2. Effect of kinesio tape application on the lymphatic system during knee arthroscopy surgery

Introduction: To control edema, physical therapy employs several techniques, such as elastic bandages application - Kinesio tape (KT) - to block or drain subcutaneous body fluids, due to the secondary effects of its elastic properties. Objective: To evaluate the effect kinesio tape application on the lymphatic system during knee arthroscopy surgery. Methods: Controlled clinical trial, with 28 patients, alternately divided into two groups (intervention and control) referred to arthroscopic surgical treatment of anterior cruciate ligament and meniscus injuries. Patients were evaluated in the preoperative and, on the 1st postoperative day, while the intervention group received KT application for the lymphatic system in the intraoperative period. Results: The intervention group showed statistically significant results in the non-formation of edema, according to perimetric (Point 2: p=0.010, Point 3: p≤0.001 and Point 4: p≤0.001) and ultrasound (p=0.007) analyses when compared to the control group. On the other hand, pain (p=0.056) did not present a significant difference, but in the intragroup comparison pre and postoperative, a considerable reduction (p=0.002) was observed. Conclusion: KT application for the lymphatic system in the intraoperative period of knee arthroscopy effectively minimized edema formation and reduced pain.

The article presents the consequences of the proposal of the Church "going forth", in the light of Balthasar's theological concept of ecclesial kenosis, enabling it to be more missionary and closer to people's lives, with the irreversible continuation of the Second Vatican Council. The aim of the article is to make this study a valuable contribution to the journey of the "outgoing" Church, which is moving towards today's demands, always looking at the signs of the times. It considers the concept of primeirear, necessary for this process, of going "out", as well as the act of leaning in to listen and serve better. Christian experience shows that the Church's mission is best carried out in "outgoing" communities, ready and open to dialog and service in the world. However, pastoral conversion is necessary. The transformation of structures implies continuous processes, so the theology and pastoral care of the Church "going forth" must be done on its knees, emphasizing its kenotic and synodal character. Hip dysplasia (CFD) is the most common orthopedic case in small animal clinics, and, from this, other conditions can arise, such as lateral dislocation of the patella. The objective of this work is to present the preventive surgical treatment for DCF, using the technique of juvenile pubic symphysiodesis (SPJ), and also the surgical treatment to correct lateral patellar dislocation using the trochlear chondroplasty technique in a 4-month-old canine patient., who had hip instability and a history of lameness in the right pelvic limb. SPJ constitutes a prophylactic treatment for FCD that consists of the induced suspension of bone growth in the pubic symphysis region. Trochlear chondroplasty was the technique for deepening the trochlear groove which, together with intervention in the soft tissues, had the function of fixing the patella in its anatomical position. It is concluded that the use of the SPJ technique had the main function of preventing CFD in a young animal, just as trochlear chondroplasty had the reestablishment of normal function of the affected limb. ABSTRACT Objective: To evaluate the efficacy and safety of sliding osteotomy of the lateral epicondyle in correcting rigid valgus deformity in knee arthroplasty. Methods: A retrospective study of patients undergoing total knee arthroplasty with lateral epicondyle sliding osteotomy between 2006 and 2018. The main outcome was the incidence of complications and adverse events. Secondary outcomes were Visual Analog Scale for Pain, varus stress test, and varus knee thrust during gait. Results: 19 knees (19 participants) were included in the study. The mean follow-up was 4.2 years. There were no cases of infection or reoperation due to instability. Two participants (10.5%) had mild or moderate knee pain (VAS pain = 4.6 ± 1.9). Two arthroplasties (10.5%) had mild varus stress. No participant presented varus thrust. Conclusion: Sliding osteotomy of the lateral epicondyle allows fast and safe ligament balance of knee valgus deformities. Level of Evidence I, Case series.

9. Anatomy of the Medial Meniscotibial Ligament of the Knee: A Systematic Review

Abstract Objectives To perform a systematic review of the literature on the anatomy of the medial meniscotibial ligaments (MTLs), and to present the most accepted findings, as well as the evolution of the anatomical knowledge on this structure. Materials and Methods An electronic search was conducted in the MEDLINE/PubMed, Google Scholar, EMBASE and Cochrane library databases with no date restrictions. The following index terms were used in the search: anatomy AND meniscotibial AND ligament AND medial. The review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. We included anatomical studies of the knee were included, such as cadaver dissections, histological and/or biological investigations, and/or imaging of the medial MTL anatomy. Results Eight articles that met the inclusion criteria were selected. The first article was published in 1984 and the last, in 2020. The total sample in the 8 articles was of 96 patients. Most studies are purely descriptive in terms of the macroscopic morphological and microscopic histological findings. Two studies evaluated the biomechanical aspects of the MTL, and one, the anatomical correlation with the magnetic resonance imaging examination. Conclusion The main function of the medial MTL, a ligament that originates in the tibia and is inserted in the lower meniscus, is to stabilize and maintain the meniscus in its position on the tibial plateau. However, there is a limited amount of information regarding medial MTLs, primarily in terms of anatomy, especially vascularization and innervation.

