OBJECTIVE: To study echocardiographic parameters of left ventricular systolic function and valvar regurgitation under pharmacological influence in mildly symptomatic patients with chronic mitral regurgitation (MR). METHODS: We carried out a double-blind placebo controlled study in 12 patients with MR, mean aged 12.5 years old, who were randomized in 4 phases: A) digoxin; B) enalapril; C) digoxin + enalapril; D) placebo. The medication was administered for 30 days in each phase, and the following variables were analyzed: shortening and ejection fractions, wall stress index of left ventricle, left ventricular meridional end-systolic wall stress, Doppler-derived mean rate of left ventricular pressure rise (mean dP/dt), stroke volume and MR jet area. The clinical variables analysed were heart rate and systemic arterial pressure. RESULTS: No significant variation was observed in the clinical variables analysed. The shortening and ejection fraction, the mean dP/dt and stroke volume significantly increased and the wall stress index of left ventricle, the meridional left ventricular end systolic wall stress and the mitral regurgitation jet area decreased in the phases with medication as compared with that in the placebo phase. CONCLUSION: The parameters of left ventricular systolic function improved significantly and the degree of MR decreased with the isolated administration of digoxin or enalapril in mildly symptomatic patients with chronic MR. The combination of the drugs, however, did not show better results.

OBJECTIVE: To identify the left inferior pulmonary vein as an indirect marker of increased pulmonary flow in congenital heart diseases.METHODS: We carried out a prospective consecutive study on 40 patients divided into 2 groups as follows: G1 - 20 patients diagnosed with congenital heart disease and increased pulmonary flow; G2 (control group) - 20 patients who were either healthy or had congenital heart disease with decreased or normal pulmonary flow. We obtained the velocity-time integral of the left inferior pulmonary vein flow, excluding the "reverse A" wave, with pulsed Doppler echocardiography.RESULTS: In G1, 19 out of the 20 patients had well-identified dilation of the left inferior pulmonary vein. No G2 patient had dilation of the left inferior pulmonary vein. Dilation of the left inferior pulmonary vein in conditions of increased pulmonary flow had sensitivity of 95%, specificity of 100%, positive predictive value of 100%, and negative predictive value of 95% (1 false-negative case). The integral of time and velocity of the pulmonary venous flow obtained with pulsed Doppler echocardiography was greater in the G1 patients (G1=25.0±4.6 cm versus G2=14.8±2.1 cm, p=0.0001).CONCLUSION: The identification of dilation of the left inferior pulmonary vein suggests the presence of congenital heart disease with increased pulmonary flow. This may be used as an indirect sign of increased flow, mainly in malformations of difficult diagnosis, such as atrial septal defects of the venous sinus or coronary sinus type.

