Background: The unhealthy lifestyle is growing and this can have repercussions on health status demanding actions on the occurrence of diseases and leads to increased expenses., Objective: To examine the interrelationship between the costs of medicine use and lifestyle behaviors., Methods: A cohort study with 118 participants, age around 51.7 ± 7.1 years old. It was collected personal and anthropometric data and information about medicine of continuous use to calculate the costs. Lifestyle variables included habitual physical activity (PA) assessed by pedometer, sedentary behavior by Baecke questionnaire, sleep quality by mini sleep questionnaire and self-report of smoke and alcohol consumption. Statistical analyses were performed by BioEstat (version 5.2) and the significance level set at p-value < 0.05., Results: In 12 months, 62 subjects bought 172 medicines, representing an overall cost of US$ 3,087.01. Expenditures with drugs were negatively related to PA (r = -0.194, p-value = 0.035 and r = -0.281, p-value = 0.002), but positively related with sleep quality (r = 0.299, p-value=0.001 and r = 0.315, p-value = 0.001) and age (r = 0.274, p-value = 0.003). Four multivariate models were executed considering lifestyle behaviors in different moments of cohort and medicine costs, and all these models identify important relationship between lifestyle behaviors with expenditures with drugs., Conclusion: Worse sleep quality seems to increase the costs related to medicine use in adults, while obesity and ageing play a relevant role in this phenomenon and alcohol consumption seems a variable with relevant economic impact.

Objective: To assess the prevalence and factors associated with cost-related underuse of medications in a nationally representative sample of Brazilians aged 50 years and over., Methods: Among the 9,412 participants of the Brazilian Longitudinal Study of Aging (ELSI-Brazil), 6,014 reported using at least one medication on regular basis and were included in the analysis. Underuse of medications was by stopping taking or reducing the number of tablets or the dose of any prescribed medication for financial reasons. The theoretical framework used for the selection of the exploratory variables included predisposing factors, enabling factors, and factors of need. Associations were tested by Poisson regression., Results: The prevalence of underuse of medications was 10.6%. After adjustments for relevant covariables, positive and statistically significant associations (p < 0.05) with the outcome were found for females [prevalence ratio (PR) = 1.39], sufficiency of the family income for expenses (PR = 1.74 for sometimes and PR 2.42 for never), frequency with which the physician explains about the disease and treatment (PR = 1.31 for rarely or never), number of medications used (PR = 1.39 for 2-4 and 1.53 for 5 or more), fair (PR = 2.02) and poor or very poor self-rated health (PR = 2.92), and a previous medical diagnosis of depression (PR = 1.69). Negative associations were observed for the age groups of 60-79 years (PR = 0.75) and 80 years and over (PR = 0.43), socioeconomic status of the household (PR = 0.70, 0.79, and 0.60 for the second, third, and fourth quartile, respectively), and private health plan coverage (PR = 0.79). There were no associations between hypertension and self-reported diabetes and underuse of medications., Conclusions: Cost-related underuse of medications is multidimensional and complex, and it covers socio-demographic characteristics, health conditions, and the use of health services. The explanation about the disease and its treatment to the patient and the expansion of the universal access to pharmaceutical care can minimize the risks of underuse.

This study analyzed time trends in the purchase of medicines by the Rio de Janeiro Municipal Health Department, Brazil, from 2002 to 2011, analyzing data on total annual amounts and unit prices. Annual expenditure per inhabitant and expenditures on medicines as a proportion of total municipal health spending were calculated. Expenditures were adjusted according to the Expanded Consumer Price Index to compare them to 2011. Medicines were classified according to the predominant place of use (in-hospital or primary care) and the WHO Anatomical Therapeutic Chemical classification. Adjusted total expenditures totaled some BRL 575 million, with a decline over the period. Antimicrobials and medicines for the cardiovascular and nervous systems accounted for 55% of total expenditures. There was a shift from in-hospital spending on medicines to those used predominantly in primary care. The analysis can contribute to healthcare management and emphasizes the need to reorient the municipality's policy for medicines.

The scope of this study is to analyze the determinants of the use of appetite suppressants (amfepramone, femproporex, mazindol and sibutramine) through the estimation of a dynamic panel dataset model for the Brazilian state capitals and the Federal District (DF) in the period from 2009 to 2011. The results show that consumption of appetite suppressants did not follow the geographic distribution of overweight and obese individuals across the capitals and DF. There is a recurrent consumption of appetite inhibitors, in which 79% of the current consumption of these drugs is explained by past consumption. Among the variables that explain the use of inhibitors, the percentage of obese adults, the percentage of adults who habitually consume fruit and vegetables, and the coverage rate of health plans stand out. The pharmaco-econometric analysis suggests that there are problems in the rational use of appetite suppressants in the Brazilian state capitals and the Federal District with respect to both the combined consumption of these drugs with other medicines - deemed illegal by the Federal Council of Medicine and ANVISA - and in the therapeutic prescription of these products.

