Introduction: The National Scientific Congress (NSC) is an annual event held in Peru in which medical students from all over the country present their research work. Objectives: To determine the publication rate in indexed scientific journals of research papers submitted to the NSCs held between 2010 and 2014, as well as the factors associated with their publication, and to describe the characteristics of the published papers. Materials and methods: Retrospective study in which 407 abstracts of research papers were reviewed. A publication time window of 6 years (from submission) was considered. A bivariate analysis was performed to assess differences between categorical and numerical variables using the chi-square and Mann-Whitney U tests, respectively. In addition, a multivariate analysis was performed using Poisson regression models (a crude and an adjusted model), calculating relative risk (RR) values with their respective 95% confidence intervals (95%CI) to determine the factors associated with the publication of papers. Abstract: Promoting socioemotional skills has been highlighted among the evidence to prevent suicidal behavior in childhood and adolescence. This review aimed to map and analyze national and international scientific papers on initiatives and programs for the prevention of suicidal behavior in adolescence based on the theoretical framework of socioemotional skills. It is a scoping review using the methodology proposed by the Joanna Briggs Institute. Eleven academic bibliographic databases were analyzed, and searches were conducted on institutional websites related to suicide prevention and Google. Papers in Portuguese, Spanish, French, and English from 2010 to July 2022 were included in the review, which consisted of 97 studies, analyzed through data matrix and thematic grouping. The results show that most are international and focused on suicide, not on self-harm alone. In general, they have an informational and instructional bias for professionals, institutions, and governments, proposed laws, programs and action plans, studies on the role of socioemotional skills and intervention research. Few strategies have been clearly tested and validated. The key elements are the ability to perceive, recognize, understand, express, and regulate one’s own emotions, get motivated, and build empathy in relationships. Schools are key players in this process and the health system should act as a collaborative network. National and local prevention plans are required, emphasizing the role of schools, the health sector, and intersectoral coordination to promote health and quality of life.

ABSTRACT Introduction Chronic kidney disease is a progressive and irreversible loss of kidney function and considerably affects the lives of patients and their families. Its high incidence necessitates efficient public policies for prevention and treatment. However, policies for chronic kidney disease education and awareness are scarce. Objective To evaluate global public policies for the prevention and treatment of chronic kidney disease adopted in various regions, aiming to comprehend the differences between various models. Methods This integrative review followed PRISMA recommendations and included papers published between 2016 and 2021 across several databases. Results The 44 selected articles were categorized into three themes: structural and financial aspects of the organization of renal healthcare, access to renal healthcare or management of chronic kidney disease, and coping strategies for chronic kidney disease or kidney health. Critical analysis of the papers revealed global neglect of kidney disease in political agendas. Considerable policy variations exist between different countries and regions of the same country. Our research highlighted that free and universal health coverage, especially for the most vulnerable patients, is crucial for accessing treatment owing to the prohibitively high treatment costs. Conclusion Social, economic, and ethnic inequalities strongly correlate with disease occurrence, primarily affecting minority groups who lack health support, especially for the prevention and treatment of chronic kidney disease.

Abstract Introduction: Brazilian scientific production has shown substantial growth and achieved international visibility. However, in general, the participation of women in scientific activities remains limited. Objective: This study aimed to evaluate the indicators of scientific productivity of women fellows of the Council for Scientific and Technological Development (CNPq) in the field of Medicine. Method: This cross-sectional study was conducted on 541 (211 women, 39%) researchers registered as recipients of CNPq research productivity (PQ) scholarships in Medicine according to a list provided in December 2022. Results: There was a predominance of male researchers (n=330; 61%). In both the male and female groups, most researchers were at level 2, with 62.5% women and 47.2% men (p=0.018). All 211 female PQ scholars were distributed among 37 different institutions and published 34,969 papers in scientific journals, averaging 165.7 articles per researcher. In the last five years of the study period (from 2018 to 2022), 9,679 papers were published. Over their careers, the 211 researchers supervised 5,440 undergraduate research students, 4,144 master’s degree students, and 2,923 PhD candidates. Applying appropriate training loads in accordance with the defined objectives promotes optimal physical and physiological adaptations, reduces the likelihood of illness and injury and therefore increases the possibility of success during the Rugby. The aim of this review was to compile and systematize the information in the literature on the association between training load variables (internal and external) and performance outcomes in Rugby. As such, the main objective will be to conduct a systematic review of the published literature to identify the physical and physiological performance variables in Rugby sport to monitor the training load. Following the preferred reporting item for systematic reviews and meta-analyses (PRISMA) and PICOS approach, the search was conducted systematically only in the PubMed database, which, in itself, also restricts the search spectrum of the paper, thus conferring a limitation to the present academic work. The search included specific terms - namely, Rugby; Training Load; - throughout the possible temporal spectrum in PubMed, since there is still little robustness in the literature of the content in question in the sport of Rugby. Articles were selected using pre-defined selection criteria, namely observational studies, randomized clinical studies, and clinical trial studies. The initial electronic search with the variables "Training Load", "Rugby" led to a set of 114 articles, of which only 96 articles were selected after the selection of variables for presenting observational study designs and/or randomized clinical trials. A further screening was carried out, and based on the inclusion criteria of the papers, the result of the analysis of the relevance of the studies was only 19 articles. Three duplicates were eliminated, therefore, the final set of analysis resulted in 16 articles. From the studies compiled in this review, it seems that there is a strong correlation between the perceived exertion (RPE) and the prescription and definition of the training load applied in Rugby athletes. The RPE reflects the most applied and analyzed variable throughout all the studies. Several articles reflect that there is a strong relationship between the training load, and the inter-individual capacity of each athlete and their tolerance to the load (player load).

