Background and Aim: Gram-negative bacilli with multidrug resistance are important pathogens in hospitals and can lead to high mortality rates. The main purpose of this study was to determine the prevalence rates of Gram-negative bacilli with multidrug resistance isolated from clinical specimens of neonates and childern referring to Besat Hospital in Sanandaj City in 2018. Materials and Methods: In this cross-sectional descriptive study 10,000 clinical specimens were studied, including; blood, urine, wounds discharge, respiratory secretions and body fluids in 2018. Gram-negative bacteria were identified by standard microbiological methods, then susceptibility testing to common antibiotics was performed using disk diffusion method to determine multidrug resistant strains. Results: In this study, among 10000 clinical specimens we obtained 652 Gram- negative bacilli with positive cultures including urine 523 (80%), blood 78(12%), respiratory secretions 26(4%), wound discharge 15(2.5%) and body fluids 10(1.5%) from the outpatients and hospitalized patients. Gram-negative bacilli isolated from these samples included Escherichia coli (65.7%), Enterobacter spp (10.4%), Citrobacter spp (8.9%), Acinetobacter spp (7.2%), Klebsiella spp (3.5%), Pseudomonas spp (2.6%), Proteus spp (0.92%) and Serratia spp (0.78%). Most of Gram- negative bacilli with multidrug resistance were isolated from NICU (neonate intensive care unit) and PICU (pediatric intensive care unit) wards. The highest and lowest antibiotic resistance rates belonged, to ampicillin (84.5%) and meropenem (9.9%).The average multidrug resistance (MDR) rate was 68.9%, ranging from 55.9% for Enterobacter spp. to 83% for Acinetobacter spp. Conclusion: Multidrug resistance was high among the microorganisms isolated in this center. To select appropriate and specific treatment, periodic interactions and monitoring of the pathogen resistance pattern should be considered and unnecessary use of antibiotics should be avoided.

AbstractOne of the major problems in hospitals resistance of pathogenic bacteria , including bacteria that cause urinary tract infections(UTI), antimicrobial substances , the problem of increased costs of treatment , treatment failure and mortality rates are increasing.The purpose of this research is to identify multidrug-resistant Gram-negative bacilli of urinary tract infections with antibiotics and the effect of silver nanoparticles on them.Planting 240 clinical samples from patients admitted to two hospitals ( Fatemiyeh , Besat) within 6 months of 1395 a total of 150 gram negative bacilli of urinary tract infections was isolated. To determine antibiotic resistance of these bacteria, disk diffusion method was used, and their susceptibility was evaluated according to the CLSI(clinical and laboratory Standards Institute) . Gram-negative bacilli isolated were prepared and inhibition zone diameter was measured.The most common bacteria Escherichia coli was isolated and lowest frequencies of the Proteus vulgaris. All of the samples to a solution of silver nanoparticles at concentrations 200 ppm 100 ppm and were sensitive. Pseudomonas aeruginosa (30 mm) and Enterobacter aerogenes (27 mm) maximum diameter in the presence of silver nanoparticles showed 800 ppm.Conclusion: The results show that silver nanoparticles can have an inhibitory effect on all tested Gram-negative bacilli , and the concentration of nanoparticles , the diameter of the growth of Gram-negative bacilli of urinary tract infections resistant to multiple antibiotics increases.Key words: Gram-negative bacilli, Silver nanoparticles, Urinary Tract Infection,Antibiotic Resistance, Hamedan

