A 77-year-old man presented with a 6-month history of progressive right optic neuropathy secondary to compression by the ipsilateral internal carotid artery(ICA). We performed anterior clinoidectomy and optic canal unroofing. Subsequently, we wrapped the ICA with a polytetrafluoroethylene tape, pulled the vessel laterally, and sutured the tape to the dura mater at the anterior skull base for optimal decompression. An inflammatory mass lesion was observed around the ICA, which led to further compression of the optic nerve. Histopathological examination of the resected specimen showed an inflammatory granuloma. The patient's visual field deficit showed partial improvement postoperatively. Transposition using a tape might be an effective surgical alternative for compressive optic neuropathy.

The patient was a 63-year-old man with hepatitis C. He discontinued combination therapy containing interferon and ribavirin because of the development of skin symptoms. A screening examination showed multiple early-stage hepatocellular carcinomas. He refused treatment and was followed up as an outpatient. During follow-up, his PIVKA-Ⅱ level remarkably elevated to 59,994mAU/mL. Computed tomography(CT)showed an enlarged tumor with portal invasion(vp2)in segment 8 and intrahepaticmetastasis. We performed right and partial hepatectomy. Three months later, CT showed multiple lung metastases. We initiated the daily administration of 800 mg of sorafenib. However, 6 months after hepatectomy, the lung metastases increased in size and number. We considered the therapeutic effect as progressive disease(PD)according to the RECIST criteria. We then initiated administering 120 mg of regorafenib daily as second-line therapy. In a course of the treatment containing sorafenib and regorafenib, the dose was reduced due to hand-foot skin reactions. 8.5 months after hepatectomy, the lung metastases significantly decreased in size. One year after hepatectomy, almost complete response(CR) was obtained, and no intrahepatic recurrence was found.

Chemotherapy-induced nausea and vomiting(CINV)were prospectively evaluated using MASCC Antiemesis Tool(MAT) in patients with hematological malignancies in our institution. A total of 33 patients receiving 46 chemotherapy courses were evaluated. Although vomiting was not observed in the acute phase, nausea was seen in 22.6% and 32.3% of the patients in the acute and delayed phases, respectively. Thirty percent(25 cases)of the patients receiving highly emetogenic chemotherapy presented nausea in both the phases, while 40%(18 cases)of the patients receiving moderately emetogenic chemotherapy presented nausea in the delayed phase. The oral intake was quantitated retrospectively in 31 patients with non- Hodgkin's lymphoma, who were hospitalized and received CHOP±R. Prior to the initiation of the chemotherapy, 13 patients received the first generation 5-HT3 receptor antagonist granisetron, while 18 patients received the second generation palonosetron. Oral intake was greater in the patients who were administered palonosetron. Thus, the present study suggested that antiemetic treatment could be improved at our institution.

A 68-year-old male patient, who was diagnosed with MGUS (IgG-λ) 11 years ago, was referred to our hospital because of a progressing pancytopenia. He was diagnosed with multiple myeloma (MM) and was hospitalized because of fever and pneumonia. Although empiric antibiotic and antifungal therapies were promptly initiated, his pneumonia worsened. Chest CT images revealed diffuse interstitial pneumonia. Although bortezomib/dexamethasone therapy was initiated as a treatment for MM and pneumonia, he showed little response. His pneumonia worsened and progressed to acute respiratory distress syndrome. Using mPSL (500 mg/day), sivelestat, and MM treatment switching to lenalidomide/dexamethasone (Rd), his respiratory status and CT findings rapidly improved. He received Rd therapy as an outpatient; however, after the completion of six cycles of therapy, his MM progressed, with a recurrence of pneumonia and high fever again. The onset of pneumonia was closely associated with MM progression. His pneumonia improved by treatment with mPSL half-pulse and MM treatment switching to carfilzomib/Rd. In the present study, we report the case of a patient with myeloma, who presented with multiple interstitial pneumonia, resulting in respiratory failure twice in concordance with myeloma progression.