Regular physical activity (PA) is essential for health maintenance and disease prevention. COVID- 19 and its social isolation affected patients with cardiovascular (CV) disease. This is a scoping review aiming to map, analyze, and summarize the impacts caused by the isolation of the COVID-19 pandemic concerning PA and CV health in adults. Methods: The review was based on PRISMA-ScR. PubMed, PubMed Central, BVS/BIREME, Scopus, Web of Science, CINAHL, Brazilian Digital Library of Theses and Dissertations, and SciELO databases were selected to search. Two search phases were performed: May 2021 and March 2022. English-language full-text papers were included. Initially, the titles and abstracts of the retrieved documents were read and screened based on the research question. A third reviewer resolved disagreement through consensus meetings. An assessment form was adapted for data extraction. The study was registered in the Open Science Framework platform under the DOI https://doi.org/10.17605/OSF.IO/AH6P8. Results: 11 papers were included and demonstrated that regular PA could be challenging in social isolation environments. Most participants were moderately active and performed low-intensity PA. Confinement led to the limitation of everyday activities, nutritional disorders, and reduced PA, contributing to weight gain. Quarantine was associated with stress and depression, leading to an unhealthy diet. Possible solutions point to home PA, considering the baseline health and CV status. Conclusion: The post-COVID- 19 era and its consequences point to the pressing need for planning strategies that increase and maintain home PA as a protective measure for the general population, considering CV health, resources, and household space.

The Caribbean is experiencing a worsening epidemic of obesity and noncommunicable diseases (NCDs) and it has the worst rates of premature mortality from cardiovascular diseases in the region of the Americas. Creating enabling environments to improve dietary diversity would help reduce obesity and diet-related NCDs. The Improving Household Nutrition Security and Public Health in the CARICOM project aimed to increase dietary diversity in the Caribbean, and to determine and implement effective, gender-sensitive interventions to improve food sovereignty, household food security, and nutrition in CARICOM states. Primary quantitative and qualitative research, scoping reviews, stakeholder engagement, implementation of interventions and dissemination activities were undertaken. This paper describes the overall project design and implementation, discusses challenges and limitations, and presents core achievements to inform further work in Small Island Developing States throughout CARICOM to advance the nutrition agenda in the Caribbean. The results of the project’s research activities are presented in other papers published in this special issue on nutrition security in CARICOM states.

Abstract Objective Our purpose was to facilitate the simulation of preoperative correction to enable shared doctor-patient decision-making in individuals undergoing high tibial osteotomy (HTO). Methods A total of 22 patients underwent high tibial osteotomy using internal or external fixation devices for medial compartment osteoarthritis of the knee. Preoperatively, assessment of deformity parameters and simulation of the corrective osteotomy was done in the presence of the patient, using Bone Ninja. Postoperatively, the patient's satisfaction level with the quality of explanation provided by the use of this software was assessed using the Patient Satisfaction Questionnaire-short (PSQ-18). A comparison of the correction obtained using paper cuttings and the simulation software was performed. Results All patients were satisfied with their role in the decision-making process. They showed a good understanding and comprehension of the proposed surgery. There was no statistically significant difference between simulated preoperative Medial Proximal Tibial Angle (MPTA) obtained by paper cuttings and software-assisted correction. The PSQ-18 mean score for communication was 4.24 (0.88), for technical quality it was 4.11 (0.59) and for general satisfaction it was 3.11 (0.68). Conclusion Bone Ninja is an effective, convenient, user-friendly and cost-effective deformity planning tool that supersedes the arduous traditional method of paper tracings and scissors.

HEARTS in the Americas is the regional adaptation of the World Health Organizations Global Hearts Initiative, which will be the model for risk management for cardiovascular disease (CVD) in primary health care in the Region of the Americas by 2025. It has already been implemented in 21 countries and 1045 primary health care centers throughout Latin America and the Caribbean. It takes a public health and health systems approach to systematically introduce simplified interventions at the primary health care level and focuses on hypertension as a clinical entry point. This paper introduces a new, improved application (app), the main component of which is the calculator for CVD risk and hypertension management. The paper summarizes the risk assessment approach and the methodology used by the World Health Organization to update its cardiovascular risk charts in 2019; describes the app, its use, functionality and validation process; and provides a set of practical recommendations for optimizing CVD risk and hypertension management by using the app in clinical practice. The HEARTS app is a powerful tool to improve the quality of care provided in primary health settings. The creation and dissemination of the HEARTS app is an essential step in the journey towards eliminating preventable CVD in the Americas.

