OBJETIVO: A prevalência de conjuntivite alérgica (CA) não foi estabelecida. Estimativas sugerem que alergias oculares afetam de 15 a 20% da população mundial, ainda que a maioria dos estudos epidemiológicos abranjam sintomas de alergia nasal e ocular e não sejam específicos a respeito da CA. O objetivo deste estudo foi verificar a prevalência de sintomas, comorbidades e o impacto da alergia ocular em adolescentes. MÉTODOS: Os adolescentes foram selecionados de uma amostra de escolas e preencheram, em sala de aula, um questionário previamente validado sobre os sintomas da CA. O seu diagnóstico foi considerado quando mais de três episódios de prurido ocular foram relatados nos últimos 12 meses. Sintomas relacionados, como lacrimejamento, fotofobia, sensação de corpo estranho, impacto sobre as atividades diárias e diagnóstico de conjuntivite alérgica, foram analisados. RESULTADOS: Foram obtidos questionários de 3.120 adolescentes (média de 13,3±1,1 ano). Nos últimos 12 meses, 1.592 (51%) adolescentes tiveram prurido ocular. O sintoma relacionado mais frequente foi lacrimejamento (74%), seguido de fotofobia (50,1%) e sensação de corpo estranho (37,1%). A prevalência de conjuntivite alérgica foi de 20,7%, afetando mais pessoas do sexo feminino do que do masculino (56,1% em comparação a 45,9%; p = 0,01). O risco de um adolescente com alergia ocular apresentar asma, rinite e eczema atópico foi (RC = 5,7; IC de 95%: 4,5 a 7,1); (RC = 3,6; IC de 95%: 3,0 a 4,3) e (RC = 2,6; IC de 95%: 2,0 a 3,5), respectivamente. Uma interferência grave nas atividades diárias foi relatada por 30,5%. CONCLUSÕES: Sintomas de alergia ocular são comuns, frequentemente relacionados a outras doenças alérgicas, e causam impacto sobre as atividades diárias de adolescentes. OBJECTIVE: The prevalence of allergic conjunctivitis (AC) has not been established. Estimates suggest that ocular allergies affect 15% to 20% of the worldwide population, yet most epidemiological studies encompass nasal and ocular allergy symptoms and have not been specific to AC. The aim of this study was to verify the prevalence of ocular allergy symptoms, co-morbidities, and their impact on adolescents. METHODS: Adolescents were selected from a sample of schools, and completed in classrooms a previously validated questionnaire on symptoms of AC. AC diagnosis was considered when more than three episodes of ocular itching were reported in the past 12 months. Related symptoms such as tearing, photophobia, foreign body sensation, impact on daily activities, and diagnosis of AC were analyzed. RESULTS: Questionnaires were obtained from 3,120 adolescents (mean age 13.3 ± 1.1 years). Ocular itching in the past 12 months occurred in 1,592 (51%). The most frequently associated symptom was tearing (74%), followed by photophobia (50.1%) and foreign body sensation (37.1%). The prevalence of AC was 20.7%, affecting more females than males (56.1% versus 45.9%; p = 0.01). The risks of an adolescent with ocular allergy to present asthma, rhinitis, and atopic eczema were (OR = 5.7; 95% CI: 4.5 to 7.1), (OR = 3.6; 95% CI: 3.0 to 4.3), and (OR = 2.6; 95% CI: 2.0 to 3.5), respectively. Severe interference in daily activities was reported by 30.5%. CONCLUSIONS: Symptoms of ocular allergy are common, frequently associated to other allergic diseases, and impact the daily activities of adolescents.

