This paper highlights the important leadership role of the public health sector, working with other governmental sectors and nongovernmental entities, to advance environmental public health in Latin America and the Caribbean toward the achievement of 2030 Sustainable Development Goal 3: Health and Well-Being. The most pressing current and future environmental public health threats are discussed, followed by a brief review of major historical and current international and regional efforts to address these concerns. The paper concludes with a discussion of three major components of a regional environmental public health agenda that responsible parties can undertake to make significant progress toward ensuring the health and well-being of all people throughout Latin America and the Caribbean.

This paper aims to present an ex-post analysis of industrial and science, technology and innovation (sti) policies to the Health Industrial Complex, between 2003-2017 in Brazil, from the perspective of the public health value. We concluded that the intention to meet social health needs was present in the policies' design, however, the lack of objectivity and clarity in their definitions made it difficult to articulate the three fields of policies. This paper describes the development process of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos (PNAUM - National Survey on Access, Use and Promotion of Rational Use of Medicines) based on an integrated approach to pharmaceutical services, science, technology and innovation. It starts by contextualizing health and development in Brazil and features elements of the National Policy for Science, Technology and Innovation in Health in Brazil and the National Policy for Pharmaceutical Services. On presenting pharmaceutical policy guidelines, it stresses the lack of nationwide data. This survey, commissioned by the Brazilian Ministry of Health, has two components: household survey and evaluation of pharmaceutical services in primary care. The findings point to perspectives that represent, besides the enhancement of public policy for pharmaceutical services and public health, results of government action aimed at developing the economic and industrial health care complex to improve the health conditions of the Brazilian population. RESUMO O artigo apresenta o processo de construção da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamento a partir de uma concepção integradora da Assistência Farmacêutica, Ciência, Tecnologia e Inovação. Inicia-se contextualizando a saúde e o desenvolvimento no País e apresenta elementos da Política Nacional de Ciência Tecnologia e Inovação em Saúde no Brasil e da Política Nacional de Assistência Farmacêutica. Ao apresentar as diretrizes das Políticas Farmacêuticas, destaca-se a carência de dados de abrangência nacional. A presente pesquisa, encomendada pelo Ministério da Saúde, foi estruturada em dois componentes: inquérito domiciliar e avaliação dos serviços de assistência farmacêutica na atenção básica. Cardiovascular disease (CVD) is the leading cause of death in the Americas and raised blood pressure accounts for over 50% of CVD. In the Americas over a quarter of adult women and four in ten adult men have hypertension and the diagnosis, treatment and control are suboptimal. In 2021, the World Health Organization (WHO) released an updated guideline for the pharmacological treatment of hypertension in adults. This policy paper highlights the facilitating role of the WHO Global HEARTS initiative and the HEARTS in the Americas initiative to catalyze the implementation of this guideline, provides specific policy advice for implementation, and emphasizes that an overarching strategic approach for hypertension control is needed. The authors urge health advocates and policymakers to prioritize the prevention and control of hypertension to improve the health and wellbeing of their populations and to reduce CVD health disparities within and between populations of the Americas. This paper evaluates health as a Sustainable Development Goal (SDG) in the context of the Post-2015 Development Agenda, between 2012 and 2014. Health was part of the debate since the Millennium Summit and the MDGs (2000), and it also appears in the documents discussing the Post-2015 Agenda, from the Rio+20 to the Open Working Group (OWG), whose report was submitted to the General Assembly of the United Nations (UNGA) 2014-2015, and in the Global Consultation on Health and the High-Level Panel of Eminent Persons reports. The Authors concluded that the treatment of health in all these documents is uniform. They point out that the scope of the health-related SDG is very comprehensive, but its targets are conceptually fragmented and reduced. They advocate their change as to include not only the idea of social determinants of health, but also targets in the field of public health, which were not included in the proposal of the OWG. They also warn that the global and national governance systems need to be reformed and advocate more participation of the civil society, which can influence diplomacy, which, in turn, will be responsible for the agreement signed at the UNGA in 2015.

