PURPOSE The purpose of this study was to investigate effects of activity tracker & application use on adherence, metabolic syndrome factor and body composition of middle and senior-aged. METHODS The subjects of this study were 1,196 middle and senior-aged man and women with health hazard factor. All of subjects divided to two groups, activity tracker & application use group 990(man=492, women=498) was participating in 24 weeks, and control group 206(man=71, women=135) was not in that program. They performed physical activity utilizing activity tracker & application for 24 weeks. The following items were measured before, middle and after the program : physical activity adherence, metabolic syndrome factor and body composition. RESULTS Results of this study showed that physical activity adherence appeared nothing in drop-rate of male(9.35%) and female(8.63%) : So adherence were male(90.65%) and female(91.37%). And changes of metabolic syndrome factor variables were significantly improved in total cholesterol, triglyceride, systolic blood pressure, and glucose in activity tracker & application use group and control group after 24 weeks. Also, body composition were showed significantly different in BMI, weight, waist. CONCLUSIONS Activity tracker & application use accompanied regular physical activity were effective in middle and senior-aged health promotion. This results can be utilized not only in new health promotion program of public health center but also in a local resident health management strategies.

Background: Recent studies have demonstrated the benefits of off-pump coronary bypass grafting over the on-pump technique in patients with chronic kidney disease (CKD). To further reduce the risk of acute kidney injury and the need for renal replacement therapy, even in patients undergoing off-pump coronary artery bypass grafting, we adopted protocol-based perioperative management for patients with CKD. Methods: From December 2012 to March 2015, 265 patients underwent isolated off-pump coronary artery bypass grafting. To analyze renal function in a stable condition, we excluded 12 dialysis-dependent end stage renal failure and 10 emergency or urgent cases. Among the remaining 243 patients, 208 patients had normal kidney function (normal group), and 35 patients had CKD (CKD group). Minimizing contrast exposure, ensuring adequate hydration, using strict drug dosage adjustment, and optimizing hemodynamic status were key elements of the protocol for the CKD group. Results: The risk of acute kidney injury was about ×3 higher in the CKD group than in the normal group (p=0.01). Estimated glomerular filtration rates and serum creatinine levels deteriorated until the third postoperative day in the CKD group. However, by adopting protocol-based perioperative management, this transient renal dysfunction recovered to preoperative levels by the fifth postoperative day without requiring renal replacement therapy in all cases. Conclusion: Off-pump coronary bypass surgery combined with this protocol-based perioperative management strategy in patients with non-dialysis-dependent CKD could mostly be performed without renal replacement therapy.

Background: We report our initial experiences of robot-assisted cardiac surgery using the da Vinci Surgical System. Methods: Between February 2010 and March 2014, 50 consecutive patients underwent minimally invasive robot-assisted cardiac surgery. Results: Robot-assisted cardiac surgery was employed in two cases of minimally invasive direct coronary artery bypass, 17 cases of mitral valve repair, 10 cases of cardiac myxoma removal, 20 cases of atrial septal defect repair, and one isolated CryoMaze procedure. Average cardiopulmonary bypass time and average aorta cross-clamping time were 194.8±48.6 minutes and 126.1±22.6 minutes in mitral valve repair operations and 132.0±32.0 minutes and 76.1±23.1 minutes in myxoma removal operations, respectively. During atrial septal defect closure operations, the average cardiopulmonary bypass time was 128.3±43.1 minutes. The median length of stay was between five and seven days. The only complication was that one patient needed reoperation to address bleeding. There were no hospital mortalities. Conclusion: Robot-assisted cardiac surgery is safe and effective for mitral valve repair, atrial septal defect closure, and cardiac myxoma removal surgery. Reducing operative time depends heavily on the experience of the entire robotic surgical team.

Background: This study aimed to investigate sternal healing over time and the incidence of poor sternal healing in patients undergoing coronary artery bypass graft (CABG) surgery using bilateral internal thoracic arteries. Methods: This study enrolled 197 patients who underwent isolated CABG using skeletonized bilateral internal thoracic arteries (sBITA) from 2006 through 2009. Postoperative computed tomography (CT) angiography was performed on all patients at monthly intervals for three to six months after surgery. In 108 patients, an additional CT study was performed 24 to 48 months after surgery. The axial CT images were used to score sternal fusion at the manubrium, the upper sternum, and the lower sternum. These scores were added to evaluate overall healing: a score of 0 to 1 reflected poor healing, a score of 2 to 4 was defined as fair healing, and a score of 5 to 6 indicated complete healing. Medical records were also retrospectively reviewed to identify perioperative variables associated with poor early sternal healing. Results: Three to six months after surgery, the average total score of sternal healing was 2.07±1.52 and 68 patients (34.5%) showed poor healing. Poor healing was most frequently found in the manubrium, which was scored as zero in 72.6% of patients. In multivariate analysis, the factors associated with poor early healing were shorter post-surgery time, older age, diabetes mellitus, and postoperative renal dysfunction. In later CT images, the average sternal healing score improved to 5.88±0.38 and complete healing was observed in 98.2% of patients. Conclusion: Complete sternal healing takes more than three months after a median sternotomy for CABG using sBITA. Healing is most delayed in the manubrium.

