Introdução - A cadeia de produção de semijoias e bijuterias em Limeira é responsável por um processo produtivo complexo que, chega ao domicílio residencial e, gera trabalho infantil e adolescente. Graves problemas de saúde podem surgir para as famílias que produzem semijoias e bijuterias dentro de casa, além dos riscos ocupacionais e de acidentes. Objetivo - Compreender o aprendizado expansivo coletivo de modo a possibilitar a colaboração e a integração entre os diferentes atores da rede de sistemas de atividades da COMETIL - Comissão Municipal de Erradicação do Trabalho Infantil e Adolescente de Limeira. Método - Coleta de dados utilizando entrevistas, reuniões, oficinas de apresentação, observações da atividade. Posteriormente, utiliza-se o método Laboratório de Mudanças (LM), que se apoia na Teoria da Atividade Histórico Cultural e na Aprendizagem Expansiva, para a compreensão das atividades interinstitucionais da COMETIL, com gravação e filmagem das sessões para análise. Resultados - Divididos em 2 artigos e 1 capítulo de livro. No primeiro artigo, as hipóteses são de que contradições existentes no sistema de atividades da cadeia produtiva não permitem transpor o desafio de sustentabilidade da cadeia, aumentando a precarização do trabalho e os riscos à saúde e segurança das famílias que, por não encontrarem outra alternativa, produzem semijoias e bijuterias em casa. No segundo, apresenta-se uma análise dos resultados do LM que permitiu elevar o nível de agência transformativa entre os participantes da COMETIL, além de fornecer visualização das ações com a elaboração de novos instrumentos de trabalho. No terceiro, evidencia-se que a agência transformativa efetiva-se através da compreensão de que há necessidade de atuação coletiva e integrada, entre instituições públicas e privadas, confirmando o processo de aprendizagem expansiva desencadeado pelo método. Conclusão - O LM permitiu visualizar que a COMETIL pode ser entendida como uma plataforma ou um macro sistema de atividades e que possibilita novos processos organizacionais de ações das instituições públicas de erradicação e controle do trabalho infantojuvenil junto a este setor produtivo de semijoias e bijuterias. The costume jewelry production chain in Limeira is responsible for a complex production process that gets the residential homes and creates child and adolescent labor. Consequently, severe health problems can emerge for families that produce costume jewelry inside their houses, besides the occupational risks and accidents. Objective - To understand the collective expansive learning towards the collaboration and the integration between the actors of the activity systems of COMETIL - Committee for Eradication of Child Labor and Adolescent Work Protection of Limeira. Methods - Data collection using interviews, meetings, introductory workshops, and observations of activity. Subsequently, the Change Laboratory (CL) method, based on the Cultural-Historical Activity Theory and the Expansive Learning, was carried out to comprehend the interorganizational activities of the COMETIL with video recording for analysis. Results - These are divided into two papers and one book chapter. In the first paper, the hypotheses are that the existing contradictions in the activity systems of the production chain do not allow to meet the sustainability challenge, increasing the precariousness and the health and safety risks for families that are forced to produce costume jewelry at home because of an absence of other choices. The second text presents the analysis of the CL results that promoted the transformative agency among the COMETIL participants and provided the visualization of actions with the elaboration of new work instruments. The third paper supports that the transformative agency becomes effective through the comprehension of the need for a collective and integrated action between public and private institutions, confirming the learning expansive process resulting from the method. Conclusions - The CL allowed visualizing that the COMETIL can be understood as a platform or a macrosystem of activities and it promoted a new organizational process of actions of public institutions of eradication and monitoring the child labor with this costume jewelry production sector.

A fibromialgia é uma condição de alta prevalência, que causa desconforto físico, sofrimento mental e comprometimento das relações sociais. Com o aumento da popularidade de Mobile Health, considerou-se o uso de aplicativo móvel como recurso de suporte no tratamento de indivíduos com fibromialgia. Há uma carência de aplicativos para brasileiros portadores de fibromialgia, desenvolvidos por profissionais de saúde, a partir de fontes de informação confiáveis, em linguagem adequada, com alto grau de funcionalidade, testados quanto à sua eficácia em ensaios clínicos. O presente projeto constitui-se de dois estudos. O Estudo I teve como objetivo desenvolver um aplicativo móvel para promover o autocuidado em indivíduos com fibromialgia, e o Estudo II teve como objetivo avaliar sua eficácia na melhora da qualidade de vida e dos sintomas e na promoção do autocuidado, e verificar a adesão dos pacientes ao uso do aplicativo. Desenvolveu-se o aplicativo denominado ProFibro segundo o paradigma da prototipação, em cinco etapas: (1) análise de requisitos e conteúdo e estabelecimento de objetivos do software; (2) desenvolvimento da interface pela designer; (3) desenvolvimento do protótipo pela programadora para o sistema operacional Android (Google Inc., Mountain View, CA); (4) teste-piloto para avaliação da qualidade de uso do protótipo com pacientes; (5) refinamento do protótipo e construção do produto final. Suas principais funções são: animação educativa; automonitoramento com o Questionário de Impacto da Fibromialgia Revisado; estratégias do sono com a técnica de relaxamento por imaginação guiada, terapia de controle de estímulo e higiene do sono; agenda para facilitar o planejamento da rotina diária; programa gradual de exercícios aeróbicos, alongamento e fortalecimento; diário para a prática da gratidão; livro eletrônico com atividades para a família; dicas por meio de notificações. O Estudo II foi um ensaio clínico controlado e aleatorizado com 40 indivíduos com fibromialgia, alocados em grupos experimental e controle. Os participantes foram avaliados na linha de base e após seis semanas por um avaliador cego, quanto às seguintes variáveis: qualidade de vida (Questionário de Impacto da Fibromialgia Revisado); dor (Índice de Dor Generalizada e Escala Visual Analógica); sintomas (Escala de Severidade de Sintomas); agenciamento do autocuidado (Escala de Avaliação de Agenciamento de Autocuidados Revisada). O grupo experimental recebeu smartphones com o aplicativo ProFibro para uso durante seis semanas, enquanto o grupo controle recebeu uma apostila em papel de conteúdo similar. A adesão dos pacientes ao aplicativo foi avaliada contabilizando o número de acessos às suas funcionalidades. O uso do aplicativo ProFibro durante seis semanas, sem o acompanhamento de um profissional de saúde, foi eficaz para a redução da severidade de sintomas e do impacto geral da fibromialgia na qualidade de vida, em comparação com a linha de base. Os efeitos do uso do aplicativo foram similares aos da apostila. Cerca de 40% dos participantes do Grupo Experimental tiveram uma baixa adesão ao uso do aplicativo. As estratégias do sono foram a funcionalidade mais utilizada, seguidas do programa de exercício físico e do diário Fibromyalgia is a prevalent condition that causes physical problems, mental distress and impaired social functioning. With the increasing popularity of Mobile Health, the use of a mobile application as a support tool in the management of fibromyalgia was considered. There is a shortage of applications for Brazilian patients with fibromyalgia, which are developed by health professionals, based on reliable sources of information, in proper language, with a high degree of functionality, and are tested for effectiveness in clinical trials. This project is formed of two studies. Study I aimed to develop a mobile application to promote self-care in individuals with fibromyalgia, and Study II aimed to evaluate its effectiveness in improving quality of life and symptoms, and promoting self-care, and to verify patient compliance to the use of the application. The application which was named ProFibro was developed according to the prototyping paradigm, in five stages: (1) analysis of requirements and content and establishment of software objectives; (2) interface development by the designer; (3) development of the prototype by the programmer for the Android operating system (Google Inc., Mountain View, CA); (4) pilot test with patients to evaluate the quality of use of the prototype; (5) improvement of the prototype and construction of the final product. Its main functions are: educational animation; self-monitoring with the Revised Fibromyalgia Impact Questionnaire; sleep strategies with guided imagery relaxation technique, stimulus control therapy and sleep hygiene; scheduling function to facilitate planning the daily routine; graded exercise programme with aerobic, stretching and strengthening exercises; a diary for the practice of gratitude; an eBook about family adjustments; hints through notifications. The Study II was a randomized controlled trial of 40 individuals with fibromyalgia, which were allocated into Experimental and Control groups. A blind evaluator assessed the participants at baseline and after six weeks regarding the following outcomes: quality of life (Revised Fibromyalgia Impact Questionnaire); Pain (Generalized Pain Index and Visual Analog Scale); symptoms (Severity of Symptoms Scale); self-care agency (Revised Self-Care Agency Assessment Scale). The Experimental Group received smartphones with the ProFibro application and were instructed to use it for six weeks while the control group received a paper book with similar content. Patient adherence to the application was evaluated according to the number of accesses to its functions. The use of ProFibro application for six weeks, without the health professional support, was effective in reducing the severity of symptoms and the overall impact of fibromyalgia on quality of life compared to baseline. The effects of the use of the application were similar to those of the paper book. About 40% of the participants of the Experimental Group had a low adherence to the use of the application. The sleep strategies were the most used function, followed by the exercise programme and the diary

Study model: integrative review. Objective: to identify organizational behaviors described in the literature about inter-hospital transport of newborns in critical health condition performed by means of ambulance. Methodology: integrative review with query in the databases: LILACS, CINAHL, MEDLINE, Web of Science and Scopus, during the period from September to November 2013. In order to perform the research, the non-controlled descriptors ‘newborn transport’, ‘newborn infants’ and ‘protocol’ were used, as well as the controlled descriptors ‘transport of patients’, ‘newborn’ and ‘ambulances’. The search was limited to papers published in Portuguese, English and Spanish, without an established period, which addressed the inter-hospital transport of critically ill newborn by ambulance. Results: 12 papers were selected. The identified behaviors were: pre-transport communication; composition of the team that will conduct the transport; reference and counter-reference; selection of vehicle for transportation; equipment and medications that may be used during transportation. Conclusions: The selected studies address the important measures to be applied at all stages of the inter-hospital transport of newborns by ambulance, which are important for quality and effective care

Através do fato de que os silicones pigmentados utilizados na confecção de aloplastias para a reabilitação de pacientes portadores de defeitos faciais apresentam alteração cromática com o decorrer do tempo na experiência clínica, e pela escassez na literatura específica de trabalhos experimentais relacionados ao tema, principalmente quanto aos silicones industriais de cura acética, o presente trabalho tem como objetivo verificar o comportamento de três tipos de silicones diferentes coloridos com um determinado pigmento (pó para maquiagem facial), usados rotineiramente no atendimento de próteses faciais. Os silicones usados foram um de cura por calor (HTV) para finalidades médicas, um de cura acética (RTV) para fins industriais com especificações de normas alimentícias e outro também de cura acética (RTV), porém somente de normas industriais. Para a realização do experimento confeccionou-se 30 corpos de prova, sendo 10 para cada tipo de silicone, que foram analisados por espectrofotômetro. Após análise, cada grupo foi dividido em 2 subgrupos, de 5 unidades cada, um grupo exposto (E) e um grupo controle (C). Os 3 grupos (E) passaram por envelhecimento acelerado em exposição ambiental, enquanto os 3 grupos (C) ficaram acondicionados em um lugar totalmente escuro em temperatura ambiente. A leitura inicial foi considerada padrão, enquanto as seguintes foram aos 45 e 90 dias. Os dados obtidos nas três medições foram usados para avaliação das alterações cromáticas. Com os resultados estatísticos após análise, concluímos que todos os silicones expostos apresentaram alteração na cor clinicamente inaceitável, sendo que nos grupos controle somente o silicone de cura acética (RTV), industrial com especificações de normas alimentícias, apresentou mudanças cromáticas clinicamente aceitáveis, enquanto os outros dois se mantiveram estáveis. Owing to the fact that pigmented silicons used in the making of facial prosthesis or the rehabilitation of patients with facial flaws present , in clinical experience, chromatic alteration as time passes by, and due to the shortage of experimental papers in the specific literature, specially in relation to industrial silicon of acetic cure, the present paper targets to verify the behavior of three types of different silicons colored with a determined pigment (facial make up powder), used as routine in facial prosthesis. The used silicons were: one of cure by heat (HTV) with medical purposes , one of acetic cure (RTV) for industrial purposes with specific nutritious rules and another one of acetic cure (RTV) with industrial rules only. To perform the experiment thirty bodies of proof were made , ten to each silicon type, which were analyzed through spectrophotometer. After analyzing, each group was divided into two sub groups, five units each, one group exposed (E) and one group control (C). The three groups (E) passed through accelerated environmental exposure aging, while the three groups (C) stayed in a totally dark place at normal temperature. The initial reading was considered pattern, while the following readings were within 45 and 90 days. The data from the three measurements were used for evaluating the chromatic alterations. With the statistic results at hand, we came to the conclusion that all silicons exposed showed unacceptable clinical color alteration, and in the control groups only the industrial acetic cure silicon (RTV) with specific nutritious rules, showed acceptable clinical chromatic changes while the other two remained stable.

Com o intuito de avaliar o comportamento homeostático do sulco gengival em função da quantidade de gengiva ceratinizada, direcionando-se a análise para a variação da quantidade de fluido gengival produzida pelo estimulo da mastigação de alimentos fibrosos (carne bovina), foram selecionados 16 pacientes com boas condições de saúde geral e áreas de pré-molares e molares homólogas, saudáveis clinicamente, apresentando no lado experimental faixa adequada de gengiva ceratinizada (Grupo A ?2mm) e no lado controle faixa insuficiente de gengiva ceratinizada (Grupo I < 2mm). Os parâmetros clínicos avaliados foram: Índice de placa bacteriana, Índice de sangramento gengival, Profundidade de sondagem, Quantidade de gengiva ceratinizada e Quantidade de fluido gengival. A quantidade de fluido gengival foi obtida antes e após a mastigação de alimento fibroso culinariamente preparado, por 10 minutos, sendo avaliada por meio da impregnação de tiras de filtro de papel absorvente. Conforme proposta de LÖE; HOLM-PEDERSEN, a coleta do fluido gengival foi inicialmente feita por meio da colocação do papel sobre o dente e a mucosa, para embebição do papel na região da mucosa alveolar (P1A) e da margem gengival (P1B), seguindo-se a colocação de outra tira com sua extremidade junto ao orifício de entrada do sulco gengival (P2). Uma terceira tira foi disposta intrasulcularmente (P3), conforme proposto por BRILL e KRASSE. Todas as áreas foram secas previamente com jato de ar e as tiras de papel foram mantidas por 1 minuto em cada posição de cada grupo, depois removidas, deixadas a secar pelo calor, e em seguida embebidas em solução alcoólica de ninidrina a 2%, deixadas secar, e posteriormente as áreas coradas foram medidas com paquímetro digital na sua extensão longitudinal. Os dados coletados foram estatisticamente avaliados por meio do teste "t" de Student pareado. Os dados obtidos neste estudo permitiram concluir que a maior quantidade de gengiva ceratinizada resulta em melhor comportamento homeostático do periodonto marginal, pois esta interfere no processo de extravasamento do fluido gengival; a mastigação influencia a quantidade de fluido extravasado marginalmente como fluido gengival; quanto maior a faixa de gengiva ceratinizada, tanto maior parece ser a defesa natural do sulco gengival With the intention of assessing the gingival crevice homeostatic behavior in relation to the keratinized gingiva, and more specifically, directing the analysis to the variation of gingival fluid produced by the chewing of fibrous foods (bovine meet), it was chosen 16 patients in good general health and with clinically healthy homologous pre-molars and molars. The patients presented on the experimental side adequate width of keratinized gingiva (Group A?2mm), and on the control side insufficient width of keratinized gingiva (Group B < 2mm). The clinical indexes used were: plaque index (PI), gingival bleeding index (GBI), probing depth (PD), keratinized gingiva width (KGW) and amount of gingival fluid (GF). The amount of gingival fluid was collected before and after the chewing of cooked fibrous food (bovine meat) for ten minutes by the use of absorbing filter paper strips - PerioPaper. The collection of gingival fluid according to LÖE; HOLM-PEDERSEN was initially carried out by the placement of the PerioPaper over the tooth and the mucosa for the staining of the paper strips in the alveolar mucosa region (P1A) and the gingival border (P1B). After that the tip of another strip of paper was placed at the entrance of the gingival crevice (P2). Finally, a third strip was placed intracrevicularly (P3) as proposed by BRILL. All the areas were previously dried with a jet of air, and the strips of papers were kept in place for 1min in each of the positions in each group, then removed, allowed to dry and immersed in a nihidrine solution at 2%. After drying the stained areas were measured longitudinally with a digital caliper. The data collected was statistically assessed through Student's paired "t" test. The results in this study seem to lead to the conclusion that a greater width of keratinized gingiva results in a better homeostatic behavior of the marginal periodontum, since it interferes with the gingival fluid leaking process: chewing influences the amount of fluid leaked marginally as gingival fluid: the greater the keratinized gingiva the greater seems to be the natural defenses of the gingival crevice

Resumo A Eugenia, desde seu desenvolvimento como teoria científica por Francis Galton, teve grande difusão por diversos países durante a primeira metade do século XX. Advinda de um período de intensa discussão sobre hereditariedade, influência do meio ambiente e degeneração, a Eugenia assumiu diferentes referenciais na composição de suas propostas, resultando em um corpo teórico bastante heterogêneo e multifacetado. Esse artigo tem por objetivo analisar a difusão do ideário eugênico nos discursos da Sociedade de Medicina de Porto Alegre. Tendo como objeto de estudo o periódico da instituição, o Archivos Rio Grandenses de Medicina, buscamos entender como a ciência eugênica moldou a visão de determinados médicos não só sobre doenças venéreas e patologias infectocontagiosas, mas também sobre temas como reprodução, maternidade, primeira infância, vícios e mortalidade. Abstract Eugenics, since its development as a scientific theory by Francis Galton, had widespread in several countries during the first half of the 20th century. Coming from a period of intense discussion about heredity, influence of the environment and degeneration, Eugenics assumed different references in the composition of its proposals, resulting in a very heterogeneous and multifaceted theoretical set. This paper aim is to analyze the diffusion of eugenic ideas in the speeches of the Sociedade de Medicina de Porto Alegre. Having as object of study the institution’s journal, Archivos Rio Grandenses de Medicina, we seek to understand how eugenic science shaped the view of certain physicians not only on the subjects like venereal diseases and infectious-contagious pathologies, but also on topics such as reproduction, motherhood, early childhood, addictions and mortality.

