Introduction: The National Scientific Congress (NSC) is an annual event held in Peru in which medical students from all over the country present their research work. Objectives: To determine the publication rate in indexed scientific journals of research papers submitted to the NSCs held between 2010 and 2014, as well as the factors associated with their publication, and to describe the characteristics of the published papers. Materials and methods: Retrospective study in which 407 abstracts of research papers were reviewed. A publication time window of 6 years (from submission) was considered. A bivariate analysis was performed to assess differences between categorical and numerical variables using the chi-square and Mann-Whitney U tests, respectively. In addition, a multivariate analysis was performed using Poisson regression models (a crude and an adjusted model), calculating relative risk (RR) values with their respective 95% confidence intervals (95%CI) to determine the factors associated with the publication of papers. The paper reports the experience of Supervised Internship in Community Social Psychology from the State University of Piauí (UESPI) performed in a Basic Health Unit of Teresina, Piauí. The objectives of the internship were: insert themselves in everyday community, collect and interpret data from this reality, rise possible demands of work and develop intervention activities aimed at education and health promotion. There were home visits, counseling, referrals and especially playful activities with children in the waiting room. The creation of educational and preventive spaces in the community through popular education contributed to improving the quality of services and allowed a holistic and differentiated children who attend this service attention. Health promotion that subjects provided an opportunity to express their own perceptions of reality, with the prospect of linking health and living conditions, sharing the cultural, economic, psychological, social and environmental situations that have a serious impact on the health- disease process.

The modern science influence in the health field is reproduced mainly at the upbringing and the daily basis of the health professionals, this is translated in the ways of social inequality and requiresa change in the upbringing processes. This review study indicates ponderations, directresses and guidelines that allow the critical production of knowledge, tuning the dialogue of epistemology andmethodology formalizations. For the upbringing of independent individuals there is an argumentation that researches that criticize reality is a condition for the upbringing, allowing a dialogic formation in the health field. The objectives were to reflect about researches as a way to criticize reality, upbringing of independent individuals and health practice reconstruction; to indicate guidelines for scientific production in graduation and post graduation schools. The use of guidelines and regulations for scientific papers production, in the light of epistemology andacademic formalization dialogue, enables the comprehension of theoretical possibilities, ethics, politics and science technique.

11. Anthropometric indicators of abdominal obesity: review of the papers indexed on SciELO electronic library

A systematic review of original articles published between 1993 and 2007 and indexed in SciELO (Scientific Electronic Library Online) was carried out to identify the anthropometric indicators used to assess abdominal obesity in different target subgroups, and to provide an overview of available evidence regarding the relationship between these anthropometric indicators and obesity-associated comorbidities. The articles were analyzed and the following data were collected:year of publication, sample size and age, anthropometric measure used to detect abdominal obesity, and main results of the study. The review comprised 47 articles which met all inclusion criteria, most of them published over the last 5 years. Waist-hip ratio and waist circumference were used as indicators of abdominal obesity in 34 and 11 of the articles reviewed, respectively. With respect to age range, the samples were heterogeneous in most of the studies (n=27) and ten included adolescents. Four studies were conducted on elderly subjects and three included children, but none exclusively involved adolescents as the target population. The sample size was larger than 500 in 26 studies. Despite disagreement in terms of the degree of correlation, the studies generally showed that the anthropometric indicators of abdominal obesity were correlated with hypertension, diabetes, dyslipidemias, and coronary heart risk. The results of this review suggest that abdominal obesity is associated with metabolic diseases. However, there is a lack of studies involving the adolescent population, suggesting the need for further research in this area.

13. A methodological proposal to research patients’ demands and pre-test probabilities using paper forms in primary care settings

Objective: The purpose of this study is to present a methodology for assessing patients’ demands and calculating pre-test probabilities using paper forms in Primary Care. Method: Most developing countries do not use Electronic Health Records (EHR) in primary care settings. This makes it difficult to access information regarding what occurs within the health center working process. Basically, there are two methodologies to assess patients’ demands and problems or diagnosis stated by doctors. The first is based on single attendance at each appointment, while the second is based on episodes of care; the latter deals with each problem in a longitudinal manner. The methodology developed in this article followed the approach of confronting the ‘reason for the appointment’ and ‘the problem registered’ by doctors. Paper forms were developed taking this concept as central. All appointments were classified by the International Classification of Primary Care (ICPC). Discussion: Even in paper form, confrontation between ‘reason for the appointment’ and ‘problem registered’ is useful for measuring the pre-test probabilities of each problem-based appointment. This approach can be easily reproduced in any health center and enables a better understanding of population profile. Prevalence of many illnesses and diseases are not known in each reality, and studies conducted in other settings, such as secondary and tertiary care, are not adequate for primary health care. Conclusion: This study offers adequate technology for primary health care workers that have potential to transform each health center into a research-led practice, contributing directly to patient care.