10. Simultaneous Bilateral Total Knee Arthroplasty May be Safe

Abstract Objectives This study aims to analyze the safety of a simultaneous bilateral total knee arthroplasty (SBTKA) and to investigate patients' satisfaction with the procedure performed concomitantly. Methods In a prospective study, we evaluated 45 patients undergoing SBTKA performed by two surgical teams. The mean age of the patients was 66.9 years; 33 subjects were female (73.3%) and 12 (26.6%) were male. We followed a protocol of intra- and postoperatively measures to ensure the safety of this procedure. We evaluated the surgery time and blood loss by hematocrit (Ht) and hemoglobin (Hb) levels on the first postoperative day, the percentage of patients who received a transfusion of packed red blood cells, and the number of required units. We also recorded perioperative complications, and, at the end of 3 months, we asked patients about their preference between the simultaneous or staged procedure. Results The mean surgery time was 169 minutes. In the postoperative period, there was an average decrease of 28.2% in Htc and 27.0% in Hgb. A total of 16 patients (35.5%) received a transfusion of packed red blood cells (mean, 1.75 units per patient requiring a transfusion). There were 12 minor complications (26.6%) and 2 major complications (4.4%); furthermore, no patient had a clinical diagnosis of deep vein thrombosis, and there were no deaths. Conclusions The SBTKA procedure may be considered safe if performed in selected patients and with a care protocol to prevent complications. This type of procedure was unanimously approved by patients.

11. Assessment of the Risk Factors Related to the Length of Hospital Stay and Postoperative Complications in Patients Undergoing Primary Total Knee Arthroplasty

Abstract Objective To assess the risk factors involving longer hospital stays and early postoperative complications (first 30 days after surgery) in patients undergoing total knee arthroplasty (TKA). Materials and Methods Across-sectional study was conducted with collection of data of patients who underwent TKA in a private hospital between 2015 and 2019. The following data were collected: age, gender, body mass index, and clinical comorbid-ities. We also collected intraoperative data such as the grade on the classification of the American Society of Anesthesiologists (ASA), the duration of the surgery, the length of stay, the postoperative complications, and readmission within 30 days. Statistical models were used to investigate the possible risk factors associated with longer hospital stays and postoperative complications. Results There was evidence of an increase in the length of hospital stay in older patients, with higher grades on the ASA classification or who suffered postoperative complications. For each increase in 1 year of age, we expect the length of stay to be multiplied by 1.008 (95% confidence interval [95%CI]: 1.004 to 1.012; p < 0,001). In patients who were ASA grade III, the time is expected to be multiplied by 1.297 (95%CI: 1.083 to 1.554; p = 0,005) when compared with grade-I patients. In patients who suffered postoperative complications, the time is expected to be multiplied by 1.505 (95%CI: 1.332 to 1.700; p < 0.001) compared with patients without complications. Conclusion The present study demonstrated that, in patients who underwent primary TKA, preoperative characteristics such as older age and ASA grade > III, as well as the development of postoperative complications, independently predict the increase in the length of hospital stay.

12. Microbiological Profi le of Periprosthetic Knee Infections in a Brazilian Unified Health System Hospital Specialized in Highly Complex Orthopedic Surgeries

Abstract Objective We studied the microbiological profile of periprosthetic knee infections treated in a Brazilian tertiary hospital. Methods The study included all patients undergoing revision surgery for total knee arthroplasty (RTKA) between November 2019 and December 2021, with a diagnosis of periprosthetic infection confirmed per the 2018 International Consensus Meeting (ICM) criteria. Results Sixty-two patients had a periprosthetic joint infection (PJI) per the 2018 ICM criteria. Cultures were monomicrobial in 79% and polymicrobial in 21% of cases. The most frequent bacterium in microbiological tissue and synovial fluid cultures was Staphylococcus aureus, observed in 26% of PJI patients. Periprosthetic joint infection with negative cultures occurred in 23% of patients. Conclusion Our results show the following: i) a high prevalence of Staphylococcus as an etiological agent for knee PJI; ii) a high incidence of polymicrobial infections in early infections; iii) the occurrence of PJI with negative cultures in approximately one fourth of the subjects.

Introduction: Brazilian agriculture is a major source of economic growth for the country, being one of the largest in the world, particularly prominent since colonial times. The study of postures in milking parlors is important to evaluate factors that may lead to the overload of body structures. Objective: To identify the main postural changes resulting from work activities in milkers, and to relate the degree of postural alteration with the years worked in the field, and their main functional complications. Method: This study is a cross-sectional, observational analytic research, conducted in the city of Pilar de Goiás. The STROBE methodology was used. Results: The main postural changes resulting from work activities found in milkers were rightward head tilt, forward head positioning, higher left acromion and anterior superior iliac spine, genu varum and genu flexum, and feet in valgus position. Conclusion: The main musculoskeletal dysfunctions found in rural producers are acquired due to years of work in incorrect postures, which are the main cause leading to their disability. Thus, an early change in postural habits can promote an improvement in the quality of life of these workers during old age. [ABSTRACT FROM AUTHOR]

14. Relationship of the Cruciate and Meniscofemoral Ligaments with the Knee Osteology. An Anatomical Study

Abstract Objective To analyze the dimensions of the posterior cruciate ligament (PCL), anterior cruciate ligament (ACL), the presence of meniscus-femoral ligaments MFLs in human knees, and the correlation with the dimensions of the knee skeleton. Methods Anatomical study on 29 specimens of human knees in which we measured the length and width of the cruciate and meniscus-femoral ligaments and the dimensions of femoral and tibia condyles and the femoral notch. The ACL length was calculated with different degrees of knee flexion. The relationship between the ligaments and bone dimensions were analyzed. Results The length of the ACL and the PCL were similar. Posterior MFL was more frequent and longer than the anterior MFL. We found the posterior MFL in the 72.41% of the knees and anterior MFL in 20.69%. The ACL presented 30% of its maximum length up to 60°, approximately half of its length between 90° and 120°, reaching its maximum length at 170°. We found a strong correlation between the length of the ACL and that of the PCL (p= 0.001). However, the lengths of the ACL and PCL were not related with the bone dimensions. Conclusion We have found no correlations between the cruciate and MFLs and the anatomical dimensions of the intercondylar notch and the proximal tibia and distal femur. The presence of the posterior MFL was more frequent and longer than that of the anterior ligament.

