本研究の目的は、地域中核病院で実施している自宅退院困難者のスクリーニングによって困難者であると判断された患者の状態及び退院先の実態を把握し、退院先に影響を及ぼす要因について明らかにすることである。自宅退院困難者スクリーニングにより自宅退院困難者として抽出された患者104名分の自宅退院困難者スクリーニングシートをデータとして分析を実施した。対象を実際の退院先により自宅退院群と自宅外退院群の2群に分割して分析した。自宅退院群は、ADLはJAランクと自立度が高く、入院期間は短く家族構成人数は多い傾向で本人の希望も自宅退院を明らかにしているものが多かった。自宅外退院群は脳血管疾患が多く、ADLはBCランクと自立度の低いものが多く入院期開も長い傾向にあった。退院先に関連した要因は、ADLと家族構成人数と本人の自宅退院への意思表示および看護師の自宅退院に対する判断の4因子であった。, A local core hospital includes screening of discharged patients having difficulty after discharge. Therefore, the purpose of this study was to explore the status of patients having difficulty with home discharge and to identify factors associated with a choice of discharge destinations. Of 398 patients receiving screening, 104 completed the screening sheets and were judged as having difficulty. They were divided into two groups according to their actual discharge destinations: the home discharge group (n=72)and the non-home discharge group(n=32). Analyses based on the screening sheets showed that those in the home discharge group were ranked higher in ADL, had shorter hospital stays, and had larger families and a desire for home discharge. Also, there were more cerebrovascular patients in the non-home discharge group. In fact, it appears that factors associated with discharge destination were the ADL function, family size, and patient desire.
