Treatment of the cervical cancer patients, especially young women, is a very important clinical problem. This is related to the standard treatment methods, used in this malignancy and the treatment complications. In most patients, the oncological treatment, be it surgery or radiotherapy results in gonadal dysfunction with unavoidable loss of the ability to procreate. As cervical cancer affects mostly young women, fertility preserving treatment remains a considerable challenge. Due to the fact that the recent decade has brought significant progress in the diagnosis of prognostically important features characterizing the tumor biology their precise determination enables specialists to detect a subpopulation of patients with malignancies, who do not require the total resection of the uterus and ovaries. Currently in such cases, fertility-sparing surgery is considered in every young woman with early cervical cancer who wishes to preserve the reproductive function. The aim of the paper is to revive the current opinions concerning fertility sparing treatment options in gynecological malignancies.

Recent epidemiologic studies indicate that use of combined oral contraception is associated with a increase in the incidence of cardiovascular disease (venous thromboembolism, pulmonary embolism, myocardial infarction and stroke). The risk of cardiovascular disease is strongly related to estrogen dose, progestogen type and other factors for example thrombogenic mutations and cigarette smoking among female over age 35. The progestogen only contraception is safe alternative to combined hormonal contraception. Progestogen only pill (POP) has different levels of action (local and/or central) which may vary from one drug to another. As for the cardiovascular disease risk, progestogens are not considered to be risk factors. Desogestrel containing POP is advised in the following cases: bad tolerance of exogenous oestrogens; in order to counteract an endogenous hyperoestrogenosis; medical, metabolic or cardiovascular contraindications to estroprogestogen contraception. Lastly, POP should be used as a prime contraception in some particular situations (breast feeding, endometriosis, adenomyosis, cigarette smoking, contraception for older women). These recommendations present the actual system of care in that population of women in Poland.

Anorexia nervosa is an eating disorder characterized by low body weight and body image distortion with obsessive fear of gaining weight. It is a complex condition involving psychological, neuroendocrine, hormonal and metabolic components. The following is a case report of a young patient diagnosed as anorexia nervosa and primary amenorrhea, without the development of secondary female characteristics. Hormonal examination confirmed hypothalamic origin of the described disorders. There was an important impairment in the mood sphere (Beck's Depression Inventory and Hamilton Depression Rating Scale) of this young woman. Densitometry of lumbar spine revealed significant bone loss (diagnozed as osteoporosis) in this patient. All of the abovementioned examinations were repeated after one and a half years of hormonal treatment (estrogen-progestins). A return of normal menstrual cycle (with ovulation function) and normal hormonal profile have been observed. After one and a half years of treatment the patient had regular feeding patterns and none of the anorexia nervosa symptons have been found. A significant improvement in Beck's Depression Inventory and Hamilton Depression Rating Scale was found after one and a half years of hormonal treatment. The patient was also characterized by important increase of bone mass density--osteopenia was described by the densitometry of the lumbar spine.

One of the chief findings in polycystic ovaries syndrome may be the defective luteal phase of the menstrual cycle. There is a growing list of publications indicating a defective progesterone production by granulosa cells of ovarian follicles. In consequence, women with polycystic ovaries often require both the luteal phase and early pregnancy supplementation. From different routes of progesterone administration during the menstrual cycle, as the most clinically reliable in the early pregnancy either intramuscular or intravaginal route has been recommended.