This paper analyzes the evolution of health expenditures using own revenues by Brazilian municipalities after the promulgation of Constitutional Amendment 29/2000, in order to verify if they grew homogeneously in different regions of the country and if regional inequalities decreased, maintained or increased with time. It uses Moran’s Global and Local Index to look for patterns of spatial correlation and clusters formation. [ABSTRACT FROM AUTHOR]

This paper is about the consequences of pandemics on the macroeconomic behavior, with an empirical analysis of such effects on the Brazilian economy. Two features of the macroeconomic of pandemics are highlighted: the emergence of a dual economy, in which we have the coexistence of a sector where consumption and production are impaired by public health policies, side by side with a full employment sector; and a large redistribution of income between the public and the private sectors. Regarding the latter, we found that the redistribution had strong effects on the flows of savings in the Brazilian economy, and consequently on the stocks of net wealth: there was a sharp rise in public debt and an increase in private net wealth of 12% of GDP. Like many nations in the Americas, the countries of the Caribbean are facing increasing morbidity and mortality associated with noncommunicable diseases (NCDs). In 2007, based on advocacy by Sir George Alleyne and others, the Caribbean Community (CARICOM) convened the first in the world Heads of Government NCDs Summit. This summit issued the Port of Spain Declaration that served not only as a rallying point to accelerate the regional NCDs response, but also as a catalyst for the first United Nations high-level meeting on NCDs in September 2011. In 2014, seven years after the CARICOM NCDs Summit, a formal evaluation of the Port of Spain Declaration (POSDEVAL) was conducted to investigate its impact and learn lessons to support and further accelerate its implementation. One mechanism was to review successes and challenges in the NCDs response and effectively communicate findings to stakeholders and the public. The results of the evaluation and the implementation of the Port of Spain Declaration are presented by other papers in this supplement. The present paper describes the communication and social information process used for sharing POSDEVAL findings across multiple layers of social influence in the Caribbean, primarily in 2016 – 2017. The main steps of the communication process are shared as a possible strategy for disseminating NCDs research information and accelerating control and policy adoption in other Small Island Developing States and middle-income countries.

Objective: this study examined scholarly output relating to nursing advocacy contributions toward influencing policy by authors in countries of the Pan American Health Organization. Method: the study utilizes a bibliographic analysis of papers indexed in Scopus authored by PAHO member state scholars. VOSviewer conducted coauthor and cooccurrence analysis to generate visualizations of the relationships between authors, countries of origin and keywords. Results: 7,773 papers with 21,523 authors met the inclusion criteria. An increase of publications on policy starting in 1962 was found. Co-authorship identified a fragile relationships structure with few authors bridging networks of collaboration. By country of origin, 22 of 35 member states contributed to policy literature; 17 in a connected network and 5 contributing but neither connected to peers nor other member states. Keyword analysis identified 20 specific data clusters. Conclusion: our findings are aligned with the Nursing Now Campaign. This bibliographic analysis provides an important benchmark into current policy advocacy activity in PAHO against which future progress in the region can be assessed. There is scope for greater collaboration amongst authors and this could be targeted toward engagement of nurses in member states not-yet or only partially active in this space.

The Republic of Guinea-Bissau (RGB), a fragile state marked by a context of political and institutional instability, has since 1993 carried out the Strategic Planning for the Health Sector (SPHS). Sectoral strategic documents - National Plans of Sanitary Development (NPSD) - demonstrate a capacity for resistance to governmental adversities. This longitudinal policy review has a double purpose, namely to analyse the SPHS processes and to guard these memories to allow an understanding of the trajectories in question. This paper concludes that the SPHS mainly responded to the requirements of funding institutions, and it has not always been implemented. The NPSD III (2018-2022) aims to reverse this situation by implementing a strategy which aims to integrate the different health actors. [ABSTRACT FROM AUTHOR]

The paper discusses the articulation among epidemiological information systems, scientific production and men's health policies. Three secondary sources were used: data from the Ministry of Health (Mortality and Hospital Information Systems, Surveillance of Protection and Risk Factors for Chronic Diseases through Telephone Survey), papers published in SciELO, and documents of the Ministry of Health regarding men's health. The results indicate that, in terms of morbidity and mortality, men are more exposed to risks than women. In the scientific production, studies focus predominantly on injuries and diseases that affect exclusively the male population, to the detriment of other health-related aspects. Legal documents highlight the epidemiological panorama of male morbidity and mortality and the policy-making methodology. Researchers need to expand the use of data from the epidemiological information systems of the Ministry of Health and to incorporate the gender relational perspective critically.

