Iranian men are at risk of developing gastrointestinal cancer caused by H. pylori. It is very imperative to find effective methods to control this bacterium as there are currently no very effective treatments for it. Honey has been shown to have antimicrobial properties against various pathogens. This study analyzed 15 honey samples from A. florea bees, collected from different floral and geographical origins, for their antimicrobial efficacy against H. pylori. Using atomic absorption measurements, the honey samples were also tested for their phenolic and flavonoid content, protein concentration, and mineral content. Antioxidant activity was determined using the FRAP, DPPH, and ABTS methods. The antibacterial activity of honey samples was investigated both in-vitro and in-vivo in the gastrointestinal tract of mice. Statistical analysis revealed a significant positive correlation between antioxidant activity and antibacterial activity. All honey samples showed antimicrobial activity in-vitro, among which jujube honey from Bushehr exhibiting the highest activity. Differences in antioxidant and antimicrobial activities were likely due to the flora of the plants and the geographic region from which the honey was harvested. Based on these results, A. florea honey may be used in the prevention and treatment of H. pylori-associated infections and inflammation of the gastrointestinal tract. This feature can be applied to the control of Helicobacter pylori along with other available measures.

This study deals with the relationship between H. pylori virulence factors and pathological findings in the patients with gastroduodenal disorders in a particular geographical area in the west of Iran. Materials and Methods: In this study, gastric biopsies were taken from 120 patients with indigestion (60 patients with gastritis and 60 patients with peptic ulcer). H. pylori virulence factors, vacA, cagA, oipA and iceA were determined by polymerase chain reaction (PCR). SPSS 16.0 software was used for data analysis. p≤0.05 was considered statistically significant. Results: The frequencies of the genotypes were as following: vacA (s1 51.7 %, s2 21.7%, s1s2 15.8 %, m1 32.51 %, m2 45 %, m1m2 7.5 %, s1m1 29.2 %, s1m2 27.5%, s2m1 7.5 %, s2m2 7.5 %), cagA 64.2%, oipA 57.5 %, iceA2 55% and iceA1/iceA2 15% respectively. Also, a significant relationship was observed between virulence factors such as cagA (p=0.013), vacA (p=0.041) and iceA (p=0.035) on one hand, and type of disorder (gastritis and peptic ulcer) on the other hand. Considering the relationships between the virulence factors and pathological findings, a significant relationship was found between iceA and acute inflammation (p=0.039). Also oipA and vacA showed significant relationships with bacterial density of H. pylori (p=0,000) and (p=0.011) respectively. Conclusion: The results of this study showed role of vacA and cagA in the type of disease and the impact of oipA and iceA in microscopic findings. Background and Objective: Helicobacter Pylori (H. pylori) is one of the reasons for the gastric inflammation and peptic ulcers. It is a predisposing factor of gastric adenocarcinoma. Cytotoxin A encoded by the cagA gene is one of the major virulence factors in bacterial pathogenicity, which is of special importance due to genetic diversity in different geographical areas. The purpose of this study was to evaluate the prevalence of the cagA gene in the patients suffering from gastric ulcers. Materials and Methods: Seventy-five paraffin blocks of stomach biopsy samples from patients infected with H. pylori and suffering from gastric ulcers were randomly evaluated in the province of Kermanshah in the spring of 2016, Iran. DNA samples extracted by the boiling method were investigated using PCR by the use of particular primer pairs for a protected area in the glmM gene, and the existence of the cagA gene. Results: From the 75 samples, 56 cases (%74.66) were glmM-positive and among them, 39 cases (%69.64) tested positive for cagA gene. Comparing patients in terms of gender, the frequency of the cagA gene was 22 (%39.28) and 17 (%30.35) in males and females, respectively. Finally, comparing patients in terms of age, 16 cases (%21.33) were younger than 40 years old and 23 cases (%41.07) were older than 40 years old. Conclusion: The results of this study showed significant association between the frequency of the cagA gene and peptic ulcer in the studied patients.

Introduction: Gastritis is one of the most common diseases affecting the stomach. Helicobacter pylori infection could lead to DNA damage in gastric epithelial cells, followed by error-prone DNA repair pathways that increase the accumulation of damage at the site of DNA double-stranded breaks, exacerbate genome instability, and facilitate the emergence of gastric cancer. This study aimed to examine the expression level of ATM, POLM, and TP53 genes involving in the DNA Damage Response and the cell cycle arrest in the gastric precancerous stage. Materials & Methods: Among 180 gastric biopsy specimens, 30 samples taken from patients with moderate chronic gastritis infected by H. Pylori were regarded as the case group, and 30 other samples taken from non-infected patients with mild chronic gastritis were regarded as control. Following that, RNA extraction and cDNA synthesis was performed. Afterward, the expression levels of ATM, POLM, and TP53 genes were evaluated by the Real-Time PCR method. Ethics code: 98-02-27-43392 Findings: The ATM, POLM, and TP53 genes in the cases showed a 4.07, 3.35, and 5.13 increase in the gene expression at the transcriptional level, compared to the controls. Discussion & Conclusions: In general, ATM, POLM, and TP53 genes showed a higher increased expression level in the case group, compared to the controls, which might indicate the activation of noted genes after H. pylori infection. This may subsequently activate the error-prone DNA repair and non-homologous recombination pathways.