Introdução: O impacto positivo no curto e longo prazos do aleitamento materno exclusivo (AME) na saúde da criança, da mulher que amamenta e para a sociedade estão bem documentados. Apesar da tendência secular crescente do AME no Brasil, a prevalência de 36,6 por cento constatada na última Pesquisa Nacional de Saúde, realizada em 2013, está aquém dos 50 por cento considerados satisfatórios pela Organização Mundial da Saúde. Mediante esse cenário, faz-se importante identificar fatores de risco modificáveis para reduzir a interrupção precoce do AME. Embora o uso de chupeta seja apontado como um dos múltiplos determinantes do AME, não existe consenso sobre o efeito desse hábito na interrupção precoce dessa prática. Objetivo: Analisar a associação entre uso de chupeta e interrupção do AME, bem como a evolução do uso de chupeta e sua influência no AME em menores de 6 meses nas Capitais Brasileiras e no DF, 1999- 2008. Métodos: Foram produzidos 4 manuscritos. O primeiro consistiu de uma revisão sistemática e meta-análise para investigar a associação entre uso de chupeta e interrupção do AME nos menores de 6 meses. Para tanto, realizou-se uma busca ampla em cinco bases de dados (CINAHL, Scopus, Web of Science, LILACS, Medline) sem restrição de data ou idioma de publicação, o que resultou em 1.866 estudos submetidos à critérios de exclusão e inclusão previamente estabelecidos (Protocolo PROSPERO CDR42014014527). O segundo e o terceiro manuscritos utilizaram dados provenientes da I e II Pesquisa de Prevalência de Aleitamento Materno (PPAM) nas Capitais e DF realizadas em 1999 e 2008, respectivamente. Os inquéritos, metodologicamente equivalentes, utilizaram amostras representativas com sorteio em dois estágios. Os questionários foram compostos por questões fechadas relativas ao uso de chupeta e à alimentação da criança no dia anterior à pesquisa. No manuscrito 2, para testar a associação entre o uso de chupeta e o risco de interrupção do AME, foram utilizados modelos de regressão múltipla na amostra agrupada e para cada inquérito, ajustados por covariáveis socioeconômicas, demográficas e biomédicas da mãe da criança. No terceiro manuscrito, calculou-se a fração de impacto potencial (FIP) da influência da variação temporal do uso de chupeta no declínio da interrupção do AME. O manuscrito 4 resultou do aprofundamento na metodologia aplicada no manuscrito anterior, no qual se discute como as estimativas de FIP podem ser utilizadas na análise de mudanças em desfechos populacionais à luz de variações em seus fatores determinantes, tomando como exemplo os dados de AME e uso de chupeta no Brasil. Resultados: O efeito agrupado do uso de chupeta sobre a interrupção do AME em crianças menores de 6 meses dos estudos incluídos na meta-análise foi de OR 2,48 (IC95 por cento =2,16-2,85); entretanto, constatou-se alta heterogeneidade, explicada majoritariamente pelo delineamento do estudo (40,2 por cento ). Os dois ensaios clínicos randomizados incluídos apresentam validade externa limitada e encontraram associação nula; os 44 observacionais, incluindo 20 estudos de coorte prospectivos, encontraram associação consistente (OR=2,28; IC95 por cento =1,78-2,93). Essa associação foi confirmada pela análise agrupada das amostras da I e II PPAM (manuscrito 2), revelando o uso de chupeta como o fator mais fortemente associado à interrupção precoce do AME no Brasil no período de 1999 a 2008 (OR ajustado= 2,77; IC95 por cento =2,63-2,91). Nesse período, no Brasil, houve declínio de 15,2 pontos percentuais na prevalência de interrupção do AME (passando de 74,9 por cento para 59,7 por cento ) e uma redução de aproximadamente 17 pontos percentuais no uso de chupeta (passando de 58,5 por cento para 41,6 por cento ). Um terço do declínio na interrupção precoce do AME pôde ser atribuído à variação temporal do uso de chupeta (manuscrito 3). A redução no uso de chupeta como estratégia de prevenção para interrupção do AME no cenário 1999-2008 apresentou 41 por cento de eficácia potencial, 13,3 por cento de efetividade e 18,5 por cento de eficiência (manuscrito 4). Conclusões: O uso de chupeta é um fator de risco associado à interrupção do AME em crianças menores de 6 meses, sendo um forte determinante no declínio temporal da interrupção precoce dessa prática no Brasil. Como um fator de risco modificável, revelou-se seu potencial na prevenção da interrupção precoce do AME. Estratégias preventivas com abordagem universal e seletiva para redução do uso de chupeta em crianças amamentadas exclusivamente poderiam acelerar a melhoria das taxas da AME no Brasil, rumo às recomendações internacionais Introduction: The short- and long-term health positive impact that exclusive breastfeeding (EBF) confers to children are well documented. Despite the secular trend for EBF in Brazil has increased, the prevalence of 36 per cent observed in the last national survey conducted in 2013 is still below the 50 per cent considered satisfactory by the World Health Organization. In this scenario, it is important to identify relevant modifiable key risk factors for the premature interruption of EBF. Although pacifier use has been pointed as one of the multiple determinants of exclusive breastfeeding there is no consensus on the effect of this habit in the early interruption of EBF. Objective: To analyze the association between pacifier use and EBF interruption as well as the influence of temporal trends of pacifier use on the EBF in children under 6 months old in Brazilian state capitals and Federal District, 1999-2008. Methods: 4 manuscripts were produced. The first consisted in systematic review and meta-analysis to investigate the association between pacifier use and interruption of exclusive breastfeeding in infants less than 6 months. A wide search in 5 databases (CINAHL, Scopus, Web of Science, LILACS, Medline) from inception through 30 December 2104 without restriction of language yielded 1.866 publications submitted to predetermined inclusion/exclusion criteria peer reviewed (PROSPERO protocol CDR42014014527). In the second and third manuscripts data from 2 waves of infant feeding surveys conducted in 1999 and in 2008 in the Brazilian state capitals and Federal District (I and II PPAM) was used. Methodologically equivalent surveys used representative samples selected based on complex sampling procedures and systematic random selection of children in the queue of each selected immunization center. Questionnaires were composed of closed-ended questions regarding the pacifier use and infant feeding on the day before the survey. In the second manuscript, to find out whether or not pacifier use is an independent risk factor for the EBF interruption were used multivariate regression models in the pooled sample and for each survey wave adjusting for socioeconomic, demographic and biomedical confounders. In the third paper, we calculated the potential impact fraction (IF) of the influence of temporal trends of pacifier use on the decline of EBF interruption. Manuscript 4 resulted from methodological deepening for application of the analysis strategy applied in the previous manuscript, into it we present how estimates of IF that might be used in the analysis of variation in the population outcomes considering shift in its determinants, taking as an example the data about EBF interruption and pacifier in Brazil. Results: The meta-analysis pooled effect was OR 2.48 (CI95 per cent =2.16-2.85) for the association between pacifier use and EBF interruption. The majority heterogeneity was explained by study design (40.2 per cent ). Two RCTs with very limited external validity found a null association, but 44 observational studies, including 20 prospective cohort studies, did find a consistent association between pacifier use and EBF interruption (OR=2.28; CI95 per cent =1.78-2.93). This association was confirmed by the pooled analysis for I and II PPAM (manuscript 2) revealing pacifier use as the strongest risk factor for EBF interruption in Brazil between 1999-2008 (OR adjusted= 2.77; CI95 per cent =2.63-2.91). In this period was observed 15.2 per cent of decline in EBF interruption prevalence (from 74.9 per cent to 59.7 per cent ) and a reduction of approximately 17 per cent in pacifier use (from 58.5 per cent to 41.6 per cent ). A third of the total decline of EBF interruption could be attributed to the temporal trends of pacifier use (manuscript 3). The decrease in the pacifier use as a prevention strategy for EBF interruption in the scenario 1999-2008 showed 41 per cent of potential effectiveness, 13.3 per cent effective and 18.5 per cent efficiency (manuscript 4). Conclusion: Pacifier use is a risk factor associated with EBF interruption, being a strong determinant in the temporal trends of EBF in Brazil. As a modifiable risk factor, the pacifier proved its potential in preventing EBF interruption. Preventive public health strategies with ecological and selective approach to reduce the pacifier use in exclusive breastfed infants could accelerate the improvement of the EBF rates in Brazil, towards the international recommendations

Immature teratoma of the ovary is a malignant neoplasm composed of undifferentiated tissue that arises from embryonic cells. And the mature teratoma is a benign neoplasm composed of differentiated tissues. This paper aims to report the case of a 21-year-old pacient with giant immature teratoma in left ovary associated with mature ovarian teratoma bilateral, due to the association of teratoma with mature teratoma ipsilateral and contralaterally, combined with its outstanding increased size (36 cm), is unusual. Exploratory laparotomy, with cytoreductive pan-hysterectomy and bilateral salpingooferectomia were performed. The patient received adjuvant chemotherapy with BEP regimen (bleomycin, etoposide and cisplatin) in four cycles with intervals of 21 days, coursing with good evolution

O exame de palpação é um procedimento no qual um profissional da saúde pressiona uma região específica do corpo de um paciente com os dedos a fim de detectar a presença ou ausência de características e anormalidades sob a pele. Um simulador que forneça subsídios para o treinamento deste procedimento pode contribuir para o aprendizado da técnica e o aprimoramento de sua execução em pacientes, além de, dentre outras vantagens, eliminar a necessidade de exposição de pacientes aos riscos das situações reais, minimizar o uso de objetos físicos e tornar possível a simulação de diferentes casos e ações. O objetivo principal deste trabalho consiste em desenvolver um simulador de palpação mamária com retorno háptico realista. No contexto do exame de palpação, existe uma lacuna no que concerne a definição de técnicas e parâmetros de retorno de força que ofereçam uma experiência mais realista de interação para o usuário. Após uma revisão sistemática da literatura foi feito o mapeamento dos principais parâmetros para representar diferentes nódulos durante o treinamento virtual visando ao diagnóstico de anomalias mamárias. Tais parâmetros foram equacionados com a finalidade de obter uma representação realista em um dispositivo háptico. Além disso, interfaces foram implementadas para permitir a geração de casos com a variação desses parâmetros referentes aos nódulos (tamanho, consistência, localização e contorno). Os resultados obtidos a partir de uma avaliação com uma profissional da medicina mostraram que o retorno de força aplicado a cada uma das características foi realista para todos os parâmetros, com limitação na representação de nódulos espiculados, que deve ser melhorada. Além de contribuir com a área de Computação no sentido de ter definido técnicas e parâmetros de retorno de força importantes para a simulação de exames de palpação, o trabalho disponibiliza uma ferramenta que pode contribuir para a aquisição efetiva da habilidade necessária para a execução do procedimento em questão Palpation exam is a procedure in which a health care professional presses a specific region of a patient\'s body with the fingers in order to detect the presence of features and abnormalities under the skin. A simulator that aids the training this procedure may contribute to the learning of the technique and the improvement of its implementation in real patients. The main objective of the Master\'s degree work proposed in this paper is to develop a breast palpation simulator with haptic feedback. The results of a Systematic Review conducted in order to assess the state-of-art of the simulation of the palpation procedure showed that despite being a new area, there are already many innovative works that received positive reviews. However, in the context of palpation examination, a gap exists regarding the definition of techniques and force feedback parameters to provide a more realistic experience to the user. After a Systematic Review from the literature the mapping of the main parameters was done to represent different nodules during the virtual training for the diagnosis of breast abnormalities. These parameters were transformed into equations with the goal of obtaining a realistic representation on a haptic device. Besides, user interfaces were implemented to allow the creation of cases with the variation of the parameters relative to the nodules (size, consistency, localization and contour). The results obtained from an assessment done with a medical professional showed that the force feedback applied to each one of the characteristics was realistic to all the parameters, with limitations on the representation of spiculated nodules, which must be improved. Aside from the contribution to the Computing area in the sense that force feedback techniques and parameters important to the simulation of palpation exams were defined, this work offers a tool that can contribute to the effective acquisition of the necessary skills to the execution of the procedure in question

