4. Comparing the Rehabilitation Structures in the Health Systems of Iran, Germany, Japan, Canada, Turkey, and South Africa.

Objective Because of the increasing number of people with disabilities and the lack of awareness of disability prevention methods in developing countries, creating a proper rehabilitation structure and providing appropriate services are among the important goals of every health system. Conducting comparative studies is one of the research methods for reviewing the structure of the rehabilitation system in the country's health system. This study aimed to compare the rehabilitation structure in Iran with 5 other countries. Materials & Methods This research is a comparative study conducted in 2022. In this study, sampling was purposeful. Five countries of Germany, Japan, Canada, Turkey, and South Africa were compared in terms of organizational and health management, financial and legal components, and social policy components with Iran. Reliable databases and related resources in the rehabilitation structure were used to collect data. In this study, the obtained data were analyzed using the Bereday model containing four stages: Description, interpretation, proximity, and comparison. The findings were evaluated in a comparative table. Results The findings indicated that in terms of organization and health management, the Ministry of Health should be responsible for health management and rehabilitation, but in Iran, in addition to the Ministry of Health, the Red Crescent, the Martyr Foundation, and Exceptional Education Organization are also involved in the management of rehabilitation. Financial issues and problems are significant barriers for people with disabilities to access rehabilitation services in these countries. In Iran, a large part of these costs is paid from the pockets of families. Regarding legal components in Iran, as in other countries, specific rules exist to provide services to people with disabilities. But sometimes, these rules are not properly implemented. From the sociodemographic perspective, Iran has an aging population, welcomes immigrants, and holds diverse cultures. The access of all people in need of rehabilitation in this diverse population to the services needed is limited. In terms of policy components, regulating national health policy requires review and, if necessary, changes in existing health system policies. Conclusion The existence of a specific position for rehabilitation in the Iranian health system, provision of services, and management of rehabilitation affairs by a single organization such as the Ministry of Health, as well as proper implementation of laws and policies, can lead to structural improvement and management of the health system and rehabilitation. Financial obstacles and problems to rehabilitation services should be reduced, and government and insurance must cover most of the costs of rehabilitation services. Also, a review of national health policies and legislation in the country should be done to improve the access of all people with disabilities to health and rehabilitation services. Introduction: The transition from institutional care to services provided in the community, which is namely care for the elderly and people with disabilities, is legally guaranteed in the Republic of Bulgaria. The priority is not only the reformation of the existing institutional-type services, but also the creation of new-type services for nearly 5,300 users. In this sense, it would be good to study and use the experience of leading EU countries, such as Germany. Aim: The purpose of the present study is to analyze the information from the preparation activities for the implementation of the reform in long-term care (deinstitutionalization) for persons with disabilities and/or dementia syndrome in need of medical and social care. Material and methods: 12 focus groups were formed for the needs of the research. Semi-standardized questionnaires were prepared. The results were evaluated and analyzed. For the current methodology, we chose the following principles: аccessibility, оbjectivity, рrofessionalism and аdaptability. Results: We focus on three key questions from the proposed questionnaires regarding respondents‘ awareness of deinstitutionalization and care. 78% of the respondents answered that they were not aware of the upcoming reform, and only 22% stated that they were aware of the expected changes. 64% of those asked are not inclined to place their loved ones in a social service in the community. Willingness to trust specialists in the field of long-term care is expressed by only 26%. When specifying the type of social service, the results change: 65% trust professionals and 27% reject the possibility. Conclusion: The transition from institutional to community-based services is overdue. The public is not aware of the upcoming long-term care reform. The available infrastructure for new homes in our country is insufficient. Social entrepreneurship in Bulgaria is not developed enough to replace the state in the care of persons with disabilities and dementia syndrome. [ABSTRACT FROM AUTHOR]

In this study, the influence of extrusion conditions on the protein oxidation and degree of protein hydrolysis of chickpea extrudates was determined. The protein oxidation was expressed by the level of protein carbonyl groups determined spectrophotometrically, and the degree of protein hydrolysis was calculated as the free amino nitrogen content was divided to the total nitrogen in the sample, determined by the Lowry method. To investigate the influence of extrusion conditions (independent variables): moisture content (17, 20, 23, 26, 29%), barrel temperature (140, 150, 160, 170, 180ºC), screw speed (130, 150, 170, 190, 210 min-1), and feed screw speed (20, 35, 50, 65, 80 min-1) on the protein oxidation and degree of protein hydrolysis of chickpea extrudates, the response surface methodology was applied using a central composite rotatable design. Chickpea extrudates were obtained using а laboratory singlescrew extruder, Germany. The screw compression ratio was 3:1. The temperatures of the feed and metering zone were 100 and 150ºC, respectively. The die diameter was 4 mm. The level of carbonyl groups, expressing the oxidative changes in the protein of the obtained extrudates, was from 0.259 to 0.779 nmol DNPH/mg protein. The degree of protein hydrolysis of the chickpea extrudates varied from 12.76 to 17.86%. Statistical analysis indicated that the moisture content and screw speed had an effect on the degree of protein hydrolysis (P<0.05) whereas the feed screw speed and barrel temperature had no effect on it. The European building stock needs fast and affordable digitalization for facilitating the renovation process to achieve higher energy efficiency in buildings and in houses. Most of the residential buildings built between 1925-1975 lack proper documentation and digital building information, as opposed to newer buildings. Digital building information (geometry and attribute data) is required to plan retrofitting projects or renovations of facades. This work can only be efficient if Building Information Modelling (BIM) is available for stakeholders (owners, architects, contractors etc.). Whereas city planners of municipalities or social housing companies are striving to manage their properties and building portfolio on multiple scales, these stakeholders are seeking an approach that enables spatial planning, efficient asset management and energy consumption and management services for their buildings. In a EUREKA labelled project (E!12649 Ready2BIM) three partners worked together, two from Hungary (Geonardo Ltd. and SZTAKI) and one from Germany (CAALA GmbH). [ABSTRACT FROM AUTHOR]