Background: Drug-resistant strains and poor treatment response is the main reason for the failure in Helicobacter pylori (H. pylori) eradication. Unsaturated fatty acids with more than one double bond (PUFAs) have inhibitory effect on bacterial growth. Therefore, the aim of this study is to investigate the effect of Omega fatty acids on caspase-8 gene expression in H. pylori infected patients with dyspeptic symptoms. Methods: In this double blind clinical trial study in which 34 H.pylori infected patients with dyspeptic symptoms were randomly divided into two groups of 17 patients, the control group receiving standard triple therapy (including amoxicillin, clarithromycin and omeprazole) and the intervention group receiving standard therapy with omega-3, omega-6 and omega-9 fatty acids for two weeks. After treatment, Gene expression levels of Caspase-8 enzymes as well as the semi-quantitative of these proteins were studied with PCR Real-time method and frozen section and immunohistochemistry, respectively. Results: In the case group compared with the control group, a significant increasing in the expression of caspase -8 genes was observed (p=0.002). The protein level of active caspase -8 enzymes was significantly increased in the case group compared with the control group (0.004). After a long time of searching for microbial agents responsible for gastric disorders, in 2005 the noble prize was given to Robin Warren and Barry Marshall for the isolation of Helicobacter pylori (H. pylori) from human stomach and discovering the association between H. pylori infection and gastritis, peptic ulcers, and gastric cancer. Currently, an important part of research on digestive diseases is concerned with microbiology of H. pylori and its control, with the aim of preventing peptic ulcers and gastric cancer. Reports indicate that peptic diseases are developed as the consequence of interaction between H. pylori infection, genetic background of the human host, and the lifestyle of individuals. As most of H. pylori-infected individuals are asymptomatic, the role of host genetics and lifestyle might be more important than what previously suggested. Up to now investigators have not been successful to show that H. pylori isolates from gastritis, ulcer, and cancer are different. H. pylori acquisition occurs during childhood and food ingestion and close contact of infant with mother are the most probable routes of bacterial transmission. High frequency of H. pylori infection in many human populations indicates that the bacterium might have been established in yeast as an environmental host. Inside yeast H. pylori is protected from environmental stresses and can have access to plenty of food. Furthermore, yeast can serve as a reservoir for H. pylori, which facilitates bacterial spread in the environment as well as within human hosts. It has been suggested that H. pylori could have jumped from an intermediate host to humans and migrated along with human host from Africa to different geographic regions of the world. Peptic ulcer and gastric cancer are the important consequences of H. pylori infection that can be treated or prevented, in most of the cases, by antimicrobial therapy. Reports indicate a considerable decrease in peptic ulcers; however gastric cancers is one of the important issues in medical practice. Reports show high incidence of gastric cancer in northern provinces of Iran including Mazandaran, Guilan, Golestan, and Ardabil. High-incidence regions of gastric cancer are located in Asia and comprise a cancer belt that passes through Japan, Korea, certain regions in China, and Northern Iran. In Iran gastric cancer is the most prevalent cancer in men and the third cancer in women. Ardabil is the highest in the incidence of gastric and esophageal cancers. Eradication of H. pylori has been recommended in cases of gastric ulcer and those patients at the risk of gastric cancer. Occurrence of a considerable number of H. pylori-infected individuals in different human populations without symptoms indicates that H. pylori might be a beneficial human commensal microorganism rather than a pathogen. Background: CagA, a 120- to 145-kD Helicobacter pylori (H. pylori) protein, increases the risk of atrophic gastritis and gastric cancer (GC). The pathogenic CagA contains a highly polymorphic Glu-Pro-Ile-Tyr-Ala (EPIYA) repeat region in the C-terminal portion of the protein. The aim of this study was to determine the number and type of EPIYA (glutamine–proline–isoleucine–tyrosine–alanine) motifs within the cagA 3' variable region among H. pylori isolates and their association with GC. Materials and Methods: The total number of 206 individuals (170 controls and 36 patients with GC) referring to the endoscopy units of several cities in Iran (2008-2014) were recruited. The polymerase chain reaction (PCR) amplification was performed to determine the presence of H. pylori, cagA gene, and EPIYA motifs. Results: In this study, 81 (47.6%) and 22 (61.1%) of the controls and patients with GC were carriers of cagA+ strains, respectively. The overall frequency of EPIYA-AB, EPIYA-ABC, EPIYA-ABCC, and