Introdução: O café é uma das bebidas mais apreciadas e consumidas no mundo, por suas características organolépticas e efeito estimulante. Pelos potenciais efeitos na saúde causados por esta bebida surgiu, desde cedo, o interesse da comunidade científica. Como ainda ha controvérsias a respeito do café quanto aos seus reais efeitos, justifica-se este estudo em voluntários saudáveis e consumidores habituais de café. Objetivo: Comparar o consumo de dois diferentes graus de torras de café (torra média e escura) em 3 tipos de café: 100% arábica, blend e blend descafeinado e seus efeitos sobre o perfil metabólico em indivíduos saudáveis. Métodos: Em estudo prospectivo, foram avaliados 70 indivíduos sem qualquer doença associada, sendo 50 mulheres, com idade média de 47 ± 12 anos. Durante o período de seguimento no estudo, os voluntários que preencheram os critérios de inclusão, assinaram o Termo de Consentimento Livre e Esclarecido e iniciaram no estudo, que teve um total de 77 dias. Cada voluntário realizou quatro visitas (T0, T1, T2 e T3). Primeiramente, os participantes foram randomizados para cada tipo de café: 100% arábica, blend ou blend descafeinado. Depois uma nova randomização foi feita para estabelecer a ordem de consumo dos dois graus de torra (média e escura), num estudo do tipo crossover. Na visita T0, o participante foi orientado a parar a ingestão do café ou qualquer alimento fonte de cafeína por 21 dias. Na visita T1 foram randomizados para iniciar o consumo de café filtrado primeiro com um tipo de torra (torra média ou torra escura) por 4 semanas e então com \"crossover\" para o outro tipo (visita T2), com um período total de 8 semanas de consumo de café. O café foi fornecido aos pacientes, sendo do mesmo tipo, do mesmo produtor e com a forma de preparo padronizada e consumo diário de café de 450-600 ml/dia. Após período de \"washout\" (basal) e após cada período de tomada de café por tipo de torra, os pacientes foram submetidos aos exames laboratoriais (Colesterol total e frações, triglicérides, glicemia de jejum, PCR, Lp(a), homocisteína e acido fólico) e avaliação antropométrica (peso e IMC). Resultados: Foi observado aumento do colesterol total, do LDL e do HDL no grupo que consumiu café do tipo Blend, para os dois tipos de torra quando comparados ao basal. Houve aumento significativo da homocisteína no grupo que consumiu café 100% arábica e Blend. No grupo que consumiu café 100% arábica houve aumento significativo da homocisteína somente na torra média quando comparado ao basal. Já no grupo que consumiu o café tipo blend a diferença significativa foi apenas entre as duas torras escura e média, na qual a escura apresentou valor menor que a torra média. Conclusão: Café promove aumento discreto nas dosagens de homocisteína quando consumido o café blend e 100% arábica. Aumentou também o colesterol total, LDL, e ao mesmo tempo o HDL, quando consumido café tipo Blend. Apesar de significativas, as alterações no perfil metabólico foram muito discretas. Seria muito difícil determinar a influência que o café tem no metabolismo lipídico dos indivíduos, mas talvez esse discreto aumento juntamente com algum efeito antioxidante pode contribuir para a redução de morte por doenças cardiovasculares como já foi observado em estudos epidemiológicos Introduction: Coffee is one of the most popular beverages in the world. Given its high consumption, potential health effects caused by this beverage brought the interest of the scientific community. Considering the current knowledge and controversies about drinking coffee daily, justifies this study in healthy volunteers and habitual coffee drinkers. Objectives: To compare the consumption of two different coffee roasts degrees (medium and dark roast) on 3 types of coffee: 100% arabic, blend and decaffeinated blend coffee and its effects on metabolic profile in healthy subjects. Results: 70 healthy subjects, age 47 ± 12 years old participated in the trial. Most subjects were female (71,4%). This randomized crossover clinical trial lasted 77 days. All the volunteers performed four visits (T0 - T1 - T2 -T3). In the visits, the participants had blood samples taken, held clinical examinations and nutritional evaluation, aside from receiving orientation. Randomly, participants should drink 100% Arabic, blend coffee or decaffeinated coffee. Then, in a second stage, again randomly, volunteers were draw to see which roast they would start drinking. In T0, subjects were oriented to stop consuming all the foods and beverages, which contained coffee or caffeine for 21 days. In T1 subjects start consuming medium or dark roast paper-filtered coffee for 4 weeks. In T2 they start the other roast for another additional 4 weeks. In T3, subjects stopped the coffee consumption, thus the study was completed. Participants received the coffee powder according to randomization criteria previously described. It is the same kind of coffee from the same producer and way to prepare. The coffee daily consumption were between 450 -600 ml. After \"washout\" period and after each period drinking coffee, all the subjects had sample bloods taken, to assess total cholesterol and fractions, triglycerides, glycaemia, homocisteine, acid folic and anthropometric (weight and BMI - body mass index). After analyzing the results, we observed an increase in total cholesterol, LDL and HDL levels after blend coffee intake. A significative increase in homocisteine levels after 100% Arabic and blend coffee intake. The significant difference in the group that consumed 100% arabic coffee were in between basal and medium roast. But in in group that consumed blend coffee this significant difference in homocisteine were in between the two roasts, higher during medium roast. Conclusions: Those results demonstrate a slight increase in homocisteine, when consumed blend coffee and 100% Arabic coffee. Also increased total cholesterol and LDL but at the same time increased the HDL, when they drank blend coffee. Despite significant, those metabolic alterations were discrete. It is difficult to determine the coffee influence on lipid metabolism, but maybe this slight increase tin HDL together with some antioxidant effect can contribute to the reduction of death from cardiovascular disease as already noted in epidemiologic studies

In the present paper the behavior of the heterochromoso-mes in the course of the meiotic divisions of the spermatocytes in 15 species of Orthoptera belonging to 6 different families was studied. The species treated and their respective chromosome numbers were: Phaneropteridae: Anaulacomera sp. - 1 - 2n = 30 + X, n +15+ X and 15. Anaulacomera sp. - 2 - 2n - 30 + X, n = 15+ X and 15. Stilpnochlora marginella - 2n = 30 + X, n = 15= X and 15. Scudderia sp. - 2n = 30 + X, n = 15+ X and 15. Posldippus citrifolius - 2n = 24 + X, n = 12+X and 12. Acrididae: Osmilia violacea - 2n = 22+X, n = 11 + X and 11. Tropinotus discoideus - 2n = 22+ X, n = 11 + X and 11. Leptysma dorsalis - 2n = 22 + X, n = 11-J-X and 11. Orphulella punctata - 2n = 22-f X, n = 11 + X and 11. Conocephalidae: Conocephalus sp. - 2n = 32 + X, n = 16 + X and 16. Proscopiidae: Cephalocoema zilkari - 2n = 16 + X, n = 8+ X and 8. Tetanorhynchus mendesi - 2n = 16 + X, n = 8+X and 8. Gryliidae: Gryllus assimilis - 2n = 28 + X, n = 14+X and 14. Gryllodes sp. - 2n = 20 + X, n = 10- + and 10. It was pointed out by the present writer that in the Orthoptera similarly to what he observed in the Hemiptera the heterochromosome in the heterocinetic division shows in the same individual indifferently precession, synchronism or succession. This lack of specificity is therefore pointed here as constituting the rule and not the exception as formerly beleaved by the students of this problem, since it occurs in all the species referred to in the present paper and probably also m those hitherto investigated. The variability in the behavior of the heterochromosome which can have any position with regard to the autosomes even in the same follicle is attributed to the fact that being rather a stationary body it retains in anaphase the place it had in metaphase. When this place is in the equator of the cell the heterochromosome will be left behind as soon as anaphase begins (succession). When, on the contrary, laying out of this plane as generally happens (precession) it will sooner be reached (synchronism) or passed by the autosomes (succession). Due to the less kinetic activity of the heterochromosome it does not orient itself at metaphase remaining where it stands with the kinetochore looking indifferently to any direction. At the end of anaphase and sometimes earlier the heterochromosome begins to show mitotic activities revealed by the division of its body. Then, responding to the influence of the nearer pole it moves to it being enclosed with the autosomes in the nucleus formed there. The position of the heterochromosome in the cell is explained in the following manner: It is well known that the heterochromosome of the Orthoptera is always at the periphery of the nucleus, just beneath the nuclear membrane. This position may be any in regard of the axis of the dividing cell, so that if one of the poles of the spindle comes to coincide with it, the heterochromosome will appear at this pole in the metaphasic figures. If, on the other hand, the angle formed by the axis of the spindle with the ray reaching the heterochromosome increases the latter will appear in planes farther and farther apart from the nearer pole until it finishes by being in the equatorial plane. In this way it is not difficult to understand precession, synchronism or succession. In the species in which the heterochromosome is very large as it generally happens in the Phaneropteridae, the positions corresponding to precession are much more frequent. This is due to the fact that the probabilities for the heterochromosome taking an intermediary position between the equator and the poles at the time the spindle is set up are much greater than otherwise. Moreover, standing always outside the spindle area it searches for a place exactly where this area is larger, that is, in the vicinity of the poles. If it comes to enter the spindle area, what has very little probability, it would be, in virtue of its size, propelled toward the pole by the nearing anaphasic plate. The cases of succession are justly those in which the heterochromosome taking a position parallelly to the spindle axis it can adjust its large body also in the equator or in its proximity. In the species provided with small heterochromosome (Gryllidae, Conocephalidae, Acrididae) succession is found much more frequently because here as in the Hemiptera (PIZA 1945) the heterochromosome can equally take equatorial or subequatorial positions, and, furthermore, when in the spindle area it does offer no sereous obstacle to the passage of the autosomes. The position of the heterochromosome at the periphery of the nucleus at different stages may be as I suppose, at least in part a question of density. The less colourability and the surface irregularities characteristic of this element may well correspond to a less degree of condensation which may influence passive movements. In one of the species studied here (Anaulacomera sp.- 1) included in the Phaneropteridae it was observed that the plasmosome is left motionless in the spindle as the autosomes move toward the poles. It passes to one of the secondary spermatocytes being not included in its nucleus. In the second division it again passes to one of the cells being cast off when the spermatid is being transformed into spermatozoon. Thus it is regularly found among the tails of the spermatozoa in different stages of development. In the opinion of the present writer, at least in some cases, corpuscles described as Golgi body's remanents are nothing more than discarded plasmosomes.

A new high performance liquid chromatographic method has been established for simultaneous determination of chlorogenic acid, caffeic acid and caffeine in hydroalcoholic and aqueous extracts of Ilex paraguariensis. Analytical curves showed good linear regression in the concentration ranges 0.49-7.8 µg/mL for chlorogenic acid, 0.25-3.9 µg/mL for caffeic acid and 0.244-7.8 µg/mL for caffeine. Reduction of the DPPH· radical was used to determine the antioxidant capacity of the extracts. Our method for the simultaneous determination of chlorogenic acid, caffeic acid and caffeine was highly sensitive, having lower detection and quantitation limits than other papers that used similar methodology.Um novo método de cromatografia líquida de alta eficiência foi desenvolvido para a determinação simultânea de ácido clorogênico, ácido caféico e cafeína no extrato hidroalcoólico e aquoso de Ilex paraguariensis. As curvas de calibração mostraram boa regressão linear nas faixas de concentração 0,49-7,8 µg/mL para o ácido clorogênico, 0,25-3,9 µg/mL para ácido caféico e 0,244-7,8 µg/mL para cafeína. A redução do radical DPPH· foi usada para determinar a capacidade antioxidante dos extratos. Nosso método para a determinação simultânea de ácido caféico, ácido clorogênico e cafeína foi altamente sensível, possuindo limites de detecção e de quantificação menores do que em outros trabalhos que empregaram metodologias semelhantes.

O presente estudo teve como objetivo analisar a integração da assistência prestada aos doentes de tuberculose (TB) a partir de instrumentos de registro em Ribeirão Preto-SP. Estudo exploratório com abordagem quantitativa. A população do estudo constituiu-se de portadores de TB notificados e residentes no município que finalizaram o tratamento entre 2012 e 2013, exceto os pacientes pertencentes ao sistema prisional, transferidos ou que tiveram mudança de diagnóstico. A pesquisa foi realizada por meio da coleta de dados secundários em 4 fontes: no sistema de notificação estadual (TB-WEB) para identificação dos doentes de TB; no prontuário clínico dos doentes selecionados, na ficha de acompanhamento do tratamento supervisionado (ficha amarela) e no livro registro de pacientes e acompanhamento de tratamento dos casos de tuberculose (livro verde), que permitiu acesso aos registros referentes às informações clínicas, resultados de exames e situação de encerramento. As variáveis selecionadas foram agrupadas de acordo com as dimensões de elenco de serviços e coordenação da assistência. Os dados foram analisados por meio de técnicas de estatística descritiva. Os resultados mostraram que, dentre os serviços ofertados, 72,5% dos doentes passaram por 5 ou mais consultas médicas, enquanto 9,2% tiveram de 1 a 2 consultas com o enfermeiro. Os registros das orientações de retorno às consultas realizada pela equipe médica (96,3%) e de enfermagem (84,4%) foi a que prevaleceu nos prontuários. O procedimento mais realizado pelo médico foi a evolução clínica (97,2%) e pela equipe de enfermagem foi a pré e pós consulta (69,7%). Segundo dados do prontuário e da ficha amarela, 75,2% receberam tratamento supervisionado e o predomínio da supervisão foi no domicílio (57,8%). A baciloscopia de escarro foi realizada em 89,0% dos doentes e o exame Anti-HIV em 69,7% dos casos. A cesta básica foi ofertada a 23,8% e 18,3% dos doentes receberam atendimento com assistente social. Em 71,3% dos casos não houve registros sobre o controle de comunicantes. Com relação ao acompanhamento dos casos, 29,4% apresentaram faltas em consultas agendadas e a conduta que prevaleceu para estas situações foi a visita domiciliar (10,1%). Com relação à utilização de outras unidades de saúde, 10,1 % dos doentes foram encaminhados para outros serviços pela unidade que realizou o tratamento e os demais atendimentos ocorreram sem que houvesse encaminhamentos (24,8%). A análise dos prontuários, Livro Verde e Ficha Amarela revelou um baixo número de registros e inúmeras lacunas no seu preenchimento tanto com relação ao acompanhamento dos casos, bem como das ações ofertadas pelos serviços de saúde no decorrer do tratamento. Tal fato dificulta a integração e continuidade do cuidado prestado e pode prejudicar a verificação da proporção real de serviços que foram ofertados e utilizados pelos doentes. Deste modo, verifica-se a necessidade do adequado preenchimento dos instrumentos a fim de facilitar a elaboração de estratégias de planejamento da assistência por parte da coordenação dos programas. O cuidado integral centrado no usuário e uma adequada articulação dos serviços de saúde podem melhorar o atendimento prestado aos doentes de TB This study aimed to analyze the integration of assistance to tuberculosis patients (TB) from recording instruments in Ribeirão Preto. Exploratory study with a quantitative approach. The study population consisted of TB patients notified and living in the city who completed treatment between 2012 and 2013, excluding patients in the prison system, transferred or had their diagnoses changed. The survey was conducted by collecting secondary data sources in 4: in the state reporting system (TBWEB) for identification of TB patients; in the clinical records of patients selected in the covering sheet of supervised treatment (yellow plug) and in the book registry of patients and follow-up treatment of tuberculosis cases (Green Paper), which allowed access to records related to medical information, test results and foreclosure situation. The selected variables were grouped according to the cast of dimensions of services and care coordination. Data were analyzed using descriptive statistics. The results showed that among the services offered, 72.5% of patients underwent five or more medical visits, while only 9.2% had 1-2 consultations with the nurse. The return guidance to consultations held by the medical staff (96.3%) and nursing (84.4%) was the one that prevailed. The procedure most often performed by the physician was the clinical outcome (97.2%) and nursing staff was the pre and post consultation (69.7%). According to data from medical records and the yellow plug, 75.2% received supervised treatment and the predominance of supervision was at home. Sputum smear microscopy was performed in 89.0% of patients and the Anti-HIV test in 69.7% of cases. The basket was offered to 23.8% and 18.3% of patients received care with social worker. In 71.3% of cases there were no reports on the control contacts. Regarding the monitoring of cases, 29.4% had faults in scheduled appointments and conduct that prevailed for these situations was the home visit (10.1%). Only 10.1% of patients were referred to other services by the unit that carried out the treatment and other care occurred without any referrals (24.8%). The analysis of medical records, Green and Yellow Sheet revealed a low number of records and numerous gaps in their fulfillment both in relation to the monitoring of cases, as well as the shares offered by health services during treatment. This makes the integration and continuity of care provided and impairs verification of the actual proportion of services that were offered and used by patients. Thus, there is a need for adequate filling of the instruments in order to facilitate the development of planning strategies of assistance from the coordination of programs. The full user-centered care and an appropriate coordination of health services can improve the care provided to TB patients

