Cardiopulmonary arrest (CRA) is a very serious clinical emergency that can have the ultimate consequence of death. The success of cardiac arrest care is determined by rapid recognition of signs of cardiac arrest and prompt emergency care. This study aimed to verify the role of nurses in the care of cardiorespiratory arrest. This is an integrative review research on the proposed theme, without interference from the researcher in the research environment. Data collection was performed from information collected in the electronic databases LILACS, SCIELO and BDENF. The authors surveyed emphasize that Nursing is responsible for the patient most of the time, so the nurse must observe the main factors that can develop cardiorespiratory arrest and care in hypothermia after CRA. The nurse is important in the face of a CRA, as a professional active in decision-making and able to identify and perform the necessary procedures for the patient's survival. [ABSTRACT FROM AUTHOR]

The extension project named "First Aid for Laypeople - Rescue Lives" (Primeiros Socorros para Leigos - Resgatando Vidas) arises from the perspective of generating proximity to one of the main themes of an imminent threat to life, Cardiopulmonary Arrest (CPA), seeking to guide in its recognition and conduct as a resource capable of increasing victims' chances of survival. Thus, the report aimed to describe the experiences of the project in a high school and at a university scientific event. Between April and December/2019, interventions were carried out through lectures and practical activities. The information presented was collected based on the attendance of the grantee and the project volunteers. In this sense, it was possible to observe evidence that students have little or no knowledge of first aid in the school environment. The commitment of the binders during the project taught them the importance of always being attentive and committed to their social and professional role in health promotion, although they were undergraduate students. Finally, it was verified the positive impact of the extension in the awakening and the modification of behaviors, since the high school students expressed at the end of the interventions that they feel able to share knowledge, as well as know the main behaviors in scenarios of recurrent emergencies outside the hospital environment. 8. Quality of life and performance status after cardiopulmonary resuscitation: A study in Cova da Beira University Hospital Center's intensive care unit.

Introduction and Objectives: Cardiac arrest (CA) is associated with high morbidity and mortality. Many studies focus on survival, but few explore the outcomes. The aim of this study is to analyze the survival curve, independence, quality of life, and performance status after CA., Methods: This retrospective study included adults admitted to the intensive care unit of Cova da Beira University Hospital Center after CA between 2015 and 2019. We analyzed patient records and applied a questionnaire including EuroQoL's EQ-5D-3L and ECOG performance status., Results: Ninety-seven patients were included (mean age 75.74 years). Thirty-one patients (32.0%) survived to hospital discharge. There was a significant loss of independence for activities of daily living, with 50.0% of those previously independent becoming dependent and 47.5% of those previously at home being institutionalized. Diabetes, female gender, and length of hospital stay were especially impactful on these findings. One year after CA, only 20.6% were alive and only 13.4% (65% of the one-year survivors) were independent. Nine patients answered our questionnaire. Mean EQ-5D quality of life index (0.528±0.297) and the most affected domains ('Pain/discomfort' and 'Anxiety/depression') were similar to the Portuguese population aged >30 years. However, 66.6% reported a decline in their quality of life. Lastly, seven respondents had a good performance status (ECOG 0-1)., Conclusions: There was a significant loss of independence after CA. Moreover, despite the acceptable performance status and the quality of life results being similar to the general population, there was a perceived deterioration post-CA. The population and geographic characteristics of each mobile emergency service directly interfere with its effectiveness, and there are several reports that the local actions developed by the service have immediate results that are little known. Thus, the objective was to identify the epidemiology of cases treated by the Mobile Emergency Care Service in the city of Palmas, Tocantins, Brazil. This is a retrospective, documentary research, with a quantitative approach, carried out at the Mobile Emergency Care Service in the municipality of Palmas, with data collected from May to June 2019. 3,037 registration forms for the year 2018 were evaluated. The data from the analyzed records were tabulated in an excel spreadsheet, and descriptive statistics of the data were performed. There was a prevalence of attendance of male victims (53.1%), aged between 19-29 years (19.7%). There was also a prevalence of clinical causes (25.7%), followed by traumatic causes (20.4%). Regarding external causes, there was a prevalence of traffic accidents (11.9%); among the clinical causes, seizures (3.1%), cardiopulmonary arrest (2.3%) and syncope (2.5%) predominated; and in obstetric care, labor prevailed (66.9%). The destination of care was predominant in hospitals. Therefore, it is concluded that most of the emergency cases attended were young male victims whose prevalence of causes varied between clinical and traumatic. It is noteworthy that these data, if included in the agenda of decision makers, can support better and more effective practices of containment and mitigation of recurrent diseases, as well as the co-responsible promotion of health through the proper use of mobile emergency service equipment. To describe the experience of professors and students on nursing protocols in the cardiac arrest of patients with COVID-19. This is a bibliographical research, with a qualitative approach and experience report between teacher and student, through the communication networks involving the use of remote technologies. The databases of the Brazilian Ministry of Health, Virtual Health Library (VHL), MEDLINE, IBECS and LILACS, were used. Data were collected from scientific papers published between July 2015 and July 2020. The workload of the nursing staff is a conditioning factor for mental and physical illness; fear of the pandemic; there are gaps in care delivery and training addressing cardiac arrest. The pandemic context revealed the essential role nurses play in care delivery and the urgency of basic and advanced life support training to improve safety, mitigate fears and prepare nursing workers to provide care in emergencies resulting from SARS-CoV-2. Abstract Background and objectives: The perioperative cardiac arrest (CA) and mortality rates in Brazil, a developing country, are higher than in developed countries. The hypothesis of this review was that knowledge of the epidemiology of perioperative CA and mortality in Brazil enables the comparison with developed countries. The systematic review aimed to verify, in studies conducted in Brazil, the epidemiology of perioperative CA and mortality. Method and results: A search strategy was carried out on different databases (PubMed, EMBASE, SciELO and LILACS) to identify observational studies that reported perioperative CA and/or mortality up to 48 hours postoperatively in Brazil. The primary outcomes were data on epidemiology of perioperative CA and mortality. In 8 Brazilian studies, there was a higher occurrence of perioperative CA and mortality in males; in extremes of age; in patients in worse physical status according to the American Society of Anesthesiologists (ASA); in emergency surgeries; in general anesthesia; and in cardiac, thoracic, vascular, abdominal and neurological surgeries. The patient's disease/condition was the main triggering factor, with sepsis and trauma as the main causes. Conclusions: The epidemiology of both perioperative CA and mortality events reported in Brazilian studies does not show important differences and, in general, is similar to studies in developed countries. However, sepsis represents one of the major causes of perioperative CA and mortality in Brazilian studies, contrasting with studies in developed countries in which sepsis is a secondary cause.

