A 59-year-old man presented with epigastralgia. A diagnosis of advanced gastric cancer MLU, Circ, Type 3, 160 mm, tub2, cT4b (SI: panc), cN1, cM0, cH0, cP0, cCY0, cStage ⅢB was made. Because of difficulty with oral intake due to malignant outlet obstruction and tumor bleeding, endoscopic self-expanding metallic stent placement was performed. We administered chemotherapy involving docetaxel, cisplatin, and S-1(DCS). After 2 courses of chemotherapy, the primary lesion and regional lymph nodes had reduced in size. His response was judged as SD according to the RECIST criteria. The patient elected to undergo explorative laparotomy for assessment of the gastric cancer. The intraoperative findings showed that there was no pancreatic invasion, peritoneal dissemination, or distal metastasis, so a total gastrectomy and D2 lymph node dissection was performed. The pathological findings showed that there were very few cancer cells in the primary lesion, and a lymph node metastasis was found. The final stage was gastric cancer MLU, Circ, Type 3, 100 mm, muc, ypT4a(SE), ypN3a (13/51), ypM0, ypH0, ypP0, ypCY0, ypStage ⅢC. The therapy evaluation was Grade 1b. In summary, we encountered a patient with gastric cancer in whom curative surgery was made possible by undergoing chemotherapy and metallic stent placement.

A 71-year-old man was diagnosed with gastric cancer (LM, Less, type 2, T4aN2M0, cStageⅢb). A diagnostic laparoscopic surgery revealed serosal invasion without peritoneal dissemination. Two courses of neoadjuvant chemotherapy (NAC) for locally advanced gastric cancer using DCS (DTX: 20 mg/m2 on day 1, CDDP: 50 mg/m2 on day 1, S-1: 120 mg/day, twice a day on days 1-14) was performed, which resulted in a clinical partial response. Consequently, distal gastrectomy with D2 lymph node dissection, and BillrothⅠreconstruction were carried out. Histopathological examination revealed no residual cancer cells both in the primary lesion and in the lymph nodes, indicating a pathological complete response (grade 3). Six courses of S-1 (120 mg/day on days 1-28, followed by 2 weeks of rest) were administered as adjuvant chemotherapy. At the 2 years 10 months follow-up after adjuvant therapy, the patient has had no recurrence. Combination chemotherapy with NAC-DCS can be a safe and effective regimen for locally advanced gastric cancer.

Background: We investigated the efficacy of nutritional support in patients treated with chemoradiotherapy (CRT) for locally advanced esophageal cancer (LAEC)., Methods: Eleven patients treated with CRT for locally advanced esophageal squamous cell carcinoma were included. Oral intake energy expenditure (OIE) and total energy expenditure (TEE) of all patients were calculated. Oral nutrition supplementations (ONSs) were utilized as nutritional therapy for the patients with malnutrition (OIE/TEE<0.6). Enteral nutrition (EN) was used in the patients with tumor obstruction., Result: Two patients (18.9%) received ONS and 2 other patients received EN. Seven patients were able to take enough energy in the meal. The mean energy charge was increased from 67.9%to 84.9%. Nine patients (81.8%) completed the treatment regimen. During the CRT period, the prognostic nutritional index (PNI) and C-reactive protein level (mg/dL) were not significantly different. The body mass index decreased to 0.39 kg/m2 (p=0.039) and the mean weight loss was 1.57%. The overall response rate was 81.8%., Conclusion: The nutritional support in the patients treated with CRT for LAEC is effective for maintaining nutritional status. Moreover, the response rate is satisfactory.

A 77-year-old man underwent total gastrectomy with D1+ lymph node dissection after being diagnosed with cT4aN2M0, cStage ⅢB gastric cancer. Peritoneal dissemination was detected in the bursa omentalis. The pathological diagnosis after surgery was pT4aN3b (21/41) M1 (P1). He was treated with 6 courses of S-1 chemotherapy. Two years after surgery, upper gastrointestinal endoscopy revealed the presence of a tumor in the mid-thoracic esophagus. It was diagnosed to as metastatic esophageal cancer and treated with combination chemotherapy consisting of docetaxel (25 mg/m2, days 1, 8, 15) and cisplatin (25 mg/m2, days 1, 8, 15) in a 28-day cycle. A clinically complete response was observed after 5 courses of chemotherapy. Currently, the patient is alive with no signs of recurrence 12 months after the initial recurrence.