Introduction: The growth of oral antidiabetic drugs spending has led to concerns questioning the health gains and benefits for the health system generated by these drugs. This study contributes to answering these questions., Material and Methods: Initially we present estimates of the three variables central to the analysis: 1) the prevalence of treated diabetes per year and health region, based on the consumption of oral antidiabetic drugs, 2) the number of hospitalization episodes and their costs attributable to diabetes, based on the relative risks and 3) a quantitative characterization of the consumption of oral antidiabetics by calculating average vintages by year and region. Through the use of observations for 10 years and five regions, a total of 50 observations,it was possible to estimate an econometric model explaining statistically both hospitalizations and hospital costs attributable to diabetes by regional characteristics, treated prevalence and the average vintage of oral antidiabetic drugs., Results: The results of the multiple regression models show that hospital costs are proportional to the treated prevalence, all else constant, but that the more recent the oral antidiabetic drugs used, the lower are hospital costs. The effects on the number of hospitalizations are similar., Discussion and Conclusions: For an average observation in the sample, if the average vintage of oral hypoglycemic agents had one additional year, then hospital costs would have a 5.3% reduction (about 11 million € in 2009) and the number of admissions would suffer a 3.8% reduction (about 3965 less episodes in 2009). A counterfactual exercise allows us to estimate that for 2009 the introduction of the class of DPP IV inhibitors reduced the number of hospitalizations attributable to diabetes by 8480 and saved € 23.3 million in hospital costs.

Objective: To describe the expenses of the Ministry of Health of Brazil with users of High-Cost Drug Program that began treatment between 2000-2004, according to their demographic and clinical characteristics., Methods: We made a probabilistic-deterministic linkage of national databases of drugs and mortality, resulting in a historical cohort of patients using high-cost medications in 2000-2004. The per capita spending on medicines were stratified by a follow-up period and described according to demographic, clinical and type of drug used., Results: The total population atended by the program was 611,419, being 63.5% female, average age 46 years. 41.9% of patients living in the Southeast and 29.7% in the Northeast of Brazil. 24.5% of patients began treatment in 2000, 12.4% in 2001, with increasing trend until 2004. The most prevalent diagnosis referred to the genitourinary system diseases and the most common use of chemical groups were antianemic preparations. 40,941 deaths were detected (6.7% of total). The total expenditure per capita was R$4.794,34. Higher spending per capita was observed in males, aged 47, who lived in the Southeast of Brazil and began treatment in 2000, had diagnoses of infectious and parasitic diseases and used blood substitutes and perfusions solutions., Conclusion: The understanding of the expenses involved subsidizes restructuring actions and scheduling drug programs, also provides information for therapeutic groups which are priorities for analysis., (Copyright © 2011 International Society for Pharmacoeconomics and Outcomes Research (ISPOR). The present article approaches rational drug use (RDU) from the economical point of view. The implementation of RDU implies in costs and involves acquisition of knowledge and behavioral changes of several agents. The difficulties in implementing RDU may be due to shortage problems, information asymmetry, lack of information, uncertain clinical decisions, externalities, time-price, incentives for drug prescribers and dispensers, drug prescriber preferences and marginal utility. Health authorities, among other agencies, must therefore regularize, rationalize and control drug use to minimize inefficiency in pharmaceutical care and to prevent exposing the population to unnecessary health risks.

The aim of this study was to analyze the structure of private drug expenditures by individuals 60 years and older in Belo Horizonte, Minas Gerais, Brazil. The study population consisted of a representative sample of retirees under the National Social Security Institute (INSS) in the city of Belo Horizonte, interviewed through a household survey. Monthly out-of-pocket drug expenditures were calculated, and a drug cost structure analysis was performed according to drug characteristics. 667 elders answered the survey. Mean drug expenditures per month were US$ 38.91. The therapeutic groups representing the majority of drug expenditures were: cardiovascular system (26%), nervous system (24%), and digestive/metabolic system (15%). Considering drug registration categories, brand-name drugs accounted for the majority of expenditures (54%). The results of this study can support policies to improve both access to medicines and overall health conditions for the Brazilian elderly population.

Introduction: Due to their morbidity and mortality, cardiovascular diseases are one of the most significant public health problems in industrialized countries, including Portugal. The appropriate use of lipid-lowering drugs to prevent and treat these diseases is therefore of major and increasing importance in therapy today, in terms of both public health and allocation of financial resources by the health system., Objectives: The present study aims to characterize use and expenditure for lipid-lowering drugs in Portugal, and the impact of current policies on their consumption and cost., Methods: The analysis refers to lipid-lowering drugs prescribed and dispensed to outpatients in mainland Portugal covered by the National Health Service (NHS) between January 1, 1995 and June 30, 2004. The Anatomical Therapeutic Chemical classification index recommended by the World Health Organization was used in order to calculate various utilization and expenditure indicators., Results: The use of lipid-lowering drugs in Portugal showed a large increase in terms of defined daily doses per 1000 inhabitants per day (DID), from 10.21 DID in 1995 to 67.93 DID in 2004, mainly due to increased use of statins (an average annual growth of 34.5%). Expenditure on lipid-lowering drugs reached around 123 million euros in 2003, representing. 5% of total NHS expenditure on outpatient medicines. However, the daily treatment cost of these medicines has decreased, particularly in the case of statins and more markedly following increased availability of generic medicines., Conclusions: The study indicates a change in the pattern of lipid-lowering drug utilization and a more rational use of these medicines with the introduction of statins. The increased use of statins follows changes in clinical guidelines for the treatment of dyslipidemia and suggests a favorable evolution in the number of individuals taking statins, although this has had a significant impact on pharmaceutical expenditure, representing a major expense for patients and the NHS. In conclusion, the measures studied for expenditure containment have been successful in the case of statins, with significant development of the generics market (31% market share in 2004), reduction of costs (35% between 1995 and 2004) and hence increased access of patients to such therapy.