Objective: To verify the knowledge of high school students about cardiopulmonary arrest and cardiopulmonary resuscitation and to evaluate learning immediately and late after theoretical and practical training. Methods: Longitudinal, prospective, quantitative experimental study, carried out in a public and a private institution, using a questionnaire on basic life support. The statistics were descriptive and inferential by the Friedman and Mann Whitney tests. Results: The students had insufficient knowledge, showing a significant improvement immediately and late. The two institutions had an equivalent performance in terms of correct answers in the immediate and late post-training periods. The stop signs, resuscitation maneuvers and steps to use the external defibrillator had lower success rates. Introduction: Community health workers are considered a professional health category in Brazil who are in direct daily contact with community users and, therefore, more likely to face emergency or first aid situations. Objective: To assess the level of knowledge and confidence of community health workers on first aid and cardiopulmonary resuscitation. Materials and Methods: A descriptive quantitative study was conducted between June and October 2018 in 22 Family Health Centers in a city in Ceará, Brazil with 186 community agents grouped as follows: a group that has already received training on these matters and another group that has not had such training. For data collection, a questionnaire containing the following items was administered: 1) sample characterization; 2) questions on cardiopulmonary resuscitation; 4) questions on first aid; 4) confidence scale. Results: A significant difference among both groups was found in questions related to the knowledge of the four items, especially for explaining CPR, checking for responsiveness of an unconscious person, the sequence for placing hands and arms in cardiopulmonary resuscitation (CPR) and rotating resuscitators. Regarding first aid, a statistically significant difference was also found in four items related to first aid questions when attending a victim in an emergency such as convulsions, drowning and choking in children. Both groups showed low confidence to intervene in cardiac, neurological and respiratory emergencies. Conclusions: Research results indicate that there is a lack of knowledge among community health workers to respond effectively to situations involving cardiopulmonary resuscitation and first aid, which is also associated with low confidence to perform interventions in emergencies. Objective: To analyze the scientific evidence on the effectiveness of educational interventions in acquiring knowledge about first aid for the lay population. Methods: Integrative literature review based on the guiding question: "What is the scientific evidence on the use of educational interventions to acquire knowledge about first aid among the lay population?". The search was carried out in nine databases. Results: The review sample consisted of ten studies. There was a predominance of studies in 2017, published mainly in Brazil with a target audience formed by school staff, teachers and students. The main educational interventions were: health education through a training program, lecture, conversation circle, theoretical-practical class. The main subjects covered were: fractures, cardiopulmonary arrest, hemorrhage, seizure and burn. Objective: to evaluate the knowledge of nursing students about the care of patients in cardiorespiratory arrest. Results: the students who positively evaluated themselves showed a good level of knowledge about the theme (r=0.29; p=0.001), as well as those who had attended the first aid course in previous periods (p=0.012). The questions they made mistakes were those regarding the routes of administration of the drug and ventilatory support in non-intubated patients. Conclusion: the nursing students demonstrated an adequate level of understanding of the protocols on the care of patients in cardiorespiratory arrest and had a theoretical framework obtained in the discipline of first aid, as well as in improvement courses. Death still causes people in general with varied mental states, among them disbelief, hopelessness, spiritual anguish, mental-moral-spiritual suffering, fear, anxiety, chronic sadness. In this context and considering the experience of the dying and death process by nursing professionals, spirituality and care are approaches used in the therapeutic sphere to prevent, interrupt and overcome possible psychopathological conditions in the process of dying and in the face of death. Chico Xavier's bibliographic and mediumistic collection has a particular aspect of message books, attributed to dead family members. Who are the communicating family spirits, what is the quantity of these communications, data from the communicators, in which works were published and what are the possible psychopathological pictures of fathers and mothers who made them seek spiritual and mediumistic help? OBJECTIVE: - to systematize the works of family letters, received mediumship by Chico Xavier, between 1951 and 2012; - explain the possible psychopathological conditions of fathers and mothers and their overcoming after receiving the psychographed letters. METHOD: the research has a historical approach, the source is documentary, the data collection technique is bibliographic, the analysis technique is quantitative. RESULTS: a) 74 thematic books published between 1951 and 2012, 550 communicating family spirits, 1726 written mediumistic letters, 61 communicating spirits excluded due to lack of registered biographical data. b) the greatest number of causes of death is an automobile and motorcycle accident, murder with a firearm, drowning, cardiac arrest and suicide. CONCLUSION: The works psychographed by Chico Xavier, referring to family letters from the disincarnated to the incarnated, according to the unanimous testimonies of the family members, are antidotes for the reduction or cessation of uncertainties and possible psychopathological suffering during the process of dying and in the face of death. Objective: To report the experience of training caregivers of patients with chronic renal failure about basic life support based on the problematization methodology. Methods: It is an experience report of a training developed with 15 caregivers of patients with Chronic Renal Failure in June 2018. To meet the problematization method, we proposed to use the Arco de Manguerez through four stages: observation of reality, survey of key points, theorization and application to reality. Results: Strategies such as conversation wheel, semi-structured interview, training with practical simulation of cardiopulmonary arrest recognition and cardiopulmonary resuscitation maneuvers were used, with subsequent evaluation of the activity developed. The training provided the dissemination of essential knowledge for people who manage the care of patients with chronic diseases, such as Chronic Renal Failure, who can progress to cardiac arrest. Conclusion: The problematization made it possible for caregivers to participate in the educational process as active subjects, extrapolating the condition of spectators. [ABSTRACT FROM AUTHOR]