A 45-year-old man was transferred to our hospital because of advanced gastric cancer and peritoneal dissemination. After he received an S-1 plus cisplatin( CDDP) regimen for 6 courses, the primary lesion and ascites had disappeared. However, the primary lesion recurred, and he underwent treatment with 16 courses of an S-1 plus docetaxel regimen. He subsequently developed peripheral neuropathy, and was switched to the irinotecan (CPT-11) regimen. As he experienced appetite loss, it was impossible to continue the chemotherapy. Therefore, he underwent a salvage surgery and an R0 resection was performed. However, 9 months after the surgery, he experienced paraaortic lymph node recurrence and peritoneal dissemination. The patient died 13 months after the surgery.

An 82-year-old man underwent total gastrectomy(D2 lymph node dissection)in August 2006. The pathological findings indicated T4a, N3, M0, Stage IIIC gastric cancer, but adjuvant chemotherapy was not initiated. In October 2009, he presented to the hospital with dyschezia. During colonoscopy, the scope could not pass through the colon, thus indicating rectal stenosis. The biopsy findings indicated the presence of signet ring cell carcinoma, which was determined to be due to the peritoneal dissemination from the gastric cancer. To avoid the need for creating a stoma, radiation therapy(2 Gy×20; total dose, 40 Gy)and chemotherapy(weekly paclitaxel and S-1)were initiated. Rectal stenosis was improved and complete remission was maintained until May 2013.

Background: Postoperative anastomotic hemorrhage is a relatively rare complication. However, when it does occur, immediate treatment is needed., Methods: In all, 1,700 patients underwent curative gastrectomy between 2000 and 2010. Anastomotic hemorrhage was observed in 9 patients after surgery. The clinical course of these 9 patients was analyzed., Results: The median age of the patients was 62 years, and all patients were men. Two patients underwent distal gastrectomy, 1 underwent laparoscopic distal gastrectomy, and 6 underwent total gastrectomy. Bleeding occurred as follows: 5 were at gastro- or esophagojejunostomic site, 2 were at gastroduodenostomic site, and 2 were at jejunojejunostomic site. Five patients received conservative treatment and 2 underwent re-operation. Two additional patients achieved complete hemostasis with endoscopic treatment. The patients who received endoscopic treatment were discharged earlier than those who received other treatments., Conclusions: Endoscopic intervention was useful for the diagnosis and treatment of postoperative anastomotic hemorrhage.

Background: Body weight loss is a common outcome in patients with gastric cancer who have undergone gastrectomy. However, the rate of body weight loss after surgery is unknown., Methods: In this retrospective study, we selected patients who underwent radical gastrectomy for gastric cancer and were diagnosed with Stage II or III disease. Further, we compared the body weight loss after surgery between patients in the surgery alone group and the S-1 adjuvant chemotherapy group., Results: We evaluated 163 patients, of which 81 underwent only surgery, and 82 underwent surgery followed up with S-1 adjuvant chemotherapy. The body weight loss rate at 1, 3, and 6 months in the surgery alone group were 93.1%, 92.9%, and 94.9%, while those in the S-1 adjuvant group were 92.9%, 90.4%,and 91.9%, which was a significant difference., Conclusions: Body weight loss after gastrectomy was higher in the S-1 adjuvant group than in the surgery alone group. Further, nutritional support is required for these patients to maintain body weight after surgery.

We report a case of gastric small cell carcinoma with metastatic liver tumors responding to surgery and chemotherapy. The patient was a 67-year-old man with advanced gastric cancer, clinically diagnosed as P0H1M0CY0T3N1. He was registered in a phase III trial, and was scheduled to undergo gastrectomy and S-1 plus CDDP chemotherapy after surgery. He underwent D1 total gastrectomy and Roux-en-Y reconstruction. Small cell carcinoma of the stomach was diagnosed from the histopathological findings. After surgery, he received the following chemotherapy: 13 courses of CPT-11 plus CDDP chemotherapy, 2 courses of S-1, 5 courses of paclitaxel, and 6 courses of CPT-11. The patient is alive 22 months after his operation. We conclude that the combination of surgery and chemotherapy was effective for small cell carcinoma of the stomach, which was considered to have a poor prognosis.

