4. Palliative care referral in an internal medicine ward

Abstract

Introdução: Em Portugal, os Cuidados Paliativos (CP) tem vindo a ser desenvolvidos como resposta às necessidades emergentes de uma sociedade envelhecida, aliada ao aumento da prevalência de doenças incuráveis, progressivas e incapacitantes. Objectivo: Caracterizar a referenciação de doentes para unidades de internamento de CP, num serviço de Medicina Interna. Material e Métodos: Estudo observacional, transversal e descritivo, através da análise retrospectiva dos processos clínicos dos doentes referenciados, no nosso serviço, para unidades de internamento de CP, entre Janeiro de 2011 e Junho de 2014. Foram analisadas variáveis socio-demográficas, variáveis clínicas e variáveis de caracterização. A codificação, registo e análise estatística dos dados foi feita em Epi Info™ 7. Resultados: Dos 53 doentes referenciados para unidades de internamento de CP, a maioria (87%) eram doentes oncológicos, do sexo feminino (56,60%), com média de idades de 72 anos, sendo o prestador de cuidados maioritariamente familiar. Os principais motivos de referenciação encontrados foram: o controlo sintomático e a dependência nas actividades de vida diária. O tempo médio de espera para admissão nas referidas unidades foi de 48 dias, sendo que 60,38% dos doentes faleceram antes de serem admitidos. Conclusões: Neste estudo, verificou-se que os doentes são referenciados para unidades de internamento de CP numa fase muito avançada da sua doença e que o tempo de espera é longo, acabando a maioria por falecer antes de serem admitidos. Verificou-se, também, que a maioria dos doentes referenciados para estas unidades tinham patologia oncológica.

Abstract

A Aspergilose é a mais frequente das micoses pulmonares, representando um grupo díspar de doenças de patogénese variável que têm em comum o agente causal - o Aspergillus. Este é saprófita das vias respiratórias, tornando-se patogénico em doentes imunocomprometidos. Existem diferentes formas anatomo-clínicas de Aspergilose, algumas fatais, exigindo um diagnóstico e tratamento atempados. Os autores descrevem um caso de Aspergilose Pulmonar Invasiva num doente submetido a corticoterapia de longa duração. Iniciou terapêutica com Anfotericina B lipossómica, descontinuada por nefrotoxicidade e substituída por Itraconazol, com evolução clínica, laboratorial e radiológica favorável.

Abstract

O objetivo deste trabalho foi avaliar o efeito de quatro épocas de semeadura sobre os teores de óleo e proteínas e sobre a produtividade de grãos de cinco cultivares de soja na região oeste da Bahia. Os ensaios foram conduzidos na Fazenda Maria Gabriela, município de São Desidério, BA, no ano agrícola 2006/2007. O delineamento foi em blocos ao acaso no esquema de parcelas subdividas, em que as parcelas principais foram constituídas pelas épocas de semeadura: primeira época ou Ep1 (29/11/2006), segunda época ou Ep2 (14/12/2006), consideradas preferenciais para a região, terceira época ou Ep3 (28/12/2007) e quarta época ou Ep4 (12/1/2007), sendo as duas últimas consideradas tardias. Em cada época, foram conduzidas cinco cultivares de soja: Msoy 8411 (precoce), BRS Corisco (médio), BRS 263 (precoce), BRS Barreiras (tardio) e Msoy 9350 (tardio), que constituíram as subparcelas, em quatro repetições. Foram determinados a produtividade e os teores de óleo e proteínas nos grãos. Os dados coletados foram submetidos à análise de variância, sendo as médias comparadas pelo teste de Tukey a 5% de probabilidade. As épocas de semeadura do período preferencial - Ep1 (29/11/2006) e Ep2 (14/12/2006) - tendem a aumentar o teor de óleo e reduzir os teores de proteína, enquanto as semeaduras tardias - Ep3 (28/12/2006) e Ep4 (12/01/2007) - não foram favoráveis à obtenção de produtividades superiores para a soja no oeste da Bahia. Abstract