Abstract Introduction: Brazilian scientific production has shown substantial growth and achieved international visibility. However, in general, the participation of women in scientific activities remains limited. Objective: This study aimed to evaluate the indicators of scientific productivity of women fellows of the Council for Scientific and Technological Development (CNPq) in the field of Medicine. Method: This cross-sectional study was conducted on 541 (211 women, 39%) researchers registered as recipients of CNPq research productivity (PQ) scholarships in Medicine according to a list provided in December 2022. Results: There was a predominance of male researchers (n=330; 61%). In both the male and female groups, most researchers were at level 2, with 62.5% women and 47.2% men (p=0.018). All 211 female PQ scholars were distributed among 37 different institutions and published 34,969 papers in scientific journals, averaging 165.7 articles per researcher. In the last five years of the study period (from 2018 to 2022), 9,679 papers were published. Over their careers, the 211 researchers supervised 5,440 undergraduate research students, 4,144 master’s degree students, and 2,923 PhD candidates. 17. Prevalence of Active Play in Brazilian children and adolescents: an updated systematic review for the Brazil's Report Card

This paper is an update of the systematic review on Active Play published in 2018 by Mendes et al. This systematic review included studies published between 2018 and 2019. The search for potential articles was performed on the following electronic databases: Pubmed/Medline, Web of Science, Bireme, Scielo and Scopus. Initially, 471 papers met the eligibility criteria. However, after deeply analyzed, only two studies remained and were included in the present review. From these articles only one presented individual information on play activity among adolescents age (12-17 years) from Curitiba-PR and it reveals that 77,2% of them were physically active in their free time. The other study included was about the preferences for leisure activities among adolescents from Florianopolis-SC participants in two cross sectional studies in 2001 and 2011. It showed a significant reduction in the preferences for practicing physical activities, while the opposite occurs with sedentary activities like watching TV, playing video games, and using computers. These results along with the inconsistencies in benchmarks of the domain active play among countries involved in the Global Matrix 3.0 made clear the necessity of a standard definition and a proper tool to measure it.

18. Sedentary behavior in children and adolescents: an update of the systematic review of the Brazil’s Report Card

This review updated data on sedentary behavior in Brazilian children and adolescents for the Brazil’s Report Card 4.0. The searching was carried out in eight databases (PubMed, Scopus, Web of Science, LILACS, SPORTDiscus, BIREME, Scielo, and Google Scholar), based on the the following criteria: original research; samples including Brazilian children and adolescents; to be a observational survey with the prevalence of at least one component of sedentary behavior. In this updated review were included 118 studies (corresponding to 159 papers), being 71 studies (104 papers) previously reviewed in the Report Card 3.0 and 47 studies (55 papers) found in update from 2018 to 2019. Screen time (34.7%) and TV viewing (28.2%) remains the most investigated components, however, two studies investigated cell phone use, and there was an increase in other types of sedentary behavior such as sitting time (from 9% to 25.6%). We found only four studies involving pre-scholars, but four of them covered almost all age groups. Self-reported questionnaire was the instrument more used; however, increased the studies using accelerometers (from 2 to 8 studies). The cut-off point more frequent was 2 hours/day (47.5%), but the use of other measures doubled. Almost 70% of the studies reported that less than 50% (general range: 9.4% to 97.7%) of individuals had < 2 hours/day of sedentary behavior. The updated review found few studies with prescholars and children; using validated instruments; using accelerometers, with standardization of cutoff points, and prevalences very close to what was observed in the previous review.

A odontologia legal é uma especialidade imprescindível para soluções de casos jurídicos por negligência ou imprudência no ramo da odontologia, em processos éticos e pode estar relacionada a atuação da busca pela identidade biológica, como identificação de corpos não identificados com auxílio de documentos odontológicos. O objetivo deste trabalho foi conhecer, através de revisão da literatura, a atuação do cirurgião-dentista especialista em Odontologia Legal dentro de sua capacidade na identificação humana e utilização da documentação odontológica, bem como negligência na publicidade e exposição inadequada de paciente. Trata-se de uma revisão da literatura artigos científicos dos últimos 10 anos. Sobre a odontologia legal e documentação odontológica, foi observado que a atuação do dentista elevou os conhecimentos específicos da equipe, melhorando o desdobramento dos casos, e toda documentação de origem odontológica foi muito importante para chegar a conclusões e definições de laudos periciais, possibilitando que a corpos não identificados fosse dado, então, sua real identidade e processos jurídicos fossem esclarecidos. Conclui-se que a atuação da odontologia legal dá um acréscimo bastante expressivo nos resultados dos quais participam, e toda equipe de forense deveria contar com um dentista para alavancar seus conhecimentos e dar mais resultados na busca pela verdade e identidade de cadáver. THE IMPORTANCE OF FORENSIC ODONTOLOGISTS AND DENTAL RECORDS TO FORENSIC AND LEGAL RESOLUTIONS ABSTRACT Forensic Dentistry is an important dental science to legal investigations of negligence or recklessness cases in ethical processes and can be related to the search for biological identity, such as identification of unidentified bodies with the aid of dental records. This paper aimed to understand, through literature review, the performance of dental surgeons in Forensic Dentistry in terms of her/his capacity for human identification and use of dental records, as well as negligence in the publicity and inadequate exposition of the patient. It is a literature review of scientific papers from the last 10 years. It was used the database LILACS SciELO, by means of the descriptors “Forensic Dentistry; Forensic Anthropology; Legal liability”. Concerning Forensic Dentistry and dental records, it was observed that the dentist’s performance increased the dental team’s specific knowledge, improving the development of cases and all documentation of odontological origin was important to achieve conclusions and definitions of legal reporting, making possible that unidentified bodies have their real identity and legal processes would be resolved. It is concluded that forensic dentistry gives an improvement in the results of the cases that they enroll, and all the forensic team should have one forensic dentist to raise their knowledge and give more results in the search for the truth and body identity.