O processo de envelhecimento pode gerar perdas cognitivas que, se agravadas, podem acarretar o diagnóstico de demência, sendo a Demência de Alzheimer (DA) a mais comum. A DA é uma doença neurodegenerativas que afeta principalmente a memória e a linguagem. Procedimentos de ensino baseados no paradigma de equivalência de estímulos e no ensino por exclusão, apesar de testados com sucesso em diferentes populações com problemas de desenvolvimento, têm gerado poucas pesquisas com idosos com perdas cognitivas. Esta pesquisa apresenta três estudos, com objetivos distintos, mas complementares. O Estudo 1 teve como objetivo avaliar se idosos institucionalizados sem DA formariam classes de estímulos equivalente a partir de um procedimento de ensino de relações condicionais entre palavras ditadas, fotos, nomes impressos de profissões e de graus de parentesco; adicionalmente, pretendia-se verificar que essas relações se mantinham ao longo de um mês, em três testes de manutenção. Cinco idosos institucionalizados passaram pelo ensino de relações arbitrárias, em uma estrutura de treino linear com ensino de novas relações por exclusão. Testes de equivalência foram conduzidos em seguida à Fase de Ensino, e após cinco, 15 e 30 dias. Todos os idosos demonstraram desempenho por exclusão na Fase de Ensino e aprenderam todas as relações com o número mínimo de blocos de ensino programados (exceto uma participante). Todos os idosos apresentaram formação de classes de equivalência. Quatro dos idosos apresentaram manutenção dessas classes em todos os testes de manutenção. O procedimento, portanto, mostrou-se efetivo para esses idosos e gerou formação de equivalência, apesar da sua condição de institucionalização. O Estudo 2 teve como objetivo avaliar se idosos com DA, também institucionalizados, demonstrariam desempenho por exclusão e aprenderiam relações condicionais entre quatro nomes ditados e quatro fotos, em um procedimento de ensino por exclusão. Seis idosas com diagnóstico de DA passaram por tarefas de emparelhamento ao modelo, em que as relações entre modelos ditados e fotos foram ensinadas por exclusão. Apesar de todas as idosas terem respondido por exclusão na primeira apresentação de um estímulo modelo novo e seu comparação correspondente em pelo menos duas das três tentativas em que tal desempenho era possível, apenas duas mantiveram essa relação ao longo das demais tentativas e aprenderam as relações ensinadas. Possíveis motivos para esse desempenho são discutidos. O Estudo 3 avaliou se idosos institucionalizados, com DA e comprometimento cognitivo, formariam classes de equivalência a partir de um ensino de relações arbitrárias com estrutura de treino um-para-muitos, procedimento de dica atrasada e ensino por exclusão. Quatro idosas passaram pelo ensino de relações condicionais entre estímulos visuais (nomes escritos, fotos e nomes de profissões). Todas aprenderam as relações ensinadas com o número mínimo de blocos programados, porém apresentaram dificuldade em formar classes de equivalência e manter tais relações nos testes de manutenção após cinco e oito dias. Os procedimentos de ensino empregados foram eficientes em gerar a aprendizagem das relações, mas tal aprendizagem, nessas idosas, não sustentou a formação e manutenção de classes de equivalência. The aging process can cause cognitive impairments that, if aggravated, may lead to the diagnosis of dementia, with Alzheimers Dementia (AD) being the most common. Alzheimers Dementia is a neurodegenerative disease that primarily affects memory and language. Teaching procedures based on the paradigm of stimulus equivalence and on teaching by exclusion, although successfully tested in different populations with developmental problems, have generated little research with elderly people with cognitive impairments. This paper presents three studies, with different but complementary aims. Study 1 aimed to assess whether institutionalized elderly people without AD would form classes of equivalent stimuli from a procedure of teaching conditional relationships among dictated words, photos, printed names of professions and degrees of relatedness; additionally, it aimed to verify whether these relationships would be maintained over a month, using three maintenance tests. Five hospitalized elderly subjects underwent teaching of arbitrary relationships in a linear training structure with the teaching of new relationships by exclusion. Equivalence tests were conducted following the teaching phase, and after five, 15 and 30 days. All the elderly subjects demonstrated performance by exclusion in the teaching phase and learned all the relationships within the minimum number of programmed teaching blocks (except for one participant). All the subjects presented the formation of equivalence classes. Four of the elderly subjects presented maintenance of these classes in all the maintenance tests. The procedure, therefore, was effective for these elderly subjects and generated equivalence formation, despite their hospitalization condition. Study 2 aimed to assess whether institutionalized AD patients would demonstrate performance by exclusion and learn the conditional relationships between four dictated names and four photos, in a teaching by exclusion procedure. Six elderly women diagnosed with AD underwent pairing to the sample tasks, in which the relationships between dictated sample and photos were taught by exclusion. Despite all the elderly women responding by exclusion in the first presentation of a new sample stimulus and its corresponding comparison in at least two of the three trials in which this performance was possible, only two maintained this relationship throughout the other trials and learned the relationships taught. Possible reasons for this behavior are discussed. Study 3 evaluated whether hospitalized elderly subjects, with AD and cognitive impairments, would form equivalence classes from the teaching of arbitrary relationships using a one-to-many training structure, a procedure of delayed hints and teaching by exclusion. Four elderly subjects underwent the teaching of conditional relationships among visual stimuli (written names, photos and names of professions). All learned the relationships taught within the minimum number of programmed blocks, however, they presented difficulty forming equivalence classes and maintaining such relationships in the maintenance tests at five and eight days. The teaching procedures employed were efficient in generating the learning of relationships, however, such learning, in these elderly subjects, did not support the formation and maintenance of equivalence classes.

O conhecimento anatômico da mandíbula, do canal mandibular e outros reparos presentes na região mandibular são essenciais para o sucesso em vários procedimentos odontológicos, variando desde o mais básico como a anestesia, até procedimentos mais complexos, tais como a instalação de implantes, cirurgias ortognáticas e até mesmo cirurgias para correções estéticas de ângulo mandibular. O objetivo deste trabalho foi analisar as distâncias presentes da cortical vestibular mandibular, até a cortical vestibular do canal mandibular em 3 regiões específicas além da avaliação da região de fusão entre o córtex vestibular e lingual em exames de TCFC. Foram selecionadas 100 imagens de exames de TCFC pertencentes ao arquivo de imagens do Departamento de Estomatologia da Faculdade de Odontologia de Bauru. Para análise das medidas foi estudado o canal mandibular sendo investigadas as distâncias em pontos específicos que seriam de grande importância durante a realização de osteotomias em ramo e corpo mandibular, bem como a possibilidade de injúria neurosensorial. A distância da cortical vestibular do canal mandibular até a cortical vestibular da mandíbula foi de 2.95 mm na região da entrada do NAI na mandíbula, 3.88 mm na região de transição do ramo mandibular para corpo mandibular e 4,72 mm para a região mesial de segundos molares. A distância média da cortical lingual do canal mandibular até a cortical lingual da mandíbula foi de 2.07 mm na região de transição de ramo mandibular para corpo mandibular e 2.22 mm na região mesial de segundos molares. A distância da cortical do soalho do canal mandibular até a base mandibular foi correspondente a 8.54 mm na área de transição de ramo mandibular para corpo mandibular e de 6.97 mm na região mesial de segundos molares. A distância média da cortical do teto do canal mandibular até a cortical óssea superior da mandíbula na região de transição de ramo mandibular para corpo mandibular foi de 16.35 mm e de 16.08 mm para a zona corresponde à região mesial de segundos molares. No forame mandibular, local de entrada do NAI na mandíbula, a média obtida para o diâmetro do canal foi de 2.78 mm, enquanto que na região de transição entre ramo mandibular e corpo mandibular a média para o diâmetro do mesmo foi de 3.85 mm e na região mesial de segundo molar esta medida foi de 2.99 mm. Em 16% dos pacientes analisados havia algum tipo de variação anatômica presente no canal mandibular, fosse esta através de bifurcação ou trifurcação. A média de altura onde ocorre a fusão entre as placas cortical mandibular vestibular e lingual, acima do forame mandibular foi de 8,31mm e 8,48 mm para os exames realizados nos ramos mandibulares do lado esquerdo e direito respectivamente. Podemos concluir que os reparos anatômicos, assim como o conhecimento das medidas médias da anatomia do corpo humano são de grande valia durante a realização de procedimentos cirúrgicos, porém estes não podem e não devem ser utilizados de maneira aleatória pelo cirurgião, a individualidade e as variações anatômicas dos pacientes devem ser analisadas previamente ao planejamento cirúrgico, sendo que os materiais presentes no mercado para fixação interna não condizem com o esperado de acordo com a média anatômica do país encontrada em nosso estudo. The anatomical knowledge of the jaw, the mandibular canal and other anatomical landmarks in the mandibular region are essential for success in various dental procedures, from anesthesia to implants insertions, orthognatic surgery and aesthetic corrections of mandibular angle.The aim of this paper was to locate the mandibular canal to the mandibular corticals canal in 3 specific regions and to evaluate union between the buccal and lingual cortical. We selected 100 CBCT exams of belonging to the image files of the Stomatology Department of the School of Dentistry in Bauru - Brazil.The distance of the mandibular canal cortical to the buccal plate of the mandible was 2.95 mm in the area of the entrance to the alveolar inferior nerve (AIN) into the mandibula, 3.88 mm in the transition region of the mandibular ramus to mandibular body and 4.72 mm for mesial region of second molars.The average distance of mandibular canal corticalto the lingual plate of mandible was 2.07 mm in the transition region of mandibular ramus to mandibular body and 2.22 mm in the mesial of second molars. The distance from the floor of the cortical mandibular base canal was corresponding to 8.54 mm in the area of transition of mandibular ramus to mandibular body and 6.97 mm in the mesial of second molars.The average distance from the ceiling of the cortical mandibular canal until the upper bone in mandibular cortical transition region of mandibular ramus for mandibular body was 16.35 mm and 16.08 mm for zone corresponds to the mesial area of second molars. In the mandibular foramen, place of entry of AIN in the mandibular the average obtained for the diameter of the canal was of 2.78 mm, while in the transition region between mandibular ramus and mandibular body the average for the same diameter was 3.85 mm and in the mesial of second molar this measure was 2.99 mm. In 13 of the patients surveyed had some sort of anatomical variation present in the mandibular canalwas this through bifurcation or trifurcation. The average height where the merger occurs between the cortical vestibular and lingual mandibular plates above the mandibular foramen was 8.31 mm and 8.48 mm for exams conducted in mandibular ramus on the left and right respectively. We can conclude that the repairs, as well as the anatomical knowledge of the average measures of anatomy of the human body are of great value during surgical procedures, but these cannot and should not be used randomly by the surgeon, individuality and anatomical variations of patients should be analyzed prior to surgical planning, moreover materials on the market for internal fixation are not consistent with the expected according to the anatomical national average found in our study.

INTRODUÇÃO: A Doença de Fabry (DF) é uma doença lisossomal de herança ligada ao X, causada pela deficiência da enzima alfa-galactosidase A (alfa-Gal A), que leva ao acúmulo gradual de glicoesfingolipídeos, em especial a globotriaosilceramida, nos lisossomos do endotélio vascular de tecidos cardíaco, renal, cerebral, olhos e pele. Os principais sintomas iniciais são: dores neuropáticas de extremidades, hipoidrose, dores abdominais recorrentes, angioqueratomas e córnea verticillata. As complicações, que aparecem a partir da terceira década de vida, incluem morte prematura por insuficiência renal, cardíaca e alterações cerebrovasculares. Angioqueratomas são uma das manifestações mais precoces da DF. OBJETIVOS: detectar os portadores da DF a partir de casos de angioqueratoma diagnosticados através de exames anatomopatológicos de biópsia cutânea; descrever o quadro clínico dos afetados e portadoras heterozigotas; realizar aconselhamento genético. MÉTODOS: Uma revisão sistemática de biópsias de pele de 2003 a 2012 foi feita em quatro hospitais universitários. Os pacientes foram convocados para anamnese, exame físico e coleta de história familial. A dosagem enzimática de alfa-Gal A por papel filtro e em leucócitos em homens e a análise molecular por PCR e sequenciamento do gene GLA em homens e mulheres foram realizados naqueles com suspeita de DF. RESULTADOS: Foram localizados 125 registros de angioqueratomas, sendo possível convocar 52 pacientes. Destes, 45 (M:21 e F:24) compareceram para serem examinados. O diagnóstico de DF foi confirmado 3/45 (6,6%), todos do sexo masculino, pela baixa atividade enzimática da alfa-Gal A. A idade dos pacientes eram 16, 21 e 32 anos. Foram identificados outros 13 familiares (cinco homens e oito mulheres) destas 3 famílias. Os principais achados clínicos nos oito homens afetados foram: dores de extremidades (62,5%), angioqueratomas (87,5%), alterações renais (87,5%), cardíacas (12,5%) e presença de córnea verticillata(37,5%) Nas oito mulheres os achados clínicos foram: dores de extremidades (75%), angioqueratomas (12,5%), alterações renais (37,5%) cardíacas (12,5%) e presença de córnea verticillata(0%). A frequência desses achados mostrou-se semelhante à descrita na literatura. As mutações patogênicas no gene GLA, herdadas da mãe, foram identificadas nas 3 famílias e já haviam sido descritas anteriormente. CONCLUSÕES: A DF foi identificada em 3/45 pacientes (6,6%) pelos registros histopatológicos. Portanto, o angioqueratoma é um marcador útil para a detecção da doença. A dosagem enzimática de alfa-Gal A e/ou estudo molecular foram fundamentais para a confirmação diagnóstica da DF. Foi possível identificar outros 13 familiares afetados (5 homens, 8 mulheres), o que reforça a importância do aconselhamento genético. Todos os pacientes apresentavam outros achados clínicos da DF além dos angioqueratomas, os quais, no entanto, não haviam sido diagnosticados. O diagnóstico da DF ainda é tardio e há necessidade de reconhecimento dos médicos sobre as manifestações dessa doença para possibilitar um diagnóstico precoce INTRODUTION: Fabry disease (FD) is an X-linked lysosomal disorder, caused by the deficiency of the enzyme alfa-galactosidase A (alfa-Gal A), which leads to gradual accumulation of glycosphingolipids, especially globotriaosylceramide, in lysosomes of the vascular endothelium of the cardiac tissue, kidney, brain, eyes and skin. The main initial symptoms are: neuropathic pain in the extremities, hypohidrosis, recurrent abdominal pain, angiokeratomas and cornea verticillata. Complications, which appear from the third decade of life on, include premature death from renal, cardiac and cerebrovascular abnormalities. Angiokeratomas are one of the earliest manifestations of FD. OBJECTIVES: to identify relatives with FD from cases of angiokeratoma diagnosed by pathological examinations of skin biopsy; to describe the clinical features of the affected individuals and heterozygotes; to perform genetic counseling. METHODS: A systematic review of skin biopsies from 2003 to 2012 was done in four university hospitals. Patients were submitted to anamnesis, physical examination and family history collection. The enzymatic assay of alfa-Gal A using dried blood spot in filter paper and leukocytes in men and molecular analysis by PCR and sequencing of the GLA gene in men and women were performed in patients with suspected FD. RESULTS: We found 125 biopsy records of angiokeratomas, and were able to contact 52 patients. Of these, 45 (M: 21 and F: 24) were evaluated. The diagnosis of FD was confirmed in 3/45 (6.6%), all male, with low enzymatic activity of alfa-Gal A. The patients\' ages were 16, 21 and 32 years. We identified 13 other family members (five men and eight women) of these three families. The main clinical findings in the eight affected men were pain in the extremities (62,5%), angiokeratomas (87,5%), renal abnormalities (87,5%), cardiac abnormalities (12,5%) and cornea verticillata (37,5%). In the eight female patients the clinical findings were: pain in the extremities (75%), angiokeratomas (12,5%), renal abnormalities (37,5%) cardiac abnormalities (12,5%) and cornea verticillata (0%). The frequency of these findings was similar to that described in the literature. The pathogenic mutations in the GLA gene, maternally inherited, were identified in three families and had been described previously. CONCLUSIONS: FD was identified in 3/45 patients (6.6%) by histopathologic records. Therefore, angiokeratoma is a useful marker for the detection of disease. The enzymatic measurement of alfa-Gal A and / or molecular studies were essential to confirm the diagnosis of FD. It was possible to identify 13 other affected family members (5 men, 8 women), which reinforces the importance of genetic counseling. All patients, in addition to angiokeratomas, had other clinical manifestations of FD; however these had not been diagnosed before. The diagnosis of FD is still late and doctors need to be aware of the manifestations of this disease to enable early diagnosis

Este estudo avalia a qualidade de vida de sujeitos acometidos por tumores musculoesqueléticos de membros inferiores, submetidos a cirurgias de ressecção, com salvamento de membro ou amputação. A casuística foi composta por 56 sujeitos, avaliados no período de 14 meses (agosto de 2011 a setembro de 2012), que foram divididos em três grupos: 1- 12 amputados; 2- 16 em pós-operatório de salvamento do membro (colocação de endoprótese ou reconstrução biológica); 3- sujeitos saudáveis (sem diagnóstico de câncer), selecionados entre a população em geral, de forma pareada com os outros grupos, segundo sexo, idade, escolaridade e condição socioeconômica. Além do Critério de Classificação Econômica Brasil (2010), foram aplicados dois protocolos de avaliação, um de qualidade de vida (Medical Outcomes Study 36 - Item Short-Form Health Survey - SF-36) e o outro de capacidade funcional (Toronto Extremity Salvage Score - TESS), ambos adaptados culturalmente e validados no Brasil. Os dados foram estatisticamente analisados, conforme preconizado por cada protocolo. Os resultados indicaram que ambos os tipos de abordagem cirúrgica (amputação ou reconstrução) produziram prejuízos na capacidade funcional e na qualidade de vida dos sujeitos acometidos por tumores musculoesqueléticos, quando comparados à população saudável. Diferentemente do que apontaram outros trabalhos internacionais com casuística semelhante, os sujeitos submetidos a amputações apresentaram resultados melhores relacionados à sua capacidade funcional e à qualidade de vida, na maioria dos domínios do SF36 e com relevância estatística (p=0,001) no escore final do TESS, do que aqueles submetidos a técnicas de salvamento de membro. Por fim, é importante analisar a percepção dos sujeitos sobre sua funcionalidade e qualidade de vida, de modo a encontrar caminhos mais adequados no processo de reabilitação desta população. This study appraises the quality of life of individuals with musculoskeletal tumors in lower extremities, undergoing resection surgeries with extremity salvage or amputation. The casuistry was composed by 56 subjects assessed in the course of 14 months (from August, 2001 to September, 2012), who were divided into 3 groups: 1- 12 amputated; 2- 16 in postsurgical extremity salvage (endoprosthesis implant or biological reconstruction); 3- healthy subjects (without cancer diagnosis), selected from the whole population, paired off with other groups, according to sex, age, schooling and socioeconomic status. Besides the Critério de Classificação Econômica Brasil (2010), two assessment protocols were applied, one about quality of life (Medical Outcomes Study 36 - Item Short-Form Health Survey - SF-36), and the other one about functional capacity (Toronto Extremity Salvage Score - TESS), both culturally adapted and validated in Brazil. The data were statistically analyzed according to what each protocol proclaims. The results revealed that both surgical approach types (amputation or reconstruction) caused harm to the functional capacity and to the quality of life of the subjects suffering from musculoskeletal tumors, when compared to the healthy population. Unlike what other international papers with similar casuistry have shown, the subjects undergoing amputation presented better results related to their functional capacity and quality of life, in the majority of the domains of SF36, with statistical relevance (p=0.001), than those submitted to the techniques of extremity salvage. Finally, it is important to analyze the subjects\' perception on their functionality and quality of life in order to discover the most suitable ways in the process of rehabilitating this population.