Objective: Objective: To verify if the clinical simulation strategy improves the knowledge and self-confidence gain of the healthcare team in situations of cardiopulmonary arrest (CPR) and/or foreign body airway obstruction (OVACE) in primary health care (APS). Methodology: Quantitative, quasi-experimental study, developed in basic health units. Sample composed of 68 PHC health professionals. A semi-structured questionnaire and self-confidence scale were applied. Descriptive analysis was performed and inferential analysis was considered significant with p≤0.05. Results: A significant gain of knowledge (p<0.001) and self-confidence in assisting in CRP and OVACE situations was identified by the nursing staff (p=0.001) and community agents (p=0.005), after the use of simulation as a strategy for teaching. Conclusion: The simulation showed a positive effect to gain knowledge and self-confidence for health professionals, including those who do not participate directly in care, such as community health agents. Objective: to know the nurse's perception about the admission process of children in the immediate postoperative period of renal transplantation. Method: Descriptive, qualitative study, carried out from January to March, 2019 by means of the focal group technique for data collection. Participants were 13 nurses who worked in the high complexity postoperative unit of a referral hospital in transplants. The testimonies were recorded, transcribed and organized into categories. To analyze the data, the content analysis technique was used. Results: The analysis of the empirical material revealed the following thematic categories: Nurses' perception about the process of admission of children in the immediate postoperative period; Creation of a protocol in the postoperative period of renal transplantation aimed at children; Professional insecurities related to volume replacement and cardiorespiratory arrest. Conclusion: The study contributed to the situational diagnosis regarding the care provided by nurses, highlighting: volume replacement, vital data, mechanical ventilation parameters and drug dilution. Objective: to analyze the scientific production on cardiorespiratory arrest in Brazilian nursing journals. Method: bibliometric study carried out through electronic access to the websites of the 14 Brazilian nursing journals, classified in the Sucupira Platform, for analysis of articles published between 2010 and 2019. Results: 18,266 articles were analyzed, of which 50 were selected to integrate the sample. It was possible to observe that 50% of the articles were published in Qualis A extract journals and 80% of the productions were derived from the Southeast Region. Regarding the type of study, 54% were descriptive and 64% quantitative. Regarding the most investigated theme, 28% portrayed the knowledge of nurses and 22% contemplated nursing training on cardiorespiratory arrest. Conclusion: the scientific production of Brazilian journals showed a predominance of articles published in Qualis A journals, from the Southeast Region, descriptive, quantitative, which predominantly investigated the knowledge and training of professionals. 30. Extracorporeal mechanical support and aspiration thrombectomy in treatment of massive pulmonary embolism: a case report

RESUMO A embolia pulmonar maciça aguda é a forma mais grave de tromboembolismo venoso, que pode causar choque obstrutivo e levar à parada cardíaca e morte. Neste relato de caso, os autores apresentam o caso de uma mulher de 49 anos que se recuperou com sucesso de uma embolia pulmonar maciça com o uso combinado de oxigenação por membrana extracorpórea venoarterial e trombectomia por aspiração pulmonar, sem complicações decorrentes desses procedimentos. Embora a evidência de benefício do suporte mecânico não tenha sido estabelecida em pacientes com embolia pulmonar maciça, a implementação de suporte cardiocirculatório extracorpóreo durante a reanimação pode permitir a melhora da perfusão sistêmica dos órgãos e mais chances de sobrevida. Diretrizes recentes da European Society of Cardiology afirmam que a oxigenação por membrana extracorpórea venoarterial combinada com tratamento por cateter pode ser considerada em pacientes que apresentem embolia pulmonar maciça e parada cardíaca refratária. O uso de oxigenação por membrana extracorpórea como técnica autônoma com anticoagulação permanece controverso. Deve-se considerar outras terapias, como trombectomia cirúrgica ou percutânea. Como essa intervenção não é sustentada por estudos de alta qualidade, acreditamos ser importante relatar casos concretos de sucesso. Com este relato de caso, ilustramos o benefício derivado da reanimação assistida por suporte mecânico extracorpóreo e trombectomia por aspiração precoce em pacientes com embolia pulmonar maciça. Além disso, enfatizamos a sinergia que deriva de sistemas multidisciplinares integrados para fornecer intervenções complexas, sendo exemplos notáveis a oxigenação por membrana extracorpórea e a Cardiologia Intervencionista. ABSTRACT Acute massive pulmonary embolism is the most serious presentation of venous thromboembolism that can ultimately cause obstructive shock, leading to cardiac arrest and death. In this case report, the authors present a case of a 49-year-old female who successfully recovered from a massive pulmonary embolism with the combined use of venoarterial extracorporeal membrane oxygenation and pulmonary aspiration thrombectomy, with no complications from these procedures. Although evidence of benefit from mechanical support has not been established for patients with massive pulmonary embolism, the implementation of extracorporeal cardiocirculatory support during resuscitation may allow improvement of systemic organ perfusion and better chance of survival. Recent guidelines from the European Society of Cardiology state that venoarterial extracorporeal membrane oxygenation in combination with catheter-directed treatment may be considered for patients presenting with massive pulmonary embolism and refractory cardiac arrest. The use of extracorporeal membrane oxygenation as a stand-alone technique with anticoagulation remains controversial, and additional therapies, such as surgical or percutaneous embolectomy, must be considered. Since this intervention is not supported by high-quality studies, we believe it is important to report real-world successful cases. With this case report, we illustrate the benefit derived from resuscitation assisted by extracorporeal mechanical support and early aspiration thrombectomy in patients with massive pulmonary embolism. Additionally, it emphasizes the synergy that derives from integrated multidisciplinary systems for providing complex interventions, of which extracorporeal membrane oxygenation and Interventional Cardiology are clear examples.