15. The Effects of Hydrolyzed Collagen and Collagen Peptide in the Treatment of Superficial Chondral Lesions: An Experimental Study

Abstract Objective To evaluate the effects of hydrolyzed collagen and collagen peptide in the treatment of superficial chondral lesions in rats. Method This research employed 18 Rattus norvegicus. A single intraarticular infiltration of sodium iodoacetate (2 mg solution) through the patellar ligament induced joint damage in previously anesthetized animals. We divided the animals into three groups: a control group, a collagen peptide group, and a hydrolyzed collagen group. Treatment consisted of oral administration of collagen peptide or hydrolyzed collagen for 30 days. Afterwards, we euthanized the animals and studied the joint chondral changes. We evaluated the results according to the chondrocyte clusters count and a histological evaluation, as per Pritzker et al. Results There was no statistical significance in injury stages between the control, hydrolyzed collagen, and collagen peptide groups (p= 0.11). Regarding scores, there was a statistical significance between the groups treated with hydrolyzed collagen and collagen peptide (p< 0.05), but not in comparison with the control group. Conclusion The proposed treatments of the induced chondral lesion with the oral administration of hydrolyzed collagen or collagen peptides were effective, resulting in lesion stabilization or regression, and warranting further experimental research to understand and improve the primary outcome of this study.

The knee is one of the main joints in the human body, and due to its conformation and functionality, it is easily susceptible to injuries. Surgical reconstruction of injuries around the knee and proximal region of the leg represents a major challenge due to the exposure of osteoarticular, tendinomuscular and neurovascular structures and has been performed using various muscle and musculocutaneous flaps. Other flaps are also described for treating these lesions, although less frequently in the literature, such as the cutaneous flap based on the pedicle of the superior lateral genicular artery. Next, It presents a report of five cases of patients who were victims of motorcycle accidents in which the lesion was covered around the knee and proximal region of the leg using a skin flap based on the pedicle of the superior lateral genicular artery, in which good coverage of lesions, high flap survival rate, good clinical and aesthetic results. The use of the superior lateral genicular artery presents aesthetic advantages since the color and texture of the flap are similar to those of the knee region and provide a better quality appearance, in addition to not causing joint mobility problems.

ABSTRACT Objective Compare the results of medial unicompartmental knee arthroplasty (UKA) using a mobile platform and total knee arthroplasty (TKA) in patients with isolated medial osteoarthritis. Methods Retrospectivecross-sectional study. Preoperative radiographs of 602 patientswho underwent knee arthroplastybetween February 2017 and February 2020 were evaluated. Isolated medial osteoarthritis was found in 125 patients. Of these, 57 underwent UKA and 68 TKA. With chart analysis and telephone interviews, we compared patients’ clinical outcomes and degree of satisfaction. The statistical analysis used a confidence level of 5%. Results The group of UKA patients obtained 65.8% of favorable results against 79.1% of those undergoing TKA in the function questionnaire (p0.5). Most patients were satisfied or very satisfied in both groups (88.6% of UKA and 91.2% of TKA) (p>0.999). Conclusion Patients submitted to UKA or TKA have presented the same degree of satisfaction and rate of postoperative complications when comparing patients with isolated medial osteoarthritis. UKA patients had less favorable results onthe clinical functional questionnaire than patients undergoing total arthroplasty. Level Of Evidence III;Retrospective Study.

ABSTRACT Objective: To report and compare the results of posterior cruciate ligament (PCL) and anterior cruciate ligament (ACL) reconstructions. Methods: In total, 42 patients were retrospectively evaluated, 20 with isolated PCL injuries (group 1) and 22 with isolated ACL ones (group 2) who were subjected to arthroscopic ligament reconstruction with autologous grafts and followed up for at least two years. To evaluate the results in group 1, objective IKDC and Lysholm scores, posterior drawer tests, and evaluations by a KT-1000 arthrometer were used, whereas for group 2, subjective IKDC and Lysholm score and the Lachman test were employed. To compare groups, objective IKDC and Lysholm scores and assessment via a KT-1000 arthrometer were considered. Results: Intragroup analysis showed improved results for all variables (p < 0.001) in both groups. Comparisons between groups showed a significant difference in objective IKDC scores (p < 0.001), but no such disparities for Lysholm ones (p = 0.052), clinical tests (p = 0.058) or evaluation by KT-1000 (p = 0.129). Conclusion: Treatment restored knee stability and function in both groups. Comparisons between groups showed that PCL reconstructions had inferior results than ACL ones according to patients’ objective IKDC scores. Level of Evidence II, Retrospective Study.