As hepatites virais continuam sendo uma preocupação em nível de saúde pública no Brasil e no Mundo, tanto pelo número de indivíduos atingidos, como pela possibilidade de complicação das formas agudas e crônicas. Segundo a Organização Mundial de Saúde (OMS), 170 milhões de pessoas são portadoras crônicas de hepatite C e 350 milhões portadoras crônicas de hepatite B. No Brasil, a estimativa de portadores de hepatite B crônica é de aproximadamente 600 mil pessoas e de hepatite C crônica, 1,5 milhão. Quanto à hepatite aguda A foram confirmados no país, em 2010, 5943 casos. O objetivo deste estudo foi conhecer a prevalência de marcadores sorológicos dos vírus das hepatites A, B e C em crianças e adolescentes matriculados em creches e escolas de ensino infantil e fundamental da rede municipal na cidade de Santos; conhecer os aspectos moleculares dos vírus das hepatites B e C, identificando o genótipo dos dois agentes e estudar modo de aquisição nos casos com sorologias positivas. Tratou-se de um estudo transversal realizado no período de 28 de Junho a 14 de Dezembro de 2007 onde foram coletadas 4680 amostras de sangue colhidas através de punção capilar e ao mesmo tempo aplicado um questionário nos familiares das crianças e adolescentes. Os exames sorológicos foram realizados utilizando a técnica de ELISA. O estudo molecular foi realizado pela técnica de reação em cadeia de polimerase \"in House\". A idade da população estudada variou de 7 meses a 18 anos e 1 m. A prevalência geral do anti-HVA IgG reagente foi de 9,7% e desses 74,6% foi anti-HVA IgM reagente. A prevalência de anti-HVA IgG foi maior entre as crianças mais velhas, meninas, aquelas que brincavam em córregos, sem esgoto em sua moradia, de pais com baixa instrução, de baixa renda familiar e aquelas que não eram moradoras da Orla. A prevalência de anti-HVA IgM, não foi diferente entre as diferentes categorias, exceção feita à faixa etária (pico no primeiros anos e posterior queda) e morro e Zona Noroeste foi mais baixa. A prevalência geral do anti-HBc reagente foi de 0,1%, do AgHBs de 0,02% e do anti-HVC foi de 0,02%. Conclui-se que a prevalência geral em crianças dos marcadores sorológicos para hepatites A, B e C na cidade de Santos foi baixa, quando comparada com os dados de literatura. Apesar dos nossos dados confirmarem uma mudança no perfil epidemiológico da hepatite A, as medidas preventivas atuais quanto ao saneamento, grau de instrução, habitação, ainda permanece com uma deficiência em Santos, cidade balneária com o maior porto do Brasil. A vacinação para hepatite B foi altamente eficaz com a baixa prevalência encontrada dos marcadores sorológicos. A utilização do papel de filtro em estudos epidemiológicos para hepatite A foi eficaz. Entretanto para o vírus da hepatite C ainda necessita de estudos comparativos utilizando sangue venoso, uma vez que a prevalência de crianças infectadas com hepatite C foi muito baixa na cidade de Santos. Viral hepatitis are still a concern in the public health level in Brazil and around the Word, due both to the number of affected subjects and the possibility of complication in the acute and chronic forms. According to the World Health organization (WHO), 170 million people are chronic carriers of hepatitis C and 350 million chronic carriers of hepatitis B. In Brasil, the estimate of people with chronic hepatitis B is approximately 600 thousand people and chronic hepatitis C, 1,5 million. It has been confirmed in the country, in 2010, 5943 cases of acute hepatitis A. The The aim of this study was to learn the prevalence of serological markers of hepatitis A, B and C virus in children and teenagers enrolled at the municipal education network in the city of Santos, to learn molecular aspects of hepatitis B and C, identifying the genotype of the two agents and to study the acquisition mode in cases with positive serology. Cross-sectional study carried out over the period from June 28 to December 14, 2007, in which 4680 fingerprick blood samples were collected; at the same time, a survey questionnaire was applied to the family members of the children and teenagers. The serological tests were performed using the ELISA technique. The molecular analysis was performed using the technique of polymerase chain reaction \"in House\". Age of the population studied ranged from 7 months to 18 years and 1 month. . The general prevalence of serological markers anti-HAV IgG reagent was 9.7% and between them 74,7% was anti-HAV IgM reagent. There was higher prevalence among older children, females, those who used to play in streams near their home, the absence of a sewage system in home, parents with low education, low household income and among those who did not live in the seashore. The prevalence of anti-HAV IgM was not different between the categories, except for the age (peak in the early years and subsequent fall) and lower on the Hills and Northweast Zone. The general prevalence of anti-HBc reagent was 0,1%, AgHBs was 0,02% and anti-HCV was 0,02%. It is concluded that, in children, the general prevalence of serological markers for hepatitis A, B and C in the city of Santos was low when compared with literature data. Although our data confirm a change in the epidemiological profile of hepatitis A, the current preventive measures regarding sanitation, education level and housing still remain with a deficiency in Santos, the coastal city with the largest harbor in Brazil. Vaccination for hepatitis B was highly effective because it found a low prevalence of serological markers. The use of filter paper in epidemiological studies for hepatitis A was effective. However for the vírus of hepatitis C still requires comparative studies using venous blood because the prevalence of infected children was very low in the city of Santos.

Um fator importante no estudo da oclusão refere-se à correta detecção da presença de contatos oclusais no lado de balanceio, sua intensidade e localização. No entanto, a maioria dos estudos que se propuseram a detectar a presença de contatos em balanceio, o fizeram utilizando papel carbono em movimentos conduzidos de máxima intercuspidação até topo-a-topo. O objetivo deste estudo foi estudar a frequência de contatos oclusais detectados no lado de balanceio em 42 indivíduos jovens, com idade média de 26 anos comparando dois métodos: 1- durante o ato mastigatório, utilizando como material de registro uma mistura de verniz cavitário e pó de fosfato de zinco; 2- durante movimentos conduzidos, utilizando como material de registro papel carbono. A frequência de contatos oclusais no lado de balanceio durante o ato mastigatório e utilização de verniz, foi de 100%, sendo que 97 % dos pacientes possuíam contatos bilaterais e 3 % apresentaram apenas contatos unilaterais. A frequência de contatos oclusais no lado de balanceio para os mesmos indivíduos, porém, com a utilização de papel carbono e movimentos conduzidos foi de 76%, sendo que 64% dos pacientes possuíam contatos bilaterais, 12% apresentavam apenas contatos unilaterais e 24% dos pacientes não possuíam contatos. Com relação aos métodos empregados, a mistura de verniz cavitário e pó do fosfato de zinco mostrou um número maior estatisticamente significante de marcações comparado ao papel carbono. De acordo com a metodologia empregada, concluiu-se que o verniz se mostrou mais sensível e eficaz por detectar uma quantidade maior de contatos oclusais comparado ao papel carbono. An important factor in the study of occlusion refers to the correct detection of the occlusal contacts in the balancing side, intensity and location. However, most studies that proposed itself on detect the presence of balancing contacts used carbon paper led movements in maximal intercuspal to topo a topo. The objective of this study was to evaluate the frequency of occlusal contacts in the balancing side in 42 young people, with a mean age of 26 years through two methods: 1 - during the masticatory act, using as registration material a mixture of cavity varnish and dust zinc phosphate, 2 - during movements performed, using as registration material carbon. And assess if the methods show similar results. The frequency of occlusal contacts on the balancing side during the masticatory act and using of varnish, was 100% and 97% of patients had bilateral contacts and 3% had presented only unilateral contacts. The frequency of occlusal contacts on the balancing side for the same individuals, however, by analyzing method with carbon and led movements was 76% and 64% of patients had bilateral contacts, 12% had only unilateral and 24 contacts % of patients had no relation to contacts. Regarding the method study, it presented statistical differences between the methods, regardless of the side studied. All differences point to a greater number of contacts on the coating method, indicating that in general this method detects more markings than the carbon method. According to the methodology, it concluded that the method was more sensitive varnish that carbon method for being able to detect more numbers occlusal contacts than carbon.

A busca dos artigos foi realizada nas bases de dados Lilacs, IBECS, MEDLINE e SciELO, por meio dos seguintes descritores da área da saúde (DeCS): doença pulmonar obstrutiva crônica, reabilitação, respiração, hiperinsuflação e dispneia, e suas respectivas versões na língua inglesa (MeSH), além do termo pursed-lip breathing. Após a eliminação dos títulos repetidos, foram selecionados somente os estudos que abordavam a RFL como tema principal, resultando em 12 artigos científicos, 10 ensaios clínicos e 2 revisões bibliográficas. Segundo os achados, a RFL proporciona: alterações sobre a gasometria arterial, caracterizada pelo aumento da saturação e pressão parcial de oxigênio; padrão ventilatório, com diminuição da frequência respiratória e aumento de tempo expiratório e do volume corrente; mecânica ventilatória, por meio do recrutamento de musculatura abdominal expiratória e dos músculos da caixa torácica e acessórios da inspiração; diminuição no consumo de oxigênio; alterações na modulação autonômica cardíaca induzida pelo aumento da atividade parassimpática e, por fim, melhora na qualidade de vida destes pacientes. A RFL é considerada uma manobra de grande importância, por repercutir de forma positiva em diversos sistemas e sobre a qualidade de vida de pacientes portadores da DPOC. This review aimed at standing out the main findings published in the last ten years on the effects of pursed-lip breathing (PLB) in patients with chronic obstructive pulmonary disease (COPD). The search for articles was performed in Lilacs, IBECS, MEDLINE, and SciELO database by using the descriptors of health (DeCS): doença pulmonar obstrutiva crônica, reabilitação, respiração, hiperinsuflação e dispneia, and their respective versions in the English language (MeSH), the term pursed-lip breathing was also in such list. After removing the repeated titles, only studies addressing PLB as the main theme were selected, resulting in 12 papers, 10 clinical trials, and 2 systematic reviews. According to the findings, the PLB provides: changes on arterial gases, which are characterized by increased oxygen saturation and partial pressure of oxygen; ventilatory pattern with decreased respiratory rate and increased expiratory time and tidal volume; respiratory mechanics, by recruiting the expiratory abdominal muscles and muscles of the rib cage and accessories inspired; decrease in oxygen consumption; alterations in cardiac autonomic modulation induced by increase in parasympathetic activity and, ultimately, improved quality of life of these patients. The PLB is considered a maneuver of great importance to have a positive effect on various systems and on the quality of life of patients with COPD. A história dos transplantes vem rompendo barreiras e enfrentando desafios embora de forma incipiente, por diversas décadas, conquistando impressionantes resultados, demonstrando a evolução científica e tecnológica aplicada ao setor. Assim, observa-se que, ao longo dos tempos, o número de doações tem aumentado. Porém, não o suficiente para reduzir a lista de espera, cabendo então a reflexão acerca das causas que dificultam esse processo. Diversos são os motivos pelos quais afetam a doação de órgãos, ressaltando-se a recusa dos familiares e a não notificação de possíveis doadores. Assim, vale ressaltar a importância do preparo profissional no que diz respeito à abordagem familiar, momento decisivo para que se possa dar continuidade ao processo de captação de órgãos. Diante do exposto, o estudo tem por objetivo explorar, recolher, organizar, sintetizar e compartilhar visão dos familiares em relação a atuação dos profissionais no processo de doação de órgãos. A metodologia retrata uma revisão sistemática da literatura brasileira sobre o tema central baseando na busca de artigos que demonstrassem resultados de pesquisas realizados no País, entre 2001 e 2011, no portal PubMed e nas bases de dados SCOPUS, CINAHL, EMBASE, Web of Science, Science Direct, LILACS, BDENF. Tal busca guiou-se pela pergunta: Qual a visão dos familiares, que passaram pela abordagem para doação de órgãos, referente à atuação dos profissionais que atuam neste processo? Como resultados dos 265 trabalhos encontrados, foram considerados pertinentes, à temática em apreço, 06 artigos, os quais foram submetidos à leitura criteriosa da metodologia utilizada, dos sujeitos investigados, dos resultados obtidos e das conclusões. As sínteses foram construídas a partir da análise temática dos resultados. Surgiram 03 categorias. Sendo elas: 1) Falta de confiança na atuação profissional; 2) Atuação profissional sem compreensão e acolhimento no momento familiar; 3) Falta de informação pelo profissional. Desta última, emergiram duas subcategorias: 1) Falta de informação pelo profissional à família referente à possível Morte Encefálica; 2) Falta de informação pelo profissional à família referente aos trâmites pós-doação. Consideramos, portanto, que a complexidade de ações que são necessárias no processo de doação de órgãos, foi revelada como burocrático, demorado, desgastante e cansativo, resultando em sofrimento e submetendo tanto a família quanto os profissionais a situações estressantes. Disto depreendemos haver necessidade de maiores investimentos na formação dos profissionais que atuam neste processo, melhorando seu suporte emocional e sua atuação, a qual é considerada incipiente pelos familiares. Transplants\' history are breaking barriers and facing challenges for several decades achieving impressive results. It demonstrates scientific and technological developments applied to this sector. Although an increasing on organs donations have been noted, it is not enough to reduce waiting list. Therefore there is a need for reflection on the causes, which makes this process. There are several reasons that affect organ donation, and shall be emphasized: relatives\' refusal and failure in notifying potential donors. It is also worth highlighting the importance of professional competence in respect of family approach, a decisive moment that gives continuity to the process of organ retrieval. In this light, the study aims to explore, collect, organize, synthesize and share the opinion of relatives about the role of professionals during process of organ donation (POD). As methodology a systematic review of Brazilian literature was conducted between 2001 and 2011. Data was collected from (i) PubMed, (ii) SCOPUS, (iii) CINAHL, (iv) EMBASE, (v) Web of Science, (vi) Science Direct, (vii) LILACS, and (viii) BDENF. Such data collection was performed based on one question: What is the opinion of relatives, who went through organ donation approach, referring to professionals\' actions working in this process? Six out of 265 papers were considered relevant, and then a careful reading executed to assess methodology, subjects investigated, results and conclusions. Analyses have enable to define three categories: (a) lack of confidence in the professional, (b) professional attitude without understanding of relatives emotional state, (c) professional apathy in provide information to family. The last category was subdivided in (i) lack of info about a possible brain death, and (ii) subsequent procedures about POD. These study revealed a complexity in POD particularly on bureaucracy, delay, family\'s fatigue and wear. Thus, we may conclude to be necessary investing efforts on professional training involved in the POD to protect family\'s psychological state.