OBJETIVO: Avaliar as possibilidades diagnósticas e terapêuticas, através de microcateteres introduzidos nas veias do coração, em estudo angiográfico da circulação venosa coronária. MÉTODOS: Foram realizadas venografias superseletivas do coração em 30 portadores de taquicardia ventricular sustentada, utilizando-se cateter específico que providenciava a oclusão temporária do óstio do seio coronário. A idade média foi de 52,7 (variação de 24 a 76) anos, sendo 25 do sexo masculino e cinco do feminino. As veias foram analisadas de acordo com o número, calibre e distribuição nas paredes anterior e posterior do ventrículo esquerdo (VE). RESULTADOS: O seio coronário foi cateterizado em todos os pacientes, não havendo desconforto adicional ou complicações em nenhum caso. O número de veias da parede posterior do ventrículo esquerdo foi 3,1 e na parede anterior, 1,9 pPURPOSE: To study the angiographic anatomy of human coronary veins and the possibility of epicardial venous mapping through microelectrode catheters. METHODS: We evaluated 30 patients with sustained ventricular tachycardia using a catheter which provided occlusion of the coronary sinus ostium during venous angiography. They were 25 males, 5 females, ages ranging from 24 to 76 years (mean=52.7). The veins were studied according to their number, caliber and distribution in the anterior and posterior wall of the left ventricle. RESULTS: Coronary sinus was catheterized in all patients. Objectives: To compare the morbidity and mortality of patients with and without metabolic syndrome (MS) (according to the First Brazilian Guidelines for Diagnosis and Treatment of MS) undergoing coronary artery bypass graft (CABG) during hospitalization and after 30 days., Methods: A total of 107 patients who underwent CABG with no associated procedures were assessed between August and October 2005; 74 (69.2%) of them had MS. Criteria of in-hospital outcome were: acute atrial fibrillation (AF), prolonged mechanical ventilation (MV), need for transfusion of blood products, number of hours spent in the ICU, length of hospital stay (days), respiratory and surgical wound infection, and death. For 30 days, the combination of events such as the need for readmission to hospital, surgical wound infection and death was assessed., Results: The mean age was 60.6 +/- 9.7 years, and the group with MS presented a higher body mass index and body surface, and a higher prevalence of diabetes. No statistically significant difference was observed in the occurrence of in-hospital events such as AF (15.2% vs. 16.2%), MV (3.0% vs. 2.7%), number of units of blood products used (3.2 +/- 2.7 vs. 2.6 +/- 2.9), length of ICU stay (53.7 +/- 27.3 vs. 58.9 +/- 56.5), length of hospital stay (9.2 +/- 8.7 x 8.5 +/- 8.5), respiratory infection (6.1% vs. 2.7%), surgical wound infection (3.0% vs. 5.4%), and mortality rate (3.0% vs. 0.0%). The occurrence of combined events in 30 days was similar in both groups (12.2% vs. 20.3%, p = NS)., Conclusion: Patients with MS did not have increased morbidity and mortality when undergoing CABG, however they had a higher trend of occurrence of surgical wound infection.

In a previous study, ticlopidine decreased the parasitemia and mortality of mice infected by Trypanosoma cruzi. Therefore, this drug was administered to 12 patients with Chagas' disease, in the chronic phase. For 90 days, 150, 200 or 250 mg were utilized according to whether the recipients were children, adolescents or adults, respectively. A fully unsuccessful outcome was documented, both serologically as well as parasitologically.

Patients in the chronic phase of Chagas' disease and receiving corticoid because of concommitant diseases were treated with benznidazole, which was initiated at the same time of the use of corticoid in a group of 12 patients or 15 days afterwards in 6 patients. It has been proved in another paper that in the chronic phase of Chagas' disease corticoid use is associated with increased parasitemia, as evaluated by xenodiagnosis. In this study benznidazole use prevented this increase, and we suggest that in immunocompromised patients with chronic Chagas' disease the use of this drug could be useful.

The microsporidia have been involved in several clinical manifestations in patients with AIDS, of whom diarrhoea is the commonest. The diagnosis of microsporidiasis depended on invasive procedures and the identification of the organisms is made by electron microscopy. The modified trichrome staining method allows that the diagnosis be made without such procedures by using light microscopy. In the present work, the modified trchrome method was applied in stools from 62 patients with diarrhoea, who had asymptomatic HIV infection or AIDS. Of the 62 samples analyzed, there was detection of microsporidial spores in one. This work confirms the existence of such protozoans in our patients, associated with manifestations of chronic diarrhoea in patients with AIDS who have severe immunodeficiency and ascertains that this staining method allows satisfactory identification of microsporidia from faeces, as well points out some directions to further studies.

The authors report a case of a 89 years-old woman with mucocutaneous leishmaniasis and previous diabetes mellitus and high blood pressure, who had been treated with allopurinol for 10 months without healing of lesions. Afterwards, she has been treated with meglumine antimonate, "glucantime" for 4 days, with a total dose 2,380 mg of Sbv, but developed cardiac side effects and hypokalemia, hence the treatment was withdrawn. However, this patient developed total clinical regression of lesions, in spite of she has been received low dose of this drug.