Oobjetivo deste trabalho foi avaliar, por meio do acúmulo de matéria seca e do incremento da área foliar, o desempenho de cinco cultivares de soja com diferentes ciclos de maturação recomendados para a região Oeste da Bahia em duas épocas de semeadura. Os ensaios foram instalados no campo experimental da Fundação Bahia, na Fazenda Maria Gabriela, município de São Desidério - BA, no ano agrícola 2006/2007. Utilizou-se o delineamento em blocos casualizados no esquema fatorial 2 x 5 (duas épocas de semeadura: 29/11/2006, 15/12/2006 e cinco cultivares: M-SOY 8411, BRS Corisco, BRS 263 (Diferente), BRS Barreiras e M-SOY 9350) com quatro repetições. Foram realizadas coletas quinzenais de cinco plantas de soja por parcela, a partir de 30 dias após a emergência (DAE) até a maturação plena R , para determinação da matéria seca e da área foliar total da planta.O maior acúmulo de matéria seca e incremento de área foliar das cultivares na primeira época de semeadura a indica como preferencial, dentro do período de semeadura na região Oeste da Bahia, independente da cultivar ou do seu ciclo de maturação. Abstract

Os autores descrevem um caso de um doente com uma neoplasia da bexiga inoperável, internado por um quadro de síncope e hipotensão arterial. Na avaliação do doente é efectuado um ecocardiograma que mostra a existência de um trombo livre ao nível das cavidades direitas e dilatação das mesmas que levou à hipótese diagnóstica de embolia pulmonar. Perante a existência de uma neoplasia com hemorragia recente, algumas dúvidas terapêuticas surgiram, tendo sido iniciado heparina. Devido a agravamento da situação, com hipertensão pulmonar grave e presença de volumosos trombos ao nível de ambos os ramos da artéria pulmonar visualizados por ecocardiografia transesofágica, o doente acabou por ser submetido a trombólise, embora com algumas alterações ao esquema habitualmente realizado. O doente melhorou, sendo o ecocardiograma final normal.

15. [Detection of subclinical hypothyroidism in pregnant women with different gestational ages].

Abstract

Aim: To detect subclinical hypothyroidism in pregnant women., Subjects and Methods: Seventy-five pregnant women who resided in the town of Itabuna, state of Bahia, were voluntarily studied. Inclusion criteria were age < 40 years, no history of previous thyroid disease, autoimmunopathy or diabetes mellitus, and any gestational age; a clinical evaluation (an interview obeying to a questionnaire); laboratory evaluation (free T4, TSH, anti-TPO antibody, total and HDL cholesterol, triglyceride determinations); thyroid ultrasonography., Results: Average age was 21.6 +/- 5.1 (14-40 years); gestation age was 24.2 +/- 8.2 (5-39 weeks); an elevated TSH with normal free T4 was found in 3 cases (4.0%). Anti-TPO antibodies were positive in 8.0% on the pregnant women. In 5.4% of them, thyroid ultrasonographic changes were documented., Conclusion: Based on finding of a 4% prevalence of elevated TSH during pregnancy, the authors consider important the inclusion of thyroid function laboratory evaluation in the routine prenatal examination. Further studies appear necessary to establish at what gestational age thyroid function evaluation should be started in pregnant women and how frequently it should be repeated during the course of gestation.

Abstract

Magnesium is a predominantly intracellular ion, and it is a cofactor in more than 300 enzymatic reactions, like tyrosinokinase activity. Its deficiency may increase insulin resistance, especially in patients with metabolic syndrome or type 2 diabetes. This study evaluated in 27 patients with poorly controlled type 2 diabetes if there was correlation between intracellular magnesium levels, laboratorial indexes of insulin resistance and glycemic control. Decreased serum and intracellular magnesium depletion were found in 75% and 30.8% of patients, respectively. A negative correlation between intracellular magnesium levels (ICMg) and BMI and HbA1 was found. The homeostasis model assessment for insulin resistance (HOMA-IR) was higher than 3.0 in 59.2% of patients and there was a tendency to negative correlation with ICMg levels, although without statistical significance. Despite the small number of patients, this study shows that magnesium deficiency is frequent in patients with diabetes and its correlation with insulin resistance should be more studied.