Estudo descritivo, quantitativo que teve como objetivo comparar a Capacidade Funcional de mulheres com câncer de mama, que realizaram tratamento oncológico (G1), com a de mulheres que nunca apresentaram diagnóstico de câncer (G2), e identificar se a qualidade de vida de mulheres com neoplasia mamária correlaciona-se com a capacidade funcional. O G1 foi composto por 42 mulheres submetidas ao tratamento para o câncer de mama, até um ano do período da coleta de dados, e que frequentavam um núcleo de reabilitação. O G2 foi composto por 42 mulheres, pareadas com o G1, por idade e nível sócio-econômico. A coleta de dados se deu no período de janeiro a julho de 2009. Os instrumentos utilizados para a comparação da Capacidade Funcional foram Índice de Katz e Índice de Lawton, e para a Qualidade de Vida, o EORTC QLQ-C30 e EORCT QLQ-BR23. Os resultados mostraram que o G1 apresentou leve comprometimento nas condições físicas, como fadiga, imagem corporal, função sexual, função física e sintomas na mama e no braço, e moderadas alterações psicossociais na função emocional e nas atividades de lazer e de participação social, comprometendo, assim, sua QV. A comparação entre G1 e G2 mostrou que o tratamento oncológico prejudicou muito a realização das AIVDs das integrantes do G1, porém não interferiu nas ABVD. Acredita-se que os resultados referentes às ABVDs se devam ao fato de elas frequentarem um núcleo de reabilitação e receberem orientações para continuar realizando as ABVDs, mesmo no período pós-operatório. Além disso, participar do núcleo pode ter contribuído para a manutenção do autocuidado e ser também uma importante estratégia para o melhor enfrentamento das limitações físicas e emocionais impostas pelo câncer de mama. O período de realização da coleta dos dados, ao término dos tratamentos, também pode ter favorecido a indicação, pelas componentes do G1, de um menor comprometimento de alguns eventos adversos ao tratamento e alterações psicossociais. No entanto, evidenciou-se ainda, com os resultados relativos às AIVDs, a importância de um maior investimento no cuidado à mulher com câncer de mama, em relação à sua QV e CF, no que se refere às funções física, emocional e social. Sugere-se a realização de estudos de abordagem qualitativa que façam uma avaliação subjetiva dos comprometimentos dos eventos adversos dos tratamentos, das alterações psicossociais para as mulheres com câncer de mama e das estratégias de que elas se utilizam para vivenciá-los. Outras investigações poderão abordar ainda aspectos individuais no DO, como as AIVDs e atividades de lazer; avaliar a eficácia de serviços de reabilitação no manejo dos sintomas, como dor, fadiga, insônia, perda de apetite, aderência cicatricial e outros, além do papel frente às alterações psicossociais; e avaliar as demais categorias que integram o DO de mulheres com câncer de mama, para compreender a influência desta doença sobre aspectos como os de repouso e sono, educação, trabalho e participação social, a fim de possibilitar uma melhor intervenção multidisciplinar junto a esta população. This descriptive study aimed to compare the functional capacity of women with breast cancer who underwent treatment (G1), with women who never had cancer (G2), and identify if the quality of life of women with breast cancer correlates with functional capacity.G1 was composed of 42 women undergoing treatment for breast cancer, up to a year of data collection, and who attended a rehabilitation center. The G2 is the G1 paired by age and socioeconomic status. Data collection took place from January to July 2009. The instruments used for comparison were the Functional Capacity Index Katz and Lawton Index, and Quality of Life, EORTC QLQ-C30 and QLQ-BR23 EORCT. The results showed that G1 showed mild impairments in physical conditions such as fatigue, body image, sexual function, physical function and symptoms in breast and arm, and moderate changes in the psychosocial and emotional function in leisure activities and social participation, compromising thus their QOL. The comparison between G1 and G2 showed that cancer treatment was very damaging to the performance of IADL members of G1, but did not interfere with ABVD. It is believed that the results concerning the ABVD be attributed to the fact that they attend a rehabilitation unit and received guidance to continue performing the ADL, even in the postoperative period. Moreover, the core part may have contributed to the maintenance of self-care. Thus, attending a rehabilitation service can be an important strategy to better cope with the emotional and physical limitations imposed by breast cancer. The timing of data collection at the end of treatments, may also have favored the statement, the components of the G1, a lower commitment of some adverse events related to treatment and psychosocial changes. However, there was still, with the results for the AIVD, the importance of increased investment in care for women with breast cancer in relation to their QOL and CF, with regard to the functions physically, emotionally and socially. It is suggested for studies of a qualitative approach to make a subjective evaluation of the commitments of the adverse events of treatment, the psychosocial changes for women with breast cancer and strategies that they use to experience them. Further investigation will also address individual aspects of the DO, as the IADL and leisure activities, evaluate the effectiveness of rehabilitation services in the management of symptoms such as pain, fatigue, insomnia, appetite loss, scar tissue and others, besides the paper front psychosocial changes, and evaluate the other categories that comprise the OD of women with breast cancer, to understand the influence of this disease on aspects such as rest and sleep, education, employment and social participation, to enable a better multidisciplinary intervention with this population.

A tendência de globalização dos ensaios clínicos, observada nos últimos anos, trouxe à tona questões antes não discutidas como a continuidade do tratamento com os medicamentos em investigação, após a conclusão da pesquisa. A inclusão de países em desenvolvimento nesses estudos traz consigo a preocupação com a vulnerabilidade dos participantes da pesquisa. No Brasil, há cerca de cinco anos, os Comitês de Ética em Pesquisa, CEP, e, particularmente, a Comissão Nacional de Ética em Pesquisa, CONEP, passaram a solicitar a manutenção do fornecimento do(s) medicamento(s) do estudo após sua finalização. Embora baseada nos principais documentos éticos que norteiam a pesquisa clínica, essa solicitação apresenta dificuldades práticas para implantação, principalmente no caso de doenças crônicas. O objetivo deste trabalho foi identificar as questões envolvidas na continuidade do fornecimento de medicamentos após a conclusão de ensaio clínico e analisar a perspectiva de atores que compõem o cenário da pesquisa clínica nacional. Questionários e respectivos Termos de Consentimento Livre e Esclarecidos, TCLE, foram enviados por correioeletrônico, entre outubro de 2009 e janeiro de 2010, a membros de CEPs (todos os CEPs credenciados pela CONEP naquela data), pesquisadores (em duas áreas terapêuticas, HIV/AIDS e Diabetes mellitus) e patrocinadores. Aos pesquisadores foi solicitado que aplicassem o questionário a seus pacientes de pesquisa. A taxa de resposta dos CEPs foi de 20,7% (124 responderam, de 599 questionários enviados), 20% para os pesquisadores (58 de 290) e 45,3% para os patrocinadores (24 de 53). Cinquenta e quatro pacientes convidados por seus médicos responderam. Com relação à informação contida no TCLE, o item menos informado é relativo a como obter o medicamento após o estudo, para todos os grupos pesquisados. Com relação à motivação dos pacientes ao participar de uma pesquisa, 96,2% dos pacientes responderam como \"muito importante\", na decisão, a busca de melhores cuidados médicos e atenção à própria saúde, e 94,2% o fato de colaborar para o desenvolvimento da ciência (altruísmo). Entretanto, os demais grupos entrevistados não pensam da mesma forma: para eles, a maior motivação dos pacientes, ao participar de pesquisas clínicas, é a busca de melhores cuidados médicos e atenção à sua saúde, seguido da busca pelo acesso a alternativas de tratamento para sua doença. Ao serem perguntados sobre quem deveria receber o medicamento em investigação após o estudo, os pacientes responderam que todas as pessoas deveriam receber o medicamento após o estudo (60,4%); entre os pesquisadores, a maior parte (43,1%) respondeu que o medicamento deveria ser fornecido às pessoas participantes do estudo e 39,7% deles responderam que o medicamento deveria ser fornecido às pessoas que se beneficiariam do medicamento em estudo. Já os representantes de CEP concordaram com os pacientes que todas as pessoas deveriam receber o medicamento, mas em proporção bem menor (35,3%). Os patrocinadores opinaram que o medicamento do estudo deveria ser fornecido aos participantes da pesquisa que dele se beneficiariam (50%). Houve consenso entre os grupos em que, havendo a continuidade do tratamento, este deveria ser fornecido pelo patrocinador e de forma gratuita. Ao responder a questão relativa a quanto tempo deveria o medicamento ser fornecido, pesquisadores e patrocinadores consideraram que o medicamento deveria ser fornecido até estar disponível na rede pública, enquanto que os membros de CEP, opinaram que isso deveria acontecer durante o período que o paciente fosse beneficiado. Os pacientes responderam que o benefício deveria ser mantido pela vida toda. Devido às várias limitações deste estudo (representatividade da amostra, população restrita a usuários da Internet), seus resultados podem não ser generalizados, mas podem contribuir para a discussão do tema, ao analisar os pontos de vista de vários atores do cenário da pesquisa clínica nacional. The recent trend of globalization of clinical trials, observed in the last years, raised some non-previously discussed issues, such as the continuity of treatment after the conclusion of the study. The inclusion of developing countries in these trials brings together the concern with the participants\' vulnerability. In Brazil, about 5 years ago, the Ethics Committees (EC) and the National Commission of Ethics in Research (so called CONEP) started requiring the access to study medication after the trial ends. Even being based on the main documents related to ethics and research, some practical issues make this requirement difficult to implement, mainly in the arena of chronic diseases. The goal of this research was to identify the questions related to continuing the supply of the investigational medicine and analyze the perspective of all stakeholders involved in clinical research. Questionnaires and informed consents were sent through e-mail, between Oct 2009 and Jan 2010, to EC members (all ECs mentioned at CONEP site at that moment), clinical investigators (in two therapeutical areas, HIV/AIDS and Diabetes) and sponsors. We also asked the clinical investigators to submit the questionnaire (in paper) to their patients who already participated in clinical research. The response rate in each group was: 20.7% in EC members\' group (599 questionnaires sent, 124 replied), 20% in clinical investigators\' group (58 of 290) and 45.3% in sponsors\' group (24 of 53). Fifty four patients answered the questionnaire through their doctors. Regarding to the information displaced in the informed consent, the item less informed, according to all groups, was related to how to obtain the study medication after the conclusion of the trial. Concerning the motivation of patients which made them accept to participate in a clinical trial, 96.2% of patients answered as \"very important\" to obtain better health care and attention and 94.2% mentioned also as \"very important\" the possibility to collaborate with the progress of the science (altruism). However, among the other groups, the responses were different: for them, the major motivation for the patients would be to have access to better health care and attention (similar to patients) and search for access to treatment alternatives for their diseases. When asked to whom should be given the study medication after the trial, the patients answered that all patients should receive it (60.4%); for the clinical investigators, 43.1% believe that the medication should be given to the participants of the study and 39.7% to the subjects who participated and benefited from the study treatment. The EC members agreed (but to a lesser degree, 35.3%) with the patients that all individuals with the disease should receive the medication after the trial. For 50% of the sponsors, the study medication should be assured to the participants who had benefit from the treatment. In the case of need of access extension after the trial, a consensus could be observed among all groups, concerning to whom should provide the medication (the sponsor) and how (completely free). One question addressed the issue of how long the treatment should be assured after the trial: clinical investigators and sponsors replied that the medication should be kept until be available in the public health sector; the EC members thought it should be furnished while the patient keeps the benefit. The patients answered that the benefit should be assured for all life. Due to the several limitations of this research (such as the sample representativeness, population restrict to internet users), their results can\'t be generalized; however, the data can contribute to the discussion of this very complex topic through analyzing the views of the several stakeholders of the scenario of clinical research in Brazil.

Introdução: O consumo excessivo de sódio tem se associado com vários efeitos prejudiciais à saúde, como aumento da pressão arterial (PA) e doença cardiovascular. Objetivos: Os objetivos da tese foram avaliar a disponibilidade de sódio no Brasil, analisar os efeitos de dietas com teores reduzidos de sódio em indivíduos com Síndrome Metabólica (SM) ou resistência à insulina (RI) e avaliar o impacto de uma intervenção que promoveu a redução da adição de sal aos alimentos. Sujeitos e métodos: A tese está composta de três artigos. O primeiro deles avaliou as aquisições de alimentos e bebidas realizadas pelos domicílios na Pesquisa de Orçamento Familiar de 2002/2003 do Instituto Brasileiro de Geografia e Estatística. O segundo artigo revisou a literatura de forma sistemática acerca dos ensaios clínicos realizados em adultos entre anos de 2004 e 2008. E o último artigo avaliou o impacto de uma intervenção, baseada no modelo ecológico de promoção de saúde, na redução da adição de sal nos alimentos consumidos por trabalhadores de empresas da cidade de São Paulo. Resultados: A quantidade diária de sódio disponível para consumo nos domicílios brasileiros foi de 4,7 g per capita e para uma ingestão diária de 2.000 kcal. A maior parte do sódio disponível para consumo proveio do sal de cozinha e de condimentos à base desse sal (76,2%). A fração proveniente de alimentos processados com adição de sal representou 9,7por centodo total de sódio no quinto inferior da distribuição da renda per capita e 25,0por centono quinto superior. A restrição no consumo de sódio associou-se ao aumento da RI em dois artigos e à diminuição em três outros. Em sete dos nove artigos, uma dieta com teor reduzido de sódio determinou redução da PA e em dois artigos ocorreram efeitos adversos em marcadores da SM. Nas análises multivariadas, homens (IRR = 0,51) e aqueles considerados normotensos (IRR = 0,61) alocados no grupo intervenção no momento final apresentaram taxas menores de adição de sal ao prato quente. Conclusões: O consumo de sódio no Brasil excede largamente a recomendação máxima para esse nutriente. A maioria dos estudos da revisão sistemática mostrou efeitos benéficos da restrição moderada de sódio da dieta, associada ou não a outras modificações nutricionais ou ao aumento da atividade física. A intervenção realizada no ambiente de trabalho reduziu a taxa de relato de adição de sal no prato quente entre trabalhadores do sexo masculino e entre aqueles categorizados como normotensos Introduction: The excess of sodium consumption has been associated with several adverse health effects, such as increased of blood pressure (BP) and cardiovascular disease. Objectives: Evaluate the availability of sodium in Brazil, analyze the effects of diets with reduced sodium content in subjects with Metabolic Syndrome (MS) or insulin resistance (IR) and evaluate the impact of an intervention which promoted the reduction of adding salt to foods. Subjects and Methods: The thesis is composed of three articles. The first one assessed the food and beverage purchases made by households in the Household Budget Survey of 2002/2003, by the Brazilian Institute of Geography and Statistics. The second article reviewed the literature in a systematic way about clinical trials performed in adults between the years 2004 and 2008. And the last paper assessed the impact of an intervention, based on the ecological model of health promotion, in reducing the added salt to foods consumed by employees of Sao Paulo based companies. Results: The daily amount of sodium available in Brazilian households was 4.7 g daily per person for 2,000 kcal intake. Most of the sodium available for consumption comes from the salt and salt based seasonings (76.2 per cent). The fraction derived from processed foods with added salt represented 9.7 per cent of total sodium intake at the bottom of per person income distribution and 25.0 per cent at the top fifth. The restriction in sodium intake was associated with increased IR in two articles and the decrease in three others. In seven of the nine articles, the salt intake restriction had caused decreased in BP and in two articles have occurred adverse effects on markers of MS. In multivariate analysis, men (IRR = 0.51) and those considered normotensive (IRR = 0.61) allocated in the intervention group at the final moment had lower rates of salt addition in the hot meal. Conclusions: The results indicate that sodium intake in Brazil far exceeds the maximum recommended intake for this nutrient. Most studies have shown beneficial effects of a diet with moderate sodium restriction, with or without other nutritional modifications or increased physical activity. The intervention carried out at the workplace reduced the reporting rate of salt addition in the hot meal by male workers and those categorized as normotensive

Os acidentes causados por serpentes peçonhentas são considerados um problema de saúde pública no Brasil, e é muito importante que sejam feitos mais estudos para um melhor conhecimento desse agravo. O objetivo deste trabalho é descrever as características clínicas e epidemiológicas que foram observados no Hospital de Doenças Tropicais de Araguaína, Estado do Tocantins, Brasil, que é uma unidade de saúde que cuida de pacientes que sofrem de doenças tropicais, no período de 2007 a 2009. Foram estudados 556 indivíduos e as características mais comuns observadas foram: sexo masculino, idade entre 19-40 anos, picada em membros inferiores e os ferimentos causados por serpentes do gênero Bothrops. Os sinais e sintomas mais freqüentes foram dor, edema e hemorragia. A manifestação sistêmica mais observada foi alteração da coagulação sanguínea. As complicações mais freqüentes foram abscesso, insuficiência renal aguda, necrose e infecção bacteriana. Os principais microorganismos identificados foram Morganela morganii (17,1%), Pseudomonas aeruginosa (14,6%) e Staphylococcus epidermidis (4,9%). Esta flora bacteriana foi semelhante às descritas no veneno e na boca das serpentes por outros pesquisadores. Com base nos resultados dos testes de suscetibilidade sugere-se a continuidade no uso de ciprofloxacin para o tratamento desses abscessos que não respondem à simples drenagem. O conjunto dos resultados obtidos representa um completo levantamento da atual situação dos acidentes envolvendo serpentes no estado do Tocantins e arredores e certamente contribuirá para o aprimoramento das ações de combate aos problemas de Saúde Pública da região Norte. Accidents caused by venomous snakes are considered a public health problem in Brazil, and it is very important that more studies be done to a better understanding of this injury. The aim of this paper is to describe the clinical and epidemiological characteristics that were observed in the patients at the Hospital for Tropical Diseases in Araguaina, Tocantins State, Brazil, during the last three years. We have studied 556 individuals and the most common features were: male, aged 19-40 years, stinging in his legs and injuries caused by Bothrops snakes. Signs and symptoms were pain, swelling and bleeding. The most observed systemic manifestation was alteration of blood coagulation. The most frequent complications were abscess, acute renal necrosis and bacterial infection and the main microorganisms identified were Morganela morganii (17.1%), Pseudomonas aeruginosa (14.6%) and Staphylococcus epidermidis (4.9%). The bacterial flora was similar to those described in the mouth of snakes by other researchers. The susceptibility tests results suggest the continuity in the use of ciprofloxacin for the treatment of abscesses that do not respond to simple drainage. The set of results represents a complete survey of the current situation of accidents involving snakes in the state of Tocantins and the surrounding area and will certainly contribute to the improvement of actions to combat public health problems in the North region of Brazil.