A 64-year-old man was diagnosed with acute myeloid leukemia M2 (FLT3-ITD-positive). After induction chemotherapy and four courses of consolidation therapy, he underwent umbilical cord blood transplantation (CBT) in his first remission. He developed acute graft-versus-host disease (skin stage 2) after successful engraftment. On post-transplantation day 147, he was admitted to the hospital suffering from pneumonia. During the treatment, drastic thrombocytopenia was observed on day 251. Both platelet-associated immunoglobulin G and platelet antibody producing B cells were detected, and he was diagnosed with immune thrombocytopenia (ITP). Treatment with prednisolone (1 mg/kg/day), eltrombopag (25 mg/day), and intravenous immunoglobulin (400 mg/kg) was commenced, but there was no improvement in his platelet count. After switching from eltrombopag to romiplostim (350 µg/week), and addition of cyclosporine, the platelet count rapidly elevated to 150,000/µl. ITP after allogenic stem cell transplantation is a rare complication, and it is often refractory to the 1st-line treatment such as steroids. Herein, we report successful treatment using a combination of romiplostim and an immunosuppressive agent in the case of treatment failure in ITP that developed after CBT.

The patient was a 57-year-old man who was diagnosed as having hepatocellular carcinoma (HCC). The tumor, 13 cm in diameter, occupied almost the entire right lobe and directly permeated the gallbladder. In addition, invasion of the portal vein umbilical region was seen from an inside area progress department. Because of the small remnant liver volume due to 3 procedures for right area excision and because liver failure was a concern after the operation, we decided to perform extended right hepatectomy for which we retained one S4 Glisson's capsule. Radiotherapy was planned for portal vein tumor thrombus after the operation. We started radiotherapy( 2.0 Gy×25) on postoperative day 14, and the patient was discharged without complications on postoperative day 55. We initiated the internal administration of tegafur-uraci(l UFT 400 mg/day) on postoperative day 44 and continued the treatment for half a year; however, metastasis was frequently noted in the lungs on computed tomography (CT) at 9 months after the operation. We administered low-dose 5-fluorouracil plus cisplatin( FP, intravenous) therapy combined with sorafenib( 800 mg/day), and the patient is being followed up. No recurrence has been noted in the liver at 16 months after the operation.

A 44-year-old man with advanced hepatocellular carcinoma (HCC) was admitted to our institution. Abdominal computed tomography( CT) revealed diffuse HCC in the right lobe, with a tumor thrombus in the main trunk and contralateral branch of the portal vein. Right lobectomy and tumor thrombectomy were performed. Three months later, oral tegafur-uraci( l UFT) administration( 300 mg/day) was initiated. Twelve months after surgery, intrahepatic recurrence was detected on CT scans. Therefore, the patient was treated with transcatheter arterial infusion( TAI) and percutaneous ethanol injection therapy (PEIT). Since then, no evidence of HCC recurrence has been noted for 46 months after the initial treatment.

We report a case of bilobar multiple hepatocellular carcinoma (HCC) successfully treated by preoperative percutaneous isolated hepatic perfusion(PIHP) and subsequent hepatectomy. A 77-year-old man with elevated serum PIVKA-II level and hepatomas was referred to our hospital. Abdominal dynamic computed tomography and gadoxetate disodium-enhanced magnetic resonance imaging demonstrated advanced HCCs: 11 tumors with a maximum tumor size of 4.8 cm diameter in the right lobe, and 1 tumor of 3.6 cm diameter in the left lobe. Curative resection was impossible due to insufficient liver function and due to the locations of the tumors. Therefore, we performed preoperative PIHP in order to control the multiple HCCs in the right lobe. This resulted in normalization of serum PIVKA-II level and complete necrosis of all tumors in the right lobe, as observed on dynamic computed tomography after 4 weeks of PIHP. Then, at 7 weeks after PIHP, we performed extended left lobectomy for residual tumor in the left lobe. Finally, complete remission was thought to have been achieved. The patient is alive 6 months after PIHP, without recurrence.