The patient was a 31-year-old man with advanced gastric cancer, clinically diagnosed as ML, Less, Type 3, sig, cT3, cN0, cH0, cP0, cM0, cCY0, cStage IIA. He underwent D2 distal gastrectomy. On microscopic examination, tumor cells were detected in the distal margin of the resected stomach. After surgery, he received 1 course of S-1 followed by chemoradiation therapy(1.8 Gy×25, a total of 45 Gy) with 90 mg/m2 of paclitaxel and 40 mg/m2 of CDDP on days 1, 15, and 29 over 5 weeks. Subsequently, he received 5 cycles of S-1 chemotherapy. To date, no recurrence has been observed 5 years after surgery. This sequential therapy is an option to consider for enabling local and systemic control after gastric cancer surgery.

The primary aim of the revision in the new Japanese guidelines (JGL) was to provide general as well as specialized clinicians in Japan and the world with knowledge on standard treatments, and that of the Japanese classification of gastric carcinoma (JC) was to provide the basic rules to describe the status of a tumor and the evaluation of treatment outcome. For developing good clinical practice in the world, the definitions identical with those in the UICC/TNM 7th edition are adopted so that the Japanese experience can be expressed using the international terminology. The major revision points are summarized in this paper.

Adjuvant chemotherapy is aimed at eradicating residual micro-metastatic tumor cells existing at the distant site outside of the surgical field. From evidence gathered from around the world, several courses of intensive neoadjuvant chemotherapy are promising even in Japan, where D2 surgery is the standard for local control. The Japan Clinical Oncology Group (JCOG) plays a central role in the development of neoadjuvant chemotherapy in Japan. Two JCOG Phase II trials clarified that neoadjuvant chemotherapy was effective for bulky nodal disease. A JCOG Phase III is now on-going to confirm the efficacy of neoadjuvant chemotherapy of S-1+CDDP for cancer of the schirrhous type. In another group, two randomized Phase II trials are also ongoing to compare different regimens and courses of neoadjuvant chemotherapy. These studies will set the direction of neoadjuvant chemotherapy development in the future.

The importance of surgery (gastrectomy plus lymph node dissection) for the treatment of advanced gastric cancer is unquestionable, although there has been a disparity in methods used to achieve local control in Asia and the West. The superiority of D2 dissection has not been confirmed in a large multiinstitutional trial, and the long-term follow-up results of a Dutch trial revealed that the recurrence rate was lower in the D2 group. Thus, the European Society for Medical Oncology and the US National Comprehensive Cancer Network guidelines recommend D2 dissection, leading to a worldwide consensus. Meanwhile, the focus of oncology should be on multimodality treatment for cure, and numerous large, randomized clinical trials have established effective adjuvant treatment. In gastric cancer, different evidence emerged first in the USA, followed by Europe, and Japan/the Republic of Korea to become the standard for each: adjuvant chemoradiation, perioperative adjuvant chemotherapy, and postoperative chemotherapy, respectively. The Japanese standard has become adjuvant S-1 chemotherapy for 1 year after surgery, and the optimal regimen for stage III should be further investigated in consideration of other robust results. Other issues include the role of surgery in local control with regard to adjuvant treatment such as radiation and molecular-targeted treatment to establish a worldwide standard.

Background: S-1 mono-therapy for advanced gastric cancer is hard to continue in some patients due to adverse events. We retrospectively examined the patients who received S-1/Krestin( PSK) immunochemotherapy for advanced gastric cancer., Methods: During September 2006 to August 2008, 3 patients had received S-1/PSK. S-1 was given for 6-week schedule or 3-week schedule, and PSK was for everyday during the S-1 treatment., Results: Cases 1 and 2 had lymph node metastases after surgery. Case 3 had peritoneal metastasis. The median treatment duration was 112 days. The median overall survival was 552 days. All patients stopped the treatment due to progression of disease. There were no patients who stopped the treatment due to adverse events., Conclusions: PSK may increase the compliance of S-1.

Background: Survival and clinical course were unknown in patients with recurrence of para-aortic lymph node after curative D2 gastrectomy and adjuvant S-1 chemotherapy., Methods: The study examined clinical characteristics and prognosis of 3 patients who had para-aortic nodal recurrence after curative D2 gastrectomy and S-1 adjuvant chemotherapy., Results: Initial stage was III C in all of the patients. Median treatment interval of S-1 was 8 . 8 months. Para-aortic nodal metastasis was single in 2 and multiple in 1 patient. Median survival time was 14 .3 months. Other recurrent sites than para- aortic node were not seen in all of the patients., Conclusions: In the times of S-1 adjuvant chemotherapy, it is valuable to know whether para-aortic node should be resected prophylactically, or be treated after recurrence, or be treated by locally or systemically.