ABSTRACT Patellofemoral pain (PFP) may contribute to less activation of the quadriceps muscle, favoring joint overload and pain. Neuromuscular efficiency (NME) is a variable that evaluates the relationship between the amount of neural stimuli and the ability to generate force of in a given muscle, with the most efficient being the one that produces greater muscle force, with less activation of muscle fibers. In this sense, this study aimed to evaluate the strength and NME of knee extensors in women with and without patellofemoral pain. A total of 24 adult women, recruited via a questionnaire, aged from 18 to 30 years, with and without patellofemoral pain, participated in this study. Anamnesis, anterior knee pain scale, and numerical visual scale were applied. Subsequently, the knee extensor strength, with a hand-held dynamometer, and the NME of vastus medialis (VM), vastus lateralis (VL), and rectus femoris (RF) were assessed. Abstract Objective To compare magnetic resonance imaging (MRI) using a body coil with computed tomography (CT) in measuring the tibial tubercle-trochlear groove distance (TT-TG) and the patellar tendon-cartilaginous trochlear groove (PT-CTG) distances, and evaluate interrater reliability. Methods The study group consisted of 34 knees from 17 asymptomatic subjects with no history of knee pathology, trauma or surgery. A low-dose CT scan and an axial T1-weighted MRI sequence of the knees were performed with rigorous standardization of the positioning with full extension of the knees and parallel feet. Two musculoskeletal radiologists performed the measurements independently. The reliability of the TT-TG and PT-CTG distances on CT (17.1 ± 4.2 mm and 17.3 ± 4.2 mm) and of MRI (16.2 ± 3.7 mm and 16.5 ± 4.1 mm) was assessed by intraclass correlation coefficient (ICC [2,1]) and Bland-Altman graphs, as well as the interrater reliability for both methods. Results Good reliability and agreement was observed between CT and MRI measurements for TT-TG and PT-CTG, with an ICC of 0.774 (p< 0.001) and 0.743 (p< 0.001), respectively, and no systematic bias was observed. The interrater reliability was excellent for all measurements on both imaging methods. Conclusion This was the first study that compared MRI using a body coil with CT in measuring the TT-TG distance, with the potential clinical implication that the CT in this clinical setting could be avoided.

23. Long Term Outcomes of Computer-Navigated Lateral Opening Wedge Distal Femoral Osteotomy for Lateral Compartment Knee Arthrosis

Abstract Objective The primary aim of the present study was to evaluate the long-term outcomes including survivorship of computer navigated distal femoral lateral opening wedge osteotomy (DFLOWO). The secondary aim was to identify the potential factors that may influence its survivorship. Methods A retrospective analysis of prospectively collected data for patients with lateral compartment arthritis who underwent navigated DFLOWO from December 2006 to November 2012 was performed. The International Knee Documentation Committee (IKDC) and Knee Injury and Osteoarthritis Outcome Score (KOOS) scores were analyzed for outcome measures. Conversion to arthroplasty during the follow-up was the end point. Results A total of 19 DFLOWOs were performed in 17 patients with a mean age of 46.6 ± 6.5 years formed the study cohort. The coronal alignment was corrected from a mean of 7.1° (2-11°) valgus to a mean of 2.1° (0.5°-3°) varus. The IKDC scores improved from mean of 39 preoperatively to 53 at the mean long-term follow-up of 9.1 years. The mean KOOS scores at the long-term follow-up were pain 71, symptoms 56, activities of daily living 82, sports and recreation 59, quality of life 43. Survivorship of the DFLOWO was 78.9% at a follow-up of 9.1 years. Presence of ≥ grade 2 according to the International Cartilage Repair Society (ICRS) cartilage degeneration in the medial compartment of the knee and >7° preoperative valgus deformity were strongly correlated with conversion to total knee arthroplasty (TKA) at the long-term follow-up (r= 0.66). ConclusionsComputer navigated DFLOWO has satisfactory clinical outcomes and 79% survivorship in long-term follow-up. Presence of more than ICRS ≥ grade 2 degenerative changes in the medial compartment of knee with > 7° preoperative valgus deformity negatively affects the survivorship of DFLOWO in the long-term follow-up.

ABSTRACT The World Health Organization (WHO) estimates that by 2025 about 2.3 billion adults will be overweight, with more than 700 million obese. Obese patients with joint pain and reduced physical function represent a challenging group to treat effectively. Objective: To evaluate patients undergoing bariatric surgery and the implications of this surgery on knee joint pain and to conduct anamnesis and apply specific questionnaires to deepen the discussion and elucidate the knee joint symptoms related to obesity. Methods: Observational cross-sectional study with tabulation and analysis of collected data. Results: We obtained a significant result when comparing knee pain pre and post-surgery, in which pain increased by 15.8%. Conclusion: Although worsening or maintenance of pain may occur, this fact is associated to factors such as the increase of functional activities of a joint that was previously in disuse and the loss of muscle mass as a sustainer. We concluded that the improvement of joint pain complaints were mainly due to the reduction of joint overload. Level of Evidence IV, Case Series.