EPIYA-ABCCC in patients with GC were 0%, 59%, 9%, and 31.8%, respectively. The results of regression analysis showed a significant association between EPIYA-ABCCC motif and the risk of GC [OR = 9.99 (95%CI: 2.17-45.88), p = 0.003]. Conclusion: We propose that patients infected with H. pylori strains harboring more than one CagA EPIYA C motif (EPIYA-ABCCC) have an increased risk of GC, thus, testing for this genotype may have clinical usefulness. Introduction: Morbid obesity is well-known risk factor of cholelithiasis. On the other hand, rapid weight loss following bariatric surgery, as a treatment of morbid obesity, can cause the formation of gallstones. In this inquiry we decided to investigate the risk factors of cholelithiasis and evaluate the need for prophylactic cholecystectomy during bariatric surgery. Materials and Methods: All desired information of patients with morbid obesity who underwent bariatric surgery during 2011-2017 in surgical ward of Imam Reza hospital of Mashhad were extracted from patients' documents. All data were analyzed using SPSS software. Results: Totally 150 patients were enrolled in this study from which 127 patients did not have any history of cholelithiasis, 46.5% of patients without any history of cholelithiasis and 73.9% of patients with a history of this condition were positive for gastric H. pylori (p<0.05). Conclusion: In conclusion in our study the only factor associated with gallstone formation was H. pylori infection. Although H. pylori infection cannot be considered as a concrete reason for prophylactic cholecystectomy merely based on our results, its treatment should be kept in mind as a treatable risk factor of cholelithiasis. BACKGROUND AND OBJECTIVE: Gastric cancer is the fourth most common cancer and the second most common cause of cancer deaths worldwide. This study was designed to compare demographic characteristics, clinical symptoms and endoscopy findings in patients diagnosed as gastric ulcer versus gastric cancer.METHODS: This cross sectional study was carried out on all patients underwent upper endoscopy in Shahid Beheshti hospital of Babol, Iran during the years 2005-2010. Demographic characteristics, BMI, smoking, opium addiction, H. pylori status, clinical symptoms like: dysphasia, weight loss, nausea and vomiting, dyspepsia, anemia, upper gastrointestinal tract bleeding, and anatomic location of lesion in different parts of the stomach were main variables for comparison.FINDINGS: Of these 961 patients, 601 (62.5%) were men and 359 (37.4%) were women. Background: Helicobacter pylori (H. pylori) is the cause of most cases of peptic ulcers and gastric cancers and cause some of the most important cause of stomach cancer and lymphoma. The objective of this study was to evaluate the time trend of Helicobacter pylori prevalence and presence of intestinal Metaplasia over the period of 7 years in gastritis Iranian patients. Materials and Methods: In this cross-section study data related to H. pylori and intestinal Metaplasia (IM) among 14,860 consecutive gastritis patients, who referred to the gastrointestinal department of Tehran's Taleghani Hospital in Iran, was examined across the sex and age group. Results: The overall prevalence rate among patient with H. pylori infection was 83.5% (12406/14860) and 11,394 (84.1%) of them were related to the gastritis. The prevalence rate of H. pylori among patient with gastritis significantly higher (p<0.05) compared to patients without gastritis. In addition, the prevalence decreased with age while the presence of intestinal metaplasia increased with age (p<0.05). Conclusion: The results of this study showed that the prevalence of H. pylori infection in Iranian population has declined in recent years; nevertheless it seems to be highly prevalent in Iran. We also find a significant positive relationship between H. pylori infection and IM with gastritis. Background and Objective: Resistance of Helicobacter Pylori (H. pylori) to antibiotics is the main cause of relapse into Helcobacterial infections. With the use of several antibiotics that have synergistic effect, we can inhibit this antibiotic resistance. Thus, we aimed at determining resistance patterns and assessing the synergy of combining multiple antibiotics on H. pylori. Material and Methods: Biopsy specimens were taken from 100 patients with gastric ulcer referred to Imam Reza hospital in Amol, north of Iran. After isolation and identification of H. Pylori, antibiogram was performed with different antibiotic disks containing one antibiotic, a combination of two antibiotics (metronidazole + clarithromycin) and three antibiotics (metronidazole + Claritromycin + Ciprofloxacin). Results: In this study, H. pylori were isolated from 53 (53%) biopsy specimens. Of these, 49 (5.92%) were resistant to metronidazole, 14 (26%) to amoxicillin, 10 (19%) to clarithromycin, 7 (13%) to tetracycline, 13 (5/24%) to furazolidone and 7 (13%) to ciprofloxacin. In survey of synergistic effect, an increase in inhibition zone diameter around of combined disks was seen up to 5mm compared to the most effective antibiotic.
