3. Kwestionariusz Postaw Personelu wobec Przymusu (SACS) -- polska adaptacja.

- Abstract
Aim. The aim of the study was to prepare the Polish adaptation of the SACS. The scale is a self-assessment tool designed to measure staff attitudes towards direct coercion of psychiatric wards patients. Methods. The sample consisted of 120 adults, staff from 7 psychiatric wards. The SACS is a tool created in Norway by Tonje Husum, comprising of 15 items describing psychiatric healthcare professionals attitudes towards direct coercion. The validation procedure incorporated three basic methods to be applied in the reliability analysis -- the comparison of double tests with the same method, the analysis of statistical properties of test items as well as analysis of the relation of test items and subscales with the general test result. Results. After a 'think aloud' type pilot study and a language validation, the internal consistency was assessed. The Cronbach's alpha ranged from 0.57 to 0.81 in the subscales, and it was 0.82 for the total score. The best solution obtained in exploratory factor analysis was a three-factor model, almost identical to the original one, confirming the division into three subscales: coercion as offending (critical attitude), as care and security (pragmatic attitude) and as treatment (positive attitude). Conclusions. The psychometric characteristics of the Polish adaptation of the SACS are similar to those reported in the original version. The results allow to recommend the method for scientific research. However, further analyses are necessary to assess validity and discriminative power in larger settings. [ABSTRACT FROM AUTHOR]

Aim. The study aimed to analyse the bipolarity features between patients with treatmentresistant depression and patients with remission of a depressive episode. Methods. The population studied included patients aged 18-65 who had a diagnosis of first or recurrent major depressive disorder. Each psychiatrist included 5 consecutive patients, from all the patients he is treating, where the depressive episode was assessed as treatment-resistant (TR) and 5 consecutive patients with a depressive episode in remission (RE). One thousand and fifty-one patients entered the study: 570 in the treatment-resistant depression group and 481 in the remission group. All patients were assessed using the Mood Disorder Questionnaire (MDQ) and Hypomania Checklist (HCL-32). The assessment of current depressive symptoms was done by means of a 17-item Hamilton Depression Rating Scale. Results. Bipolar disorder (BD) is one of the most difficult to diagnose among all psychiatric disorders. BD is often misdiagnosed as recurrent depressive disorder, schizophrenia, anxiety or personality disorders. In 40% of patients BD is not detected at the first examination and in 2/3 of cases the proper diagnosis is made after about 10 years. The consequence of this condition is inadequate treatment leading to the worsening of the course of BD or increased risk of substance misuse and suicidal behavior. Those data reveal that the better detection of bipolar spectrum is strongly needed. It may be achieved by the application of the Mood Disorder Questionnaire (MDQ) in clinical practice. MDQ is a self rating questionnaire created by Hirschfeld et al. for screening of bipolar spectrum. According to previous data the usefulness of MDQ in populations of psychiatric patients (especially those with affective disorders) is unquestionable high. The significant limitation of MDQ is a relatively low sensitivity for BD type II. That’s why the concomitant use of MDQ (as an excellent tool for detection of BD type I) and the other instrument characterized by the higher sensitivity for hypomania and BD type II (e.g. The Hypomania Symptom Checklist by Angst et al.) seems to be the best and recommended solution. This article describes the properties and structure of MDQ. The capabilities, advantages and limitations of MDQ were also presented. The authors discussed the results of validation studies of different language versions of MDQ and summarized – DEP-BI and TRES-DEP – Polish studies using MDQ.