The supply of medicines in response to court orders or injunctions has become a common practice in the State of São Paulo, Brazil. This 'judicialization' of the health system clashes with basic principles of the Brazilian Unified National Health System (SUS), such as equal opportunity to access health services. The aim of this paper is to analyze the legal action used to obtain medicines through the São Paulo State Health Department, from two main angles: judicialization of public policies and breach of the equity principle. This is a descriptive study of legal action taken to obtain medicines through the São State Health Department, as listed in the Electronic Court Docket System for the year 2006. Most cases were filed through private attorneys; 47% of the patients had obtained their prescriptions through private care; and 73% of the cases involved patients from the three wealthiest areas in the city of São Paulo. The data demonstrate that such legal action violates key principles of the SUS such as equity, thereby privileging individuals with higher purchasing power and more access to information.

This paper presents a preliminary overview of policies and practices in health focusing equity. It begins with a brief description of the actions developed by the Collective Health Nursing Department of the School of Nursing of the University of Sao Paulo, Brazil, to support the struggle for social equity. It also presents the different perspectives of the concept that can be identified in health literature, each of them shaping distinct policies and practices.

This paper analyzes the policy model of the Brazilian Ministry of Health from 1990 to 2002. The methodology included interviews with key actors in the national health policy, document review, and analysis of the Federal budget and official databases. The Brazilian Ministry of Health underwent major changes under the influence of the health reform agenda and the liberal State reform agenda prevailing in the 1990s, shaped by two movements: institutional unification of national policy control and political/administrative decentralization. The Federal role was diminished in terms of direct services provision, and there were changes in financing and regulation. The model in the late 1990s featured strong Federal induction of States and municipalities and the adoption of market regulation strategies. There is no record of a long-term planning effort, which favors distortions in the Federal intervention model and hinders solutions to structural problems in the Brazilian health system.

This paper puts health questions within the context of national development and industrial policy. It follows the idea of structuralist, Marxist and Schumpeterian approaches, in which industry and innovations form determining factors for the dynamism in capitalist economies and relative positions within the world economy. All countries that have developed and started to compete under better conditions with advanced countries have had an association between strong industry and an endogenous knowledge, learning and innovation base. However, in the field of health, this vision presents problems because business interests move according to the economic logic of profit rather than to meet health needs. The notion of the health-industrial complex is an attempt to provide a theoretical reference that enables linkage between two distinct types of logic: health and economic development. This study has sought to show, on the basis of foreign trade data, how disregard for the logic of health policy development has led to a situation of economic vulnerability in this sector, which may limit the objectives of universality, equality and comprehensiveness. Within this context, a cognitive and political break with these antagonistic visions that put health needs on one side and industrial needs on the other is proposed. A country that aims to reach a condition of development and independence requires strong innovative industries and an inclusive and universal health system, at the same time.

Policies are made in response to the rationale of pressure and legitimization, which join forces in many different ways. This work analyzes the planning and adoption of the health reforms undertaken in Costa Rica between 1988 and 1998. It questions whether political parties, international financial institutions and the technical and bureaucratic elites in each sector can be taken as sufficiently explanatory of themselves. Empirical evidence would suggest that apart from investigating the individual interests of the agents involved, one must also consider the sector reforms that are actually available internationally. The paper draws this discussion into the larger scenario of policy making in Latin America and draws links between this and other stages in policy making and other moments in the construction of the State.

Recognizing the importance of generating and disseminating innovation for the social and economic development of capitalist countries and the systemic, institutional, and historical nature of this process, this paper emphasizes the need to deal with innovation in the health sector within a framework linking economic, institutional, and policy dynamics. Analysis of the main health goods industries (manufacturers of pharmaceuticals, vaccines, and diagnostics) shows that in the developed countries there is a certain compatibility between the health system and the innovation systems in terms of requirements, and that the state is an essential player in this articulation. In Brazil, however, there is a marked disconnection between the health and innovation systems. The few initiatives aimed at this link have enhanced the competitiveness of national agents and have been essential elements in national health policy. Based on this analysis, it is argued that beyond its social dimension, health policy is a strategic means to consolidate a dynamic innovation system, with direct effects on national development.

