Trabalho de projeto em Enfermagem Comunitária apresentada na Escola Superior de Saúde do Instituto Politécnico de Viana do Castelo Reconhecida a sua importância no bem-estar e saúde global das pessoas, bem como o seu impacto transversal na sociedade, a evolução das respostas e serviços na área da saúde mental não tem acompanhado o progresso das políticas públicas neste âmbito, persistindo importantes barreiras ao acesso equitativo a respostas adequadas e de qualidade. Importa, assim, o desenvolvimento de projetos comunitários, voltados para as pessoas com doença mental e para as suas reais necessidades e das suas famílias e cuidadores, capazes de os reconhecer como os agentes principais da sua recuperação e integração social. Neste desígnio, assumiu-se como objetivo principal deste Trabalho de Projeto qualificar, numa lógica participada e colaborativa, uma resposta comunitária de intervenção em saúde mental existente no território de Barcelos o Projeto CuiDando. O Projeto CuiDando é um projeto de intervenção comunitária em saúde mental desenvolvido pela Casa de Saúde S. João de Deus em Barcelos que, com dez anos de trabalho desenvolvido com e para as pessoas com doença mental, e sólidos ganhos em saúde demonstrados ao longo do seu percurso, se apresenta hoje muito orientado para a sua operacionalização no imediato, urgindo criar uma visão estratégia que potencie a sua efetividade e impacto e coloque definitivamente os cidadãos no centro dos seus cuidados, potenciando o seu empowerment e recovery, na prossecução da sua verdadeira integração comunitária. Considerando o planeamento em saúde baseado em metodologias participativas estruturante do desenvolvimento de respostas mais integradas e efetivas em saúde pública, assumiu-se este como pressuposto central do projeto. Assim, centrada nas pessoas com doença mental, seus familiares e cuidadores, a qualificação do Projeto Cuidando pretende contribuir para a valorização da multiplicidade de perspetivas e para a participação ativa e efetiva de todos os envolvidos nos processos de diagnóstico, planeamento, ação e avaliação, rumo à real transformação social. Mais, assume-se neste trabalho o planeamento e pensamento estratégicos como base para uma ação estratégica, capaz de considerar a globalidade da cadeia de causas e consequências, ao longo do tempo, do problema social que o projeto CuiDando se propõe resolver, e por isso contribuir para um processo partilhado, dinâmico e interativo e para uma tomada de decisão verdadeiramente intencional, informada e plural. O presente Relatório, em que culmina o percurso desenvolvido para a obtenção do grau de Mestre e Especialista em Enfermagem Comunitária, espelha, assim, o resultado de um plano de ação e intervenção que seguiu os pressupostos da Metodologia do Planeamento em Saúde, com recurso à revisão bibliográfica, análise de orientações estratégicas e de políticas públicas e institucionais e à facilitação de sessões de trabalho com a equipa do projeto, os seus beneficiários e respetivos familiares e parceiros comunitários, na elaboração, participada e colaborativa, de um Documento de Consenso do Projeto CuiDando, consubstanciado pela co-construção do Plano Estratégico que o norteia. A partir da elaboração participada deste Plano, o CuiDando desenvolveu e concretizou de uma forma mais estruturada e precisa o seu modelo de criação de valor, objetivos, metodologias de atuação e resultados esperados, otimizando-os e reformulando-os, e definiu, assim, uma estratégia de consolidação e crescimento a longo prazo, de comunicação e avaliação de impacto, que permitirá potenciar a sua sustentabilidade e impacto social e económico. Paralelamente, crê-se que o desenvolvimento deste Trabalho contribuirá, também, para o reconhecimento e valorização do desenvolvimento profissional dos Enfermeiros, designadamente dos Enfermeiros habilitados com o Mestrado de Enfermagem Comunitária, no seio das organizações da sociedade civil e da economia social, como parte ativa na construção de soluções colaborativas e sustentadas para desafios sociais e globais na área da saúde. Recognized its importance in the well-being and global health of population, as well as its transversal impact on society, the evolution of responses and services in the mental health field of intervention has not kept pace with the progress of public policies in this field, , there are still important barriers to access to an equitable response to adequate and quality