6. Znaczenie zaburzeń osobowości wobec przerywania leczenia na przykładzie wybranych oddziałów psychiatrycznych.

- Abstract
The World Health Organization's estimations indicate that about 50% of patients in well-developed countries may not adhere to long-term therapies. In the field of psychiatry, drop-outs from psychiatric treatment are particularly important. Personality disorders are a significant part of this sphere. Aim. The aim of this research was to empirically verify the hypothesis regarding the relation between comorbid personality disorders and drop-outs from treatment among patients of psychiatric wards. Method. This study was a prospective cohort study. 110 patients, hospitalized in 3 different psychiatric wards, were included. Personality disorders were assessed with the Structured Clinical Interview For DSM-IV Personality Disorders (SCID-II). The research was financed by the Polish National Science Center (DEC-2011/01/N/NZ5/05364). Results. The response rate was 89.1%. 72.56% of patients suffered from personality disorders (SCID-II) (among them the most prevalent were: personality disorder -- not otherwise specified -- 40.7% and borderline personality disorder -- 12.38%; 22.95% of patients dropped out from treatment). However, occurrence of personality disorders was not relevant for those drop-outs. On the other hand, relationships at the level of certain criteria of borderline personality disorders and passive-aggressive personality have been revealed. These relationships became stronger when considered from the perspective of differences in the organization of treatment at individual wards. Conclusions. Some personality disorders may play an important role in drop-outs from psychiatric treatment. Presented results require further research. [ABSTRACT FROM AUTHOR]

7. Rozpowszechnienie i czynniki ryzyka epizodów depresji wśród wrocławskiej młodzieży akademickiej - wyniki badania epidemiologicznego.

- Abstract
Aim. The aim of this study was to identify risk factors and estimate prevalence of depressive episodes among Wroclaw's universities students. Method. Polish adaptation of the Composite International Diagnostic Interview (CIDI) was implemented to gather epidemiological data from 370 students of public universities in Wroclaw. Proportional stratified sampling was performed to obtain distinct, independent strata representing sex, year of study and educational profiles. Randomisation was ensured by recruitment procedures. Results. Prevalence of depressive episodes among students in Wroclaw is high - 14.7% throughout life, 9.8% within 12 months prior to the interview. High prevalence of severe and moderate depressive episodes seems to be worrisome (respectively 5.1% and 6.6% throughout life, 3.8% and 3.9% within 12 months prior to the study). Year of study, profile and lack of partner relationship remain risk factor for depression. Conclusions. High prevalence of depressive episodes indicates the need for prevention and therapy based on epidemiological data and tailored to the students' needs. - Abstract
Introduction: Schizophrenia is a chronic and one of the most severe mental disorders. From many years studies regarding mortality among people suffering from schizophrenia are being conducted as a way of controlling indirectly the effectiveness of medical care and therapy. Aim: The aim of this paper was to determine the course of studies of mortality in schizophrenia and to analyze possibilities of using this kind of studies to evaluate changes in mental care system in Poland. Material: This paper is a review of European literature concerning studies of mortality in schizophrenia. Most of the analyzed publications were created in Northern and Western Europe. There are no international publications originating from Southern and Eastern Europe. Directions of current studies include changes of causes of death in persons with a diagnosis of schizophrenia over years, coexistence of somatic diseases, medicine's impact on mortality in schizophrenia and worse access to medical care in comparison to mentally healthy people. Conclusions: Mortality in schizophrenia is a useful factor in clinical studies enabling evaluation health effects of changes in mental health care system, what allows creation of system based on scientific evidence. 9. Rozpowszechnienie wybranych zaburzeń psychicznych w populacji dorosłych Polaków z odniesieniem do pici i struktury wieku -- badanie EZOP Polska.