Background: This is a randomized phase II trial to evaluate non-inferiority of progression-free surviva(l PFS) by comparing S-1 and S-1/PSK for advanced or recurrent gastric cancer. Sample size was calculated to be 120. However, the trial was terminated because of slow accrual. This exploratory analysis was done by collecting the minimum data., Results: Only 8 patients were enrolled. Four patients were randomly assigned to S-1 and the others to S-1/PSK. Performance status was 0 in all 8 patients. Median age was 64. Median overall survival was 13.7 months in all 8 patients, 8 . 9 months in S-1 group, and 13 . 7 months in S-1/PSK group., Conclusion: When considering PS 0 in 8 patients enrolled, an overall survival was comparable to that observed in S-1 group of other phase III trials. Standard chemotherapy for advanced gastric cancer was changed to S-1/CDDP by SPIRITS phase III trial which was presented just after this trial was initiated, which would be a major cause of slow accrual. When conducting phase III trial, we should carefully determine design and standard arm by considering on-going phase III trial.

Background: The cost of laparoscopic gastrectomy (LG) has not been fully clarified yet., Patients and Methods: The actual cost for the instruments used for surgery was examined between LG and conventional open gastrectomy( OG) by separating distant (l D-) and total (l T-) gastrectomy in a total of 20 patients(5 for each)during Oct 2010 and Feb 2011. The profit was defined as the difference of the actual cost and the operation fee including the instruments determined by the insurance and compared., Results: The fee for D-OG, T-OG, D-LG, and T-LG were 708, 700 yen, 856 , 400 yen, 783 , 600 yen and 922 , 300 yen, respectively. The mean profits of D-OG, T-OG, D-LG, and T-LG were 408 , 297 yen, 475 , 812 yen, 308, 681 yen and 269 , 478 yen, respectively., Conclusion: Appropriate surgical and instrumental fees should be determined considering the actual cost.

In Japan, elderly gastric cancer(GC)patients have been rapidly increasing, and in 2020 when the babyboomers' GC incidence reaches its peak, the number of deaths due to GC will reach seventy thousand per year. Few clinical trials have been done for surgery and chemotherapy for GC so far, and little information is available on the risk benefit for elderly patients. Reports from single institutes described comparable efficacy and safety, while a large national database indicates age as an independent risk factor. Similarly, little study and information are available on chemotherapy for elderly with GC. In future, standardization of comprehensive geriatric assessment(CGA)and preoperative risk assessment in clinical trials with risk-stratified protocol treatment, and a large surgical database with similar standard risk assessment are warranted.

Prognosis of scirrhous gastric cancer with minimal peritoneal metastasis was poor, and the role of resection has not been clarified yet. Analysis 1: Overall survival was examined in 79 patients who underwent R0/R1 resection during 1970-1995 at Kanagawa Cancer Center (Group A), and in 47 patients who underwent R0/R1 resection and received S1 chemotherapy at the 30 hospitals of Japan Clinical Oncology Group (Group B). Hazard ratio (HR) of group B to group A was examined. HR was 0.64 at 1 year, 0.76 at 2-year, and 0.92 at 3-year. Analysis 2: HR of S1 group in SPIRITS phase III trial to FU group in JCOG 9205 phase III trial was examined. HR was 0.64 at 1 year and 0.84 at 2-year. Analysis 3: HR was compared each other including HR of ACTS-GC phase III trial. HR was ACTS

Intraperitoneal (i.p.) chemotherapy targets i.p. regional metastasis by maintenance of high-dose intensity. Thus, it is applied for cancers in which i.p. lesions limit the prognosis or QOL of patients. In ovarian cancer, ample evidence to support survival benefit of i.p. chemotherapy has been established, however, its standardization has not yet been accomplished. NCI has made efforts for the dissemination of and education in i.p. therapy, and GOG's research focuses on a less toxic i.p. regimen. In gastric cancer, the advantage of i.p. therapy has not been proved, and for its approval, phase I-III trial of i.p. with or without cytoreduction surgery should be initiated.