Abstract

Introduction: Although the therapeutic effects of GnRH analogues (GnRHa) in central precocious puberty (CPP) have been established, clinical aspects may vary according to racial groups., Objective and Methods: To describe the profile of 175 girls from Bahia, Brazil, treated with GnRHa for idiopathic CPP., Results: The mulatto racial group predominated in the sample. The chronological age at puberty was 6.3 +/- 0.1 years. At diagnosis, height was 2.2 +/- 0.1 SD and body mass index (BMI) was 1.5 +/- 0.1 SD (> 2 SD in 32%), this one, inversely associated with age at onset of puberty (R = -0.20, p = 0.008). At treatment, chronological and bone ages were 8.4 +/- 0.1 years and 10.3 +/- 0.1 years, respectively. At the end of treatment (n = 52) age, BMI and height were 10.6 +/- 0.1 years, 1.6 +/- 0.2 SD and 1.9 +/- 0.2 SD, respectively., Conclusion: Clinical presentation and evolution were similar to other studies, independently of racial characteristics. In this sample, treatment with GnRHa was not associated to significant weight gain.

Abstract

Nutritional disorders (undernutrition and obesity) are associated to increases in morbidity and mortality. This paper consists on a review of literature with the purpose of describing the main methods of nutritional assessment, in order to facilitate the diagnosis of nutritional disorders and the follow-up of dietetics interventions. It describes the clinical method, details procedures of anthropometry and laboratory evaluation and enrolls some of the multiple indices used for diagnosis and prognosis. Information about bioelectric impedance is given. Some aspects of non-conventional methods are reminded. Nutritional status of the Brazilian population is discussed, using the analysis of data collected from three populational surveys (1974, 1989, and 1997).

Abstract

The authors describe a case of a patient with inoperable bladder cancer, who was admitted with syncope and arterial hypotension. After examination, an echocardiogram was requested, which showed a free-floating thrombus in the right atrium and dilatation of the right chambers. A diagnosis of pulmonary embolism was made, and heparin was started. The patient's clinical status worsened, with development of severe pulmonary hypertension due to the presence of large thrombi in the right and left pulmonary arteries. Despite a recent hemorrhagic event related to the bladder cancer, thrombolytic therapy was begun with improvement of the patient. The final echocardiogram was almost normal, without pulmonary hypertension and no dilatation of the right chambers.

Abstract

Aspergillosis, the most frequent of pulmonary mycosis, represents a group of varying pathogenesis diseases with the same etiological agent--Aspergillus. This fungus is a saprophyte of normal respiratory airways, but the disease occurs in immunocompromised hosts. There are several anatomo-clinical forms, but it is important to know that some could be fatal, being necessary an early diagnosis and therapy. The authors describe an Invasive Pulmonary Aspegillosis case, in a immunocompromised patient submitted to long term corticotherapy treatment. Was started with liposomal Amphotericin B, discontinued due to nephrotoxicity and substituted by Itraconazol, with clinical, laboratorial and radiologic favourable evolution, as it's proven by imagiological studies.

21. [Gangrenous cystitis].

Abstract

Gangrenous cystitis is a rare condition, there are no typical symptoms or clinical findings, which hampers diagnosis and may worsen the prognosis. The authors report the case of a diabetic male patient, hospitalized because of poor general condition, dehydration and diffuse abdominal pain. The diagnosis was made by typical pelvic CT findings. Medical treatment must be intensive and begun as soon as possible. As in the present case, the situation may have a favorable outcome.

Abstract

This study aimed at identifying and characterizing the alimentary and nutritional situation of different socio-economic strata of the urban population of Ponte Nova, in Minas Gerais State, Brazil. The sample comprised 161 families stratified according to their earnings in terms of the official minimum wage (salário mínimo--SM), classified in 6 stratum. In addition to socio-economic data, the analysis also takes into consideration the nutritional and caloric adequacy of the ingredients of the basic diet as well as the nutritional state of children up to 6 years old, employing the criteria of Gomez and Seoane-Lathan. The results show that 40% of the sample population earn less than 2 SM, while 12% reach and income level above 10 SM. The population considered has only and indirect relation to production and is employed in the services sector. Access to sanitary services is greater for higher income strata, which also feature better living conditions and health resources. The analysis of the basic diet shows that nutritional adequacy increases with increasing income, reaching more adequate levels only in the highest stratum. The level of perception as to what is "sufficient" in terms of diet supports the results of diet adequacy for each stratum. In so far as the nutritional condition of children is concerned it was observed that the prevalence of malnutrition is consistent with socio-economic condition, decreasing within creased earnings and disappearing altogether above the 10 SM range.