- Abstract
The cardiorespiratory arrest is one of the main causes of death in the world's population. It is an event that yearly affects about 200,000 victims in na out-of-hospital and inhospital environment in Brazil. It is characterized by cardiac and pulmonary abnormal functions, where the organs stop receiving oxygen necessary for their vital maintenance. The study aim was to identify the difficulties found by the nursing team regarding a cardiorespiratory arrest according to the literature. It is about an integrative reading considering the articles published in The Virtual Health Library (VHL), in the databases of LILACS, BDENF and SciELO, through the descriptors: Cardiac arrest, Cardiopulmonary resuscitation and nursing care, in the period from 2013 to 2017. The analysis of the material collected resulted in the construction of two categories described below: difficulties of theoretical knowledge in cardiopulmonary resuscitation and the lack in continuous training in cardiopulmonary resuscitation. It is concluded that there is still a need to invest in permanent education programs in order to collaborate with the theoretical knowledge of the professionals, being these factors interconnected and that directly influence the quality of care offered. Cardiopulmonary arrest (CPA) is an event that causes the sudden interruption of cardiovascular activity, as well as the respiratory process, in which this stage presents the absence of respiratory movements, consciousness and heart rate. The study aims to analyze through the literature the strategies used to prevent cardiorespiratory arrest in the hospital environment. The study was carried out through an integrative review (IR). In this sense, two thematic categories were established in the study approach. The category entitled “strategies for the prevention of cardiorespiratory arrest in the hospital environment” presents preventive measures to avoid this clinical condition and reduce mortality, since CRA is most of the time irreversible, and the proposals for interventions are fundamental for the resoluteness. assistance, as well as for the promotion of health problems. The second category entitled “importance of the multidisciplinary team for the quality of care during cardiorespiratory arrest”, shows the resoluteness of teamwork for holistic care. Thus, the study contributed to new scientific evidence on the magnitude of this topic, despite the comprehensiveness in the literature, few studies answered the research objective. El paro cardiopulmonar (PCA) es un evento que provoca la interrupción súbita de la actividad cardiovascular, así como del proceso respiratorio, en el que esta etapa presenta ausencia de movimientos respiratorios, conciencia y frecuencia cardíaca. El estudio tiene como objetivo analizar a través de la literatura las estrategias utilizadas para la prevención de la parada cardiorrespiratoria en el ámbito hospitalario. El estudio se llevó a cabo a través de una revisión integradora (RI). En este sentido, se establecieron dos categorías temáticas en el abordaje del estudio. La categoría titulada “estrategias para la prevención de la parada cardiorrespiratoria en el ámbito hospitalario” presenta medidas preventivas para evitar esta condición clínica y reducir la mortalidad, ya que la PCR es la mayoría de las veces irreversible, y las propuestas de intervención son fundamentales para la resolución asistencial, así como para la promoción de problemas de salud. La segunda categoría, titulada “Importancia del equipo multidisciplinario para la calidad de la atención durante la parada cardiorrespiratoria”, muestra la determinación del trabajo en equipo para la atención integral. Así, el estudio contribuyó a nuevas evidencias científicas sobre la magnitud de este tema, a pesar de la exhaustividad en la literatura, pocos estudios respondieron al objetivo de la investigación. A Parada Cardiorrespiratória (PCR) é um evento que ocasiona a interrupção súbita da atividade cardiovascular, bem como do processo respiratório, no qual esse estágio apresenta a ausência de movimentos respiratórios, da consciência e frequência cárdica. O estudo tem por objetivo analisar por meio da literatura as estratégias utilizadas para prevenção da parada cardiorrespiratória no ambiente hospitalar. O estudo foi realizado por meio de uma revisão integrativa (RI). Nesse sentido, foram estabelecidas duas categorias temáticas na abordagem do estudo. A categoria intitulada “estratégias para a prevenção da parada cardiorrespiratória no ambiente hospitalar” apresenta medidas de prevenção para evitar essa condição clínica e reduzir a mortalidade, uma vez que a PCR na maioria das vezes é irreversível, sendo as propostas de intervenções fundamentais para a resolutividade da assistência, bem como para a promoção de agravos de em saúde. A segunda categoria intitulada “importância da equipe multiprofissional para a qualidade da assistência durante a parada cardiorrespiratória”, mostra a resolutividade do trabalho em equipe para a assistência holística. Deste modo, o estudo contribuiu para novas evidências científicas sobre a magnitude desse tema, apesar da abrangência na literatura, poucos estudos responderam o objetivo da pesquisa.

to know the adverse effects and complications experienced by the health team and cognize the nursing care performed during the application of therapeutic hypothermia. Methodology: this research was developed with a qualitative approach, conducted in 2014, with semi-structured interviews with health workers from Intensive Care Units of two hospitals in the extreme south of Brazil, which applied therapeutic hypothermia after cardiorespiratory arrest. The textual discursive analysis was used. Results: Adverse effects included tremors, bradycardia, hypotension and complications such as skin burns. Nursing care is mainly dedicated to skin and extremity care, ice use, sedation and comfort. Conclusion: Therapeutic hypothermia is possible to be applied, in the reality of the researched institutions, in a safe, effective and with low cost way, however, it is necessary to carry out the qualification of the teams, in order to clarify doubts and favor adherence and the necessary care to be provided. Objective: to evaluate the knowledge of the specialist in the training of Cardiology and Hemodynamics in emergency cardiac arrest, according to the new guidelines of the American Heart Association - 2015. Methodology: descriptive / exploratory study, with a quantitative approach. Data collection was performed between March and April 2017, through the application of a structured questionnaire, the final sample being composed of 25 post-graduate students obtained for convenience. Data were analyzed using simple descriptive statistics. Results: of the sample of 25 participants, 20 (80%) had knowledge to identify a cardiorespiratory arrest and 21 (84%) indicated the correct behavior after the detection of this event, 20 (80%) reported having already taken courses on the subject and 23 (92%) sought better information on the subject in the literature. Conclusion: the postgraduate students have sufficient knowledge about the proposed topic, presenting an interest in the technical-scientific qualification. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the effectiveness of rapid response teams using early identification of clinical deterioration in reducing the occurrence of in-hospital mortality and cardiorespiratory arrest. Data sources: The MEDLINE, LILACS, Cochrane Library, Center for Reviews and Dissemination databases were searched. Study selection: We included studies that evaluated the effectiveness of rapid response teams in adult hospital units, published in English, Portuguese, or Spanish, from 2000 to 2016; systematic reviews, clinical trials, cohort studies, and prepost ecological studies were eligible for inclusion. The quality of studies was independently assessed by two researchers using the Newcastle-Ottawa, modified Jadad, and Assessment of Multiple Systematic Reviews scales. Data extractions: The results were synthesized and tabulated. When risk measures were reported by the authors of the included studies, we estimated effectiveness as 1-RR or 1-OR. In pre-post studies, we estimated effectiveness as the percent decrease in rates following the intervention. Results: Overall, 278 studies were identified, 256 of which were excluded after abstract evaluation, and two of which were excluded after full text evaluation. In the meta-analysis of the studies reporting mortality data, we calculated a risk ratio of 0.85 (95%CI 0.76 - 0.94); and for studies reporting cardiac arrest data the estimated risk ratio was 0.65 (95%CI 0.49 - 0.87). Evidence was assessed as low quality due to the high heterogeneity and risk of bias in primary studies. Conclusion: We conclude that rapid response teams may reduce in-hospital mortality and cardiac arrests, although the quality of evidence for both outcomes is low. [ABSTRACT FROM AUTHOR]

