Migraine is a disease that is difficult to be recognized by those around the patients, even though it causes significant hindrances. In this study, we conducted an exploratory comparison of the perceptions on migraine among patients, family members living with them, and physicians treating migraine patients. Patients and family members shared a common understanding on the pain of migraine, and hoped to spend more/better time together as a family. However, although family members felt compassion for the patients, lack of understanding by and patients' concern for the surroundings led to feelings of resignation and endurance on the side of patients. Regarding physicians' medical care, our results suggested the importance to understand the wishes and obstacles of each patient and to propose treatment accordingly. In order to reduce the burden of migraine, it is necessary to create an environment and raise awareness that allows people around the patients to understand and support the pain and hopes that each patient feels.

The Japanese Headache Society and the Japanese Society of Neurology has published the 2013 guidelines for the diagnosis and treatment of chronic headache. A new CQ has been set up in the guidelines on the topic of "How to treat chronic migraine." In the past, lomerizine was the only prophylactic medication of migraine that was eligible under insurance coverage. However, afterward in 2010, valproate was added to the list of antimigraine medications approved under insurance coverage, followed by propranolol, amitriptyline, and verapamil, with rapid advances in the treatment of migraine. Valproate, topiramate (not approved under insurance coverage until date), and amitriptyline could potentially be used in the treatment of chronic migraine in Japan; further, considering the clinical outcomes thus far, lomerizine could also be added to the list. As a drug for migraine prophylaxis, valproate is contraindicated in pregnant women and needs to be used with caution.

A case is 61 years old woman. In February 2008, she was aware of swelling, skin redness and edema in her left breast and visited our hospital. We diagnosed it as inflammatory breast cancer with positive hormone receptor (ER+, PgR+) and unexpression of HER2 (HercepTest 1 +). We started preoperative chemotherapy with weekly paclitaxel followed by FEC100, but we canceled chemotherapy because she developed cerebral infarction when we administered paclitaxel twice. Then, hormonotherapy using anastrozole (Arimidex) was therefore attempted. Three months later, treatment with anastrozole alone reduced the swelling, skin redness and edema in her left breast. After eight months of administration, the breast swelling, skin redness and edema were completely disappeared. MRI revealed the disappearance of the enhanced area. Then mastectomy with auxiliary dissection was performed.

The reliability and validity of the Japanese version of Structured Interview for Disorders of Extreme Stress (SIDES) were evaluated in a group of normal subjects (n=60) and a group of victims of interpersonal trauma (n= 53). SIDES was developed in the United States in 1997 and is a tool for assessing Disorders of Extreme Stress Not Otherwise Specified (DESNOS). Self-reporting and semi-structured versions of the SIDES have been developed. Cronbach's alpha coefficient for the self-reporting version was .92 for an individual's lifetime, and .85 for the present. Cronbach's alpha coefficient for the semi-structured interview version was .95 for an individual's lifetime, and .88 for the present. Using the Dissociative Experiences Scale (DES) and physical symptom scale as external standards, the validity of SIDES was confirmed. Although the present results indicate that the Japanese version of SIDES has good internal reliability and validity, it remains necessary to conduct further research on more victims of many kinds of trauma.

We investigated prevalence and character of primary headache for 2,462 examples of high school student. An overall headache prevalence was 41.0 in boys, 55.3% in girls, and average of the headache onset was 12.7 in boys, 12.9 in girls years old. A prevalence of migraine is 5.5 in boys, 6.1% in girls according to ICHD-I and 13.7 in boys, 17.5% in girls according to modified ICHD-II diagnosis criteria. For the tension type headache, it was 23.0 in boys, 30.6% in girls according to modified ICHD-I diagnosis criteria. High school students with primary headache tend take painkillers easily, but are much less likely visit physicians (only 15.0%). Our data showed that the prevalence of headache especially migraine in Japanese high school student was almost similar to past world wide findings, and suggest that its impact on the quality of life can not be ignored. Therefore, headache education must be important and necessary in a high school.