Se estudia la disponibilidad en acceso abierto de la producción científica del personal del Servicio Gallego de Salud (Sergas) de los años 2014 a 2019, su evolución a través de las distintas vías de acceso y qué volumen se podría depositar en el repositorio institucional. METODOLOGÍA: A partir de una exportación de Sophos, la herramienta que mide la producción científica del Sergas, se analiza el conjunto de publicaciones en las que participa al menos un autor con filiación institucional del Sergas. Se utilizaron las APIs de Unpaywall, DOAJ y la función Find Full Text de EndNote para determinar si el artículo tenía una versión de texto completo disponible. RESULTADOS: La producción científica del Sergas de 2014 a 2019, excluidas las comunicaciones a congresos, asciende a 7.792 artículos de los cuales el 44.10% de pueden clasificar como de acceso abierto real (dorado, bronce o híbrido), el 51.16% están publicados en revistas que permiten el auto-archivo y solo un 4.75% no admiten acceso abierto o no ofrecen información sobre su política de acceso. Estos artículos se publicaron en 1.695 revistas diferentes, de las cuales 409 siguen el modelo de acceso abierto dorado. CONCLUSIONES: El 95% de los artículos revisados pueden estar disponibles para el usuario final por alguna de las vías de acceso abierto y podrían ser depositados en el repositorio institucional./ This paper studies the open access availability to the Galician Health Service (SERGAS) staff´s scientific production between 2014 and 2019, its evolution through the different open access routes and the volumen of publications that could be deposited in the institutional repository. METHODOLOGY: Based on an export from Sophos, (the tool that measures the scientific production of the SERGAS), this paper analizes the set of publications in which at least one author with SERGAS institutional affiliation. The APIs of Unpaywall, DOAJ and the Find Full Text function of EndNote were used to determine whether the article had a full-text version available. RESULTS: The SERGAS scientific production between 2014 and 2019, excluding conference contributions, amounts to 7,792 articles. 44.10% of those articles can be classified as real open access (gold, bronze or hybrid), 51.16% are published in journals that allow self-archiving and only 4.75% do not support open access or do not provide information on their access policy. These articles were published in 1,695 different journals, 409 of wich follow the gold open access model. CONCLUSIONS: 95% of the articles reviewed would be available to the end user through one of the open access routes and could be deposited in the institutional repository.

Objectives. This paper describes and analyzes embedded implementation research and the empirical processes of planning for utilization, strategies to promote utilization and the sustainability of utilization of results from research led by decision-makers in Latin America and the Caribbean. Methods. This qualitative, descriptive and interpretive study is based on the findings from semistructured interviews with members of teams working under the Embedding Research for the Sustainable Development Goals initiative (2018–2019) as well as their responses to a self-assessment follow-up questionnaire 1 year after the project was completed. Results. Altogether 13 teams from 11 countries participated in the Initiative. Nine teams had a core team composed of a decision-maker as the principal investigator assisted by a researcher as co-principal investigator. Four teams included more than one co-principal investigator; and in five teams, the originally assigned principal investigator was replaced. There was an interesting relationship between the expected utilization of research results, the utilization strategies, the sustainability of research uptake and the teams’ collaboration modalities. When decision-makers and co-principal investigators were active participants, the intention to use the results and strategies for utilization were clearly oriented to improve implementation. In teams with basically a formal collaboration between the two principals, plans for utilization were unclear or focused on producing academic knowledge. The participation of implementers below the rank of principal investigator decision-maker may be relevant. Conclusions. Embedded implementation research is an innovative tool that may foster the utilization of research and strengthen health programs and services. Considering the internal dynamics of such research teams will enhance planning and strategies for research utilization as well as the sustainability of practical and actionable findings.

This paper highlights the important leadership role of the public health sector, working with other governmental sectors and nongovernmental entities, to advance environmental public health in Latin America and the Caribbean toward the achievement of 2030 Sustainable Development Goal 3: Health and Well-Being. The most pressing current and future environmental public health threats are discussed, followed by a brief review of major historical and current international and regional efforts to address these concerns. The paper concludes with a discussion of three major components of a regional environmental public health agenda that responsible parties can undertake to make significant progress toward ensuring the health and well-being of all people throughout Latin America and the Caribbean.

Abstract Preoperative planning (PP) is critical to ensure a successful outcome in orthopaedic trauma surgery. Since it was first described thirty years ago, no modifications have been made to the original preoperative planning (OPP) technique, which was written by hand using pen and paper. We believe that the use of presentation software (such as Microsoft PowerPoint, Microsoft Corp., Redmond, WA, US) can ease, improve and update the OPP and complement three-dimensional PP. The objective of the present paper is to describe our method for PP using a presentation software (PS).

The Pan American Health Organization (PAHO) and its Member States have been leading the efforts to eradicate wild poliovirus in the Region of Americas since smallpox's successful elimination in 1971. The region became the first to be certified free of wild poliovirus in 1994. However, in July 2022, an unvaccinated patient with no recent travel history was diagnosed with poliomyelitis in the United States of America. In response to the emergence of a circulating vaccine-derived poliovirus in the United States, PAHO established the Polio Incident Management Support Team. This team has been coordinating response efforts, focusing on: coordination, planning, and monitoring; risk communication and community engagement; surveillance and case investigation; vaccination; and rapid response. In this paper, we identified and documented best practices observed following establishment of the Incident Management Support Team (September 2022–2023) through a comprehensive review and analysis of various data sources and country-specific data from the polio surveillance dashboard. The aim was to share these best practices, highlighting technical support and implementation of polio measures by Member States. Despite several challenges, the Americas region remains polio-free. Polio risk is declining, with a July 2023 assessment showing fewer countries at medium, high, and very high risk. This progress reflects improved immunization coverage, surveillance, containment, health determinants, and outbreak preparedness and response. The PAHO Polio Incident Management Support Team has played a key role in supporting these efforts.

Introduction: There seems to be a fairly close relationship between genital and anal lesions associated with human papillomavirus (HPV) and with the pathogenesis of malignancies in such sites. Objective: This review seeks to analyze published papers that suggest the necessity for anal intraepithelial lesions screening in women with HPV genital intraepithelial lesions. Methods: A systematic review of original research published between 2001 and 2014 was performed in the databases PubMed, HighWire, Cochrane Library and Google Scholar. The keywords for the search were: “screening” or “early detection of cancer” and “anus” or “anus diseases” or “neoplasms anus” and “woman” or “women” and “HPV” or “DNA probes, HPV”. Results: 1,203 articles were selected in the first search. After the reviewers applied exclusion and inclusion criteria and analyzed the papers, 15 were selected to compose this review. Most of them showed higher rates of anal intraepithelial lesions in women with genital intraepithelial lesions than in regular women. We also found a correlation between genital neoplasms and anal infection by oncogenic HPV types and the role of the virus in the pathogenesis of anal cancer. Conclusion: Screening for anal lesions in women with HPV genital lesions should be indicated, and further studies are required to determine the best methods.