ABSTRACT Objective: To compare the application of partial meniscectomy concomitant with primary ACL reconstruction, using the graft from the patellar tendon with individuals who underwent only ACL reconstruction, in clinical functional criteria and degree of osteoarthritis (OA), after 10 years of the surgical process. Methods: This is a retrospective cross-sectional study with 37 patients who underwent ACL reconstruction with a graft from the patellar tendon, associated or not with partial meniscectomy, divided into 2 groups: with meniscal injury (n = 22) and without meniscal injury (n = 15). Anthropometric data and four outcome measures were used to analyze the results: SF-36 questionnaire, arc of motion assessment, Knee injury and Osteoarthritis Outcome Score (KOOS), and Ahlbäck Radiographic Classification. Results: No differences were found for health-related quality of life, arc of motion, functional condition and knee OA severity/grade in patients who underwent partial or no meniscectomy in conjunction with ACL reconstruction (p > 0.05). Conclusion: Participants who underwent partial meniscectomy in conjunction with primary ACL reconstruction with a graft from the patellar tendon, after 10 years of the surgical process, showed no significant differences in the clinical functional criteria and severity of knee OA, compared to individuals who underwent only ACL reconstruction. ABSTRACT Objective: To evaluate the epidemiological and clinical characteristics of knee injuries in Brazilian Jiu-Jitsu (BJJ) practitioners. Methods: Cross-sectional study, using a mixed questionnaire, based on the Referred Morbidity Index. Results: 198 amateur and professional BJJ fighters, of both sexes, aged between 18 and 60 years, participated in the study. The majority (88%) of the fighters had only one knee injury (p < 0.001). In total, 29.8% proportion of knee injuries (p < 0.001) was identified, which were mainly from the medial collateral ligament (38%), caused by a sprain mechanism (86%) and conservative treatment (65%). Conclusion: A high prevalence of knee injuries in JJB fighters was found, compared to other sports that also perform rotational movements and have great body contact, such as mixed martial arts (MMA), judo, soccer, basketball and handball. Some JJB strikes, such as the key and the projection, can cause greater knee joint stress, both in the attacking fighter and in the opponent. The knowledge of the epidemiological characteristics of sports injuries is important in the elaboration of prevention and training protocols more specific to the sport and for the understanding of the complex mechanisms involved with this outcome in sport. 29. Treatment of Chondral Knee Lesions with Autologous Chondrocytes Embedded in a Fibrin Scaffold. Clinical and Functional Assessment

Abstract Objective The aim of our study is to analyze the clinical and functional results obtained using autologous chondrocytes embedded in a fibrin scaffold in knee joint injuries. Methods We included 56 patients, 36 men and 20 women, with a mean age 36 years. Six of the patients were professional athletes, with single knee injuries that were either chondral or osteochondral (43 chondral, 9 osteochondral, 2 cases of osteochondritis dissecans and 2 osteochondral fractures), 2 to 10 cm2 in size and ≤ 10 mm deep, with no signs of osteoarthritis. The location of the injury was in the patella (8), the medial femoral condyle (40) and lateral femoral condyle (7) and one in the trochlea. The mean follow-up was 3 (range: 1–6) years. The clinical course was assessed using the Cincinnati and Knee Injury and Osteoarthritis Outcome (KOOS) scores, 6 and 12 months after surgery. The paired Student t-test was used to compare pre-and postoperative results. Results Six months after the implant, patients resumed their everyday activities. On the assessment scores, their condition was improving in comparison with their presurgical state (p < 0.05). They were also able to carry out their sporting activities more easily than prior to surgery (p < 0.05). Conclusion The seeding of chondrocytes in fibrin may provide a favorable microenvironment for the synthesis of extracellular matrix and improved the clinical condition and activity of the patients 1 year after surgery.

ABSTRACT Objective: To analyse the distances between the femoral insertions of the popliteus tendon (PT) and the lateral collateral ligament (LCL) through dissections of cadaveric specimens in a mixed population. Methods: Fresh cadavers were dissected, and the anthropometric data of all specimens were recorded. The distances from the origin of the PT to the LCL in the femoral region and the diameter of each structure were measured using a digital calliper. Results: In total, 11 unpaired knees were dissected, eight men and three women, with an average age of 71.5 ± 15.2 years, weight of 57.2 ± 15.6 kg, and a mean height of 170.5 ± 8.2 cm. The distance from the center of the femoral footprint of the LCL to the PT was 10.0 ± 2.4 mm. The distances between the edges closest to each other and those more distant from each other were 3.1 ± 1.1 mm and 16.3 ± 2.4 mm, respectively. Conclusion: The distance between the midpoints of the PT and the LCL in our mixed population is smaller than the distances often reported in the literature. PLC reconstruction with separate tunnels for the LCL and PT may not be technically possible for individuals of any population. Level of Evidence III, Diagnostic studies.

ABSTRACT Objective: To compare the use of intravenous and topical tranexamic acid (TXA) in unilateral primary total knee arthroplasty (TKA) in relation to blood loss and complications inherent to the medication. Method: Three groups with 14 patients each were constituted, and all of them were operated using the same surgical technique. In Group 1, usual measures for bleeding control were performed. Group 2 patients received TXA topically on the joint surface. In Group 3, intravenous TXA was used. Hemoglobin (HB), hematocrit (HTC), platelets (PLAT), prothrombin time, activated partial thromboplastin time and volume of blood drained observed 24 hours after arthroplasty were compared to the values of tests found before surgery. Results: There was a decrease in the concentration of HB, HTC and PLAT in all groups in relation to the preoperative, however without significant difference. Group 3 had a lower mean volume of drained blood than the other groups, with statistical significance. No adverse effects or thromboembolic events were observed in the groups that received TXA. Conclusion: This study showed superiority in the use of intravenous TXA in decreasing the volume of bleeding, without increasing the risk of thromboembolic events. Level of Evidence I, High quality randomized trial with statistically significant difference or no statistically significant difference but narrow confidence intervals.