Malignant cerebral infarction as postoperative complication after pulmonary resection occurs rarely, but can be rather serious. We report a case of 81-year-old man who suffered from malignant cerebral infarctions after pulmonary resection for lung cancer. He had a history of well-controlled hypertensions, but no evidences of arrhythmia, and neither stenosis nor atheroma in the carotid arteries and intracranial arteries. There were no specific events during his operation except that an inadvertent left carotid artery puncture occurred during the central line insertion. In intensive care unit (ICU), he had a delayed recovery of consciousness and dysarthria with right hemiplegia. Computed tomography revealed malignant middle cerebral infarctions due to the occlusion of left middle cerebral artery. It could be the thromboembolism due to pulmonary resections or carotid artery punctures in the patient without high risk factors. [ABSTRACT FROM AUTHOR]

In infrapatellar bursitis, an acute or chronic inflammation of the infrapatellar bursa causes pain and swelling at the front of the knee. To date, only a few studies have reported cases of infrapatellar bursitis that are treated using Korean medicine. The aim of the present study is to report a case of a 27-year-old male with front knee pain caused by infrapatellar bursitis that was treated with integrated Korean medicine treatments (e.g., acupuncture, pharmacopuncture, herbal medicine). The patient- reported numeric rating scale confirmed that the patient’s pain was relieved, while the physical function tests also indicated a significant improvement. Based on the findings of this study, it can be suggested that integrated Korean medicine treatments could be an effective therapeutic option for patients diagnosed with infrapatellar bursitis.

The triangular fibrocartilage complex (TFCC) is crucial for stabilizing the ulnar side of the wrist but can be partially torn due to degenerative changes, falling, or sudden injuries. However, only a few studies have focused on the use of Korean medicine treatments for TFCC-related wrist pain. This case report presents the successful outcomes of integrated Korean medicine treatment in a patient with a partial TFCC tear. The diagnosis was confirmed via magnetic resonance imaging, which revealed the tear. Subsequently, the patient received integrated Korean medicine treatments. The assessment of the pain scale scores revealed a significant reduction in wrist pain and an improvement in daily life activities. Based on the findings of this study, integrated Korean medicine treatments have been proven to be an effective treatment option for patients with a partial TFCC tear. However, further research is needed to determine the potential benefits and mechanisms of these treatments in managing TFCC-related wrist pain.

Incidence of virus diseases in red pepper of open field in Yeongyang-Gun, Gyeongbuk Province was investigated by reverse transcription-polymerase chain reaction in 2012-2016. The infection rate of Cucumber mosaic virus (CMV), Broad bean wilt virus 2 (BBWV2), Pepper mottle virus, Potato virus Y, Pepper mild mottle virus and Tomato spotted wilt virus was 46.1%, 41.5%, 2.0%, 2.0%, 4.4% and 0.1% respectively. Incidence rate of single and mixed infection was 31.2% and 62.6%. Most of single infections were CMV and BBWV2. Among mixed infections, the incidence rate of CMV+BBWV2 mixed infection was the highest as 49.3% and most of mixed infections of triplex and tetraplex included CMV+BBWV2 mixed infection. CMV single infection caused mosaic, chlorosis, yellowing and vein necrosis and BBWV2 single infection induced cholosis and mosaic. CMV+BBWV2 mixed infection caused severe mosaic with chlorosis or malformation.

Objectives : The purpose of this study was to investigate effect of Exercise on inhibition Blood pressure by Auricular-plaster Vaccaria seeds Methods : This study picked 40 peoples from 20 May 2009 to 30 June 2009 and experiment them. Attached Vaccaria seeds to auricular acupuncture of the experiment group. Did not attach them to the control group. Them to exercise using a Bike-Ergometer exercise. I measured their blood pressure before the exercise, 15 minutes after the exercise and 30 minutes after the exercise. I compared the difference between the systolic blood pressure and diastolic blood pressure. The method to choose the subjects was Random allocation. Results : 1. Comparing the systolic blood pressure of the experiment group and the control group, the average systolic blood pressure of the experiment group, who did the Bike-Ergometer exercise putting on Auricular-plaster Vaccaria seeds, was 125.45 mmHg before the exercise, 121.20 mmHg 15 minutes after the exercise and 120.30 mmHg 30 minutes after the exercise. Therefore, the group’s systolic blood pressure after the exercise was more controlled than the systolic blood pressure before the exercise. The control group’s systolic blood pressure increased compared to the beginning. To measure the change before and after the exercise, I carried out paird-t test. The result was statistically significant. 2. Comparing the diastolic blood pressure of the experiment group and the control group, the average diastolic blood pressure of the experiment group, who did the Bike-Ergometer exercise putting on Auricular-plaster Vaccaria seeds, was 81.45 mmHg before the exercise, 79.65 mmHg 15 minutes after the exercise and 79.05 mmHg 30 minutes after the exercise. As a result of carrying out paird-t test to measure the change of the diastolic blood pressure, the change of the dilating blood pressure was statistically significant. However, the difference of the dilating blood pressure between 15 minutes after the exercise and 30 minutes after the exercise was not statistically significant. Comparing the systolic blood pressure and the diastolic blood pressure of the experiment group and the control group, the blood pressure of the experiment group, who did the Bike-Ergometer exercise putting on Auricular-plaster Vaccaria seeds, decreased compared to the beginning and the blood pressure of the control group, who did not put on Auricular-plaster Vaccaria seeds, increased compared to the beginning. Conclusions : The hypothesis was supported that the increase of the systolic and diastolic blood pressure of the experiment group, putting on Auricular-plaster Vaccaria seeds, was more controlled than that of the control group. In future, it can be medically used by verifying the various effects through repeated studies.