Abstract: Since May 2020, we have been conducting a comprehensive study to understand the natural history of SARS-CoV-2 infection in Rio de Janeiro, Brazil. Our focus has been on following families, systematically collecting respiratory tract swabs and blood samples, monitoring symptoms, and gathering data on vaccine status. This paper aims to describe the household cohort across five epidemic waves of SARS-CoV-2, providing an overview of the collected data and a description of the epidemiological, clinical, and immunological characteristics and incidence of SARS-CoV-2 infection. Our cohort includes 691 participants from 189 households. During the five epidemic waves, we detected 606 infections. The incidence density of SARS-CoV-2 infection ranged from 4 (Delta) to 56 (B.1.1.33) per 1,000 person-week, with a peak in wave B.1.1.33 in all age groups. The seroprevalence of SARS-CoV-2 antibodies (IgG anti spike protein) varied from 37%, in the pre-VoC period, to 99%, in the Omicron period, progressively increasing after each wave in a similar manner regardless of age. As we have monitored the cohort continuously since the beginning of the pandemic, we were able to collect data across different scenarios according to the predominant lineage in circulation. Via active monitoring of families, we were able to carry out an epidemiological surveillance on SARS-CoV-2, including its variants, persistence of symptoms, and changes in immunity over time in the population, contributing to knowledge of the natural history of SARS-CoV-2 infection.

BACKGROUND AND OBJECTIVES: Music therapy promotes positive effects on cognition, psychosocial aspects, control of somatic symptoms and quality of life. The influence on pain is still a controversial topic. The objective of this study was to investigate the state of the art of musical interventions and music therapy in the treatment of acute or chronic pain, as well as developing criteria to facilitate the creation of strategies for applying music in clinical research. CONTENTS: This research was based on papers published between August 2010 and March 2023 in the Pubmed database. Of these, open access literature review articles were selected. A total of 44 studies whose results and conclusions showed that there is evidence of pain relief in many clinical and environmental conditions were selected, although there is great heterogeneity in methods and controversy in the literature regarding positive and adverse or side effects. The music chosen by a professional or patient alleviates significantly the painful experience by many mechanisms, although statistical data is limited or controversial. CONCLUSION: Music appears to have a positive impact on various environmental situations or biological conditions that result in painful experience and, therefore, can significantly alleviate pain, which is supported by evidence of complex neural mechanisms involving the descending pain inhibitory system. The benefits justify the use of music as a low-cost and low-risk therapeutic strategy, and the methodological challenges justify a more careful design in future research, including details of the type of musical intervention performed.

More than 8 million older people in Latin America depend on long-term care (LTC), accounting for 12% of people aged ≥ 60 years and almost 27% of those aged ≥ 80. It is crucial to develop sustainable strategies for providing LTC in the area, including institutional care. This special report aims to characterize institutional LTC in four countries (Brazil, Chile, Costa Rica and Mexico), using available information systems, and to identify the strategies adopted to support institutional care in these countries. This narrative review used nationwide, open-access, public data sources to gather demographic estimates and information about institutional LTC coverage and the availability of open-access data for the proportion of people with LTC needs, the number of LTC facilities and the number of residents living in them. These countries have a larger share of older people than the average in Latin America but fewer LTC facilities than required by the demand. National surveys lack standardization in defining disability, LTC and dependency on care. Information about institutional care is mainly fragmented and does not regularly include LTC facilities, their residents and workers. Data are crucial to inform evidence-based decisions to favor prioritization and to support advances in promoting policies around institutional LTC in Latin America. Although information about institutional care in the region is fragmented and insufficient, this paper profiles the four selected countries. It highlights the need for a better structure for datadriven LTC information systems. The lack of information emphasizes the urgency of the need to focus on and encourage research into this topic.

Abstract: Although food insecurity presents a decreasing trend worldwide, some regions recently observed an increase in hunger levels. Such was the case in Brazil between 2014 and 2018, during and after the great Brazilian recession, and between 2020 and 2021, during the COVID-19 pandemic. This paper describes the evolution of food insecurity in Brazil between 2004 and 2022 using Brazilian National Household Sample Survey (PNAD), Brazilian Household Budget Survey (POF) and Continuous PNAD. Households were classified in 20 types of arrangements, and the most vulnerable living arrangements between 2004 and 2018 were identified by multinomial logistic models. Overall, households headed by women (single blacks, whites or in couples) with or without children were the most prone to food insecurity. As for the evolution of food insecurity in Brazil between 2018 and 2022, logistic models were applied to estimate moderate and severe food insecurity levels among the 20 household types. Additionally, effects of the emergency aid and idiosyncrasies of the COVID-19 pandemic were estimated.

