It is important to obtain accurate anatomical information with little distortion in preoperative examination of brain tumors. Using PETRA, which is an ultrashort echo time (UTE) sequence that is less affected by magnetic susceptibility artifacts, we determined the optimal imaging conditions (radial views [RV] and inversion time [TI]) for IR-PETRA using the inversion recovery (IR) method and compared it with MPRAGE. IR-PETRA was found to be slightly inferior to MPRAGE in sharpness under the optimum conditions (RV=100,000 and TI=500 ms), but it was significantly improved by using a high RV value, and SNR and CNR were higher than or equal to MPRAGE. It is suggested that IR-PETRA may be an alternative sequence of MPRAGE in preoperative examination of brain tumors.

In recent years, re-rupture due to endoleaks after thoracic endovascular aortic repair (TEVAR) for ruptured thoracic aortic aneurysms has become a problem. Hemoptysis has been reported in patients after pneumocentesis. We report a patient who developed delayed hemoptysis not related to endoleak after TEVAR. An 80-year-old male underwent emergent TEVAR due to a ruptured thoracic aortic aneurysm accompanying sudden hemoptysis. Eleven days after the operation, recurrent hemoptysis was noted, but contrast-enhanced computed tomography (CT) revealed no endoleak or re-rupture. Bronchoscopy demonstrated hemorrhage from the left upper lobe. As hemostasis was difficult by conservative treatment, left upper lobectomy was performed. The aortic rupture hole exhibited thrombus, and there was no hemorrhage. On histopathological examination of the resected lung, formation accompanied by severe intra-alveolar fibrosis and cholesterin clefts, and marked foreign body reactions in the interstitium and small blood vessels of the lung were observed. Cholesterol embolism is associated with not only organ ischemia due to microembolism, but also immunological mechanisms. Thus, cholesterol embolism due to aorta-derived cholesterin may have led to the delayed pulmonary hemorrhage. Differentiation from re-rupture due to endoleaks is important.

Progressive multifocal leukoencephalopathy (PML) is caused by opportunistic infection by JC virus and presents with progressive demyelinating lesions in the central nervous system. A 59-year-old man with a history of alcoholic liver dysfunction presented with progressive weakness of his left leg over a period of one month. MRI showed multiple white matter lesions that were of low intensity on the T1 image and high intensity on the T2 image, heterogeneously high intensity on the diffusion image, and were not enhanced with contrast media. The patient underwent open biopsy of the right parietal lesion. The histological findings were the demyelination and the enlargement of nuclei of oligodendrocytes. Electron microscopic examination showed numerous viral particles in the nuclei of the oligodendrocytes. Infection by JC virus in the central nervous system was diagnosed with the polymerase chain reaction (PCR) products sampled from the cerebrospinal fluid. The incidence of PML has significantly increased in immunosuppressive patients, such as AIDS (acquired immunodeficiency syndrome). We presented the first case of PML in an immune-compromised state with alcoholic liver dysfunction.

Previous studies have revealed that mothers of children with attention-deficit/hyperactivity disorder (AD/HD) have an authoritarian parenting style. However, the psychological process of developing an authoritarian parenting style has yet to be clearly defined. To clarify this psychological process, the present study examined the hypothesis that the characteristic behaviors of children with AD/HD initially increase the mothers' parenting stress, which influences their parenting style. Thirty-six mothers of children with AD/HD (children's mean age: 8.1 years) and the same number of controls (children's mean age: 8.4 years) participated in the present study. The mothers' parenting stress was assessed using the Japanese Parenting Stress Index. Parenting styles were assessed using the TK-style scale for evaluating the relationships between parents and children. The results indicated that the mothers of children with AD/HD had significantly higher scores than controls for all parenting stress items and negative parenting style variables (dissatisfaction, reproach, strictness, interference, inconsistency and disagreement of 10 attitudes). Stepwise multiple regression analysis revealed that the characteristic behaviors of children with AD/HD were associated with the degree of attachment in mothers, which was related to the strict and reproachful parenting style in the AD/HD group. These results suggest that mothers of children with AD/HD are likely to have a strict and reproachful parenting style as a result of a lack of attachment with the child.

A 17-month-old boy was admitted to our hospital in a semicomatose state after a minor head injury. Computed tomography (CT) scans revealed a posterior interhemispheric and a thin convexity subdural hematoma. Two and a half hours later, repeated CT scans disclosed a development of the right hemispheric diffuse low density and an enlargement of the interhemispheric subdural hematoma. Decompressive craniectomy and removal of the hematoma was performed immediately. On the second postoperative day, the diffuse low density areas developed in the contralateral frontal and temporal lobes. On the 14th postoperative day, the gyri which had been low in density were markedly enhanced with contrast enhancement. On the 16th postoperative day, CT scans showed a diffuse hemispheric gyral high density with bleeding in the right parieto-occipital lobe. A right occipital lobectomy and removal of the hematoma was performed. Histological examination suggested cerebral infarction probably due to venous congestion. It is also suggested that the diffuse hemispheric gyral high density observed on the 16th postoperative day was the hemorrhagic cerebral infarction.