The objective of the study was to analyze the understanding of the nursing team of the basic health care network in the care of the victims in CRP. Method used integrative literature review. The results show that survival with CRP is related to a series of fundamental interventions characterized by the immediate recognition of PCR signals, request for help, immediate onset of CPR maneuvers, and that there is a need to update the team about the Guidelines for cardiopulmonary resuscitation and the standardization of nursing behaviors in the care of these victims. Conclusion: With the reduced number of scientific publications, the low production and understanding of these professionals on the process of attending to the victims in CRP is already evident. It should be noted that the broad incorporation of this subject into the curricula of all levels of education is necessary, especially a broader approach in technical and undergraduate courses. This study continues to support this research, supporting the need to invest in the training of nursing staff since its action is directly linked to the direct care of these patients to better elucidate the issue. Objective: This study aimed to identify and examine studies on CPR-IC and its main conducts. Method: It was a scoping review study conducted according to the recommendations of The Joanna Briggs Institute, in the MEDLINE, CINAHL, Scopus and Web of Science databases. Results: Fourteen studies were included in the review. All selected studies were published in English, between 2009 and 2017, mostly from Europe and North America. Evidence described CPR-IC as an under-recognized event by health professionals, however, when it occurs, the main conducts performed by the multidisciplinary team include interruptions of cardiopulmonary resuscitation to check the pulse and physical restraint to continue resuscitation maneuvers and sedation. Nevertheless, it was observed that there is no consensus among specialists in managing this phenomenon. Conclusion: The studies indicate the need to standardize care management guidelines and conduct guidance. Objetivo: Identificar y examinar estudios sobre RCP-CI y principales conductas. Método: Estudio de revisión de alcance realizado según las recomendaciones de The Joanna Briggs Institute, en las bases de datos MEDLINE, CINAHL, SCOPUS y Web of Science. Resultados: Catorce estudios fueron incluidos en la revisión, estos fueron publicados en inglés, entre 2009 y 2017, distribuidos principalmente en Europa y América del Norte. Las evidencias describen la RCP-CI como evento poco reconocido por profesionales de la salud, sin embargo, cuando ocurre, las principales acciones realizadas por el equipo multidisciplinario son: interrupciones de la reanimación cardiopulmonar para controlar el pulso, restricción física para continuar con las maniobras de reanimación y sedación. Introduction: Cardiorespiratory arrest in pregnant women is a rare event, however, it is essential that health professionals have adequate knowledge of how to perform the maneuvers if it happens, since the pregnant woman has some particularities that are different from other adults. Objective: To emphasize the importance of correctly performing cardiopulmonary resuscitation maneuvers in pregnant women Methodology: A search was carried out in the Virtual Health Library (BVS) in January 2022. health sciences (DeCS) Pregnancy; cardiorespiratory arrest; Cardiopulmonary resuscitation. The "and" baller was used and the following criteria were adopted for the selection of articles: complete materials, in Portuguese, in article format and with a time frame of ten years (2012-2022). 43. Parada cardíaca após peridural para cirurgia plástica estética: relato de caso.

- Abstract
Resumo A parada cardíaca durante anestesia neuroaxial é um evento adverso grave, que pode ocasionar sequelas neurológicas importantes e morte se não tratada em tempo hábil. Os mecanismos associados são insuficiência respiratória negligenciada, bloqueio simpático extenso, toxicidade por anestésicos locais, raquianestesia total, além da crescente consciência da predominância vagal como fator predisponente. No caso reportado, a paciente tinha 25 anos e estado físico ASA I e foi programada para lipoplastia estética. Após sedação com midazolam e fentanil, foi feita anestesia peridural nos interespaços T12‐L1 e T2‐T3 e inserção de cateter na punção inferior. A paciente foi mantida em decúbito dorsal horizontal durante 10 minutos. Em seguida, foi posicionada em decúbito ventral, evoluiu com parada cardíaca em assistolia 20 minutos após o bloqueio do neuroeixo. A equipe médica imediatamente colocou a paciente em decúbito dorsal e iniciou as manobras de ressuscitação cardiorrespiratória. O retorno da circulação espontânea foi obtido após 20 minutos de reanimação. É discutida neste relato a resposta vagal exacerbada como principal mecanismo causal do evento. O sucesso do desfecho da paciente em questão ressalta a importância da vigilância do anestesiologista, e do pronto reconhecimento e tratamento de mudanças de ritmo no eletrocardiograma. Cardiac arrest during neuraxial anesthesia is a serious adverse event, which may lead to significant neurological damage and death if not treated promptly. The associated mechanisms are neglected respiratory failure, extensive sympathetic block, local anaesthetic toxicity, total spinal block, in addition to the growing awareness of the vagal predominance as a predisposing factor. In the case reported, the patient was 25 years old, ASA I , scheduled for aesthetic lipoplasty. After sedation with midazolam and fentany, epidural anesthesia in interspaces T12‐L1 and T2‐T3 and catheter insertion into inferior puncture were performed. The patient remained in the supine position for 10 minutes. Then, she was placed in the prone position, developing asystolic cardiac arrest 20 minutes after the completion of neuraxial blockade. The medical team immediately placed the patient in the supine position and began cardiopulmonary resuscitation. Spontaneous circulation was achieved after twenty minutes of resuscitation. We discuss in this report the exacerbated vagal response as the main event mechanism. The patient's successful outcome emphasizes the importance of anaesthetic monitoring by anesthesiologists, prompt recognition and treatment of rhythm changes on the electrocardiogram. The objective of this descriptive and cross-sectional study was to assess the theoretical knowledge of the nurses in a hospital in the state of Minas Gerais regarding the basic life support used in attending cardiorespiratory arrest. The sample was constituted by 16 nurses, and an instrument constructed based on the recommendations of the American Heart Association for cardio-pulmonary resuscitation was used for data collection. The results showed gaps in the knowledge on the rhythms identified in cardio-respiratory arrest, the sequence of steps, number of cycles versus ventilation, opening of the airways, where to place the defibrillator electrode pads, the procedure to be undertaken immediately after the shock, and the loads, in joules, recommended for the defibrillation. The re-assessment and continuous training of the nurses is recommended, for their adequate performance in treating cardiorespiratory arrest, contributing to the population’s survival.