20. Small Bowel Obstruction by Broad Ligament Hernia: Three Case Reports, Management and Outcomes

Internal hernias are a rare cause of bowel obstruction (1%) and can be caused by broad ligament defects in 4% to 7% of the cases. These defects may be congenital or acquired and are classified according to its anatomical location. This paper reports three cases of small bowel obstruction by broad ligament hernia. The patients, three women aged from 35 to 51 years old, were admitted to the emergency department with small bowel obstruction. An exploratory laparotomy was performed during which an internal hernia through a broad ligament defect was identified. In all cases the hernia content was reduced and the defect closed. One of the patients required a segmental enterectomy. All patients had a favorable outcome. This paper aims to raise awareness about the broad ligament hernia as a cause of bowel obstruction, namely in middle-aged women with no surgical history.

21. A warning to readers about the term metanalysis in non-systematic reviews about diabetic retinopathy: documental study

BACKGROUND: Meta-analysis is a very seductive term in scientific papers because it summarizes a compilation of quantitative results; however its interpretation must be done carefully. OBJECTIVE: Search the diabetic retinopathy intervention literature for the occurrence of the term meta-analysis in non-systematic reviews, showing the reader the frequency that they occur and alert to the dangers that inadequate interpretation can cause. METHODS AND MATERIALS: An extensive search in Pubmed was performed for works with the term meta-analysis in the title, abstract and keywords (ti, abs, kw) without date or language restriction. The selected papers were read fully in search of characteristics of systematic review (SR) or not. The results are presented objectively with a critical analysis of each analysed term. RESULTS: We found 39 papers with the term meta-analysis on (ti, abs, kw). 12 (30.8%) of them did not write the study design in the abstract. 14 (35.9%) did not present the search strategy, 15(38.5%) did not mention having a language or date restriction. 23 (59%) did not mention the SR model that was followed and only five (12.8%) registered the protocol. Only two articles fulfilled the RS requirements. CONCLUSION: We found in the literature reviews of selected studies with meta-analysis, not configuring an SR that can induce the reader to interpret it as an SR. The term meta-analysis cannot be interpreted as the panacea for the solution of all doubts.

22. A model of knowledge management and cultural intelligence to reduce talent deficit in Brazil and Peru: a comparative study with Germany

This paper raises the discussion of the importance to create relevant knowledge and experience by developing the Culture Intelligence - CI of Brazilian students and has the follow objectives: i) to propose a model that can capture the relationship between culture, knowledge and intelligence and ii) to provide qualitative evidence of its effectiveness to reduce brain drain. In order to explain the impact of CI on Knowledge Management- KM and Organizational Intelligence - OI this work explores the development of a CI model based on KM and OI (CKI model) and change the “culture of student’s dependence” and therefore reduce the talent deficit in Brazil. The CKI model is constructed based on the results of 35 interviews in two Peruvian Universities, two Brazilian Universities and one German University and empirically tests 3 hypotheses through Structural Equation Modelling. The main conclusion is that Cultural Intelligence impacts maturity and democracy and therefore it would be very important to build a more robust Exchange program than the current Latin American Alianza del Pacifico Program, in partnership with the European Erasmus program.

Background: Mental health is an important issue in general practice/family medicine. The Patient Health Questionnaire for Depression and Anxiety (PHQ-4) allows depression and anxiety screening and psychologic distress evaluation. PHQ-4 cross-cultural adaptation and concurrent validation was the objective of this paper in a population of general practice/family medicine (GP/FM) doctors. Methods: Translation, linguistic verification and back-translation, internal consistency, and reliability of the final translated tool were performed for the cultural adaptation of the PHQ-4 scale. Introduction: Physicians’ main responsibility is to practice medicine for the benefit of patients. However, there are situations where secondary interests affect this commitment and give rise to conflicts of interest. Objective: To analyze currently available systematic reviews and meta-analyses on conflicts of interest in medicine to summarize relevant evidence in this regard. Material and methods: A literature search was performed in the MEDLINE and LILACS databases using the following search strategy: systematic reviews and meta-analysis on conflicts of interests in medicine published in serialized scientific journals; no publication time or language limits were applied. Studies that met the inclusion criteria were grouped according to the medical activity they assessed, and information on the type and number of studies and conclusions of all publications included in the review was collected. Results: 29 publications were included, and they were classified as follows: studies based on research articles, on clinical practice guidelines, on clinical practice, and on patient-oriented papers. Conclusions: It was found that the authors of the original research papers included in the meta-analyses and systematic reviews analyzed here do not always state if they have conflicts of interest or not. Nevertheless, when said conflicts are reported, they tend to present results favoring the drugs or medical technologies of their sponsor.

However, to make optimal use of magnetic resonance imaging in multiple sclerosis, the use of a standard, reproducible and comparable imaging protocol is of uttermost importance. In this context, the Portuguese Society of Neuroradiology and the Group of Studies of Multiple Sclerosis, after a joint discussion, appointed a committee of experts to create recommendations adapted to the national reality on the use of magnetic resonance imaging in multiple sclerosis. This document represents the second part of the first Portuguese consensus recommendations on the use of magnetic resonance imaging in multiple sclerosis in clinical practice. Material and Methods: The Portuguese Society of Neuroradiology and the Group of Studies of Multiple Sclerosis, after discussing the topic in national meetings and after a working group meeting held in Figueira da Foz, May 2017, appointed a committee of experts that have developed several standard protocols on the use of magnetic resonance imaging on multiple sclerosis by consensus. The document obtained was based on the best scientific evidence and expert opinion. Portuguese multiple sclerosis consultants and departments of neuroradiology scrutinized and reviewed the consensus paper; comments and suggestions were considered. Standardized strategies of magnetic resonance imaging referral in clinical practice for diagnosis and follow-up of multiple sclerosis were published in the first part of this paper. Results: We provide magnetic resonance imaging acquisition protocols regarding multiple sclerosis diagnostic and monitoring and the information to be included in the report for application across Portuguese healthcare institutions. Conclusion: We hope that these first Portuguese magnetic resonance imaging guidelines will contribute to optimize multiple sclerosis management and improve patient care in Portugal.