Resumo: O angioedema hereditário devido à deficiência de inibidor de C1 (AEH-C1-INH) é caracterizado por episódios recorrentes de angioedema do tecido subcutâneo e do trato gastrointestinal e respiratório. Nosso objetivo foi identificar mutações no gene SERPING1, que codifica o C1-INH, e investigar o impacto do tipo de mutação nas características clínicas de pacientes com AEH-C1-INH. Além disso, investigamos a ancestralidade em pacientes com AEH com C1-INH normal com mutações no gene que codifica o fator XII da coagulação, designado como AEH-FXII. Métodos: A análise de SERPING1 foi realizada em 205 indivíduos de 16 famílias de pacientes-índice com AEH-C1-INH. Os dados clínicos foram coletados durante entrevistas médicas. DNA genômico foi isolado do sangue total e PCR foi realizada com primers específicos. A análise funcional prevista das mutações foi realizada usando as ferramentas de bioinformática SIFT, PolyPhen2 e MutationTaster. A gravidade da doença foi avaliada por dois escores independentes. Os pacientes foram classificados como portadores de deleções, inserção, duplicação, mutação nonssense e mutações que afetam Arg466 no centro reativo (grupo 1, n = 48); e mutações missense, exceto as de Arg466 (grupo 2, n = 12). Análise de ancestralidade da mutação dominante encontrada em pacientes com AEH-FXII, c.983C>A (p.Thr328Lys), foi realizada em 6 pacientes com AEH-FXII e seus pais (sendo um portador da mutação) avaliando hapótiplos previamente descritos em pacientes europeus. Resultados: Sessenta pacientes (33F: 27M), de 10 meses a 63 anos de idade, apresentaram mutações em SERPING1. Cinquenta e oito apresentaram AEH-1 e dois tinham AEH-2. As mutações de SERPING1 incluíram 10 mutações missense e 6 não-missense (mutações estruturais que levam a frameshifts ou mutações nonssense), seis das quais eram novas. Não foram observadas diferenças significantes entre os pacientes dos grupos 1 e 2 em relação à idade, sexo, sensibilidade ao estrogênio, idade de início dos sintomas, locais de apresentação do angioedema, necessidade de profilaxia a longo prazo e escores de gravidade clínica. Conclusões: Os presentes resultados mostraram que uma variedade de mutações leva a manifestações clínicas heterogêneas de AEH-C1-INH e que a gravidade da doença pode não estar estritamente relacionada ao tipo de mutação. Foi encontrado nos pacientes com AEHFXII o mesmo padrão de hapótiplos de pacientes europeus, sugerindo que esta mutação tenha o mesmo fundador comum europeu. Background Hereditary angioedema due to C1 inhibitor deficiency (C1-INH-HAE) is characterized by recurrent episodes of angioedema of the subcutaneous tissue and the gastrointestinal and respiratory tract. Our objective was to identify mutations in the SERPING1 gene, which codes for C1-INH, and to investigate the impact of the mutation type on the clinical characteristics of patients with A C1-INH-HAE. In addition, we investigated the ancestry in patients with normal C1-INH-HAE with mutations in the gene encoding coagulation factor XII, designated as AEH-FXII. Methods: SERPING1 analysis was performed on 205 individuals from 16 families of C1-INH-HAE index patients. Clinical data were collected during medical interviews. Genomic DNA was isolated from whole blood and PCR was performed with specific primers. Predicted functional analysis of mutations was performed using the bioinformatics tools SIFT, PolyPhen2 and MutationTaster. Disease severity was assessed by two independent scores. Patients were classified as having deletions, insertion, duplication, nonsense mutation and mutations affecting Arg466 in the reactive center (group 1, n = 48); and missense mutations, except those of Arg466 (group 2, n = 12). Ancestry analysis of the dominant mutation found in patients with AEH-FXII, c.983C> A (p.Thr328Lys), was performed on 6 patients with AEH-FXII and their parents (being a carrier of the mutation) evaluating previously described multiple subjects Europeans. Results: Sixty patients (33F: 27M), aged 10 months to 63 years, presented mutations in SERPING1. Fiftyeight had HAE-1 and two had HAE-2. SERPING1 mutations included 10 missense and 6 non-missense mutations (structural mutations leading to frameshifts or nonssense mutations), six of which were new. No significant differences were observed between patients in groups 1 and 2 regarding age, gender, estrogen sensitivity, age at onset of symptoms, angioedema presentation sites, need for long-term prophylaxis, and clinical severity scores. Conclusions: The present results showed that a variety of mutations lead to heterogeneous clinical manifestations of C1-INH-HAE and that the severity of the disease may not be closely related to the type of mutation. AEH-FXII patients were found to have the same pattern of Europeans\' multiple subjects, suggesting that this mutation has the same common European founder.

O angioedema hereditário (AEH) é uma doença autossômica dominante, caracterizada por episódios recorrentes de edema subcutâneo, do trato gastrointestinal e das vias aéreas superiores. Em sua forma clássica, o AEH é causado por deficiência do inibidor de C1 (C1- INH) e está associado a mutações no gene SERPING1. Recentemente, foi descrito o AEH com inibidor de C1 normal, resultante de mutações no gene F12, que codifica o Fator XII da coagulação (FXII), denominado AEH-FXII; e de mutações nos genes PLG e ANGPT1, que codificam Plasminogênio e Angiopoietina-1, caracterizando os tipos AEH-PLG e AEHANGPT1. O AEH com inibidor de C1 normal não cursa com diminuição de C1-INH quantitativo e/ou funcional, ou de C4, sendo o diagnóstico realizado pela presença de sintomas clínicos sugestivos e história familiar. Os objetivos do presente estudo foram: avaliar a discriminação alélica como método alternativo ao sequenciamento por método de Sanger, para o diagnóstico de AEH com inibidor de C1 normal por mutação c.983C>A no gene F12; determinar se a discriminação alélica seria uma técnica de melhor custo-benefício; e investigar outras mutações