ABSTRACT Objective: to assess the effectiveness of 5% Brazilian green propolis (ointment) in individuals with chronic ulcers. Methods: a randomized clinical trial, developed with 40 patients randomized equally to control group (treated with essential fatty acid) and experimental group (treated with 5% green propolis) for 30 days. The outcomes of interest were sociodemographic, clinical and laboratory characteristics, lesion characteristics, such as type of tissue in the bed, presence of exudate, edge characteristics, microbial content and pain. Results: regarding sociodemographic, clinical and laboratory characteristics, the two groups did not show statistically significant differences. After assessment in 30 days, an effect was observed for both treated groups, but for the experimental group, greater effectiveness in terms of the type of tissue in the bed, type of exudate, edge characteristics, microbial content and pain. Conclusion: propolis-based ointment showed a healing effect, presenting itself as a potential tool in healing chronic ulcers.

Erythema multiforme is a mucocutaneous disease characterized by the acute appearance of blisters and ulcers, with or without oral involvement. In this report, a 12-year-old female patient presented with extensive ulcerative lesions on the upper and lower lips, with painful symptoms and areas of bleeding and suppuration. Despite these lesions being restricted to the lips and skin around, the patient was diagnosed with erythema multiforme. Dexamethasone was then prescribed, in addition to the use of lip balm. At the first return evaluation, there was no regression of the clinical picture, and the medication was replaced by prednisone, associated with amoxicillin and 0.12% chlorhexidine mouthwash. In the second return visit, there was considerable regression of the lesion, and the process of gradual discontinuation of the corticosteroid was initiated. After 1 week, the lesions had healed, and the drug was discontinued. The oral cavity may be the only site affected by erythema multiforme, so knowledge of this disease is necessary for its correct diagnosis and treatment.

Abstract Trichosporon asahii is the causal agent of trichosporonosis. Patients with immunosuppression or hematological malignancies are at higher risk of infection. Skin and mucosal involvement appear as fast-growing papulonodular lesions and necrotic ulcers. Internal organ dissemination is lethal. Therapeutic success depends on the underlying disease. Here, the authors present the first case of disseminated mucocutaneous trichosporonosis in a patient with a post-mortem diagnosis of histiocytic sarcoma, a rare and aggressive haematolymphoid neoplasm. Regretfully, death occurred despite treatment with liposomal amphotericin B and supportive measures, showcasing the fatality of both diseases.

Vulvar ulcers are a frequent reason for doubt and difficulty of etiological diagnosis. Among the causes of genital ulcers, we can mention sexually transmitted infections such as herpes simplex virus infection, syphilis, chancroid, granuloma inguinale or Donovanosis, and lymphogranuloma venereum, as well as non-infectious etiologies including psoriasis, sexual trauma, Behçet’s syndrome, Wegener’s granulomatosis, and Zoon Vulvitis. In the present study, a case of a seven months evolution genital ulcer patient (who did not respond to the various proposed therapies) was referred to the Service of Vulvar Pathology of the Hospital Universitário Antônio Pedro (Universidade Federal Fluminense). A biopsy of the lesion was performed, and the histopathological report was compatible with Zoon Vulvitis. The treatment with corticosteroids was initiated. Patient presented a successful evolution. An extensive review of the literature on differential diagnoses of vulvar ulcers was carried out in order to guide a clinical reasoning and a therapeutic approach to the different etiologies.

Objective: to analyze the effects of educational technologies in the prevention and treatment of diabetic ulcers., Method: a systematic review conducted in seven databases, a bibliographic index, an electronic library and the Gray Literature. The sample consisted of 11 randomized controlled clinical trials. The synthesis of the results was descriptive and through meta-analysis., Results: the predominant educational technologies were training sessions and verbal guidelines, with soft-hard technologies standing out. When compared to usual care, the educational technologies presented a protective factor to prevent the incidence of diabetic ulcers (RR=0.40; 95% CI=0.18-0.90; p=0.03) and the certainty of the evidence assessment was low. The educational technologies also had a protective factor to prevent the incidence of lower limb amputations (RR=0.53; 95% CI=0.31-0.90; p=0.02) and certainty of the evidence was very low., Conclusion: soft-hard educational technologies such as structured verbal guidelines, educational games, lectures, theoretical-practical training sessions, educational videos, folders, serial albums and playful drawings, and hard technologies such as therapeutic footwear, insoles, infrared digital thermometer, foot care kits, Telemedicine app and mobile phone use, were effective for the prevention and treatment of diabetic ulcers, although more robust studies are required.