We report a rare case of breast metastasis of gastric cancer in a 61-year-old female. She was diagnosed as primary gastric cancer with peritoneal dissemination and received systemic chemotherapy after distal gastrectomy for a primary lesion. A tumor developed in her right breast 4 years after the surgery, and was confirmed to be the metastasis from gastric cancer by aspiration cytology. In Japan, there are 25 reports of breast metastasis from gastric cancer. Two possible pathways from gastric lesion were lymphatic and one vascular, but the mechanism of breast metastasis has remained controversial.

Curative resection is considered to be a standard therapy for gastric cancer with localized peritoneal metastases. For tumors with diffuse dissemination, chemotherapy may play a major role, however, the benefits of reduction surgery and standard chemotherapy have not yet been clarified. Median survival time after reduction surgery was reported to be 4-13 months for patients diagnosed by surgery and/or CT and 5-6 months for chemotherapy for those diagnosed by CT alone. Reduction surgery has a high risk, with a morbidity of 12-44% and a mortality of 3-14%. Palliative surgery should be indicated for stenosis or bleeding due to primary tumors. 5-FU, MTX-5-FU, TS-1, paclitaxel, and their combination are candidates for practice and clinical trials. It is important to evaluate the severity of peritoneal dissemination by diagnostic laparoscopy or laparotomy for decision making.

We report a 65-year-old man who underwent distal gastrectomy for advanced and metastatic gastric cancer in June 2000. TS-1 was administered for remnant metastatic lesions as first-line chemotherapy, but a recurrence was found in June 2001. Paclitaxel 80 mg/m2 was then administered weekly in a 1-h infusion on day 1, 8, and 15 as one cycle. After two cycles of paclitaxel administration, the patient was relieved from abdominal pain, and a barium enema revealed marked improvement of the colonic stenosis due to the peritoneal metastasis. There have been few effective chemotherapeutic agents against peritoneal metastasis from gastric cancer to date; however, a weekly paclitaxel regimen is considered to be one of the promising regimens for metastatic and recurrent gastric cancer.

The clinical and diagnostic significance of abdominal computed tomography (CT) was evaluated in 123 patients with primary gastric adenocarcinoma and disseminated peritoneal metastases. The peritoneal metastases were diagnosed by CT findings such as ascites (30.1%), wall thickness of the intestine (1.6%), increase in fat density of peritoneal fat tissue (4.9%), peritoneal nodules (2.4%), and hydronephrosis (4.9%). The overall diagnosis of peritoneal metastasis was 37.4%. The CT findings of peritoneal metastases were significantly correlated with anemia, poor performance status, distant metastases, P3, and resectability of the primary tumor. The survival of the patients with positive findings was significantly poor (150 days of median survival), compared with those without findings (230 days). The abdominal CT thus provides information for diagnosis of peritoneal metastases and on the prognosis in patients with primary gastric cancer.

Unlabelled: We compared the clinicopathological factors and treatment results with regard to symptoms and screening in patients who underwent gastrectomy for primary gastric cancer., Patients: The subjects included 797 patients with asymptomatic gastric cancer (SCG) and 1090 patients with symptomatic cancer (SYG) who had undergone gastrectomy for gastric cancer from 1989 to 2000., Results: The proportion of early gastric cancer, N0 cases, and curative cases in the SCG and SYG patients were 70% & 33%, 77% & 44%, and 98% & 80%, respectively (p = 0.000, for each). Five-year survival rates of the SCG and SYG patients were 87% and 63% (p = 0.000)., Conclusion: Screening for gastric cancer in order to detect asymptomatic disease may contribute to patient survival.

To evaluate the relationship between radical surgery of recurrent tumor and prognosis in cases of recurrent gastric cancer, we analyzed data on 202 patients with relapsed gastric cancer, focusing on surgical recurrent tumor removal. In our series, 18 of the 202 patients underwent radical recurrent tumor resection. Resected tumors were located in the ovarium (n = 4), colorectum (n = 3), liver (n = 3), lymph node (n = 2), locoregional stoma (n = 2), and peritoneum, adrenal gland, brain, and lung (n = 1 each). No surgery-related mortality occurred. One patient remains alive over 5 years after hepatectomy without recurrence, and 17 died within 3 years: 7 patients from primary recurrence and 10 from multiple modes of recurrence. Median survival after recurrence (MSTAR) in the 18 radical surgery patients was 14 months, against 5 months in those treated palliatively (p = 0.0001). MSTAR for the ovary and the liver were 30 months and 15 months in the radical surgery cases, and 2.5 months for the ovary and 5 months for the liver in the palliative cases. Significant differences were thus seen between radical and palliative cases in the ovary (p = 0.010) and in the liver (p = 0.036). Median survival after gastrectomy was 45 months in the radical surgery cases, and 28 months in the palliative cases (p = 0.024). In postoperative gastric cancer follow-up, early detection of recurrence and radical surgery may well benefit patients with relapse, especially in the liver and ovary, in terms of survival.