- Abstract
Aim. The article presents lifetime (LT) prevalence of common mental disorders (CMD) in accordance with the DSMIV classification, based on assessment of representative population sample of 10.081 Poles aged 18-64. Methods. Computer based WHO C1DI3.0 was adapted for the Polish population according to WMH protocol. The survey was performed by certified and supervised interviewers. Results. Out of the 18 CMDs analyzed the most common was alcohol abuse, significantly more often in males (18.6%) than in women (3.3%), (p<0.01). The second most common disorder was panic attacks, also more frequent in women (8.5%) than in men (3.9%), (p<0.01). Similarly, depression occurred in women (4.0%) two times more often than in males (1.9%), (p<0.01). GAD, agoraphobia, panic disorder, specific phobia (p<0.01), and dysthymia(p<0.05) were also more prevalent in women. On the other hand, alcohol abuse, alcohol and drug dependence (p<0.01), and hypomania (p<0.05) were more common in males. For most analyzed disorders significantly higher prevalence was found in the older age groups. Social phobia, specific phobias, and drug abuse occurred most often in men from the youngest group. No statistically significant differences related to age were found for the prevalence of hypomania both in men and women. Conclusions. Indices of prevalence obtained in the EZOP Poland study differ from the indices of prevalence of mental disorders described earlier in other countries. Lower values were found in Poland for affective disorders and some anxiety disorders. Only alcohol abuse was diagnosed more often than in other studies using similar methods except Ukraine, where this disorder was diagnosed with similar frequency. [ABSTRACT FROM AUTHOR]