responses. Therefor, it is important to develop community projects, aimed at people with mental disorders and their real needs and that of their families and caregivers, capable of recognizing them as the main agents of their recovery and social integration processes. In this context, the main objective of this Project was to qualify, in a participatory and collaborative logic, a community mental health service established in the territory of Barcelos - the CuiDando Project. Saúde S. João de Deus, in Barcelos, which, with ten years of intervention with and for people with mental illness, has shown solid health gains. However, its path, today, is too oriented towards its immediate operationalization, requiring the creation of a strategic vision that enhances its effectiveness and impact and definitively puts the citizen at the center of its intervention, enhancing their empowerment and recovery, in the pursuit of their real community integration. Considering that health planning based on participatory methodologies structures the development of more integrated and effective responses in public health, this was assumed as the central assumption of the project. Thus, centered on people with mental illness, their families and caregivers, the qualification of the Project "CuiDando" aims to contribute to the consideration of the multiplicity of perspectives and to the active and effective participation of all those involved in the processes of diagnosis, planning, action and evaluation, towards real social transformation. Furthermore, this Project assumes strategic planning and startegic thinking as the basis for strategic action, capable of considering the global chain of causes and consequences, over time, of the social problem that Project CuiDando aims to solve, contributing for a shared, dynamic and interactive process and for a intendend, informed and plural decision process. This Report, which represents the culmination of the path developed to obtain the degree of Master and Specialist in Community Nursing, concretizes the result of an action and intervention plan that obeyed the assumptions of the Health Planning Methodology, using bibliographic review, analysis of public guidelines, institutional policies and policies and facilitation of working sessions with the project team, its beneficiaries, their families -construction of the Strategic Plan that guides it.. Based on the participatory elaboration of this Plan, CuiDando developed and defined its model of value proposal, its objectives, operating methodologies and expected results in a more structured, concrete and precise way, optimizing and reformulating them, defining a long-term consolidation and growth strategy, a communication strategy and an impact assessment methodology, which will enhance its sustainability and social and economic impact. At the same time, it is believed that the development of this Project will also contribute to the recognition and appreciation of professional nursing development, namely of qualified nurses with a Master's Degree in Community Nursing, within the scope of civil society and social economy organisations, as an active part in the construction of collaborative and sustained solutions to social and global health challenges.

The execution of public policies, which attain the reduction of the iniquities in oral health, requires, from managers of Brazil’s Health Unified System (SUS), a planning of simplified actions with universal reach, as well as the participation of social actors who are involved in this process. The Community Oral Health Indicator – ICSB aims at consisting in “new technology” in order to obtain data in an easy and simple way that can be able to provide agility to planning organization towards the equity, as well as improving the population’s oral health condition. As a strategy, involving the community health agent (ACS) from the Family Health Program (PSF), the tool (ICSB) was developed to survey the data on oral health epidemiological status of the population. The methodological design consists, initially, in a training workshop with the ACS and a posterior data survey along the population. The dentists involved in the project will be the preceptors of the training workshop for the ACS, in what concerns the identification of oral health problems and their degree of severity. According to the need of treatment, the dentist will develop oral health actions based on the principle of equity.