The objective of this study was to describe the prevalence of sports participation in Brazilian children and adolescents using a systematic review approach. This review follows the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement and the protocol was previously registered on PROSPERO (95441), the searches were performed in the following electronic databases: PubMed, ScIELO, Bireme, and EMBASE. The search strategy included a combination of keywords: Children and adolescents, prevalence and sports participation. Four papers included 7104 adolescents aged from 10 to 18 years old, living in four different cities located at Southeast (n= 1) and South (n= 3) of Brazil data collections were carried out between 2004 and 2012. Most of the studies had cross-sectional design and the findings of all surveys were published in international journals. The definition of “sports participation” was not the same in all papers. The overall prevalence of sports participation among adolescents was 58.1%, higher among boys (65.9%) than in girls (51.5%). The systematic review identified that the prevalence of sports participation among Brazilian adolescents is low and significantly affected by gender.

A systematic review of the prevalence of Active Play in Brazilian children and adolescents was performed. Only fully available scientific papers that measured Active Play or leisure time physical activity independently from other types of physical activity in Brazilian children and/or adolescents were considered for inclusion. The search for potential articles was performed on the following electronic databases: Pubmed/Medline, Web of Science, Bireme, Scielo and Scopus. Initially, 63 papers met the eligibility criteria. However, after deeply analyzed, seven studies remained and were included in the present review. The overall prevalence of Active Play was 36%, in which varied from 27.2% to 79.3%. Boys presented a prevalence of 47%, ranging from 39,1% to 79.9%, while the prevalence in girls reached 26%, varying from 13.9% to 78.7%. Although the prevalence of Active Play in Brazil in not satisfactory, the potential to present a reliable data regarding this theme should be highlighted. In agreement with some other studies, boys presented a higher prevalence of physical activity than girls. Also, any socioeconomic pattern regarding the prevalence of Active Play in children and adolescents was observed when the results from this study were compared to other studies developed in other countries from several socioeconomic status. The Active Play should be further investigated individually, as a pivotal component of physical activity, as well as its impact on physical inactivity-related comorbidities.

Results: We provide some practical guidelines to promote standardized strategies to be applied in the clinical practice setting of Portuguese healthcare professionals regarding the use of magnetic resonance imaging in multiple sclerosis. Conclusion: We hope that these first Portuguese magnetic resonance imaging guidelines, based in the best available clinical evidence and practices, will serve to optimize multiple sclerosis management and improve multiple sclerosis patient care across Portugal.

The practice of academic misconduct seems to be routine in undergraduate studies. Therefore, addressing ethical conduct in training is a starting point to promote debate on the construction of knowledge. This study aimed to analyze the academic misconduct of nutrition undergraduates. A cross-sectional study was performed with Nutrition students at a public university in Brazil. Participants included 105 students took part in the study: 42.9% were on the 1st to the 5th semesters and 57.1% on the 6th to the 9th semesters. Students on the final semesters had a higher prevalence of “letting the colleagues copy the answers” (p=0.05), “using ready-made work” (p=0.04), and “included their name on a paper without collaboration” (p=0.01). The main motivations for academic misconduct were: colleagues cheating (71.4%), believing that professors had committed misconduct (70.5%), difficult subjects (52.4%), and maintaining good grades (50.5%). Students on the last semesters reported lack of time (p=0.05) as a reason, and 10.5% mentioned having performed nutrition appointments without supervision. Given the high prevalence of academic misconduct in undergraduate courses, it is suggested that the discipline on ethics be taught in the initial semesters, in addition to offering courses and conversation circles on intellectual property, ethical conduct, time management, and teaching methodologies.

Introduction: A literature review in databases was carried out on suicidal behavior in indigenous peoples to identify its characterization as a public health problem. Objective: To systematize the knowledge on suicide in indigenous peoples based on papers and research reports published until 2014. Materials and Methods: Literature review of 149 papers published regarding suicide in indigenous communities, which were analyzed according to topographic, chronological, approach and content aspects. Results: Scientific research on suicide in indigenous peoples is limited. However, the literature reviewed indicates that suicide rates are higher in indigenous peoples than in the general population, especially among young people, worldwide. Research on suicide in indigenous communities is predominantly based on risk factors, and should be understood more as a social rather than an individual event resulting from an imbalance between destructive processes and health and quality of life programs. Conclusions: Suicide in indigenous peoples is a public health problem whose comprehensive study should be encouraged from an intercultural approach, thus facilitating the implementation of culturally appropriate interventions.

A modest archetypal of satiation is that it boils down primarily to two categories of signals transmitted from the gastrointestinal tract to the brain: stomach and intestine sensing and metabolism. Novel investigations endow us with the view that an extension of this traditional model is conceivable and called for, wherein intestinal satiation is the byproduct not only from signals related to the caloric content of ingested nutrients, but also from noncaloric properties of ‘tastant molecules’ in foodstuff. On this paper we discuss a recently published paper regarding the impact of tastants (e.g., noncaloric substance widely used as taste enhancers) on hunger and food intake. We gather the in vivo results (nasoduodenal infusions of tastants) with a recently developed mathematical model for ghrelin by the author and co-workers and we successfully replicate in silico part of the findings. The key results from the abovementioned paper and replicated herein is that tastant can inhibit hunger; with different levels of impact, umami being the strongest one, and the union of them being even stronger.

