Purpose of the Work: To assess bone mineral density (BMD) in girls with menstruation disorders in the first years after menarche and to demonstrate that BMD value may be one of the indicators confirming an early diagnosis of hypothalamic hypogonadism or androgenisation complex., Material and Methods: The research included 155 girls at the ages of 16-17, observed due to rare menstruation or secondary amenorrhea. In case of 71 girls functional, hypothalamic disorders were found (Group A), in 35 girls increasing androgenisation features were diagnosed additionally (Group B), and 49 healthy girls represented Group C--control group. BMI was determined, as well as FSH, LH, T, E2, PRL concentrations and the BMD test of lumbar spine section was made in all of the cases. RESEARCH RESULTS: The average values of BMI and BMD in Group A were the lowest compared to Group B (p<0.001) and Group C (p<0.001). E2 concentrations in Group A was the lowest and in Group B--the highest (p<0.001). T concentrations in Group B girls was the highest and SHBG--the lowest compared to Groups A and C (p<0.001; p<0.01). FSH concentrations in Group A were the lowest compared to Groups B and C (p<0.01), while LH in Group B was the highest (p<0.001; p<0.01). Basic PRL concentrations were not essentially different. In groups (A + B + C) together, a statistically significant correlation between BMD and BMI values was found (r=0.67; p<0.001), E2 (r=0.38; p<0.01) and T (r=0.56; p<0.001)., Conclusions: Low bone mineral density value compared to the age value may signify long-term hypoestrogenism, and high value - a complex of metabolic disorders with high concentration of androgens. Therefore, bone mineral density may be a reliable and highly sensitive metabolic and hormonal indicator of menstruation disorders of hypothalamic, functional type or androgenisation complex.

Objectives: Menstrual disorders are very common during the early postmenarcheal years. This study was undertaken to determine the causes of menstrual disorders in adolescents., Materials and Methods: In the research there were included 117 girls 14-18 years old without evidence of androgen excess (hirsutism, acne): the study group consisted of 87 adolescents with menstrual disorders and the control group of 30 girls with regular menstrual cycles. All the patients underwent gynecological and ultrasound examination and an analysis of the hormonal status., Results: The data from our investigation show that 43.7% of adolescents with menstrual dysfunction have eating disorders, 16.1%--immaturity of hypothalamic-pituitary-ovarian axis, 13.8%--polycystic ovary syndrome, 9.2%--hyperprolactinemia, 4.6%--delayed puberty, 3.4%--genetic defect and 9.2% other reasons., Conclusions: A thorough history and physical examination focusing on the basic understanding of pubertal and menstrual physiology and pathology allows to obtain an accurate diagnosis. The prognosis for later fertility depends on the preventive measures token in adolescence.

A study presents two cases of menstrual abnormalities in women with primary biliary cirrhosis and chronic active hepatitis treated with liver transplantation. The gonadotropin, sex hormones levels and biochemical parameters were presented. The menstrual disorders we observed may be a consequence of high concentration of oestrogens in our patients.

In 173 women with oligomenorrhea or amenorrhea, serum level of prolactin (PRL) was determined by radioimmunoassay method. In 22 cases (12.7%) the concentration of PRL was above 15 ng/ml. From the above women in 9 cases (5.2%) of all examined patients pituitary microadenoma was diagnosed. In 1 patient with pituitary microadenoma, coexisting partially empty sella was diagnosed. The mean basal PRL concentration in women with menstruation disturbances without hyperprolactinemia (group I) was found to be 7.1 +/- 3.1 ng/ml while the women with hyperprolactinemia without pituitary microadenoma (group II) was 32.9 +/- 5.8 ng/ml. In women with diagnosed pituitary microadenoma (group III) PRL concentration was 131.8 +/- 47.7 ng/ml. The group II and III demonstrated statistically significant increase of serum prolactin level in comparison to group I. Moreover in comparison with group I, statistically significant reduction of serum level of FSH, LH and estradiol 17 beta was confirmed in group with pituitary microadenoma. In group II significant decrease of serum concentration of LH and estradiol 17 beta was stated as well as lack of significant difference in serum level of FSH, in comparison to group I.