ABSTRACT Objective: By analyzing our cases of posterior cruciate ligament (PCL) tibial avulsion fracture, we noted that a U-shaped image was present in the anteroposterior plain radiographs view of the affected knee, even in cases where the profile view of the knee had been inconclusive as to tibial PCL avulsion fracture, a “hidden” fracture. Therefore, we aimed to investigate whether there was an anatomical correlation between this radiological U sign and the tibial insertion of the PCL and to ascertain the intra- and inter-rater reliability of this sign in clinical practice. Methods: The data of the widths and heights area of the PCL tibial insertion area, and the U sign area were measured and compared to the largest width of the tibia. Two moreover, the reliability and reproducibility of this imaging were analyzed. Results: The areas height of the U-sign area and the anatomical insertion area of the posterior cruciate ligament showed no difference, and both were topographically located in the two central quarters of the proximal end of the tibia. The radiographic assessment showed excellent Kappa agreement rates between interobserver and intraobserver, with high reliability and reproducibility. Conclusion: The U sign is a radiographic feature of PCL tibial avulsion fracture seen on the radiograph AP view, there is a high association between the ratios of the U-sign area height in the X-ray and the anatomical height of the PCL tibial insertion site MRI with the largest width of the proximal tibia. The radiographic U sign showed excellent rates of interobserver and intraobserver agreement with Kappa values higher than 0.8. 33. Portuguese Cross-Cultural Adaptations of the Pediatric International Knee Documentation Committee Score

Introduction: The aim of this study was to translate the Pediatric International Knee Documentation Committee Score (Pedi-IKDC) into European Portuguese language. The Pedi-IKDC was originally developed in the English language and its main construct is functional assessment of knee disorders in children and adolescents. Material and Methods: The original English version of the questionnaire was translated to European Portuguese using the forwardbackwards method. Patients aged eight to 17 with knee disorders were considered eligible for the study. An online platform was implemented to collect patient responses, including general patient information, the questionnaires Pedi-IKDC, EuroQol-5 Dimension Youth and Childhood Health Assessment Questionnaire. Three surveys were sent: at the moment of first evaluation (T0), after two (T1) and ten (T2) weeks. These surveys followed the Consensus-based Standards for the Selection of Health Measurement Instruments - COSMIN Checklist recommendations. The internal consistency, reliability, error of measurement, structural and construct validity (by means of correlation with previously validated scales), responsiveness and interpretability (floor/ceiling effects, MIC and ROC curve) were evaluated. Results: Forty-seven patients completed T0, 42 patients completed T1 and 40 patients completed T2. The factorial analysis confirmed that the scale has one dimension. Cronbach alpha (α) was 0.94; interclass correlation coefficient was 0.92; smallest detectable change was 19.04 for individuals and 3.31 for groups; standard error of measurement was 6.87; minimum important change was 18.48; floor and ceiling effects were absent. More than 75% of the hypotheses tested for construct validity were confirmed, showing its adequacy. The variation of scores between T0 and T2 correlated with the clinical evolution of the participants (r = 0.421, p < 0.05). Conclusion: The Portuguese version of Pedi-IKDC demonstrated good psychometric properties, being a valuable tool for clinical assessment of pediatric patients with knee disorder.

35. Functional Outcome of Patients Undergoing Knee Arthrodesis after Infected Total Arthroplasty

Abstract Objective Retrospectively assess the functional outcome of patients undergoing arthrodesis after septic failure of total knee arthroplasty. Methods Eighteen patients were evaluated, with a mean time of 3.7 years of follow-up. Arthrodesis surgery was performed using a narrow anterior dynamic compression plate, and medial or external fixator in two planes of the joint (Orthofix Bone Growth Therapy, Lewisville, TS, USA), at the surgeon's discretion. Results The most frequent pathogen found was Staphylococcus aureus methicillin sensitive (38.9%). The mean lower limb discrepancy was 3.63 cm. The mean knee society score was 68 points. According to the visual analog scale for pain, 44% of the patients had a 0 score at the time of assessment, and 22.2% were very dissatisfied, despite the bone healing achieved. Patients were assessed for the ability to walk with or without assistance. Conclusion Knee arthrodesis surgery was effective in controlling the infectious process and decreasing pain complaints, but satisfaction with the procedure was low.

36. Report of a clinical and laboratory management of cell therapy for knee cartilage in the face of mycoplasma contamination

ABSTRACT To describe a case of autologous chondrocyte implantation after cell culture contamination by Mycoplasma pneumoniae and the measures taken to successfully complete cell therapy in a patient with focal chondral lesion. A 45-year-old male patient, complaining of chronic pain on the knee and no history of trauma. He had a chondral lesion in the trochlear region of the femur and clinical tests compatible with pain in the anterior compartment of the knee. Conservative treatment failed to alleviate symptoms. Surgical treatment was indicated, but due to the size of the lesion, membrane-assisted autologous chondrocyte implantation was the technique of choice. Cartilage biopsies were collected from the intercondylar region of the distal femur. After isolation, chondrocytes were expanded ex vivo in a trained laboratory, for three weeks, and seeded onto a commercially available collagen membrane prior to implantation in the knee. Two days before surgery, a cell culture sample tested positive for Mycoplasma pneumoniae. The source of contamination was found to be autologous blood serum, extracted from the patient´s peripheral vein, and used to supplement the cell culture medium. After treating the patient with antibiotics, all procedures were repeated and the new final cell product, free from contaminants, was successfully implanted. We discuss the strategies available to deal with this situation, and describe the results of this particular case, which led to modifications in the autologous chondrocyte implant protocol.

ABSTRACT Objective: The synovial fold is an intra-articular structure found in more than 50% of the knees, which can cause symptoms similar to meniscal injuries. These symptoms are mostly related to hypertrophy of the synovial fold resulting from inadequate physical activity. Conservative treatment with readjustment of sports activity and muscle rebalancing solves most cases. Rare cases require surgical treatment, which is indicated due to the persistence of instability, blockage and pain. We present our experience in the treatment of this pathology. Methods: 58 patients (70 knees), with 62 knees treated conservatively and 8 treated surgically exclusively for the pathological synovial fold. Results: Description of the series and treatment results are reported. Conclusion: The non-surgical treatment of the pathological synovial fold of the knee provided good results within 60 days of rehabilitation program in almost 90% of the patients. Arthroscopic resection of the synovial fold is a surgery that has a longer and laborious rehabilitation period, despite good results in most cases. Level of Evidence IV, Case series.