Acute porphyrias are a group of rare genetic metabolic disorders, caused by a defect in one of the enzymes involved in the heme biosynthesis, which results in an abnormally high accumulation of toxic intermediates. Acute porphyrias are characterized by potentially life-threatening attacks and, for some patients, by chronic manifestations that negatively impact daily functioning and quality of life. Clinical manifestations include a nonspecific set of gastrointestinal, neuropsychiatric, and/or cutaneous symptoms. Effective diagnostic methods are widely available, but due to their clinical heterogeneity and non-specificity, many years often elapse from symptom onset to diagnosis of acute porphyrias, delaying the treatment and increasing morbidity. Therefore, increased awareness of acute porphyrias among healthcare professionals is paramount to reducing disease burden. Treatment of acute porphyrias is centered on eliminating the potential precipitants, symptomatic treatment, and suppressing the hepatic heme pathway, through the administration of hemin or givosiran. Moreover, properly monitoring patients with acute porphyrias and their relatives is fundamental to preventing acute attacks, hospitalization, and long-term complications. Considering this, a multidisciplinary panel elaborated a consensus paper, aiming to provide guidance for an efficient and timely diagnosis of acute porphyrias, and evidence-based recommendations for treating and monitoring patients and their families in Portugal. To this end, all authors exhaustively reviewed and discussed the current scientific evidence on acute porphyrias available in the literature, between November 2022 and May 2023.

The current situation shows the importance of the insertion of society in the relationship between the State and the private sector, with the objective of collaboration to improve government programs. Shared Governance is one of the great challenges that the Italian government is currently facing to avoid the crises of governance (society) and governability (negotiation with opposition and work with the public administration), in particular after the decision of the new government (September 2022) to extinguish the Reform of the Public Administration - National Recovery and Resilience Plan - PNRR, and also the largest social participation initiative in Italy, the Tuscany Regional Policy for Social Participation - TRPP. This article recuperates the good points of PNRR and TRPP and another case study in Emilia-Romagna region. Offering a new perspective to the existing literature, this paper share the “famous” model for the Public Administration, the New Public Service (NPS), created to spread the idea that public service is seen as an extension of citizenship, and therefore both government and citizens need to abandon short-term interests and imitation of the competition of private sector of the New Public Management (NPM), assuming collaborative roles in building an educated and mature civil society. Based in a literature review on the NPS model, Knowledge Management - KM, Governmental Intelligence -OI and Cultural Intelligence -CI, the article presents a theoretical model of CI, OI and Social Participation – SP (CIGISP) to improve the effectiveness of public policies. This work concludes that CIGISP model is useful to identify how the learning by comparison with other values, believes and assumptions (CI) and use of GI Practices leads to a better quality of social participation.

This paper presents a case report of a perimenopausal woman who repeatedly attended health care institutions due to chronic insomnia, and underwent pharmacological treatments and psychiatric hospitalizations without achieving any positive result for nearly three years. After compiling all the information of the case, as well as analyzing previous and recent paraclinical studies, the patient was diagnosed with hypothyroidism. The purpose of this paper is to draw attention on the adequate use of clinical diagnostic tools and processes to optimize the medical practice and to offer a better service to patients.

Resumen La discapacidad, como una construcción social, implica las representaciones sociales y las prácticas que, en consecuencia, se derivan y tienen implicaciones sobre la vida de individuos, grupos o poblaciones. Algunas de las respuestas a las necesidades de las personas con discapacidad se han gestado en instituciones que nacieron con fines filantrópicos y se han ido transformado. Este escrito forma parte de una investigación sobre las construcciones sociales de discapacidad en Colombia en las últimas cuatro décadas, la cual fue desarrollada a través de una etnografía multisituada con perspectiva crítica, cuyas fuentes de indagación fueron experiencias institucionales, grupales, personales y documentos; aquí, se ilustran las transformaciones de instituciones dedicadas a la atención de esta población en lo relacionado con su inserción en la lógica del mercado de la salud a partir de la Ley 100 de 1993. Igualmente, este estudio muestra cómo las instituciones han instaurado una carrera desmedida para poder entrar y mantenerse en los juegos del mercado de la venta de servicios, situación que confirma el posicionamiento de la salud como un negocio, y en este campo, a la rehabilitación funcional como el estandarte a seguir. Artificios como la acreditación de servicios, el mercadeo social, la oferta de las últimas tecnologías y las diversas especialidades y subespecialidades, además de los eufemismos de cambios de nombre, se esgrimen a manera de capitales simbólicos, culturales y económicos que mantienen estos actores en el campo de la discapacidad como un asunto de prestación de servicios que aleja la relación entre la salud pública y la discapacidad. Summary Disability as a social construct involves social representations and practices that are a consequence of it, and which have implications on the lives of individuals, groups or populations. Some of the answers to the needs of people with disabilities had their birth in institutions that were create with philanthropic purposes and have been experiencing a transformation processs. This paper is part of an investigation about social constructions of disability in Colombia over the past four decades, developed through a critical perspective multipurpose ethnography whose sources of inquiry were expediencies from institutions, groups, and people, as well as documentation related to this topic. In this paper the transformation of institutions devoted to the care of this population with regard to their entry into the health market, since the enactment of Law 100 of 1993, is illustrated. Furthermore, this study shows how the institutions take part in an inordinate career to enter and stay in services provision market , a scenario that confirms the positioning of Health as a business, and in this field, functional rehabilitation as the standard to follow. Artifices like service accreditation, social marketing, modern technologies offer and the different specialties and subspecialties, in addition to the euphemisms related to name changes, are put forward as symbolic, cultural and economic capitals that keep the players on the field disability as a matter of service delivery, which takes away the relationship between public health and disability. 38. The use of complementary and integrative practices in oral health