Background and Aim: In the last few decades co-trimoxazole, an antibacterial combination of trimethoprim and sulfamethoxazole, has been used for treatment of bacterial infections, but due to the vast usage of these drugs, resistant strains have appeared throughout the world. One of the reasons for resistance to co-trimoxazole is related to drf genes, which are responsible for trimethoprim resistance, while the sulfamethoxazole resistance is due to sulfonamide sul genes. The aim of this study was to investigate drug resistance and frequencies of resistance genes in gram-negative bacteria isolated from clinical specimens. Materials and Methods: Clinical samples were collected from patients in Pars Hospital, Tehran, Iran, in which presence of gram-negative bacteria was confirmed by biochemical tests. Then antibiotic susceptibility tests were performed for 5 antibiotics by disk diffusion agar technic. DNA was extracted from bacteria resistant to co-trimoxazole, followed by PCR using specific primers for sul1, sul2, sul3, and drf7 genes. Results: In the co-trimoxazole-resistant bacteria, 26%, 74%, 2% and 16% of the isolates contained sul1 gene, sul2 gene, sul3 gene and drf7 gene, respectively. Further analysis of the data showed that 51% of the isolates were resistant to gentamicin, 74% to ceftriaxone, 65% to ciprofloxacin and 3% to colistin. For resistance to trimethoprim only the drf 7 gene was used. Conclusion: The results of this study show that in the isolates of co-trimoxazole-resistant gram-negative bacteria, the sul 2 gene has a major role in development of resistance to sulfonamides. In this study only the drf 7 gene was used to assess the resistance of trimethoprim, so we recommend to conduct studies also on other drf genes, so that the importance of each in resistance to co-trimoxazole can be determined. Background: The object of this study was to investigate the antimicrobial resistance pattern among common nosocomial Gram-negative bacilli isolated from patients with nosocomial infections. Methods: From June 2004 to December 2005, 380 isolates of common Gram-negative bacilli (Klebsiella, Pseudomonas, Acinetobacter and E. coli) from 270 patients with nosocomial infections in Sina and Imam Hospitals, Tehran, Iran, were evaluated for susceptibility to Imipenem, Cefepime, Ciprofloxacine, Ceftriaxone and Ceftazidime by Disc diffusion and E-test methods. Results: The most frequent pathogens isolated were Klebsiella spp. (40%), followed by Pseudomonas (28%), Acinetobacter spp. (20%) and E. coli (12%). The most active antibiotic was imipenem (84%). 26% of all isolates were sensitive to Cefepime, 26% to Ciprofloxacin, 20% to Ceftazidime and 10% to Ceftrixone. The susceptibility rates of Klebsiella to Imipenem, cefepime, ciprofloxacin, Ceftazidime and Ceftriaxone were 91, 25, 21, 13 and 7 percent, respectively and 91, 19, 17, 21 and 21 percent, respectively, for E. coli. Among Acineto- bacter spp., the susceptibility rate was 77% for Imipenem and 21% for Ciprofloxacin. Among Pseudomonas spp., 75% of isolates were susceptible to Imipenem and 39% to Ciprofloxacin. The comparison of the resistance status of microorganisms by both Disc diffusion and E-test methods showed a clinically noticeable agreement between these two tests. Conclusions: Since antibiotic resistance among Gram-negative bacilli has increased, enforcement of policy regarding proper antibiotic use is urgently needed in order to delay the development of resistance. Although it is widely accepted that E-test is more accurate in determining the resistance of microorganisms, our study showed that the Disc diffusion test will give the same results in most occasions and is therefore still considered useful in clinical practice.

Background: Infectious diseases are of the most important causes of mortality all around the world particular in developing countries. Recently, the most important thing that has worried medical society is antibiotic resistance. Multi-resistant gram_negative rods are important pathogens in hospitals, causing high rate of mortality. The main goal of this study was to investigate the antimicrobial resistance patterns among common gram-negative bacilli isolated from patients of Vali-Asr Hospital. Material and Methods: This is a cross-sectional descriptive study conducted between the years 2010-2012 in Vali-Asr hospital in Arak. In this study 1120 specimen were examined. Bacterial strains were isolated by conventional methods from various clinical samples of patients including: blood, urine, wound, sputum, CSF, andetc. All isolates were examined for antimicrobial resistance using disc diffusion method. Results: In this study 737 specimen were positive cultures. A total of 332 isolates of Gram-negative bacilli were identified. The most frequent gram negative bacteria were isolated from urine, wound, blood, respiratory secretion and catheter. The most frequent pathogens were E.coli followed by k. pneumonia, entrobacter, p.oaeruginosa, Acinetobacter spp, citrobacter and proteus. High rate of resistance to third generation of cephalospoins & carbapenems observed amang isolates of Acintobacter spp. Prodution of extended spectrum beralactamases (ESBLS) was found in 51.4% of all Gram negative bacteria. Conclusion: Antibiotic resistance, particularly multi-drug resistance is frequent among microorganisms of ValiAsr Hospital. Resistance in our country, like other countries have been shown to be increased, so it is highly recommended to prohibit unnecessary prescription of antibiotics. [ABSTRACT FROM AUTHOR]