ABSTRACT Objective: Although the mobile-bearing Oxford unicompartmental knee arthroplasty (OUKA) seems an appropriate procedure to treat spontaneous osteonecrosis of the knee (SONK), aseptic tibial component loosening was the leading cause for medial UKA failure. This study aimed to observe short-term and midterm clinical outcomes following OUKA and determine whether tibial lesion affects the procedure clinical and radiographic outcomes. Methods: Sixty patients (mean age 73.1 ± 6.6 years) diagnosed with SONK in the medial femoral condyle and treated with OUKA were separated into two groups using T1-weighted preoperative magnetic resonance imaging (MRI): group F (necrotic lesion confined to the femur) and group T (necrotic lesion spread to the tibia). The Oxford Knee Score (OKS), maximum flexion angle (MFA), and radiographic findings (radiolucent line and subsidence) were compared between the two groups using unpaired t-test. Results: Both groups showed significant improvement in OKS and MFA values at the final follow-up, but without significant differences in the clinical and radiographic outcomes. Conclusion: OUKA is a reliable treatment procedure for SONK in the short and midterm. The presence of tibial lesions on preoperative MRI does not affect postoperative radiographic and clinical outcomes. Level of Evidence IV, Case Series.

41. Effectiveness of a musculoskeletal injury preventive program in elite beach volleyball players: a retrospective study

Objective: to evaluate the effectiveness of a preventive program in athletes of the elite beach volleyball. Method: this is a case series study of seventeen athletes from Brazilian beach volleyball team, between 15 and 20 years. Information about demographic and anthropometric data, and information about hours of practice and number of physiotherapy sessions were obtained in the medical records. The intervention program was performed for 12 weeks, including exercises for the joint complex of the shoulder and lower limbs. The severity of the injury and the functional tests (glenohumeral internal rotation, dynamic valgus, posterior chain flexibility, and vertical jump) was used as outcome measures. The pre and post-intervention outcome were compared. Results: the preventive program showed a low injury rate (1.37/1000 hours). The functional tests showed contradictory results in post-intervention evaluation. The left shoulder internal rotation (before=80º versus after=86º, p=0.024) and posterior chain flexibility (before=30º versus after=33,1º, p=0,090) showed improvement. There was no change in dynamic knee valgus (before p=0.305, after p=0.894) and vertical jump (before=300 cm versus after=296 cm, p=0.12). Conclusion: the preventive program performed showed a low injury rate and the functional tests do not show a significant improvement in post-intervention evaluation.

ABSTRACT Objective: To systematically review and meta-analyze the performance of return to play (RTP) and non-RTP patients in different assessment tools after anterior cruciate ligament reconstructions (ACLR). Methods: Out of 182 studies searched on PubMed, 11 presented RTP and non-RTP groups assessing the performance of young individuals, practitioners of different sports, with different tools. Results: There was higher limb symmetry (7.13% [95%CI 4.55; 9.70], p < 0.001), Tegner activity scale (2.41 [95%CI 0.18; 4.64], p = 0.03), functional scores such as International Knee Documentation Committee (x7.44 [95%CI 4.69; 10.19], p < 0.001), Knee Osteoarthritis Outcome score for quality of life (14.75 [95%CI 10.96; 18.54], p < 0.001) and for sports/recreation (11.86 [95%CI 8.87; 14.86], p < 0.001); and lower knee laxity (-0.25 mm [95%CI -0.36; -0.14], p < 0.001) in RTP compared to non-RTP patients following ACLR. Conclusion: We confirmed that these different tools can differentiate RTP for non-RTP patients, which may contribute to the physician’s decision about the ideal time for RTP. This work seeks to, based on the product development methodology developed by Pahl et al. (2005), present the design and development of a CPM (Continuous Passive Motion) prototype indicated for the rehabilitation of lower limbs. In this project, the product development methodology was employed until the final variant of the lower limb rehabilitation system was obtained. The preliminary design and manufacturing details were developed, so that the concept could be validated, the prototype was manufactured to test its functioning. To ensure that the drive system is designed and manufactured in the most correct way possible, the installation position of the actuator was defined by means of 3D software simulation, this simulation, using the imaginary axis that crosses the patient's knee as a reference. The articulation points and the stroke of the actuator were simulated, ensuring that the patient's leg movement was performed correctly. The objective of the kinematic study in software was to ensure that the behavior of the prototype was correct before manufacturing, what can be proven with the tests with the prototype. ABSTRACT Objective: To evaluate the correlation between knee axis and hindfoot axis in patients with advanced gonarthrosis, and the association between ankle function and angular deformities. Methods: 72 patients were enrolled in the study: 66% were women, and mean age was 58.7 years. The anatomical axis of the knee and hindfoot were measured by short knee radiographs and long axial view of the hindfoot. Results: Among the study group, 79.2% presented varus knee (mean 15º ± 7.69º) and 20.8% valgus (mean 15.9º ± 7.7º). 63.9% had hindfoot varus (mean 8.5º ± 6.07º) and 36.1% valgus (mean 3.9º ± 3.92º) (p < 0.05). The mean value for the American Orthopaedic Foot and Ankle Society (AOFAS) score was 74.26 points, and values were significantly higher among patients with hindfoot varus (p < 0.05). We found no correlation between gender or AOFAS score and knee and hindfoot axes, nor between deformities in the knee and hindfoot axes (p > 0.05). The subgroup genu valgum - hindfoot varus presented a moderate correlation (r = 0.564; p < 0.05). Conclusion: We found no association between the anatomical axes of the knee and hindfoot. Patients with gonarthrosis and hindfoot varus presented a better ankle function. ABSTRACT Objective: We aimed to investigate the change in bone mineral density (BMD) in the first postoperative year in patients that underwent total knee arthroplasty (TKA) due to primary osteoarthritis of the knee. Methods: Preoperative and first postoperative year dual-energy X-ray absorptiometry measurements of 76 patients with knee osteoarthritis, who undergone surgery between 2016 and 2018 due to the recommendation for TKA, were statistically evaluated in the study. Results: Of the 19 patients with a normal BMD in the preoperative period, 73.7% (n = 14) continued to have a normal BMD in the postoperative period. Of the 34 patients with a low BMD (osteopenia) in the preoperative period, 91.2% (n = 31) did not show any change, whereas osteoporosis was observed in two patients (5.9%) in the postoperative period. Of the 23 patients with osteoporosis in the preoperative period, 95.7% (n = 22) did not show any change, whereas osteopenia was observed in one patient (4.3%) in the postoperative period. Both the T and Z scores of the spine (L1-L4) and proximal femur showed a slightly positive trend, however, with an insignificant statistical difference (p ≥ 0.05). Conclusion: Patients that underwent TKA experienced a statistically insignificant bone gain at the spine and proximal femur twelve months after the surgery. Abstract Objective To compare the plantar pressure distribution and the kinematics of the rearfoot on the stance phase of subjects with or without patellofemoral pain syndrome (PFPS). Methods A total of 26 subjects with PFPS and 31 clinically healthy subjects, who were paired regarding age, height and mass, participated in the study. The plantar pressure distribution (peak pressure) was assessed in six plantar regions, as well as the kinematics of the rearfoot (maximum eversion angle, percentage of the stance phase when the maximum angle was reached, and percentage of the stance phase in which the rearfoot was in eversion). The data were analyzed by descriptive and inferential statistics, with a significance level of p≤ 0.05. Results The pressure on the six plantar regions analyzed and the magnitude of the maximum eversion angle of the rearfoot when walking on flat surfaces did not present differences among the subjects with PFPS. However, the PFPS subjects showed, when walking, an earlier maximum eversion angle of the rearfoot than the subjects on the control group, and stayed less time with the rearfoot in eversion. Conclusion The PFPS seems to be related to modifications on the temporal pattern on the kinematics of the rearfoot.