The World Health Organization recognizes the medical practices derived from the culture of each country, as Traditional Medicine, but all over the world, there has been no consensus about the denomination of these practices. InBrazil, the Ministry of Health recognizes the use of the following, as Complementary and Integrative Practices (CIP): Acupuncture, Homeopathy, Medicinal Plants/Phytotherapy, Thermal Water/Crenotherapy, Anthroposophic Medicine. Based on the therapies recognized by the Ministry of Health, the Brazilian Federal Council of Dentistry regulated the use of CIP in oral health: Acupuncture, Homeopathy, Phytotherapy, Laser Therapy, Flower Therapy and Hypnosis. This article presents the results of a systematic review about positive and negative evidence of the use of PIC in oral health. We analyzed 91 papers published between 2000 and 2010. Laser therapy and Phytotherapy were the CIP most tested. Laser therapy studies reported more negative results, while the Phytotherapy studies reported more positive results. Papers on Flower Therapy were not included because we found no RCT about this CIP. We found more positive than negative outcomes for the use of CIP in oral health and analysis of the papers showed that only a few authors reported the clinical outcomes. Due to the holistic perspective of CIP, we believe it is possible to show more consistent results of use of these practices in oral health, provided that a broader view (beyond quantitative parameters) is incorporated from the planning to analysis of studies in this area.

39. Dengue disease diagnosis: A puzzle to be solved

Dengue is an infection caused by dengue virus and is the most important arthropod transmitted viral disease in the world, causing near 100 million cases and 50 000 fatalities each year. Health authorities believe that these numbers will grow in coming years. In Colombia, almost 600 municipalities are in regions with Aedes aegypti circulation, and the presence of four dengue serotypes has been demonstrated. Despite the increasing knowledge about disease pathogenesis and the dengue virus, some technical or scientific difficulties with diagnosing dengue remain, negatively affecting both public health surveillance and the appropriate attention to patients in health settings and hospitals. This paper reviews the principles and developments of the current diagnostic techniques for dengue, pointing out the difficulties with making accurate dengue diagnoses and case confirmations in public health and specialized laboratories. The principles and limitations of MAC-ELISA, IgG serology, viral NS1 detection and viral isolation by cell culture are presented. In addition, the review of immunochromatography techniques (rapid diagnostic tests) that have been put forward to help the point-of-care diagnosis is proposed. This paper is intended to bring forward some points of view about the issues related to dengue diagnosis and contribute to improve the discussion surrounding the strategies and techniques needed for reducing the impact of the disease and favoring its control.

This paper presents the case of an 11 year-old male who attended the Internal Medicine Service at a high complexity pediatric hospital. Initially, the patient attended due to a clinical profile consisting of autoimmune hemolytic anemia that was partially responsive to steroid treatment and, after exhaustive complimentary analysis, was associated to a Hodgkin lymphoma. Similar cases found in the scientific literature were reviewed in order to analyze this case. Through this paper, the authors intend to remind the medical community about the importance of a prompt and deep study of all autoimmune hemolytic anemia cases found in pediatric patients, without overlooking possible malignant causes related to this condition such as a lymphoproliferative disorder. Thus, before diagnosing a hemolytic anemia as idiopathic, the practitioner must be certain that the condition is not a clinical manifestation of an underlying disease.

The mechanisms and concepts of perceived exertion have changed over the years. Thus, the objectives of the present literature review were i) to investigate the literature reviews that have been published since 1982 and to describe the proposed causal mechanisms of perceived exertion, and ii) to determine whether the concepts of perceived exertion have accompanied the changes of its proposed mechanisms. Searches were per-formed in PubMed, Science Direct and Scielo using the terms ‘rating of perceived exertion and review’ and ‘perceived exertion and review’. Eighteen papers whose main objective was to determine the overall perceived exertion during physical exercise were included in the present study. It was observed that, despite the existence of the early corollary discharge theory, after Borg’s propositions most of the literature has explained perceived exertion using the afferent feedback theory based on correlational studies. It was only in 2009 that there was a resurgence of the original corollary discharge theory in perceived exertion papers. Most of the perceived exertion concepts are related to the afferent feedback mechanism, using constructs such as physiological responses, stress, discomfort and pain. However, these constructs are incongruent with the corollary discharge theory, presenting physiological mechanisms that differ from those of perceived exertion. Therefore, Borg’s studies have influenced contemporary views of the mechanisms and concepts of perceived exertion. Over the years, additional constructs have been incorporated into the original Borg’s concept, although always on the basis of the afferent feedback theory.

This paper aims to describe educational activities, developed with a group of the elderly in the city of Itaporanga-PB. These activities relate to the practice of health education for discussion of hypertension and diabetes, constituted by professionals in primary care of psychology, pharmacy, physiotherapy and social service core to support family health (NASF), along with the health teams family (FHS ) of the city. The groups were built by elderly between 68 and 75 years of both sexes, these being conducted in community centers, health facilities close to family. To implement these activities, were used: data projector, computer, stereo and CD player, blowing balls, sheets of paper, projection screen, speaker , microphone, chairs and tables. It was considered that were discussed pathologies that afflict this group, this action caused the service users could reflect not only on hypertension and diabetes, but also about themselves, about their daily lives through knowledge and practices shared that enabled their involvement with the care of the self and the production of life through a conscious and autonomous attitude .