Pregnant people with coronavirus disease 2019 (COVID-19) have a higher risk of adverse maternal and fetal outcomes compared with pregnant people without COVID-19. In 2021, large increases in maternal mortality were reported in Jamaica, almost half of which were attributable to COVID-19. COVID-19 vaccination has been shown to reduce these risks, but low- and middle-income countries lack free, publicly available data, known as open data, on COVID-19 vaccine uptake for their pregnant populations. The objectives of this paper were to: review how high-income countries use open data to detect trends in COVID-19 vaccine uptake among pregnant people and develop vaccination distribution strategies; outline barriers to making open data available for maternal COVID-19 vaccination in the Caribbean; and propose a multipronged strategy that would increase the availability of open data on maternal COVID-19 vaccination in the Caribbean. A multipronged strategy to fill the data void would involve: (i) utilizing existing Caribbean maternal immunization data collection entities; (ii) adapting digital software tools to establish maternal electronic immunization registries; and (iii) collaborating with local partners skilled in data analytics. Making open data available for COVID-19 vaccine uptake among pregnant people in the Caribbean could offer substantial benefits, including the development of measurable maternal COVID-19 vaccination goals and the facilitation of vaccine decision-making discussions between providers and pregnant people.

Abstract Articular cartilage injuries are common and lead to early joint deterioration and osteoarthritis. Articular cartilage repair techniques aim at forming a cartilaginous neo-tissue to support the articular load and prevent progressive degeneration. Several techniques are available for this purpose, such as microfracture and chondrocyte transplantation. However, the procedural outcome is often fibrocartilage, which does not have the same mechanical resistance as cartilaginous tissue. Procedures with autologous osteochondral graft have a morbidity risk, and tissue availability limits their use. As such, larger lesions undergo osteochondral transplantation using fresh or frozen grafts. New techniques using minced or particulate cartilage fragments or mesenchymal stem cells are promising. This paper aims to update the procedures for treating chondral lesions of the knee.

Abstract Lumbar facet syndrome stands out as a significant cause for the increasing prevalence of back pain complaints. Alternatives such as radiofrequency (RF) ablation may be a therapeutic option to relieve the chronic pain associated with this condition. It is critical to analyze the effectiveness of lumbar facet syndrome treatment using the traditional RF ablation technique and the relief generated by it in chronic low back pain (CLBP). This study is a systematic review using the following inclusion criteria: title, observational studies, clinical trials, controlled clinical trials, clinical studies, and publications over the last 17 years (from 2005 to 2022). The exclusion criteria included papers addressing other themes and review articles. The databases used for data collection included the Medical Literature Analysis and Retrieval System Online (Medline), PubMed, Scientific Electronic Library Online (SciELO), Lilacs, and Biblioteca Virtual em Saúde (Virtual Health Library in Portuguese). The query used the following terms: facet, pain, lumbar, and radiofrequency. The application of these filters yielded 142 studies, and 12 were included in this review. Most studies indicated that the traditional RF ablation technique was beneficial in relieving CLBP refractory to conservative treatment.

In the English-speaking Caribbean, an estimated 46% of men and 61% of women are currently overweight or obese, and 8% of children younger than 5 years are also overweight. To combat this worsening epidemic, driven by unhealthy dietary patterns, the Heads of Government of the Caribbean Community (CARICOM) issued the 2007 Port-of-Spain Declaration, which included mandates on the provision of healthy school meals, promotion of healthy dietary patterns, and reintroduction of physical education in schools. These mandates are aligned with evidence-based approaches used in childhood obesity prevention programs. School-based interventions, including curriculum revisions, are part of a multipronged approach to improve nutrition in children and are designed to complement and reinforce other interventions in schools. However, formal evaluation of the Port-of-Spain Declaration showed that most CARICOM member countries had difficulty implementing the mandates related to schools and diet. The Improving Household Nutrition Security and Public Health in the CARICOM project, in collaboration with regional institutions, the CARICOM Secretariat, and the Caribbean Examinations Council, sought to enhance nutrition education through revision of region-wide primary and secondary school curricula to increase the focus on prevention of noncommunicable diseases. This paper describes the process of revising the Caribbean Examinations Council’s Human and Social Biology syllabus for secondary schools and the CARICOM Health and Family Life Education Regional Curriculum Framework for primary schools, which was achieved through multisectoral collaboration. We used the Framework for Reporting Adaptations and Modifications-Enhanced model to describe the process through which the modifications were made.

Objectives. The aim of this initiative was to assess whether a novel training program – Understanding Stigma and Strengthening Cognitive Behavioral Interpersonal Skills – could improve primary health care providers’ confidence in the quality of mental health care they provide in the Caribbean setting by using the Plan-Do-Study-Act rapid cycle for learning improvement. Methods. We conducted a prospective observational study of the impact of this training program. The training was refined during three cycles: first, the relevance of the program for practice improvement in the Caribbean was assessed. Second, pilot training of 15 local providers was conducted to adapt the program to the culture and context. Third, the course was launched in fall 2021 with 96 primary care providers. Pre- and post-program outcomes were assessed by surveys, including providers’ confidence in the quality of the mental health care they provided, changes in stigma among the providers and their use of and comfort with the tools. This paper describes an evaluation of the results of cycle 3, the official launch. Results. A total of 81 participants completed the program. The program improved primary care providers’ confidence in the quality of mental health care that they provided to people with lived experience of mental health disorders, and it reduced providers’ stigmatization of people with mental health disorders. Conclusions. The program’s quality improvement model achieved its goals in enhancing health care providers’ confidence in the quality of the mental health care they provided in the Caribbean context; the program provides effective tools to support the work and it helped to empower and engage clients.

