize the existing literature and identify how functional cognition is evaluated in this population, and identify implications for homeless and housing services. This scoping review was conducted using the Joanna Briggs Institute protocol. Included studies assessed functional cognition, or assessed a component of cognition and related it to functional performance. Of 8,848 articles retrieved from the initial search, thirty-seven articles were determined to meet the inclusion criteria for data extraction. Categories for data extraction included functional domain addressed, tools used for assessment of cognition and/or function, and how reported outcomes were interpreted. Narrative analysis identified domains of cognition and function assessed, and relationships between cognition and function domains. Only the functional domain of Activities of Daily Living/Instrumental Activities of Daily Living (ADL/IADL) were found to have a consistent relationship between cognition and performance and primarily when performance-based assessments were used. Challenges with generalizing study findings broadly include: study populations do not reflect demographics of those experiencing homelessness; a variety of assessment tools for cognition and function are utilized; and the broad use of the term function encompasses several different areas of independent living in the community. Several studies identified that even with the presence of decreased cognition, those with the experience of homelessness are able to maintain housing when given individualized and appropriate supports. This scoping review indicates inconsistent methods to assess cognition and functional cognition within people experiencing homelessness. Performance-based assessments of ADL/IADL are more likely to identify the impact of cognition on function, while neurocognitive assessments are more beneficial to assess social/community integration. Findings indicate that to support the transition from homelessness to housing and increase the likelihood of housing stability; assessments should focus on identifying strengths and necessary/individualized ongoing supports.

Abstract

The purpose of the present investigation was to identify the effects of a 130-km cycling race on indices of biochemical indirect markers of muscle damage and muscle soreness responses during a 72-hour recovery period. Fifteen endurance-trained male cyclists which were competing for more than 2 years and were involved in systematic training at least of 3 days/wk underwent a collection of indirect biochemical markers of muscle damage (CK, LDH, Myo) and delayed onset of muscle soreness (DOMS), at five different moments of data collection: before (PRE) and immediately after (POST) a 130-km cycling race, and 24, 48, 72 hours following the cycling race. CK and LDH plasma concentrations significantly increased POST-race (p < 0.001) and remained high throughout the 72 hour recover period (CK: p < 0.05; LDH: p < 0.001). Myo increased significantly POST-race (p < 0.001) and returned to the PRE-race values 24 hours thereafter (p < 0.05). DOMS increased significantly POST-race (p < 0.001) and returned to the PRE-race values at 48 hours after (p > 0.05). A 130-km cycling race has a noteworty effect on indices of biochemical indirect markers of muscle damage and muscle soreness responses, indicating that 72 hour recovery period do not seems to be enough for long-distance cyclist, and reinforce the propositions of scientific literature about the need of a sufficient recovery period for cycling endurance athletes.

Abstract

This is an examination of the carotid choke, also known as the “sleeper hold.” We will be concerned with types of sleeper holds, what part of the neck is affected, and how. This is commonly referred to as a “blood choke.” There is differentiation between blood chokes and air chokes (strangulation). This article will cover application, effects, result over short and long terms, and ramifications of drug usage and mental state. We will not include the effects of a choke on the trachea, or pressure or strikes to the back of the neck. This article is presented from two perspectives; that of a martial arts practitioner/instructor and that of a physician.