This paper presents a reflection on the practice with a focus group, denominated by yours members of Group Discussion, being that accomplished in the period of one year, in a Centro de Atenção Psicossocial (CAPS I), in the city of Itaporanga-PB. Tends as theoretical perspective the foundations of the applied psychoanalysis to the practice among several, it was aimed at to contemplate about the activities in group accomplished in the referred institution. The Group of Discussion made use of several methodological resources: posters, mass of modeling, illustrations, drawings, paper leaves in white, colored pencil, glues and scissors, apparel of DVD, films and pictures. Rethinking the accomplished practice, he/she is considered that the rules of the Group were rethought, the schedules were flexibilizados, the discussed themes became lifted up for the users, through given suggestions in a verbal way (he/she speaks oral), and through the art (art therapy), tends as consequence the construction of the Group of Filming and of the Group of Picture.

The Journal of the Faculty of Medicine (Revista de la Facultad de Medicina) started its operation back in 1932, and has undergone interesting changes in the last decade, achieving greater visibility and impact on the national and international sphere. Similarly, it has managed to maintain the periodicity of its publication and to be included in important bibliographic indexes. Currently, biomedical journals published in English often make a greater impact and rank better in scientific databases in Spanish, thus giving prestige to the institutions that support them (1). Given this scenario, the medium-term objective of this journal is to publish papers in English only, which leads to a series of structural changes in editorial management and administrative academic support in order to maintain the current number of published scientific articles with excellent quality and a greater international visibility and, therefore, with a higher citation possibility. Publishing the results from the scientific activity of authors through papers in scientific journals is very important, since this has been the traditional way of doing so. Thus, a Latin American journal published in English is the perfect medium to present patents and other research products of the authors from this region and the best way to make visible the results of their work in the world.

45. The Genetic Psychosocial Risk Instrument (GPRI): A Validation Study for European Portuguese

Introduction: Screening instruments specifically developed to identify genetic testing applicants who may need professional psychosocial support are much needed. However, there are no screening instruments validated for the Portuguese language. This paper presents the translation, adaptation, and validation process of the Genetic Psychosocial Risk Instrument in a sample of 207 Portuguese applicants to genetic testing in the context of inherited cancer risk. Material and Methods: Participants were mainly female (84.06%), with a mean age of 40.08 (SD = 12.89) and were recruited from the Portuguese Oncology Institute of Porto. Confirmatory factor analysis was conducted to confirm the Genetic Psychosocial Risk Instrument factorial structure. Convergent validity was assessed with the Impact of Events Scale, the Clinical Outcome Routine Evaluation – Outcome Measure, and the Hospital Anxiety and Depression Scale. Results: A model composed by the factors ‘Internal Impact of Genetic Testing’, ‘External Impact of Genetic Testing’ and ‘History of Mental Health Concerns’ was confirmed. These factors showed good internal consistency, convergent and discriminant validity. The factor ‘Personal Loss to Cancer’ proposed in the Canadian and French versions did not converge. We propose excluding this factor from the European Portuguese version of the scale. Conclusion: The European Portuguese version of the Genetic Psychosocial Risk Instrument is a reliable and valid instrument, although more research is needed to effectively use it in routine clinical oncogenetic departments.

DOI: http://dx.doi.org/10.5007/1980-0037.2012v14n4p483This paper aimed to review the results of studies on neuromuscular adaptations to strength training (ST) and concurrent training (CT) in elderly men. A literature search was conducted using PubMed, Scopus, and SciELO. The search was limited to studies published from 1980 to 2012. A total of 3,390 articles were retrieved. After reading their titles, 127 studies were further evaluated by reading their abstracts. This resulted in 92 papers that were read in full; 25 of these were selected and their results were described in the present review. Several studies showed that, in elderly subjects, ST can produce increases in muscle strength, power, activation and mass. ST-induced strength gain may be explained by neural and morphological adaptations. The main neural adaptations to ST included increased recruitment of motor units and increased motor unit firing rate. Morphological adaptations included increases in the physiological cross-sectional area (CSA) of the muscle, in muscle thickness, in muscle fiber pennation angle, and changes in muscle myosin heavy-chain isoforms, resulting in the conversion of muscle fiber from subtype IIx to IIa. The inclusion of moderate-to-high inten- sity (60-85% of maximum strength) ST in the routine of this population is recommended to improve neuromuscular function. CT can promote significant neuromuscular adaptations, but these gains may be of a lower magnitude than those obtained with ST. Although CT has an interference effect on neuromuscular adaptations, it also promotes improvement in cardiovascular function and is therefore the most frequently recommended intervention for health promotion in the elderly.