previamente descritas no exon 9 do gene F12, e nos genes PLG e ANGPT1 em pacientes com suspeita clínica de AEH com inibidor de C1 normal, e negativos para mutação c.983C>A em F12. Cento e oitenta e quatro indivíduos incluindo 51 pacientes- índice com suspeita clínica de AEH com inibidor de C1 normal, e seus familiares, foram previamente investigados por sequenciamento de Sanger. Esses indivíduos foram investigados por discriminação alélica, e a concordância entre os resultados para a mutação c.983C>A foi verificada por estatística Kappa. Custos e tempo de execução entre os dois métodos foram avaliados. Casos-índice negativos para a mutação c.983C>A no gene F12 foram avaliados para outras mutações no exon 9 do gene F12 e para mutações previamente descritas c.988A>G e c.807G>T em PLG e ANGPT1, respectivamente, usando sequenciamento por método de Sanger. Mutação no gene F12 foi identificada em 96 indivíduos (24 pacientes- índice e 72 familiares). Em todos esses pacientes foi encontrada a mutação c.983C>A. 88 indivíduos foram negativos para esta mutação. Os resultados foram 100% concordantes entre os dois métodos. Setenta e um dos 96 pacientes positivos para mutação c.983C>A eram do sexo feminino; 78,9% e 56% eram pacientes sintomáticos do sexo feminino e masculino, respectivamente. A discriminação alélica apresentou custo e tempo de execução 79,7% e 82,3% menores em comparação ao sequenciamento por Sanger, respectivamente. Outras mutações no gene F12, e mutações em PLG e ANGPT1 não foram encontradas. Entre os pacientes negativos para mutações associadas ao AEH com inibidor de C1 normal, 11 foram diagnosticados como AEH-desconhecido e 16 como angioedema idiopático adquirido não histaminérgico. Os resultados do presente estudo nos permitiram construir um algoritmo para diagnóstico de pacientes com AEH com inibidor de C1 normal. Em áreas onde a mutação c.983C>A em F12 é predominante entre pacientes com AEH-FXII, a discriminação alélica pode ser um método adequado para screening inicial. Em pacientes com resultados negativos, sequenciamento do exon 9 de F12 pelo método de Sanger estaria indicado. Pacientes remanescentes com resultados negativos seriam genotipados para mutações previamente descritas em PLG e ANGPT1. Estudos futuros em outros locais serão necessários para estabelecer se a discriminação alélica apresentaria melhor custo-benefício, com potencial para mais acessibilidade ao diagnóstico em pacientes portadores da doença no Brasil Hereditary angioedema (HAE) is an autosomal dominant disease characterized by recurrent episodes of edema of subcutaneous tissue, gastrointestinal tract, and upper airways. In its classical form, HAE is caused by deficiency of C1 inhibitor (C1-INH) and it is associated with mutations in the SERPING1 gene. Recently, HAE with normal C1 inhibitor has been described, resulting from mutations in F12 gene, which encodes coagulation factor XII (FXII), designated as FXII-HAE; and from mutations in PLG and ANGPT1 genes, which encode Plasminogen and Angiopoietin-1, characterizing PLG-HAE and ANGPT1-HAE types. HAE with normal C1-INH does not present with decrease in quantitative and/or functional levels of C1-INH or C4, and diagnosis is carried out by presence of suggestive clinical symptoms and family history. Diagnosis can only be confirmed by genetic sequencing. The objectives of the present study were: to evaluate the allelic discrimination as an alternative method to sequencing by Sanger method for the diagnosis of HAE with normal C1-INH due to mutation c.983C>A in F12 gene; to determine whether allelic discrimination would be a better cost-effective technique; and to investigate presence of other mutations previously described in exon 9 of F12 gene and in PLG and ANGPT1 genes in patients with clinical suspicion of HAE with normal C1-INH who were negative for c.983C>A mutation in F12. One hundred and eighty-four individuals including 51 index patients with clinical suspicion of HAE with normal C1-INH and their relatives were previously investigated by Sanger sequencing. These individuals were investigated for allelic discrimination, and concordance of the results for the c.983C>A mutation was verified by Kappa statistics. Costs and execution time of the two methods were evaluated. Negative index cases for c.983C>A mutation in F12 gene were evaluated for other mutations in exon 9 of the F12 gene and for mutations previously described c.988A>G and c.807G>T in PLG and ANGPT1, respectively, using sequencing by Sanger method. Mutation in F12 gene was identified in 96 individuals (24 index patients and 72 relatives). In all these patients, the c.983C>A mutation was found. 88 subjects were negative for this mutation. The results were 100% concordant between the two methods. Seventy-one of the 96 patients positive for the c.983C>A mutation were female; 78.9% and 56% were symptomatic female and male patients, respectively. The allelic discrimination presented cost and execution time 79.7% and 82.3% lower in comparison to sequencing by Sanger, respectively. Other mutations in F12 gene, and mutations in PLG and ANGPT1 were not found. Among patients who were negative for mutations associated to HAE with normal C1-INH, 11 were diagnosed as unknown HAE and 16 as idiopathic nonhistaminergic acquired angioedema. The results of present study allowed us to construct an algorithm for diagnosis of patients with HAE with normal C1-INH. In areas where the c.983C>A mutation in F12 is predominant among patients with FXII-HAE, allelic discrimination may be a suitable method for initial screening. In patients with negative results, sequencing of F12 exon 9 by the Sanger method would be indicated. Remaining patients with negative results would be genotyped for mutations previously described in PLG and ANGPT1. Future studies at other sites will be needed to establish whether allelic discrimination would be more cost-effective, with potential for more accessibility to diagnosis in patients with the disease in Brazil