This paper presents the initial findings of a study carried out with siblings of autistic children. Its aim is to identify possible obstacles along such children's development process. Knowing that an individual is constructed through the instinctual relationship established with the Other and that in autism it does not occur, it is supposed that in the siblings of autistic children obstacles may arise for the creation of this bond.

As flutuações nos níveis dos hormônios sexuais, ao longo da vida do ser humano, trazem modificações anátomo-fisiológicas. Esses hormônios exercem papéis determinantes no desenvolvimento anatômico da laringe e na fisiologia vocal diferindo significativamente entre os sexos o que torna a voz uma característica sexual secundária. Estudos já documentaram a presença e, mais especificamente a localização dos receptores dos hormônios sexuais na laringe e pregas vocais, evidenciando que estes órgãos podem ser responsivos aos hormônios sexuais e sofrerem as conseqüências das flutuações hormonais. Existem algumas divergências quanto ao efeito hormonal sobre a qualidade vocal. Alguns estudos afirmam que ocorrem mudanças na voz relacionadas à menopausa, enquanto outros divergem desses resultados. Tal fato nos leva a questionar se as metodologias utilizadas foram realmente as mais adequadas, uma vez que existem evidências clínicas destas mudanças. São freqüentes as queixas entre as mulheres de alterações vocais após a menopausa, particularmente entre as mulheres cantoras, que enfatizam perdas dos tons agudos. O objetivo deste trabalho foi verificar se há influência dos hormônios sexuais sobre a qualidade vocal da voz cantada de mulheres menopausadas cantoras de coral, através da avaliação da tessitura e do tempo máximo de fonação (TMF). A tessitura corresponde ao número de notas da mais grave até a mais aguda que o indivíduo consegue produzir com qualidade vocal. O tempo máximo de fonação (TMF) avalia a habilidade do paciente em forças aerodinâmicas da corrente pulmonar e as forças mioelásticas da laringe. As voluntárias foram divididas em dois grupos. Grupo jovem (n=8) mulheres com idade entre 20 e 40 anos, que apresentavam ciclos menstruais regulares que não estivessem grávidas ou amamentando durante o estudo. Grupo menopausada (n=22) mulheres com idade de 45 a 60 anos, menopausadas há no mínimo 2 anos que não faziam uso de terapia hormonal (TH). Para mensuração do TMF, foi dada a instrução de inspirar e produzir isoladamente as vogais /a/, /i/ e /u/ e as consoantes fricativas /s/ e /z/ em altura e tons habituais de fala. Para avaliar a tessitura foi escolhida uma música conhecida do folclore brasileiro que exigia as variações de freqüência da mais grave a mais aguda. Os valores de cada nota musical e sua freqüência correspondente em hertz (Hz) foram escritos manualmente. Foram comparados para o grupo jovem e menopausada os valores máximos (F2) e os valores mínimos (F1) e a diferença entre o valor máximo e mínimo (F2-F1) entre os grupos medidos em freqüência (Hz) e em semitons (St), e foram encontradas diferenças significativas entre os grupos. Não foi encontrada diferença significativa entre o grupo jovem e menopausada quando foram comparados o tempo máximo de fonação das vogais /a/, /i/, /u/ e as consoantes /s/ e /z/. Os resultados do presente estudo demonstraram haver diferença significativa na tessitura da voz cantada entre mulheres cantoras de coral jovens e pós-menopausa. Estes resultados sugerem que as alterações na tessitura da voz cantada podem ser decorrentes de modificações fonatórias da articulação, respiração e fonação realizadas durante o canto, influenciadas pelos hormônios sexuais. E não serem atribuídas às alterações nas pregas vocais como edemas, espessamentos e a hiperemia, as quais não foram encontradas ao exame videolaringoscópico e tampouco no TMF quando comparados os grupos jovem e menopausada. Fluctuations in the levels of sexual hormones, throughout the life of the human being, bring anatomic physiological modifications. These hormones exert determinative papers in the anatomical development of the larynx and in the vocal physiology differing significantly between the sexes becoming the voice a secondary sexual characteristic. Studies already had registered the presence and, more specifically the vocal localization of the receivers of sexual hormones in the larynx and vocal folds, evidencing that these agencies can be responsive to sex hormones and suffer the consequences of the hormonal fluctuations. Some divergences how much the hormonal effect exist on the vocal quality. Some studies affirm that changes in the voice related to the menopause occur, but others had demonstrated different results. Such fact takes in them to question if the used methodologies really had been adjusted a time that exist practical clinical evidences of these changes. The complaints between the women of vocal alterations are after the menopause, particularly between the women singers, who emphasize losses of the acute tones. In literature, however, we do not find references to this fact that are of clinical comment and stories, and for this reason we decide to study this phenomenon. The objective of this work was to verify if it has influence of sexual hormones on the vocal quality of the sung voice of menopausadas women chorale singers, through the evaluation of the tessitura and the maximum phonation time (MPT). The vocal tessitura corresponds to the number of notes from the lowest to the highest that an individual is able to produce with vocal quality. Maximum phonation time (MPT) is used to assess clinically the respiratory and phonatory components of the mechanism of speech production and to demonstrate the efficiency of vocal fold vibration. The volunteers had been divided in two groups. Young group (n=8) women aged 20 to 40 years, with regular menstrual cycles who were not pregnant or breast-feeding during the study. Menopausal group (n=22) women aged 45 to 60 years, who had been in menopause for at least 2 years and who did not use hormonal therapy. For MPT measurement, the women were instructed to inspire and to produce separately the vowels /a/, /i/ and /u/ and the fricative consonants /s/ and /z/ at habitual speech loudness and tones. The voice tessitura profile was obtained evaluation a well known Brazilian folk song which required variations in frequency from the lowest to the highest. The values of each musical note and its corresponding frequency in Hertz (Hz) and semitones (St) were written manually. The maximum (F2) and minimum (F1) values and the difference between them (F2-F1) were compared for the two groups in frequency (Hz) and semitones (St) and significant difference was detected between them. No significant difference was observed between groups when they had been compared for the vowels /a/, /i/ and /u/ and the consonants /s/ and /z/. The results of the present study had demonstrated with there was significant difference in tessitura between young and menopausal women. These results suggest that the alterations in the tessitura of the sung voice can be decurrently of phonatory modifications of the joint, breath and phonation during the sing influenced for sexual hormones. And not attributed to the alterations in the vocal folds as edemas, thicken and the hyperemia, which had not been found to the otorhinolaryngologic evaluation and neither in the MPT when compared young and menopausal women.

Considering that autism brings up questions about the most primitive moment of the subjectivity constitution, this paper first approaches the particularities of the enjoyment that, according to psychoanalytic theory, establishes representation as an inscription on inconscient tying up the subject to the Other. Then, since in autism the enjoyment is situated beyond representation, it is envisaged from the viewpoint proposed by Lacan with the concept of lalangue, the precursor of representation as enjoyment.

Historicamente, as diferentes concepções de Saúde Coletiva influenciaram tanto no ensino específico deste campo do saber como na formação geral do médico. Partindo deste pressuposto, o presente trabalho teve como objetivo estudar o ensino da Saúde Coletiva na graduação médica, buscando apreender suas configurações e as conseqüentes implicações nas propostas curriculares de três universidades do Paraná (Brasil): Universidade Estadual de Londrina (UEL), Universidade Federal do Paraná (UFPR) e Universidade Positivo (UnicenP). Constituíram-se também em eixos de investigação, além das concepções de Saúde Coletiva, a relação ensino-aprendizagem, as ênfases assumidas pelo programa e os vínculos com os serviços de saúde. A pesquisa qualitativa foi realizada com base na análise documental dos projetos pedagógicos (os cursos instituídos) considerando como eles são efetivamente vividos, de acordo com a ótica dos agentes envolvidos. Para tanto, foram feitas entrevistas com 11 gestores, 18 professores, e foram constituídos 4 grupos focais de estudantes, considerados atores-chave nos três cursos de Medicina. Os resultados evidenciaram a inserção de 5% a 20% da área de Saúde Coletiva nos currículos, dependendo das estratégias de ensino, mas sempre em abordagens acadêmicas fortemente vinculadas aos serviços de saúde, uma realidade que é fortalecida pelo grau de consolidação do SUS em ambas as cidades (Curitiba e Londrina). Independentemente da natureza da universidade, da configuração organizacional e acadêmica do curso e dos diferentes modos de inserção docente, a Saúde Coletiva se faz presente e se assume com considerável relevância para a formação médica. Ainda que não se constitua um eixo articulador da graduação em dois casos estudados, a área traz à dimensão técnica do saber médico o equilíbrio necessário representado pela consciência dos desafios e o compromisso com a realidade. De modo inverso, em função da complexidade da formação, a presença maciça da Saúde Coletiva ao longo de um curso não garante necessariamente o ideal da profissionalização médica. Historically, different concepts of Public Health have influenced the specific teaching of this field of knowledge as well as medical education. The objective of this paper is to study the teaching of Public Health in medical schools, focusing on its structure and implications in curriculum design in three universities in Paraná - Brazil: State University of Londrina (UEL), Federal University of Paraná (UFPR) and the Positivo University (UnicenP). The research questions focused on the content of Public Health selected in their respective curricula, the teaching-learning relationships, program emphasis and the partnerships established with public health services. Qualitative research data collection from the perspective of key informants was carried out based on the analysis of pedagogical projects and on how they were effectively experienced. Eleven managers and 18 teachers were interviewed and 4 focus groups with students were developed in the three universities. Outcomes showed the presence of between 5% to 20% of Public Health themes in the course syllabi, depending on the teaching strategies used. However, they always appeared associated with academic issues strongly linked to health services, which were strengthened by the local development of the Comprehensive Health Care System in the two cities, Curitiba and Londrina in which studies were carried out. Public Health is present and very relevant in the curriculum required for doctors qualification regardless of the characteristics of the university studied, the bureaucratic and academic course structures and the different methods for hiring teachers. Besides not being a main articulator axis in two of the medical courses studied, Public Health provides the necessary balance for the technical dimension of medical knowledge, represented by the awareness of the challenges and commitment to the reality. On the contrary, because of the complexity of medical education, the strong presence of Public Health in the other medical course studied not necessarily guarantees the ideal qualification of the medical professional

Antecedentes. Alguns princípios e diretrizes organizativas do Sistema Único de Saúde (SUS) têm sido mais freqüentemente avaliados como a descentralização da gestão, a participação e controle sociais e a extensão de cobertura. A integralidade da atenção à saúde se destaca como um de seus princípios pouco estudado, particularmente no que se refere à sua dimensão sistêmica como a incorporação desse princípio na política e na gestão da saúde no âmbito municipal. Objetivo. Esta pesquisa foi desenvolvida com o objetivo de avaliar se e como a integralidade na assistência à saúde foi incorporada nas políticas de saúde priorizadas e implementadas no processo de (re)construção do SUS no Município de São Paulo (SP), no período de 2001 a 2008. Métodos. A estratégia metodológica utilizada foi a do estudo de caso, utilizando-se como dados e informações coletadas junto a distintas fontes de evidência: (a) atores sociais privilegiados, envolvidos no processo de gestão (informantes-chaves), por meio de entrevistas; (b) documentos de gestão (leis, decretos, portarias, normas técnicas, relatórios de gestão, atas de reunião); e (c) observação participante, desenvolvida em espaços institucionais e instâncias político-administrativas do SUS no município. A análise da incorporação da integralidade da assistência à saúde foi realizada prioritariamente nas dimensões da política de saúde e da gestão do sistema de saúde. Resultados. São apresentados três artigos formatados com recortes temáticos dos resultados da pesquisa e que pudessem ter consistência e densidade suficientes para serem submetidos a periódicos do campo da Saúde Pública/Coletiva. Os artigos elaborados são: (1) Política de saúde e gestão no processo de (re)construção do SUS em município de grande porte: um estudo de caso de São Paulo, Brasil. Espera-se que os resultados e sua divulgação possam contribuir para a análise crítica do SUS no Município de São Paulo e outros município de grande porte, bem como com a identificação de elementos de reflexão, de temas para novas pesquisas, ou fornecer subsídios para a (re)formulação teórico-conceituais sobre política e gestão de sistemas de saúde e o fortalecimento do SUS. Background. Some principles and organizational guidelines of the Brazilian National Health System (SUS) have been evaluated more frequently such as decentralization of management, the participation and social control and the span of coverage. The integrality of attention to health is one of the issues less analyzed, especially in terms of systemic dimension as the incorporation of such principle in the policy and management in the city scope. Objective. This research was carried out aiming at evaluating if and how the global approach of health care was incorporated in prioritized health policies and executed in the process of (re)construction of the SUS in the City of São Paulo (SP), between 2001 and 2008. Methods. The research was performed by utilizing the methodological strategy of case study derived from the following different sources of evidence: (a) social and institutional actors as privileged performers involved in management process (key-informers) by means of interviews; (b) management documents and papers (laws, decrees, plans, technical specifications, management reports, minutes of meetings); (c) active observation, developed in health services and political administrative areas of the SUS in the city. The analysis of the incorporation of the global approach of health care was made mainly under the aspects of health policy and health system management. Results. Three articles comprising theme excerpts of the research results which can have enough reliability and concentration to be submitted to journals in the field of Public Health. The articles written are: (1) Health policy and management in the (re)construction process of the Brazilian National Health System (SUS) in a large municipality: a case study of São Paulo city, Brazil. Cadernos de Saúde Pública, Rio de Janeiro, v.25, 2009 (in priting); (2) The challenge of the unified health services management in the Brazilian National Health System (SUS) in a large municipality: the case of São Paulo, Brazil. (written for the magazine Physis not submitted). Final Considerations. It is expected that these results and their presentation may contribute to a significant analysis of SUS in the São Paulo city and other large municipalities, as well as identifying the elements of consideration, of subjects for new researches or provide subsidies for the theoretical and conceptual (re)organization on the policy and management of health systems and the strengthening of SUS.

HENRIQUES, D. A. Molecular characterization of arboviruses isolated from mosquitoes fauna (Diptera: Nematocera) in Rondonia State (western Brazilian Amazon). 135 f. Doctor thesis Instituto de Ciencias Biomedicas, Universidade de Sao Paulo, Sao Paulo, 2008. The majority (2/3) of the Rondonia State territory is covered by the Amazon rainforest, an area in which a rich biodiversity of flora and fauna are present. The region vegetation’s, hydrography and equatorial climate create favorable ecosystems to the existence of different arthropods species. However, few studies are conducted focusing on the species involved, their distribution and capabilities as well as their hability in the arboviruses transmission. The present work aimed to detect Flavivirus, Orthobunyavirus and Alphavirus, arboviruses of medical importance, using RT-PCR and RT-PCR Duplex assays, in the mosquitoes species (Diptera: Nematocera) of the fauna from the Rondonia state, as well as the molecular characterization of the distinct identified arboviruses from the brazilian western amazon through a nucleotides sequencing reaction. A total number of 37.519 mosquitoes were collected and these belonged to 20 different families and genus that were subsequently distributed in approximately 700 pools. Four hundred and fifty mosquitoes pools were macerated in PBS albumin 4% and after centrifugation, the supernatant was aliquoted for isolation in cell culture and to carry out RT-PCR and RT-PCR Duplex. RT-PCR was able to detect the positive controls of Mayaro and Oropouche viruses in pools of 15 mosquitoes up to 10 4 and 10 1 TCID50/mL, respectively. The techniques failed to detect the Dengue virus 2 in those pools containing less than three orally infected mosquitoes. The β-actin primer was used as an endogenous control and RT-PCR was positive in 66,8% of samples. In pools containing from one to three mosquitoes, the endogenous control was detected in 47,56% of the samples. From those pools containing from 11 to 15, the detection of β-actin occurred in 86,42% of them. In total, 0,66% was positive for Oropouche virus and 0,66% for Cacipacore Flavivirus. The results observed by RT-PCR Duplex were similar to those of RT-PCR. The Oropouche virus was detected in two pools of genus Coquillettidea sp and in a pool of genus Deinocerites sp. The Cacipacore virus was found in two pools of Anopheles sp and in a pool of the genus Culex sp. Thus, the knowledge of the role of vectors in the dynamic process of arboviruses infections is extremely important, due to the fact that the epidemic prevention has been focused on insect control. In this way, sensitive techniques to get quick results become useful and appropriate in studies of epidemiological and vector surveillance and based on the results of this paper, it was shown that RT-PCR and RT-PCR Duplex permitted the detection of arboviruses in pools of mosquitoes collected in the field.