This work presents discuss on the occupational therapist professional role as a technician acting on the social field. A brief history of occupational therapist acting in this field and the present need for action insertion and projects' development, to which is presented an intervention based on the concepts of autonomy, citizenship and rights. The social field is understood as interdiscipline and intersection sphere which challenges reflexions and methodologies to many professional categories, as occupational therapist, to create and/or increase personal and supporting social nets from people and comunities, the population to whom the actions in the social field are targeted. The purpose is so that implanted directives target the concept of staff work and the build of a net for the social field - whcih structures in different levels of assistance. It works from the concepts of fields and knowledge cores, understanding field as an interdisciplinar activities sphere and the core as a center for specific actions from each professional. The challenge is in the necessary junction between technical and political intervention, tasks presents in the field, adding contributions from different cores. The occupational therapist acts in the field and also bid their specificity from the core of occupational therapy - from the activity and the comprehension of it as path activity, and not 'only' as the end. The occupational therapist´s role in social field is discussed, in this context. O presente trabalho discute o papel profissional do terapeuta ocupacional enquanto técnico em atuação no campo social. Apresenta-se um breve histórico da terapia ocupacional nesta área e a demanda atual de sua inserção e desenvolvimento de projetos, para os quais defende-se uma intervenção calcada nos conceitos de autonomia, cidadania e direito. Compreende-se o campo social enquanto uma área interdisciplinar e intersetorial que pauta para diversas categorias profissionais, dentre elas para os terapeutas ocupacionais, a necessidade de aprimoramento de reflexões e metodologias para que se criem e/ou se fortaleçam as redes pessoais e sociais de suporte de pessoas e comunidades, população-alvo de tais intervenções. Defende-se que as diretrizes implantadas direcionem-se para o conceito de trabalho em equipe e constituição de rede, estruturada em diferentes níveis de assistência. Trabalha-se a partir dos conceitos de campo e núcleos de saberes, entendendo-se o campo enquanto um espaço interdisciplinar de atividades comuns e o núcleo como centro específico de cada profissional. O desafio encontra-se na necessária junção entre intervenção técnica e política, tarefa constante no campo, bem como na soma que redefine as contribuições dos diferentes núcleos presentes. O terapeuta ocupacional intervém no campo e também disponibiliza as especificidades nucleares da terapia ocupacional - a partir do uso da atividade e da mediação para compreensão destas como meio e não 'apenas' fim. Discute-se, nesse contexto, o papel profissional do terapeuta ocupacional no campo social.

INTRODUÇÃO: A disciplina de Atenção Básica em Saúde constitui um programa interdepartamental na Faculdade de Medicina da USP, apoiada por três Unidades Básicas de Saúde com o Programa de Saúde da Família. Este estudo avalia sua instalação. MÉTODOS: Questionários de avaliação de ambiente, Delphi e de auto-avaliação com escala de capacidades. Resultados:Cento e sessenta e dois primeiranistas em 2004 (93%), 142 em 2005 (81%) e 129 terceiranistas (74%) responderam. Orientar a utilização dos serviços de saúde, visitar famílias, promover saúde, oferecer suporte e identificar situações de risco, entre primeiranistas, e "consulta médica com adultos" e "reunião de grupo com pacientes", com alunos do terceiro, alcançaram o nível "fiz o procedimento com ajuda de alguém". Capacidades adquiridas: realidade social e de saúde, trabalho do agente comunitário, entendimento do Programa de Saúde da Família, Atenção Primária e Sistema de Saúde. As percepções de ambiente diferiram entre os locais ou equipes. DISCUSSÃO: As avaliações sugerem razoável conhecimento e capacitação para o trabalho dos agentes comunitários. O vocabulário dos alunos incorporou SUS, PSF e Atenção Básica. CONCLUSÃO: A disciplina alcançou avaliação positiva no seguimento de pessoas, famílias e comunidades. INTRODUCTION: The discipline Basic Care is an interdepartmental program of the Faculty of Medicine of the São Paulo University, supported by three local Basic Care Units offering the Family Health Program. This study evaluates its implementation. METHODS: Questionnaires aimed at evaluation of the environment, Delphi and self-assessment with a competence scale. Results:One hundred and sixty two freshmen in 2004 (93%), 142 in 2005 (81%) and 129 third year students (74%) responded. Among the freshmen, provide orientation about how to use the health services, household visits and health promotion, offer support and identify risk situations, and among the third year students "medical appointments with adults" and "group meetings with patients" reached the level "procedure performed with guidance of somebody". Acquired capacities were sense of the social and health reality, perform Health Agent functions and understand the Family Health Program, Basic Care and Health System. The perception of the environment differed between local Units or Teams. DISCUSSION: The evaluation suggested reasonable capacity for performing the functions of a Health Agent. The terms Health System, Family Health Program and Basic Care were part of the vocabulary of the students. CONCLUSION: The discipline reached a positive evaluation with regard to following-up individuals, families and the community.