Since the Healthy City movement is a health promotion strategy, an update study was carried out to put into context the health promotion evaluation debate stressing the principles to be considered in an evaluation initiative and in the problematic of Health Cities projects. A literature review of "Healthy Cities" was conducted. A typology based on of Healthy Cities papers found in MEDLINE, LILACS and published in the "Health Promotion International" between 1985 and 2000 was proposed taking into consideration the main focus of each paper. Articles on evaluation were analyzed in more details according to stressing methods, research tools, indicators, study results, and critical appraisal of their models. Finally, it is highlighted initiatives that would come close to the principles of "Health Promotion" and adopt evaluation as a tool for building up capabilities and the empowerment of community groups involved with Healthy Cities initiatives.

The present paper aims at presenting a short history of health policies implemented in São Paulo in the 1920's and 1930's, trying to reach a more accurate understanding of the processes that led to the formulation of governmental health policies. The paper emphasizes Geraldo Horácio de Paula Sousa's contribution to the history of health administration in Sáo Paulo in the first half of the twentieth century and his proposition of a new model of health assistance based on health units that would work as the structural axis of all public health activities in the state. Finally, it focuses on the support given by Rockefeller Foundation to Paula Sousa's activities and on the disputes involved in the establishment of public health policies.

This paper analyzes responses by welfare states to globalization in some OECD countries in the 1980s and 90s, demonstrating that the hypothesis concerning the dismantling of social welfare systems cannot be proven due to lack of evidence of substantial changes in the funding and selection of beneficiaries. The paper also focuses on the effects of internationalization of the Brazilian economy on the country's social protection system. The conclusion is that restrictions imposed on public expenditures severely limited the capacity to promote broad coverage in social policies. During the decade the option was made for targeted, selective social policies based on the adoption of a diagnosis of low effectiveness in the Brazilian social protection system. The case of health policy was an exception, due to capacity for interest mediation in the social arena.

The paper deals with a historical recovery of the genesis of Nursing and its development in Brazil in order to elucidate its influence in current nursing education and practice. It emphasizes the role played by the Rockefeller Foundation in the organization of public health nursing, the creation of the first school of nursing and the consolidation of nursing education and practice in hospitals. Certain public health policies were also examined, given their influence in the development of the profession. Nursing maintained a scientific and politically neutral position during the implementation of these health policies, possibly because of the non-critical makeup brought about by the adoption of the flexnerian model of teaching, in its link to Medicine.

Based on the authors' experience, this paper discusses a series of problems during the implementation of so-called Indigenous Health Districts (DSEI) in Brazil, related to organization of health services provision as viewed by both health professionals and anthropologists. The authors report on the Health District model's underlying concepts and present the different approaches used to implement the DSEI. The authors' experience refers to the Rio Negro area - in the northwestern Amazon, representing 10% of Brazil's total indigenous population - and the Brazilian Northeast, specifically the State of Pernambuco, with an indigenous population estimated at 20,000.

This paper is a state-of-the-art review of scientific knowledge on both the health effects of asbestos fiber inhalation and possibilities for safe and sustainable prevention, from an ethical, political, and technological point of view. The author provides scientific background and arguments from the ongoing discussion in Brazil concerning the need to redefine current asbestos policy, in order to establish a more advanced and appropriate policy whose priority is the protection of life, human health, and the environment. The first part deals with several technological and economic aspects of asbestos-chrysotile. In the second part, the author presents and discusses a bibliographic review of the construction of scientific knowledge on the health effects of asbestos fibers, first within an international perspective, and then (in the third part), from a Brazilian view. The fourth part analyzes the current debate on the fibrogenicity and carcinogenicity of asbestos-chrysotile. Some current responses from the international community towards the asbestos-chrysotile ban are also discussed. Finally, the author discusses the historical inadequacy of Brazilian asbestos policy and the urgent need to revise it to include a ban on asbestos-chrysotile in this country.