Abstracts should not exceed one page, state the official title of the dissertation, the academic degree achieved, date and place where the thesis was defended and indicate the name of the supervisor and where the dissertation is available for consultation.In the section Letters to the Editor, readers are invited to express their opinion and make suggestions to the journal.Articles should be submitted by electronic mail, directed to the Secretariat of the Brazilian Society of Family and Community Health or to the Editor. Tipos de Trabalho A revista está estruturada com as seguintes seções: Editorial Artigos Originais Artigos de Revisão Diretrizes em Medicina de Família e Comunidade Ensaios Relatos de Experiência Teses Cartas ao Editor O Editorial é de responsabilidade do editor da revista, podendo ser redigido por terceiros por solicitação dele. A seção Artigos Originais é composta por artigos resultantes de pesquisa científica, apresentando dados originais de descobertas com relação a aspectos experimentais ou de observação, voltados para investigações qualitativas ou quantitativas em áreas de interesse da APS. Artigos originais são trabalhos que desenvolvem crítica e criação sobre a ciência, tecnologia e arte das ciências da saúde que contribuem para a evolução do conhecimento sobre o homem, a natureza e a inserção social e cultural. O texto – contendo introdução, material ou casuística, métodos, resultados, discussão e conclusão – deve ter até 25 laudas. A seção Artigos de Revisão é composta por artigos nas áreas de Gerência, Clínica, Educação em Saúde. Os artigos de revisão são trabalhos que apresentam síntese atualizada do conhecimento disponível sobre matérias das ciências da saúde, buscando esclarecer, organizar, normalizar e simplificar abordagens dos vários problemas que afetam o conhecimento humano sobre o homem e a natureza e sua inserção social e cultural. Têm por objetivo resumir, analisar, avaliar ou sintetizar trabalhos de investigação já publicados em revistas científicas e devem ter até 20 laudas, contendo introdução, desenvolvimento e conclusão. A seção Diretrizes em MFC é composta por artigos estruturados dentro das normas da Associação Médica Brasileira para diretrizes clínicas, validados pela SBMFC. Sua confecção, sob orientação da Diretoria Científica da SBMFC, é uma proposta de organizar e referendar o trabalho dos MFC no Brasil. A seção Ensaios visa à divulgação de artigos com as análise crítica sobre um tema específico relacionado à Medicina de Família e Comunidade e deve ser apresentada em uma média de 5 a 10 laudas. A seção Relatos de Experiência é composta de artigos que relatam casos ou experiências os quais explorem um método ou problema por meio do exemplo. Os relatos de caso apresentam as características do indivíduo estudado com indicação de sexo e idade, podendo este ser humano ou animal –, ressaltam sua importância na atuação prática e mostram caminhos, condutas e comportamentos para a solução do problema. Essa parte deve ocupar até 8 laudas, com a seguinte estrutura: introdução, desenvolvimento e conclusão. A seção Resumos de Tese, que deve ter apenas 1 lauda, tem como proposta a divulgação da produção científica na temática do periódico. Nela, devem ser expostos resumos de dissertações de mestrado ou teses de doutoramento/livre-docência defendidas e aprovadas em universidades brasileiras ou não. Os resumos deverão ser encaminhados com o título oficial da Tese, informando o título conquistado, o dia e o local da defesa. Devem ser informados, igualmente, o nome do Orientador e o local onde a tese está disponível para consulta. Em Cartas ao Editor, opiniões de leitores e sugestões sobre a revista são bem recebidas. As cartas, contendo comentários sobre material publicado, devem ter no máximo 2 laudas. Os trabalhos a serem submetidos à apreciação do Conselho Científico deverão ser encaminhados por e-mail para a Secretaria da Sociedade Brasileira de Medicina de Família e Comunidade ou ao Editor da revista. O padrão de formatação exigido é Word for Windows versão 6.0 ou superior -, página padrão A4, letra Arial (tamanho 11), espaçamento entre linhas 1,5 e numeração seqüencial em todas as páginas. As notas de rodapé devem ser limitadas ao máximo possível, assim como as tabelas e os quadros que devem ser de compreensão independente do texto. Os autores deverão informar seus nomes e endereços completos e quais organizações de fomento à pesquisa apoiaram os seus trabalhos, fornecendo inclusive o número de cadastro do projeto. Os trabalhos que envolverem pesquisas com seres humanos deverão vir acompanhados da devida autorização do Comitê de Ética da Instituição. Todos os textos são submetidos à avaliação de dois consultores provenientes de instituição diferente daquela do(s) autor(es), em um processo duplo cego, que os analisam em relação aos seguintes aspectos: adequação do título ao conteúdo; estrutura da publicação; clareza e pertinência dos objetivos; metodologia; informações inteligíveis; citações e referências adequadas às normas técnicas adotadas pela revista e pertinência à linha editorial da publicação. Os consultores preenchem o formulário de parecer, aceitando, recusando ou recomendando correções e/ou adequações necessárias. Nestes casos, os artigos serão devolvidos ao(s) autor(es), para os ajustes e reenvio, e aos consultores para nova avaliação. O resultado é comunicado ao(s) autor(es), e os artigos aprovados ficam disponíveis para publicação em ordem de protocolo. Não serão admitidos acréscimos ou modificações após a aprovação. Declaração de responsabilidade dos autores Todas as pessoas responsáveis como autores devem responder pela autoria dos trabalhos, tendo como justificada a sua participação de forma significativa no trabalho para assumir responsabilidade pública pelo seu conteúdo. Deverão, portanto, assinar a seguinte declaração de autoria e de responsabilidade: “Declaro que participei de forma significativa na construção e formação deste estudo ou da análise e interpretação dos dados, como também na redação deste texto, tendo, enquanto autor, responsabilidade pública pelo conteúdo deste. Revi a versão final deste trabalho e aprovo para ser submetido à publicação. Declaro que nem o presente trabalho nem outro com conteúdo semelhante de minha autoria foi publicado ou submetido à apreciação do Conselho Editorial de outra publicação. This paper seeks to outline a conceptual framework for child care during the hospitalization, by the light ofBetty Neuman’s Theory, which views are based on the effects caused by coping situations in the body together with thevariables: duration, occurrences, present or past individual’s conditions, nature and intensity of the stressor, and theamount of energy required by the body to adapt to situations. The stressors mentioned by Neuman occur by intrapersonal,interpersonal and extra personal factors. This paper describes the interventions that permeate this conceptual frameworkand its application in the care of hospitalized children to enable nurses to develop a therapeutic approach with the childclient in order to minimize disruption and breakdown of family dynamics.