Objective: To identify the main problems encountered by nursing professionals when performing the necessary procedures in CPA. Methodology: This is an integrative literature review, based on the Latin American and Caribbean Literature on Health Sciences (LILACS), the Scientific Electronic Library Online (SCIELO) and PubMed, covered by the Virtual Library in Health (VHL). Articles written in Portuguese and English, published between 2012 and 2021, were used. Results: A total of 2,347 articles were found, after methodological rigor through the inclusion and exclusion criteria, 13 articles were selected, 7 in LILACS, 4 in PubMed and 2 in SCIELO. The studies highlighted the importance of training aimed at teams to obtain a quick response to a CPA. Conclusion: In view of the study, it was possible to observe that the cardiopulmonary resuscitation (CPR) maneuver is effective and reduces the level of mortality if performed quickly, and correctly following the necessary technique step-by-step. It is worth emphasizing the importance of the nurse in the rapid identification and implementation of the CPR protocol, thus preserving the quality of the procedure. Objetivo: Identificar los principales problemas que encuentran los profesionales de enfermería al realizar los procedimientos necesarios en la CPA. Metodología: Se trata de una revisión bibliográfica integradora, basada en la Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS), la Biblioteca Científica Electrónica en Línea (SCIELO) y PubMed, cubierta por la Biblioteca Virtual en Salud (BVS). Se utilizaron artículos redactados en portugués e inglés, publicados entre 2012 y 2021. Resultados: Se encontraron un total de 2.347 artículos, luego del rigor metodológico a través de los criterios de inclusión y exclusión, se seleccionaron 13 artículos, 7 en LILACS, 4 en PubMed y 2 en SCIELO. Los estudios destacaron la importancia de la formación dirigida a los equipos para obtener una respuesta rápida a un CPA. Conclusão: Diante do estudo, foi possível observar que a manobra de ressuscitação cardiopulmonar (RCP) é eficaz e reduz o nível de mortalidade se feita com agilidade, e seguindo corretamente o passo a passo da técnica necessária. Valendo ressaltar a importância do enfermeiro na identificação rápida e execução com o protocolo de RCP, preservando desta forma a qualidade do procedimento.

Objective: Discuss the importance of knowledge of nursing professionals facing a situation of cardiac arrest in the emergency department. Methods: This is an integrative literature review, carried out in the Virtual Health Library database, using the descriptors "Knowledge", "Nursing", "Emergency Nursing", "Emergency" and "Heart Arrest" combined with each other through of the Boolean operators "AND" and "OR", covering studies published in the last 5 years (2016-2021), available in full and that answered the guiding question: How the knowledge of nursing professionals implies in their performance in relation to cardiopulmonary arrest in Emergency? Results: Based on the inclusion and exclusion criteria, a final sample of 18 articles was obtained, of which 21.6% corresponding to descriptive study articles, with a predominance of studies published in 2017. Conclusion: It is noticed that constant updates for these professionals are important, contributing to the recognition of the main signs and symptoms of a CPA, performing necessary interventions and qualified maneuvers without interfering with the patient's health. These types of updates can occur through ongoing education projects aimed at the development, autonomy and appreciation of nurses. Objetivo: Discutir la importancia del conocimiento de los profesionales de enfermería ante una situación de paro cardíaco en urgencia. Metodología: Se trata de una revisión integradora de la literatura, realizada en la base de datos de la Biblioteca Virtual en Salud, utilizando los descriptores "Conocimiento", "Enfermería", "Enfermería de Emergencia", "Emergencia" y "Paro Cardiaco" combinados entre ellos a través de los operadores booleanos " Y "y" O ", que abarcan estudios publicados en los últimos 5 años (2016-2021), disponibles en su totalidad y que respondieron a la pregunta orientadora: ¿Cómo implica el conocimiento de los profesionales de enfermería en su desempeño ante la parada cardiopulmonar en el ¿Emergencia? Resultados: A partir de los criterios de inclusión y exclusión se obtuvo una muestra final de 18 artículos, de los cuales el 21,6% corresponden a artículos de estudio descriptivo, con predominio de estudios publicados en 2017. Conclusión: Se percibe entonces que la actualización constante de estos profesionales es importante contribuyendo al reconocimiento de los principales signos y síntomas de un CPA, realizando las intervenciones necesarias y maniobras calificadas sin interferir con la salud del paciente. Este tipo de actualizaciones pueden ocurrir a través de proyectos de educación continua orientados al desarrollo, la autonomía y la valoración del enfermero. Metodologia: Trata-se de uma revisão integrativa de literatura, realizada nas base de dados da Biblioteca Virtual em Saúde, utilizando os descritores “Conhecimento”, “Enfermagem”, “Enfermagem em Emergência”, “Emergência” e “Parada Cardíaca” combinados entre si através dos operadores booleanos “AND” e “OR”, abrangendo estudos publicados nos últimos 5 anos (2016-2021), disponíveis na íntegra e que respondessem a questão norteadora: Como o conhecimento dos profissionais de Enfermagem implica na sua atuação frente à parada cardiorrespiratória na Emergência? Resultados: A partir dos critérios de inclusão e exclusão, obteve-se uma amostra final de 18 artigos dos quais 21,6% correspondem a artigos do tipo estudo descritivos, tendo predominância estudos publicados no ano de 2017. Conclusão: Percebe-se então que as atualizações constantes para os estes profissionais são importantes contribuindo no reconhecimento dos principais sinais e sintomas de uma PCR, realizando intervenções necessárias e manobras qualificadas sem interferir na saúde do paciente. Estes tipos de atualizações podem ocorrer através de projetos de educação permanente visando o desenvolvimento, autonomia e valorização dos enfermeiros.