Objective. To map geographic clusters of rare disorders and congenital anomalies reported in South America. Methods. Qualitative systematic review conducted in Medline/PubMed, Lilacs, and Scielo electronic databases to identify studies meeting eligibility criteria. The strategy resulted in 1 672 unique articles, from which 164 were selected for full reading by a pair of reviewers. Results. Fifty-five articles reported at least one cluster of genetic disorders or congenital anomalies in South American territory. From these papers, 122 clusters were identified, of which half (61) were related to autosomal recessive disorders. Sixty-five (53.3%) of the clusters were located in Brazil. Conclusions. The results of the review reinforce that rare diseases and congenital anomalies can occur in a non-random way in space, which is discussed in the perspective of the complex history of formation, social organization, and genetic structure of the South American population. Mapping clusters in population medical genetics can be an important public health tool, given that such places concentrate cases of rare diseases that frequently require multiprofessional, specialized care. Therefore, these results can support important agendas in public health related to rare diseases and congenital anomalies, such as health promotion and surveillance.

Abstract Surgical correction is an effective treatment for adolescent idiopathic scoliosis (AIS) with deformities over 45°. In the Brazilian Unified Health System (SUS, Sistema Único de Saúde), if the surgical procedure is indicated, the patients are placed on a waiting list and wait until the treatment can be performed. An extended waiting period can be harmful due to worsening symptoms and increased treatment costs. Additionally, it has negative effects on the mental health and quality of life of these patients. This paper is a systematic review protocol to answer the following question: “What is the impact of the delayed surgical correction of AIS considering costs and quality of life?” Collecting health status information is the first step to improve high complex public health actions. Future publications from this protocol may serve as a subsidy to point out potential priority criteria to enhance the global health of AIS patients and the management of Brazilian public health financial resources.

Vinpocetine is a multipurpose medicine with several pharmacological targets. It is known for its neuroprotective properties. Its mechanism of action in brain pathology is due to a selective inhibitor of phosphodiesterase type 1, inhibition of voltage-gated sodium channels, and a decrease in calcium influx into neurons. Vinpocetine combines high clinical efficacy and safety of use. Therefore it successfully competes in the pharmaceutical market with an ever-increasing number of new medicines. The purpose of the work was to experimentally study the effects of vinpocetine in cats at a dose of 5 mg/kg on the indicators of the acid-base state (ABS) of arterial and venous blood in the simulation of total cerebral ischemia. The effect of vinpocetine on acid-base status indicators should be considered as the most important aspect of its action, contributing to the neuroprotective effect. The paper discusses possible molecular targets and mechanisms for implementing the protective effect of vinpocetine in ischemia, considering its favorable effect on ABS. These new findings may point to increased use of vinpocetine in human cardiovascular diseases accompanied by ABS deterioration.

Systemic Lupus Erythematosus (SLE) is an autoimmune disease featuring pleomorphic clinical manifestations whose etiology basically depends on the abnormalities in the functioning of the immunological system coupled to environmental and hormonal factors characterized by the production of antibodies and immunocomplex deposition. People suffering from SLE are prone to have cardiovascular diseases when compared to people in general. Further, several studies suggest that intestine microbiota may have an important role in the progress of such diseases. Microbiota is generally associated with the functioning of the immunological system whose response gets worse due to intestine dysbiosis which may result in a chronic inflammatory stage and, consequently, great cardiovascular risk. Several authors discuss the mechanism by which response to microbiota takes place and the possibility of dietetic interventions. These may comprise probiotics to modify intestine dysbiosis to thwart the disease´s progress and frequent cardiovascular disorders in the population. Current paper revises the available literature on the relationship between intestine microbiota and its nutritional characteristics in SLE patients and the development of cardiovascular diseases.

Abstract Introduction the use of light emitting diodes (LED) in domestic and public vias have increased in the last 20 years. In addition, the LED light has been used as a light source for medical applications. Objective since humans are increasingly exposed to LEDs, there is an urgency to investigate the possible biological effects on tissues caused by this exposure. So, researchers have been focused their investigations in the application of this light in the health field. Material and method in this review, a search in important databases was performed on the biological effects caused after application of different LED light protocols in in vitro and in vivo studies. Result although most published papers have shown positive results, some of them reported negative biological effects of light LEDs technology on humans’ cells/tissues. Conclusion therefore, the comprehension of the biological effects caused by light LEDs will provide a better assessment of the risks involved using this technology.

Acute porphyrias are a group of rare genetic metabolic disorders, caused by a defect in one of the enzymes involved in the heme biosynthesis, which results in an abnormally high accumulation of toxic intermediates. Acute porphyrias are characterized by potentially life-threatening attacks and, for some patients, by chronic manifestations that negatively impact daily functioning and quality of life. Clinical manifestations include a nonspecific set of gastrointestinal, neuropsychiatric, and/or cutaneous symptoms. Effective diagnostic methods are widely available, but due to their clinical heterogeneity and non-specificity, many years often elapse from symptom onset to diagnosis of acute porphyrias, delaying the treatment and increasing morbidity. Therefore, increased awareness of acute porphyrias among healthcare professionals is paramount to reducing disease burden. Treatment of acute porphyrias is centered on eliminating the potential precipitants, symptomatic treatment, and suppressing the hepatic heme pathway, through the administration of hemin or givosiran. Moreover, properly monitoring patients with acute porphyrias and their relatives is fundamental to preventing acute attacks, hospitalization, and long-term complications. Considering this, a multidisciplinary panel elaborated a consensus paper, aiming to provide guidance for an efficient and timely diagnosis of acute porphyrias, and evidence-based recommendations for treating and monitoring patients and their families in Portugal. To this end, all authors exhaustively reviewed and discussed the current scientific evidence on acute porphyrias available in the literature, between November 2022 and May 2023.