ABSTRACT Objective: To determine the factors that affect the functional outcome of Oxford Phase 3 unicompartmental knee arthroplasty (UKA). Methods: We assessed a total of 52 UKA knees of 49 patients with a minimum follow-up of 2 years (24-72 months). We recorded the results for Range of motion (ROM) and body mass index (BMI) and the presence o patello-femoral arthrosis (PFA). In the radiological evaluation, we measured the posterior tibial slope (PTS), the tibial plateau angle (TPA) and the femorotibial angle, in addition to an assement using the Oxford radiological criteria. Patients were grouped by age, follow-up time, BMI, radiological criteria, PFA presence, occurance of complications and revision surgery. The clinical and functional results of these groups were compared statistically. Results: A total of 40 women and 9 men participated in the study, with an average age of 60 years, and a mean BMI of 34.6. No significant differences were found among the age and PFA groups. Postop VAS scores were high and knee evaluation scores were significantly lower in the morbidly obese group and in the groups with postop TPA 90º. The revision ratio was 11.5%. Conclusion: Postop TPA, PTS and morbid obesity are the most significant factors that can lead to revision surgery. Level of Evidence IV, Case series.

ABSTRACT Objective: To provide an analysis of the 50 most cited articles on knee surgery in the Brazilian medical literature. Methods: This is a study of systematic review and meta-analysis, level of evidence 3. It was carried out through search in the SCOPUS database to identify scientific articles published in the Brazilian medical literature. Eighty-six Brazilian journals were analyzed, and articles with ten or more citations and which had the word “joelho” or “knee” in the title, abstract or keywords were selected. Results: All articles were published as of the year 2000 in 14 journals. The Journal Clinics presented the largest number of publications, followed by Acta Ortopédica Brasileira. The main focus of the studies was on anatomy and biomechanics, mainly from Brazilian authors. Most of the authors were Brazilian, from Brazilian and public research institutions. Conclusion: Biometric analysis has been gaining ground in recent years, but its interpretation must consider various aspects related to the paramount analytical importance.

ABSTRACT Introduction To compare the functional results, satisfaction rates, and revisions of total knee arthroplasties performed by the same surgical team using either Brazilian or imported implants, with a minimum follow-up of 5 years after surgery. Materials and Methods A retrospective cohort study analyzing the medical records and interviews of patients who underwent total knee arthroplasty with Brazilian or imported implants with a minimum of 5 years after surgery. Results One hundred and fifty patients were evaluated (164 knees). In the functional questionnaire, 71% of patients had favorable answers in the group of patients who underwent surgery using the Brazilian prosthesis and 74.8% in the group with imported implants (p=0.634). There was no statistical difference in satisfaction between the groups, with 78.4% of patients satisfied or very satisfied in the Brazilian implant group and 90.7% in the imported implant group (p=0.053). Loosening of the implants was reported in 5.3% versus 4.7% (p>0.999). Conclusion The total knee arthroplasties performed by the same surgical team with a minimum follow-up period of 5 years showed similar levels of satisfaction, function, and complications with both the Brazilian and imported implants.