Abordaram-se pontos críticos das diretrizes do International Liaison Committee on Resuscitation (ILCOR). RESULTADOS: Foram avaliados 213 profissionais (76 enfermeiros, 35,7%; 38 auxiliares, 17,8%; e 99 técnicos, 46,7%). JUSTIFICATIVA E OBJETIVOS: A parada cardiorrespiratória (PCR) ocorrida em ambulatório tem elevada mortalidade, sendo a sobrevida entre 5% e 35%. Dos pacientes que são reanimados uma percentagem elevada permanece com déficits neurológicos, resultantes das lesões ocorridas, tanto no período de ausência de circulação ou durante a reperfusão. No entanto a compreensão dos mecanismos da lesão cerebral não tem traduzido na melhoria do prognóstico. A hipotermia terapêutica após a reanimação parece ser uma opção válida associada à diminuição destas seqüelas neurológicas. O objetivo deste estudo foi rever a evidência científica relativa à abordagem do paciente reanimado após PCR. CONTEÚDO: Descrição e abordagem dos principais fatores de risco associados à lesão neurológica após PCR, bem como dos seus critérios de prognóstico.Feita pesquisa não sistemática na base de dados PubMed dos artigos referentes à abordagem terapêutica dos pacientes reanimados de parada cardíaca. As referências bibliográficas dos artigos de revisão foram igualmente analisadas. Elaboradas normas práticas para essa abordagem. CONCLUSÕES: Os pacientes que sobrevivem à PCR têm elevado risco de permanecer com lesões neurológicas graves. A hipotermia terapêutica e o controle das variáveis fisiológicas, com otimização da perfusão cerebral, podem melhorar o seu prognóstico.BACKGROUND AND OBJECTIVES: Out-of-hospital cardiac arrest is a major cause of death with survival rates as low as 5% to 35%. A large number of patients who survive resuscitation will face significant neurological damage, as a result of the ischemia that occurs both during cardiac arrest and reperfusion. However understanding of the mechanisms responsible for brain damage has not resulted in prognostic improvement. Therapeutic hypothermia after resuscitation may be a valid option associated to reduction of neurological damage. The purpose of this study was to review scientific evidence related to a therapy for patients resuscitated from cardiac arrest. CONTENTS: Description and analysis of the main risk factors associated with neurological damage after resuscitation from cardiac arrest as well as prognostic criteria was carried out. A non-systematic search was conducted in the PubMed data base for papers on a therapeutic approach for patients resuscitated from cardiac arrest. Bibliographic references of reviewed papers were also analyzed. Practical rules were drafted for such an approach. CONCLUSIONS: Patients resuscitated from cardiac arrest face a high level of risk of neurological damage. Therapeutic hypothermia and control of physiological parameters to optimise brain perfusion, may improve prognosis.

Trabalho de Projeto de Mestrado em Enfermagem Médico-Cirúrgica apresentada na Escola Superior de Saúde do Instituto Politécnico de Viana do Castelo As doenças cardiovasculares representam a maior causa de morte na Europa, sendo responsáveis por mais de 3,9 milhões de mortes por ano, o que representa 45% de todas as mortes. Por comparação, as doenças oncológicas são responsáveis por aproximadamente 1,1 milhões de mortes por ano na Europa, o que simboliza 24% das mortes (Wilkins [et al.], 2017). As paragens cardiorrespiratórias em contexto extra-hospitalar, são responsáveis por aproximadamente 350 000 vítimas na Europa, mas quando essas vítimas têm acesso a desfibrilhação precoce, as suas possibilidades de sobrevivência duplicam. Contudo, apenas 4% das vítimas têm acesso a esta desfibrilhação precoce (Jarrah, Judeh &AbuRuz, 2018). Sanghavi [et al.] (2015) referem que os serviços de emergência nos Estados Unidos da América, anualmente, socorrem 380000 vítimas em paragem cardiorrespiratória em contexto extra-hospitalar, sendo que mais de 90% dessas pessoas chegam ao hospital sem sinais de vida. Para minimizar o impacto da paragem cardiorrespiratória em ambiente extra-hospitalar é consensual que é necessário um início precoce de suporte básico de vida com desfibrilhação automática externa. A redução dos tempos de deslocação dos meios de emergência até ao local é uma solução apresentada, mas atendendo à relação custoefetividade, esta estratégia é inviável. Assim, sugere-se que mais pessoas sejam capazes de realizar suporte básico de vida com desfibrilhação automática externa, sendo necessário dotar a comunidade de competências através de programas de formação (Ong, Perkins, Cariou, 2018) Este estudo permitiu construir e implementar um programa de formação em suporte básico de vida com desfibrilhação automática externa, na comunidade, por forma a dar resposta às necessidades da mesma perante uma vítima em paragem cardiorrespiratória. Adotou-se a metodologia de trabalho de projeto, recorrendo à experiência in loco do investigador, à realização de entrevistas não estruturadas a informantes privilegiados, à análise documental de dados fornecidos pelo Instituto Nacional de Emergência Médica, e à aplicação de um questionário a elementos integrantes do trabalho como instrumentos de diagnóstico. A prossecução desta metodologia permitiu corroborar a importância do desenvolvimento deste estudo, tendo sido desenvolvidas várias atividades para capacitar a comunidade com competências teóricas e práticas para fazer face ao problema com que nos deparamos. Os resultados obtidos demonstram-nos que uma grande percentagem da comunidade em que nos inserimos não detém conhecimento acerca de suporte básico de vida com desfibrilhação automática externa, e que programas de formação como este são capazes de preparar a população em geral para socorrer vítimas de paragem cardiorrespiratória, contribuindo para a diminuição da taxa de mortalidade e morbilidade associadas a este evento. Cardiovascular diseases are the leading cause of death in Europe, being responsible for more than 3.9 million deaths per year, representing 45% of all deaths. By comparison, cancer diseases are responsible for approximately 1.1 million deaths per year in Europe, which represents 24% of the deaths (Wilkins [ et al.] 2017). Cardiorespiratory arrests in an extra-hospital context are