Abstract: Malnutrition in all its forms has risen on global agendas due to the recognition of its magnitude and consequences for a wide range of human, social, and economic outcomes. Implementing strategies and programs with the needed scale and quality is a major challenge. The Brazilian National Survey on Child Nutrition (ENANI-2019) pointed out several advances but numerous challenges. In this paper, we reflect on the implementation progress of breastfeeding, complementary feeding and young children malnutrition strategies and programs in Brazil and how existing challenges can be overcome through the lens of implementation science. First, we present a brief history of such programs. Second, we selected two breastfeeding initiatives to illustrate and reflect on common implementation challenges. In these case studies, we used the RE-AIM (Reach, Effectiveness, Adoption, Implementation, Maintenance) framework to analyze the implementation and scaling up barriers and facilitators. We found common barriers related to unclear goals about the reach of programs, challenges in assessing effectiveness and fidelity/quality during the real-world implementation, discontinuation or lack of funding, and lack of monitoring and evaluation impacting the sustainability of programs. We also discuss the use of implementation science to achieve adequate nutrition by 2030 and present critical elements for successful scale implementation of nutrition programs based on global evidence. Despite the investment to implement different actions aimed at facing infant feeding and malnutrition, high-quality implementation research must become a priority to catalyze progress in Brazil.

The significant challenge faced by medical schools is the effortful process of writing a high quantity of high-quality case-based multiple-choice questions (MCQs) to assess the higher-order skills of medical students. The demand for a high volume of MCQs in education has led to the development of Automatic Item Generation (AIG), specifically template-based AIG, which involves creating cognitive and item models by subject matter experts to generate hundreds of MCQs at once using software. It demonstrated significant success in various languages and even being incorporated into national medical licensure exams. However, this method still heavily depends on the efforts of subject matter experts. This paper introduces a detailed ChatGPT prompt for quickly generating case-based MCQs and provides important research questions for future exploration into ChatGPT's potential in generating items, signaling the beginning of the artificial intelligence era in medical education, encouraging health professions education researchers to delve deeper into its potential.

Objective. Systematize the experience and identify challenges and lessons learned in the implementation of an initiative for integrated serosurveillance of communicable diseases using a multiplex bead assay in countries of the Americas. Methods. Documents produced in the initiative were compiled and reviewed. These included concept notes, internal working papers, regional meetings reports, and survey protocols from the three participating countries (Mexico, Paraguay, and Brazil) and two additional countries (Guyana and Guatemala) where serology for several communicable diseases was included in neglected tropical diseases surveys. Information was extracted and summarized to describe the experience and the most relevant challenges and lessons learned. Results. Implementing integrated serosurveys requires interprogrammatic and interdisciplinary work teams for the design of survey protocols to respond to key programmatic questions aligned to the needs of the countries. Valid laboratory results are critical and rely on the standardized installment and roll-out of laboratory techniques. Field teams require adequate training and supervision to properly implement survey procedures. The analysis and interpretation of serosurveys results should be antigen-specific, contextualizing the responses for each disease, and triangulated with programmatic and epidemiological data for making decisions tailored to specific population socioeconomic and ecologic contexts. Conclusions. Integrated serosurveillance as a complementary tool for functional epidemiological surveillance systems is feasible to use and key components should be considered: political engagement, technical engagement, and integrated planning. Aspects such as designing the protocol, selecting target populations and diseases, laboratory capacities, anticipating the capacities to analyze and interpret complex data, and how to use it are key.

The tobacco endgame is rapidly moving from aspirational and theoretical toward a concrete and achievable goal and, in some cases, enacted policy. Endgame policies differ from traditional tobacco control measures by explicitly aiming to permanently end, rather than simply minimize, tobacco use. The purpose of this paper is to outline recent progress made in the tobacco endgame, its relationship to existing tobacco control policies, the challenges and how endgame planning can be adapted to different tobacco control contexts. Examples of implemented policies in three cities in the United States and national policies in the Netherlands and New Zealand are outlined, as well as recent endgame planning developments in Europe. Justifications for integrating endgame targets into tobacco control policy and the need to set concrete time frames are discussed, including planning for ending the sale of tobacco products. Tobacco endgame planning must consider the jurisdiction-specific tobacco control context, including the current prevalence of tobacco use, existing policies, implementation of the World Health Organization’s Framework Convention on Tobacco Control, and public support. However, the current tobacco control context should not determine whether endgame planning should happen, but rather how and when different endgame approaches can occur. Potential challenges include legal challenges, the contested role of e-cigarettes and the tobacco industry’s attempt to co-opt the rhetoric of smoke-free policies. While acknowledging the different views regarding e-cigarettes and other products, we argue for a contractionary approach to the tobacco product market. The tobacco control community should capitalize on the growing theoretical and empirical evidence, political will and public support for the tobacco endgame, and set concrete goals for finally ending the tobacco epidemic.

HEARTS in the Americas is the Pan American Health Organization flagship program to accelerate the reduction of the cardiovascular disease (CVD) burden by improving hypertension control and CVD secondary prevention in primary health care. A monitoring and evaluation (M&E) platform is needed for program implementation, benchmarking, and informing policy-makers. This paper describes the conceptual bases of the HEARTS M&E platform including software design principles, contextualization of data collection modules, data structure, reporting, and visualization. The District Health Information Software 2 (DHIS2) web-based platform was chosen to implement aggregate data entry of CVD outcome, process, and structural risk factor indicators. In addition, PowerBI was chosen for data visualization and dashboarding for the analysis of performance and trends above the health care facility level. The development of this new information platform was focused on primary health care facility data entry, timely data reporting, visualizations, and ultimately active use of data to drive decision-making for equitable program implementation and improved quality of care. Additionally, lessons learnt and programmatic considerations were assessed through the experience of the M&E software development. Building political will and support is essential to developing and deploying a flexible platform in multiple countries which is contextually specific to the needs of various stakeholders and levels of the health care system. The HEARTS M&E platform supports program implementation and reveals structural and managerial limitations and care gaps. The HEARTS M&E platform will be central to monitoring and driving further population-level improvements in CVD and other noncommunicable disease-related health.