- Abstract
Aim. Since the second half of the twentieth century, with the development of structured diagnostic tools, population based studies on epidemiology of mental disorders are carried out. A special role is played by World Mental Health Survey Initiative which brings together a group of countries from different continents in order to carry out research projects according to standard methodology using the Composite International Diagnostic Interview. Polish EZOP study, which was conducted in accordance with the guidelines of WMH joined that group. The project was implemented under the Norwegian Financial Mechanism and the Financial Mechanism of the European Economic Area. Its aim was to estimate the prevalence of mental disorders in the Polish population of adults, assess the distance to mental disorders and to obtain knowledge about the perception of psychiatric disorders and treatment. Methods. The research tool was the Polish version of CIDI-WHO ver.3.0., which was developed for the project. The study was performed in accordance with the guidelines of WMH (cognitive interviews, interviewers training, standardization of field procedures) using electronic version of CIDI questionnaire (CAPI) within the 2-stages procedure in randomly selected representative sample of the Polish population aged 18-64. The quality of the study was systematically controlled and reported by MB SMG/KRC, and completed data (10,081 interviews) were sent to the Department-Centre of Monitoring and Analyses of Population Health NIPH-N1H. - Abstract
Aim. The purposes of our study were to determine the level of total social support, informational support, instrumental support, appraisal support and emotional support received by patients of primary care as well as factors related to this level with reference to health status and sociodemographic variables. Method. The sample for current analysis included 516 patients of primary care clinics in Poland cooperating with medical universities. Questionnaires: STAI (State-Trait Anxiety Inventory), SCL-90 (Symptom Checklist-90), EPQ-R (Eysenck Personality Questionnaire -Revised), GHQ (General Health Questionnaire)and SSS (Social Support Scale)were used in the study. Results from last two questionnaires are presented in the paper. Results. The highest mean levels were obtained for instrumental support, while the lowest levels were observed for emotional support. The highest means were indicated in the GHQ-28 - social dysfunction, the lowest - GHQ-severe depression. Statistically significant relation was found between the level of social support and gender. Less subjectively evaluated total social support as well as instrumental and appraisal support were obtained by women. The highest Spearman score was found in the case of total GHQ-28 score, somatic symptoms, anxiety and insomnia, severe depression and total support. Taking into account the ANOVA findings, it was observed that an increasing GHQ score was associated with intensively increasing emotional support, informative support, appraisal support and the most- instrumental support. Conclusions. The results display the underestimated role of emotional, informational and appraisal support and the overestimated role of instrumental support in primary care. The consequence may be a more frequent using health care accompanied by low level of patients' satisfaction, severity of social dysfunction disorders, particularly in patients with chronic diseases, who constitute an increasingly large group of primary health care. The author reviewed the relatively poor literature on the topic and its key clinical trial reports on perazine, a classical antipsychotic most frequently prescribed in Poland for psychoses, especially for schizophrenia. Based mainly on the Leucht and Hartung's metaanalysis as well as on other authors' trials with broader context of typical and atypical neuroleptics comparisons, it could be concluded that the perazine bearing balanced profile of psychotropic action (antipsychotic, anti-autistic and sedative) has also some atypical features which explain its broad applications in clinical practice. Not without a meaning are also a few dozen-fold lower costs of treatment, which are even more meaningful when costs of necessary laboratory tests required for monitoring atypical antipsychotics use are taken into consideration. Aim. In view of the fact that schizophrenia is a disorder increasing the risk of mortality and development of somatic disturbances, the presented study was undertaken with the aim to analyse to what extent psychiatrists assess the somatic condition of their patients diagnosed with schizophrenia. Method. The participants of a nationwide study ordered by Bristol-Myers Squibb and carried out by a survey agency were 404 physicians. The study was based on a specially devised questionnaire, administered to doctors in June and July 2009. Responses from 184 psychiatry specialists were obtained during the conference "Psychiatry 2009" held in Kraków, whereas the remaining respondents were questioned at their workplaces. The questionnaire consisted of ten questions concerning various aspects of somatic health, control of health parameters and effects of pharmacological treatment on the condition of schizophrenic patients. Results. The majority (73.05%) of the respondents were found to consider the somatic condition of their patients as important or very important. In the opinion of 70.54% of the physicians, metabolic disturbances were the most significant aspect of somatic health. Doctors in private practice paid more attention to sexual and endocrine system dysfunctions than those employed in hospital wards. Most psychiatrists performed physical examinations and checked laboratory parameters "almost on every visit". Conclusions. The questioned physicians appreciate the importance of somatic condition assessment in schizophrenic patients and check the important laboratory parameters regularly. Taking into account the essence of the problem, establishing closer cooperation in this area with first-contact physicians should be considered. In view of definitely positive opinions expressed by psychiatrists, concerning their involvement in the evaluation of the somatic condition of schizophrenic patients, asking the patients themselves whether that meets their needs in this respect would render the obtained results more objective. This article provides a condensed discussion and explanation of the Delphi method. This technique is used for problem solving and decision making. The Delphi Method was originally developed during the cold war by the U.S. Intelligence think tank, the RAND Corporation, Santa Monica, California in the 1950's and 1960's. It was designed for use on complex or ambiguous problems that exceed the capabilities of a single person. Most researchers make distinctions between the conventional Delphi and the policy Delphi. The last decades have seen increasing interest in the use of Delphi in a wide range of health-care applications. The Delphi method has been applied in a wide range of European Commission funded research projects, e.g. in EUNOMIA and DEMoB.inc projects. The Delphi method is characterized by the following features: anonymity of responses from panel experts, asynchronicity, controlled feedback and statistical description of responses. - Abstract
Objectives: Anxiety disorders are significant predictors of suicidality and are proposed to be independent risk factors for suicide attempts. They are common in people with type 2 diabetes (T2DM) and are associated with longer duration of diabetes and poorer treatment outcomes. The aim was to examine associations between anxiety disorders and suicidal thoughts and behaviour in people with T2DM, to establish the prevalence of suicidality among people with T2DM in the selected European countries and to examine whether anxiety disorders were predictive of current outcomes of suicidality in this population using data from the International Prevalence and Treatment of Diabetes and Depression study., Methods: The study sample comprised 1063 adults with T2DM from 6 European countries. The presence of anxiety disorders and suicidality was assessed with the MINI International Neuropsychiatric Interview. The group of participants with current suicidal risk was compared with the group of participants with no suicidal risk., Results: The participants from Germany were more likely to report suicidality than those from other countries, whereas people from Serbia and Ukraine were less likely to report it. Depression and anxiety disorders significantly contributed to the increased presence of suicidality among people with T2DM. Agoraphobia was a significant predictor of suicidality when controlling for depression. The participants with T2DM and comorbid agoraphobia had 4.86 times higher odds to report suicidality than those without agoraphobia., Conclusions: Agoraphobia was a significant predictor of suicidality in people with T2DM.

43. Evaluation of mental health services delivered before and after the introduction of pilot Mental Health Centers in Poland using monitoring indicators.