responsible for approximately 350 000 victims in Europe, but when these victims have access to early defibrillation, their chances of survival double. However, only 4% of victims have access to this early defibrillation (Jarrah, Judeh & AbuRuz, 2018). Sanghavi [ et al.] (2015) refer that emergency services in the United States of America, annually, succor 380,000 victims in cardiorespiratory arrest extra-hospital, more than 90% of these people arrive at the hospital without life signs. To minimize the impact of cardiorespiratory arrest in an extra-hospital environment, it is consensual that an early start of basic life support with automatic external defibrillation is necessary. The reduction of the travel time of emergency services to the location is a solution presented, but given the cost-effectiveness, this strategy is impracticable. Thus, it is suggested that more people can perform basic life support with automatic external defibrillation, and it is necessary to endow the community with skills through training programs (Ong, Perkins, Cariou, 2018). This study allowed create and implement a training program in basic life support with automatic external defibrillation, in the community, in order to respond to their needs when having to assist a victim in cardiorespiratory arrest. It was adopted the project work methodology, using the researcher's on-site experience, the conduction of unstructured interviews with privileged informants, document analysis of data provided by the National Institute of Medical Emergency and the application of a questionnaire to integrant elements of the work as diagnostic tools. The pursuit of this methodology allowed to corroborate the importance of this study, having been developed several activities to empower the community with theoretical and practical skills to face the problem we are coming across. The results obtained show us that a large percentage of the community in which we are integrated does not have knowledge about basic life support with automatic external defibrillation, and that training programs like this can prepare the general population to help victims of cardiorespiratory arrest, contributing to the decrease in the mortality and morbidity rate associated with this event.

Introduction: Cardiorespiratory arrest (CPA) is called the absence of heartbeat and apnea. This condition is classified as a serious clinical condition that is increasingly incident in Intensive Care Units, for this reason, health professionals need to be prepared to act in this context. Objective: To analyze cases of cardiac arrest in different care sectors. Methodology: A search was carried out in the Virtual Health Library (VHL) in July 2021. The search for materials that contained in their abstract the descriptors in health sciences (DeCS) was started Nurse; Cardiac arrest; Intensive care unit. The "and" rounding tool was used and the following criteria were adopted for the selection of articles: complete materials, in Portuguese, in article format and with a ten-year time frame (2011-2021). Results: Three articles were analyzed and, based on a thorough reading, the category with the theme "Assignments and challenges encountered by nursing professionals at the time of CPR" was created. Final Considerations: The daily problems experienced by professionals nurses in the assistance to victims of CPA are several and among them, the lack of qualification of workers, the lack of investments in permanent education and the lack of fast, standardized and high-quality support is accentuated. are conditions that pose a high danger of obstacles and even death. Introducción: El paro cardiorrespiratorio (CPA) se denomina ausencia de latidos cardíacos y apnea. Esta condición está catalogada como una condición clínica grave y cada vez más incidente en las Unidades de Cuidados Intensivos, por lo que los profesionales de la salud deben estar preparados para actuar en este contexto. Objetivo: Analizar casos de paro cardíaco en diferentes sectores asistenciales. Metodología: Se realizó una búsqueda en la Biblioteca Virtual en Salud (BVS) en julio de 2021. Se inició la búsqueda de materiales que contenían en su resumen los descriptores en ciencias de la salud (DeCS) Enfermera; Paro cardiaco; Unidad de terapia intensiva. Se utilizó la herramienta de redondeo "y" y se adoptaron los siguientes criterios para la selección de artículos: materiales completos, en portugués, en formato de artículo y con un plazo de diez años (2011-2021). Resultados: Se analizaron tres artículos y, a partir de una lectura exhaustiva, se creó la categoría con el tema "Asignaciones y desafíos que enfrentan los profesionales de enfermería en el momento de la RCP". Consideraciones finales: Son varios los problemas cotidianos que experimentan los profesionales de enfermería en la atención a las víctimas de CPA y entre ellos se encuentra la falta de calificación de los trabajadores, la falta de inversiones en educación permanente y la falta de un apoyo rápido, estandarizado y de alta calidad. acentuado., son condiciones que plantean un alto peligro de obstáculos e incluso la muerte. Objetivo: Analisar os casos de parada cardiorrespiratória em diferentes setores da assistência. Metodologia: Realizada busca pela Biblioteca Virtual de Saúde (BVS), no mês de julho de 2021. Iniciou-se a busca de materiais que contivessem em seu resumo os descritores em ciências da saúde (DeCS) Enfermeiro; Parada cardíaca; Unidade de Terapia Intensiva. Foi usado o boleador "and" e adotados os seguintes critérios para seleção dos artigos: materiais completos, em língua portuguesa, no formato de artigo e com recorte temporal de dez anos (2011-2021). Resultados: Foram analisados três artigos e a partir da leitura minuciosa foi elaborada a categoria com a temática “Atribuições e desafios encontrados por profissionais de enfermagem no momento da RCP”. Considerações Finais: Os problemas diários vividos por profissionais enfermeiros na assistência às vítimas de PCR são vários e entre eles acentua-se a escassez de qualificação dos trabalhadores, a carência de investimentos em educação permanente e a falta de suporte rápido, padronizado e de alta qualidade, são condições que possuem um alto perigo de obstáculos e até morte.