Unlabelled: We performed laparoscopy before and after chemotherapy in two patients with relapsed and advanced gastric cancer, whose major metastatic sites had been diagnosed as being in the peritoneum. A change in tumor responses when assessed by laparoscopy was found. Case 1: A 63-year-old man presented with an umbilical metastasis and suspected peritoneal metastases after gastrectomy. Laparoscopy revealed peritoneal metastases before chemotherapy. After one course of chemotherapy the umbilical tumor disappeared (CR). Laparoscopy after two courses of chemotherapy revealed increasing peritoneal metastases (PD). The overall response was PD. Case 2: A 67-year-old woman was referred to our hospital with a diagnosis of type 4 gastric cancer. Staging laparoscopy revealed massive lymph node metastases and the patient was positive in peritoneal washing cytology. After four courses of chemotherapy, the primary tumor and the metastatic lymph nodes had decreased in size (PR). In contrast, laparoscopy revealed increasing peritoneal metastases (PD). The overall response was PD., Conclusion: In patients with peritoneal and other modes of metastasis, tumor response to chemotherapy may be misjudged by conventional imaging alone. Intraperitoneal examination by laparoscopy provides accurate information, including the tumor response to chemotherapy.

We attempted to clarify the accumulation of radiolabeled lymphocytes to tumors and regional lymph nodes in patients with gastric carcinoma. The effects of oral administration of OK-432 were also studied. Five patients with gastric cancer and one who underwent endoscopic mucosal resection, were entered in the study. Prior to the tracer study in 3 patients with gastric cancer, 5 KE of OK-432 was administered for 2 days. Peripheral mononuclear cells were separated and labeled with [111] In-tropolone. After the resection of stomach, tumor tissue, normal gastric mucosa, regional lymph nodes, and non-regional lymph nodes were dissected and radioactivity was measured by a gamma-counter. Accumulation of lymphocytes to the tumor tissue and n1 lymph node station was more than two times that in the normal gastric mucosa and ten times that in non-regional lymph nodes. Accumulation of lymphocytes to the n2 station was strongly enhanced by oral administration of OK-432.

Matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of matrix metalloproteinase-1 (TIMP-1) in peripheral blood were measured in 54 patients with gastric carcinoma; 46 were primary and 8 were recurrent cases. There were no significant associations between MMP-9 concentrations and clinicopathological factors. TIMP-1 levels were significantly increased in advanced and recurrent cases, and in cases with peritoneal dissemination and lymph node metastasis, suggesting that TIMP-1 concentration in the peripheral blood could be a new tumor marker for the recurrence of gastric carcinoma.

The effect of glucose infusion on portal and peripheral blood immunoreactive insulin (IRI) concentrations and arterial ketone body ratio (AKBR) during gastrectomy were evaluated on twenty patients divided into two groups. Portal and peripheral blood IRI concentrations, AKBR, total ketone body concentration (TKB: acetoacetate + beta-hydroxybutyrate), and blood glucose were determined before and 30, 60, and 120 minutes after 25g glucose infusion in 30 minutes in ten patients (group 1) and 50 g glucose infusion in 30 minutes in ten patients (group 2). In both groups, the rate of increase in the portal blood IRI concentration was markedly higher than that in the peripheral blood IRI concentration after glucose infusion and AKBR increased and TKB decreased with the increase of the portal blood IRI concentration. These findings suggest that the peripheral blood IRI concentration does not reflect the pancreatic insulin secretion after glucose infusion during surgery and that portal insulin plays an important role for elevating and maintaining AKBR at higher levels. On the other hand, in both groups, the blood glucose had its peak just after completion of glucose infusion and then decreased gradually. After glucose infusion, however, the blood glucose in group 2 increased markedly and was significantly higher than that in group 1. It is suggested that, during surgery, glucose infusion rate of 50 g in 30 minutes may be too rapid.