Objective. To summarize the results of research conducted in Costa Rica in which mathematical and statistical methods were implemented to study the transmission dynamics of mosquito-borne diseases. Methods. Three articles with mathematical and statistical analysis on vector-borne diseases in Costa Rica were selected and reviewed. These papers show the value and relevance of using different quantitative methods to understand disease dynamics and support decision-making. Results. The results of these investigations: 1) show the impact on dengue case reports when a second pathogen emerges, such as chikungunya; 2) recover key parameters in Zika dynamics using Bayesian inference; and 3) show the use of machine learning algorithms and climatic variables to forecast the dengue relative risk in five different locations. Conclusions. Mathematical and statistical modeling enables the description of mosquito-borne disease transmission dynamics, providing quantitative information to support prevention/control methods and resource allocation planning.

Introduction: Screening instruments specifically developed to identify genetic testing applicants who may need professional psychosocial support are much needed. However, there are no screening instruments validated for the Portuguese language. This paper presents the translation, adaptation, and validation process of the Genetic Psychosocial Risk Instrument in a sample of 207 Portuguese applicants to genetic testing in the context of inherited cancer risk. Material and Methods: Participants were mainly female (84.06%), with a mean age of 40.08 (SD = 12.89) and were recruited from the Portuguese Oncology Institute of Porto. Confirmatory factor analysis was conducted to confirm the Genetic Psychosocial Risk Instrument factorial structure. Convergent validity was assessed with the Impact of Events Scale, the Clinical Outcome Routine Evaluation – Outcome Measure, and the Hospital Anxiety and Depression Scale. Results: A model composed by the factors ‘Internal Impact of Genetic Testing’, ‘External Impact of Genetic Testing’ and ‘History of Mental Health Concerns’ was confirmed. These factors showed good internal consistency, convergent and discriminant validity. The factor ‘Personal Loss to Cancer’ proposed in the Canadian and French versions did not converge. We propose excluding this factor from the European Portuguese version of the scale. Conclusion: The European Portuguese version of the Genetic Psychosocial Risk Instrument is a reliable and valid instrument, although more research is needed to effectively use it in routine clinical oncogenetic departments.

Objetivo. Describir la calidad de vida del personal de salud, el ambiente laboral y la interacción entre el empleado y ambiente laboral durante la pandemia. Métodos. Se realizó una revisión exploratoria. Se utilizaron las bases de datos electrónicas PubMed, Cumulative Index of Nursing and Allied Literature Complete, Google Scholar y los repositorios de la Organización Mundial de la Salud y los Centro para el Control y Prevención de Enfermedades. Se incluyeron estudios primarios, secundarios y literatura gris publicados entre diciembre de 2019 y marzo de 2021 en idioma español, inglés y portugués. La calidad metodológica fue evaluada por la lista de verificación de Autoridad, exactitud, cobertura, objetividad, fecha e importancia (AACODS, por su sigla en inglés) A measurement tool to assess systematic reviews (AMSTAR) y Critical Appraisal Checklist for Text and Opinion Papers. Se efectuó un análisis temático basado en el modelo de calidad de vida y bienestar. Resultados. De un total de 208 artículos, se incluyeron 11. La calidad de vida del personal de salud durante pandemia por COVID-19 estuvo influenciada por las características del personal de salud, el ambiente laboral y la interacción entre el empleado y el ambiente laboral; se observaron falencias relacionadas con factores psicosociales y ocupacionales. Discusión. La calidad de vida del personal de salud estuvo caracterizada por estigmatización, estrés, ansiedad y fatiga. La gestión organizacional y la aplicación de intervenciones psicológicas evidencian un efecto en la interacción entre el empleado y el ambiente laboral, e influyen en su calidad de vida.

Objective. To describe the editorial processing time of published COVID-19 research articles and compare this with a similar topic, human influenza, and analyze the number of publications, withdrawals, and retractions. Methods. A descriptive-analytical study using PubMed on research articles with the MeSH terms human influenza and COVID-19. Time to acceptance (from submission to acceptance) and time to publication (from acceptance to publication) were compared. Retractions and withdrawals were reviewed both qualitatively and quantitatively. Results. There were 31 319 research articles on COVID-19 and 4 287 on human influenza published during 2020. The median time to acceptance for COVID-19 was lower than that for human influenza (8 vs. 92 days). The median time to publication for COVID-19 articles was shorter than those on human influenza (12 vs. 16 days); 47.0% of COVID-19 research articles were accepted within the first week of submission, and 19.5% within one day. There were 82 retractions and withdrawals for COVID-19 articles, 1 for human influenza, and 5 for articles that contain both terms; these were mainly related to ethical misconduct, and 27 (31.0%) were published by the same group of authors in one highest-quartile journal. Conclusions. The conundrum between fast publishing and adequate standards is shown in this analysis of COVID-19 research articles. The speed of acceptance for COVID-19 manuscripts was 11.5 times faster than for human influenza. The high number of acceptances within a day or week of submission and the number of retractions and withdrawals of COVID-19 papers might be a warning sign about the possible lack of a quality control process in scientific publishing and the peer review process.