We report a patient with chronic asymptomatic Chagas' disease that presented Trypanosoma cruzi reactivation after kidney transplantation and immune depression. The only clinical manifestation of the disease was ulcerative skin lesions, which is unusual in Chagas' disease.

Ten patients with mucosal lesions caused by American tegumental leishmaniasis were treated with pentamidine isethionate at the dose 4 mg/kg on alternate days by the intravenous route. The mean posology was 2,140 mg. Healing of the lesions occurred in 9 (90%) of the patients who completed treatment. There was no recurrence during a follow-up time of 1 to 24 months (mean, 7,7 months). One patient discontinued treatment before healing of the lesion because be developed diabetes mellitus. In 3 (30%) patients, blood exams showed increased urea and creatinine levels and leucopenia, which were corrected by increasing the interval between administrations of the drug. Pentamidine isethionate is efficient in bringing about cicatrization of the lesions but needs further evaluation in terms of its value in preventing recurrence.

The diagnosis of ocular toxoplasmosis is based primarily on clinical presentation and laboratory tests described else where. Eventually, unusual clinical presentation, as well unusual pathologic findings occur, mainly in immunosuppressed individuals. The authors report a case of a female patient from Paraná with a progressive ocular disease, for at least three years, without definite diagnosis. After an exhaustive clinical and laboratory investigation, as well empiric medical treatment, without success, it was decided to enucleate the atrofic organ and to proceed to a histologic examination. It revealed a cronic and inspecific inflammatory process without any specific agent. However the immunohistochemical technique was able to detect Toxoplasma gondii antigens in the ocular tissue. This technique provides an alternative method for the diagnosis of unusual forms of toxoplasmosis.

The authors report a case of mucosal leishmaniasis, in which tracheomalacia and respiratory insufficiency occurred. The nature of the illness, the patient's general condition and the great extension of the affected tracheal segment made segmental resection and anastomosis inviable. The silicone T tube, which proved itself useful, was the choice, thus motivating this report of the procedure.

Health professionals (HP) are frequently exposed to accidents with materials contaminated with blood and/or body fluids, thus representing a population at risk for the acquisition of human immunodeficiency virus (HIV) and hepatitis B virus (HBV). PURPOSE--To determine the prevalence of HIV and HBV infection among HP admitted to the University Hospital, Faculty of Medicine, University of São Paulo (UH-FMUSP), and to identify the possible sources of contamination. METHODS--A total of 345 HP who sequentially passed the hiring examination at UH-FMUSP from March 1 to June 23 1993 were evaluated and compared to 2521 blood donors using serologic tests for the detection of AgHBs and total anti-HBc of HBV. The ELISA method was also used for HIV-1, with subsequent confirmation by Western blot. Questionnaires were distributed to the HP for the identification of possible sources of contamination. RESULTS--Positivity to HIV-1 was 0.48% for HP and 0.23% for blood donors, and positivity to HBV was 8.89% and 6.17%, respectively. CONCLUSION--There was no statistically significant difference in the results of the serologic tests for HIV and HBV detection between the two groups, even taking into consideration the different professions of those who work in the health area and their potential exposure to blood and secretion.

We report the second published case of human dirofilariasis acquired in Brazil. The patient had two pulmonary coin lesions seen in conventional chest roentgenogram and confirmed as solid round nodules by computerized X-ray chest scan. Diagnosis was done by surgical removal of one of the lesions and anatomopathological study of tissue removed.

We report a case of Plasmodium falciparum malaria in a 21 day old child that was born from a mother that developed clinical signs of falciparum malaria just after giving birth and died. Transmission could have occurred either by the transplacental route or by maternal blood reaching the child during the childbirth. Literature review shows that malaria should be considered as a cause of a febrile disease in the newborn, particularly if there is a low birthweight and the newborn is a child of a woman coming from an endemic malaria region.