Objectives: The study assessed the performance of Mental Health Centres compared to previous non-integrated mental health facilities., Methods: The study used National Health Fund (NHF) data of individuals over 18 years treated in 27 centres (January 2017 - February 2020). Performance indicators from 19 months before (N = 124,497) and after the introduction of Mental Health Centres (N = 182,789) were compared for outpatient care, community treatment teams, inpatient wards and day wards., Results: The total number of patients who received mental health care increased, compared to before the establishment of those Centres; whereas the number of hospitalisations decreased by 6% and the number of patient days per person decreased by 9%. Day care saw a 14% increase in admissions following the introduction of Mental Health Centres, with a 5% decrease in patient days per person. The proportion of patients in community care increased by 86%, in outpatient care by 62% and in day care by 14%. The number of first-time patients after the introduction of Mental Health Centres increased and the number of follow-up patients decreased. For all groups of mental disorders, the priority indicator in inpatient care decreased, while increasing in outpatient and community care for most groups of disorders., Conclusions: The results confirmed the effectiveness of the National Mental Health Protection Programme project in relation to the availability and reduction of inpatient treatment through Mental Health Centres. The methodology used in this study can be used for assessing the effectiveness of Mental Health Centre activities in subsequent stages of the pilot.

44. Social support against depression in young adults' group during COVID-19 pandemic.

Objectives: The present study aimed at analyzing the prevalence of depressive symptoms in a group of learning young adults, as well as the social support as a predictor of depression in late adolescence during the COVID-19 pandemic., Methods: An online national survey of young adults attending schools was conducted. The semi-structured questionnaire, including a section on sociodemographic data, stressors, needs and sources of support, as well as brief version of the Kutcher Adolescent Depression Scale, was distributed to young Poles. The final sample consisted of 1,500 students aged 18-23. Multivariable logistic regression assessed the relationships between support needs and sources and depressive symptoms., Results: The support needs that are most commonly reported by young adults are support from loved ones (family, friends) (n = 843; 56.2%) and teachers (e.g., care, appreciation) (n = 603; 40.2%). Fifty-six per cent of Polish young adults are experiencing significant depressive symptoms (95% confidence interval [CI]: 1.82-13.96). Young adults with no social support had higher KADS score than participants reporting some form of support (b = 1.794; 95% BCa CI = 0.659, 2.947). Participants receiving support from mother, both parents and siblings had a lower severity of depressive symptoms, while young people receiving support from father had more severe symptoms., Conclusions: This study provides initial findings in terms of depressive symptoms and support needs of young adults during the COVID-19 pandemic. It is imperative for mental health services, communities and schools to provide the necessary support to young adults.

- Abstract
The authors, in their study, refer to the changes taking place in Polish psychiatry under the National Mental Health Program (NMHP). They present their thoughts on the restructuring of psychiatric health care for adults in Lower Silesia. They refer to the European experiences described in the book Better Mental Health Care by G. Thornicroft and M. Tansella, and in the practical and scientific dimension - to the substantive premises, derived from demographic analyzes, epidemiological indicators obtained in the EZOP study, from Maps of Health Needs and other official statistics. Lower Silesia is the fifth voivodeship in Poland in terms of population, with four psychiatric hospitals in need of restructuring, a lower than the national rate of beds per 100,000 population and a relatively high rate of the so-called common mental disorders should be included in a targeted pilot project under the NMHP. In conclusion, the authors strongly support the fastest possible implementation of the community model of psychiatric services in Lower Silesia, indicating the methodology that should accompany the transformation process, the risks that may occur, but most of all, making people aware of the benefits that society should achieve in a human and economic sense.