Abstract

The efficacy of a well-operated day care center in providing better nutrition to poor children in Brazil was assessed. We collected data for 9 months from 180 under-five children attending a day care center located in a slum. Every working day each child received at least 100% of the age-group recommended dietary allowances plus iron fortified milk and, every six months, treatment for helminth infections. Statistical analyses were restricted to the 168 children (93%) who had measurements from at least five months. As outcome variables, weight-for-height, height-for-age, and weight-for-age Z scores were dichotomized so that a Z score < -1 indicated a child was at risk of malnutrition. We examined the proportion of children at nutritional risk in each month and used multivariate statistical techniques to adjust for confounding and to account for intra-subject correlation. The percentage of children at nutritional risk decreased over time, from 10.1% to 3.4% for weight for height, 29.8% to 15.2 % for weight for age, and from 50.0% to 44.8% for height for age. Most of the reduction took place between the third and fourth months of enrollment. The correlated data models for the three dichotomized outcomes showed a two to three-fold reduction in the occurrence of wasting and underweight. We concluded that attendance to well-operated daycare protects against nutritional risk, and approximately four months are required for a benefit to be seen.Avaliou-se a eficácia da freqüência a creche padrão em promover recuperação nutricional de crianças pertencentes a famílias de baixa renda. Foram coletados dados, por um período de nove meses, de 180 crianças que frequentavam uma creche localizada em uma favela. Em todos os dias úteis cada criança recebia no mínimo 100% das recomendações dietéticas diárias, acrescidas de leite fortificado com ferro e, semestralmente, tratamento para parasitoses intestinais. As análises estatísticas se limitaram a 168 crianças (93%) para as quais se dispunham de medidas de pelo menos cinco meses. Como variáveis resposta, os escores Z das relações peso para estatura, estatura para idade e peso para idade foram dicotomizados de forma que escore Z < - 1 indicava risco nutricional. Foram estudadas as proporções de crianças em risco nutricional a cada mês e utilizadas técnicas estatística multivariadas para controlar eventuais fatores de confundimento e correlacão entre as medidas subsequentes da mesma criança. O percentual de crianças em risco nutricional diminuiu no decorrer do acompanhamento, de10.1% para 3.4% na relação peso estatura, de 29.8% para 15.2 % na peso idade, e de 50.0% para 44.8% na estatura idade. A maior parte das reduções ocorreram entre os terceiro e quarto meses de frequência. Os modelos de dados correlacionados para as três variáveis resposta dicotomizadas evidenciaram reduções de duas a três vezes nos riscos nutricionais descritos pelos indicadores de peso-estatura (wasting) e peso-idade (underweight). Concluimos que a creche padrão diminui riscos nutricionais, sendo necessários aproximadamente quatro meses para que os benefícios sejam identificados.

Se denomina habitualmente “estrangulación sanguínea”. Existe una diferenciación entre las estrangulaciones sanguíneas y las estrangulaciones respiratorias. Este artículo tratará la aplicación, efectos, resultados a corto y largo plazo, y ramificaciones respecto a la utilización de drogas y al estado mental. No incluiremos los efectos de una estrangulación sobre la tráquea, o de la presión o los golpes a la parte posterior del cuello. Abstract

Solid pseudopapillary neoplasm (SPN) of the pancreas is a rare neoplasm. Diagnosis of SPN requires an integrated approach with aid of radiology, biopsy, cytology, and immunohistochemical stains. Although morphological features in combination with nuclear positivity of β-catenin IHC have been the gold standard of SPN diagnosis, but overlapping morphology and immunohistochemical findings with other entities in differential diagnoses such as pancreatic neuroendocrine tumors and pancreatic ductal adenocarcinoma make the diagnosis of SPN difficult particularly in limited cytology specimens. Lymphoid enhancer-binding factor 1 (LEF1), a key player in the Wnt signaling pathway, has shown promising diagnostic utility in SPN in recent literatures., Methods: In this retrospective study, we evaluated the diagnostic utility of LEF1 IHC in SPN in cytology specimens. LEF1 IHC was performed and compared with β-catenin, synaptophysin, and chromogranin immunostains in 13 SPN and 23 pancreatic neuroendocrine tumors (PanNETs) cytology cases with retrievable cell blocks., Results: LEF1 was positive in 13 of 13 (100%) SPNs and was negative in all PanNETs (0%)., Conclusion: LEF1 shows 100% sensitivity and specificity in cytology specimens for SPN and can be valuable immuno-stain in the diagnosis of SPN in cytology cell blocks., (© 2021 S. 36. [Variations in the detection of congenital Zika syndrome associated with changes in protocolsVariación de la detección del síndrome congénito por el virus del Zika en función de los cambios de los protocolos].

Abstract

In 2015, Brazil faced a Zika virus epidemic that spread to other countries in the world. As a result, recommendations regarding reporting criteria for congenital Zika syndrome (CZS) were issued in the form of protocols. The frequent changes in these recommendations may have affected clinical management and the access to post-diagnostic support by children who were affected by CZS, but who ended up not being identified. In the present study, 39 cases of CZS reported in the state of Espírito Santo, Brazil, from 2015 to 2016 were re-classified in terms of diagnosis using the current protocol, which is different from the protocol used in 2015. According to this re-classification, only eight out of 36 cases would be confirmed, based on the criterion of two or more signs or symptoms of CZS with or without microcephaly plus positive serologic results. Also, considering the decrease in the head circumference cut-off point defining microcephaly, 17 cases would no longer meet the definition for this condition. Even though the current protocol does not rely on head circumference alone for CZS reporting and confirmation, it should be noted that this is still the main sign considered by health care teams, and therefore the decrease in the cut-off point might have compromised early CZS detection. A review of "ruled out" cases would be advisable in moments of protocol transition to determine whether these cases have been correctly classified., Competing Interests: Conflito de interesses.