We report 2 cases of lung cancer incidentally detected following pneumothorax. Case 1:A 40-year-old man complaining of dyspnea was admitted with right pneumothorax. Chest computed tomography (CT) after chest drainage showed a cavitary nodule with pleural indentations in the right lower lobe. It was indicated at surgery that pneumothorax was caused by perforation of the tumor into the pleural cavity. Right lower lobectomy was performed because the pathological diagnosis of the nodule was a large cell carcinoma. The final histopathological diagnosis was stage II A (pT2aN1M0). The patient died of recurrence 14 months after surgery. Case 2:A 47-year-old man who admitted with right pneumothorax was found to have a nodule with pleural indentations closely a bulla at the apex of the right lung by chest CT after chest drainage. Pneumothorax was indicated to be caused by rupture of the bulla at surgery. Right upper lobectomy was performed because the pathological diagnosis of the nodule was a squamous carcinoma. The final histopathological diagnosis was stage I A (pT1bN0M0). The patient is alive at 2 years after the operation without recurrence. Lung cancer detected following pneumothorax which was caused by perforation of the tumor is generally considered to have poor prognosis. Whereas, prognosis of lung cancer incidentally detected following pneumothorax depends on its staging.

Preoperative patients have various kinds of anxiety. However we do not know much about their psychological burden. Therefore, we investigated types of their anxiety with questionnaires prior to operation and analyzed them according to their anesthetic method, type of surgery and age. Concerning anesthetic methods, patients' anxiety varied depending on their histories of anesthesia, anesthetic method previously received and age. Concerning the type of surgery, patients scheduled to undergo abdominal and cardiac surgery had more intense anxiety. Patients scheduled for gynecological surgery were anxious about the accident which might be induced by anesthesia. A large number of patients scheduled for orthopedic surgery were anxious about postoperative pain. Concerning patients' age, patients whose ages ranged from 10s to 50s were anxious about pain. While, in patients over 60s, their anxiety was focussed on giving trouble to their surrounding people. Patients whose age was 10 to 19 had anxiety about their anesthetic method. Anesthesiologists should recognize that preoperative patients have various kinds of anxiety. It is important for us to examine the patients appropriately according to their psychological and physical condition.

The effects of intravenous diltiazem (0.5 and 1.0 mg.kg-1) on muscle compound action potential (CAP) elicited from gastrocnemius muscle by sciatic nerve stimulation in cats were investigated. Analysis was performed on the amplitude of CAP, recovery curve (RC) and train-of-four ratio (TOFR). There was no response in any parameter after administering 0.5 mg.kg-1. On the other hand, diltiazem 1.0 mg.kg-1 produced significant depression only in the amplitude of CAP, but no change was observed in both RC and TOFR. It is concluded that approximately ten times the therapeutic dose of diltiazem is necessary to modify the neuromuscular transmission.

Effects of alpha-bungarotoxin (alpha BuTX) on muscle compound action potentials (CAPs) which were elicited from gastrocnemius muscle by sciatic nerve stimulation in cats were studied, and the results were compared with those of non-depolarizing relaxants including d-tubocurarine, pancuronium and vecuronium. The amplitude of CAP by the second member of the paired stimuli (test response) was compared with that evoked by the first component (conditioning response). The interval between the two components of the paired stimuli (the pair interval) was increased stepwise from 7 to 1,000 msec and a curve (recovery curve, RC) was obtained by relating the changes in pair interval to the difference in amplitude of the test and conditioning responses. The following results were obtained. (1) A progressive and irreversible neuromuscular block was observed after intravenous administration of alpha BuTX. During the progress of the blockade, RC of CAPs showed a pattern which was characterized by profound potentiations of test responses at shorter intervals of paired stimuli, followed by gradual recovery to similar amplitude with conditioning responses at 500 msec or over of pair interval. (2) With non-depolarizing relaxants, RC changed to the pattern of slight potentiation at a very short interval of stimuli, followed by a notable depression at longer intervals. (3) The mechanism of the development of these difference of RCs between alpha BuTX and the relaxants was discussed, and it is concluded that the notable depression of RCs derived from muscle relaxants may be caused by inhibitory effect on nerve terminals of these relaxants, and the inhibition was more prominent with d-tubocurarine than with other two relaxants.