Objectives: The aim of the study was to prepare the Polish adaptation of the SACS. The scale is a self-assessment tool designed to measure staff attitudes towards direct coercion of psychiatric wards patients., Methods: The sample consisted of 120 adults, staff from 7 psychiatric wards. The SACS is a tool created in Norway by Tonje Husum, comprising of 15 items describing psychiatric healthcare professionals attitudes towards direct coercion. The validation procedure incorporated three basic methods to be applied in the reliability analysis - the comparison of double tests with the same method, the analysis of statistical properties of test items as well as analysis of the relation of test items and subscales with the general test result., Results: After a "think aloud" type pilot study and a language validation, the internal consistency was assessed. The Cronbach's alpha ranged from 0.57 to 0.81 in the subscales, and it was 0.82 for the total score. The best solution obtained in exploratory factor analysis was a three-factor model, almost identical to the original one, confirming the division into three subscales: coercion as offending (critical attitude), as care and security (pragmatic attitude) and as treatment (positive attitude)., Conclusions: The psychometric characteristics of the Polish adaptation of the SACS are similar to those reported in the original version. The results allow to recommend the method for scientific research. However, further analyses are necessary to assess validity and discriminative power in larger settings.

- Abstract
Objectives: The World Health Organization's estimations indicate that about 50% of patients in well-developed countries may not adhere to long-term therapies. In the field of psychiatry, drop-outs from psychiatric treatment are particularly important. Personality disorders are a significant part of this sphere. The aim of this research was to empirically verify the hypothesis regarding the relation between comorbid personality disorders and drop-outs from treatment among patients of psychiatric wards., Methods: This study was a prospective cohort study. 110 patients, hospitalized in 3 different psychiatric wards, were included. Personality disorders were assessed with the Structured Clinical Interview For DSM-IV Personality Disorders (SCID-II). The research was financed by the Polish National Science Center (DEC-2011/01/N/NZ5/05364)., Results: The response rate was 89.1%. 72.56% of patients suffered from personality disorders (SCID-II) (among them the most prevalent were: personality disorder - not otherwise specified - 40.7% and borderline personality disorder - 12.38%; 22.95% of patients dropped out from treatment). However, occurrence of personality disorders was not relevant for those drop-outs. On the other hand, relationships at the level of certain criteria of borderline personality disorders and passive-aggressive personality have been revealed. These relationships became stronger when considered from the perspective of differences in the organization of treatment at individual wards., Conclusions: Some personality disorders may play an important role in drop-outs from psychiatric treatment. Presented results require further research.

- Abstract
Objectives: The aim of this study was to identify risk factors and estimate prevalence of depressive episodes among Wroclaw's universities students., Methods: Polish adaptation of the Composite International Diagnostic Interview (CIDI) was implemented to gather epidemiological data from 370 students of public universities in Wroclaw. Proportional stratified sampling was performed to obtain distinct, independent strata representing sex, year of study and educational profiles. Randomisation was ensured by recruitment procedures., Results: Prevalence of depressive episodes among students in Wroclaw is high - 14.7% throughout life, 9.8% within 12 months prior to the interview. High prevalence of severe and moderate depressive episodes seems to be worrisome (respectively 5.1% and 6.6% throughout life, 3.8% and 3.9% within 12 months prior to the study). Year of study, profile and lack of partner relationship remain risk factor for depression., Conclusions: High prevalence of depressive episodes indicates the need for prevention and therapy based on epidemiological data and tailored to the students' needs. Depression among students requires further epidemiological studies.

- Abstract
Introduction: Schizophrenia is a chronic and one of the most severe mental disorders. From many years studies regarding mortality among people suffering from schizophrenia are being conducted as a way of controlling indirectly the effectiveness of medical care and therapy., Aim: The aim of this paper was to determine the course of studies of mortality in schizophrenia and to analyze possibilities of using this kind of studies to evaluate changes in mental care system in Poland., Material: This paper is a review of European literature concerning studies of mortality in schizophrenia. Most of the analyzed publications were created in Northern and Western Europe. There are no international publications originating from Southern and Eastern Europe. Directions of current studies include changes of causes of death in persons with a diagnosis of schizophrenia over years, coexistence of somatic diseases, medicine's impact on mortality in schizophrenia and worse access to medical care in comparison to mentally healthy people., Conclusions: Mortality in schizophrenia is a useful factor in clinical studies enabling evaluation health effects of changes in mental health care system, what allows creation of system based on scientific evidence.