O aumento da expectativa de vida é um dos objetivos da saúde pública, porém, não menos importante, é a preservação da capacidade e independência funcional. A dificuldade de locomoção é um dos fatores mais limitantes das atividades de vida diária dos idosos. O envelhecimento e a inatividade contribuem para a diminuição da força muscular e da flexibilidade, aumento da incidência de quedas e perda da capacidade para as atividades da vida diária. O objetivo deste estudo foi avaliar a marcha de indivíduos idosos: sedentários e praticantes de exercícios, gerais ou resistidos. Foram feitas as medidas espaciais e temporais da marcha com utilização de pegadas com tinta marcadas em uma passadeira de papel e de um cronômetro. Os parâmetros avaliados foram medidos em marcha de dez metros. Resultados: O grupo de sedentários apresentou menor velocidade e maior tempo da marcha; menor comprimento dos passos e das passadas, e menor cadência. Entre os dois grupos praticantes de exercícios, não houve diferenças. A largura do passo não foi diferente entre os três grupos, pois depende mais dos fatores antropométricos para o posicionamento dos pés. Os exercícios físicos podem ter influenciado nos parâmetros da marcha do que a altura dos indivíduos. Este estudo mostra que o exercício físico regular melhora os parâmetros da marcha The rise of life expectancy is one of the objectives in public health but not less important is the preservation of the capacity and functional independence. The locomotion difficulty is one of the most limiting factors of the elderly daily activities (ADL). Ageing and inactivity contribute to the decrease of the muscular strength, flexibility, functional capacity an falls. The purpose of this study was to evaluate the gait of elderly subjects: sedentary, practitioners of general exercises and practitioners of resistance exercises. Spatial and temporal measures have been taken from the gait with the use of footprints with marked paints on a paper walkway and a stopwatch. The parameters evaluated were measured in gait of 10 meters. Results: the sedentary group presented lower gait speed, shorter length of steps and strides and decrease of the cadence. There were no differences between the exercises group. There was no difference in the step width among the three groups. Physical exercises have more influence on the gait parameters than the subjects height. This study demonstrates that regular physical exercise improve the gait parameters

Uma das maiores preocupações relacionadas a segurança transfusional é a possibilidade de transmissão de doenças infecciosas atraves de sangue transfundido, entre as quais se encontram as hepatites B e C, o HIV e a sífilis. Este trabalho teve como objetivo estudar doadores que passaram pelo processo de seleção pré-doação no Hemocentro de Ribeirão Preto e apresentaram resultados de testes sorológicos positivos para essas doenças/infecções.Estudou-se a frequencia dos resultados positivos entre os individuos que doaram sangue no hemocentro de Ribeirão Preto ou em seu posto de Coleta Central,de 1o.de julho de 2005 a 31 de julho de 2006. Os indivíduos com resultados sorológicos positivos confirmados em um segundo teste foram caracterizados segundo algumas variáveis demográficas e socioeconomicas, tendo sido identificados seus fatores de risco e as causas determinantes de sua não-detecção na triagem clínica (TC). Estudaram-se também o comportamento de doadores em relação a auto-exclusão confidencial (AEC) e os fatores determinantes desse comportamento. Participaram da pesquisa 106 doadores, predominantemente do sexo masculino e casados, com menos de 40 anos de idade e baixa escolaridade. Eram principalmente doadores de primeira vez, procedentes de Ribeirão Preto e da região de Ribeirão Preto e pertencentes aos estratos econômicos C e D. As frequencias de marcadores sorologicos positivos encontradas foram: 0,07% para o HBsAg; 0,03% para o anti-HIV; 0,13% para o VDRL; 0,21 para o anti-HCV. Cerca de 40% dos participantes assumiram ter omitido fatores de risco na TC. Os motivos mencionados foram: não se sentir a vontade para falar ou não achar relevante fazê-lo; confiar totalmente nos exames; ter como objetivo conhecer sua condição sorológica; encontrar problemas relacionados a entrevista/triador; não confiar no sigilo das informações; sentir constrangimento diante de acompanhantes. Apenas 1,9% dos participantes utilizaram a AEC, e as justificativas alegadas foram: estar se sentindo bem e, por isso, não julgar necessário; não se considerar de risco;não achar que tivesse mentido na entrevista. Os achados indicam a necessidade de mudar a abordagem dos doadores na TC, de rever os procedimentos de captação de doadores e de reavaliar profundamente os procedimentos de orientação/conscientização dos candidatos a doação, procurando tornar esses procedimentos mais eficazes. One of the main concerns related to the safety of transfusion is the possibility of diseases/infections transmitted through blood transfusion,amongst which we may include B and C hepatitis, HIV and syphilis.This paper aimed at studying the donors who have undergone the pre-donation selection at Blood Center of Ribeirao Preto and have presented positive serological results for theses diseases/infections.We have studied the frequency of positive results amongst the individuals who have donate blood at Blood Center of Ribeirao Preto and at the Central Collection Unit, from July 1st 2005 to July 31st 2006. The individuals who have had their positive serological results confirmed at a second test were classified according to demographic and socioeconomic variables, the risk factors present amongst them were identified, as well as the determinant causes for detection failure at the clinical screening. A study on the donors\' behavior towards the confidential self-exclusion process and the determinant factors that caused such behavior was also carried out. The research reached 106 donors, most of whom were males, married, under 40 and with low level of education. Most of them were first-time donors from the City of Ribeirão Preto and surrounding towns, belonging to the C and D economic classes. The frequencies of the serological markers were: 0,07% to HbsAg; 0,03% to anti-HIV; 0,13% to VDRL; 0,21 to anti-HCV. Around 40% of the participants affirmed having omitted risk factors at the clinical screening (CS). The reasons mentioned were the following: feeling unease to talk about it or not finding it relevant; relying completely on test results; having the aim of being aware of their real serological condition; issues related to the interview or screener; lack of trust in the confidentiality of the information he/she would give; feeling constrained in the presence of their companions. Only 1.9% of the participants used the confidential self-exclusion process, and the reasons given as justifications were: being feeling well and therefore not finding it necessary; not considering himself/herself a risk; not agreeing that he/she had lied during the interview. The findings indicate the necessity of changes as to the approach to donors at the CS, the need of a review of the procedures carried out to motivate people to become donors, and a deep revaluation of the procedures for orientation/awareness by candidates as to donation, aiming at turning them more effective.

Objetivo. Este estudo teve como objetivo analisar os resultados de um programa de triagem auditiva neonatal em recém-nascidos pré-termos de um hospital público do município de Itapetininga, Estado de São Paulo. Métodos. A triagem auditiva neonatal foi realizada em 230 neonatos com idade gestacional variando de 28 a 37 semanas, por meio do exame de emissão otoacústica evocada produto de distorção, utilizando-se o equipamento AUDX Biologics. Foram analisados os seguintes aspectos do programa: a) as condições operacionais da realização da triagem auditiva no hospital pesquisado; b) as características da prematuridade presentes na população estudada; c) os resultados do programa de triagem em relação à identificação de casos com alterações auditivas; d) as amplitudes médias de resposta da emissão otoacústica evocada produto de distorção encontradas na população avaliada. Resultados. Foram avaliados 90,55% dos prematuros nascidos num período de 15 meses de coleta. Verificou-se que as condições operacionais para a realização do programa foram adequadas. Não foram encontrados casos de alterações auditivas. As amplitudes médias de respostas da emissão otoacústica evocada produto de distorção (PD-RF) variaram de 11 a 17 dBNPS, nas freqüências de 2 a 5 kHz; tendo sido observadas respostas superiores para as orelhas direitas do sexo feminino. Conclusões. Os prematuros constituíram uma população de baixo risco para deficiência auditiva. As amplitudes de respostas da emissão otoacústica com equipamento portátil mostraram-se semelhantes às obtidas com equipamentos clínicos. O programa de triagem auditiva neonatal pode ser realizado no hospital estudado. Porém, modificações na rotina devem ser realizadas para que possa ser implantado um programa universal. Objective. The aim of this paper is to analyze the results of a newborn hearing screening programme applied to preterm infants from the city of Itapetininga, state of São Paulo. Methods. Newborn hearing screening was performed to 230 preterm newborns with gestational age from 28 to 37 weeks. The equipment used was AUDX Biologics performing the registry of otoacoustic emissions distortion product. The presence of cocleopalpebral reflex was performed too. It were analyzed: a) operational conditions of newborn hearing screening programme; b) the characteristics of infant?s prematurity; c) incidence of hearing impairment; d) amplitude of otoacoustic emissions distortion product responses (PD-NR). Results. The operational conditions of newborn hearing screening are suitable. No hearing impairment was identified, probably because the prematurity characteristics had load this group to low risk for hearing loss. The mean values from amplitude (PD-NR) extended from 11 to 17 dBSPL from 2 to 5 kHz. They were observed superior values to right ears of the female group. Conclusions. Newborn?s characteristics of prematurity showed low risk for hearing impairment in this study. Portable equipment responses are similar to clinic ones. Newborn hearing screening programme is feasible to be implemented at that kind of hospital. However, some adjusts need to be done in order to extends it to the universal protocol.

O objetivo do presente estudo foi avaliar comparativamente in vivo, o efeito antimicrobiano do hipoclorito de sódio a 1%, da clorexidina a 2% e do detergente derivado do óleo de mamona a 10% (Endoquil) na irrigação de canais radiculares de 18 dentes anteriores superiores humanos com necrose pulpar e lesão periapical visível radiograficamente. Após os procedimentos de antissepsia, realizou-se a abertura coronária e a 1ª colheita microbiológica, utilizando quatro cones de papel esterilizados. A instrumentação foi efetuada com limas tipo K, juntamente com a aplicação de 1,8mL de cada uma das soluções irrigadoras. Em seguida, realizou-se a neutralização das substâncias antimicrobianas e a 2ª colheita microbiológica. Decorridas 72h, efetuou-se a 3ª colheita, e as amostras foram transportadas em solução de PBS. Procedeu-se a diluição decimal seriada e as amostras foram semeadas em placas contendo o meio Tryptic Soy Agar e incubadas em aerobiose e anaerobiose para posterior contagem do número de unidades formadoras de colônia. Baseados na análise dos resultados concluiu-se que: todas as substâncias irrigadoras utilizadas mostraram uma redução significante do número de bactérias da 1ª para a 2ª colheita, tanto em anaerobiose como em aerobiose, não sendo significante apenas a clorexidina em aerobiose; da 2ª para a 3ª colheita houve um aumento considerável do número de bactérias, com diferença significante entre as substâncias irrigadoras tanto em anaerobiose como em aerobiose, não sendo significante apenas a clorexidina em aerobiose; a constatação do aumento do número de bactérias da 1ª para a 3ª colheita evidenciou que só o preparo biomecânico do canal radicular não foi suficiente para conter a recolonização bacteriana; em relação à 2ª colheita, as três substâncias irrigadoras se comportaram de maneira semelhante na redução do número de microrganismos; em relação à 3ª colheita em anaerobiose, a clorexidina a 2% e o detergente derivado do óleo de mamona a 10% foram significantemente melhores que o hipoclorito de sódio a 1% quanto à presença bacteriana. The objective of this study was to evaluate in vivo the antimicrobial activity of 1% sodium hipochlorite, 2% chlorexidine and 10% mamone detergent in 18 root canals of humans anterior teeth whit pulpar necrosis and periapical lesion. After antiseptic access surgery, the first microbiological sample was taken with four sterile absorvent paper cones. The instrumentation was realized with K files using 1,8mL of the irrigants solutions and the second microbiological sample was taken. The third collects was made 72h after the biomechanical prepare. The samples were transported on PBS solution, submitted to serial decimal dilution, streaked in plates with Tryptic Soy Agar, and incubed in aerobic and anaerobic conditions. The number of colony-forming units (cfu) was counted after incubation periods. According to the sample studied, the applied methodology and the results obtained, we concluded that all the irrigant solutions showed significant numerical reduction after biochemical preparation, and an increase in number of microorganisms after 72 hours. The biochemical preparation was not able to prevent the bacterial recolonization. After 72 hours the 2% chlorexidine and 10% mamone were more effective than 1% sodium hipochlorite

A hipertrofia ventricular esquerda (HVE) é importante fator de risco cardiovascular. O objetivo deste estudo retrospectivo foi verificar a associação de critérios eletrocardiográficos de HVE com as características anatômicas e histológicas do coração, em 51 pacientes submetidos à necropsia. Procedeu-se à medição do diâmetro transverso dos cardiomiócitos e da porcentagem de fibrose do ventrículo esquerdo e direito. Entre os pacientes que apresentavam HVE anatômica, o critério de Romhilt foi positivo em 92,3%, sendo superior aos demais critérios avaliados, com especificidade de 89,5% e sensibilidade de 68,8%, Foi o único que se associou a características anatômicas e histológicas do coração The left ventricular hypertrophy (LVH) is an important cardiovascular risk factor. The purpose of the present retrospective paper is to examine the association of LVH electrocardiographic criteria with both anatomical and histological characteristics of the heart on 51 patients submitted to the necropsy. The study carried out the measurement of the transverse diameter of cardiomyocytes, as well as the percentage of fibrosis at both left and right ventriculi. Among those patients who presented anatomic LVH, the Romhilt criterium resulted positive in 92.3% of the cases, thus surpassing the other criteria evaluated, with specificity and sensibility up to 89.5% and 68.8% respectively. Quimicamente, o ecstasy é a 3,4-metilenodioximetanfetamina (MDMA), um composto sintético com propriedades estimulante central e alucinogênicas. Algumas substâncias análogas à MDMA, já identificadas em comprimidos de ecstasy, são principalmente: 3,4-metilenodioxietilanfetamina (MDEA), 3,4-metilenodioxianfetamina (MDA), metanfetamina e anfetamina. Os adulterantes mais comuns, normalmente encontrados são: cafeína e efedrinas. O objetivo deste trabalho foi a validação de um método analítico para quantificar a MDMA em comprimidos e cápsulas de ecstasy, através da cromatografia em fase gasosa com detector de nitrogênio/fósforo (GC/NPD). Substâncias análogas à MDMA e adulterantes também foram identificados. Amostras de comprimidos e cápsulas de 25 diferentes lotes, apreendidos como ecstasy em São Paulo (SP), foram analisadas pelo método proposto. Desse total de amostras, 21 continham somente MDMA (84%) e apenas 1 delas apresentou MDMA associada com cafeína (4%). A concentração total de MDMA nessas amostras variou entre 30,9 e 92,7mg, resultando em uma média aritmética de 63mg. Chemically, \"ecstasy\" is 3,4-methylenedioxymethamphetamine (MDMA), a synthetic compound with stimulant and hallucinogenic properties. Some MDMA analog substances such as 3,4-methylenedioxyethylamphetamine (MDEA), 3,4-methylenedioxyamphetamine (MDA), methamphetamine and amphetamine have already been identified in \"ecstasy\" tablets. Caffeine and ephedrines are the most common adulterants also found. The aim of this paper is to describe the validation of an analytical method to quantify MDMA in \"ecstasy\" tablets and capsules. Gas chromatography with nitrogen/phosphorus detector was used in the method. Analog substances to MDMA and adulterant compounds were also identified. Samples from 25 lots of tablets seized in the city of São Paulo were analyzed. From that total, 21 showed only MDMA (84%) and just 1 of them presented MDMA plus caffeine (4%). MDMA total concentration in these samples had a variation between 30.9 and 92.7mg, resulting in an arithmetic average of 63mg.