Chronic ulcers of the lower limbs are common and recurrent, especially in the elderly population, they are disabling injuries that generate a great socioeconomic burden. This scenario encourages the development of new, low-cost therapeutic alternatives. The present study aims to describe the use of bacterial cellulose in the treatment of lower limb ulcers. This is an integrative literature review, carried out in the PubMed and Science Direct databases by associating the descriptors, with the inclusion criteria being clinical studies in the last 5 years, available in full in English, Portuguese and Spanish. Five clinical trials were analyzed and the main therapeutic effects obtained in the experimental groups that used bacterial cellulose dressings were a reduction in the area of the wounds, one of the studies showed a reduction of 44.18cm2 in the area of the wound, the initial lesions measured on average 89.46cm2 and at the end of the follow-up, they had an average of 45.28cm2, since the reduction in pain and the decrease in the number of exchanges were advantages described in all groups that used the BS. It is concluded that BC dressings are an alternative for the treatment of lower limb ulcers, their use also reduces operational costs related to the treatment of ulcers.

Objective: To analyze the repercussions of sickle cell disease and sickle cell ulcer for men in the world of work and discuss the challenges faced to remain in the work environment., Method: A qualitative study, developed at the dressing clinic and at a stomatherapy clinic. Twenty men with sickle cell disease and sickle cell ulcer participated, applying a semi-structured interview script. The software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires was used for treatment and lexical data analysis., Results: The Descending Hierarchical Classification enabled the creation of classes: Man with sickle cell disease and sickle cell ulcer: experiences and repercussions; and Coping measures adopted by men with sickle cell disease and sickle cell ulcer to stay at work., Conclusion: Disease and injury repercussions involve biopsychosocial dimensions, highlighting the need for professional training to assist with competence and humanity. Strategies adopted to maintain work are breaks in the working day, use of analgesics to relieve pain, allocating time during work to apply dressings.

Background: The physiological stress of critically ill patients can trigger several complications, including digestive bleeding due to stress ulcers (DBSU). The use of acid secretion suppressants to reduce their incidence has become widely used, but with the current understanding of the risks of these drugs, their use, as prophylaxis in critically ill patients, is limited to the patients with established risk factors., Aim: To determine the appropriateness of the use of prophylaxis for stress ulcer bleeding in acutely ill patients admitted to intensive care units and to analyze the association of risk factors with adherence to the prophylaxis guideline., Methods: Retrospective, analytical study carried out in three general adult intensive care units. Electronic medical records were analyzed for epidemiological data, risk factors for DBSU, use of stress ulcer prophylaxis, occurrence of any digestive bleeding and confirmed DBSU. The daily analysis of risk factors and prophylaxis use were in accordance with criteria based on the Guidelines of the Portuguese Society of Intensive Care for stress ulcer prophylaxis., Results: One hundred and five patients were included. Of the patient days with the opportunity to prescribe prophylaxis, compliance was observed in 95.1%. Of the prescription days, 82.35% were considered to be of appropriate use. Overt digestive bleeding occurred in 3.81% of those included. The occurrence of confirmed DBSU was identified at 0.95%. Multivariate analysis by logistic regression did not identify risk factors independently associated with adherence to the guideline, but identified risk factors with a negative association, which were spinal cord injury (OR 0.02 p <0.01) and shock (OR 0.36 p=0.024)., Conclusion: The present study showed a high rate of adherence to stress ulcer prophylaxis, but with inappropriate use still significant. In the indication of prophylaxis, attention should be paid to patients with spinal cord injury and in shock.

Objectives: to compare the effect of adjuvant low-laser light therapy versus conventional treatment alone on venous ulcer healing., Methods: this is a randomized clinical trial with 40 patients randomized equally to a control group (topical and compressive treatment) and intervention group (adjuvant low-laser light therapy). Outcomes of interest were Wound Healing: Secondary Intention and Tissue Integrity: Skin & Mucous Membranes, as described in the Classification of Nursing Outcomes/NOC., Results: groups with similar sociodemographic and clinical characteristics. Eighty-two ulcers were assessed, with an average duration of 1 to 5 years, in 1,066 nursing consultations, with a statistically significant difference in the time and number of healed ulcers (intervention group). There was a significant improvement in the nursing outcomes under study and in eight clinical indicators., Conclusions: low-laser light therapy improves and reduces tissue regeneration time, contributing to advances in wound treatment.

Objective: to describe the social representations of patients with vasculogenic ulcers about quality of life and analyze such representations from the social context of patients' insertion in relation to the healthcare service., Method: a qualitative research, based on social representations, developed in a Family Clinic in Rio de Janeiro with 30 patients with vasculogenic ulcers for over 90 days. Data were produced through interviews and submitted to lexicographical analysis using the Alceste software., Results: the weaknesses of care in the healthcare service to which patients were linked contributed to exacerbate the reconfiguration of daily life that the occurrence of ulcers implies, due to the presence of pain and physical restrictions, influencing the representation of quality of life., Final Considerations: the representation was objectified as a counterpoint to the image of "being happy", and actions expressed proactivity in the search for a cure or passivity due to lack of clinical results.
